Critically ill COVID 19 patients requiring prolonged mechanical ventilation, receive analgesics and sedatives to prevent agitation which may cause patient ventilator asynchrony. Anticipating the physiological response to painful stimuli, such as endotracheal tube suctioning or repositioning, is difficult and titrating analgesics and sedatives may be improved by continuous non-invasive monitoring of levels of analgesia. We describe our initial experience using the PMD-200 monitor with the NOL™ index algorithm (Medasense Biometrics, Ramat Gan Israel) to provide continuous, non-invasive monitoring of analgesia in COVID 19 ventilated patients. Publication of deidentified patient data was determined EC exempt.

Tekst przedstawia karierę szachisty Henryka Kahane. Henryk (Hersz Tsalel) Kahane urodził się w Tomaszowie Rawskim (Mazowieckim) w 1906 roku i uczył się grać w szachy w swoim rodzinnym mieście. Później studiował chemię na Politechnice w Brnie w latach 1927-1931. Reprezentując żydowski klub Makkabi Brno, Kahane został mistrzem Głównego Związku Czechosłowackich Szachistów (ÚJČŠ) w roku 1929 na turnieju rozgrywanym w Brnie, a w roku 1931 został szachowym mistrzem Brna. Po powrocie do Polski Kahane był członkiem Łódzkiego Towarzystwa Zwolenników Gry Szachowej. Brał udział w Szachowych Mistrzostwach Łodzi (4 miejsce w 1932 roku, 3 miejsce w 1934), był także członkiem zespołu Łodzi na Mistrzostwa Polski w szachach w 1934 r. W 1935 r. wyemigrował do Rumunii, później (1940), poprzez Bułgarię i Turcję do Palestyny. W Izraelu Henryk Kahane mieszkał w mieście Ramat Gan (Dystrykt Tel Awiw), gdzie założył fabrykę chemiczną, której był dyrektorem. Nie wiadomo, czy w Izraelu grał w szachy i nieznana jest data jego śmierci. Henryk Kahane – a forgotten chess master The article presents the career of a chess player, Henryk Kahane. Henryk (Hersz Tsalel) Kahane was born in Tomaszów Rawski (Tomaszów Mazowiecki) in 1906 and learned to play chess in his home town. Later, in the years 1927–1931, he studied chemistry at the Brno Polytechnic. Representing the Jewish club of Maccabi Brno Kahane became a chess master of ÚJČŠ (i.e. the Main Society of Czecho-Slovakian Chess Players) in 19129 in Brno. In 1931 he was a chess master of Brno. After he had returned to Poland, Kahane became a member of the Chess Enthusiasts Association in Łódź. He took part in the Łódź Chess Competition (4th place in 1932, 3rd place in 1934). He was also a member of the Łódź team in the Polish Chess Competition in 1934. In 1935, he emigrated to Romania, and later, in 1940, to Palestine (through Bulgaria and Turkey). In Israel, Kahane lived in Ramat Gan (Tel Aviv District) where he set up a chemical factory. He was the director of the company. Whether he played chess in Israel remains unknown. It is also unknown when he died.

An apparently healthy 2½-year-old male Grant’s Zebra weighing approximately 200kg located at Nandankanan Zoological Park, Odisha, India, procured from Zoological Centre, Tel Aviv- Ramat Gan, Israel during September 2015 was noticed in a sitting position making frequent attempts to get up. The zebra was immobilised the same day with a combination of 1.96mg etorphine hydrochloride, 8.0mg of acepromazine and 40.0mg of xylazine hydrochloride to facilitate diagnosis and treatment. Clinical examinations did not reveal any signs suggestive of disease or disorder of the musculoskeletal system. Microscopic examination of blood smears stained in Giemsa’s stain revealed the presence of intra-erythrocytic inclusions, either single or pairs, suggestive of haemoprotozoans, i.e., B. caballi and/or T. equi. The zebra was administered with two divided doses of imidocarb injection @4.0mg/kg b.wt. deep intramuscularly in the neck region with supportive therapy. Progressive improvement in posture, gait, and appetite were noticed following 24h of medication. Three more doses of imidocarb were administered at 72h intervals, each time after immobilisation.

