The recursive model index (RMI) has recently been introduced as a machine-learned replacement for traditional indexes over sorted data, achieving remarkably fast lookups. Follow-up work focused on explaining RMI's performance and automatically configuring RMIs through enumeration. Unfortunately, configuring RMIs involves setting several hyperparameters, the enumeration of which is often too time-consuming in practice. Therefore, in this work, we conduct the first inventor-independent broad analysis of RMIs with the goal of understanding the impact of each hyperparameter on performance. In particular, we show that in addition to model types and layer size, error bounds and search algorithms must be considered to achieve the best possible performance. Based on our findings, we develop a simple-to-follow guideline for configuring RMIs. We evaluate our guideline by comparing the resulting RMIs with a number of state-of-the-art indexes, both learned and traditional. We show that our simple guideline is sufficient to achieve competitive performance with other learned indexes and RMIs whose configuration was determined using an expensive enumeration procedure. In addition, while carefully reimplementing RMIs, we are able to improve the build time by 2.5x to 6.3x.

Abstract The information content, that is, the predictive capability, of a forecast system is often quantified with skill scores. This paper introduces two ranked mutual information skill (RMIS) scores, RMISO and RMISY, for the evaluation of probabilistic forecasts. These scores are based on the concept of mutual information of random variables as developed in information theory. Like the ranked probability skill score (RPSS)—another and often applied skill score—the new scores compare cumulative probabilities for multiple event thresholds. The RMISO quantifies the fraction of information in the observational data that is explained by the forecasts. The RMISY quantifies the amount of useful information in the forecasts. Like the RPSS, the new scores are biased, but they can be debiased with a simple and robust method. This and additional promising characteristics of the scores are discussed with ensemble forecast assessment experiments.

Aims. To assess whether replacing CA125 with HE4 in the classical formulas of risk of malignancy indices (RMIs) can improve diagnostic performance. Methods. For each of 312 patients with an adnexal mass, classical RMIs 1–4 were computed based on ultrasound score, menopausal status, and serum CA125 levels. Additionally, modified RMIs (mRMIs) 1–4 were recalculated by replacing CA125 with HE4. Results. Malignant pathology was diagnosed in 52 patients (16.67%). There was no significant difference in diagnostic performance (area under the receiver operating characteristic curve [AUC]) between each classical RMI and its corresponding mRMI. In the entire sample, the AUC was 0.899, 0.900, 0.895, and 0.908 for classical RMIs 1–4 compared to 0.903, 0.929, 0.930, and 0.931 for mRMIs 1–4. In premenopausal patients, the AUC was 0.818, 0.798, 0.795, and 0.802 for classical RMIs 1–4 compared to 0.839, 0.875, 0.876, and 0.856 for mRMIs 1–4. In postmenopausal patients, the AUC was 0.906, 0.895, 0.896, and 0.906 for classical RMIs 1–4 compared to 0.907, 0.923, 0.924, and 0.930 for mRMI 1–4. Conclusions. Use of HE4 instead of CA125 did not significantly improve diagnostic performance of RMIs 1–4 in patients with an adnexal mass.

This paper documents research into repetitive motion injuries (RMIs) occurring at a used garment sorting facility, with a focus on the Poisson distribution model and associated time interval analysis. Time interval analysis is used to confirm existence of a Poisson process. The Poisson process and distribution is then implemented to model the occurrence of RMIs at the target facility, as well as employed in a proactive effort to track and reduce RMIs. As a major player within this labor-intensive industry, the industrial partner experiences a significant number of repetitive motion injuries (RMIs). Analysis is provided on the Poisson process, and the salient RMI risk factors and ergonomic principles that might affect application of the Poisson distribution model. This paper also reviews proposed methodologies for collecting RMI risk factor data, tracking RMI accident or “incident” data, gathering population-specific anthropometric data, and developing RMI hazard reduction strategies. The Poisson model is presented as a structured methodology for the prediction and control of repetitive motion injuries.

Purpose Existing robot-assisted minimally invasive surgery (RMIS) system lacks of force feedback, and it cannot provide the surgeon with interaction forces between the surgical instruments and patient’s tissues. This paper aims to restore force sensation for the RMIS system and evaluate effect of force sensing in a master-slave manner. Design/methodology/approach This paper presents a four-DOF surgical instrument with modular joints and six-axis force sensing capability and proposes an incremental position mode master–slave control strategy based on separated position and orientation to reflect motion of the end of master manipulator to the end of surgical instrument. Ex-vivo experiments including tissue palpation and blunt dissection are conducted to verify the effect of force sensing for the surgical instrument. An experiment of trajectory tracking is carried out to test precision of the control strategy. Findings Results of trajectory tracking experiment show that this control strategy can precisely reflect the hand motion of the operator, and the results of the ex-vivo experiments including tissue palpation and blunt dissection illustrate that this surgical instrument can measure the six-axis interaction forces successfully for the RMIS. Originality/value This paper addresses the important role of force sensing and force feedback in RMIS, clarifies the feasibility to apply this instrument prototype in RMIS for force sensing and provides technical support of force feedback for further clinical application.

Background Natural language processing has been established as an important tool when using unstructured text data; however, most studies in the medical field have been limited to a retrospective analysis of text entered manually by humans. Little research has focused on applying natural language processing to the conversion of raw voice data generated in the clinical field into text using speech-to-text algorithms. Objective In this study, we investigated the promptness and reliability of a real-time medical record input assistance system with voice artificial intelligence (RMIS-AI) and compared it to the manual method for triage tasks in the emergency department. Methods From June 4, 2021, to September 12, 2021, RMIS-AI, using a machine learning engine trained with 1717 triage cases over 6 months, was prospectively applied in clinical practice in a triage unit. We analyzed a total of 1063 triage tasks performed by 19 triage nurses who agreed to participate. The primary outcome was the time for participants to perform the triage task. Results The median time for participants to perform the triage task was 204 (IQR 155, 277) seconds by RMIS-AI and 231 (IQR 180, 313) seconds using manual method; this difference was statistically significant (P<.001). Most variables required for entry in the triage note showed a higher record completion rate by the manual method, but in the recording of additional chief concerns and past medical history, RMIS-AI showed a higher record completion rate than the manual method. Categorical variables entered by RMIS-AI showed less accuracy compared with continuous variables, such as vital signs. Conclusions RMIS-AI improves the promptness in performing triage tasks as compared to using the manual input method. However, to make it a reliable alternative to the conventional method, technical supplementation and additional research should be pursued.

