Abstract The rock reliefs of Sirkeli represent an important testimony among the Hittite monuments with hieroglyphic inscriptions. In addition to the relief of King Muwatalli, a second relief was identified in 1994, whose hieroglyphic inscription seemed irretrievably lost. Based on a cooperation between the Swiss Archaeological Mission at Sirkeli and the Centro Interistituzionale Euromediterraneo of the University Suor Orsola in Naples, a 3D survey with technologically advanced instruments was carried out in 2017. This contribution presents the first results of this project and the new perspectives that they offer for further research.

Purpose The purpose of this paper is to describe and discuss the way internships are currently evaluated in Campania Universities by host institutions. Design/methodology/approach The author collected and described questionnaires used by the universities of the Regional Observatory of Campania University System. These questionnaires are given by the host institutions to interns to fill out, and they evaluate skills growth in terms of initial and final assessment. Data related to these questionnaires are not publicly available, therefore the main data source comes from 732 post-internship questionnaires collected by the University of Suor Orsola Benincasa in 2013-2014. Findings Out of the 732 participants considered, 65 per cent rated excellent for motivation and interest, 55 per cent were rated excellent for the responsibility demonstrated, but only 37 per cent were rated excellent for the level of work autonomy achieved: a signal of the need to better prepare students for internships. Cross-technical and professional skills, which increase of +37 and +32 per cent, suggest that the experience has a very positive value. Regarding the experience outcome, the author detected a positive indicator: in 6 per cent of cases, an early employment occurred. Research limitations/implications It has only been possible to analyze the questionnaires from one of the Campania universities. The results encourage us to explore further, extending instruments and research methods at a national level, too. Originality/value The author built a research database form to compare current questionnaires in Campania. The author collected internship evaluation data from Suor Orsola University to propose a common questionnaire among Campania Universities. By sharing the findings, other universities, national and international, could benefit from the results.

La scialoadenite cronica ostruttiva rappresenta una delle più frequenti patologie non-neoplastiche delle ghiandole salivari e la scialoendoscopia è sempre più utilizzata nella sua diagnosi e nel suo trattamento, associata o meno con la litotripsia laser. La scialoendoscopia può essere inoltre associata ad approcci esterni mini-invasivi nelle litiasi troppo voluminose per essere rimosse con un approccio unicamente endoscopico. Il presente articolo riporta l’esperienza delle Cliniche Otorinolaringoiatriche dell’Ospedale Sant’Orsola-Malpighi di Bologna e dell’Azienda Ospedaliero Universitaria di Cagliari, Italia. È stata eseguita un’analisi retrospettiva su 48 pazienti (26 femmine, 22 maschi; età media di 45,3 anni; range 8-83 anni) trattati per patologia cronica ostruttiva delle ghiandole salivari maggiori mediante procedure chirurgiche endoscopiche o combinate da novembre 2010 ad aprile 2016 presso l’Azienda-Ospedaliero-Universitaria di Cagliari. I risultati dell’Ospedale Sant’Orsola-Malpighi di Bologna erano stati precedentemente pubblicati. Gli aspetti tecnici della scialoendoscopia sono stati accuratamente descritti. I pazienti trattati presso l’Azienda Ospedaliero Universitaria di Cagliari presentavano una patologia unilaterale in 40 casi e bilaterale in 8 casi; sono state trattate 56 ghiandole salivari maggiori (22 sottomandibolari e 34 parotidi). 5 pazienti sono stati sottoposti a scialoendoscopia bilaterale per parotite ricorrente giovanile, 10 per patologia ostruttiva non litiasica e 33 (68,75%) presentavano calcoli salivari (1 paziente presentava una litiasi parotidea bilaterale). Solo 8 pazienti sono stati sottoposti a scialectomia radicale per via esterna (5 scialectomie sottomandibolare e 3 parotidectomie). La chirurgia conservativa nei pazienti con scialoadenite cronica ostruttiva appare efficace e può essere realizzata mediante un approccio puramente endoscopico o combinato, con un’alta percentuale di successo. La procedura richiede una strumentazione adeguata e deve essere eseguita da un chirurgo esperto, che abbia svolto un training specifico scialoendoscopico, in modo da evitare le possibili complicanze maggiori e minori. La scialectomia tradizionale rappresenta la “extrema ratio”, limitata nei casi in cui un approccio conservativo sia risultato inefficace o controindicato.

