Pathological gambling accumulates frequently in connection with other diseases. This is documented both by studies and clinical observations, whereby no causalities can be derived, which underlying disease led to further diseases. Nevertheless different studies show similar results that pathological gambling arises increased in connection with substance dependence, depressive disorders, obsessive compulsive disorders and also personality disorders. These results stated in the literature also reflect the clinical observations of the Anton Proksch Institute.In the following presentation we will try to figure out the results of inpatients with the diagnosis pathological gambling, that were treated in the Anton Proksch Institute in the years 2008, 2009 and 2010. We find a range of comorbititys longing from substance abuse to personality disorders.

Knowledge of the morphological structure of atypicalforms of lichen planus is necessaryfor the correct diagnosis of lichen planus. Due to the large number of atypicalforms, diagnosis often requires a histological examination of the affected skin. The article describes pathological features of typical and some atypical forms of lichen planus, observed in recent years in the department of dermatology of Moscow Regional Research and Clinical Institute.

The article presents historical information about the development of pediatrics and pediatric education in Primorsky Region. It started from the establishment of the pediatric faculty of the Vladivostok Medical Institute in 1969 to the present day. Main directions of scientific and pedagogical activities of pediatric profile departments are highlighted. The departments were reorganized in 2016 into the Institute of Pediatrics; departments of normal and pathological physiology, pathological anatomy, and foreign languages. The article also indicates those people who were at the origins of pediatrics and pediatric education in Primorye.

According to the June 18, 1863 University Charter, the fourth department of pathological anatomy in Russia (after Moscow, St.Petersburg and Derpt) was created at the Kazan University Medical Faculty. On October 21, 1863 A.V. Petrov (1837-1885) was appointed the first senior lecturer of the Department of Pathiatomy, who left his mark not only as an organizer of educational process, but also as a scientist and public figure. His studies on typhoid fever and uremia are classic. His experimental work, which showed the contagiousness of tuberculosis, anticipated the discovery of Mycobacterium tuberculosis by R. Koch. He gave much strength and energy to the Kazan Society of Physicians, of which he was the chairman for many years.

Introduction: Benign breast lesions are more common than malignant tumours. Hence recognizingthem is important from the viewpoints of therapy and prognosis. Methodology: The materials forthe study comprised of resected specimens of breast lesions received in the pathology departmentKarnataka Institute of Medical Sciences, Hubballi from May 2010 to April 2015. Results: A total of600 cases of benign breast lesions were recorded. Fibroadenoma was the commonest benign breastlesions comprising 70.82%, followed by fibrocystic disease 12%, benign phyllodes tumour 8.3%.The majority of benign breast lesions occurred in the 2nd decade and 3rd decade. Conclusion:Benign breast lesions were common. Conclusion: The most frequently encountered breast lesionswere fibroadenomas. So recognizing them is important for therapy and prognosis.

Abstract Out of the enormous amount of material generated by the research on the higher nervous activity in dogs using the method of conditional reflexes, I will discuss three points today, specifically related to pathological disorders of this activity. They are: the strength of the two basic nervous processes of excitation and inhibition, then the correlation of their strengths—their balance—and finally their mobility. These points, on the one hand, form the basis of the typology of higher nervous activity—and types, notably, play a big role in the genesis of nervous and so-called psychiatric diseases; on the other hand, they make it possible to capture the characteristic changes that occur in the pathological states of this activity.

The chapter examines common definitions (vascular cognitive impairment, vascular dementia, post-stroke cognitive impairment), the DSM-5 criteria (Diagnostic and Statistical Manual of Mental Disorders-5), ICD-10 criteria (International Classification of Diseases), NINDS-AIREN criteria (National Institute of Neurological Disorders and Stroke and the Association Internationale pour la Recherche et l’Enseignement en Neurosciences) for vascular dementia, and vascular mild cognitive impairment. The VASCOG (vascular cognitive disorder) criteria are also described. A pragmatic approach to investigations and various assessment scales, a description of important clinical trials, and the management of these disorders are also included.

Although the National Institute of Mental Health characterizes its Research Domain Criteria program as a “paradigm shift,” its change in emphasis from discrete mental disorders to psychophysiological constructs does not depart dramatically from prior research strategies. The RDoC program supports psychopathology research that utilizes newer, biologically-based investigational methods; avoids pre-existing conceptions of mental disorders; and provides insight into the full range of psychological functioning, from normal to pathological. However, it maintains the methodological rules of the existing psychopathological paradigm. This chapter traces the conceptual history of construct validity to show its consistency across disorder-based and construct-based approaches to the study of psychopathology. Because the RDoC research program is grounded in construct validity and aims to develop existing theory, it does not constitute a paradigm shift in the Kuhnian sense, but simply rejects mental disorders as scientifically legitimate constructs.

