Unlike stroke after cardiac and carotid surgery, stroke after general; orthopedic; peripheral vascular; and ear, nose, and throat surgery has not been investigated extensively. The incidence, predisposing factors, and etiological mechanisms of stroke in patients undergoing these procedures are reviewed. Recommendations to prevent, recognize, and treat stroke following these surgical procedures are provided to minimize postoperative stroke risk and its associated morbidity and disability. Although these recommendations can help to decrease the incidence of perioperative stroke, there is an unmet need to find novel and effective neuroprotective strategies that can be used pre- or intraoperatively to minimize the effects of stroke on brain tissue and resulting disability. Future studies should evaluate the potential usefulness of neuroprotective therapies or interventions, including various anesthetic agents that can be used prophylactically in the perioperative setting.

Neuraxial anesthesia has a long history of use in pediatric surgery. It can be used as a sole anesthetic or as an adjunct to sedation or general anesthesia. Neonatal spinals and single-shot caudal anesthesia are effective for lower abdominal, urological, and lower extremity orthopedic surgeries. When a thoracic, lumbar, or caudal epidural catheter is utilized, postoperative analgesia can also be provided to surgeries involving the chest and upper abdomen. There is renewed interest in neuraxial anesthesia due to concerns of the effect of volatile anesthetics on the developing brain. Though research studies on the long-term effects of volatile anesthetics are still inconclusive, neuraxial anesthesia has been shown to provide effective surgical anesthesia and postoperative analgesia. When used in combination with general anesthesia, it has the benefit of reducing the amount of sedatives required. The use of ultrasound may increase success and margin of safety.

The increasing utilization of radiation for diagnostic and therapeutic procedures has provided impetus for improved strategies of radiation protection for interventionalists. The associated discomfort, disability, and career-shortening effects of lead aprons for heavy fluoroscopy users have served as an impetus for the development of lighter and more comfortable models. A suspended radiation protection system employs the use of a “weightless” shield resembling a thick large lead apron with head shield and arm shields. The shield moves with the operator like a garment, providing extensive protection without orthopedic strain or discomfort while maintaining full user functionality. Utilization of a suspended radiation protection system provides the operator with optimum radiation protection, without any additional weight, and maintains procedural flexibility.

This chapter examines the options for managing pain after orthopedic spinal surgery in the lumbosacral spine. It reviews the pain syndromes associated with different approaches to the lumbar spine. The chapter explores specific pain syndromes such as failed back syndrome while noting that the majority of pain after spinal surgery results from dissection of soft tissue and muscles. The chapter then discusses oral and parenteral methods for analgesia, as well as spinal and regional nerve blockade. It provides details on the common regimens for pain management including the use of opioids, nonsteroidal anti-inflammatory drugs, gabapentin, acetaminophen, ketamine, and patient-controlled analgesia (both classical intravenous and transdermal iterations). The chapter also notes the use of multimodal analgesic regimens to promote pain control while reducing the risk of opioid-related adverse effects.

This chapter establishes definitions of sexual health and intimacy, provides insight into how they interact and affect successful relationships, and communicates the importance of addressing the sexual and intimacy needs of injured service members and their partners. Common deployment-related diagnoses, including post-traumatic stress disorder, traumatic brain injury, orthopedic injuries, spinal cord injuries, genital injuries, burns, and facial injuries, and their possible corresponding limitations are reviewed, along with how such limitations can result in sexual avoidance and intimacy avoidance, thus leading to more relationship issues and poor quality of life. The experience of partners, including perceptions about the dependence of the injured service member, is discussed as well.