Within the cooperation project between the University Suor Orsola Benincasa of Naples and the archaeological mission in Hattuša of the German Archaeological Institute of Istanbul, directed by Andreas Schachner, in agreement with the Turkish Ministry of Culture and Tourism, the workgroup of the University of Naples, has carried out, in September 2015, a first survey campaign of the whole rocky site of Yazılıkaya. <br><br> The experimentation has been finalized at constructing a global 3D territorial and monumental model of the site, capable that is, through the application of differing scanning procedures, according to the different components (topography, rocky complex, the cultural spaces therein, complex of sculptural reliefs, inscriptions accompanying the divine representations), of virtually reproducing in detail, for safegaurd, exhibition and study purposes (in particular from an epigraphical and historic-artistic point of view) all the aspects characterizing the artefact and not completely visible to the naked eye today.

ABSTRACT The purpose of this study was to evaluate the diagnostic performance of LIAISON Treponema Screen (DiaSorin, Saluggia, Italy), a new automated chemiluminescence immunoassay (CLIA), in comparison with that of rapid plasma reagin (RPR) and the following currently used treponemal tests: hemagglutination test (TPHA), immunoenzymatic assay (EIA), and Western blot (WB). First, a retrospective study was performed with a panel of 2,494 blood donor sera, a panel of 131 clinical and serologically characterized syphilitic sera, and 96 samples obtained from subjects with potentially interfering diseases or conditions. A prospective study was also performed by testing 1,800 unselected samples submitted to the Microbiology Laboratory of the St. Orsola Hospital in Bologna, Italy, for routine screening for syphilis. As expected, RPR was the least specific method, especially when potentially cross-reacting sera were tested. On the contrary, all of the treponemal tests proved to be very specific (99.9%) and they performed with the following sensitivities: 100% (WB), 99.2% (CLIA), 95.4% (EIA), and 94.7% (TPHA).

IL PATROCINIO FRANCESCANO E PAPALE AL CONVENTO DELLE CLARISSE DI SAN COSIMATO A TRASTEVERE, 1440–1560Questo articolo esamina gli aspetti relativi agli studi sul ruolo della donna nel patrocinio papale e francescano al convento delle Clarisse di San Cosimato a Roma durante il Rinascimento. Le monache erano trattate come dipendenti indigenti, e le varie strategie di patrocinio per loro erano formulate da rappresentanti maschili della chiesa. Una cruciale fonte di informazione è rappresentata dalla cronaca del convento scritta da Suor Orsola Formicini alia fine del XVI secolo. San Cosimato era fermamente collocato in una complessa rete di connessioni francescane, che andavano dall'individuale all'istituzionale, e che tutte esercitavano una qualche forma di patrocinio. Il patrocinio finanziario e sull'edificio esercitato da papa Sisto IV (lui stesso un francescano) e da papa Alessandro VI viene analizzato in dettaglio. Il fenomeno del patrocinio di due parenti di sesso femminile di questi papi, Franchetta della Rovere e Vannozza Catanei, è anche discusso.

Fondamentale di ogni progetto di aiuto è, quindi, una corretta comunicazione. In questo articolo, esamineremo quella tra infermiere e paziente in ambito dialitico. L'indagine è stata condotta dal 28 Maggio al 15 Giugno del 2012 con la distribuzione di un questionario ai pazienti uremici e al personale infermieristico operante nell'Unità Operativa di Emodialisi, Predialisi e Dialisi Peritoneale dell'Ospedale S. Orsola di Bologna. Obiettivo Della Ricerca: – Verificare l'esistenza di un rapporto comunicativo educativo tra malati e infermieri. – Stabilire se il livello qualitativo è percepito ugualmente sia dagli infermieri che dai malati. – Verificare se la comunicazione instaurata ha un effetto benefico sulla psicologia del paziente, tale da permettergli di affrontare e superare eventuali disagi sociali, psicologici e familiari apportati dalla malattia e dal trattamento.

As breast reconstruction is an important adjunct after mastectomy to regain physical integrity and also to improve affect, the present aim was to evaluate patients' subjective perceptions of body image during the whole breast reconstruction period and to assess the importance of their psychological reaction in terms of negative affectivity. Participants were 62 women, 43 women ( M age = 46.4, SD = 9.8) who had had mastectomies and 19 healthy women ( M age = 39.9, SD= 13.99). Patients were admitted for surgery at the Hospital S. Orsola in Bologna. Healthy subjects were relatives of the women and students, all with no history of breast pathology. The Body Satisfaction Scale and the State Anxiety Inventory-Y were administered to the two groups before, post, and 6 mo. after surgery. Analysis of scores indicated that during the period of the study, the women with mastectomies reported higher anxiety and also greater dissatisfaction with their body image than the healthy group, even when the breast had been reconstructed. This unexpected finding suggests patients' unrealistic expectations of the breast reconstruction and the surgical outcome.

