Résumé :
The aim of this study was to compare the time course of recovery following four different resistance exercise protocols in terms of loading magnitude (60% vs. 80% 1RM—one-repetition maximum) and velocity loss in the set (20% vs. 40%). Seventeen males performed four different protocols in full squat exercise, which were as follows: (1) 60% 1RM with a velocity loss of 20% (60-20), (2) 60% 1RM with a velocity loss of 40% (60-40), (3) 80% 1RM with a velocity loss of 20% (80-20), and (4) 80% 1RM with a velocity loss of 40% (80-40). Movement velocity against the load that elicited a 1 m·s−1 velocity at baseline measurements (V1-load), countermovement jump (CMJ) height, and sprint time at 20 m (T20) were assessed at Pre, Post, 6 h-Post, 24 h-Post, and 48 h-Post. Impairments in V1-load were significantly higher for 60-40 than other protocols at Post (p < 0.05). The 60-20 and 80-40 protocols exhibited significant performance impairments for V1-load at 6 h-Post and 24 h-Post, respectively (p < 0.05). CMJ height remained decreased for 60-20 and 60-40 until 24 h-Post (p < 0.001–0.05). Regarding T20, the 80-40 protocol resulted in higher performance than 60-40 at 24 h-Post and the 80-20 protocol induced a greater performance than 60-40 protocol at 48 h-Post (p < 0.05). A higher velocity loss during the set (40%) and a lower relative load (60% 1RM) resulted in greater fatigue and slower rate of recovery than lower velocity loss (20%) and higher relative load (80% 1RM).

Résumé :
The role of Kupffer cells (KCs) in liver regeneration is complicated and controversial. To investigate the distinct role of F4/80+ KCs at the different stages of the regeneration process, two-thirds partial hepatectomy (PHx) was performed in mice to induce physiological liver regeneration. In pre- or post-PHx, the clearance of KCs by intraperitoneal injection of the anti-F4/80 antibody (α-F4/80) was performed to study the distinct role of F4/80+ KCs during the regenerative process. In RNA sequencing of isolated F4/80+ KCs, the initiation phase was compared with the progression phase. Immunohistochemistry and immunofluorescence staining of Ki67, HNF-4α, CD-31, and F4/80 and Western blot of the TGF-β2 pathway were performed. Depletion of F4/80+ KCs in pre-PHx delayed the peak of hepatocyte proliferation from 48 h to 120 h, whereas depletion in post-PHx unexpectedly led to persistent inhibition of hepatocyte proliferation, indicating the distinct role of F4/80+ KCs in the initiation and progression phases of liver regeneration. F4/80+ KC depletion in post-PHx could significantly increase TGF-β2 serum levels, while TGF-βRI partially rescued the impaired proliferation of hepatocytes. Additionally, F4/80+ KC depletion in post-PHx significantly lowered the expression of oncostatin M (OSM), a key downstream mediator of interleukin-6, which is required for hepatocyte proliferation during liver regeneration. In vivo, recombinant OSM (r-OSM) treatment alleviated the inhibitory effect of α-F4/80 on the regenerative progression. Collectively, F4/80+ KCs release OSM to inhibit TGF-β2 activation, sustaining hepatocyte proliferation by releasing a proliferative brake.

