Texte intégralThis dissertation aims at improving our understanding of the links between health and wealth, and between health programs and macroeconomic outcomes. Because the former might be bi-directional, it seemed sensible to tackle this issue for each direction of the causality. In the 1st paper, I examine using microsimulation the financial solvability of alternative policies against HIV. Health improvements at the individual level generate productivity gains which translate into an economic surplus that outweighs programs’ costs. In the 4th paper, I examine the relationship between child health and social background using an international survey. Analysis reveals a substantial gradient in health: across the globe, poorer children have worse health. Yet, the effect of wealth is moderated by country-level income and health-supply variables. The two other papers focus on rather methodological issues raised by the fact that curative programs save lives but increase the prevalence of the disease. One study highlights that average income could fall if treatments cannot guarantee a sufficient level of productivity among sick workers. Despite this adverse effect, the microsimulation model demonstrates that treatment policies can raise per capita income in the context of HIV. The 3rd paper of the thesis extends this message to welfare measurement. By restricting attention to the living population, standard indicators of welfare ignore the fact that individuals who would otherwise be dead can be kept alive through treatment, but with a lower than average welfare. Cross-country comparisons based on indicators that are made invariant to the population size may therefore be biased

Texte intégralThe development of connected objects in the health/wellness sector has the potential to revolutionize private health insurance, just like other sectors of the economy that have been impacted by digitalization. Indeed, connected objects technically offer various opportunities to health insurers: individualizing risks, segmenting them more acutely, profiling, resulting in a predictive logic, and even modulating premiums based on anticipated disbursements relying on information reflecting the behavior or even the health of their users. However, this technical potential cannot be exploited to change the "software" of the insurance business, due to French regulations. Thus, the deployment of connected objects in the private health insurance business is impeded in a market that is too constrained to appeal Gafa.Therefore, private health insurers have sought to enhance the use of connected objects in their relationship with policyholders. In the United States, as part of the so-called “healthism” movement, the accountability of policyholders has led to behavioral prescriptions controlled by connected health/wellness objects whose reliability and security are still imperfect. In France, this logic of quantified self, which can lead to compliance with prescriptions as a condition of coverage, is open to criticism with regard to health determinants and social inequalities in health. It is only very rarely explored by insurers because of a different legal environment from that of the United States, where one of the objectives is to protect individuals against any form of discrimination on the basis of their health status

Texte intégralAbstract : Choir singing is known to be beneficial : this study chose to measure its impacts on elderly people. The quantitative and quasi-experimental design included three groups : the Choir, Hebdo-Bistro (conference workshops), the comparison group, and the Control group. This longitudinal, intergenerational study lasted for three seasons. Cognition (Mattis, 3MS, Trail Making, Numeral WAIS Scale, Formal and Categorical Fluences), humor (Psychological Well-being of American Adults, Geriatric Depression Scale), self-efficacy (Schwartzer), and autonomy (QAF questionnaire) were administered three times (pre, post, 2nd post). In addition, weekly measurements were administered on health related topics (medical consultations, medication, falls) and social participation (activities). The analyses showed no significant differences between groups. However, the intra-group analyses showed a significant improvement for the Choir (3MS, social activities), and a tendency for improvement (Formal Fluence) for the Choir and Hebdo-Bistro groups. The small sample (n =21) excludes any generalization, but the results are promising in today’s aging context.

Texte intégralThe regional Health Agencies (ARS) and the regional Directorates for Youth, Sports and social cohesion (DRJSCS) are invited by their Ministry of Guardianship to implement the plan "Sport, health and well-being", the objectives of which have been adopted by The central state. With the recognition of physical activity as a determinant of health enabled by the accumulation of knowledge, its institutionalization also follows the mobilization of many cause entrepreneurs. The analysis of the conditions of its territorialisation in the new-Aquitaine region from a perspective of instrumentation of public action, gives to see the effects of this plan in many ambivalent points. The use of public and private operators, their remotely-controlled competition by incentive-type instruments, the separation between sponsor and provider, are included in the processes leading to the establishment of a quasi-market of the " Health sport in this region. The forms of Labour Division observed reflect a tangible shift that appears to be gradually confining the health and sports associations to the rank of functional operators. They also accelerate the mechanisms of diversifying the associative sports supply within a field long refractory to this type of evolution. All these dynamics allow, more generally, to participate in the analytical debates on the recomposition of relations between the State and the territories