We prove that the tautological ring of ${\mathcal{M}}_{2,n}^{\mathsf{ct}}$, the moduli space of $n$-pointed genus two curves of compact type, does not have Poincaré duality for any $n\geqslant 8$. This result is obtained via a more general study of the cohomology groups of ${\mathcal{M}}_{2,n}^{\mathsf{ct}}$. We explain how the cohomology can be decomposed into pieces corresponding to different local systems and how the tautological cohomology can be identified within this decomposition. Our results allow the computation of $H^{k}({\mathcal{M}}_{2,n}^{\mathsf{ct}})$ for any $k$ and $n$ considered both as $\mathbb{S}_{n}$-representation and as mixed Hodge structure/$\ell$-adic Galois representation considered up to semi-simplification. A consequence of our results is also that all even cohomology of $\overline{{\mathcal{M}}}_{2,n}$ is tautological for $n<20$, and that the tautological ring of $\overline{{\mathcal{M}}}_{2,n}$ fails to have Poincaré duality for all $n\geqslant 20$. This improves and simplifies results of the author and Orsola Tommasi.

Drawing upon results of fieldwork conducted from 2005 to 2018 in Florence, Italy, the article discusses renovation of a medieval convent and other urban renewal activities in the city center. It examines how the projects intersect with broader concerns regarding identity, change, and workways in the San Lorenzo neighborhood. San Lorenzo has deep associations with trade, including outdoor vending. Hundreds of men and women work in its homonymous open-air market. Notwithstanding vending's historical prominence, questions have arisen regarding whether the outdoor market today is an environment congruent with local values and traditions. Some “requalification” proposals call for drastic reductions in vendors' numbers, their transfer to a repurposed convent, or new restrictions. Others presuppose the right of market vendors to remain in place. To address this disjuncture and its implications, the article compares the mission statements and activities of two non-profit associations. The first is composed largely of residents and shop owners, the second of long-time Florentine street vendors. Findings reveal that tourism figures importantly but differently in their experiences and that urban renewal has created opportunities to exercise new forms of social control over micro-business operators who depend on tourism.

In the research started within the SINAPSIS project of the Università degli Studi Suor Orsola Benincasa an underwater stereoscopic scanning aimed at surveying of submerged archaeological sites, integrable to standard systems for geomorphological detection of the coast, has been developed. The project involves the construction of hardware consisting of an aluminum frame supporting a pair of GoPro Hero Black Edition cameras and software for the production of point clouds and the initial processing of data. <br><br> The software has features for stereoscopic vision system calibration, reduction of noise and the of distortion of underwater captured images, searching for corresponding points of stereoscopic images using stereo-matching algorithms (dense and sparse), for points cloud generating and filtering. <br><br> Only after various calibration and survey tests carried out during the excavations envisaged in the project, the mastery of methods for an efficient acquisition of data has been achieved. The current development of the system has allowed generation of portions of digital models of real submerged scenes. A semi-automatic procedure for global registration of partial models is under development as a useful aid for the study and musealization of sites.

Abstract The didactic-performative experience at a distance, centred on the body in action of the training laboratories activated at the University of Study Cattolica del Sacro Cuore of Milan and the University of Study Suor Orsola Benincasa of Naples, encouraged, through the exercise of ‘simplex property’ of the separation of functions, the crossing of borders of ‘presence/distance’, using Didactics at Distance (DAD) as an opportunity to search in everyone the potential patrimony of the action, able to go beyond the classical meanings of interaction, relationship, experience, emotion, through an effort of imagination and simulation, which even interaction with the machine can stimulate. The research work recalls the ambivalence and the plurality of interpretative keys of the teaching experience and includes its analogies with the performing arts. Starting from the ‘stage presence’, as a scenic ‘bios’ and source of energy, a third energetic space has been investigated, which can amplify the relational dimension. A space which the biologist Sheldrake defines as ‘morphogenetic field’ is the one in which it is possible, through the activation of an empathic climate, to make all the processes of emotional sharing, expressive rediscovery and acquisition of awareness sprout, making the experience of DAD a formative and transformative one.

The scientific literature offers a wide range of studies evidencing the progress done in the retrofit actions dealing with the current building stock; however, renovations of hospitals are still an open field of research due to their typical complexity that is usually associated with a very challenging updating processes to maintain or increase operational level. The paper provides a synthesis of a study developed by a team of the Department of Architecture for Saint Orsola Hospital in Bologna with the scope to explore innovative retrofitting strategies. The brief provided by the management unit of the hospital was connected to the general renovation plan involving the entire site and particularly some existing buildings taking into account some limitations concerning budget availabilities and everyday activities needed to ensure acceptable service level for the end users. The design approach starts from defining a basic unit (a typical hospital room) that is deeply analyzed to report the starting conditions (indoor environmental parameters) and then used to simulate the potential impacts of retrofitting actions on its performances. The results allowed to parametrically develop a step by step strategy scaled on each building sector and on the building as a whole to evaluate the global impact on energy performances while considering time and costs of each retrofitting options.