Résumé :
Abstract Background 10% of people over 65 years have frailty, rising to between a quarter and a half of those over 85. Comprehensive Geriatric Assessment (CGA) is the gold standard for the management of frailty in older people; it’s a process that involves a holistic, interdisciplinary assessment of the older person. Evidence shows that CGA is effective in improving outcomes for older people. The National Clinical Programme for the Older Person outlines the role of the occupational therapist in CGA. A clinical audit was completed to investigate whether the occupational therapists in the Medicine for the Older Person service are adhering to guidelines for CGA. Methods Following a literature review, a retrospective audit of occupational therapy (OT) initial assessments was completed using paper count method. Data pertinent to CGA OT assessment was collected. Audit results were presented to the OT team alongside a CGA education session. A re-audit was completed, using above methods. Results 10 inpatient initial assessment forms reviewed pre and post audit. Activities of Daily Living Assessment: Pre result: 60% Post Result: 90% Home Environment: Pre: 80% Post: 100% Performance Components Assessment: Vision: Pre: 90% Post: 100% Hearing: Pre: 80% Post: 100% Upper Limb: Pre: 70% Post: 100% Mood: Pre: 60% Post: 80% Cognition: Pre: 90% Post: 100% Sleep: Pre: 70% Post: 80% Social Assessment: Pre: 60% Post:90% Falls Assessment: Pre: 60% Post: 90% Drving: Pre: 60% Post: 90% Conclusion This audit identified that the occupational therapists in the medicine for the older person service were not CGA compliant. CGA training now forms an important part of our OT induction programme. We are now “playing our part” in CGA with improvements in all domains. CGA is now commonplace practice within our team, in line with international and national best practice guidance for management of frailty.

Background Edaravone slowed the rate of functional decline in subjects with amyotrophic lateral sclerosis (ALS) in phase 3 study MCI186-19 (Study 19). One of the Study 19 inclusion criteria was forced vital capacity (FVC) ≥80% of predicted (≥80%p). Therefore, the study provided no information on edaravone efficacy in subjects with FVC <80%p. In Study 19, 24-week, double-blind treatment was followed by open-label treatment where all subjects received edaravone. At 24 weeks, some subjects had FVC <80%p (FVC24 <80%p). This allowed for post-hoc assessment of the effects of edaravone in subgroups of subjects with FVC24 ≥80%p vs <80%p. Objective To address the question of the efficacy of edaravone in ALS patients with FVC <80%p. Methods Post-hoc analysis of Study 19 comparing edaravone efficacy at week 48 in subjects with FVC24 ≥80%p vs <80%p. Results With edaravone treatment, subjects in both the FVC24 ≥80%p and the FVC24 <80%p subgroups experienced a reduction in ALS Functional Rating Scale-Revised (ALSFRS-R) score loss vs placebo subjects through week 48. For the FVC24 ≥80%p subgroup, the changes in ALSFRS-R scores from baseline to week 48 were −7.63 for edaravone-edaravone vs −9.69 for placebo-edaravone, a difference of 2.05 (P = .034; 95% CI: 0.16, 3.94). For the FVC24 <80%p subgroup, the changes in ALSFRS-R scores from baseline to week 48 were −10.26 for edaravone-edaravone vs −15.20 for placebo-edaravone, a difference of 4.94 (P = .0038; 95% CI: 1.64, 8.25). Linear regression analysis indicated that, in the FVC24 <80%p subgroup, there was a notable change in the slope of the ALSFRS-R score-vs-time graph after the start of edaravone treatment. Conclusion ALS subjects in the Study 19 placebo arm had a slowing in disease progression, even when edaravone was added with an FVC of <80%p prior to starting edaravone. A randomized, placebo-controlled study is needed to validate these post-hoc findings.

Résumé :
ObjectiveThe aim of this study was to evaluate if varying levels of compliance with an enhanced recovery after surgery (ERAS) protocol impacted post-operative outcomes (length of stay, complications, readmissions, and re-operations) in gynecologic surgery at a tertiary center.MethodsWe included 584 patients who had open gynecologic surgery between November 1, 2014 and December 31, 2016. Patients were categorized into subgroups according to their date of surgery from the time of the ERAS protocol implementation. Patients were categorized by their per cent compliance into two groups:<80% versus ≥80%. We analyzed compliance with the elements of the protocol over time and its relation with post-operative outcomes, length of stay, post-operative complications, readmission, and re-operations rates. We modeled the probability of having a post-operative complication within 30 days of surgery as a function of overall compliance.ResultsOverall compliance was 72.3%. Patients with compliance ≥80% had significantly less complications (P<0.001) and shorter length of stay (P<0.001). Readmission and re-operation rates were not impacted by compliance (P=0.182, P=0.078, respectively). Avoidance of salt water overload, early mobilization, early oral nutrition, and early removal of Foley catheter were significantly associated with less post-operative complications within 30 days.ConclusionsCompliance with an ERAS pathway exceeding 80% was associated with lower complication rates and shorter length of stay without impacting on re-operations or readmissions.

