Laboratory error is defined by ISO 22367 as “Failure of planned actions to be completed as intended or use a wrong plan to achieve an aim”. Lundeberg in 1981 outlined the concept of Total Testing Process (TTP) and Plebani elaborated it further and classified the whole testing process into five phases of Pre-Pre Analytic, Pre Analytic, Analytic, Post Analytic and Post - Post Analytic. The errors have to be identified and resolved in each phase of the process. The medical laboratories have to run Internal and External Quality Control programs and abide by the guidelines of ISO 15189 in order to be accredited by bodies like JCI, CAP or NABL. Active communication and regular interaction between the clinicians and the laboratory is recommended during Pre Analytic and Post Analytic phases of TTP in order to achieve the target of Best Laboratory Practices.

Background: Health laboratory services are an integral part of health services to the community and expected to provide accurate information in each stage: pre-analytic, analytic and post-analytic.Aims: This study aimed to determine the relationship between laboratory experts’ performances in Puskesmas against community perceptions conducted in 24 Puskesmas in Surabaya with a sample of 30 laboratory experts and 30 communities.Methods: This is a multivariate analysis research and data was obtained from the questionnaires given to the respondents and then performed data analysis using Pearson correlation.Results: The results indicated there was a significant relationship between the performance of laboratory experts with the perception of the community (ρ = 0.001). There is a relationship between the pre-analytic stage performance (ρ = 0.002), the analytic stage (ρ = 0.004) and post-analytic (ρ = 0.007).Conclusion: Based on the performance appraisal of laboratory personnel in the Puskesmas including significant pre-analytic, analytic and post-analytic activities on public perceptions there was a significant relationship, and the most significant relationship was found at the pre-analytic stage.

Background. Lack of awareness about the role of laboratory services in many developing countries causes a lack of resources which is the main factor that affects the poor quality of laboratory services. Quality control aims to reduce or minimize errors that occur in the laboratory. The laboratory error ranged from 0.012-0.6%. Errors that occur in the pre-analytic stage are the largest, reaching 60-70% of the total laboratory errors. Pre-analytical error data from one of the Puskesmas laboratories in Bandung is 4.7%. Destination. To find out the description of organizational resources, description of pre-analytical errors and to determine the relationship of organizational resources to pre-analytical errors in the Bandung City Health Center Laboratory. Method. This research is a correlation analytic research. The research subjects were laboratory officers at the Bandung City Health Center Laboratory as many as 20 respondents. The resource variable was measured using a questionnaire and the pre-analytic error variable was derived from secondary data. Results. The results of this study indicate that the organizational resources at the Bandung City Health Center Laboratory are 65% quite good, 30% good and 5% not good. Pre-analytical errors in the Bandung City Health Center Laboratory, as much as 20% did not have pre-analytic errors, as many as 80% had pre-analytical errors. There is no relationship between organizational resources and pre-analytic errors in the Bandung City Health Center Laboratory (Sig. 2-tailed 0.422). Conclusion. Most (65%) of Bandung City Health Center Laboratories have good organizational resources. Pre-analytical errors in the Bandung City Health Center Laboratory varied between 0.10-27.03%. There is no relationship between organizational resource variables and pre-analytic errors in the Bandung City Health Center Laboratory.

A clinical laboratory is a health laboratory that carries out clinical specimen examination services to obtain information about individual health, especially to support efforts to diagnose disease, cure disease, and restore health. The results of laboratory tests are strongly influenced by the pre-analytical, analytical and post-analytic stages. The biggest error contribution in the laboratory, namely in the pre-analytic stage, occurred at 77.1%. One of the health services at the forefront of laboratory services is phlebotomy. Phlebotomy is one of the main reasons behind pre analytic errors. Currently medical laboratory technology experts pay less attention to pre-analytical processes in the laboratory such as the process of taking blood samples (phlebotomy) so it is necessary to socialize and educate about phlebotomy for students of SMK Indo Health School (IHS) majoring in medical laboratory technology which aims to increase knowledge and understanding students regarding the pre-analytic stage, especially in blood sampling (phlebotomy) so as to prevent errors in the post-analytic stage. Solutions that can answer the problems faced by medical laboratory technology experts, it is necessary to carry out socialization and education about phlebotomy, so as to increase the knowledge and understanding of students of SMK Indo Health School (IHS) majoring in medical laboratory technology, totaling 30 participants at the pre-analytic stage, especially in taking blood samples (phlebotomy) as well as the selection of poster education media aims to explain about phlebotomy, demonstrate the stages of phlebotomy and the importance of a good phlebotomy stage in laboratory examination.