Sustainable education requires the proper usage of technological devices. Among these is the smartphone, which is used by millions of young people around the world in today’s society. The objective of this study was to understand the smartphone usage profile of Spanish and Italian university students. It also aimed to identify possible problematic uses, and the differences in smartphone use (or abuse) between the participating subjects. The study was descriptive and comparative, with the intentional sampling of N = 1412 subjects studying at the education faculties of the University of Alicante (UA) and the Suor Orsola Benincasa University (UNISOB). A previously-validated questionnaire with 27 items was employed during the 2019–20 academic year. The data was analysed using the SPSS 25 programme. Descriptive and inferential analyses were carried out. The results obtained after the analysis of the data indicated that, of the four possible user types—occasional, habitual, at risk, and problematic—more than half of the sample identified themselves as habitual users. It can be concluded that there are significant differences between the universities: the UA students exhibited more problematic use than the UNISOB students. It is therefore necessary to prepare training programmes that are designed to avert problematic behaviours related to smartphone use.

In this study we assessed the prevalence and predictors of pharyngeal infections due to Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) in a cohort of patients attending an STI Clinic. From January 2016 to September 2018, 893 patients attending the STI Clinic of St Orsola-Malpighi Hospital (Bologna, Italy) and reporting unprotected oral sex were enrolled. A pharyngeal swab for the molecular detection of CT and NG was collected from each patient. Positive CT samples were further genotyped by an omp1 gene PCR. A total of 134 cases of gonorrhea (15%) and 34 chlamydial infections (3.8%) were detected in the pharyngeal site, with no significant differences between males (all men who have sex with men [MSM]) and females. More than 90% of the infections were completely asymptomatic, suggesting that symptoms were not predictors of a pharyngeal infection ( P = 0.7). On the contrary, a history of sexual contacts with a partner positive for CT and/or NG ( P < 0.0001), HIV positivity ( P = 0.01), and the presence of concurrent genital and/or rectal infections ( P < 0.0001) were significantly associated with CT/NG pharyngeal infections. Pharyngeal lymphogranuloma venereum cases were rare (prevalence: 0.4%), asymptomatic, and only found in MSM. Better knowledge about pharyngeal CT/NG infections could help to set up effective strategies for their prevention.

Liver Transplantation (LT) has become the gold standard treatment for End-Stage Liver Disease (ESLD). One of the main strategies to manage life-threatening complications, such as cardio-respiratory failure, is Extracorporeal Membrane Oxygenation (ECMO) in the peri-transplantation period, with different configurations of the technique and in combination with other extracorporeal care devices such as Continuous Renal Replacement Therapy (CRRT). This retrospective study includes three clinical cases of planned ECMO support strategies in LT and evaluates their application compared with current literature exploring PubMed/Medline. The three LT supported with ECMO and CRRT were performed at IRCCS Polyclinic S. Orsola-Malpighi, Bologna. All three cases of patients with compromised organ function analysed produced positive outcomes. The planned use of ECMO and CRRT support in peri-transplantation has allowed the patients to overcome contraindications and successfully undergo LT. In recent years, only a few reports have documented successful LT outcomes performed with intraoperative ECMO in critically ESLD patients. However, the management of LT with ECMO and/or CRRT assistance is an emerging challenge, with the need for more published evidence on this topic to guide treatment choices in patients with severe, acute and reversible respiratory and cardiovascular failure after LT.

This retrospective observational study investigated hospital staff requests for job fitness visits, addressed to occupational medicine. Specific objectives were to analyze: (1) health workers’ requests, sociodemographic characteristics, psychiatric diagnoses, assigned doctor’s fit notes, and (orthopedic, psychiatric) limitations; (2) associations between psychiatric diagnoses, sociodemographic (sex, age), and work-related (job, department) characteristics; (3) associations between the same psychiatric diagnoses/orthopedic limitations, fit notes, and/or psychiatric limitations. Data of St. Orsola-Malpighi Polyclinic health workers (N = 149; F = 73.8%; mean age = 48 ± 9.6 years), visited by both the occupational medicine physician and psychiatrist (January 2016–May 2019), were analyzed. 83.2% of the sample presented with at least one psychiatric diagnosis, including mood (47%), anxiety (13.4%), and anxious-depressive (10.7%) disorders. Significant differences between psychiatric diagnoses according to sex and fit notes (both p < 0.01) have been found, whereas no significant associations based on age and work-related characteristics have been observed. Analysis of frequencies of participants with the same psychiatric diagnosis (orthopedic limitation being equal), according to doctor’s fit notes and psychiatric work limitations, showed a high heterogeneity of assignments. The current occupational medicine procedure for fit notes/job limitations assignments does not allow taking into consideration clinical factors possibly associated with more specific assignments. To standardize the procedure and translate the psychiatrist’s clinical judgment into practice, further studies to test the usefulness of clinimetrics, which might represent a reliable approach in considering different fit notes and job limitations, are needed.

