Pain signalled by a normal sensory system, nociceptive pain, serves a vital protective function. The peripheral and central nervous somatosensory systems permit rapid localization and identification of the nature of painful stimuli, prior to appropriate action to minimize or avoid potentially tissue damaging events. A reduction or absence of pain resulting from neurological disease emphasizes the importance of this normal protective function of pain. For example, tissue destruction occurs frequently in peripheral nerve diseases which cause severe sensory loss such as leprosy, and in central disorders such as syringomyelia. Neuropathic pain results from damage to somatosensory pathways and serves no protective function. This chapter provides an overview of neuropathic pain, considering its context, clinical features, pathophysiology, and treatment.In the peripheral nervous system, neuropathic pain is caused by conditions affecting small nerve fibres, and in the central nervous system by lesions of the spinothalamic tract and thalamus, and rarely by subcortical and cortical lesions. The clinical feature common to virtually all conditions leading to the development of neuropathic pain is the perception of pain in an area of sensory impairment, an apparently paradoxical situation. The exception is trigeminal neuralgia.Neuropathic pain is heterogeneous clinically, aetiologically, and pathophysiologically. Within a given diagnostic category, whether defined clinically or aetiologically, there are wide variations in reports of pain by patients. This heterogeneity poses one of the greatest challenges in understanding the mechanisms of neuropathic pain. Knowledge of the pathophysiology is an obvious pre-requisite to the development of effective treatments. The goal of a pathophysiologically based understanding of the symptoms and signs of neuropathic pain is, of course, just such a rational and specific approach to treatment. While this is not yet achievable, clinical-pathophysiological correlations have led to some recent advances in treatment.

The discovery of pain biomarkers has profound implications for both pharmacology and neurobiology; in 2013, in the landmark paper discussed in this chapter, Wager et al. presented a neurologic signature of pain based on human brain imaging performed in healthy individuals administered experimental heat-pain stimuli. Using advanced analytic methods based on machine learning and multivariate pattern analysis, Wagner et al. provide very convincing support for the idea that pain is encoded in a distinctive pattern of brain activity in one or several brain areas typically referred to as the ‘pain matrix’, which acts as a saliency detection system for the body. Although the usage of such tool to infer pain in patients poses major challenges and is clearly not indicated in medico-legal contexts, the study provides experimental proof of concept in favour of a pattern theory of pain as well as for a specificity theory of pain.

Veterans of military service pose a special case both in their susceptibility to addiction and their risk for a co-occurring disorder, posttraumatic stress disorder. The elements of a military history are provided; this is not intuitive, and is salient both for determining entitlement status and to assess the cumulative injury. Numerous pain syndromes can occur as the result of both long-term wear and tear and abrupt injury, requiring consideration for disability compensation. PTSD and traumatic brain injury are more common here than perhaps among any other occupational group. Unique risks and concerns accompany the experience of warfare in different theaters; special sections are devoted to Iraq and Afghanistan; the first and second Gulf wars; and to Vietnam. Women constitute a newly enfranchised military group as subject to injury as their male cohorts. Several pain care innovations have been championed by the Veterans Administration, and these are described.

Spinal cord stimulation (SCS) has been a long established therapy for various pain conditions including low back pain, failed back surgery syndrome, complex regional pain syndrome, and other neuropathic and nociceptive pain states. Since the first report of SCS in 1967 by Shealy, advances have occurred in the technology used to achieve clinical analgesia. Developments in both the hardware and software involved have led to significant improvements in functional specificity, as seen in dorsal root ganglion stimulation, along with increasing breadth and depth of the field of neuromodulation. The patient experience during the implantation of the systems, as well as post-procedurally has been enhanced with improvements in programming. These technological improvements have been validated in quality evidenced-based medicine: what was a static area now is a dynamic field, with neuromodulation poised to allow physicians and patients more viable options for better pain control for chronic painful conditions.

The general approach for a child who has suffered burns, as for any injured child, should be assessment and management of airway, breathing, and circulation (ABC). A child status post-burn injury may require immediate intubation due to smoke inhalation and ensuing respiratory distress. The resuscitation of the child with burn injury poses a number of challenges to the anesthesiologist and requires an understanding of the pathophysiology of this type of injury. Appropriate vascular access and pain management are also of high priority in this scenario. This chapter discusses the approach to resuscitation of the child with acute burn injuries, reviews the key issues in developing a plan to secure the airway, and describes effective pain management in the setting of an acute burn.