The two dictionaries under review represent the product of decades of assiduous research and persistent effort on the part of Professor Michael Sokoloff of Bar Ilan University. Previoiusly, he has contributed major works in the Aramaic field in collaboration with other scholars. There is, first of all, A Corpus of Christian Palestinian Aramaic (Gröningen: Styx Publications, 1997), a multivolume edition of texts prepared in collaboration with Christa Müller-Kessler. This was followed by a Hebrew work, [Jewish Palestinian Aramaic Poetry from Late Antiquity] (Jerusalem: Israel Academy of the Sciences and Humanities, 1999), prepared in collaboration with Joseph Yahalom. However, the dictionaries reviewed here, which represent his most ambitious projects, bear his name alone, with only technical and electronic assistance in their actual preparation provided on the part of others. Sokoloff has also published A Dictionary of Judean Aramaic (Ramat Gan: Bar Ilan University, 2003), covering sources from 150 BCE to 200 CE, which includes the rich material preserved in the Aramaic papyri from the Judean Desert.

Rabbi Tovia Ben-Chorin was born on 15 September 1936 in Jerusalem, the son of journalist and religious scholar Schalom Ben-Chorin (formerly Fritz Rosenthal) and artist Gabriella Rosenthal, the couple having moved from Germany to Palestine in 1935. He graduated with a BA in Bible and Jewish studies from the Hebrew University, Jerusalem and was ordained as a rabbi by Hebrew Union College-Jewish Institute of Religion in Cincinnati in 1964. He served initially as a rabbi in Israel in Ramat Gan, then in Manchester Reform Synagogue, Jackson's Row (1977–81), and from 1981 to 1996 at the Har El congregation in Jerusalem, which he had helped to establish with his father and stepmother, and which became the ‘mother community’ of today's Israel Movement for Progressive Judaism (IMPJ). During this period, he established the Israeli Progressive Youth Movement and guided the ‘garin’ which established the second Reform kibbutz in the Arava, Kibbutz Lotan. He has written that ‘wanderlust’ led him to become the rabbi of Or Chadash congregation in Zurich (1997–2007) and subsequently Berlin's Pestalozzi Strasse Liberal synagogue from 2009 to 2015, and to a small congregation in St Gallen in Switzerland where he served until his death.

Abstract: Introduction: CO2 laser- assisted sclerectomy surgery (CLASS) can be used for the surgical treatment of open-angle glaucoma. Aim: To introduce our results with CLASS. Method: We performed 21 CLASS operations using OT-134-IOPtiMate (IOPtima Ltd, Ramat-Gan, Israel). Patients were examined on the 1st day, and in the 1st, 3rd, 6th, 9th and 12th months postoperatively. We evaluated intraocular pressure (IOP), antiglaucomatous medication-use, visual acuity, complications. Results: Mean age was 65.6 yrs. Complete success (no hypotensive medication required to target IOP) was achieved in 61.1% (18 patients) at 6 months, whereas in 50% (10 patients) at 12 months. Qualified success (hypotensive medication required to target IOP) was achieved in 72.2% and in 70%, preoperative mean IOP was 29.2 ± 9.4 Hgmm, which falled to 17.7 ± 4.9 Hgmm and 17.3 ± 4.3 Hgmm, respectively. Antiglaucomatous medication use falled significantly from 2.90 ± 0.83 to 2.05 ± 1.46. Apart from 1 macroperforation, no serious complication occurred. Objective To compare postural balance ability in patients with low back pain between groups with and without lumbosacral radiculopathy.Methods Patients who were referred for electromyography because of low back pain during the period from April 2017 through June 2018 were chosen as subjects. They were divided into groups with and without lumbosacral radiculopathy based on the results of electromyography. We used Tetrax (Sunlight Medical Ltd., Ramat Gan, Israel) to objectively evaluate postural balance ability, and to measure the fall risk, stability index, weight distribution index, and Fourier index.Results Patients in the lumbosacral radiculopathy group showed significantly higher fall risk (73.25 vs. 38.00; p<0.05), weight distribution index (8.57 vs. 5.00; p<0.05), and stability index (21.19 vs. 13.16; p<0.05) than those in the group without lumbosacral radiculopathy. The Fourier index at high-medium frequency was significantly increased in the lumbosacral radiculopathy group (8.27 vs. 5.56; p<0.05), whereas weight-bearing on the side of radiculopathy was significantly decreased.Conclusion Patients with lumbosacral radiculopathy have decreased postural balance compared with patients without this condition. Somatosensory disturbances in lumbosacral radiculopathy might cause postural balance impairment. Assessment and treatment plan not only for pain reduction but also for postural balance improvement should be considered in the management of patients with lumbosacral radiculopathy.