Strategic management of wildlands for fire is increasingly a mix of traditional firescience, climatology and human perceptions. Not only must managers be expert atmodeling fuels and fire behavior, they must also understand human behavior, and theeffects of climate on landscapes. We hypothiszed that areas in national forests differspatially in their importance to stakeholders, including both the public and landmanagers. That this difference is based upon the inclusion of factors not typically foundin wildland fire models. To test this hypothesis we used a multidimensional approach toassess the spatial variability several factors including recreation, property values and fuelmoisture. This approach combined a geographic information system with the analytichierarchy process to predict and test the current distribution of areas in national forestsimportant to stakeholders.Inclusion of stakeholders appears to improve the validity and useability of aspatial decision support system. Comparing the model created in this dissertation withseveral others demonstrates that it is important to strike the right balance betweenstakeholders and technical experts when designing and creating a model. It is alwaysbeneficial, however, to a significant level of stakeholder involvement.Areas important for fire mitigation efforts depended on the stakeholder oraudience rating the model. Raters from the U.S. Forest Service tended to favor areas withhigh fire probability scores, while those from the Park Service prefered recreation areasand places people value. In both cases, locations people had easy access to, such as alongroads and trails were favored.These results confirmed the hypothesis that areas of importance are differentbased on the individual rating the model. Further testing and refinement of the modelincludes expanding the study area beyond the southwestern United States as wells asobtaining better sources of data with finer spatial resolutions.

Introduction: It is recommended to place all the vacuum tubes directly on a sample cradle after vein puncture to prevent analytic error. This recommendation is not always easy to follow because the samples are taken by different professionals under different situations.  The three most common analyses, platelets count, haemoglobin and prothrombin time were tested.  Therefore, it was interesting to compare results from the three most common analyses with or without sample cradle, to evaluate the influence of this step on the result. Methods: Three analyses were preformed, using blood from 50 different persons. Each person gave two vacuum tubes, each contained 4.5mL of venous blood for the study. Tubes containing EDTA were used for platelet counts and measurement of haemoglobin and tubes containing citrate were used for prothrombin time-analysis. One of the tubes was placed, as recommended, directly on the sample cradle while the other tube was placed flat on a bench for 10 minutes before it was placed on the sample cradle. Results: There was a clear difference in platelet counts with and without immediate cradling but only minor difference between the results for haemoglobin and International Normalized Ratio. Conclusion: Some analyses seem to be more sensitive for variation in cradling than others. For platelet count it was important to immediately rock the tubes but for determination of prothrombine time and hemoglobin it had a small impact. The small impact on the results is probably due to the efficiency of the anticoagulant in the vacuum tubes.

The automotive manufacturing facility is extremely complex and expensive system. Managing and understanding the dynamics of automotive manufacturing is a challenging endeavour. In the current era of dynamic global competition, a new concept such as Collaborative Lean Manufacturing Management (CLMM) can be implemented as an alternative for organisations to improve their Lean Manufacturing Management (LMM) processes. All members in the CLMM value chain must work together towards common objectives in order to make the LMM achievable in the collaborative environment. The novel research approach emphasises the use of Knowledge-Based (KB) approach in such activities as planning, designing, assessing and providing recommendations of CLMM implementation, through: a) developing the conceptual CLMM model; b) designing the KBCLMM System structure based on the conceptual model; and c) implementing Gauging Absences of Pre-requisites (GAP) analysis and Analytic Hierarchy Process (AHP) approach in the hybrid KBCLMM. The development of KBCLMM Model is the most detailed part in the research process and consists of five major components in two stages. Stage 1 (Planning stage) consists of Organisation Environment, Collaborative Business and Lean Manufacturing components. Stage 2 (Design stage) consists of Organisation CLMM Capability and Organisation CLMM Alignment components. Each of these components consists of sub-components and activities that represent particular issues in the CLMM development. From the conceptual model, all components were transformed into the KBCLMM System structure, which is embedded with the GAP and AHP techniques, and thus, key areas of potential improvement in the LMM are identified for each activity along with the identification of both qualitative and quantitative aspects for CLMM implementation. In order to address the real situation of CLMM operation, the research validation was conducted for an automotive manufacturer's Lean Manufacturing Chain in Malaysia. Published case studies were also used to test several modules for their validity and reliability. This research concludes that the developed KBCLMM System is an appropriate Decision Support System tool to provide the opportunity for academics and industrialists from the fields of industrial engineering, information technology, and operation management to plan, design and implement LMM for a collaborative environment.