Purpose: Computer-aided methods for mandibular reconstruction have improved both functional and morphological results in patients who underwent segmental mandibular resection. The purpose of this study is to evaluate the overlaying of virtual planning in terms of measures of the Computer Assisted Design/Computer Assisted Manufacturing CAD/CAM plate for mandibular reconstruction in patients who are ineligible for the insertion of reconstructing the titanium plate supported by fibular free flap, due to their poor health status, or in the presence of specific contraindications to autologous bone flap harvest. Materials and methods: The retrospective study performed analyzed the results of nine patients. The patients were treated at the Maxillofacial Surgery Unit of Policlinico S. Orsola of Bologna, Italy, and Policlinico San Marco, Catania, Italy, from April 2016 to June 2021. Superimposition between planning and post operative Computed Tomography CT scan was performed to assess the accuracy. Results: All reconstructive procedures were carried out successfully. No microsurgery-related complications occurred. In two cases, we had plate misplacement, and in one case, plate exposure that led to plate removal. The average accuracy of the series assessed after CT superimposition, as previously described, was 0.95 mm. Conclusions: Considering that microvascular bone transfer is a high-risk procedure in BRONJ patients, we can conclude that the positioning of a customized bridging mandibular prosthesis (CBMP), whether or not it is associated with a microvascular soft tissue transfer, is a safe technique in terms of surgical outcome and feasibility.

Mycoplasma pneumoniae (MP) is one of the main causes of both upper and lower respiratory infections in school-aged children, accounting for up to 40% of community-acquired pneumonia. Younger children are also affected, and extrapulmonary manifestations have been recently reported in the pediatric population. We carried out a retrospective analysis of MP-positive patients admitted to the Pediatric Emergency Unit of S. Orsola Malpighi University Hospital in Bologna, the largest tertiary pediatric referral center in the Emilia–Romagna region, Northern Italy, between 2012 and 2020. We identified 145 patients with MP infection (82 males and 63 females), 27% of which were younger than 2 years; the median age was 5 years (interquartile range 1–9). The clinical presentation partially differed between age groups. School-aged children were more likely to have a chest X-ray-confirmed pneumonia (p = 0.013), while younger children required oxygen therapy more often (p = 0.048). Seventy-four children (51%) showed extrapulmonary manifestations, mainly gastrointestinal (30%) and dermatological (14%). Neurological symptoms were more frequent in children older than 6 years (p = 0.006). The rate of other extrapulmonary manifestations did not differ significantly between age groups. This study shows that MP infection is a frequent cause of pediatric hospitalization, including of children younger than 2 years. Clinicians should be aware of the variable clinical expressions of MP, including extrapulmonary manifestations, to achieve a correct diagnosis and determine appropriate treatment.

ObjectiveOur study purpose was to evaluate morbidity and postoperative mortality in patients who underwent pelvic exenteration (PE) for primary or recurrent gynecological malignancies.MethodsWe identified 230 patients who underwent PE, referred to the gynecological oncology units of 4 institutions: Charitè University in Berlin, Friedrich-Schiller University in Jena, S. Orsola-Malpighi University in Bologna, and Catholic University in Rome and in Campobasso.ResultsThe median age was 55 years. The tumor site was the cervix in 177 patients, the endometrium in 28 patients, the vulva in 16 patients, and the vagina in 9 patients. Sixty-eight anterior, 31 posterior, and 131 total PEs were performed in 116 women together with hysterectomy. A total of 82.6% of the patients required blood transfusion. The mean operative time was 446 (95–970) minutes, and the median hospitalization was 24 (7–210) days. We noted a major complication rate of 21.3% (n = 49). We registered 7 perioperative deaths (3%) calculated within 30 days. The operation was performed within clear margins in 166 patients (72.2%). The overall mortality rate depending on tumor site at the end of the study was 75% for vulvar cancer, 57.6% for cervical cancer, 55.6% for vaginal cancer, and 53.6% for endometrial cancer.ConclusionsAlthough an important effort for surgeons and for patients, PE remains a therapeutic option with an acceptable complication rate and postoperative mortality. A strict selection of patients is mandatory to reach adequate surgical and oncologic outcomes.

