Résumé :
Introduction. The most common cause of pathological fractures are skeletal metastases. Ten percent of patients with diagnosed skeletal metastases will sustain a pathological fracture. Skeletal metastases can be treated by non-surgical methods, including analgesics, bisphosphonates, and radiotherapy, with the primary goal of relieving pain and slowing down tumor growth. Surgical treatment is indicated for impending or existing fractures. It includes stabilization with internal fixation using various nails, plates, and screws with or without osteoplasty, and endoprosthetic joint replacement, especially in lesions around major joints - hip, knee, and shoulder. Material and Methods. The study included patients operatively treated at the Institute of orthopedics "Banjica" and pathohistologically analyzed at the Institute of pathology in Belgrade during the period from February 2021 to January 2022. Inclusion criteria were an existing or impending pathological fracture of long bones, operative treatment with tissue sampling, and the consequent pathohistological diagnosis of metastatic carcinoma. Patients with biopsy-proven processes other than metastatic carcinomas were excluded from the study. The total number of patients included in the study was 69. Results. The mean age of patients at the pathological fracture occurrence was 67.7 (ranging from 42 to 88). Malignant diseases diagnosed were: breast cancer 36.1%, lung cancer 24.5%, kidney cancer 14.5%, prostate cancer 13.1%, colorectal cancer 2.9%, other cancer (8.9%). The radiological presentation was in the form of lysis in 75.4% and in blastic form in 24.6%. Operative treatment included arthroplasty in 53.6% of patients and stabilization with nail or plate in 46.4%. Conclusion. Pathological fractures represent the final outcome of tumor activity in a bone and cause significant suffering in patients expressed through severe pain and often immobility, which accelerates all the pathological processes and leads to death. Joint methods of contemporary chemotherapy, radiotherapy, and surgery enabled a significant life quality improvement and extension in these patients.

Background: Minor salivary glands are found throughout the entire upper aerodigestive tract. Minor salivary gland neoplasms represent less than 25% of intraoral salivary neoplasms. There are no uniform guidelines at present for treatment for minor salivary gland tumors as these are rare entities. The objective of the study was to evaluate the clinicopathological profile of minor salivary gland tumors and to evaluate follow up of patients after treatment. Methods: The present study was a prospective observational study conducted on 11 patients of the Pacific Medical College and Hospital, Udaipur from 1 December 2020 to 31 December 2023 with complaints of intra-oral swelling with biopsy proven MSGT were included. Results: Out of 11 patients, 7 were female and 4 were male. Highest incidence was found in patients in the 4th-5th decade of life. For most patients, painless swelling was the primary presentation. The most common site for minor salivary cancer was the hard palate 55%. The most common histological types were adenocystic carcinoma. In 10 cases complete surgical excision with clear margins was the preferred mode of treatment. In 1 palliative therapy was required. Among 11 patients, 2 required radiotherapy. Conclusions: MSGT are rare, early diagnosed and resection with wide margins/neck dissections + SOS adjuvant therapy is key to treatment.

Résumé :
Abstract Introduction. Many situations in pregnancy create a thrombophilia, which is believed to have a protective function, which is the result of the hormone influence, primarily by gestagenic hormones. This is a physiological condition and it helps in successful implantation of the embryo and prevention of bleeding. The presence of additional, hereditary or acquired cause of thrombophilia disrupts the natural balance and creates conditions for venous thromboembolism and consequences for pregnancy, such as miscarriage, intrauterine fetal death, delayed intrauterine growth, eclampsia and abruption of the placenta. Methods. The study was designed as a retrospective epidemiological cross-sectional study and was conducted at the Institute of Transfusion Medicine in Skopje in the period from June to October 2016. The study included patients with current pathological pregnancies. The following laboratory analyses were conducted in all patients: hemostasis, D-dimer, antithrombin III, protein C, protein S, antiphospholipid antibodies, lupus anticoagulant, MTHFR, factor V Leiden and prothrombin mutation. Results. The study included 41 patients, of whom 3 were excluded due to an insufficient medical history, 3 who were receiving corticosteroid therapy, 2 due to incomplete laboratory analyses and one patient with proven systemic lupus. Data analysis showed that family history of thrombophilia was present in 25%. The most common MTHFR mutation was recorded in 26 (82%) patients, followed by the factor V mutation present in 18 (55%) of the examined patients. AT III deficiency was detected in only one patient (2%). In 12 patients (35%) a disturbed ratio of DD vs. markedly shortened APTT or inappropriately low levels of DD was registered, which is an indirect indicatorof hypofibrinolysis. Conclusion. Pathologic pregnancy is a condition that is often associated with thrombophilia. Advanced age of a patient, family and personal history of thrombotic conditions indicate a high risk of complications in pregnancy. Due to the current increased frequency of high risk pregnancies associated with circulation problems with the placenta, there is a need for more frequent hemostatic examinations in pregnancy.

