Texte intégralABSTRACT: PERSONALITY AND PSYCHIATRIC REHABILITATION INTRODUCTION: psychiatric rehabilitation in Therapeutic Communities is aimed to let in care patients regain an adequate relational and occupational functionality, allowing their social reintegration. However, there are no reports in the literature concerning the predisposition to participate to the activities that have been previously agreed upon, according to patients’ individual care plans. Therefore, the relationship between personality traits of borderline inpatients of the Therapeutic Community “Villa Ratti”, assessed using MMPI-2, and percent of interventions performed, is investigated. METHODS: 62 patients (27 Men, 35 Women, mean age=28, sd=6,6) have performed a total of 14540 interventions, comprising 24 categories, between January 2009 and December 2010. MMPI-2 scores and school degree were available only for 50 patients (20 M, 30 W). 4 women of them were clearly outliers because of their extremely low scores in all scales. For the remaining 46 patients (20 M, 26 W), correlations and multiple regressions were analyzed, in order to evaluate predictive values of mmpi 2 scores on the percentiles of interventions. RESULTS: some mmpi-2 scales were significantly correlated with 8 out of 24 percent interventions. Moreover, some scales were significantly predictive, with R2 values spanning the range 0,1 - 0,4. However, not all scales resulted to be predictive: Lie (L), Hypochondria (Hs) and Psychopathic Deviate (Pd) do not exert significant effects onto the attendance to activities. CONCLUSIONS: In this study, significant relationships between some of the mmpi-2 scores and percentiles of interventions performed, were found. These results give an indication of which aspects of personality can affect the tendency of patients to partecipate in specific activities, independently of what had been previously decided in the Therapeuthic-Rehabilitative Project. The limits of this approach and the future potential developments are discussed.

Texte intégralIntroduction: Cognitive dysfunction is a major feature of bipolar disorder (BD), strongly associated with patients’ functional outcome. Functional Remediation (FR) is a psychosocial intervention, developed by the Bipolar and Depressive Disorders Unit of the Hospital Clinic of Barcelona, exclusively designed on the specific characteristics of bipolar patients, aimed at improving neurocognition in order to achieve a functional recovery (Martinez-Arán et al., 2011). FR program is built on a neuro-cognitive-behavioral approach, manualized and evidence-based, and its distinctive feature is that it focuses on the overall psychosocial functioning, in a group format (Martinez-Arán et al., 2011; Torrent & Vieta 2016). FR, within a highly ecological context, provide neurocognitive strategies and techniques, including psychoeducation about the main neurocognitive impairments associated with BD, such as attention, memory and executive functions (Martinez-Aran et al. 2004) and their influence on everyday life, which can result in psychosocial difficulties (Vieta et al., 2018). The main aim of FR is to facilitate generalization and the transfer of learned cognitive abilities and useful strategies to better manage daily life, with the ultimate goal to restore psychosocial functioning in BD (Martínez-Arán et al., 2011; Bonnin et al., 2014). FR efficacy was validated in a multicentric, randomized, rater-blind trial, comparing FR with psychoeducation and treatment as usual - TAU, which indicated an improvement in general psychosocial functioning, principally in the interpersonal and occupational domains (Torrent et al., 2013). FR is also effective in improving psychosocial functioning in both bipolar disorder type I and II (Solé et al., 2015).¬¬ Objectives: to evaluate the effectiveness of FR in improving psychosocial functioning (measured by Functioning Assessment Short Test - FAST) and neurocognitive impairments (measured by Brief Assessment of Cognition in Affective Disorders - BAC-A), in a sample of euthymic patients with BD. Methods: In a randomized, rater-blinded, controlled study of 24 out-patients with BD-I and BD-II, according to DSM-5 criteria, FR (N=12) was compared with TAU (N=12) over 21 weeks. Pharmacological treatment was kept stable in both two groups. The primary outcome was change in psychosocial functioning measured by means of the FAST, from baseline to endpoint. Results: At the end of the study, all 24 patients finalized the treatment phase. Statistical analyses revealed that the FR group improved significantly with respect to functional outcome variables (p=.011), from the baseline to the end of the programme, at 6 months, compared to the TAU group. Significant improvements have been identified in the FAST sub-domains relating to autonomy, professional functioning, financial issues and interpersonal relations. No significant effects of the FR intervention group on neurocognitive variables were detected. Conclusions: Findings are promising and confirm the importance to new personalized non-pharmacological interventions in BD, in order to improve not only affective symptoms, but also cognitive and functional dysfunctions, with the final goal to achieve full functional recovery and ameliorate the quality of life of BD patients’.

Texte intégralIn the first paper, the relationship between hospital admissions due to psychiatric disorders and the severe economic downturn caused by the 2008 financial crisis was studied. Also, the buffering mechanism exerted by social protection (SP) was considered. Among women, increased unemployment was associated with increased hospitalizations due to organic and senile mental disorders, while decreased gross domestic product (GDP) was associated with increased hospitalizations due to all psychiatric disorders, mood disorders, alcohol-related disorders, and substance-related disorder. Among men, increased unemployment was associated with increased hospitalizations due to all psychiatric disorders, mood disorders, schizophrenia and other psychotic disorders, and organic and senile mental disorders. Decreased GDP was associated with increased hospitalizations due to all psychiatric disorders and alcohol-related disorders. SP buffered the negative mental health outcomes caused by decreased GDP in both genders, specifically with respect to alcohol-related disorders. Between 2008 and 2014 hospitalizations cost exceeded 79,425,797 euros at national level, 11,346,542 euros per year. In the second paper, the efficacy and effectiveness of vocational rehabilitation programs (VRPs) carried out at the Modena Mental Health Department (MHD), and their impact on users’ employability were assessed. A retrospective study was used. All users of the Modena MHD included in VRPs in 2018 were enrolled. A before-after analysis was carried out. Also, exposed users were compared to a non-experimental control group, made up of users not included in VRPs in 2018, homogeneous in terms of socio-demographic characteristics and variables, diagnoses and severity level. In 2018, 62 users ended the VRP (women 29, 46%; mean age 43±13 years old), thus representing the sample of exposed users. The before-after comparison showed that after the beginning of VRPs, the number and days of hospitalization significantly decreased, and no compulsory hospitalization was needed. Also, the median of urgent health interventions declined. In 2018, VRPs made it possible to reduce costs up to 49,243.50 euros, i.e. 794.25 euros per user. Of the 62 users who ended VRPs, on 31 December 2018 twenty-seven were employed (44% of the sample). Eleven users had an open-end contract while sixteen users had a fixed-term contract. Of the latter, six were later changed in open-term contracts. The third paper investigated the association between work environment factors and risk of depression after retirement. A secondary retrospective analysis was carried out using data from the Survey of Health, Ageing and Retirement in Europe (SHARE), wave 6 and 7. The latter provided retrospective data on the respondents’ working conditions before retirement. Binary logistic regressions were used to analyze the association between presence of depression after retirement (detected by the Euro-D scale) and work environment factors in 584 individuals. With respect to work environment factors only fair salary was associated with reduced risk of depression after retirement (OR=0.75, p=0.047). Female gender, number of chronic diseases and presence of the partner in the household increased the risk of depression (OR=1.84, p<0.01; OR=1.58, p<0.01; OR=1.45, p<0.01, respectively), while higher education decreased it (OR=0.88, p=0.048). With respect to personality, the risk of depression after retirement increased a higher level of neuroticism (OR=1.49, <0.01). Differently, increased consciousness was associated with decreased risk of depression (OR=0.73, p=0.06).