Homologous recombination is a high-fidelity repair pathway for DNA double-strand breaks employed during both mitotic and meiotic cell divisions. Such repair can lead to genetic exchange, originating from crossover (CO) generation. In mitosis, COs are suppressed to prevent sister chromatid exchange. Here, the BTR complex, consisting of the Bloom helicase (HIM-6 in worms), topoisomerase 3 (TOP-3), and the RMI1 (RMH-1 and RMH-2) and RMI2 scaffolding proteins, is essential for dismantling joint DNA molecules to form non-crossovers (NCOs) via decatenation. In contrast, in meiosis COs are essential for accurate chromosome segregation and the BTR complex plays distinct roles in CO and NCO generation at different steps in meiotic recombination. RMI2 stabilizes the RMI1 scaffolding protein, and lack of RMI2 in mitosis leads to elevated sister chromatid exchange, as observed upon RMI1 knockdown. However, much less is known about the involvement of RMI2 in meiotic recombination. So far, RMI2 homologs have been found in vertebrates and plants, but not in lower organisms such as Drosophila, yeast, or worms. We report the identification of the Caenorhabditis elegans functional homolog of RMI2, which we named RMIF-2. The protein shows a dynamic localization pattern to recombination foci during meiotic prophase I and concentration into recombination foci is mutually dependent on other BTR complex proteins. Comparative analysis of the rmif-2 and rmh-1 phenotypes revealed numerous commonalities, including in regulating CO formation and directing COs toward chromosome arms. Surprisingly, the prevalence of heterologous recombination was several fold lower in the rmif-2 mutant, suggesting that RMIF-2 may be dispensable or less strictly required for some BTR complex-mediated activities during meiosis.

This study aims to examine and analyze whether human resource competence, implementation of regional management information systems (RMIS), and organizational commitment affect treasurer performance of regional apparatus organizational expenditures with ease of use of information systems as a moderating variable in the government of Serdang Bedagai Regency. The research used in this research is associative causality research using primary data. The sampling method is the census method by giving a questionnaire to the Treasurer of Expenditure Organization of the Regional Government of Serdang Bedagai Regency. The hypothesis testing method uses Partial Least Squares (PLS). The results showed that human resource competence and organizational commitment had a positive and significant effect on the treasurer performance of regional apparatus organizational expenditures. The implementation of RMIS had a positive and no significant effect on the treasurer performance of regional apparatus organizational expenditures in the Serdang Bedagai Regency. The variable ease of use of the information system can moderate the influence of human resource competence on the treasurer performance of regional apparatus organizational expenditures in the Serdang Bedagai Regency. However, it cannot moderate the effect of the implementation of RMIS and organizational commitment on the treasurer performance of regional apparatus organizational expenditures in the Serdang Bedagai Regency. AbstractDue to its monolithic structure and high dexterity, the compliant mechanism becomes an emerging solution to miniaturize surgical forceps for minimally invasive procedures. However, it is complicated and inefficient to use traditional rigid-link-based kinematic method to synthesize compliant forceps. In this paper, we present a topology-optimization-based method to automatically synthesize compliant forceps for robot-assisted minimally invasive surgery (RMIS). The basic geometry modeling tool and the automatic synthesis algorithm were both implemented in Matlab. Several synthesis examples were presented to show the performance of the proposed method. The realized forceps and a continuum manipulator have been constructed and 3D-printed, which demonstrated the application of the automatic synthesis method in RMIS.

Background and objectives: To compare the diagnostic accuracy and clinical utility of the Early-stage Ovarian Malignancy (EOM) score with the Risk of Malignancy Index (RMI) in the presurgical assessment of women presenting with adnexal masses. Materials and Methods: A secondary analysis was carried out in a retrospective cohort of women who presented with an adnexal mass and were scheduled for surgery at Phrapokklao Hospital between September 2013 and December 2017. The clinical characteristics, ultrasonographic features of the masses, and preoperative CA-125 levels were recorded. The EOM and the RMI score were calculated and compared in terms of accuracy and clinical utility. Decision curve analysis (DCA), which examined the net benefit (NB) of applying the EOM and the RMI in practice at a range of threshold probabilities, was presented. Results: In this study, data from 270 patients were analyzed. Fifty-four (20.0%) women in the sample had early-stage ovarian cancer. All four RMI versions demonstrated a lower sensitivity for the detection of patients with early-stage ovarian cancer compared to an EOM score ≥ 15. An EOM ≥ 15 resulted in a higher proportion of net true positive or NB than all versions of the RMIs from a threshold probability of 5% to 30%. Conclusions: It also showed a higher capability to reduce the number of inappropriate referrals than the RMIs at a threshold probability between 5% and 30%. The EOM score showed higher diagnostic sensitivity and has the potential to be clinically more useful than the RMIs to triage women who present with adnexal masses for referral to oncologic gynecologists. Further external validation is required to support our findings.

