Algorithms are the hidden methods that computers apply to process information and make decisions. The book tells the story of algorithms from their ancient origins to the present day and beyond. The book introduces readers to the inventors and events behind the genesis of the world’s most important algorithms. Along the way, it explains, with the aid of examples and illustrations, how the most influential algorithms work. The first algorithms were invented in Mesopotamia 4,000 years ago. The ancient Greeks refined the concept, creating algorithms for finding prime numbers and enumerating Pi. Al-Khawrzmi’s 9th century books on algorithms ultimately became their conduit to the West. The encounter of “personalized experiences”—targeted advertisements, tailored information feeds, and “recommended” content, among other things—is now a common and somewhat inescapable component of digital life. More often than not however, “you” the user are not primarily responsible for personalizing your web engagements: instead, with the help of your search, browsing, and purchase histories, your “likes,” your click-throughs, and a multitude of other data you produce as you go about your day, your experience can “conveniently”—and computationally—be personalized on your behalf. This book explores a host of new questions that emerge from web users’ encounters with these forms of algorithmic personalization. What do users “know” about the algorithms that apparently “know” them? If personalization practices seek to act on users’ behalf (for instance, by deciding what content is personally relevant), then how do users retain or relinquish their autonomy? Indeed, what kinds of selfhoods are made possible when personalization algorithms intervene in identity construction? Making It Personal is the first full-length monograph to critically analyze the sociocultural implications of algorithmic personalization through the accounts and testimonies of web users themselves. At the heart of the book are interviews and focus groups with web users who—through a myriad of resistant, tactical, resigned, or trusting engagements—encounter algorithmic personalization as part of their lived experience on the web. The book proposes that for those who encounter it, algorithmic personalization creates new implications for knowledge production, autonomy, cultural capital, and formations of self.

Evaluation of the obese patient requires identification of all comorbidities and health conditions, including underlying cardiac and pulmonary conditions that could have a negative outcome on noncardiac surgery. Once comorbidities have been recognized, aggressive optimization of these medical conditions can provide improved outcomes after bariatric surgery. Estimating medical risk can be performed by several validated classification systems. The preoperative checklist and clinical practice guidelines (CPG) were updated in 2013. The CPG recommendations for preoperative evaluation of the bariatric surgery patient include lab testing, nutritional screening, endocrine assessment, and cardiopulmonary assessment, including sleep apnea screening. The CPG suggest an extensive multidisciplinary team approach to the preoperative bariatric surgery patient. Finally, the medical evaluation includes an algorithm for a seven-step approach to the preoperative visit. Also recommended for evaluation of the morbidly obese patient is an algorithm that uses a five-step approach after a comprehensive history and physical exam and lab testing.

Peripheral polyneuropathy is a common clinical presentation, and most cases seen in clinical practice are axonal, dying-back, sensorimotor polyneuropathies. Axonal polyneuropathies should be distinguished from the acquired demyelinating peripheral polyneuropathies which are often immune-mediated and amenable to treatment. This case presents a typical patient with a dying-back sensorimotor axonal peripheral polyneuropathy and presents a practical approach to the etiologic diagnoses of peripheral neuropathy. It then highlights its distinguishing features on nerve conduction studies. A comparison between axonal and demyelinating polyneuropathies is revealed. An algorithmic approach to the workup of a patient presenting with a peripheral neuropathy is recommended.

Recently, a number of point-of-care tests for evaluation of platelet function and the efficacy of antiplatelet therapy has been investigated. Thrombophilia contributes to the risk of thrombosis, and a battery of complex assays is required to identify all thrombophilias. Disseminated intravascular coagulation is characterized by microthrombosis and clinical bleeding. A scoring system for overt disseminated intravascular coagulation provides a five-step diagnostic algorithm. The cornerstone of the management of disseminated intravascular coagulation is treatment of the underlying triggering condition. Heparin-induced thrombocytopenia is an adverse immunological effect of heparin therapy. Besides thrombocytopenia, the major clinical consequence of heparin-induced thrombocytopenia is an increased risk of thrombosis. The diagnosis is based on clinical symptoms and detection of platelet-activating heparin-induced thrombocytopenia antibodies. When heparin-induced thrombocytopenia is strongly suspected, it is recommended to stop heparin treatment, investigate for heparin-induced thrombocytopenia antibodies, and initiate non-heparin anticoagulant treatment.

Patient comfort is a primary goal in ICU, but achieving and maintaining the appropriate balance of analgesia, sedation, and treatment of delirium is frequently challenging. International guidelines recommend keeping critically-ill patients calm and cooperative, awake in daytime and asleep at night, always avoiding deep sedation. To state the actual level of sedation and the desired one, it is necessary to frequently perform a sedation assessment with validated tools. Subjective methods are the most useful guides in ICU consciously-sedated patients, representing the gold standard for good clinical practice. Use of such a scale is a key component of sedation algorithms. The ideal scoring system should be easy, reliable, sensitive, and with minimal interobserver variability, giving no or minimal additional discomfort to the patient. Most of the proposed tools are a compromise between accuracy and time required for evaluation; the most used are the Richmond Agitation-Sedation Scale and the Sedation-Agitation Scale.