Résumé :
Purpose Reports are mixed on whether post-swallow residue increases with age. This question was investigated using a quantitative measure of residue in a large cohort of healthy adults. It was hypothesized that post-swallow residue would differ between younger and older healthy adults, and significantly increase in individuals ≥ 80 years. Method In this observational study, 275 healthy volunteers aged 18–99 years underwent videofluoroscopic swallow studies. Post-swallow residue was quantified using the Bolus Clearance Ratio (BCR) for a 20-ml thin liquid barium bolus and 3-cm 3 barium paste bolus. Number of swallows required per bolus and aspiration were also determined as additional markers of swallow efficiency and safety. Results Most BCR values were low, < 0.05 (< 5% of the bolus) for 78% of 20 ml and 66% of paste swallows. BCR values were significantly larger in healthy older adults, although, clinically, the differences are arguably minimal (differences in mean BCR values between age groups: 20 ml: 0.01 [1%]; paste: 0.03 [3%]). For 20 ml, 56% of adults ≥ 80 years performed multiple swallows (oral residue and dose metering) versus 9% of adults 18–39 years. One aspiration event was observed. Conclusions Post-swallow residue was slightly increased in older adults; many individuals ≥ 80 years performed more than one swallow per bolus to achieve this similar level of clearance. This suggests a reduction in swallow efficiency with age without compromising swallow safety. Post-swallow residue that poses a risk of airway invasion should be considered a sign of swallowing dysfunction, not healthy aging.

Résumé :
Two experiments were conducted over a two-year period for large crabgrass control in centipedegrass. The experiments were: (a) sequential applications of PRE and POST herbicides, and (b) POST herbicides applied as sequential or tank-mixed applications. Large crabgrass control was greater than 80% throughout the spring and summer when dithiopyr was applied PRE at 0.14 kg/ha late in February or when tank mixed at the same rate with sethoxydim at 0.11 kg/ha and applied POST in late May. Control was similar from a sequential program of dithiopyr applied PRE at 0.14 kg/ha in February and followed either by dithiopyr at 0.14 kg/ha or sethoxydim at 0.11 kg/ha as POST in early to mid-summer. Dithiopyr at 0.28 kg/ha as a POST application in late May did not control large crabgrass consistently. Control was > 80% for 6 to 8 wk when the POST dithiopyr treatment was repeated at the same rate 1 mo later. Sethoxydim applied POST controlled large crabgrass greater than 80% throughout the spring and summer when applied once at 0.22 kg/ha or twice at 0.11 kg/ha in each of two applications. Dithiopyr applied alone or with sethoxydim as sequential or tank-mixed treatments caused slight to moderate centipedegrass injury but was commercially acceptable (< 30%). However, injury from tank-mix herbicides was similar to either applied alone. Quinclorac severely injured (> 30%) centipedegrass, and 8 to 10 wk were required for recovery.

With the advent of the Corona virus/COVID-19, airline traffic has dropped off so much because of lack of passenger/shipper demand, and/or governmental sanitation rules, that airlines may wish to use less than 80% of their allocated slots, but then risk a loss of slots. It is a catch 22 situation: either you fly a (near) empty aircraft and keep your slot for better, post COVID times (so-called ghost flights), or you go under 80% and you lose your grandfathered slot right. Governments, other authorities and not in the least the EU, have stepped in to relax the 80/20 rule, at least temporarily. The article is divided into four parts: introduction; air transport under the Corona virus; slot allocation under Corona; slot allocation post Corona. Corona, EU, IATA, Slots, 80/20 rule

