Short-Term Mission (STM) is a reality that is intertwined and integrated into the fabric of world mission and Christian experience. Many discussions revolving around short-term mission have been descriptive and about best practices. This article moves beyond an appraisal of short-term mission as a phenomenon and instead looks at why and how OMF International, a long-term mission agency, aligned STM with long-term mission, vision and strategy by focusing on the mentorship and discipleship of its STM volunteers. It also looks at with-ness and consociation as a model for mentorship, and discipleship as a means of addressing the opportunities and challenges that STM presents to long-term mission agencies.

OBJECTIVE The development of new treatment approaches for degenerative lumbar spondylolisthesis (DLS) has introduced many questions about comparative effectiveness and long-term outcomes. Patient registries collect robust, longitudinal data that could be combined or aggregated to form a national and potentially international research data infrastructure to address these and other research questions. However, linking data across registries is challenging because registries typically define and capture different outcome measures. Variation in outcome measures occurs in clinical practice and other types of research studies as well, limiting the utility of existing data sources for addressing new research questions. The purpose of this project was to develop a minimum set of patient- and clinician-relevant standardized outcome measures that are feasible for collection in DLS registries and clinical practice. METHODS Nineteen DLS registries, observational studies, and quality improvement efforts were invited to participate and submit outcome measures. A stakeholder panel was organized that included representatives from medical specialty societies, health systems, government agencies, payers, industries, health information technology organizations, and patient advocacy groups. The panel categorized the measures using the Agency for Healthcare Research and Quality’s Outcome Measures Framework (OMF), identified a minimum set of outcome measures, and developed standardized definitions through a consensus-based process. RESULTS The panel identified and harmonized 57 outcome measures into a minimum set of 10 core outcome measure areas and 6 supplemental outcome measure areas. The measures are organized into the OMF categories of survival, clinical response, events of interest, patient-reported outcomes, and resource utilization. CONCLUSIONS This effort identified a minimum set of standardized measures that are relevant to patients and clinicians and appropriate for use in DLS registries, other research efforts, and clinical practice. Collection of these measures across registries and clinical practice is an important step for building research data infrastructure, creating learning healthcare systems, and improving patient management and outcomes in DLS.

The nationwide monument protection system was (re-)established in Hungary in 1957 and operated uninterruptedly during the previous political system. Even though by now both the profession and its institutional system has been transformed numerous times, the dedicated professional journal, titled “Monument Protection”, has been continuously published since 1957. It was first published by the Union of Hungarian Architects and from 1960 the National Monument Protection Inspectorate (OMF) was the main publisher until the end of the researched period in 1990. It describes and discusses case studies, theoretical investigations and informs colleagues and the interested nationwide audience about the international norms and trends in that field. This contribution looks at the first circa three decades (1957-1990) of this journal as a basis to investigate the profession in Hungary, its operational scope as well as its national and international status. Melinda Harlov-Csortán – The Hungarian monum ent... 59 This investigation contributes not just to exploration of an important historical period through a specialized profession but to understanding of our contemporary built environment and the views and concepts behind them as well. The research process involves analysis of printed materials (such as policies or conference publications) as well as interviews with members of the profession. The current contribution analyzes the first 33 years of the publication by looking at both its content, structure and language. Comparative and discourse analysis were conducted to understand both the given period of the Hungarian profession and the role of the journal in question. The aim of the article is to discuss the self-representation of the Hungarian monument protection profession during the researched period (1957-1990) not just by presenting the history of the field but also by investigating tasks and challenges that Hungarian monument professionals identified themselves. ‟Monument Protection” serves also as a great basis for comparative analysis together with other Hungarian publications or even as a basis of international comparison with alike foreign journals.