Risk management information system (RMIS) is becoming an important element in MIS system of banking sector in Vietnam in recent years and in future. We can use mathematics formulas applied in risk model to strengthen RMIS for banks. Nowadays, mathematics functions expressed high roles in economics and finance fields. Under effects from Covid 19 and industry 4.0, Vietnam banks realie need to enhance risk management and corporate governance system with RMIS channels to deliver proper risk information to clients and investors to attract more capitals via enhancing financial accounting data transparency. This study mainly use combination of quantitative methods including mathematics applied to calculate weighted beta CAPM, a common systemic risk measurement, and qualitative methods including synthesis, inductive and explanatory methods. And it emphasizes again sustainable modern bank management, using 5 bank cases: Eximbank (EIB) and Saigon Hanoi Bank (SHB), Asia Commercial Bank (ACB), Sacombank (STB) and Saigon Hanoi bank (SHB), 5 big listed banks in our country. Research results show us that risk model in banks can be done via some steps. First, calculating weighted beta CAPM or systemic risk via support of mathematics formula applied from market value and beta values of banks. Second, we run OLS regression for weighted beta and found out: for external impacts, exchange rate has negative correlation with weighted beta and for internal effects, CPI and R (lending rate) have negative relationship with weighted beta, so that policy makers can adjust policies to manage risks. Besides, this study also give out recommendations for enhancing management information system (MIS) for upgrading roles of banks in Vietnam economic development. Then, we can suggest suitable plans for sustainable management strategies. Our research limitation is within bank sector, then we can expand for other industries and markets as well.

It has been more than 20 years that robot-assisted minimally invasive surgery (RMIS) has brought remarkable accuracy and dexterity for surgeons along with the decreasing trauma for the patients. In this paper a novel method of the tissue’s surface profile mapping is proposed. The tissue surface profile plays an important role for material identification during RMIS. It is shown how by integrating the force feedback into robot controller the surface profile of the tissue can be obtained with force feedback scanning. The experiment setup includes a 5 degree of freedoms (DOFs) robot which is equipped with a strain-gauge ball caster as the force feedback. Robot joint encoders signals and the captured force signal of the strain-gauge are transferred to developed surface transformation algorithm (STA). The real-time geometrical transformation process is triggered with force signal to identify contact points between the ball caster and the artificial tissue. The 2D surface profile of tissue will be mapped based on these contact points. Real-time capability of the proposed system is evaluated experimentally for the artifical tissues in a designed test rig.

This study aims to examine empirically the Influence of Government Internal Control Systems (GICS), Regional Management Information Systems (RMIS), and Application of Government Accounting Standards (GAS) to the Quality of Financial Statements (QFS). This research was carried out at the regional government of Bengkulu Province, using a sample of 84 respondents. Data collection methods use surveys, while data analysis techniques used are validity and reliability testing, hypothesis testing is done using linear regression analysis.The results of the study show (1) the application of GICS has a positive effect on the QFS. By continuously strengthening GICS, it will have an impact on improving the QFS. (2) The application of RMIS has a positive effect on the QFS. This shows that the better the implementation of the regional management information system, the more qualified local government financial reports will be. (3) GAS implementation has a positive effect on the QFS. This proves that the better the level of implementation of QFS that is carried out in the preparation of financial statements, it will have a positive impact on improving the QFS.

Laparoscopic surgery (LS) is a minimally invasive technique that offers many advantages over traditional open surgery: it reduces trauma, scarring, and shortens recovery time. However, an important limitation is the loss of tactile sensations. Although some progress has been made in robotic-assisted minimally invasive surgery (RMIS) setups, RMIS is still not widely accessible. This review aims to identify which tactile display technologies have been proposed and experimentally validated for the restoration of tactile sensations during conventional laparoscopic surgical tasks. We conducted a systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We identified relevant articles published over the past 10 years through a search on Web of science, Scopus, IEEE Xplore Digital, and PubMed repositories. A total of 143 articles met the inclusion criteria and 24 were included in the final review. From the reviewed articles, we classified the proposed tactile displays into two categories based on the use of skin contact: (i) skin tactile displays, which include vibrotactile, skin-indentation, and grip-feedback devices, and (ii) non-contact tactile displays based on visualization tools. This survey aims to contribute to further research in the area of tactile displays for laparoscopic surgery by providing a better understanding of the current state of the art and identifying the remaining challenges.

This paper proposes a new type of tactile transfer cell which can be effectively applied to robot-assisted minimally invasive surgery (RMIS). The proposed tactile device is manufactured from two smart materials, a magnetorheological fluid (MRF) and a magnetorheological elastomer (MRE), whose viscoelastic properties are controllable by an external magnetic field. Thus, it can produce field-dependent repulsive forces which are equivalent to several human organs (or tissues) such as a heart. As a first step, an appropriate tactile sample is made using both MRF and MRE associated with porous foam. Then, the microstructures of these materials taken from Scanning Electron Microscope (SEM) images are presented, showing the particle distribution with and without the magnetic field. Subsequently, the field-dependent repulsive force of the sample, which is equivalent to the stress relaxation property of viscoelastic materials, are measured at several compressive deformation depths. Then, the measured values are compared with the calculated values obtained from Young’s modulus of human tissue data via the finite element method. It is identified from this comparison that the proposed tactile transfer cell can mimic the repulsive force (or hardness) of several human organs. This directly indicates that the proposed MR materials-based tactile transfer cell (MRTTC in short) can be effectively applied to RMIS in which the surgeon can feel the strength or softness of the human organ by just changing the magnetic field intensity. In this work, to reflect a more practical feasibility, a psychophysical test is also carried out using 20 volunteers, and the results are analyzed, presenting the standard deviation.

And these risk results and models can be delivered to suitable users in a management information system (MIS).

Research results indicate that GDP growth (G), CPI and Risk free rate (Rf) have highest effects on both ACB beta CAPM and stock price. Besides, this study also give out recommendations for enhancing management information system (MIS) for upgrading roles of banks in Vietnam economic development. Then, we can suggest suitable plans for sustainable management strategies. Our research limitation is within bank sector, then we can expand for other industries and markets as well.

It is very prevalent in medical industry to adopt robotic technology such as robotic minimally invasive surgery (RMIS). This causes the surgeon not to sense viscosity and stiffness of the tissue or organ. In order to generate the tactile recognition of human organ, this work proposes a novel tactile device using magnetorheological (MR) fluid. The MR fluid is fully inserted into diaphragm and several pins. Due to controllability of rheological properties by the intensity of the magnetic flux density, the operator can feel different force from the proposed tactile device. In order to evaluate the performance of tactile device, two method is proposed. One is to measure the repulsive force with applying several control inputs. The other method is to check the performance of tactile sensation using psychophysical experiments. From these results, the usability of the tactile device is proved.