ABSTRACTSerology has a pivotal role in the diagnosis of congenital syphilis (CS), but problems arise because of the passive transfer of IgG antibodies across the placenta. The aim of this study was to assess the diagnostic value of a comparative Western blot (WB) method finalized to match the IgG immunological profiles of mothers and their own babies at birth in order to differentiate between passively transmitted maternal antibodies and antibodies synthesized by the infants againstTreponema pallidum. Thirty infants born to mothers with unknown or inadequate treatment for syphilis were entered in a retrospective study, conducted at St. Orsola-Malpighi Hospital, Bologna, Italy. All of the infants underwent clinical, instrumental, and laboratory examinations, including IgM WB testing. For the retrospective study, an IgG WB assay was performed by blottingT. pallidumantigens onto nitrocellulose sheets and incubating the strips with serum specimens from mother-child pairs. CS was diagnosed in 11 out of the 30 enrolled infants; 9/11 cases received the definitive diagnosis within the first week of life, whereas the remaining two were diagnosed later because of increasing serological test titers. The use of the comparative IgG WB testing performed with serum samples from mother-child pairs allowed a correct CS diagnosis in 10/11 cases. The CS diagnosis was improved by a strategy combining comparative IgG WB results with IgM WB results, leading to a sensitivity of 100%. The comparative IgG WB test is thus a welcome addition to the conventional laboratory methods used for CS diagnosis, allowing identification and adequate treatment of infected infants and avoiding unnecessary therapy of uninfected newborns.

ABSTRACTDespite recent technological advances, the diagnosis of syphilis remains a challenging enterprise. Actually, most high-volume laboratories have adopted the “reverse algorithm” due several factors, including the potential to automate testing. Recently, immunoassays processed on random-access systems have been proposed as screening tests. The purpose of this study was to evaluate diagnostic performances of BioPlex 2200 Syphilis IgG and BioPlex 2200 Syphilis IgM, tests based on Multiplex Flow technology, in comparison with the performance of Architect Syphilis TP, a chemiluminescent immunoassay for the detection of IgG and/or IgM anti-Treponema pallidumantibodies. A retrospective study was performed with a panel of 100 blood donor sera, a panel of 350 clinical and laboratory-characterized syphilitic sera, and 170 samples obtained from subjects with potentially interfering conditions. Moreover, 200 unselected samples submitted to the Microbiology Laboratory of St. Orsola Hospital in Bologna for routine screening for syphilis were evaluated. As confirmatory tests,T. pallidumhemagglutination and Western blot assays were used. Considering the IgG Western blot (WB) assay to be the gold standard method, BioPlex 2200 Syphilis IgG specificity was far higher than Architect Syphilis TP specificity (89.7% versus 78.4%, respectively), whereas the sensitivity was 100% for both automated methods. Compared to the IgM WB assay, BioPlex 2200 Syphilis IgM performed with a specificity of 94.9%, whereas the sensitivity was 84.8%. Considering the excellent ease of use and automation, the high sample throughput and its valuable analytical performances, BioPlex Syphilis 2200 IgG could represent a suitable choice for high-volume laboratories. BioPlex Syphilis 2200 IgM could be considered a good addition to IgG testing for uncovering active infections.

Objective: We measured blood levels of obestatin, total ghrelin, and the ghrelin/obestatin ratio and their relationship with anthropometric and metabolic parameters, adiponectin and insulin resistance, in overweight/obese and normal-weight women. Design: Outpatients Unit of Endocrinology of the S Orsola-Malpighi Hospital of Bologna, Italy. Methods: Fasting obestatin, ghrelin, adiponectin and lipid levels, fasting and glucose-stimulated oral glucose tolerance test insulin, and glucose levels were measured in 20 overweight/obese and 12 controls. The fasting ghrelin/obestatin ratio was calculated; the homeostasis model assessment-IR (HOMA-IR) and insulin sensitivity index (ISIcomposite) were calculated as indices of insulin resistance. Results: Obese women had higher obestatin and lower ghrelin blood levels, and a lower ghrelin/obestatin ratio compared with controls. In all subjects, obestatin was significantly and positively correlated with total cholesterol and triglycerides, but not with ghrelin, anthropometric, and metabolic parameters. In the obese women, however, obestatin and ghrelin concentrations were positively correlated. By contrast, the ghrelin/obestatin ratio was significantly and negatively correlated with body mass index, waist, waist-to-hip ratio, fasting insulin, and HOMA-IR, and positively with ISIcomposite but not with adiponectin. None of these parameters were correlated with the ghrelin/obestatin ratio in the obese. Conclusions: Increased obestatin, decreased ghrelin levels, and a decreased ghrelin/obestatin ratio characterize obesity in women. This supports the hypothesis that the imbalance of ghrelin and obestatin may have a role in the pathophysiology of obesity. On the other hand, some relevant differences between our data on circulating levels of obestatin and the ghrelin/obestatin ratio in obese subjects and those reported in the few studies published so far imply that further research is needed.