Résumé :
Abstract Objective Malignant peripheral nerve sheath tumor (MPNST) arises from nerve sheaths, mostly seen in peripheral nerves but rare in craniospinal nerves. The information available in the literature to build up treatment strategy and improve clinical outcomes is scarce. We are reviewing cases from our institute, with emphasis on radiological features for early differentiation from its benign variants. Methods We analyzed pathologically diagnosed cases retrospectively from January 2007 to December 2018 at our institute. Clinicoradiological details and treatment parameters were collected from medical records for evaluation. Each case was contacted telephonically for final clinical follow-up at the time of writing the manuscript. Results A total of seven cases of MPNST were diagnosed in the last 10 years. It included four intracranial and three spinal cases. The mean age for the cohort was 34.3 years, with five females. We could achieve gross total resection (GTR) and subtotal resection in four (57.1%) and two (28.6%) cases, respectively. We could achieve an overall survival of 57.1% in the average follow-up of 28.2 months (range: 8–84 months). Conclusion MPNST is a rare tumor with a bad prognosis. Radical surgical resection is the mainstay of the treatment, but it is not always possible to achieve it because of the inaccessible location and large size of lesions. Preoperative diagnosis is challenging; however, few radiological findings may give a clue toward it. As a disease entity overall, it has a poor outcome with a high rate of fatality.

Introduction: To check the impact of the clinical and pathologic features at diagnosis and treatment given, on the outcome of synovial sarcoma in children and adolescents. Methods: Retrospective analysis of patients below 18 years old diagnosed by synovial sarcoma and treated at Children CancerHospital Egypt 57,357 (CCHE) between July 2007 and December 2016. We reviewed Clinical characteristics, pathological information, treatment modalities and survival data. Results: We included thirty one patients with median age at diagnosis was 14.8 years. Initial surgical excision was possible in 10 patients (58.8%) while 5 (29.4%) patients underwent surgical excision after response to preoperative chemotherapy. Two patients had unresectable tumor, showed no response to chemotherapy and received radiotherapy as the only local control therapy. Adjuvant radiotherapy only was given in 2 patients and 5 patients received chemotherapy without local radiotherapy and 10 patients received both modalities. The estimated 5-year overall survival and failure free survival rates for the entire group were 84.4% ± 7.2% and 63.8% ± 9.7% respectively, the 5-year OS and FFS were studied in correlation to age of patient, tumor size, different pathological types, site of the lesion and methods of local control. In patients with tumor size below 5 cm when compared to size more than 5 cm with OS was (100%) in comparison to (79.3 ± 9.3) who underwent complete surgical excision either initial or post chemotherapy as it was 66.7% versus 55.6% for those with gross or microscopic residual (p-value = .38). Also, the 3-year failure free survival was 75% versus 56.3% for those smaller than or equal 5 cm and those larger than 5 cmrespectively (p-value = .3).Conclusion: Increasing number of patients to increase sample size to assess tumor size and complete surgical excision as important prognostic factors as in our study, we found them statistically insignificant. Preoperative chemotherapy can help for delayed excision in patients presented initially with unresectable tumors