Résumé :
Abstract Background: The optimal hemoglobin (Hb) in the setting of acute myocardial infarction (MI) is unknown. Anemia reduces the oxygen carrying capacity of blood and may theoretically exacerbate ischemia increasing myocardial injury. The benefit of transfusion to correct anemia in acute coronary syndromes is also controversial. The goal of this study was to determine the optimal Hb in the setting of acute MI and whether transfusion reduces myocardial injury and improves outcome. Experimental design: Ninety-two male Sprague-Dawley rats (170–190g) were divided into 8 groups: 1: Normal Hb, sham operation; 2: Normal Hb, MI; 3: Hb 80–90g/L, sham; 4: Hb 80–90g/L, MI; 5: Hb 70–80g/L, sham; 6: Hb 70–80g/L, MI; 7: Hb 80–90g/L, MI with transfusion to 100g/L; 8: Hb 80–90g/L, MI with transfusion to 120g/L. To determine the effect of blood transfusion, fresh blood was immediately transfused following MI. Induction of myocardial infarction was preformed by left coronary artery ligation. Sham-operated rats underwent the same surgical procedure without ligation. At 24 hours post-MI, rats were re-anaesthetized and hemodynamic measurements preformed. Area at risk and infarct size was measured by Evans blue and triphenyltetrazolium chloride, respectively. Results: In the normal Hb group (140–150g/L), survival following sham or MI surgery was 100%. Reduction of Hb to 80–90 and 70–80g/L significantly decreased survival post-MI to 42% and 47%, respectively. Survival was significantly improved after transfusion of fresh blood to raise the hemoglobin from 80–90g/L to 100g/L (P<0.05). However, there was no improvement in survival when Hb was raised by transfusion to 120g/L (P=NS). Twenty-four hours post-MI, the ischemic to non-ischemic left ventricle (LV) weight ratios were not significantly different between any groups indicating similar areas of myocardial ischemia among all groups (P=NS). However, the infarct size to area at risk ratios were significantly increased in both 70–80g/L and 80–90g/L groups compared to the normal Hb group (P<0.05). Transfusion from Hb 80–90g/L to 100g/L significantly decreased infarct size compared to the Hb 80–90g/L group (P<0.05). However, transfusion to Hb 120g/L resulted in a significantly larger infarct size compared to the Hb 100g/L transfused group (P<0.05). Cardiac function was determined at 24 hours post-MI. Heart rate, MAP, LVSP and LVEDP were not significantly different among all groups but anemic groups 80–90 and 70–80g/L showed a significant decrease in LV +dP/dtmax and −dP/dtmin in both sham and MI rats (P<0.01) with the most significant decrease noted in MI rats compared to sham operated rats (P<0.05). Blood transfusion post-MI from a Hb 80–90 to 100g/L significantly improved LV +dP/dtmax (P<0.05). Transfusion to Hb 120g/L did not result in any further improvement in cardiac function. Conclusions: In the setting of acute MI, anemia increased mortality and infarct size compared to non-anemic controls. At 24 hours post-MI, anemia also impaired cardiac function. Transfusion of anemic animals up to a Hb of 100g/L with fresh RBCs reduced mortality and infarct size and improved cardiac function. However, transfusion to a Hb of 120g/L did not demonstrate any additional benefit and was associated with larger infarcts.

