Texte intégralChronic pain is a prevalent disorder that is difficult to treat and has the potential to be stigmatizing for individuals with chronic pain. An exploratory literature review was conducted to examine chronic pain stigma. The review specifically investigated studies that addressed individuals’ experiences of chronic pain stigma, identified sources of chronic pain stigma, and chronic pain stigma’s impact on treatment adherence. The following key terms were used to search 15 computerized databases (i.e. chronic pain AND stigma OR stigmatization OR disbelief OR stereotype OR invisible OR misconception). Studies were reviewed if they were relevant to chronic pain stigma, were conducted in the United States, the majority of participants were adults with a chronic pain condition, chronic pain condition was non-malignant, chronic pain was defined as persistent or intermittent for at least 3 months, and stigma was defined as being labeled, stereotyped, devalued, or discredited. The final review included 18 qualitative and quantitative studies from 1989-2015 that examined chronic pain and stigma. The findings of the study included twelve key themes that shed light on the experiences of stigma, sources of stigma, and impact on treatment adherence. Experiences of stigma included: estrangement, self-stigmatization, invisibility, medication stigma, and disbelief. The participants in the studies identified three sources of stigma: doctors, public, and family. Four themes emerged from the few studies that addressed treatment adherence: difficulty communicating about pain, management strategies, to seek care or not seek care, and to not take medications due to fears of addiction and stigma. The findings are further discussed in relation to the United State’s societal values, beliefs, and culture. A cultural shift is suggested in how we view chronic pain and illness. Additionally, recommendations are made for the role of psychologists addressing chronic pain stigma from a multi-systems level approach. Texte intégralThe cultural movement toward abbreviation and multitasking detracts from the desire and also, potentially, the ability to begin engaging in deep reading. This is problematic, considering that the deep reading of narrative fiction has many potential benefits. Through the use of both heuristic and hermeneutic methodology, this thesis takes a look at the psychological and scientific literature that explores the relationship between reading narrative fiction and an increase in both empathy and theory of mind skills; how the art of reading narrative fiction can be viewed from a depth psychological perspective; and the author’s personal experience with reading narrative fiction. This information is then utilized to explore how these components can be applied within the psychotherapy process through the use of bibliotherapy, as well as how it can be a valuable tool for one’s own personal growth and exploration outside of a therapy setting.

Texte intégralBy focusing on the predominance of semi-conscious and unconscious states in both nineteenth-century British literature and psychology, this dissertation outlines the recognizable and multi-faceted relation existing between literature and psychology. Besides their obvious prevalence in sensation novels later in the period, these states, which I call ecstatic states, appeared in many of the most prominent, canonical novels of the nineteenth century. Prominent Victorian psychologists, such as Robert MacNish, John Abercrombie, James Cowles Prichard, and Forbes Winslow among others, connected ecstatic states, including fiction reading, to insanity, since these states exhibited an underlying component of self-loss in which the boundaries of the conscious self--time, will, and identity--dissolved. They were a troubling, yet common phenomenon of the mind that preoccupied the entire spectrum of middle class Victorian intellectual life--businessmen, novelists, literary critics, and psychologists--and these states are still fascinating neuroscientists today. This study shows how the Victorian medical practice of moral management sought to control these states by calling for the regulation and often the confinement of the imagination. What began as a method used solely in the insane asylum came to undergird much of Victorian life, including the many hostile reactions to the addictive and class-leveling powers of the novel. My dissertation emphasizes how certain Victorian novelists not only took up the role of psychologists themselves but also resisted and revised accepted psychology within their novels. Charlotte Brontë, Charles Dickens, and George Eliot reacted in distinctive ways against the oppressive tenets of moral management. My readings of the novels Jane Eyre, Villette, Hard Times, Our Mutual Friend, The Mill on the Floss, and Romola show how it is the unrelenting regulation of the imagination that creates the various forms of mania and becomes ultimately devastating to the self. For these novelists, the dismantling of conscious thought and will, so alarming to the advocates of moral management, formed the crux of personal growth, moral choice, and ethical responsiveness