The article presents the history of the development of school medicine in Ukraine, the birth of national school hygiene as an independent science. It was emphasized that school medical services are very important both in maintaining the health of the younger generation and in providing the best conditions for its formation, health promotion, and prevention of pathological conditions. In 1922–1935, institutes of health care for children and adolescents and institutes for the protection of motherhood and childhood were opened in Ukraine for the scientific development of health care issues for schoolchildren and adolescents. In October 1922, the first Ukrainian State Institute for Maternal and Child Health was opened in Kharkiv, which is now called SI «Institute of Children and Adolescents Health Care of the National Academy of Medical Sciences of Ukraine». The Institute has become a leading research institution in Ukraine, which coordinates research on «Health Care of schoolchildren and adolescents», trains highly qualified personnel, publishes methodological and informational materials for implementation in practice, conducts scientific and practical conferences, and scientific symposiums on topical issues, including school medicine. According to the Resolution of the Cabinet of Ministers of Ukraine № 31 of January 20, 2021 «On approval of the Procedure for providing medical care to students of general secondary education» medical care for students is now provided by a medical worker who is a member of the educational institution. Ways to improve the organization of medical care for students in secondary schools for more effective development of school medicine, the expediency of using the resources of the SI «Institute of Children and Adolescents Health Care of the National Academy of Medical Sciences of Ukraine» are presented. Therefore, only the combined work of educational and health care institutions can provide the most favorable conditions for maintaining the health of students.

Résumé :
Abstract Background Hemangiopericytoma (HPC) is an extremely rare aggressive tumor of mesenchymal origin, which constitutes less than 1% of primary tumors of the central nervous system. Typically it occurs in young adults (mean age at diagnosis 30-50 years). Initially described as gemmangiomas, was first called hemangiopericytoma in 1942 but it was considered as a variant of meningioma until 1993 when it was finally recognized as a distinct clinical-pathological entity by the World Health Organization (WHO). In 2016, there was the unification of both HPC and Solitary Fibrous Tumor (SFT) in a single spectrum of rare and thick mesenchymal tumors due to the identification, in both tumoral types, of a molecular alteration called NAB2-STAT6 fusion. Material and Methods The present study is a case series of patients that received a pathological diagnosis of SFT/HPC at our Institute from 1981 to 2020. Clinical data included: demographic data, tumor characteristics and treatment modality. Hystological diagnosis and immunohistochemistry allowed to confirm diagnosis, based on the 2016 WHO classification. Haematoxylin staining was completed with immunohistochemistry with primary antibodies for STAT 6 and evaluation of presence of tumor necrosis. Statistical analysis was conducted considering age at first diagnosis, treatment modality, extent of tumor resection, presence of necrosis in histological analysis compared to PFS-1, OS and relapses number. Results The case series was composed by 82 patients (46 men and 36 women) with median age of 43 (± 20) that accede to our Institute from 1981 to 2020. Our statistical analysis showed significative correlation between age, extend of tumor resection and presence of necrosis with OS and relapses number. Conclusion Hemangiopericytoma is a very rare cerebral tumors, and for this reason large case series are not disposable in literature. In our study we collected all disposable data of every patient with pathological diagnosis of SFT/HPC acceded to our hospital in the long period of 39 years in the aim to verify the presence of significative correlations. We observed strong correlation between tumor resection and presence of necrosis in tumor tissue with prognosis parameters (OS and number of relapses). This result it could be important to conduct the decisional process about treatment after surgical resection, considering also pathological features, rather than only surgical resection.

Résumé :
274 Background: Primary systemic chemotherapy is a standard approach to treating women with locally advanced breast cancers, with higher survival rates reported among patients who attain a pathologic complete response. Triple-negative breast cancer is a special biological entity that remains major challenge to oncologist. Around 12%-20% of breast cancers are triple negative. The current phase II study was conducted to evaluate the pathological complete response (pCR) using cis-platinum containing regimen as neoadjuvant chemotherapy in locally advanced triple negative breast cancer. Methods: Eighteen women with stage III triple negative breast cancer who were recruited between July 2007 and February 2010 at King Fahad Specialist Hospital, Dammam, Saudi Arabia. Neoadjuvant chemotherapy consisted of 4 cycles of AC or FEC 100, followed by 4 cycles consisted of docetaxel-cisplatin every 3 weeks. Primary end point was pathological complete response. Results: Median age: 49 y (24-70); premenopausal: 16; 25% were below 35 years of age; Median tumor size: 9 cm (3.5-19); Grade III: 15; Stage IIIA: 3, IIIB:14, IIIC:1; all but 2 had positive nodes at diagnosis (89%). Clinical evaluation of response by RECIST criteria pre surgery: OR: 17/18 (94%), CR: 9 (50%); PR: 8 (44%).The second sequence with D-Cis-T doubled the rate of clinical CR obtained with AC/FEC. One patient was not operated due to disease progression. Pathological assessment, revealed that 8 (47%) pts had no residual invasive carcinoma in the breast; 3 (18%) had residual occasional scattered tumor cells less than 5 mm (pT1a); 10 (59%) had negative nodes; 8 achieved CpR and 2 nCpR. Patients with residual invasive component and/or nodal involvement had high baseline Ki 67 level. After a median follow up of 24 months, cumulative overall survival at 24 months is 88.9% for whole group. Cumulative overall survival in relation to response was 100% for patients who achieved pCR while overall cumulative survival rate for patients without pCR was 83.3% without statistical significance. Conclusions: This cisplatin based neoadjuvant chemotherapy regimen was well tolerated and achieved a high rate of pCR/npCR.