Apart from non-steroidal anti-inflammatory drugs (NSAIDs), there are only two categories of systemic analgesics, namely paracetamol (acetaminophen) and opioids, that are currently available worldwide for clinical use. Paracetamol is poorly effective in relieving pain and improving function in patients with symptomatic osteoarthritis (OA). Furthermore, its safety profile is less favourable than classically thought. In fact, there is evidence paracetamol acts as a weak inhibitor of the cyclooxygenase enzymes. Given that paracetamol poses a lower risk of severe adverse events than NSAIDs while being better tolerated than opioids, it is usually considered as the first-line systemic analgesic for OA. Commonly prescribed opioids are primarily agonists of the mu receptors, thereby producing similar desirable (analgesia) and untoward effects. Meta-analyses of short-term clinical trials showed that, on average, the modest clinical benefits of opioids did not outweigh the side effects in patients with knee or hip OA. Accordingly, most current guidelines support the use of opioids for selected OA patients only (e.g. patients who have not had an adequate response to other treatment modalities and are not candidates for total joint arthroplasty). In view of the limited efficacy and/or potential harms of available analgesics, particular attention was paid to novel painkillers, especially nerve growth factor (NGF) antagonists. Although these agents provided clinically meaningful improvements in pain and physical function in patients with hip or knee OA, they lead to severe side effects, including rapidly destructive arthropathies and neuropathies. Thus, if approved for marketing, NGF antagonists would be reserved for selected and well-defined patients with OA.

This book addresses the complexities of transnational posted work through three key topics. First, it examines how the de-territorialization of national models and employment relations systems opens up exit options for management, enabling them to use the regulatory framework creatively and at a disadvantage for workers. Second, it discusses how re-territorialization, or resistance, is possible within these spaces. Third, the book analyzes the contours of the new structure for employment relations that emerges within the pan-European labor market and its implications for worker voice, regulatory enforcement, and management power. The research presented in this book is based on a qualitative and multilevel case study approach. It examines how posted workers and actors involved in the posting relationship actually utilize and experience the European posting framework by focusing on the experiences of transnational posted workers. This distinguishes the book from macro- and national-focused approaches in comparative political economy and industrial relations by zooming in on the workplace dynamics in a transnational setting. The window to how posted workers experience intra-EU mobility is Germany and the two sectors where posting is most prevalent: the construction and meat slaughtering industries.

Orthopedic tumor surgery may be complicated by chemotherapy treatment, anesthetics can be affected by drug interactions, and postoperative pain management can be complex. In this chapter, we review common coexisting diseases in pediatric patients undergoing orthopedic surgeries. These diseases include syndromes such as Down syndrome, Marfan syndrome, and Klippel-Feil syndrome; muscular dystrophies such as Duchenne muscular dystrophy; and bony cancers such as osteosarcoma.

This introduction distinguishes ways the social dimensions of moral responsibility have been investigated in recent philosophical literature: some theories highlight the interpersonal dimensions of moral responsibility practices; some explicate the interlocutive properties of morally reactive exchanges; while others seek to explain the role of the social environment in scaffolding agency. Despite the rise of social approaches, philosophers have paid scant attention to the implications of inequalities of power for theorizing about moral responsibility. The remainder of the introduction articulates a set of problems posed by contexts of structural injustice for theories of moral responsibility and highlights the relevance of recent work in feminist philosophy on relational autonomy and social epistemology for understanding and addressing these problems. The introduction notes the overlaps and differences between the concepts of autonomy and moral responsibility and offers preliminary reflections on how debates about relational autonomy might bear on social theories of moral responsibility.

Pectus excavatum is the most common congenital chest wall deformity in children. It is characterized by a posterior depression of the sternum resulting from defective growth of the surrounding rib cartilage. This abnormality in thoracic architecture can lead to a progressive reduction in cardiopulmonary capacity. An early surgical approach popularized by Ravitch involved an open repair with removal of abnormal rib cartilage to allow for regrowth of the rib cartilage to the sternum in a more anterior position. Subsequently, Nuss popularized a minimally invasive repair of pectus excavatum which involves the internal bracing of the chest wall and anterior displacement of the sternum without cutting of the rib cartilage. It is now the most common surgical approach for pectus excavatum correction. Anesthesiologists should acutely be aware of the potential intraoperative complications associated with this procedure as well as the postoperative challenges this procedure poses in terms of pain management.

