Background: The coronavirus disease-2019 (COVID-19) pandemic has had a dramatic impact on global health, with orthopedics among the most affected specialties. An increasing number of COVID-19-related orthopedic studies have been published. The purpose of this study was to analyze the orthopedic literature published during the COVID-19 pandemic to guide future research. Methods: The Scopus database was searched for relevant literature published between 2020 and 2022. The keywords used in the retrieval process were (“COVID-19” OR “Coronavirus” OR “2019-nCoV” OR “SARS-CoV-2” OR “Betacoronavirus” OR “novel coronavirus 2019” OR “novel coronavirus” OR “coronavirus-19” OR “COVID 19” OR “nCOV” OR “COVID-2019” OR “COVID 2019”) and (“orthopedic” OR “orthopedics” OR “orthopedic” OR “orthopedical” OR “orthopedical” OR “orthopedics”). Spreadsheet software (Excel, Microsoft Corp., Redmond, WA) was used to analyze the top 10 cited authors, countries, journals, and articles. The top 5 publication types were also analyzed. VOSviewer (Center for Science and Technology Studies, Leiden, Netherlands) was used to network and visualize the literature. Results: A total of 1619 publications relevant to COVID-19 and orthopedics were reviewed. Among these publications, the most active country, author, and publication type included the United States, Vaishya R, and original articles, respectively. The most frequently used keywords were human, coronavirus disease-2019, pandemic, and orthopedics. The Journal of Bone and Joint Surgery American Volume was the most cited journal, whereas the greatest number of articles was published in the Journal of Clinical Orthopedics and Trauma. Conclusions: This study provides a perspective on the development of orthopedic publications during the COVID-19 pandemic and evidence for researchers worldwide to strengthen global cooperation in fighting the epidemic.

The beginnings of the development of orthopedic surgery in Serbia have been related to the name of Dr Nikola Krstic and his first radiography of the hand in 1908. The foundation of the Orthopedic Ward, led by Dr Nikola Krstic, within the General State Hospital in Belgrade, in 1919, marks the definition of orthopedics as a separate branch of surgery. In addition to Dr Nikola Krstic, Dr Borivoje Lalovic and Temp. Docent Dr Borivoje Gradojevic, who published the first orthopedics textbook in Serbian in 1934, also worked at the orthopedic ward between the two world wars. The work at the orthopedic ward, which grew into a clinic in 1947, was continued by Prof. Dr Milos Simovic, Prof. Dr Svetislav Stojanovic and Prof, dr Ljubisa Boric. Their successors would have high achievements: Prof. Dr Zivojin Bumbasirevic became the only orthopedist who was a regular member of the Serbian Academy of Sciences and Arts, and Chief of Staff Dr Predrag Klisic and Prof. Dr Branko Radulovic provide impetus for further development of orthopedics by founding Specialist Orthopedics Hospital "Banjica".

In Iasi, Orthopedics-Traumatology later appeared as a distinct specialty. In a first stage, orthopedics developed in surgical clinics - the period of surgical clinics (1879-1970). In each surgical clinic there were surgeons who dedicated themselves to orthopedic pathology: Radu Dimitrie, Theodor Căpățînă (Surgery I), Filimon Cicerone, Eusebiu Neagoe, Iulian Grădinaru (Surgery II), Paul Trosc (Surgery III). In 1967, at the Charity Hospital, two surgical services were carried out: The Surgery and Children's Orthopedic Clinic (Th. Economu) and the Osteoarticular Tuberculosis Clinic (A. Berneaga). Also, this year, 1970, in Iasi, the construction of a new medical unit will be completed - the Children's Hospital where the Clinic of Pediatric Surgery and Orthopedics will be moved. The Charity Hospital is disbanded and the Emergency Clinical Hospital will be established on the site of the former establishment. A second period begins - the transition period (1970-1983) - characterized by the search for optimal solutions, which involved changes and temporary until the establishment of orthopedic clinics. The newly established unit, the Emergency Clinical Hospital, was designed to include three departments: General Surgery IV (I. Jitaru), Medical Clinic (G. Popa) and an Orthopedics and Traumatology Clinic (conf. Gh. Floareș). This clinic treated all surgical pathology of orthopedics, traumatology and had didactic activity with fourth year students. The Orthopedics-Traumatology Department had 40 beds. There is also an Orthopedics-Traumatology department, with 40 beds, located in the Dr. C. I. Parhon Hospital run first by A. Berneaga and then by P. Trosc. Dimitrie Radu, Iulian Grădinaru and G. Herescu worked in this department. A new Recovery Hospital appears in Iasi. The new hospital also has an Orthopedics-Traumatology department (with 111 beds) where the orthopedics department will be transferred from Parhon Hospital. In 1983, Professor Gh. Floareș opted to move the clinic from the Emergency Hospital to the new Rehabilitation Hospital. At the Emergency Hospital there remains an Orthopedics-Traumatology Department staffed by a single doctor - Nicolae Georgescu who will develop a new team, which also have teaching activity: T. Cozma, L. Stratan, P. Sîrbu, Ovidiu Alexa, Paul Corlaci, Cezar Popescu. There are eight resident doctors (Elena Glod, Luminița Lăbușcă, Victor Pencu, G. Ghinoiu, C. Nanu, T. Bunescu, R. Malancea, L. Pacu). During this period (1992-1996) a basic A.O. course was organized in Iași. internationally, on which occasion many orthopedists are persuaded to routinely use modern means of osteosynthesis. Two more doctors come in this clinic: B. Puha, R. Asaftei, D. Cionca and A. Ciubara. After 1989, the ATOM was born: The Association of Traumatologists and Orthopedists of Moldova, congresses and postgraduate courses are organized. In 2012 the Orthopedic Clinic moved to the St. Spiridon Emergency Clinical Hospital (Prof. Ovidiu Alexa). The orthopedic clinic at the Recovery Hospital treats chronic osteoarticular pathology (prof Paul Sirbu).