Introduction: This study aims to describe the epidemiology, aetiology and therapy of orbital fractures in a large municipal hospital in The Hague, the Netherlands, and to demonstrate the effectiveness of an integrated approach of orbital trauma care with a team consisting of an ophthalmologist, an orthoptist and an oral and maxillofacial (OMF) surgeon. Methods: We obtained data from patients with an orbital fracture between June 2015 until July 2018. Primary outcomes were the classification of orbital fractures and treatment (conservative or surgical). Results: 182 patients were included in this study. The most common type of orbital fracture was an isolated orbital floor fracture (n=56, 30.8%). In most cases, therapy was conservative (69.2%). When surgical repair was indicated, a polydioxanone (PDS) plate was most commonly used (11.5%). The main cause of an orbital fracture was a fall accident (38.5%), followed by assault (32.4%). 32.4% of the patients were intoxicated with alcohol. Conclusion: In our study population, almost 70% (69.2%) of the patients were treated conservatively in our study. Compared to the international literature, this percentage is considerably higher than in other studies. In our opinion, our integrated approach to orbital trauma has led to a justifiable reduction of the proportion of cases receiving surgical intervention in orbital fractures. We would like to advocate the implementation of a similar team in other hospitals to improve the quality and cost-effectiveness of treatment in patients with orbital fractures.

Résumé Il a fallu attendre l’année 1994 pour que soit décidée la création d’une organisation internationale chargée d’encadrer le commerce international : l’Organisation mondiale du commerce (OMC). Pourtant des négociations ont commencé sur ce sujet dès 1941. Elles ont eu pour objectif de créer une Organisation internationale du commerce (OIC) et ont abouti en 1948 à la Charte de la Havane instituant une Organisation internationale du commerce. Mais l’OIC n’a jamais vu le jour. À une époque où s’exprime un désenchantement devant les promesses non tenues par les organisations internationales, il est utile de rappeler le contexte intellectuel dans lequel l’idée d’organiser le commerce mondial a émergé. L’article qui suit présente les conceptions des trois principaux économistes britanniques ayant participé à ces négociations : Keynes, Meade et Robbins.

This research embarked on how a positive peace in a typhoon-stricken place is possibly experienced. The researchers conducted this study in the simple town of Hernani, Eastern Samar. The relevant question the researchers would like to answer is how the locals of that typhoon-stricken town absorb, adapt, and recover from the insidious effects brought about by the devastating typhoon. It focused specifically on the resiliency of the locals amid the difficulties and challenges caused by a natural calamity, and how this resiliency had helped them find peace in their lives. Using the methodology and processes of narrative analysis, the research results manifested the peace concept of the locals based on what they had experienced. It has shown also the factors that made the town a peaceful place to live in as expressed through relevant themes. Consequently, the themes were categorized by using the indicators of the pillars of positive peace. The possible manifestation of the positive peace as expressed in the initiatives done by the existing concerned institutions, shown through the attitudes of the locals, and the prevailing structures that operate within the town of Hernani, was seen as a contributor to nation-building. The resiliency shown by the locals amid natural calamity is an essential characteristic of positive peace. The same resiliency of the people, being supported appropriately by a well-performing local government, provided with a healthy business environment, when reinforced and replicated in other towns and provinces can result in a creation of a stronger and peaceful nation.ReferencesBurck, C. Zur Bewältigung der Folgen der europäischen Schuldenkrise schlossen sich erstmals die Europäische Kommission, die Europäische Zentralbank und der Internationale Währungsfonds zur „Troika“ zusammen. Krisengeschüttelte Euroländer erhielten Hilfskredite verbunden mit umfangreichen Kreditkonditionalitäten. Obwohl die Troika bestrebt war, nach außen einheitliche Positionen zu demonstrieren, zeigten sich in Fragen der Schuldenrestrukturierung, der fiskalischen und strukturellen Anpassung systematisch inhaltliche Differenzen zwischen den drei Organisationen. Diese rekonstruieren wir mit Hilfe von process tracing für die Programme für Irland und Griechenland. Unter Rückgriff auf das Konzept der Organisationskultur und ein Verständnis internationaler Organisationen als Bürokratien oder „international public administrations“ interpretieren wir konfligierende Ansätze der Kreditvergabe als Ausdruck inkongruenter Organisationskulturen der Troika-Bürokratien. Unsere Befunde widersprechen sowohl staatszentrierten Perspektiven auf die Troika als auch Ansätzen, die internationale Organisationen eher als „multiple Persönlichkeiten“ denn als unitarische Akteure begreifen.

The problem of international debts is, by its character, one of the most complex problems which the world economy is now facing. It complicates both the global balance of payments and the financial – credit system stability, with pronounced negative impacts on the currency stabilisation. It hinders the development of international economic co-operation and its higher forms – international economic integration. It is one of the reasons that the symmetrical forms of interdependence are pushed off and displaced by the asymmetrical ones. The global debts problem deteriorates, namely during the last time in interaction with the negative manifestations of the global problems (for example, at present very strongly with the environmental problems), the complex international co-operation climate.