For a sustainable economic performance of apple production, the determination of efficient farming options considering production risk is crucial. Relying on a permanent crop, apple producers are less flexible to react upon disturbances. Based on data of 134 apple producers operating in the two main production areas in Germany, we compare and determine efficient production options. Furthermore, appropriate risk management instruments (RMIs) are identified using stochastic dominance criteria. In addition, we use Stochastic Efficiency with Respect to a Function to evaluate farming options for defined ranges of relative risk aversion. The results indicate that Red Prince is the most efficient variety in the north and subsidized hail insurance with frost irrigation is superior to frost irrigation as single RMI. In the south Braeburn should be chosen by rational decision makers, but the tested insurance solutions are not as efficient as the common practice of producing apple under hail nets.

Objective: This paper presents controllable force ranges of tactile devices made of smart Magnetorheological Fluids (MRF) and porous sponges (MR sponges in short). Methods: In order to identify the wide controllable range of the field-dependent repulsive force, three MR sponge samples with three different MR fluids are fabricated using polyurethane foam and cling film. Then, the repulsive forces of the samples are measured using the motor-driven experimental apparatus and the results are presented with minimum and maximum values of the repulsive forces. On the other hand, in order to investigate the feasibility of the proposed tactile device for application to Robot-assisted Minimally Invasive Surgery (RMIS), pig’s organs such as liver, lung and heart, whose viscoelastic properties are very similar to those of human tissues, are tested under same conditions. Results: It is shown that the range of the repulsive spectrum of the pig’s organs can be achieved using the proposed samples by controlling the magnetic field intensity to be applied to MRF domain.

The conserved BTR complex, composed of the Bloom's syndrome helicase (BLM), topoisomerase IIIα, RMI1, and RMI2, regulates homologous recombination in favor of non-crossover formation via the dissolution of the double Holliday Junction (dHJ). Here we show enhancement of the BTR-mediated dHJ dissolution reaction by the heterotrimeric single-stranded DNA binding protein replication protein A (RPA). Our results suggest that RPA acts by sequestering a single-stranded DNA intermediate during dHJ dissolution. We provide evidence that RPA physically interacts with RMI1. The RPA interaction domain in RMI1 has been mapped, and RMI1 mutants impaired for RPA interaction have been generated. Examination of these mutants ascertains the significance of the RMI1-RPA interaction in dHJ dissolution. Our results thus implicate RPA as a cofactor of the BTR complex in dHJ dissolution.

Previously, we showed that ANG II receptors in cultured rat renomedullary interstitial cells (RMICs) are osmotically regulated ( 19 ). The current study examined the mechanisms underlying this osmotic regulation in RMICs cultured in isoosmotic (300 mosmol/kgH2O) and hyperosmotic (600 mosmol/kgH2O) conditions. Radioligand competition analysis coupled with RNase protection assays (RPA) and ligand-mediated receptor internalization studies revealed that RMICs primarily express the type 1a angiotensin receptor (AT1aR). When cultured under hyperosmotic conditions, the density (Bmax) of AT1R in RMIC membranes decreased by 31% [Bmax (pmol/mg protein): 300 mosmol/kgH2O, 6.44 ± 0.46 vs. 600 mosmol/kgH2O, 4.42 ± 0.37, n = 8, P < 0.01], under conditions in which no detectable changes in AT1aR mRNA expression or in the kinetics of ligand-mediated AT1R internalization were observed. RNA electromobility shift assays showed that RNA protein complex (RPC) formation between RMIC cytosolic RNA binding proteins and the 5′ leader sequence (5′LS) of the AT1aR was increased 1.5-fold under hyperosmotic conditions [5′LS RPC (arbitrary units): 300 mosmol/kgH2O, 0.79 ± 0.08 vs. 600 mosmol/kgH2O, 1.17 ± 0.07, n = 4, P < 0.01]. These results suggest that the downregulation of AT1aR expression in RMICs cultured under hyperosmotic conditions is regulated at the posttranscriptional level by RNA binding proteins that interact within the 5′LS of the AT1aR mRNA. The downregulation of AT1aR expression under hyperosmotic conditions may be an important mechanism by which the activity of ANG II is regulated in the hyperosmotic renal medulla.

The problem of establishing point-to-point communication routes in a random multistage interconnection network (RMIN) is addressed. A neural network routing scheme is presented. This routing scheme is compared to two more traditional routing techniques—namely, exhaustive search routing and greedy routing. The main criterion that is examined is the ability of each routing methodology to solve routing problems. Results are obtained through simulation of the routing methodologies for three different RMINs. The sample RMINs are relatively small since the neural network router in its present form will only be competitive for small RMINs. The simulations show that the three routing schemes perform similarly for the three sample RMINs. Other criteria that will be touched upon are the speed and the resource utilization of each routing methodology and the pros and cons of each approach will be discussed. The results suggest that neural network routers may be appropriate for some communication applications involving RMINs.

Background: This study was conducted in department of obstetrics and gynaecology, to know the efficiency of risk of malignancy index (RMI) to differentiate a malignant from a benign tumor and to compare the efficiency of risk of malignancy index 1 and 2 (RMI1 and RMI2). The study was conducted from June 2012 to August 2013 in women who got admitted with adnexal tumor.Methods: It was a prospective study. A proforma was designed for each patient which included, name, age, complaints, menopausal status, parity, past and family history and associated medical condition were asked. Indivisual parameters namely ultrasound score, CA125 and menopausal status and risk of malignancy index was calculated and compared with final histopathological diagnosis and sensitivity specificity and positive predictive value was calculated for each.Results: : The sensitivity of RMI1 is 87.95%, specificity is 75%, positive predictive value is 94.8%, negative predictive value is 54.54%, percentage of false negative is 12.04% and percentage of false positive is 25%. The sensitivity of RMI2 is 86.74%, specificity is 81.25%, positive predictive value is 96%, negative predictive value is 54.16%, percentage of false negative is 13.25% and percentage of false positive is 18.75%.Conclusions: The efficiency of RMI was definitely better than indivisual parameters and efficiency of RMI 1 and RMI2 are similar.