The work of the VII Congress was held in a dynamic and fruitful format, which made it possible to achieve the goals set — modern issues of prevention, diagnosis, treatment of injuries, diseases and conditions of the musculoskeletal system, medical rehabilitation of patients were considered, which made it possible to improve existing or master new knowledge, skills, and abilities and the competence of students to carry out high-quality professional activities in the specialty “Traumatology and Orthopedics”, which will ultimately form labor functions in accordance with the professional standard “traumatologist-orthopedist”. At the end of the Congress, the delegates adopted a resolution reflecting the most problematic aspects of the service, the solution of which will significantly increase the availability and quality of the provision of traumatological and orthopedic care in the Far Eastern Federal District.

OBJECTIVEIn China, orthopedics and neurosurgery are among the most desired majors for medical students. However, little is known about the working and living status of specialists in these two fields. This study was aimed at evaluating job satisfaction, engagement, and burnout in the population of Chinese orthopedist and neurosurgeon trainees.METHODSA nationwide online survey was administered in mainland China. Questionnaires were answered anonymously. Job satisfaction, engagement, and burnout were assessed using the Job Descriptive Index, the Utrecht Work Engagement Scale, and the Maslach Burnout Inventory, respectively.RESULTSData were collected from 643 orthopedist trainees and 690 neurosurgeon trainees. Orthopedists and neurosurgeons showed no statistical difference in terms of age, sex, job titles, and preference for working in tertiary hospitals. Orthopedists had a higher marriage rate (p < 0.01), a lower divorce rate (p = 0.017), relatively shorter working hours (p < 0.01), and a higher annual income (p = 0.023) than neurosurgeons. Approximately 40% of respondents experienced workplace violence in the last 5 years. Less than 10% of respondents were satisfied with their pay, and over 70% would not encourage their offspring to become a doctor. Orthopedists were more satisfied with their careers than neurosurgeons (p < 0.01) and had a higher level of work engagement (p < 0.01). In addition, a higher proportion of orthopedists were burnt out (p < 0.01) than neurosurgeons, though the difference between the two groups was not significant (p = 0.088). Multivariate regressions suggested that younger age (≤ 25 years old), being a senior trainee, getting divorced, working in a regional hospital, long working hours (≥ 71 hrs/wk), a low annual income (<¥100,000), sleeping < 6 hrs/day, and experience with workplace violence were significantly related to burnout for both groups.CONCLUSIONSChinese orthopedic surgical and neurosurgical trainees are under significant stress. Orthopedic surgeons showed relatively optimistic data in their assessments of job satisfaction, engagement, and burnout. This study may provide valuable information for orthopedic and neurosurgical candidates considering either specialty as a career.

The nutritional support in orthopedic surgery is an unclear idea to date. In these years, the goal of this project has been to clarify what is meant by nutritional support in patients undergoing hip or knee replacement and spine surgery, demonstrating its value in relation to the perioperative path. This aim was pursued by applying a methodology of first studying the literature, then analyzing the setting, evidencing shortcomings, proposing management protocols, and verifying the effectiveness of nutritional strategies. The literature search highlighted the multiple phases that should be coordinated, including prehabilitation, perioperative, and postdischarge management. When applying optimization strategies, such as anemia correction, it may be necessary to examine the eating behavior and anticipate the metabolic responses according to the patient’s state of health. The predictive potential of hemoglobin on the clinical outcome in spine surgery has been investigated through retrospective analysis of existing data, suggesting the need to refine decision-making algorithms that identify the patients to be treated, underlining the importance of having high hemoglobin values in the preoperative period to have more chances of clinical success. Prospective observational studies confirmed that patients either after spine surgery or arthroplasty do not meet their needs for what concerns energy and proteins. It seems to be vital to redefine the tactics to promote early oral nutrition, counteract the depletion of body reserves, and acknowledge the impairment of the olfactory stimulus caused by anesthetic drugs. More transparent information on diet and nutrition should be conveyed to patients. The nutritional support program might include a healthy eating education that teaches the concepts of nutritional value, quantity, quality, and timing. Clear information on what to eat or not to eat before surgery must be accompanied by dietary advice also for postoperative recovery. New management proposals can integrate aspects of healthy eating education into a multimodal approach that includes advice on increasing physical activity and promoting night sleep. A risk management system inspired by the hazard analysis and critical control points system can be effectively applied before orthopedic surgery to manage malnutrition. The interventional trials proved that nutrient deficiencies, including those of vitamin D and iron, are common in the surgical population. Dietary supplements should be carefully selected for what concerns the pharmaceutical formula, as well as for the content of important cofactors that may be necessary to boost the metabolic response. Early cardiovascular effects can derive from correcting hypovitaminosis D with a liquid form of vitamin D, and promising data on enhancing preoperative hemoglobin values can derive from using a supplement containing iron and B vitamins. The future of perioperative medicine in orthopedic surgery should choose a pragmatic integration of dietary and nutritional aspects in orthopedics, thus possibly improving health performance and patient experience.