Background: The lung ultrasound (LUS) score has been proposed as an optimal scheme for the ultrasound study of patients with suspected/confirmed COVID-19 pneumonia. The aims of our study were to evaluate the use of lung ultrasound as a diagnostic tool for diagnosing SARS-CoV-2 pneumonia, to examine the validity of the LUS score for the diagnosis of COVID-19 pneumonia, and to correlate this score with hospitalization rate and 30-day mortality. Materials and Methods: A retrospective analysis was performed on 1460 patients who were referred to the General Emergency Department of the S. Orsola-Malpighi Hospital from April 2020 to May 2020 for symptoms suspected to indicate SARS-CoV-2 infection. The ultrasound examination was based on a common execution scheme called the LUS score, as previously described. Results and Conclusions: The LUS score was found to correlate with the degree of clinical severity and respiratory failure (paO2/FiO2 ratio and the alveolar–arterial gradient increase than expected for age). It was shown that COVID-19 patients with an LUS score of >7 require the use of oxygen support, and a value of >10 is associated with an increased risk of oro-tracheal intubation. The LUS score was found to present higher values in hospitalized patients, increasing according to the degree of care intensity. Patients who died from COVID-19 were characterized by a mean LUS score of 11 at presentation to the emergency department. An LUS score of >7.5 was found to indicate a sensitivity of 83% and a specificity of 89% for 30-day mortality in COVID-19 patients. The use of LUS seems to be an optimal first level method for pneumonia detection and risk stratification in patients with suspected SARS-CoV-2 infection.

676 Background: Twenty per cent of middle/low rectal cancer pts have synchronous distant metastasis (M1) at diagnosis. At present, the treatment strategy in this pt setting is not well defined. This mono-institutional retrospective analysis evaluates the treatment strategy and clinical outcome of three different pt group. Methods: Between January 2000-July 2011 the Bologna Multidisciplinary Rectal Group of S. Orsola-Malpighi Hospital evaluated 65 pts with rectal cancer M1 disease. Three different groups were defined: rectal cancer with resectable metastatic disease (Group A); rectal cancer with potentially resectable metastatic disease (Group B); and rectal cancer with unresectable metastatic disease (Group C). Treatment strategy in the different groups entailed an individualized approach. Results: The distribution per group was: Group A 11 (17%) pts, Group B 28 (43%), and Group C 26 (40%). The pt characteristics were: M 42 (64.6%), F 23 (35.4%); median age 62 years (35-83); median ECOG PS 0 (0-2); cT3 45(69.2%), cT4 20 (30.8%); cN- 10 (15.4%), cN+ 55 (84.6%); M1 sites: liver 32 (49.2%), lung 8 (12.3%), liver + lung 11 (16.9%), ovary/peritoneal carcinosis/abdominal lymph nodes 7 (10.8%), liver + bone 2 (3.1%), >2 sites 5 (7.7%). Forty-three (66.2%) pts underwent to surgery for primary rectal tumor, and 30 (46.2%) pts for metastasis resection (23 liver, 5 lung, 2 liver + lung). Treatments for different groups are shown (Table). Median OS according to the three different groups was: Group A not reached, Group B 32 months (95% CI 24-40), and Group C 16 months (95% CI 7-26). Patients undergoing metastasis resection have higher median OS than those not resected: 44 months (95%CI 12-76) vs 15 months (95% CI 8-23) (p=0.0006). Conclusions: Treatment strategy in synchronous metastatic rectal cancer must consider the possibility of distant metastasis resection. Long-term survival can be achieved using an integrated approach. The growing demand for brain MR scans in recent years has led to long waiting lists and indiscriminate referral with respect to the clinical need for imaging and appropriateness criteria for MR scanning. To overcome this problem, the Bologna Public Health Service in conjunction with S. Orsola-Malpighi Hospital devised an experimental project instituting a radiological assessment prior to booking MR scans, implemented on 1st November 2003. The assessment is carried out by doctors in the Radiology and neuroradiology units to establish whether referral for MR scan is appropriate and to draw up a priority scale for access to MR diagnosis. If MR investigation is deemed inappropriate, the project provides for an alternative procedure or specialist clinical examination. The patient is admitted to the charge of the service and followed throughout the diagnostic work-up, i.e. the doctor undertaking the assessment will prescribe a possible specialist clinical consultation or other radiological procedures, generally CT scans performed by the same Radiology or Neuroradiology unit. We report on neuroradiological assessment of referrals for brain MR scans on behalf of the Public Health Service and carried out at the Neuroradiology Unit in Bellaria Hospital, Bologna. From 1st November to 31st July 2004, 2659 assessments were undertaken. Of these 2163 were approved for MR scanning whereas 496 referrals were modified, mostly into CT scans and some patients were referred for otorhinolaringology, endocrinology or neurology specialist consultation. To assess the impact of this “filter”, we compared a sample period of six months from 1st February 2003 to 31st July 2003 before the radiological assessment project had been implemented with a period of the same length the following year. We found that the number of negative MR scans was halved after the project had been implemented (from 24.49% in 2003 to 12.18% in 2004), showing that in addition to shortening waiting lists for MR scans, there has been a sharp rise in the number of appropriate scans.