Résumé :
Objective: Segmental demyelination is the pathological hallmark of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), but other elementary lesions are frequently observed, configuring a series of different pathological pictures. In this article, we review the pathological findings of a large series of sural nerve biopsies from our cohort of CIDP patients. Patients and Methods: Patients with CIDP who underwent nerve biopsy were retrospectively selected from those referred to the Institute of Neurology of the “Università Cattolica del Sacro Cuore” in Rome, Italy, from 1982 to February 2020. Sural nerve biopsy was performed according to standard protocols. Results: Sural nerve biopsy was performed in 43/130 CIDP patients. Demyelinating abnormalities and axonal loss were found in 67.4% and 83.7% of biopsies, respectively. Conversely, onion bulbs and inflammatory infiltrates were rare (18.6% and 4.7%, respectively). In three cases, we observed normal pathological findings. Conclusions: A pathognomonic pathological finding of CIDP cannot be established, but we confirm the utility of nerve biopsy in this setting to confirm the diagnosis (also in atypical phenotypes) and to elucidate pathogenic mechanisms.

Résumé :
529 Background: Pathologic response of HCRM to neoadjuvant tx is associated to better outcome. In retrospective analyses Bev-based tx improved tumor regression grade (TRG), while cetuximab (Cmab) increased RECIST response and R0 resections in prospective studies. The aim of this analysis was to assess the effect of Bev vs Cmab-based tx in terms of HCRM pathological response and outcome. Methods: Fifty-eight pts with HCRM were resected at National cancer Institute of Milan from 2007 to 2012 following Bev (n=30) or Cmab (n=28) based neoadjuvant tx. Pathological response was classified from TRG 1 for pathological complete response (pCR), to TRG5 for no response; TRG 1-3, for histological response; TRG 4-5 for scarce/no response (Rubbia-Brandt L, Ann Oncol 2007). Results: KRAS missing in 9 pts (3 Bev/6 Cmab), mutated in 18 pts (60%) in Bev vs 4 (14%) in Cmab group.Median number of resected metastases: 4.6 vs 3.9; mean TRG: 2.9 (2.2-4) in Bev vs 3.8 (3-5) in Cmab group (p=0.04 at Kruskal-Wallis test). Pathological response rate was higher for Bev vs Cmab (70% vs 39%; p=0.037 at Chi-square test). Bev also increased pCRs (20% vs 0%, p=0.024 at Fisher’s test). With generalized estimating equation for all resected nodules, mean TRG was lower in Bev vs Cmab group: 2.6 (2-4) vs 3.7 (3-4), p<0.001. At univariate analysis, no PFS and OS difference was observed according to tx. Multivariable Cox models shown below: Conclusions: Bev-based tx obtained an excellent pathological response rate, despite the high proportion of KRAS mutated tumors, and further prospective confirmation of these data is planned. Résumé :
Every year, strokes are diagnosed in about 16 million people, of whom 5.7 million die and about the same number become disabled (Feigin V.L., Forouzanfar M.H., Krishnamurthi R., et al., 2014; Virani S.S., Alonso A., Benjamin E.J., et al., 2020; Kim J., Thayabaranathan T., Donnan G.A., et al. 2020). The objective of our research was to study the attitudes towards disease in patients with chronic phase of ischemic stroke. Material and methods. 100 consecutive patients with chronic phase of ischemic stroke and 17 patients with chronic cerebral ischemia were questioned in neurology in-patient department. The types of attitude towards disease were identified with the help of the Personal Questionnaire of the Bekhterev Institute. Results. We found that pathological types of attitude towards disease begin to form before the development of a stroke as a result of chronic cerebral ischemia. Among patients with chronic phase of ischemic stroke, pathological types of attitude towards disease prevailed significantly, and the harmonious type was rare (84.0% vs 16.0%, p < 0.001). The most frequently observed were neurasthenic (36.0%), harmonious (16.0%), anxious (16.0%), ergopathic (10.0%) and sensitive (8.0%) types of attitude towards disease. Our study has shown that any ischemic stroke, even in the case of complete recovery of the lost functions, can be associated with pathological types of attitude towards disease. Keywords: ischemic stroke, the Personal Questionnaire of the Bekhterev Institute, attitudes towards disease.

