Worldwide there are over 50 million deaths per year. In considering the goal of fostering optimal care towards the end of life for these individuals, high-quality population-based data about disease and symptom occurrence as well as health-care needs are essential. Such data are important so that informed planning can underpin policy, service development, and patient care. This chapter discusses epidemiology as it relates to the ‘human experience’ towards the end of life with an emphasis on diseases, symptoms, psychosocial experiences, and access to health services. Some of the methods by which such data are collated in various parts of the world are described. The dramatic variability in experiences towards the end of life across regions and nations is also highlighted. Examples of where information is available that may inform planning for populations are discussed, as are areas where data may be helpful or needed but lacking.

The Region of the Americas has made considerable progress in improving the health of its population, but it remains one of the most inequitable regions in the world. The challenges are enormous and varied: millions of people lack access to comprehensive health services, health systems provide fragmented care, and there are major disparities in human resources for health. In response to these challenges, PAHO adopted the Strategy for Universal Access to Health and Universal Health Coverage in 2014, recognizing the need to transform or strengthen health systems and services to combat health inequities and secure health and well-being for all people in the Region. This report addresses the basic aspects of implementing the Strategy, offering Member States a series of practical recommendations to help them plan and implement changes that will lead to the strengthening of their health services. It reviews the practical recommendations related to the first strategic line of the Strategy and the three interrelated lines of action. It also explains how to select the services and benefits that should be prioritized as part of the progressive expansion of health services to communities. Finally, it considers issues related to stewardship and the management of transformative change. Understanding how to address change and stimulate investment in health, well-being, and development is essential to achieving universal access to health and universal health coverage.

Facing the threats posed by dedicated suicide bombers who have access to modern technology for mass destruction and who intend to cause maximum human suffering and casualties, democratic governments have hard choices to make. On the one hand, they must uphold the basic values of democratic societies based on due process and human rights. On the other, they need to pre-empt the kind of destruction inflicted upon New York, Madrid, London, and Bali. The premise of this book is that for intelligence organizations to be able to face up to the challenges of global terrorism, they must think outside the box and utilize all of their resources effectively and creatively. To overcome the enemy, we must also secure the peace. Winning the hearts and minds of the terrorists' pool of potential recruits will be essential to cutting off the supply of suicide bombers. The support and cooperation of the people in countries where the terrorists strike must be sustained by ensuring they have confidence in the government and intelligence services. If a government and its intelligence services become so focused on pre-empting terrorist attacks that they infringe on the rights of their citizens and encroach on democratic norms, they unwittingly fall into a trap set by Al Qaeda and its kind. These organizations aim to destroy the democratic way of life so cherished in the West, and to incite the Muslim populations in democratic countries and their non-Muslim fellow citizens into a vicious circle of mutual hatred and violence. This book therefore addresses not only the question of how intelligence organizations can improve their efficacy in pre-empting terrorist outrages, but also the wider issue of removing the forces that sustain global terrorism as a scourge of the 21st century. The general public in the target countries and recruiting grounds must also be persuaded that—despite their rhetoric—the terrorists are not engaged in a holy war. Ultimately, the brand of global terrorism promoted by Osama bin Laden and his associates is meant to satisfy their own vanity and aspirations toward semi-divine status; the organization they have formed for this purpose is merely a global syndicate that commits serious crimes of a particularly heinous nature. Intelligence services of various countries need to find convincing evidence to prove this point. But it is up to governments, civil society, and the media in different parts of the world to work together if the evidence unearthed by national intelligence services is to be accepted by the general public. Unless the emotional or quasi-religious appeal of the global terrorists can be removed, the simple arrest of bin Laden and his close associates—or even the destruction of Al Qaeda as an organization—will not be sufficient to prevent others from rising to replace them.

While a number of well-developed, blended dissemination and implementation models are being used in social service settings, they all require additional documentation, research, and field experience. Nonetheless, the lessons learned in the social services may help organizations in other sectors better implement health interventions with complex consumers in complex settings.

This chapter outlines the history, underlying principles and policy context for contemporary mental health services for older people. The usual components of such services, including community health teams, memory assessment services, day facilities, consultation-liaison services in general hospitals, and in-patient psychiatric care for older people are all discussed. Alongside these more familiar elements however, there have been other recent, sometimes destabilising, changes, including moves towards age-inclusive services and changes in working patterns for psychiatrists and other professionals. Working closely with primary care, greater integration with social services and providing support to care homes are all part of the current agenda. It is important to measure the quality of services and to ensure the populations who may be disadvantaged receive fair access to effective care and treatment. Obviously, the future increase in the numbers of very old people brings both challenges and opportunities for old age psychiatry services.