Résumé :
This study involved healthy community-living older adults in an investigation of the association between moderate alcohol consumption (AC) and acute changes in postural stability and whether the association differed according to pre-AC balance skills. Thirty-nine moderate drinkers aged ≥ 65 years (62% women; mean age: 73.9 ± 6.1 years) consumed a moderate dose of alcohol (0.4 g/kg; administered as two drinks). Breath alcohol concentration and postural stability were measured at five time points (pre-AC and 40, 80, 120, and 160 minutes post-AC) using unipedal stance time (UPST) and center of pressure (CoP) displacement. Pre-AC UPST was used to categorize participants into good-balance (≥30 seconds) and poor-balance (<30 seconds) groups. Peak breath alcohol concentration was 30 mg/dL at 40 minutes post-AC. For all participants, postural stability declined significantly at 80 minutes post-AC (UPST, p = .005; anterior–posterior CoP displacement, p = .029). While the poor-balance group did not show a significant decrease in UPST duration over the course of the study, the good-balance group experienced significant decline at 80 minutes compared with baseline ( p < .001) and remained above the 30-second UPST cutoff. Both groups experienced similar worsening in anterior–posterior CoP displacement at 80 minutes post-AC. Thus, moderate AC was associated with acute decline in postural stability in older adults. The worsened anterior–posterior CoP displacement post-AC in the poor-balance group was of particular concern because these participants were already at lower balance functioning pre-AC. Larger, more representative studies of varying groups of participants are needed to further explore how this change relates to fall incidents and fall risk.

Résumé :
Background: Penicillin allergy is one of the most frequent self-reported allergies; however, only about 10% of reported allergies are accurate. Objectives: Through the creation of a continuing pharmacy education (CPE) activity, we sought to assess knowledge gaps and comfort levels in the management of penicillin allergies. Methods: A 1-hour enduring-content CPE activity was offered as an interactive course from September 20, 2019, to September 20, 2020. Participants completed 3 surveys (pre-survey, post-survey, and follow-up survey). Participants were pharmacists and pharmacy technicians who completed, at a minimum, the activity and both pre- and post-surveys. The primary outcome was the percentage of participants scoring >80% on knowledge-based questions on the post-survey compared with the pre-survey. Secondary outcomes included pre-post comparisons on knowledge-based questions, participants’ self-report of an allergy, and comfort levels dispensing cephalosporins in a patient with a self-reported penicillin allergy. Results: A total of 389 participants completed the CPE activity, with 176 included for analysis. Significantly more participants scored >80% on knowledge-based questions on the post-survey compared with the pre-survey (71.6% vs 22.7%, P < .001). There was no significant difference between the percentage of participants scoring >80% on the post-survey and the follow-up survey (71.6% vs 65%, P = .119). The majority of participants (74%) felt comfortable dispensing a cephalosporin in a patient with a penicillin allergy on the pre-survey, with similar percentages on the post- and follow-up surveys (77% and 90%, respectively). Conclusion: A targeted continuing education program improved overall knowledge, which was sustained for up to 2 months.

Résumé :
Biomechanics of post-cam mechanism is essential in determining the longevity of knee implant. Computational knee simulator is an efficient method in characterizing TKA performance under various boundary conditions. The existing knee simulators, however, were actuated only by quadriceps translation and hip load to perform squatting motion. The present computational knee simulator was developed based on lower limb of Japanese female subject having body weight, W = 51 kg and height, H = 148 cm. Two different designs of PS-type knee prostheses were tested namely Superflex and NRG. The knee motion was driven by three major muscles; quadriceps, hamstrings and gastrocnemius. The biomechanical behavior of tibiofemoral articulation associated with post-cam engagement mechanics was observed. Post-cam engagement occurred at 80° and 65° of flexion angles for Superflex and NRG, respectively. Maximum von Mises stresses at tibial post were 80 MPa and 50 MPa for Superflex and NRG, respectively. The developed computational muscle driven knee simulator has successfully assessed the performance of TKA prostheses.

Résumé :
The purpose of this study is to compare the mechanical properties in the form of ultimate tensile strength, ultimate tensile strain and Young's modulus of an epoxy resin at different curing cycles. The work carried out consisted of investigating the effect of preheating time and then the effect of post-curing time at the same temperature. Five repeats of static tensile tests were then carried out using universal test machine. Results indicated that compared to a shorter epoxy resin preheat duration of 15 min at 80°C, a longer duration of 30 min at 80°C of preheating degrades the material ultimate tensile strength and ultimate tensile strain leading to a suffer material. However, compared to no further post-curing of the epoxy resin, a two-hour post-cure duration at 80°C slightly increased the ultimate tensile strength and significantly decreased the ultimate tensile strain making the material even suffer than in the case of preheating. The implication is that in-house cure cycle tests should be carried out to characterize the resin instead of exclusively relying on resin manufacturer proposed cure cycles.