The complexity of sustainable building maintenance environment requires managers to define and implement appropriate quality benchmark system suitable for this function. Lean Six Sigma (LSS) is one of the most effective process improvement and optimization philosophy that maintenance organisations can implement in their environment. However, literature review has shown that 90% of failures in LSS implementations are due to lack of readiness to change, the unawareness of the required benchmark organisation capabilities, and improper control of priorities. The contribution of the current research approach is in developing a hybrid Knowledge-Based (KB)/GAP/AHP System, consisting of three stages (Planning, Designing and Implementation) and containing over 2500 KB rules. The KB System can assist the decision-makers in identifying the obstacles behind the organisation readiness to change into a benchmark LSS maintenance environment. Thus the KB System will be used to achieve benchmark standards by determining the gap existing between the current environment and the benchmark goal, and then suggest a detailed plan to overcome these hurdles in a prioritised and structured manner, thus achieving cost benefits. To ensure its consistency and reliability, the KB System was validated in three Oman-based maintenance organisations, and one published case study for a UK-based organisation. The results from the validation were positive with the System output suggesting list of top priorities and action plans for achieving benchmark LSS standards for these organisations. The research concludes that the developed KB System is a consistent and reliable methodology for assisting decision-makers in designing, planning, and implementing LSS for benchmark sustainable building maintenance.

The product development process for the automotive industry is normally complicated, lengthy, expensive, and risky. Hence, a study on a new concept for Low Volume Automotive Manufacturing (LVAM), used for niche car models manufacturing, is proposed to overcome this issue. The development of a hybrid Knowledge Based (KB) System, which is a blend of KB System, Gauging Absences of Pre-requisites (GAP), and Analytic Hierarchy Process (AHP) is proposed for LVAM research. The hybrid KB/GAP/AHP System identifies all potential elements of LVAM issues throughout the development of this system. The KB System used in the LVAM analyses the gap between the existing and the benchmark organisations for an effective implementation. The novelty and differences in the current research approach emphasises the use of Knowledge Based (KB) System in the planning and designing stages by suggesting recommendations of LVAM implementation, through: a) developing the conceptual LVAM model; b) designing the KBLVAM System structure based on the conceptual LVAM model; and c) embedding Gauging Absences of Pre-requisites (GAP) analysis and Analytic Hierarchy Process (AHP) approach in the hybrid KBLVAM System. The KBLVAM Model explores five major perspectives in two stages. Planning Stage (Stage 1) consists of Manufacturer Environment Perspective (Level 0), LVAM Manufacturer Business Perspective (Level 1), and LVAM Manufacturer Resource Perspective (Level 2). Design Stage (Stage 2) consists of LVAM Manufacturer Capability - Car Body Part Manufacturing Perspective (Level 3), LVAM Manufacturer Capability - Competitive Priorities Perspective (Level 4), and LVAM Manufacturer Capability - Lean Process Optimisation Perspective (Level 5). Each of these perspectives consists of modules and sub-modules that represent specific subjects in the LVAM development. Based on the conceptual LVAM model, all perspectives were transformed into the KBLVAM System structure, which is embedded with the GAP and AHP techniques, hence, key areas of potential improvement are recommended for each activity for LVAM implementation. In order to be able to address the real situation of LVAM environment, the research verification was conducted for two automotive manufacturers in Malaysia. Some published case studies were also used to check several modules for their validity and reliability. This research concludes that the developed KBLVAM System provides valuable decision making information and knowledge to assist LVAM practitioners to plan, design and implement LVAM in terms of business organisation, manufacturing aspects and practices.