This article traces transnational “life” trajectories of two rare Jewish religious music records from 1948 Communist Czechoslovakia and of their main performer Josef Weiss (ca. 1912, Veľké Kapušany – 1985 Netanya), who was a hazzan (cantor) in synagogues in Vienna, Budapest, Prague, Jerusalem, Ramat Gan, Manchester, and New York, but has remained mostly unknown to music history. It shows how these two 78-rpm records stand at the core of Weiss’s grandson’s family / music / memory project, which has revealed and pre­pared to reissue 52 audio recordings to preserve his grandfather’s legacy. While following these and other digitized and technologically modified recordings of Weiss on their recent path between the Czech Republic, Israel, Hungary, and the US, the article sheds light on how this case fits into the broader framework of the social life of things and the context of musical remembrance. Already put to use during the life-cycle rituals of Weiss’s children and grandchildren, as well as in a museum exhibition – this family project constructs a fragment of a Jewish sonic past for the present needs of its actors, while being entangled with the current practice of Jewish memory institutions, as well as with the activities of private record collectors and of one ethnomusicologist (myself).

IntroductionBreast cancer survivors treated with adjuvant endocrine therapy commonly experience weight gain, which has been associated with low adherence to therapy and worse breast cancer prognosis. We aim to assess whether a personalised postprandial glucose targeting diet will be beneficial for weight management as compared with the recommended Mediterranean diet in this patient populationMethods and analysisThe BREAst Cancer Personalised NuTrition study is a phase-2 randomised trial in hormone receptor positive patients with breast cancer, treated with adjuvant endocrine therapy. The study objective is to assess whether dietary intervention intended to improve postprandial glycaemic response to meals results in better weight and glycaemic control in this population as compared with the standard recommended Mediterranean diet. Consenting participants will be assigned in a single blinded fashion to either of two dietary arms (Mediterranean diet or an algorithm-based personalised diet). They will be asked to provide a stool sample for microbiome analysis and will undergo continuous glucose monitoring for 2 weeks, at the initiation and termination of the intervention period. Microbiome composition data will be used to tailor personal dietary recommendations. After randomisation and provision of dietary recommendations, participants will be asked to continuously log their diet and lifestyle activities on a designated smartphone application during the 6-month intervention period, during which they will be monthly monitored by a certified dietitian. Participants’ clinical records will be followed twice yearly for 5 years for treatment adherence, disease-free survival and recurrence.Ethics and disseminationThe study has been approved by the ethics committee in the Sheba medical centre (file 5725-18-SMC, Ramat Gan, Israel) and the Weizmann Institutional Review Board (file 693-2, Rehovot, Israel). The findings of this study will be published in a peer reviewed publication.Trial registration numberNCT04079270.