Texte intégralENGLISH ABSTRACT: Pregnancy has evolved from being predominantly understood as a medical phenomenon to what psychoanalytic theorists would regard as a holistic experience encompassing both physiological and psychological changes. According to psychoanalytic theorists, pregnancy is a transitional phase and a time of susceptibility and flux for most women. This often results in psychic turmoil where boundaries between conscious and unconscious process become more permeable. The pregnant woman's dreams and fantasies create an inner working model of relationships and this in tum provides a template of how her relationship with her baby will be experienced and conducted. This link to the unconscious increases insight into the process occurring between the woman and her evolving relationship with the fetus and provides the health professional with clues for early intervention. However care should be taken by health-care professionals to communicate the psychological processes during pregnancy within the cultural framework of the pregnancy mother for positive outcomes to be achieved.
AFRIKAANSE OPSOMMING: Die wyse waarop swangerskap verstaan word het ontwikkel vanaf grotendeels mediese verskynsel na wat die psigoanalitiese teoretici sal beskou as 'n holistiese ervaring wat beide die fisiologiese en die sielkundige veranderinge insluit. Die psigoanalitiese teoretici beskou swangerskap as 'n oorgangs fase en 'n periode van vatbaarheid en veranderlikheid vir die meeste vroue. Dit gee dikwels aanleiding tot psigiese wanorde waar die grense tussen bewustelike en onbewustelike prosesse meer deurdringbaar word. Die swanger vrou se drome en fantasië skep 'n innerlike werkende model van verhoudings en op sy beurt voorsien dit templet van hoe haar verhouding met haar baba ervaar en hanteer sal word. Hierdie band met die onbewustelike verleen insig in die proses wat tussen die vrou en haar ontwikkelende verhouding met die fetus voorkom en voorsien die gesondheids werker van leidrade vir vroeë intervensie. Die gesondheids werker moet egter versigtig wees om die sielkundige prosesse gedurende swangerskap binne die kulturele raamwerk van die swanger moeder te interpreteer om sodoende positiewe uitkomste verkry.

Texte intégralENGLISH ABSTRACT: Maternal filicide, the murder of a child by its mother, is a complex phenomenon with various causes and characteristics. Research, by means of the development of several classification systems and in identifying particular risk factors, has been conducted with the aim of better prevention of this emotionally evocative crime. Various disciplines have offered a wide range of perspectives on why women kill their biological children. These are intended to yield a better understanding of the aetiology of this crime. This literature review delineates three dominant perspectives: psychiatric, psychological, and sociological. The main findings of each perspective are discussed. However, these three perspectives frequently operate in conjunction with each other in that both intrapsychic and interpersonal dynamics play a role in acts of maternal filicide. The most vulnerable women appear to be those who have had a severely deficient developmental history (trauma and/or grossly inadequate parenting), those who experience current difficult psychosocial circumstances, and those who have been diagnosed with a psychiatric illness. However, not all women who experience such problems kill their children. In this regard, individual differences have an important role to play and more carefully delineated future research is suggested. One of the most significant findings of this literature review is that there appears to be a paucity of systematic research on the South African phenomenon of parental child homicide.
AFRIKAANSE OPSOMMING: Moedermoord, die moord van ’n kind deur sy of haar moeder, is ’n komplekse verskynsel met verskeie oorsake en karaktereienskappe. Navorsing deur die ontwikkeling van verskeie klassifikasiestelsels en die identifisering van spesifieke risikofaktore is uitgevoer met die doel om hierdie misdaad, wat soveel emosies ontlok, beter te voorkom. Verskeie dissiplines bied ’n wye verskeidenheid perspektiewe oor die redes waarom vroue hul biologiese kinders vermoor. Die doel van hierdie perspektiewe is om ’n beter etiologiese begrip van hierdie vorm van misdaad te verkry. Die literatuurstudie dui drie dominante perspektiewe aan: psigiatries, psigologies en sosiologies. Die hoofbevindinge van elke perspektief word bespreek. Hierdie drie perspektiewe werk dikwels saam aangesien sowel intrapsigiese en interpersoonlike dinamiek ’n rol in moedermoorddade speel. Die kwesbaarste vroue blyk dié te wees met ’n ernstig gebrekkige ontwikkelingsgeskiedenis (trauma en/of ernstig onvoldoende ouerskap), diegene wat hulle in moeilike psigososiale omstandighede bevind, en dié wat met ’n psigiatriese siekte gediagnoseer is. Nie alle vroue wat hierdie probleme ervaar, vermoor egter hulle kinders nie. In hierdie opsig speel individuele verskille ’n belangrike rol en word versigtig afgebakende toekomstige navorsing voorgestel. Een van die belangrikste bevindinge van hierdie literatuuroorsig is dat daar ’n gebrek aan sistematiese navorsing oor die Suid-Afrikaanse verskynsel van kindermoord deur ouers blyk te wees.