Summary Objective: This paper shows the use of the emerging Grid technology for gathering underused resources that are distributed among a corporate network. The work of these resources is coordinated for facing tasks which are not affordable by the individual usage of each of them. Methods: This paper shows an application for the projection, using Volume Rendering techniques, of huge medical volumes obtained from CTs and RMIs, adapted to Grid computing. Results: As a result the article shows the feasibility of the creation of an application based up on Grid technology, which solves problems that cannot be addressed by using common techniques. As an example, the article describes the projection of a huge medical dataset, which exceeds the resources of most common PCs, carried out by taking profit of idle CPU cycles from the computers of an organization. Conclusions: Grid technology is emerging as a new framework which allows gathering and coordinating resources distributed among a network (LAN or WAN), for addressing problems which cannot be solved through the single use of any of these resources. Medical Imaging is a clear application area for this technology.

This work presents a novel 4-degree-of-freedom (4-DOF) haptic master using magnetorheological (MR) fluid which is applicable to a robot-assisted minimally invasive surgery (RMIS) system. By using MR fluid, the proposed haptic device can easily generate bidirectional repulsive torque along the directions of the required motions. The proposed master consists of two actuators: an MR bidirectional clutch associated with a planetary gear system and an MR clutch with a bevel gear system. After demonstrating the configuration, the torque models of MR actuators are mathematically derived based on the field-dependent Bingham model. An optimal design that accounts for spatial-limitation and the desired torque constraint is then undertaken. An optimization procedure based on finite element analysis is proposed to determine optimal geometric dimensions. Based on the design procedure, MR haptic master with the optimal parameters has been manufactured. In order to demonstrate the practical feasibility of the proposed haptic master, the field-dependent generating repulsive force is measured. In addition, a proportional-integral-derivative (PID) controller is empirically implemented to accomplish the desired torque trajectories. It has been shown that the proposed haptic master can track the desired torque trajectory without a significant error.

During water deprivation, prostaglandin E2 (PGE2), formed by renal medullary interstitial cells (RMICs), feedback inhibits the actions of antidiuretic hormone. Interstitial PGE2 concentrations represent the net of both PGE2 synthesis by cyclooxygenase (COX) and PGE2 uptake by carriers such as PGT. We used cultured RMICs to examine the effects of hyperosmolarity on both PG synthesis and PG uptake in the same RMIC. RMICs expressed endogenous PGT as assessed by mRNA and immunoblotting. RMICs rapidly took up [3H]PGE2 to a level 5- to 10-fold above background and with a characteristic time-dependent “overshoot.” Inhibitory constants ( Ki) for various PGs and PGT inhibitors were similar between RMICs and the cloned rat PGT. Increasing extracellular hyperosmolarity to the range of 335–485 mosM increased the net release of PGE2 by RMICs, an effect that was concentration dependent, maximal by 24 h, reversible, and associated with increased expression of COX-2. Over the same time period, there was decreased cell-surface activity of PGT due to internalization of the transporter. With continued exposure to hyperosmolarity over 7–10 days, PGE2 release remained elevated, COX-2 returned to baseline, and PGT-mediated uptake became markedly reduced. Our findings suggest that hyperosmolarity induces coordinated changes in COX-2-mediated PGE2 synthesis and PGT-mediated PGE2 uptake in RMICs.

This study aims to analyse the development strategy of Bukit Lawang Ecotourism to improve the community economy and to analyse the influence of community participation, education, job opportunity, institution, and infrastructure on the community economy in Langkat Regency. This study was conducted in Bukit Lawang Ecotourism, Sei Bahorok District, Langkat Regency towards Bukit Lawang tourism visitors with a sample of 99 respondents selected using a purposive sampling technique. Data were collected using questionnaire and analysed using SWOT analysis and quantitative descriptive approach. The SWOT analysis revealed that the potential development strategy to use is S-O (Strength-Opportunity). It refers to a strategy using the strengths by taking advantage of strategic opportunities. It allows the implementation of strategies that support aggressive growth policies. Moreover, the results showed a positive influence of community participation, education, job opportunity, institution, and infrastructure on the development of Bukit Lawang Ecotourism in improving the community economy in Langkat Regency. Community participation and infrastructure significantly affect the development of the tourist site. Finally, the implication of the study is discussed further in this paper. Purpose – The purpose of this paper is to introduce a new tactile array sensor into the medical field to enhance current robotic minimally invasive surgery (RMIS) procedures that are still limited in scope and versatility. In this paper, a novel idea is proposed in which a tactile sensor array can measure rate of displacement in addition to force and displacement of any viscoelastic material during the course of a single touch. To verify this new array sensor, several experiments were conducted on a diversity of tissues from which it was concluded that this newly developed sensory offers definite and significant enhancements. Design/methodology/approach – The proposed array sensor is capable of extracting force, displacement and displacement rate in the course of a single touch on tissues. Several experiments have been conducted on different tissues and the array sensor to verify the concept and to verify the output of the sensor. Findings – It is shown that this new generation of sensors are required to distinguish the difference in hardness degrees of materials with viscoelastic behavior. Originality/value – In this paper, a new generation of tactile sensors is proposed that is capable of measuring indentation time in addition to force and displacement. This idea is completely unique and has not been submitted to any conference or journal.