This chapter recounts the first discovery of a retrovirus. This was made as long ago as 1910 by Peyton Rous, who worked at the Rockefeller Institute for Medical Research in New York. This agent, avian sarcoma virus, induced tumours in muscle, bone, and other tissues of chickens. The chapter highlights the discovery of other retroviruses which caused tumours in the 1930s. However, these viruses were regarded as laboratory curiosities until the description of feline leukaemia virus, a gammaretrovirus, which was first discovered in the 1960s and appeared to spread naturally in household cats. The chapter mentions the Visna Virus of sheep, discovered in 1954. This was shown to persistently infect sheep and could lead to ovine progressive pneumonia. The discovery if HIV-1, which causes AIDS, led to a surge of interest in this previously understudied family of viruses, and the chapter examines the properties as well as the clinical and pathological aspects of HIV in detail.

Although neurosurgery has a long history it was thanks to brave pioneering neurosurgeons such as Harvey Cushing in the United States — and in Sweden Herbert Olivecrona — that the speciality made huge progress during the first half of the 20th century. However, around 1950, the possibility to reveal pathological processes taking place inside the closed skull, was still very restricted. The only available rapid procedure was the neurological examination of the patient. X-ray of the skull is of restricted value, pneumo-encephalography was much too complicated and time-consuming for acute situations, and angiography was still in its infancy. Thus the neurosurgeon, receiving an acute patient with a suspected intracranial haematoma, had to make a qualified guess about where to start placing his trephine on the skull in order to save the life of the patient — often within minutes in the case of a bleeding between the skull bone and the dura. The mortality in those days was 40 % , often because the diagnosis was made too late. Thanks to an ingenious Swedish neurosurgeon, Lars Leksell, working at Lund University Hospital, a new approach to reveal the secrets inside the skull was introduced in clinical praxis — echoencephalography. Lars Leksell graduated from the Karolinska Institute (KI) and received his neurosurgical training in Herbert Olivecrona’s department from 1935. He volunteered as a neurosurgeon in the Finnish winter war in 1940 in Karelia. His team could operate on 24 head injuries per 24 hours and already by this point, Leksell showed his creative mind in constructing the double-action rongeur for more efficient removal of shell-splinters from the vicinity of the spinal cord. In 1941 he joined Professor Ragnar Granit (1967 Nobel laureate in Medicine) at the Institute of Neurophysiology where he presented his thesis on gamma nerve fibres in 1945. In 1946 he became the chief of the new neurosurgical unit in Lund and in 1958 he was appointed the first Professor of Neurosurgery at Lund University. In 1960 he succeeded Olivecrona as Professor and Chairman of the Neurosurgical Department at the Karolinska Institute/Hospital.

This paper describes three different versions of left heart simulators that have been developed at the Cardiovascular Fluid Mechanics Laboratory at Georgia Institute of Technology, specifically designed to provide high fidelity experimental datasets necessary for rigorous validation of computational tools. These systems are capable of simulating physiological and pathological flow, pressure and geometric conditions, and can be investigated using a variety of experimental tools to measure relevant biomechanical quantities. The development of such robust simulators is a critical step in ensuring applicability of patient specific computational tools.

Résumé :
Introduction: Uterine sarcoma accounts for nearly 3% of all uterine malignancies. They have 4 major pathology includes endometrial stromal sarcoma high grade, ESS low grade, uterine leiomyosarcoma (uLMS) and undifferentiated uterine sarcoma (UUS). Recent WHO classification 2014, recognizes low grade ESS and high grade ESS as distinct entity. They differ from endometrial carcinoma in their aggressive nature and poor prognosis. We review our database and found total 44 eligible patient treated at our institute. Materials and Methods: Its retrospective analysis of computer based database of our institute from January 2009 to December 2015. We analyzed demographic, pathological, treatment and survival data. Results: Total 44 patient treated for uterine sarcoma at our institute. Among these 16 were operated at our institute during study period. Here we reporting results of operated patients at our institute. The histological diagnosis LMS (5/16), ESS-L (4/16), MMMT (3/16), UUS (3/16) and ESS-H (1/16). Stage distribution was stage I, (6/16) stage II, (5/16) stage III, (3/16) stage IV, (0/16) and unknown stage (2/16). Two patients underwent completion surgery for outside myomectomy. The adjuvant treatment was CT in 3/16, CT with RT in 7/16, HT in 4/16 and one lost to follow up with one was put on observation. Median follow up is 30 month with 14 patients alive and one lost to follow up. At last follow up 4 patients alive with metastatic disease and 10 patients alive with no evidence of disease. Neoadjuvant treatment in breast cancer has some advantages, in that the treatment of micrometastatic disease has increased because of the higher degree of performing conservative surgery and reducing tumor burden. The complete pathological response (CPR) is defined as the absence of invasive carcinoma in the anatomopathological study of breast tissue and axillary lymph nodes. The aim of this study is to analyze patients diagnosed with breast cancer between the years 2013 and 2014, taking into account the following variables: age, chemotherapy (type of medication and duration), CPR, and histological subtypes. Medical records were analyzed. Of 31 patients who underwent neoadjuvant chemotherapy, three obtained a CPR. Of these, two had hybrid HER2 histological subtype, one negative triple, and one Luminal A. Method: A study was conducted in which the eligible population was composed of women diagnosed with breast cancer (cid c50) whose treatment and follow-up was performed by the mastology team of the Dr. Arnaldo Cancer Institute. Patients who were diagnosed from 2013 to 2014 were selected. For the description of the cases, variables such as tumor subtype, age, Qt neo performance, and CPR were analyzed. Clinical and pathological characteristics were collected regarding diagnosis and type of treatment, laterality, tumor staging, and the number of compromised lymph nodes. Subsequently, the response to the end of the first treatment — Qt neo, a classification, was performed: no evidence of the disease, partial remission, disease in progression, death, or CPR. Results: We evaluated 163 patients diagnosed with breast cancer between 2013 and 2014 at the Dr. Arnaldo Cancer Institute and also included 31 patients who underwent Qt neo. It was observed that of these three had CPR, 68 were older than 60 years and the most common histological type was invasive ductal carcinoma.