UD often pose a significant diagnostic challenge, as symptoms are largely non-specific and easily confused with other conditions such as bladder pain syndrome and recurrent urinary tract infection. As such, both misdiagnosis and delayed diagnosis are common, to the frustration of both patients and surgeons. Traditional methods of diagnosis, such as voiding cystourethrogram, relied upon urethral catheterization and contrast instillation, and were associated with poor anatomical detail. Recent advances in imaging, particularly magnetic resonance imaging, have improved the diagnosis and staging of UD, and have allowed for more accurate preoperative planning.

Comparative law in this context requires attention being paid to elements of identity and difference that must be studied in light of the questions it poses. Thus, this chapter begins by tracking the path of statehood, administration, and administrative law in Germany up to the adoption of the Basic Law (Grundgesetz). This is done in order to present an historical means with which to comprehend the present European legal space. The chapter then presents the development of administrative law under this law, and in closing, discusses understandings of administrative law.

This chapter explores the relative merits of two different event-causal libertarian views. One is Robert Kane’s well-known view, and the other is the “daring libertarian” view floated in Alfred Mele’s 2006 book, Free Will and Luck. It is argued that event-causal libertarians should prefer the latter view to Kane’s view. Special attention is paid to a problem that luck poses for libertarian theories—a problem that the two views at issue attempt to solve in different ways. The problem is applied both to the decisions of adults and to the decisions of young children. A suggestion about how human beings come to be in a position to perform their earliest free actions is developed.

There is a consensus on the diagnostic and therapeutic manage¬ment of bacterial prostatitis (acute and chronic). However, increas¬ing antimicrobial resistance rates for quinolones pose problems for the future, especially for therapy of chronic bac¬terial prostatitis. In chronic prostatitis/chronic pelvic pain syndrome, the diag¬nostic approach currently points more and more to an individu¬alized phenotypic assessment, in an effort to direct multimodal management towards improvement of specific symptom domains. Most therapy trials for single agents in CP/CPPS have been nega¬tive, therefore stratification by phenotype followed by individu¬alized multimodal treatment seems to be a promising strategy, although good evidence-based data are not available currently to substantiate this. Consensus regarding the need and option for treatment of asymptomatic prostatitis is far from being achieved. Therefore, taking the high prevalence of infections and inflammations in different asymptomatic conditions into consideration, further research is urgently needed to address this important field.

Bipolar disorder poses a difficult question for outreach workers, as patients are often well recovered between episodes—so should persisting outreach be provided? We report very good results in severe bipolar disorder where continuity of care has paid off. The chapter also deals with theories of causation and classification. The section on treatment identifies the importance of early admission in hypomania, the use of mood stabilizers, and the value of identifying and agreeing on relapse signatures. It also confirms the value of working hard to strengthen the therapeutic relationship and of psychosocial interventions such as psycho-education. Long-term work with these patients brings home just how persistent and disabling the depressive phases can be.

This book provides an account of how we might address wrongdoing given challenges to anger and retribution that arise from ethical considerations and from concerns about free will. It contends that we should dispense with basically deserved pain and harm, and with associated retributive sentiments. Without such desert, how might we understand blame? Blame can be conceived as taking on a non-retributive stance of moral protest, whose function is to secure forward-looking goals such as moral reform and reconciliation. Is it possible to justify effectively dealing with those who pose dangerous threats if they do not deserve to be harmed? Wrongfully posing such a threat, by contrast with deserving harm for posing the threat, is proposed as the core condition for the legitimacy of defensive harming. An account is then provided for addressing criminal behavior without a retributive justification for punishment, one in which the right of self-defense provides justification for measures such as preventative detention. How might we forgive if wrongdoers don’t basically deserve the pain of being resented, which forgiveness would then renounce? Forgiveness might instead be conceived as the renunciation of the stance of moral protest. But how might personal relationships function without retributive anger having a role in responding to wrongdoing? The stance of moral protest, together with non-retributive emotions, is argued to be sufficient. The book closes with a consideration of attitudes regarding the fate of humanity in a deterministic universe replete with wrongdoing, and defends the rationality of a transcendent hope for humanity.