Résumé :
Introduction: Bone tumors are rare neoplasms whose therapy depends on the location. Pelvic bones represent a special anatomical entity diagnosis and therapy of pelvic tumors in addition to the pathohistological type largely depends on the anatomical specifics of that region. Aim: The aim of this study was to show the frequency, as well as clinical and pathological characteristics of primary and secondary tumors of the pelvic bones. Material and methods: The research as a descriptive study in the period from January 1, to December 31, 2019 at the Institute of Pathology of the Medical Faculty of the University of Belgrade and includes 33 patients. Demographic, clinical and radiological data were obtained from the Bone Tumor Registry. Statistical processing and analysis were done in the Statistical Package for Social Science SPSS Windows version 22 (IBM Chicago, Ilinois, USA). Results: Out of 33 patients, 55% had secondary pelvic tumors, primarily cancer metastases (37%). In 21% of cases, primary malignant bone tumor as diagnosed, and in 12%, primary benign bone tumor. Hematopoietic neoplasms were verified in 12%. Conclusion: Pelvic bone tumors are not common, but these neoplasms, in orthopedic oncology, represents their malignant biological behavior are of great importance. The most common primary malignant tumor in adults is chondrosarcoma, and in children Ewing sarcoma. Highest frequency for pelvic bone metastases has lung cancer. Plasmacytoma in adults, Langerhans cellular histiocytosis in children are hematological neoplasms seen at this site.

Résumé :
Problem:Alzheimer disease (AD) is defined as “an irreversible, progressive brain disorder that slowly destroys memory and thinking skills” (National Institute on Aging) and is pathologically characterized by abnormal deposition of neurofibrillary tangles and amyloid plaques. However, these abnormal protein aggregates also accumulate with aging, which complicates the distinction between aging and AD.Results:This presentation will discuss the concepts of disease and then compare and contrast these with the definition of Alzheimer disease. It briefly discusses causality and examines how associations have to be conflated with causality in the pathological diagnoses of neurodegenerative diseases. It also indicates some inherent biases that pathologists have in identifying disease and the pathological changes resulting from diseases. The presentation will present examples from the Calgary Brain Bank of patients without known neurodegenerative disease who die at different ages, as well as different pathological presentations of neurofibrillary tangles and amyloid plaques. Several known causes of AD will be reviewed and contrasted with what is commonly considered “normal aging”.Discussion:This presentation argues that Alzheimer disease pathology represents a final common pattern of changes that results from several or possibly many different aetiologies. Recognizing that these changes have several different causes might better guide future research into late onset dementias.LEARNING OBJECTIVESThis presentation will enable the learner to:1.Consider observational biases used in the diagnoses of different dementias2.Distinguish several aetiologies of Alzheimer-type Neuropathology3.Contemplate how neuropathology has done a disservice in dementia research by focusing on accumulations of abnormal proteins