Force sensing in robotic-assisted minimally invasive surgery (RMIS) is crucial for performing dedicated surgical procedures, such as bilateral teleoperation and palpation. Due to the bio-compatibility and sterilization requirements, a specially designed surgical tool/shaft is normally attached to the sensor while contacting the organ targets. Through this design, the measured force from the sensor usually contains uncertainties, such as noise, inertial force etc., and thus cannot reflect the actual interaction force with the tissue environment. Motivated to provide the authentic contact force between a robotic tool and soft tissue, we proposed a data-driven force compensation scheme without intricate modeling to reduce the effects of force measurement uncertainties. In this paper, a neural-network-based approach is utilized to automatically model the inertial force subject to noise during the robotic palpation procedure, then the exact contact force can be obtained through the force compensation method which cancels the noise and inertial force. Following this approach, the genuine interaction force during the palpation task can be achieved furthermore to improve the appraisal of the tumor surrounded by the soft tissue. Experiments are conducted with robotic-assisted palpation tasks on a silicone-based soft tissue phantom and the results verify the effectiveness of the suggested method.

Abstract Objective Integrated, real-time data are crucial to evaluate translational efforts to accelerate innovation into care. Too often, however, needed data are fragmented in disparate systems. The South Carolina Clinical & Translational Research Institute at the Medical University of South Carolina (MUSC) developed and implemented a universal study identifier—the Research Master Identifier (RMID)—for tracking research studies across disparate systems and a data warehouse-inspired model—the Research Integrated Network of Systems (RINS)—for integrating data from those systems. Materials and Methods In 2017, MUSC began requiring the use of RMIDs in informatics systems that support human subject studies. We developed a web-based tool to create RMIDs and application programming interfaces to synchronize research records and visualize linkages to protocols across systems. Selected data from these disparate systems were extracted and merged nightly into an enterprise data mart, and performance dashboards were created to monitor key translational processes. Results Within 4 years, 5513 RMIDs were created. Among these were 726 (13%) bridged systems needed to evaluate research study performance, and 982 (18%) linked to the electronic health records, enabling patient-level reporting. Discussion Barriers posed by data fragmentation to assessment of program impact have largely been eliminated at MUSC through the requirement for an RMID, its distribution via RINS to disparate systems, and mapping of system-level data to a single integrated data mart. Conclusion By applying data warehousing principles to federate data at the “study” level, the RINS project reduced data fragmentation and promoted research systems integration.

In this paper, a piezoelectric tactile sensor for detecting tissue stiffness in robot-assisted minimally invasive surgery (RMIS) is proposed. It can detect the stiffness not only when the probe is normal to the tissue surface, but also when there is a contact angle between the probe and normal direction. It solves the problem that existing sensors can only detect in the normal direction to ensure accuracy when the degree of freedom (DOF) of surgical instruments is limited. The proposed senor can distinguish samples with different stiffness and recognize lump from normal tissue effectively when the contact angle varies within [0°, 45°]. These are achieved by establishing a new detection model and sensor optimization. It deduces the influence of contact angle on stiffness detection by sensor parameters design and optimization. The detection performance of the sensor is confirmed by simulation and experiment. Five samples with different stiffness (including lump and normal samples with close stiffness) are used. Through blind recognition test in simulation, the recognition rate is 100% when the contact angle is randomly selected within 30°, 94.1% within 45°, which is 38.7% higher than the unoptimized sensor. Through blind classification test and automatic k-means clustering in experiment, the correct rate is 92% when the contact angle is randomly selected within 45°. We can get the proposed sensor can easily recognize samples with different stiffness with high accuracy which has broad application prospects in the medical field.

Learned indexes, which use machine learning models to replace traditional index structures, have shown promising results in recent studies. However, existing learned indexes exhibit a performance gap between synthetic and real-world datasets, making them far from practical indexes. In this paper, we identify that ignoring the importance of data partitioning during model training is the main reason for this problem. Thus, we explicitly apply data partitioning to index construction and propose a new efficient and updatable cache-aware RMI framework, called CARMI. Specifically, we introduce entropy as a metric to quantify and characterize the effectiveness of data partitioning of tree nodes in learned indexes and propose a novel cost model, laying a new theoretical foundation for future research. Then, based on our novel cost model, CARMI can automatically determine tree structures and model types under various datasets and workloads by a hybrid construction algorithm without any manual tuning. Furthermore, since memory accesses limit the performance of RMIs, a new cache-aware design is also applied in CARMI, which makes full use of the characteristics of the CPU cache to effectively reduce the number of memory accesses. Our experimental study shows that CARMI performs better than baselines, achieving an average of 2.2X/1.9X speedup compared to B+ Tree/ALEX, while using only about 0.77X memory space of B+ Tree. On the SOSD platform, CARMI outperforms all baselines, with an average speedup of 1.2X over the nearest competitor RMI, which has been carefully tuned for each dataset in advance.

Background: An adnexal mass patient is a common cause of hospital admission. The differentiation between malignant and benign cases is an important step in the management of such patients. The risk of malignancy index (RMI) is a simple scoring system depend on ultrasound data, menopausal status and serum concentrations of CA-125 tumor marker and has a great value in differentiation between benign and malignant adnexal masses. 4 different types of risk of malignancy index are created. The objective of the study was to compare the diagnostic performance of the four malignancy risk indices in differentiating benign and malignant ovarian tumors.Methods: This prospective study was performed on 60 patients with an adnexal mass confirmed on vaginal ultrasound.Results: There was statistical significance difference between the 4 types of RMI in benign and malignant groups. RMI 1, RMI 2, and RMI 3 had nearly the same area under the ROC curve; however, at the cut-off of>58.41, RMI 3 was more sensitive and less specific than RMI 1 or RMI 2. The RMI 2 was the most specific in predicting malignancy in terms of area under the curve; however, there was no statistically significant difference in performance of RMI 2 and 4 in malignant group.Conclusions: RMI 1, RMI 2, and RMI 3 had nearly the same area under the ROC curve; however, at the cut-off of >58.41, RMI 3 was more sensitive and less specific than RMI 1 or RMI 2, on the other hand the most specific was RMI 2 more than the other 3 RMIs.