BackgroundAlprostadil is used to maintain ductus patency in neonates with ductus dependent congenital heart disease (CHD), until corrective surgery. Fever is a common side effect of alprostadil and may also be a sign of infectious disease. We aimed to identify potential parameters that may differentiate infectious from alprostadil fever.MethodsRetrospective case-control study included all neonates with ductus dependent CHD, who were admitted at the Children´s Hospital in Ramat-Gan, Israel, from August 2003 to August 2017 and developed fever on alprostadil. Cases were defined as neonates with a positive bacterial culture and controls were defined as neonates with a negative culture. Multivariate cox-regression was conducted to identify potential parameters that may differentiate alprostadil from infectious fever.ResultsThree hundred and four neonates developed fever under alprostadil. Fifty five (18%) had a positive bacterial culture and 249 (82%) had a negative culture. In univariate analysis, the duration of alprostadil infusion was 95 hours (IQR 45–116) in the case group and 72 hours (IQR 49–215) in the control group (p=0.011). The time between alprostadil initiation and fever was longer for the case group: 14.13 hours (IQR 6.5–47.5) versus 12.96 (IQR 5–30.51), (p=0.039). In multivariate cox-regression, a more than 10% increase in neutrophil count before fever was significantly associated with an increased risk for infection (HR 6.14, 95% CI 1.94–19.42). A trend towards an association was observed with CRP ≥ 10 mg/dl before fever (HR 2.5, 95% CI 0.66–9.47) and in an increase ≥ 1000 micromol/L in WBC before fever (HR 1.74, 95% CI 0.58–5.21).ConclusionsIn neonates with CHD on alprostadil therapy, an increase in neutrophil count before the appearance of fever is associated with infection. Full sepsis work-up and is still warranted in neonates who develop fever under alprostadil. Abstract Editor’s Perspective What We Already Know about This Topic The nociception level index (Medasense Biometrics Ltd., Ramat Gan, Israel), is a reliable measure of moderate to intense noxious stimulation during anesthesia and surgery What This Article Tells Us That Is New In a randomized trial in patients having major abdominal surgery, compared to standard practice, nociception level-guided analgesia resulted in 30% less intraoperative remifentanil consumption Background The multidimensional index of nociception, the nociception level, outperforms blood pressure and heart rate in detection of nociceptive events during anesthesia. We hypothesized that nociception level–guided analgesia reduces opioid consumption and suboptimal anesthesia events such as low blood pressure and use of vasoactive medication. Methods In this single-blinded randomized study, 80 American Society of Anesthesiologists class I–III adult patients of either sex, scheduled for major abdominal procedures under remifentanil/propofol anesthesia by target-controlled infusion, were included. During the procedure nociception level, noninvasive blood pressure, and heart rate were monitored. Patients were randomized to receive standard clinical care or nociception level–guided analgesia. In the nociception level–guided group, remifentanil concentration was reduced when index values were less than 10 or increased when values were above 25 for at least 1 min, in steps of 0.5 to 1.0 ng/ml. Propofol was titrated to bispectral index values between 45 and 55. The primary outcomes of the study were remifentanil and propofol consumption and inadequate anesthesia events. Results Compared with standard care, remifentanil administration was reduced in nociception level–guided patients from (mean ± SD) 0.119 ± 0.033 to 0.086 ± 0.032 μg · kg-1 · min-1 (mean difference, 0.039 μg · kg-1 · min-1; 95% CI, 0.025–0.052 μg · kg-1 · min-1; P &lt; 0.001). Among nociception level–guided patients, 2 of 40 (5%) experienced a hypotensive event (mean arterial pressure values less than 55 mm Hg) versus 11 of 40 (28%) patients in the control group (relative risk, 0.271; 95% CI, 0.08–0.77; P = 0.006). In the nociception level–guided group, 16 of 40 (40%) patients received vasoactive medication versus 25 of 40 (63%) patients in the standard care group (relative risk, 0.64; 95% CI, 0.40–0.99; P = 0.044). Conclusions Nociception level-guided analgesia during major abdominal surgery resulted in 30% less remifentanil consumption.