Purpose. Pathological Myopie (PM) causes structural chan- ges at the posterior segment of the eye that may lead to pathological changes including severe vision loss. The aim of this paper is to describe the pathologies most commonly associated with PM. Material and Methods. The literature review includes the current state of knowledge regarding the structural changes of the posterior segment due to PM, based on theoretical and methodological literature. The selected literature includes nine studies published in 2020 – 2021 as well as reports and assessments of public institutions. Publications by the World Health Organization (WHO), the World Council of Optometry (WCO), the International Myopia Institute (IMI) and the Brien Holden Vision Institute (BHVI) as well as cohort studies with children and adults from China, Singapore, Korea, Taiwan, Japan and Germany were taken into account. Results. Four eye diseases and their relationship to PM are explained and their pathological consequences described. Current therapies to improve the visual acuity in pathologies in the macula are presented. Conclusion. PM often causes structural changes in the macu- lar area. This leads to a significant loss of vision in those affec- ted. Without treatment, PM can lead to blindness. In view of the increase in myopia and the number of myopes worldwide, it is important to sensitize the population to this topic. Résumé :
Speech is the method of generating specific sounds that convey meaning to the listener. This ability of a person could be hampered due to several reasons leading to speech disorders. A speech disorder refers to any circumstance that affects a person’s capability to create sounds that produce words. According to a survey conducted by All India Institute of Speech and Hearing, Manasgangotri, Mysore, speech impairment is listed as the fifth-highest occurring disability in India. There are different types of speech disorders such as Stuttering, Apraxia, Dysarthria, Dysphonia, that affect a person’s capability to form sounds that permit him/her to converse with other people. Recently computer-based detection of speech disorders has got attention from researchers and doctors as it can be used to detect speech disorders effectively. A similar approach has been proposed in this paper to identify pathological disorders using Support Vector Machines and Gaussian Mixture Model. It can be beneficial to people in rural areas as there is a scarcity of well-trained speech pathologists. Also, equipments needed to identify speech disorders are expensive. Hence there is a need for an automated system to assist a pathologist in detecting speech disorders. In this work, speech dataset from Saarbruecken, related to the above-mentioned diseases was collected and were classified as healthy and unhealthy. An accuracy of 96% was attained using Gaussian Mixture Model.

Of 129 esophagectomies at our institute from June 2010 to March 2015, we experienced three preoperative positron emission tomography-computed tomographic (PET/CT) false positives. Bone metastasis was originally suspected in 2 cases, but they were later found to be bone metastasis negative after a preoperative bone biopsy and clinical course observation. The other cases suspected of mediastinal lymph node metastasis were diagnosed as inflammatory lymphadenopathy by a pathological examination of the removed lymph nodes. Conducting a PET/CT is useful when diagnosing esophageal cancer metastasis, but we need to be aware of the possibility of false positives. Therapeutic decisions should be made based on appropriate and accurate diagnoses, with pathological diagnosis actively introduced if necessary.

Résumé :
Aim: Supratentorial ependymoma (STE) is a rare tumor with distinct genetic alterations, whose imaging features have been scarcely studied. This study aims to review the computed tomography (CT) and magnetic resonance imaging (MRI) features of a cohort of histopathologically proven STE to identify the distinguishing features of STE, and look for specific signs of zinc finger translocation associated (ZFTA) fused STEs. Methods: Ethical clearance was obtained from the institutional ethics committee. The magnetic resonance (MR) images, CT images when available, clinical details, and pathological reports of 25 patients from a single institute with histopathologically proven STE were retrospectively reviewed. Imaging features, demographic details, pathological and molecular features, and type of surgical resection were described and tabulated. Relevant associations with imaging features were computed and tabulated. Results: The study showed that STEs are common in the pediatric population with no sex predilection. The periventricular location was the most common. A significant association between periventricular location and the presence of a cystic component (P value = 0.023) and the presence of the periwinkle sign/stellate sign (P value = 0.045) was found. Common features of ZFTA fused STEs included periventricular or intraventricular location, cystic component, necrosis, and the periwinkle sign. A significant association was found between ZFTA fusion and cystic component (P value = 0.048). Conclusions: This study attempts to identify the imaging features of STEs and their associations with molecular pathology and surgical outcome, and the distinguishing features of ZFTA fused STEs.