e20515 Background: Large cell neuroendocrine carcinoma of the lung (LCNEC) is a rare tumor, with clinical and molecular features between non-small cell and small-cell lung cancer (NSCLC and SCLC, respectively). To date, little is known about factors able to predict clinical outcome of these patients. We sought to identify prognostic factors in LCNEC. Methods: We retrospectively collected pathologically-confirmed LCNEC patients treated at the S. Orsola Malpighi Hospital of Bologna between 01/01/2009 and 31/12/2020. LCNEC was defined as tumor with neuroendocrine morphology, expression of ≥2 neuroendocrine markers (including chromogranin A, synaptophysin, or CD56) at immunohistochemistry and high tumor grade (defined as necrosis and high Ki67 ≥ 50% or high mitotic rate, ≥ 10 mitoses per 2 mm2). Clinical features, pathological features, and blood test values (including neuron-specific enolase [NSE], carcinoembryonic antigen [CEA], cytokeratin 19 fragment [cyfra 21-1]) were correlated with progression-free survival to first-line chemotherapy (PFS) and overall survival from the diagnosis of extensive disease (OS). Results: Eighty-one LCNEC patients (median age 69 years, range 63-76) were identified. Twenty-seven (33.3%) had limited-stage disease and 52 patients (67.5%) had Eastern cooperative oncology group performance status (ECOG PS) ≥ 1 at diagnosis. When extensive disease was found (ED-LCNEC, N = 67), 23 patients (34.3%) had liver metastases, 10 (14.9%) had brain metastases and 24 (35.8%) had bone metastases. Treatment in ED-LCNEC was platinum plus etoposide in 45 patients (81.9%) and platinum plus paclitaxel in 10 patients (18.1%). Among 55 patients treated with platinum-based chemotherapy, 5 patients (9.1%) received cisplatin, while 50 (90.9%) received carboplatin. Overall, OS was 9.17 months (95% confidence interval [95%CI]7.17-13.87), PFS was 4.87 months (95%CI 4.21-6.02) and ORR was 38.2% (95%CI 25.4-42.3, N = 21/56). At univariate analysis, ECOG PS ≥ 1 (P = 0.049), presence of liver (P = 0.004) and bone metastases (P = 0.009), ≥ 2 non-nodal metastatic sites (P < 0.001), elevated NSE (P = 0.009) and CEA level at diagnosis (P = 0.011) were associated with increased risk of death. At multivariate analysis, ECOG PS ≥ 1 (P = 0.020), ≥ 2 metastatic sites (P = 0.002), and elevated NSE level at diagnosis (P = 0.009) were independently associated with the risk of death. At univariate analysis brain metastases (P = 0.017), ≥2 metastatic sites (P = 0.006) and elevated value of CEA at baseline (P = 0.026) were associated with increased risk of progression. At multivariate analysis, only the presence of brain metastases (P = 0.043) retained its association with the risk of progression. Conclusions: In ED-LCNEC of the lung, ECOG PS, number of non-nodal sites of metastases and NSE at baseline are associated with worse survival, while the presence of brain metastases is associated with shorter PFS.

W świetle najnowszych badań archiwalnych historia życia i twórczości Orsona Wellesa nie jest już tak enigmatyczna i tajemnicza jak w poprzednich dekadach, jednak za sprawą nowych odkryć oraz kolejnych premier filmów odrestaurowanych i ukończonych po śmierci reżysera nadal pozostaje gorącym tematem dla jego biografów. Autorka śledzi wpływ procesu technologicznego na końcowy kształt przywróconych do życia dzieł Wellesa głównie na przykładzie Drugiej strony wiatru (2018) – ostatniego filmu reżysera oraz Too Much Johnson (1938) – odnalezionego w 2013 r. jego średniometrażowego debiutu. Wybór tych tytułów podyktowany jest z jednej strony tym, że w obu przypadkach technologia odegrała bardzo istotną rolę, z drugiej – oba te filmy nie zostały jeszcze dogłębnie opisane.