Mammalian joints, including human joints, are mostly synovial joints, which have low friction and are long lasting. With life expectancy on the rise, we are facing an increase in joint “wear and tear” resulting in cartilage damage. Biological repair has its limitations and when the articular cartilage is severely affected joint replacement is the solution. The longevity of artificial joints is also limited requiring new avenues of research to extend their durability and reduce the cost of repeated operations.

Abstract Assessment techniques for orthopedics training are primarily subjective, and often based on qualitative metrics. In this paper, we propose an analytical approach for the quantitative assessment of orthopedic surgery training, specifically, bone drilling. Our goal in this paper is to help improve orthopedics training by providing a means to assess the resident training progress. To this end, we introduce a novel metric that assigns a unique signature to an individual’s drilling activity based on their drilling trajectory, and we compare it with the signatures of expert surgeons. We conduct a simple bone-drilling experiment with surgeons (experts) and novice users on a hybrid (physical - digital) setup consisting of 3D printed bone surrogates that emulate physical and perceptual properties of a human bone across the young and old age groups. We create expert models using our drilling signature metric to evaluate drilling performance for novice users with respect to expert orthopedic surgeons. Our preliminary analysis of drilling signatures across expert and novice users showcases a perceivable distinction across two different bone types highlighting some fundamental insights on the drilling setup, bone material, and user response to each bone type. Our results indicate that the drilling signature helps capture not only a novice user’s drilling behavior, but also their relative expertise as they progress with training.

Ligaments are bands of fibrous tissues that transmit loads between bones and support internal organs. Their main function is to guide and restrain joints’ motion in order to maintain their stability. Injuries to ligaments such as sprains are among the most common orthopedic injuries. The medical treatment varies with the severity of injury from the simple RICE (rest, ice, compression, elevation) rule to graft implantation. The choice between conservative treatment with no repair and surgical treatment with repair of the torn ligaments remains controversial among orthopedics.

In this study the tribological analysis of Al2O3 nanocomposites/Al2O3 pair; proposed as a candidate material to fabricate hip prostheses was carried out. Nanopowders of Al2O3 (AKP 50, 300 nm), TiO2 (PS-25, 50 nm) and Co metallic powder (Nilaco, 28 nm) were mixed and hot pressed. Wear test was carried out in a pin-on-plate tribometer, with a frequency of 1 Hz, a load of 49 N, for 4h; the counterface used was Al2O3. Mechanical properties as Vickers hardness, fracture toughness and Young’s modulus were estimated using the indentation method. Distilled water and fetal bovine serum solution (FBSS) were used as environment. It was found that the specific wear rate of Al2O3 nanocomposites was about 10−8mm3/N*m and the coefficients of friction were around 0.3–0.5 for FBSS. Worn surfaces were observed using SEM.

Review question / Objective: Does the volume of the upper airway change after an orthodontic intervention? P: growing subjects, adults; I: orthodontic treatment, dentofacial orthopedics, extractions; C: untreated subjects and/or non-extractions; O: volumetric changes of the upper airway measured on CBCT scans. Condition being studied: The primary objective of orthodontic treatment is to establish optimal dental and/or skeletal relationship in harmony with the soft tissue morphology and functioning. In addition, un-impeding or facilitating airway growth and development is an important objective, especially in patients susceptible for airway obstruction or sleep apnea. It is therefore important to look into the effect of various orthodontic treatments on the 3D volumetric changes of the upper airway. Compared with the use of traditional 2D lateral cephalograms, CBCT scans provide the opportunity to perform measurements in more dimensions on the airway with demonstrated reliability. This systematic review therefore includes studies using CBCT scans for evaluation of the airway.

The intrathecal morphine dose achieving optimal analgesia for orthopedic surgery while minimizing side effects has not yet been determined. There is currently a lack of literature synthesis in the safety and effects of low-dose ITM on orthopedic surgery. Condition being studied: Low-dose intrathecal morphine on orthopedic surgery.