The dependency of maintenance as a manufacturing logistic function has made the considerations of maintenance decisions complex in nature. The importance of maintenance has escalated significantly by the increasing of automation in manufacturing processes. This condition switches the traditional maintenance perspective of “fire-fighter” into the business competitive driver. As a consequence, maintenance needs to consider other related aspects of decision making to achieve competitive advantage. This research aims to develop a hybrid Knowledge-Based (KB) System/GAP/AHP methodology to support the integration of maintenance decision with business and manufacturing perspectives. It constructs over 2000 KB rules on Strategic Stage (business and manufacturing aspects) and Maintenance Operations Stage (maintenance aspects). Each aspect contains KB rules attached with GAP analysis to assess the gap between current and prerequisite condition. AHP analysis is then deployed to compare those aspects structurally in a pair-wise manner to identify the critical ones to be rectified. This hybrid KB system is useful in reviewing the existing maintenance system performance and provides reasonable recommendations to improve maintenance performance with respect to business and manufacturing perspectives. Eventually, it indicates the roadmap from the current state to the benchmark goals for the maintenance system. This novel methodology of KBS/GAP/AHP to support maintenance decision is developed for a particular application in the automotive environment. The validation is conducted in two automotive companies in Indonesia and one published case study in an automotive company. The result confirms that the developed KB system can provide the valid, reasonable and consistent result to propose structured recommendation for maintenance improvement priority.

Two fundamental concepts underlie the clinical use of biomarkers: First, biomarkers should always be used in conjunction with all other clinical information. Second, in order to maximize their diagnostic and prognostic use, biomarkers should be interpreted as quantitative variables. For example, a cardiac troponin level which is 50 times the upper limit of normal has a much higher positive predictive value for the presence of an acute myocardial infarction, compared to a level just above the upper limit of normal.

Understanding the proper use of biomarkers requires clinicians to appreciate some critical pre-analytic and analytic issues, as well as how to use the markers properly. The bene?ts of such an approach will not only facilitate the care of patients today, but will also prepare clinicians to understand and embrace the new generation of markers that is coming and that will continue to make this area transformational for cardiology. Two fundamental concepts underlie the clinical use of biomarkers: First, biomarkers should always be used in conjunction with all other clinical information. Second, in order to maximize their diagnostic and prognostic use, biomarkers should be interpreted as quantitative variables. For example, a cardiac troponin level which is 50 times the upper limit of normal has a much higher positive predictive value for the presence of an acute myocardial infarction, compared to a level just above the upper limit of normal.

We employ words like “cause” both to structure an investigative architecture and to register concrete physical data in light of that strategic background. As a result, “cause”’s referential significance becomes very complicated as the word progressively enters fresh patches of application. Jim Woodward’s studies have demonstrated the central role that considerations of manipulative control play in mapping out the contours of these strategic specializations. In contrast, analytic metaphysicians have attempted to carve out an a priori pre-science of causal necessity that falters through ignoring these developmental considerations. All in all, this essay presents a critique of necessitarian thinking not unlike that offered by Quine in his well-known “Two Dogmas of Empiricism.”

Résumé :
An approximate one-dimensional analytic model is presented for the propagation of a laminar pre-mixed flame, and the diffusive burning of a condensed phase. Zero-order Arrhenius kinetics is used to describe the burning rate. Extinction of the pre-mixed and the diffusion flame systems is graphically illustrated to show the critical condition in terms of heat loss and the net generation of energy. The analysis is meant to be tutorial, and to serve as an indication of the similarity of extinction in the two systems. An example, using n-heptane as the fuel is given.

Abstract Growing renewable energy generation share causes more irregular and more flexible operational regimes of conventional power plants than in the past. It leads to long periods without dispatch for several days or even weeks. As a consequence, the required pre-heating of the steam turbine leads to an extended power plant start-up time [1]. The current steam turbine Hot Standby Mode (HSM) contributes to a more flexible steam turbine operation and is a part of the Flex-Power Services™ portfolio [2]. HSM prevents the turbine components from cooling via heat supply using an electrical Trace Heating System (THS) after shutdowns [3]. The aim of the HSM is to enable faster start-up time after moderate standstills. HSM functionality can be extended to include the pre-heating option after longer standstills. This paper investigates pre-heating of the steam turbine with an electrical THS. At the beginning, it covers general aspects of flexible fossil power plant operation and point out the advantages of HSM. Afterwards the technology of the trace heating system and its application on steam turbines will be explained. In the next step the transient pre-heating process is analyzed and optimized using FEA, CFD and analytic calculations including validation considerations. Therefor a heat transfer correlation for flexible transient operation of the HSM was developed. A typical large steam turbine with an output of up to 300MW was investigated. Finally the results are summarized and an outlook is given. The results of heat transfer and conduction between and within turbine components are used to enable fast start-ups after long standstills or even outages with the benefit of minimal energy consumption. The solution is available for new apparatus as well as for the modernization of existing installations.