Aims and Objectives: To report a series of cases with breast and ovarian carcinomas either in same patient or in a family and identifying the importance of BRCA 1, 2 genetic testing in such individuals. Materials and Methods: The medical records of breast and ovarian cancer patients operated over past 3 years at a single institute were reviewed retrospectively and their clinical profile, family history, final pathological reports and follow up data was collected. Results: 8 patients were found to have breast and ovarian malignancies, out of which 3 had synchronous breast and ovarian cancers, 4 had metachronous and 1 patient with ovarian cancer had history of breast cancer in family. Median age of presentation to the hospital was 47 years and median time interval in metachronous disease patients was 5.5 years. Conclusion: About 5% of people who have breast cancer and about 10% of women who have ovarian cancer have HBOC, caused by germline mutation in BRCA 1, 2 gene. These individuals have increased risk of developing breast cancer at younger age, TNBC, or developing a second primary in breast or ovary plus an overall risk of breast/ovarian/prostate/pancreatic malignancies in other family members due to inheritable mutation. Identification of BRCA mutation in such individuals can help family members to undergo genetic counseling and follow different screening and prevention guidelines from general population thus reducing the cancer risks.

Introduction: Immunohistochemistry, in breast cancer samples, measures the expression of biomarkers such as estrogen receptor (ER), progesterone receptor (PR), HER2, and Ki67. Using the positivity or negativity of the receptors and the Ki67 value, this method, along with the histological results, allows the doctors to classify the tumors into four types as follows: Luminal A, Luminal B, HER2, and basal/triple negative. Genetic signature is a tool involving in numerous studies in this area; however owing to the difficulty of access to the tests, its usefulness is still limited. MammaPrint® was the first test approved by the Food and Drugs Administration (FDA) in 2007 to measure prognostic value associated with breast cancer recurrence and classify patients with breast cancer into “low risk” or “high risk” of developing metastases within the first 10 years after diagnosis and elucidates the patient’s need for adjuvant chemotherapy. It categorizes tumors into subtypes based on biological homogeneity. This study aims to analyze the concordance between the results of immunohistochemistry pathological subtyping and MammaPrint®, which is accompanied by BluePrint®, for the classification and stratification of luminal breast cancer. Material and Methods: Data were collected from the medical records of 19 patients in the Instituto Sul Paranaense de Oncologia (ISPON) who presented immunohistochemistry and genetic test compatible with luminal tumors. Immunohistochemistry was evaluated through hormone receptors, HER2 and mainly Ki67, as defined by the 2013 St. Gallen guidelines (50% of the sample were centrally assessed). For classification by the genetic test, BluePrint® provided the molecular subtype data and MammaPrint® stratified the risk, establishing Luminal A tumors as low risk and Luminal B as high risk. The concordance between the immunohistochemical classification and the genetic test was evaluated with the nonparametric McNemar-Bowker test. The Ki67 cutoff value predictive for recurrence risk compared with MammaPrint® was accessed by the ROC curve. Results: The results showed that, on one side, only 33.3% of patients classified as Luminal A by immunohistochemistry were also classified by the genetic signature as Luminal A. On the other side, on the tumors classified as Luminal B, 60% presented agreement between the classifications. Overall agreement among the tests was 47.3%. The cutoff value found for Ki67 predictive of tumor recurrence risk was ≤5, with a sensitivity of 100% and a specificity of 33%. The agreement between hormonal receptors and HER2 with BluePrint® was 100%. Conclusion: This study provides preliminary data regarding the prognostic and predictive value of genetic and molecular tests — represented by MammaPrint®/BluePrint® and immunohistochemistry—in a sample of Brazilian population, evidencing a discrepancy between the methods. The cutoff value of Ki67 predictive for recurrence risk remains under discussion, since there is no standardization of its measurement methodology. As a result, new studies could be developed, with larger and multicentric samples.