Texte intégralThe significant changes to the political landscape of South Africa since the abolition of apartheid and the implementation of democracy have had far-reaching effects in social order and gender relations. With the new dispensation has come the promise of new opportunities for men and women of all races to participate fully in the creation of a multicultural society, making the issue of transformation an important agenda. As a social artifact, children's literature has also been influenced by these changes, and the didactic function of this medium make it an interesting site to explore the ways in which historical stereotypes are both perpetuated and challenged. This study focused on the representation of black masculinity in a sample of South African children's literature published after apartheid. The aim was to investigate how race, gender, and class intersect in the representation of black masculinity.

Texte intégralPsychotherapy interns often report feeling traumatized by the process that attempts to match them with an internship site. Once placed, feeling unsupported in one's supervisory relationship may lead to burnout, which contributes to high rates of attrition. A supportive relationship between supervisors and interns appears significant to the development of their early professional resilience. This integrative literature review asks, "How do therapists at all stages of their career achieve and maintain professional resilience?" and "What is inherent in the supervisory relationship that influences such growth and resilience in interns?"
Literature on psychotherapy supervision, professional trauma, compassion fatigue, and resilience was selected from the psychodynamic, psychoanalytic, and existential-humanistic traditions as well as from relational neuroscience. This literature was integrated in such a way as to define, compare and contrast these concepts.
Psychotherapists report a variety of historic traumas that contribute to their choice of psychotherapy as a profession. Therapists also identify as falling on a dynamic spectrum of resilience, reporting both personal and professional protective and risk factors. Psychodynamic, psychoanalytic, and existential-humanistic training and professional models continue to highlight the value of the supervisory and consultant relationship in support of recovery from professional overwhelm toward lasting personal and professional resilience. Therapists at all stages of their career report achieving and maintaining professional resilience by practicing individualized self-care, engaging in dynamic personal psychotherapy, and through supportive relationships in supervision and consultation. The primary element in the supervisory relationship that promotes growth and resilience in interns is the ability of the empathic supervisor to privilege the supervisee's experience in the supervisory relationship as well as with their mutual clients.
A supervisory model emerged that (a) elevates personal history and awareness of an intern's preexisting risk and protective factors, (b) promotes in-session self-awareness, and (c) draws on existential-humanistic theory, leading to the development of resilience. This model serves to support the humanity of the intern and supervisor as evolving professionals while respecting and perhaps enhancing the orientation of the training site.

Texte intégralThe present study is a critical analysis of the literature about motivation in the treatment of anorexia nervosa. This dissertation provides a comprehensive overview of the theoretical and empirical literature on motivation to change in the treatment of anorexia nervosa, including discussion of the support for factors hypothesized to be associated with treatment motivation as well as identification of factors that require additional study. It will also provide conceptual clarity of motivation as a construct in anorexia nervosa treatment research. Current literature reveals how differences in the way motivation is conceptualized, studied, and assessed may limit our understanding of its role in anorexia nervosa treatment. Recommendations are therefore made regarding the adoption of a more consistent and shared understanding of the construct.