The aim of this study was to reveal the changes of macrophyte community over time and along the course of the Ižica River. In 1996, 2000, and 2016, we surveyed the distribution and abundance of macrophyte species in the lowland Ižica River, which originates in the town of Ig and then flows through an agricultural landscape. We calculated the River Macrophyte Index (RMI), which reflects the ecological status of the river. In 2016, ecomorphological conditions of the river, using the Riparian, Channel and Environmental inventory, were also assessed. In just 10.5 km of the river, we identified 27 taxa of macrophytes, among which Potamogeton natans, Sagittaria sagittifolia, and P. perfoliatus were the most abundant. Detrended correspondence analysis showed that, in 1996, the surveyed stretches differed more according to macrophyte composition than in the following years. The assessed environmental parameters explained 43% of the variability of the macrophyte species; riverbank stability explained 20%, riverbed structure 10%, while vegetation type of the riparian zone and bottom type explained 7 and 5%, respectively. The species composition of the macrophyte community revealed significant changes over the years of the riverine ecosystem. Comparison of RMIs in 1996 revealed better conditions in the upper and middle part of the river, while in 2016, the situation was the opposite, since the conditions in the upper part deteriorated significantly over time, while the lower part of the river had the best ecological status. These changes may be due to a considerable increase in the population of the settlement Ig, while better status in the lower course of the river may be a consequence of improvements in the infrastructure and the use of sustainable agricultural practices in the catchment due to the establishment of a formal area of protection.

The proclamation of the Gospel as Good news for all men has always been at the heart of the life of the mission of the Church. This is why the new evangelization in no way modifies the content of the Gospel, the message of salvation brought by Christ. But in the current global context, it is made ever more urgent and necessary. Faced with all these new phenomena that permeate the contemporary world and the Church, renewed enthusiasm and enthusiasm is needed to respond to the urgency and the need for a new evangelization. This inner impulse can only be the result of a rich and singular personal spirituality, without which the evangelizer runs the risk of quickly becoming exhausted or discouraged: Missionary activity requires a specific spirituality which concerns in particular those whom God has called missionaries (RMis 87). This specific spirituality is nourished by the meditation of Christ the Savior - the Word of God. Through this meditation, the new evangelizers experience being healed by God through Jesus Christ. The encounter with the living God is an entirely original, transformative experience that puts everything in its place and completely upsets reality. This results in the ardor of evangelization that is constantly renewed. All evangelization requires from those who announce, a testimony of life that attests to the truth and the reality of the Gospel. To give one&#39;s testimony on mission is to give an account of what we have lived, seen and heard, and meditation is a good way to carry out this reality. Evangelization has a mystical origin; it is a gift that comes from the cross of Christ the Savior. That is why Christian meditation rooted in the tradition of the Church - especially in the Gospel - is a means for the new evangelization.

There is a growing, widespread trend of adopting robot-assisted minimally invasive surgery (RMIS) in clinical care. Dry lab robot training and virtual reality simulation are commonly used to train surgical residents; however, it is unclear whether both types of training are equivalent or can be interchangeable and still achieve the same results in terms of training outcomes. In this paper, we take the first step in comparing the effects of physical and simulated surgical training tasks on human operator kinematics and physiological response to provide a richer understanding of exactly how the user interacts with the actual or simulated surgical robot. Four subjects, with expertise levels ranging from novice to expert surgeon, were recruited to perform three surgical tasks — Continuous Suture, Pick and Place, Tubes, with three repetitions — on two training platforms: (1) the da Vinci Si Skills Simulator and (2) da Vinci S robot, in a randomized order. We collected physiological response and kinematic movement data through body-worn sensors for a total of 72 individual experimental trials. A range of expertise was chosen for this experiment to wash out inherent differences based on expertise and only focus on inherent differences between the virtual reality and dry lab platforms. Our results show significant differences ([Formula: see text]-[Formula: see text]) between tasks done on the simulator and surgical robot. Specifically, robotic tasks resulted in significantly higher muscle activation and path length, and significantly lower economy of volume. The individual tasks also had significant differences in various kinematic and physiological metrics, leading to significant interaction effects between the task type and training platform. These results indicate that the presence of the robotic system may make surgical training tasks more difficult for the human operator. Thus, the potentially detrimental effects of virtual reality training alone are an important topic for future investigation.

Background: Ovarian cancer possesses a challenge to screening tests due to its anatomical location, poor natural history, lack of specific lesion, symptoms and signs and low prevalence. Authors shall be considering RMI 2 and RMI 4 (forms of RMI) and comparing them with histopathology report to derive the sensitivity, specificity and other parameters of these tests.Methods: A prospective study was conducted from September 2016- September 2017 at Mazumdar Shaw Hospital, Narayana Hrudayalaya, Bangalore.73 patients met the inclusion criteria. RMI 2 and RMI4 were calculated for all the patients and these scores were compared to the final histopathology reports.Results: In present study of 73 patients RMI2 showed a sensitivity of 86.6%, specificity of 86.5 %, Positive predictive value of 81.25% and negative predictive value of 90.24 %. Whereas RMI4 showed a sensitivity of 86.6%, specificity of 86.5 %, Positive predictive value of 83.87 and negative predictive value of 90.48 %. These results are comparable to other studies conducted. The risk of malignancy index 2 and 4 are also almost comparable with each other and so either can be used for determining the risk of malignancy in patients with adnexal masses. These results were derived in an Indian population across all age groups showing that authors can apply this low-cost method even in resource limited settings.Conclusions: Authors found that Risk of malignancy index is a simple and affordable method to determine the likelihood of a patient having adnexal mass to be malignant. This can thus help save the resources and make the services available at grass root level.