Abstract Can women bring a fresh perspective to the provision of global sanitation services? The co-authors of this book, through their strong socio-political and scientific experiences, believe this to be the case. The book offers a critical look at the challenges and solutions needed to achieve Sanitation for All, including for vulnerable people, refugees, asylum seekers, stateless, or internally displaced persons, and especially women. We present sanitation policy and decision making from the perspective of women, providing conclusions to the prevailing debates. In 2022, 43% of the world's population lacked access to safely managed sanitation. Poor sanitation is linked to the transmission of diarrhoeal diseases, exacerbates stunting, and contributes to the spread of antimicrobial resistance. In low-income countries, 5% of deaths are associated with unsafe sanitation. Poor sanitation affects mental well-being and safety, especially for women and children. Lack of sanitation costs around 2% of GDP (a figure that increases rapidly with epidemic outbreaks) because of income losses from trade and tourism and the impact on water quality. Besides being part of our human right to water, sanitation is essential to reach the United Nations' Sustainable Development Goals on poverty, health, education, gender, water, equity, cities and the sustainable environment. Nevertheless, convincing politicians to invest in sanitation is a daunting challenge since, in contrast to drinking water, it is not an immediate vote-winner and is perceived as an unpleasant topic for public discussion. Despite this, to achieve the SDG target by 2030, a five-fold increase on current rates of progress will be required. The role of women in sanitation planning and implementation is key, due to their leadership in communities and their ability to organise and guarantee the sustainability of systems. Furthermore, women's care-giving nature at home and in their communities has led them to understand the importance of sanitation and being open to discussing it publicly; women stand ready to raise the profile of this subject, putting it on a higher level on the political agenda. This book is vital reading not just for women but all stakeholders and partners in the water industry, especially those working in the sanitation and hygiene sectors. In Latin America and the Caribbean, between 50% and 65% of new HIV infections occur in key populations or their clients or sexual partners. Improving the availability and coverage of HIV/STI services for these populations requires the support of monitoring systems that are sustainable and adapt to the needs of the countries of the region. To respond to this need, the Pan American Health Organization, through an agreement with the Global Fund, has developed the Framework for Monitoring HIV/STI Services for Key Populations in Latin America and the Caribbean. It introduces a novel system in which the impact of HIV services on key populations is determined, not only by how HIV-positive people maintain an undetectable viral load but also by how HIV-negative people remain HIV-free. This document lists the essential HIV/STI services that, based on a combination prevention approach, should be offered to people from key populations. The monitoring framework establishes one or more indicators for each of the essential services together with the methodology for their measure. Likewise, a new HIV prevention cascade is introduced, which adds to the existing HIV care cascade. Countries are encouraged to disaggregate by key population group the HIV prevention and care cascades as well as the indicators. Finally, it is urgent to show the contribution of civil society organizations to the response to HIV infection and STIs to ensure their sustainability once external donors leave the region. To this end, the framework encourages breaking down the prevention and care cascades information by the service provider, to identify the contribution of health ministries, civil society organizations, and other actors.

The emergency department provides immediate access to medical care for patients and families in crisis. As the proportion of elderly in our populations increases, complications related to advanced illness will likely drive increasing numbers of patients with palliative care needs into emergency departments. In addition to immediately life-threatening situations, severe physical symptoms, psychological distress, social upheaval, or even an unrecognized spiritual crisis near death can overwhelm patients or their caregivers, who come to the emergency department hoping for relief from their suffering. Emergency clinicians must rapidly assess these complex needs while negotiating interventions with effective and efficient communications. This chapter addresses several of these essential clinician skills for effective primary palliative care in the emergency department. These include rapid assessment of patients with palliative care needs, titration of opioids for pain emergencies, efficiently establishing goals of care, skilful and empathic death disclosure, and a structured approach to involving family presence during cardiopulmonary resuscitation.