Résumé :
Abstract Objectives: To review failure patterns in oral cavity squamous cell carcinoma treated with surgery and adjuvant radiation therapy. Materials and Methods: All patients with biopsy-proven stage I-IVB oral cavity squamous cell carcinoma (OCSCC) treated with surgery and adjuvant therapy between 2018 and 2022 were evaluated. Outcome measures were 3-years loco-regional recurrence (LRR), overall survival (OS), and progression free survival (PFS). LRR was spatially localized in relation to contour and dose distribution. Results: A total of 85 patients treated between years 2018 to 2022 were evaluated with median follow-up of 19 months. Disease subsites were oral tongue (37.6%) and bucco-alveolar complex (62.4%). Of a total of 25.9% failures, 16/85 (18.8%) were local, 9/85 (10.6%) were regional, 18/85 (21.2%) were locoregional, and 10/85 (11.8%) were distant. For LRR, total in-field failure was 11.8%, marginal failure was 4.7%, and out-of-field failure was seen in 2.4% cases. The 3-year actuarial local, regional control, and LRR in our study were 69%, 84%, and 67%, respectively. The cumulative rates of OS were 72% and PFS was 56%. On univariate analysis, independent factors for local failure were perineural invasion (PNI) (OR = 5.89, P = 0.03), lymph vascular space invasion (LVI) (OR = 3.69, P = 0.05), and pathological N3 (OR = 3.69, P = 0.02). For regional, failure was PNI (OR = 4.53, P = 0.038). For locoregional, failure was PNI (OR = 6.13, P = 0.002) and for distant, failure were PNI (OR = 5.80, P = 0.013), pathological N3 (OR = 4.35, P = 0.03) and LVI (OR = 4.66, P = 0.03). On multivariate Cox proportional hazard analysis, for local, failure risk factors were PNI (HR = 4.32, P = 0.01) and pathological N3 (HR = 3.27, P = 0.047), for locoregional, failure was PNI (HR = 4.42, P = 0.006), and for distant, failure was PNI(HR0 = 4.29, P = 0.05). Conclusions: In our cohort of patients, the most common failure was in-field LRR. PNI was significantly associated with local, locoregional, and distant failure on Cox analysis. In addition to PNI, pathological N3 was the cause of local failure.

Résumé :
This clinical case describes a rare combination of early stage cancer and sporadic neuroendocrine tumor in stomach. The patient 57 years old admitted to P. A. Hertsen Moscow Oncology Research Institute for diagnostics and treatment for the gastric adenoma. Upper GI endoscopy performed in P. A. Hertsen Institute, detected two tumors: a sporadic neuroendocrine tumor in the middle part of the body stomach and early cancer in the antrum. With NBI (Narrow-Band Imaging), signs of malignancy were found in both tumors. А plan for therapeutic and diagnostic endoscopic surgical interventions was implemented: two sequential endoscopic submucosal dissection (ESD). This treatment confirmed as radical, according to the results of the pathological examination.

Résumé :
Imaging diagnostic methods in pulmonology originate from the discovery of X-rays. Radiography is the first-line tool for chest imaging, but in many cases its capabilities are limited. The aim was to study the history of the invention of computed tomography. Since the radiogram is a two-dimensional summation image of a three-dimensional anatomical structure, there was a need to visualize separate layers of the pathological process. Since 1913, many technical solutions of tomography have been tested. Linear tomography for some time served for the diagnosis of lung pathology, helped to determine the localization more accurately, examine a prevalence, nature and structure of the pathological process, to detect small pathological formations and cavities. But the quality of tomography was not satisfactory. The two outstanding inventors of computed tomography who were awarded the 1979 Nobel Prize in Medicine were not the physicians. Allan McLeod Cormack (Professor and Head of the Institute of Physics at Tufts University, Medford, Massachusetts, USA) was the first to outline the principles of cross-sectional tissue reconstruction in an organ based on X-ray projections in 1963. Godfrey Newbold Hounsfield (Head of Medical Research at Electric and Musical Industries, Middlesex, England) not only reproduced the mathematical algorithms of the technique, but also designed the tomograph and directly participated in its clinical implementation in 1971. Among the developers of the idea of three-dimensional X-ray tomography in 19571958 were the scientists of Kyiv Polytechnic Institute Semyon Tetelbaum and Boris Korenblyum. Key words: radiology, computed tomography, history of pulmonology.

Résumé :
Proximal femoral fractures, including intertrochanetric and subtrochanteric with diaphiseal propagation represent a significant challenge in orthopedic surgery, especially in older population. Retrospective analysis of patients, after the application of 'Bi Nail (Bioimpianti Inc.) intramedullary nail for fractures and pathological conditions of proximal femur, was done at the Department of Adult Orthopedics, Institute of Orthopedic surgery Banjica, during the three year period. We present patients with complex fractures and pathological conditions (metastasis and pseudoarthrosis) of proximal femur; most of the fractures were result of effects of low energy, and the most common pathological fracture is due to metastasis of breast cancer. The analysis included the technical characteristics and duration of surgery, intraoperative and postoperative complications as well as the coalescence time of fracture healing and postoperative quality of life after rehabilitation. We believe that the described surgical method, although technically demanding, with a variable length duration of surgery and treatment of high risk elderly patients, provides stable fracture fixation with early rehabilitation to improve the quality of life.