Phantom limbs pose a philosophical problem about the location of pains. The work of Descartes first used them to make a philosophical point about the brain in relation to the body. They have traditionally been thought of as being due to nerve endings on the pathway to the original limb being activated. However, it was subsequently discovered that the phenomenon occurs even when the spinal chord is severed, suggesting that it is rather a question of brain activity, part of a neurosignature through which the brain indicates the body is one’s own. More recent resarch suggests involvement not only of the sensory systems but also the parietal cortex and the limbic system, which is concerned with emotion and motivation.

The chapter scrutinizes how German and Danish unions are navigating the increasingly integrated slaughterhouse industry, based on a case study of a large Danish multinational slaughterhouse company with operations in both Denmark and Germany. The German slaughterhouse industry is highly affected by increasing fragmentation of production and the widespread use of low-paid workers often posted from Eastern Europe, which the Danish multinational company utilizes to lower its production costs. The Danish slaughterhouse workers’ union was more successful in safeguarding workers’ wage and working conditions and preventing labour market segmentation, while their German peers faced much more precarious work. These differences are explained by the vast differences in the unions’ power resources in the two systems. Danish unions were in a much stronger position in terms of membership, where unionization remains a social custom, with stronger collective agreements and local representation compared with the German union.

Discretionary wars—conflicts in which national survival is not an issue and even vital national interests may not be at stake—pose particular challenges for any government which does not explain why the cost being paid in blood and treasure is ‘worth it’.

Elaine’s dealer, Robert Graham, had offered to donate a portrait of President Kennedy to the Harry S. Truman Library. Agreement to this plan was based on her ability to work very quickly. In late December 1962, Elaine arrived at the Winter White House in Palm Springs, California. She was dazzled by JFK. But he was constantly in motion, sitting casually, and likely to show up in sports clothes. Back in New York, Elaine painted dozens of canvases that dissatisfied her. A presidential advisor complained about JFK’s pose. She finally produced a suitable portrait; two months later, Kennedy was assassinated. Grieving, Elaine was unable to paint for a year. A Life magazine story about the portrait ran on May 8, 1964. The Truman Library finally accepted one of the paintings in February 1965, but the portrait now in the National Portrait Gallery is her triumph, a blaze of light and energy.

Dependable knowledge of the future, the past, or the distant present is thwarted by the fickle forces of chance (fortuna) and unverifiable report (fama); accurate perception of meaning is compromised by the instability or equivocality of all systems of signification, linguistic or symbolic, and by the notorious opacity of human intention; and successful strategies for dealing with power in its many forms founder on power’s chronic arbitrariness and irrationality. The key to coping with these challenges is pragmatic prudence: rational calculation issuing in an opportunistic, often amoral choice of ingenious deeds and/or eloquent words appropriate (though without guarantee) to mastering a specific crisis. The goal of pragmatic prudence is agency now, not salvation later. This book examines how ideas of war and peace have functioned as organizing frames of reference within the history of political theory. It interprets ten widely read figures in that history within five thematically focused chapters that pair (in order) Schmitt and Derrida, Aquinas and Machiavelli, Hobbes and Kant, Hegel and Nietzsche, and Thucydides and Plato. The book’s substantive argument is that attempts to establish either war or peace as dominant intellectual perspectives obscure too much of political life. The book argues for a style of political theory committed more to questioning than to closure. It challenges two powerful currents in contemporary political philosophy: the verdict that premodern or metaphysical texts cannot speak to modern and postmodern societies, and the insistence that all forms of political theory be some form of democratic theory. What is offered instead is a nontraditional defense of the tradition and a democratic justification for moving beyond democratic theory. Though the book avoids any attempt to show the immediate relevance of these interpretations to current politics, its impetus stems very much from the current political circumstances. Since the beginning of the twenty-first century , a series of wars has eroded confidence in the progressively peaceful character of international relations; citizens of the Western democracies are being warned repeatedly about the threats posed within a dangerous world. In this turbulent context, democratic citizens must think more critically about the actions their governments undertake. The texts interpreted here are valuable resources for such critical thinking.