Texte intégralChronic over-activity of the body's endocrine stress response system is detrimental to overall health and, over time, may have a negative impact on the structure and function of the hippocampus, a key brain area involved in episodic memory consolidation. Yoga is becoming an increasingly popular mind-body therapy used to reduce and prevent the harmful effects of stress on the body. This review presents a summary of the research investigating yoga as a therapeutic intervention to reduce both perceived and physiological stress in healthy adults. Studies looking at the effects of yoga on hippocampal-dependent memory function in children, adults, and elderly populations were also reviewed. Research investigating the stress-reducing effects of yoga has indicated that yoga may hold therapeutic value in reducing both perceived and physiological stress in healthy populations. In children, yoga has been shown to improve spatial, but not verbal, memory. In adults and elderly individuals, studies indicate improvements in both short and long-term verbal recall following yoga intervention. Elderly individuals also showed increased hippocampal volume following long-term yoga practice. Due to the shortage of empirical evidence, along with several shared methodological limitations, further investigation is still needed to fully determine the efficacy of yoga as a beneficial mind-body therapy for decreasing both perceived and physiological stress-response, improving memory, and preventing stress and age-related hippocampal volume loss.

Texte intégralENGLISH ABSTRACT: This paper is an attempt to explore the appropriateness of the use of the specific psychological paradigm of dissociation as a possible response to pregnancy in lowincome women. Low-income women who fail to acknowledge their pregnancies may do so in an attempt to defend against feelings of fear, anxiety and helplessness engendered by the crisis of pregnancy. A dissociative response may serve to protect the expectant mother from conscious awareness of the full impact of what is happening, in other words, to provide psychological escape when physical escape is not possible. Such a response may initially be adaptive in that it provides these women with a means of coping. Dissociative processes allow, in the context of an inescapable crisis, the economizing of already limited physical and psychological resources. However, the prolonged use of dissociative strategies during pregnancy may have far-reaching implications, and, as such, be a hindrance to adaptive functioning.

Texte intégralCette these a trois objectifs principaux et se divise en deux manuscrits distincts mais liés: Dans le premier manuscrit, intitulé A Review of Client Involvement in Psychotherapy, le but est de fournir un examen approfondi de la littérature de la participation et d'avancer une conceptualisation de la participation qui clarifiera notre compréhension de la participation. Précisément, nous maintenons que la conceptualisation de la participation est composée de deux facettes, le processus interne et le processus externe, qui indiquent si un client participera à la thérapie. Le but du deuxième manuscrit, intitulé Factor Analysis and Construct Validity of a Measure of Client Involvement, est d'évaluer un modèle des facteurs de la participation des clients basé sur les données empiriques. La mesure de la participation des clients est l'Echelle exhaustive des concepts des séances de psychothérapie, sous-échelle Participation des Clients (Comprehensive Scale of Psychotherapy Session Constructs – Client Involvement subscale [CSPSC-CI; Eugster & Wampold, 1996]), qui n'a jamais été évaluée de façon indépendente. Nous avons examiné les propriétés psychométriques de cette mesure. L'analyse des facteurs de la sous-échelle CSPSC-CI a révélé trois facteurs distincts: un facteur de Client, un facteur de Thérapeute Actif, et un facteur de Thérapeute Inattentif. Nous avons également évalué la validité de concept de la mesure, et les résultats de l'étude fournissent un appui préliminaire de la validité de concept de la sous-échelle.