ObjectiveTo assess the current use of big data and artificial intelligence (AI) in the field of rheumatic and musculoskeletal diseases (RMDs).MethodsA systematic literature review was performed in PubMed MEDLINE in November 2018, with key words referring to big data, AI and RMDs. All original reports published in English were analysed. A mirror literature review was also performed outside of RMDs on the same number of articles. The number of data analysed, data sources and statistical methods used (traditional statistics, AI or both) were collected. The analysis compared findings within and beyond the field of RMDs.ResultsOf 567 articles relating to RMDs, 55 met the inclusion criteria and were analysed, as well as 55 articles in other medical fields. The mean number of data points was 746 million (range 2000–5 billion) in RMDs, and 9.1 billion (range 100 000–200 billion) outside of RMDs. Data sources were varied: in RMDs, 26 (47%) were clinical, 8 (15%) biological and 16 (29%) radiological. Both traditional and AI methods were used to analyse big data (respectively, 10 (18%) and 45 (82%) in RMDs and 8 (15%) and 47 (85%) out of RMDs). Machine learning represented 97% of AI methods in RMDs and among these methods, the most represented was artificial neural network (20/44 articles in RMDs).ConclusionsBig data sources and types are varied within the field of RMDs, and methods used to analyse big data were heterogeneous. These findings will inform a European League Against Rheumatism taskforce on big data in RMDs.

NFκB plays a critical role mediating COX2 expression in renal medullary interstitial cells (RMICs). Thetrans-activating ability of NFκB can be modified by another nuclear factor C/EBPβ that can physically bind to NFκB and regulate its activity. Because the COX2 promoter also contains a C/EBPβ site adjacent to the NFκB site, the present study examined whether these two transcription factors cooperate to induce COX2 expression following hypertonic stress. Hypertonicity markedly induced COX2 expression in cultured medullary interstitial cells by immunoblot analysis. The tonicity-induced COX2 expression was suppressed by mutant IκB (IκBm) that blocks NFκB activation, demonstrating that tonicity-induced COX2 expression depends on NFκB activation. However, mutation of the NFκB site in the COX2 promoter failed to abolish tonicity-induced COX2 reporter activity. IκB kinase-1 (IKK1) significantly induced COX2-luciferase activity by 2.3-fold (n= 10,p< 0.01); mutation of the NFκB site also failed to abolish IKK1-stimulated COX2 reporter activity (86 ± 3.1% of wild type,p> 0.05,n= 4). Interestingly, mutation of the C/EBPβ site of theCOX2gene significantly reduced both IKK1 and hypertonicity-induced COX2 reporter activity (p< 0.01). To further examine the potential role of C/EBPβ in tonicity-induced COX2 expression, a dominant negative C/EBPβ-p20 was transduced into RMICs. C/EBPβ-p20 markedly suppressed hypertonic (550 mOsm) induction of COX2 (immunoblot) to a similar extent as IκBm. No additional suppression was observed when both NFκB and C/EBPβ were simultaneously blocked by IκBm and C/EBPβ-p20. Interestingly, IKK-induced COX2 expression was not only blocked by IκBm, but also completely abolished by C/EBPβ-p20. Further studies demonstrated physical association of C/EBPβ to NFκB p65 by coimmunoprecipitation. Importantly, this interaction between C/EBPβ and NFκB was greatly enhanced following hypertonic stress. These studies indicate C/EBPβ is required for the transcriptional activation of COX2 by NFκB, suggesting a dominant role for the C/EBPβ pathway in regulating induction of RMIC COX2 by hypertonicity.

Renomedullary interstitial cells (RMICs) are prominent in the inner medullary interstitium and have binding sites for several vasoactive agents, including angiotensin II (ANG II). Although the functional role of RMICs remains largely unknown, it is likely that the interaction between RMICs and vasoactive peptides is important in the regulation of renal function. The current investigation characterizes the cellular responses following treatment of RMICs with ANG II. Studies were performed on RMICs isolated from Sprague-Dawley rat kidneys. 125I-labeled [Sar1,Ile8]ANG II specifically bound to RMICs at sites determined by reverse transcription-polymerase chain reaction to be of the AT1A subtype. ANG II (10(-6) and 10(-10) M) had no effect on either basal or forskolin-stimulated adenosine 3',5'-cyclic monophosphate accumulation in RMICs but increased intracellular inositol 1,4,5-trisphosphate concentration after 10 s and intracellular calcium concentration after 18 s. For RMICs plated at low densities, ANG II (10(-6) M) induced an increase in [3H]thymidine incorporation, mediated through the AT1-receptor subtype. For RMICs plated at high densities, ANG II (10(-6) M) induced an increase in extracellular matrix synthesis as detected by trans-35S incorporation, an effect also mediated by AT1 receptors. We conclude that ANG II AT1A receptors on cultured RMICs are coupled to intracellular second messenger pathways leading to hyperplasia and synthesis of extracellular matrix.

ABSTRACT Genome stability requires a set of RecQ-Top3 DNA helicase-topoisomerase complexes whose sole budding yeast homolog is encoded by SGS1-TOP3. RMI1/NCE4 was identified as a potential intermediate in the SGS1-TOP3 pathway, based on the observation that strains lacking any one of these genes require MUS81 and MMS4 for viability. This idea was tested by confirming that sgs1 and rmi1 mutants display the same spectrum of synthetic lethal interactions, including the requirements for SLX1, SLX4, SLX5, and SLX8, and by demonstrating that rmi1 mus81 synthetic lethality is dependent on homologous recombination. On their own, mutations in RMI1 result in phenotypes that mimic those of sgs1 or top3 strains including slow growth, hyperrecombination, DNA damage sensitivity, and reduced sporulation. And like top3 strains, most rmi1 phenotypes are suppressed by mutations in SGS1. We show that Rmi1 forms a heteromeric complex with Sgs1-Top3 in yeast and that these proteins interact directly in a recombinant system. The Rmi1-Top3 complex is stable in the absence of the Sgs1 helicase, but the loss of either Rmi1 or Top3 in yeast compromises its partner's interaction with Sgs1. Biochemical studies demonstrate that recombinant Rmi1 is a structure-specific DNA binding protein with a preference for cruciform structures. We propose that the DNA binding specificity of Rmi1 plays a role in targeting Sgs1-Top3 to appropriate substrates.
