Résumé :
Purpose Chemotherapy preparation units face peaks in activity leading to high workloads and increased stress. The present study evaluated the impact of work overloads on the safety and accuracy of manual preparations. Method Simulating overwork, operators were asked to produce increasing numbers of syringes (8, 16, and 24), with markers (phenylephrine or lidocaine), within 1 h, in an isolator, under aseptic conditions. Results were analyzed using qualitative and quantitative criteria. Concentration deviations of < 5%, 5%–10%, 10%–30%, and >30% from the expected concentration were considered as accurate, weakly accurate, inaccurate, and wrong concentrations, respectively. Results Twenty-one pharmacy technicians and pharmacists carried out 63 preparation sessions (n = 1007 syringes). A statistically significant decrease in the manufacturing time for one syringe was observed when workload increased (p < 0.0001). Thirty-nine preparation errors were recorded: 30 wrong concentrations (deviation > 30%), 6 mislabeling, 2 wrong diluents, and 1 wrong drug. There was no statistically significant difference in the mean concentration accuracy of final preparations across the three workloads. The overall error rate increased with the number of preparations made in 1 h: 1.8% for 8 preparations, 2.7% for 16 preparations, and 5.4% for 24 preparations (p < 0.05). Conclusion Although pharmacy technicians and pharmacists were able to increase production speeds with no effect on mean concentration accuracy under stressful conditions, there were greater probability errors being made. These results should encourage actions to spread workloads out over the day to avoid peaks in activity.

Résumé :
AbstractBackgroundAdequate bowel preparation is essential for a high-quality colonoscopy. Many randomized controlled trials (RCTs) have investigated bowel preparation protocols, including split-dose and low-volume regimens. However, RCTs are conducted in an ideal, controlled setting, and translation of trial results to clinical practice is challenging. In this study, we compared the quality of bowel preparations of real-world patients from clinical practice with those enrolled in several prospective trials.MethodsBowel preparation quality, defined by the Ottawa Bowel Preparation Scale (OBPS), from four RCTs and one prospective trial were compared with two observational diary studies. Bowel preparations were polyethylene glycol preparation (PEG) or sodium picosulfate plus magnesium citrate (P/MC) taken via traditional or split-dose timing regimen. Age, sex, average number of bowel movements per day, comorbidities, colonoscopy indication and colonoscopy completion rates were also collected.ResultsPatients enrolled in prospective trials had a better OBPS by one point when compared with those in observational clinical practice studies (P<0.049), after controlling for age, sex, colonoscopy indication and type of bowel preparation used. We also found that each 10-year increase in age was associated with a 0.2 point increase in OBPS (P=0.008), and men were associated with a 0.5 increase in OBPS when compared with women P=0.014).ConclusionPatients from clinical practice have higher OBPS than prospective trial patients. Increased age and male sex were also associated with increased OBPS. We believe increased patient motivation and education around bowel preparation regimen plays an important role in the success of bowel preparations.

Résumé :
Langendorff-perfused hearts and working hearts are established isolated heart preparation techniques that are advantageous for studying cardiac physiology and function, especially when fluorescence imaging is a key component. However, oxygen and energy requirements vary widely between isolated heart preparations. When energy supply and demand are not in harmony, such as when oxygen is not adequately available, the imbalance is reflected in NADH fluctuations. As such, NADH imaging can provide insight into the metabolic state of tissue. Hearts from New Zealand white rabbits were prepared as mechanically silenced Langendorff-perfused hearts, Langendorff-perfused hearts, or biventricular working hearts and subjected to sudden changes in workload, instantaneous global ischemia, and gradual hypoxia while heart rate, aortic pressure, and epicardial NADH fluorescence were monitored. Fast pacing resulted in a dip in NADH upon initiation and a spike in NADH when pacing was terminated in biventricular working hearts only, with the magnitude of the changes greatest at the fastest pacing rate. Working hearts were also most susceptible to changes in oxygen supply; NADH was at half-maximum value when perfusate oxygen was at 67.8 ± 13.7%. Langendorff-perfused and mechanically arrested hearts were the least affected by low oxygen supply, with half-maximum NADH occurring at 42.5 ± 5.0% and 23.7 ± 4.6% perfusate oxygen, respectively. Although the biventricular working heart preparation can provide a useful representation of mechanical in vivo heart function, it is not without limitations. Understanding the limitations of isolated heart preparations is crucial when studying cardiac function in the context of energy supply and demand.