Ternary NiFeTiOOH Catalyst for the Oxygen Evolution Reaction: Study of the Effect of the Addition of Ti at Different Loadings Wenjamin Moschkowitsch and Lior Elbaz Chemistry Department, Bar-Ilan University, Ramat-Gan 5290002, Israel The demand for energy is expected to grow rapidly in the next decades, but it cannot be solely fulfilled with fossil fuel-based technologies without having a huge impact on the environment. The shift to production of clean energy from alternative sources, such as wind and sun, raise the importance of energy storage technologies. One of the most prominent solutions is storing surplus energy, harvested at peak production times and seasons, in hydrogen. However, the production of hydrogen with methods that require as little energy as possible, as well as being sustainable, environmentally friendly and cheap, are still considered to be a big challenge. Water electrolysis is the simplest industrial process for hydrogen production, and can be linked to fuel cells technology. Among the available electrolyzers, alkaline electrolyzers (ALE) are considered state-of-the-art. Although they can work with platinum-group metal-free (PGM-free) catalysts, unfortunately, this technology still requires the use of PGM catalysts in order to increase the current density, and lower the reaction activation energy. In electrolyzers, water splits into oxygen and hydrogen in two separate reactions, taking place at the anode and cathode. The cathodic reaction is the Hydrogen Evolution Reaction (HER), which is considered to be relatively facile. The anodic, Oxygen Evolution Reaction (OER), is considered to be much more difficult, since it is a four-electron process with very sluggish kinetics. The best known catalysts for this reaction in acidic medium are IrO2 and RuO2, oxides of very rare and precious metals (Ir is the scarcest metal on earth’s crust). In addition, in acidic medium, most PGM-free catalysts, based on earth abundant elements, are considered unstable (these conditions have also shown to be detrimental for Ir and Ru-based catalysts). In contrast, in ALEs, PGM-free catalysts have shown to be a good alternative to PGM catalysts. The most common OER PGM-free catalysts are first-row transition metals in their oxide, hydroxide and oxyhydroxide forms.One such catalyst is nickel oxyhydroxide (NiOOH). The structure of this specific catalyst has been studied in great detail by many different research groups, yet there are several open questions regarding the OER mechanism, i.e. the exact catalytic center and active phase.Recent studies suggest that pure NiOOH is not very active at all, and that all of the activity can be attributed to iron impurities.Indeed, NiFeOOH with iron content of 15-25 %at, has a much higher activity and a much lower overpotential compared to other PGM-free catalysts. It can thus be regarded as a benchmark for this class of OER catalysts.It is well accepted by now that the bimetallic catalyst further increases the intrinsic catalytic activity, and that addition of other transition metals,can further increase it.

Settler innocence is the primary organising principle of Israel's space production. To construct themselves as innocent political actors, oppressors disavow, justify, and deny their responsibility for oppression. The rhetoric of settler innocence allows Israel, a settler colonial state responsible for the dispossession and military occupation of Palestine, to present itself as the "only democracy in the Middle East". In this article, I perform a visual analysis of the Zoological Center Tel Aviv-Ramat Gan and the Jerusalem Biblical Zoo to examine how zoos, inherently violent colonial institutions, reproduce settler innocence. Entrenched in Israel's national projects of linking modernity and Biblical antiquity, zoos naturalise settlers, normalise indigenous displacement, and obscure colonial violence.

Zusammenfassung Hintergrund Die Gabe von Opioiden zur Schmerzunterdrückung spielt eine zentrale Rolle in der modernen Anästhesiologie. Messungen von Hypnosetiefe und Muskelrelaxierung sind im Gegensatz zur Schmerzmessung seit Jahren etabliert. Seit Kurzem ist das PMD200 („Pain Monitoring System“; Fa. Medasense Biometrics™ Ltd., Ramat-Gan, Israel) verfügbar. Dieser Schmerzmonitor misst nichtinvasiv und errechnet einen dimensionslosen Schmerzindex („nociceptor level“, NoL). Die Validität und Zuverlässigkeit des Verfahrens sind Gegenstand von klinischen Studien. Fragestellung Reduziert die Verwendung des PMD200 die Gabe von Analgetika während einer Da-Vinci-Prostatektomie? Material und Methoden In die Studie wurden 50 Patienten aufgenommen. Nach gewichtsadaptierter Sufentanilgabe zur Narkoseinduktion und einem 10 µg Bolus vor Hautschnitt erfolgte die intraoperative Analgesie durch subjektive Entscheidung (CONT) oder aufgrund eines erhöhten NoL-Index (INT). Die statistische Auswertung erfolgte durch Mann-Whitney-U-, Kolmogorow-Smirnow-Test und Levene-Statistik. Ergebnisse In der INT-Gruppe war die Anzahl der Sufentanilboli/h nicht signifikant geringer als in der CONT-Gruppe (p = 0,065). Die Varianz der Sufentanilgaben unterschied sich signifikant (p = 0,033). In der CONT-Gruppe war die Applikation normal verteilt (p = 0,2), in der INT-Gruppe hingegen nicht (p = 0,003). Diskussion Eine mögliche Interpretation der Daten ist, dass die Schmerzmittelgabe in der INT-Gruppe individualisierter erfolgte, d. h., es wurden nichterforderliche Schmerzmittelgaben vermieden, und gleichzeitig detektierte das NoL-Monitoring einzelne Patienten mit deutlich erhöhtem Schmerzmittelbedarf. Diese Schlussfolgerung ist nur unter der Voraussetzung zulässig, dass das PMD200 auch tatsächlich die Entität Schmerz misst.