Food plays an essential role in performance and well-being. Apart from its physiological necessity, food is also a source of pleasure. Since both biological needs for food and psychic satisfaction from food vary considerably among and within populations, coming up with precise, operationalizable measures of food security have proved problematic. Furthermore, the concept of food security encompasses not only current nutritional status but also vulnerability to future disruptions in one’s access to adequate and appropriate food. The complexity of the concept of food security has given rise to scores, if not hundreds, of different definitions of the term “food security.” As a result, there have also been variations in thinking about the proximate manifestations and direct and indirect causes and consequences of “food insecurity,” the complement to “food security.” Food security is commonly conceptualized as resting on three pillars that are inherently hierarchical: availability, access, and utilization. Some agencies, such as the United Nations Food and Agriculture Organization (FAO), have added a fourth dimension: stability. Food insecurity is often used interchangeably with the terms “hunger,” “undernutrition,” and “malnutrition.” Threats to food insecurity may be classified as either “covariate” or “idiosyncratic.” Based on these threats, various interventions have been implemented to promote food security by means of increasing availability (improving agricultural productivity), promoting access (economic growth and assistance programs such as food stamps or vouchers, food aid delivery, food banks, school lunch programs), or improving utilization (supplementary feeding programs, therapeutic feeding programs).

Abstract Critical Time Intervention (CTI) is a time-limited, evidence-based model of care coordination for people that is delivered during a critical period of transition in their lives. During such periods—which may include the transition from shelters, hospitals, jails, and prisons into the community—people often have difficulty re-establishing themselves with access to needed supports, placing them at risk of ongoing instability and other adverse outcomes. CTI provides direct emotional and practical assistance and strengthens individuals’ ties to their community and support systems during the critical time. Despite its time-limited approach, CTI exerts a long-term impact by effectively building enduring connections to sources of support that remain in place after the intervention ends. The model has been widely implemented in the United States, Europe, and Latin America as many communities struggle to devise effective responses to homelessness and dislocation. This edited volume explains the rationale for CTI, describing its initial development by a creative team of mental health and social service providers working in large homeless shelters in New York City; describes the main components of the model, emphasizing how it differs from standard forms of case management; summarizes research evidence supporting the model’s effectiveness; describes how CTI has been adapted for use with different high-need populations in a variety of settings; and discusses strategies and challenges related to broader implementation of CTI, including workforce training, funding, and quality assurance, and considers the implications of CTI for the design of new “time-sensitive” intervention models in social work and allied fields.

The NHS is traditionally viewed as a typically British institution; a symbol of national identity. It has however always been dependent on a migrant workforce whose role has until recently received little attention from historians. Migrant Architects draws on 45 oral history interviews (40 with South Asian GPs who worked through this period) and extensive archival research to offer a radical reappraisal of how the National Health Service was made. This book is the first history of the first generation of South Asian doctors who became GPs in the National Health Service. Their story is key to understanding the post-war history of British general practice and therefore the development of a British healthcare system where GPs play essential roles in controlling access to hospitals and providing care in community settings. Imperial legacies, professional discrimination and an exodus of British-trained doctors combined to direct a large proportion of migrant doctors towards work as GPs in industrial areas. In some parts of Britain they made up more than half of the GP workforce. This book documents the structural dependency of British general practice on South Asian doctors. It also focuses on the agency of migrant practitioners and their transformative roles in British society and medicine.

India is immersed in several simultaneous battles over the regulation and control of information. While the COVID-19 pandemic has ignited concerns over state-mandated information gathering of the health and personal information of residents, the expanded use of the Aadhaar biometric identity system threatens to make it an essential cipher for every interaction between the state and citizens. At the same time, the earlier momentum towards building strong legislative mandates to disclose public information to promote government accountability and enhance service delivery appears to have stalled. Further, the legislative efforts to regulate both public and private use of personal and non-personal information proceeds at a glacial pace. While these developments occur in different containers and niches of the legal ecosystem, they are grounded in one common conceptual, philosophical and legal puzzle: how should we regulate the access to, and the use of, information by public and private actors? This question becomes all the more salient with the surge in new forms of information collection and processing at a speed and scale made possible by big data collection and algorithmic decision-making technologies. ‘The Philosophy and Law of Information Regulation in India’ project is an effort to collate inter-disciplinary scholarship on the subject of the law and philosophy of information regulation, with a specific focus on India. We recognise that such an effort cannot be bound by legal scholarship alone, and must encompass and contend with the normative assumptions of various approaches towards information technologies.