Résumé :
The carbon dioxide sorption process at coal seams is very important for understanding the trapping mechanisms of carbon capture and storage. The gas retention capacity of coal seams can be estimated using indirect methods based on the adsorption/desorption isotherms obtained in the laboratory. However, the gas sorption capacity can be overestimated or underestimated depending on the sample preparation. This work evaluates different sample preparations and their theoretical adsorption capacity using coal samples from the Cambui coal field (Parana Basin), southern Brazil. Experiments using a thermogravimetric balance were done to calculate the theoretical adsorption capacity, while sample characterization was done through immediate analysis, elementary analysis, and mineralogical studies. The sample preparations used in this work were powder, pellets, and fragments. While the powder form presents an average behavior, without any experimental complication, the pellet is extremely sensitive to any variation in the sample preparation, such as fractures, and the fragment requires a much longer experiment time than the other sample preparations, being impracticable for some cases.

Résumé :
The post-retirement preparation of middle & young-old aged workers before their complete labor force withdrawal have received more attention in countries where aging society is progressing rapidly. Given that organizations and society may not be able to provide a full support for the later life of middle & young-old aged workers, self-directed preparation of individual workers is a key factor for the successful retirement. The purpose of this study was to examine a mediating role of mentoring between generativity and post-retirement preparation based on the theory of adult development of Erikson associated with generativity. The effect of mentoring may also be influenced by work-related factor for the old-aged workers. Thus, this study simultaneously tested the effect of work engagement as another predictor of mentoring and post-retirement preparation. Given the expected roles of the generation in this age are quite different by gender, the gender difference in the pattern of relationships among the variables was also hypothesized. Conducting a survey research to 320 employed middle and young-old aged worker, the data was analyzed by multi-group analysis using Structured Equation Modeling. The results of this study found the positive effects of generativity and work engagement on post-retirement preparation through mentoring behavior. Gender difference was also found in some paths of the research model. Suggestions for future research and practical implications were provided based on the findings.

Résumé :
As the scientific marketplace rapidly evolves, we must keep revisiting strategic preparation of our doctoral students and early career scholars to be successful innovators in these contexts. As an inherently integrative, change-oriented, community-engaged, and context-sensitive discipline, social work has enormous potential as a value-added partner for greater cross-fertilization among scientists and stakeholders not only from the academy but also from clinical settings, laboratories, industry, and systems leadership. Yet social work researchers not uncommonly encounter perception challenges as they strive to bring their scientific potential and wares to the marketplace. This article argues that innovation and impact are now central expectations of research. Current research priorities increasingly require teams to function at a transdisciplinary level of theoretical and methodological integration and to develop translational partnerships. The article concludes with thoughts on educational directions to better prepare our T-shaped emerging scholars to be perceived and function as value-added research innovators.

Résumé :
An anonymous online survey was used to understand participants’ perceptions about their preparation for online coursework. Sixty students from five graduate programs in education completed a 1-4 Likert scale survey with 20-questions and six open-ended responses. Quantitative responses from the 20 questions were divided into two subgroups, those younger than or equal to 40 years of age, and those over 40 years of age based on Prensky's date for the beginning of the digital native group. Qualitative data were collected through 6 open-ended questions from the same survey. Data revealed that both subgroups felt well prepared when registering for online courses. However, when it came to overall preparedness for using technology, those over 40 years of age did not view themselves as well prepared as those younger than 40. Using digital resources outside of the learning management system presented more significant challenges to the older subgroup of learners. Two factors impacting graduate students’ online performance became evident through this study; the impact of technology experience rather than age and what is meant for a student to be well-prepared for online course work.