The transcription factor GATA2 plays a key role in the survival and self-renewal of hematopoietic stem and progenitor cells. Autosomal dominant variants in GATA2 cause a broad spectrum of heterogeneous phenotypes. Here, we present our experience with GATA2 deficiency in a retrospective multicenter analysis of computerized medical records of adult patients (age ≥18 years) treated between 2018 and 2022 at Shaare Zedek Medical Center in Jerusalem and Sheba Tel-Hashomer Medical Center in Ramat Gan, Israel. Two male and two female patients with GATA2 deficiency were identified. Three of the patients presented with symptoms in adult life and all patients were diagnosed as adults. Age at presentation was 10.5-36 years and age at diagnosis 24-47 years. Diagnosis was delayed in all patients by 1-24.5 years. The phenotypic diversity was notable. Patients presented with myelodysplastic syndrome (n=2), pulmonary alveolar proteinosis (n=1), and recurrent viral (n=1), bacterial (n=3), and mycobacterial (n=1) infections. Bone marrow biopsy revealed cytogenetic abnormalities in one patient (monosomy 7). Patients were diagnosed by exome sequencing (n=3) and Sanger sequencing of the coding exons in GATA2 (n=1). Novel heterozygous GATA2 variants (c.177C&gt;A, p.Y59* and c.610dup, p.R204Pfs*78) were identified in two patients. Immune workup revealed B cell lymphopenia and monocytopenia in all tested patients. One patient died from overwhelming sepsis despite all patients being treated with antibiotics and anti-mycobacterials. Our cohort highlights the phenotypic diversity, late presentation, and delayed diagnosis of GATA2 deficiency. Increased awareness of this primary immune deficiency presenting in adult life is needed and should involve a high index of suspicion.

Abstract Background Cardiac electroporation is a promising novel non-thermal ablation method, gaining significant interest with recent first-in-man data suggesting effective cardiac lesion generation with no collateral damage. Nevertheless, significant knowledge gaps exist regarding its electrophysiological consequences in cardiomyocytes, including; cell specificity, protocol optimization, irreversibility threshold, recovery time-constants, and the mechanistic nature of its cytolytic and anti-arrhythmic properties. Purpose Establishing an innovative in-vitro model for the study of cardiac electroporation-ablation, utilizing human induced pluripotent stem cells (hiPSCs). Methods and results Healthy-control hiPSC-derived cardiomyocytes were enzymatically dissociated and seeded as circular cell sheets (hiPSC-CCSs). Electroporation-ablation experiments were performed using a custom designed high-frequency electroporation (HF-EP) generator. Two needle-shaped electrodes were used for HF-EP delivery (Figure 1). Subsequently, detailed voltage- and Ca2+-mapping studies of the hiPSC-CCSs were conducted (Figure 2). HF-EP application resulted in the generation of electrically isolated lesions within the hiPSC-CCSs (Figure 3). Further characterization of the temporal changes and electrophysiological properties following electroporation revealed that; (1) lesions persisted over prolonged periods of time (days), indicating irreversible electroporation, (2) a temporal decrease in lesion dimensions was noted, consistent with a significant reversible electroporation component (Figures 3–5), (3) most tissue recovery had occurred within the first 15 minutes following electroporation, with little recovery beyond that time-frame, (4) increasing pulse-number augmented lesion area as well as the proportion of irreversible damage, and (5) electroporation sensitization was achieved by increasing extracellular Ca2+, indicating its crucial role in electroporation cytolysis, potentially via direct cellular toxicity and apoptosis facilitation (Figures 5–6). Finally, evaluating for HF-EP anti-arrhythmic properties, we targeted multiple rotors or focal triggered-activity generated in the hiPSC-CCSs. HF-EP application generated sustained line-blocks, isolating arrhythmogenic substrates within the hiPSC-CCSs while blocking the propagation of arrhythmic wavefronts (Figure 7). Conclusion Our results demonstrate the ability to study cardiac electroporation utilizing hiPSC-derived cardiomyocytes, provide novel insights into its temporal and electrophysiological characteristics, facilitate electroporation protocol optimization, screen for potential electroporation sensitizers, and to study its mechanistic nature and anti-arrhythmic properties.