L’Organisation internationale de la Francophonie (oif) s’écarte-t-elle de sa vocation originelle? Cet article s’interroge d’abord sur la finalité et les objectifs de la Francophonie. Alors que les uns estiment que l’oif est un vecteur de domination hégémonique, d’autres jugent qu’elle représente un véritable acteur politique international anti-hégémonique qui possède une valeur ajoutée propre en matière de paix et de sécurité. La Francophonie s’avère cependant davantage une institution fondée sur le principe d’appropriation géopolitique d’un espace géoculturel distinct. Dans cette perspective, l’article retrace la prise en charge croissante par l’oif de ce rôle distinct et nécessaire en matière de sécurité internationale et cerne sa valeur géoculturelle ajoutée.

This paper is about investigating how different bank liquidity creation activities affect housing markets. Using data of 401 metropolitan statistical areas/metropolitan statistical area divisions (MSAs/MSADs) of the U.S. between 1990 and 2018, we show that not all bank liquidity creation activities boost the housing markets. In particular, unlike assetside and off- balance sheet liquidity creations, funding-side liquidity creation dampens housing markets. The relationships between liquidity creation activities and housing markets are stronger in regions with inelastic house supply, but flip when banks face external liquidity shocks. We also find that housing markets dominated by large banks are more sensitive to off-balance sheet liquidity creation activities. Finally, as expected, asset-side and off-balance sheet liquidity creations boost housing markets by driving house prices away from fundamental values. Our results offer a more thorough explanation of how bank liquidity creation fuels the momentum of housing markets.

ZusammenfassungTrotz verbesserter gesetzlicher Rahmenbedingungen bei der Entwicklung von Arzneimitteln (AM) für Kinder bestehen für viele pädiatrische Therapiebereiche und v. a. die jüngsten Altersgruppen immer noch große Defizite in der AM-Versorgung. Daher müssen viele AM für Kinder außerhalb der Zulassung (off-label) und somit ohne pädiatrische Fachinformationen angewendet werden. Dadurch besteht ein erhöhtes Risiko für inadäquate Dosierungen sowie fehlende oder unerwünschte Wirkungen. In der Off-label-Anwendung haben Verschreibende eine erhöhte Verantwortung, auf die bestverfügbare Evidenz zurückzugreifen, um altersentsprechende Dosierungen zu wählen sowie Nutzen und Risiko zu bewerten. Dieser Verantwortung können einzelne Verschreibende kaum gerecht werden, weil die wenigen verfügbaren pädiatrischen Daten oft von heterogener Qualität und nicht unmittelbar verfügbar sind.Das internationale Konsortium von Kinderformularien stellt eine Kollaboration in derzeit vier europäischen Ländern dar (Niederlande, Deutschland, Österreich, Norwegen) dar und baut auf dem niederländischen Kinderformularium auf. Das Konsortium hält AM-Informationen für Kinder und Jugendliche, basierend auf bestverfügbarer Evidenz, bereit; diese werden anhand von systematischen Recherchen, kritischen Evaluationen, ExpertInnenreviews, internationaler Harmonisierung sowie regelmäßigen Aktualisierungen erstellt und transparent referenziert. Die Plattform ist webbasiert und Nutzern in der jeweiligen Landessprache frei zugänglich.Das internationale Konsortium von Kinderformularien bietet die Grundlage der Off-label-Anwendung von AM bei Kindern und Jugendlichen, um nach bestverfügbarer Evidenz altersentsprechende Dosierungen zu wählen sowie Nutzen und Risiko abzuwägen.

The article focuses on the concept of sovereignty – an analytical category applicable to states. However, with the emergence of new actors in the international arena, especially new types of organisations such as the European Union, the question arises: whether it is possible to apply sovereignty to entities other than states. The authors have the assumption that in the area of social sciences, it is possible to give the concept of the sovereignty a certain trait of universality, inter alia, to better reconcile the legal and political science approaches. The aim of this study is to identify and then to define an important feature of the EU, which may be sovereignty itself or its equivalent (autonomy, claim to sovereignty, quasi-sovereignty). The results of the study may lead to a better understanding of non-state subjects of public international law such as international organisations in genere, and organisations of integrational and supranational character in specie. The article is analytical, comparative and explanatory.