Texte intégralENGLISH ABSTRACT: This literature review explores the research on the clinical presentation of childhood onset schizophrenia (COS) that has been conducted over the past ten years. A literature search was done using internet search engines and psychological databases to collect English language journals from 1994 onwards. Research indicates that COS is a stable diagnosis. Generally, there is a clear history of premorbid abnormalities, an insidious onset and a deteriorating course. For the majority of cases there seems to be a poor outcome. In conclusion, despite the limitations in the research conducted thus far, findings provide important insights regarding COS and several possibilities for future research.
AFRIKAANSE OPSOMMING: Hierdie literere oorsig fokus op navorsing wat die afgelope tien jaar gedoen is oor die kliniese aanbieding van skisofrenie wat in die kinderjare begin (COS). Daar is gebruik gemaak van Internet "soek enjins" en sielkundige databasisse ten einde Engelstalige joernale op te spoor wat vanaf 1994 tot nou oor die onderwerp verskyn het. Navorsing dui daarop dat COS 'n stabiele diagnose is. Oor die algemeen toon dit 'n duidelike geskiedenis van premorbiede abnormaliteite, 'n ongemerkte aanvang en verloop en agteruitgang oor tyd. In die meeste gevalle blyk daar 'n swak uitkoms te wees. Laastens bied die bevindinge belangrike insigte ten opsigte van COS en heelwat moontlikhede vir toekomstige navorsing, ten spyte van die beperkinge in die navorsing wat tot dusver gedoen is.

Texte intégralDepression is a serious condition experienced by many individuals diagnosed with coronary heart disease (CHD). Depression after CHD diagnosis has been associated with poor cardiac prognosis, cardiac mortality, and is postulated to influence adherence to physician recommendations, including attendance at cardiac rehabilitation programs. Cardiac rehabilitation (CR) is an empirically supported secondary intervention for cardiac patients and is recommended by the American Heart Association (AHA) and the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) to reduce disease progression, however many CR participants do not complete. A number of studies have investigated depression and phase II CR programs. The goal of the current systematic literature review and meta-analysis was to explore the association between depression and phase II CR completion. A literature search cross-referenced three electronic databases (PsycINFO, MEDLINE, Dissertation Abstracts International) up through December 2014. Studies quantifying an association between depression and phase II CR completion were reviewed. After duplicate studies were removed and study inclusion criteria applied, 17 observational studies with 19 independent samples consisting of 30,586 cardiac patients remained for meta-analysis. A random-effects model found a moderate inverse relationship between depression and phase II CR completion (g = -.44, 95% CI -.59 to -.29), indicating that depressed CR patients were significantly less likely to complete their program. A minor amount of publication bias was detected with a funnel plot and trim-and-fill analysis. No significant moderator variables were detected.

Texte intégralThis critical analysis of the literature is a comprehensive collection and review of the literature on the strengths-based perspective as it is applied to the treatment of substance use disorders. Literature was collected, analyzed, and critically evaluated to consolidate the existing research on strengths-based treatment approaches, identify ways in which the strengths-based perspective is congruent with culturally competent practice, and to identify strengths, weaknesses, and gaps in the literature. Analyses reveal that there are numerous components of the strengths-based perspective that are congruent with culturally competent practice and that strengths-based treatment approaches may serve as an effective adjunct or alternative to traditional treatment approaches for substance use disorders. While additional research is needed to further our understanding of the effectiveness of this approach, preliminary results indicate that there are numerous therapeutic advantages associated with the strengths-based perspective and its use with substance abusing populations.

Texte intégralThroughout the United States are institutions abundant with violent offenders who have been found not guilty by reason of insanity (NGI). The decision to release these insanity acquittees into the community is a vital one, both for the patient and the larger community. While these decisions should be informed by evaluations that combine clinicians’ opinions with validated tools of assessment, no standard of care regarding such evaluations exists. Forensic specialists are thus often left to base discharge decisions on clinical judgment alone. This dissertation assumed a critical review of the theoretical and empirical literature relevant to conditional release decisions of NGI patients, including the research on structured assessment of risk of future violence. Based on this critical review, the author proposed recommendations for five standards to enhance conditional release decision-making for violent offenders in forensic settings: (a) Adherence to professional and ethical conduct; (b) documentation of patient progress; (c) incorporation of empirically-validated risk assessment tools; (d) creation of a comprehensive release plan; (e) verification of patient’s commitment to successful reintegration. This dissertation additionally examined the strengths and limitations of the critical review strategy, as well as delineated areas for research to empirically evaluate the recommended standards and promote improved quality of conditional release evaluation for NGI acquittees.