Résumé :
This study aimed to quantify the effect of muscle, ageing and cooking temperature on the texture, cooking loss and shrinkage of cooked beef. Cuboids from unaged (1 day post mortem) and aged (14 days post mortem) semitendinosus, biceps femoris and psoas major muscles, from both sides of five beef carcasses, were cooked at four different cooking temperatures (50, 60, 70 and 80 °C) for 30 min. and their Warner–Bratzler shear force (WBSF), cooking loss and shrinkage (longitudinal and transverse) were quantified. The WBSF was reduced by ageing in the muscles at the specific cooking temperatures: psoas major (cooked at 50, 60 and 80 °C), semitendinosus (70 and 80 °C) and biceps femoris (80 °C). The cooking loss was 3% greater in aged compared to unaged muscles. The longitudinal shrinkage was greatest in psoas major at 80 °C amongst the muscle types and it was reduced by ageing in psoas major (70 and 80 °C) and biceps femoris (80 °C). The transverse shrinkage was reduced by ageing only in biceps femoris, across all temperatures; and the diameter of homogenized fibre fragments from semitendinosus and biceps femoris was reduced more by cooking at 50 °C in unaged compared to aged condition. WBSF was related to transverse shrinkage, and cooking loss was related to longitudinal shrinkage. The effect of muscle type on the physical changes occurring during cooking of beef is dependent on ageing and cooking temperature.

Résumé :
Recombinant antithrombin gamma (rAT) is reported as an effective drug for patients with disseminated intravascular coagulation (DIC) in Japan. As the appropriate dose and targeted AT activity remain unknown, this study aimed to determine these aspects for sepsis-induced DIC. Thirty-one patients with septic shock and DIC with AT levels <70% were treated with rAT between May 2018 and December 2020. The recovery rates from DIC were 32.2% and 63.3% on day 3 and 5 post administration, respectively. Recovery and survival rates were significantly higher in patients who achieved AT activity ≥70% or 80% on day 3 post administration. Receiver operating characteristic curve analysis revealed that the cutoff values of post-treatment AT activity on day 3 for 28-day survival and 5-day recovery from DIC were 79.5% and 81.5%, respectively. Patients who did not achieve AT activity ≥80% on day 3 presented a lower base level of AT activity and lower dose supplementation. Our results suggest that targeted AT activity should be at least 70%, and ideally 80%, and sufficient doses to maintain this activity are required to achieve better outcomes.

AbstractCadillo is an invasive species in Florida pastures and natural areas. Despite its invasiveness, relatively few studies have evaluated cadillo management. Thus, the objective of this research was to determine effective POST herbicides for cadillo control in Florida. Greenhouse and field studies were conducted at the Range Cattle Research and Education Center near Ona, FL, in 2015 and 2016. In the greenhouse study, triclopyr-ester, aminopyralid, metsulfuron, 2,4-D amine, aminopyralid+metsulfuron, aminocyclopyrachlor+metsulfuron, and imazapyr+aminocyclopyrachlor+metsulfuron provided ≥80% control of cadillo 28 d after treatment (DAT). Aminocyclopyrachlor at 17 and 35 g ha–1were the only treatments with <80% control, with 70% and 75% control, respectively. Similar results were reflected in cadillo dry biomass reduction. The herbicide treatments used in the field study were triclopyr-ester, aminopyralid, 2,4-D amine, aminocyclopyrachlor, and triclopyr+fluroxypyr. Most treatments provided excellent control in the field (≥90% control) 30 DAT, and by 60 DAT all treatments provided 100% control. Results from these studies suggest that cadillo is susceptible to many of the common POST herbicides utilized in pastures and natural areas in Florida.