We model an international union as a group of countries deciding to centralize the provision of public goods, or policies, that generate externalities across union members. The trade-off between the benefits of coordination and the loss of independent policymaking endogenously determines size, composition, and scope of the union. Policy uniformity reduces the size of the union, may block the entry of new members, and induces excessive centralization. We study flexible rules with nonuniform policies that reduce these inefficiencies, focusing particularly on arrangements that are relevant to the ongoing debate on the institutional structure of the European Union.

Abstract Valproic acid (VPA) has been shown to inhibit histone deacetylase activity, and to synergize with ATRA in the differentiation induction of leukemic myeloid blast cells in vitro. We applied VPA to 20 patients (16 sAML/MDS, 2 de-novo-AML, 2 sAML/OMF) too old or physically unfit to receive intensive chemotherapy. VPA monotherapy was targeted to reach serum concentrations of 50–100mg/ml. ATRA was added (80mg/m2/d in two divided doses, every other week) in some of the patients who did not respond or who relapsed. To enhance responses, we treated an additional 4 patients (2 sAML/MDS, 1 sAML/ET, 1 de novo AML) with VPA+ATRA from the start. Median age was 70 years (51–84). Median bone marrow blast count was 30% (10–80). 5 patients had only 10–15% marrow blasts but were included because they showed treatment failure or relapse after chemotherapy and were unable to receive further cytotoxic treatment. Median treatment duration was 99 days (20–396) for VPA and 79 days (18–339) for ATRA. Responses according to international working group (IWG, Cheson et al., 2003) criteria were observed in 5 patients (25%) on VPA monotherapy (4PR, 1CR). Of the responding patients two have ongoing responses (CR, PR) for 12 and 13 months, respectively. 1 patient reaching PR discontinued VPA when her physical condition had improved sufficiently to allow further chemotherapy. 1 patient relapsed after 2 months and was switched to VPA+ATRA, without response. 1 patient died of infectious complications. 8 additional patients showed stable disease without increases in peripheral blast count. Responses lasted for a median of 4 months (2–13). Among the 4 patients receiving VPA+ATRA from the start, 1 (25%) achieved PR. When he stopped VPA after 3 months because of side effects, he continued on ATRA, achieving a CRi (CR with incomplete recovery of platelets) lasting for 8 months. 4 of 14 nonresponders were switched to VPA+ATRA, but none of them showed a response. Response to VPA treatment was not associated with FAB subtype or karyotype. Median bone marrow blast count was 28 (13–45)% in responders, 30 (10–75)% in patients with stable and 41 (25–80)% in patients with progressive disease. Since our patients mainly had secondary AML, we also analyzed our results according to the proposals of the IWG for MDS (Cheson et al., 2000). Among patients receiving VPA monotherapy 1 patient had a major trilineage response. 2 patients showed a minor erythroid and one a minor neutrophil response. In the second group of patients one had a major erythroid response. Concerning side effects, VPA caused tremor in four cases, leading to cessation of treatment in two. Regarding ATRA, grade 1–2 skin toxicity was observed in 4, grade 1–2 gastrointestinal toxicity in 2, and pleural effusion in 1 patient. In summary, we observed responses according to IWG criteria in 25% of our patients (6/24). The best responses to VPA or VPA+ATRA in AML patients occurred in patients with low blast count, mainly in patients who showed relapsed or refractory disease shortly after intensive chemotherapy. These data indicate that VPA might be most effectively applied after or in addition to intensive chemotherapy.

本文采用制度主义研究路径,分析了国际法院在国际裁判和执行人权方面的作用,以及它在阐释和发展基本人权规则和原则方面的贡献。国际法院是联合国主要机关之一,也是其主要的司法机关,在国际裁判机制中享有重中之重的特殊地位。国际法院对阐释和发展人权法的贡献可以从三个角度来看:第一,在程序角度,它进一步推动了人权的可诉性。第二,在实体角度,它不仅分析了非殖民化背景下的民族自决权问题和一些公约项下的人权保护范围,还处理了人权公约义务的领土范围问题、国家责任归属问题以及国际不法行为的赔偿问题,为国家、国际组织及其机构、其他法律实体甚至个人制定了行为标准。第三,在制度角度,它确保了联合国人权问题报告员的独立性和不可侵犯性,对联合国大会和安全理事会在维护国际和平与安全方面的职能作出建设性的阐释,并对其遵守国际人权义务的情况进行监督。然而,一些案件体现出国际法院在审理人权案件时存在的管辖权漏洞和其他障碍,它对国际法中某些有争议的问题持谨慎立场。这意味着国际法院有时在实施人权保护方面可能施加的影响有限。 This article uses an institutional approach when discussing the role of the ICJ within the context of international adjudication and enforcement of human rights, as well as its contribution to the interpretation and development of key human rights rules and principles. As one of the main UN organs and its principal judicial organ, the ICJ enjoys a somewhat special position as first among equals. The Court’s contribution to the interpretation and development of human rights can be seen from three aspects. Firstly, from the procedural aspect, the Court furthered the justiciability of human rights. Secondly, from the substantial aspect, the Court has dealt not only with the right to self-determination within the decolonisation context and the scope of human rights protection under the convention, but also with the territorial scope of human rights treaties’ obligations, aspects of attribution of State responsibility and issues concerning reparations for internationally wrongful acts, which lays down standards of conduct for States, international organisations and their organs, other legal entities and even individuals. Thirdly, from the institutional aspect, the ICJ ensures the independence and inviolability of UN human rights rapporteurs, the constructive interpretation of the functions of the General Assembly and the Security Council in matters of international peace and security and their monitoring of compliance with international human rights obligations. However, several cases have highlighted the jurisdictional gaps and other obstacles to litigating human rights cases before the Court. These cases have also highlighted the Court’s guarded position on certain controversial issues under international law. It shows that the limitations to the Court’s potential impact on the enforcement of fundamental human rights.

Background and aims. The present study was carried out in the Department of Oral and Maxillo-Facial Surgery of the Sibiu County Emergency Clinical Hospital and aimed to evaluate the oral health status of patients who refer to this medical unit, identifying the presence and frequency of dental infections among these patients. The study also aimed to identify and characterize the comorbidities present in patients with these oral conditions. Finally, the study aims to highlight the degree of concern regarding the oral health status of patients and provide relevant information for dental health services and for the development of appropriate prevention and intervention strategies. Methods. The study was conducted retrospectively and descriptively, using the hospital’s electronic medical records. The total sample consisted of 1246 people, the inclusion criteria were: (a) patients admitted to the OMF Surgery Department; (b) outpatients who required hospitalization; (c) patients hospitalized under continuous hospitalization regime; (d) patients over 16 years of age; (e) patients diagnosed with infectious pathologies. After data registration, information on sex, background, distribution of diagnoses, associated pathologies of the patients and the existence of combined pathologies of the patients included in the study were taken into account. The duration of hospitalization of patients, the need and duration of antibiotic therapy, the frequency of use of combinations of antibiotics, and the number of antibiotics used in correlation with the duration of hospitalization were also analyzed. The data are presented as frequency and percentages. The combination of the most frequent associated pathologies was analyzed using an algorithm based on association rules and chord diagram was used for their visual representation. Statistical analyses were performed using IBM SPSS® (Statistical Package for the Social Science) version 20 and R software. Results. The total sample consisted of a total of 1246 people. From the preliminary analysis, the majority of people in the sample, 68.5%, did not suffer from infectious conditions, while 31.5% were affected. Most of the patients came from the urban environment (61.73%) and were male (54.34%). Frequent comorbidities included cardiac pathologies (11.99%) and diabetes (4.08%). These data indicate the poor state of oral health among patients who call on the OMF Surgery services. It is not an isolated case, but it reflects a pattern observed globally. Based on the results of the study, the hypothesis is validated that there is a poor state of oral health among the population, manifested by the increased incidence of dental infections. International studies confirm that dental infections are often complicated by the presence of comorbidities. Conclusion. The study concludes that patients hospitalized in the department of Oral and Maxillofacial Surgery face a poor oral health state, characterized by a high prevalence of dental infections. The presence of comorbidities and seasonal factors contribute to the aggravation of these infections. These results emphasize the need for preventive and educational interventions to improve the oral health of the population in this region, as well as the development of personalized treatment strategies.
