The night before her suicide, Anna Karenina has a strange nightmare about a muzhik, or former serf, who speaks French and is doing something with a piece of iron. Given the place of class in the novel, if mainly on the Levin side rather than the Anna side, critics of Tolstoy have said less than might have been expected about the simple fact that this is a wealthy woman dreaming uneasily about a poor man. This essay attempts an interpretation of the dream, which Anna shared, more or less, with Vronsky, relating it both to Anna Karenina as a whole and to the general issue of the marginal existence of the poor in novels by, for, and about their social superiors. Reference is made to Gabriel García Márquez’s One Hundred Years of Solitude (1967), John James Bezer’s Autobiography of One of the Chartist Rebels of 1848, Edmund Wilson’s 1942 novella, “The Princess with the Golden Hair,” and Jacques Rancière’s The Philosopher and His Poor.

Abstract Telomerase reactivation by acquisition of mutations in the TERT promoter (TERTp) region is a mechanism of tumorigenesis. The most common TERTp mutations are located in positions -146, -124, and -57 upstream the initiation codon. In non-malignant diseases, TERTp mutations only have been reported in patients with idiopathic pulmonary fibrosis (IPF) caused by germline mutations in telomere biology genes, that are also etiologic in a broader spectrum of diseases collectively named telomeropathies (such as IPF, aplastic anemia [AA], dyskeratosis congenita [DC], and cirrhosis). We screened blood from 136 patients with telomeropathies (median age=29 years; range, 1-76), 52 relatives (median age=40 years; range, 8-72), and 195 controls using a customized low-cost amplicon-based next-generation sequencing (NGS) assay for identification and quantification TERTp mutations. Patients had DC (n=21), AA (n=86), IPF with or without another telomeropathy-related phenotype (n=18), or other phenotypes (n=11). Inclusion criteria were telomere length (TL) below the 10th percentile of age-matched controls or a germline mutation in a telomere-related gene classified as pathogenic/likely pathogenic or of uncertain significance by the ACMG criteria. Patients' relatives were only studied if they carried the same germline mutation as the proband or had short telomeres, regardless of symptoms or evidence of disease. Patients with acquired AA (n=70), IPF (n=12), other inherited bone marrow failure (n=7), and acute myeloid leukemia (AML; n=106) were controls. All TERTp mutations identified by NGS were confirmed and tracked over time by droplet digital PCR. We identified the -124 or -146 mutations in leukocytes from 12 unrelated patients diagnosed with IPF (n=6), DC (n=2), or moderate AA (n=4). Five relatives also had the -146 (n=1), and -124 (n=4) mutations, all carriers of a germline mutation in telomere biology gene. The frequency of TERTp mutations was much higher in IPF patients compared to AA cases (33% vs. 4.6%; Fisher's exact test, P=0.0016). However, no difference in frequency of TERTp mutations among patients with IPF vs. marrow failure was observed (41% vs. 58%; Fisher's exact test, P>0.05), suggesting TERTp mutations occurred in both clinical presentations. MutTERTp clones positively correlated with age, as they were only present in individuals older than 18 years old and more frequent in those 60 to 80 years old. Also, TERTp mutations more frequently co-ocurred with germline TERT mutations (n=13) compared to mutations in TERC (n=2), RTEL1 (n=1), or DKC1 (n=2) (76% vs. 23%; Fisher's exact test, P=0.002). All germline variants were pathogenic or had some evidence of pathogenicity. MutTERTp clones size varied from 1.2% to 50% in total leukocytes and was at higher allele frequencies (VAF) in the granulocytic fraction from four patients. In serial samples (available for five patients), the mutTERTp clone size expanded over time, suggesting a selective growth advantage in comparison to unmutated hematopoietic cells. Despite that, mutTERTp clones did not associate with blood counts or telomere elongation; most subjects carrying a TERTp mutation, which is known to upregulate TERT expression, nevertheless had short or very short telomeres (15 out of 17 individuals). Six patients with mutTERTp clones (VAF ranging from 3-33% in myeloid cells) were treated with danazol for two years; four were responders and two were off-study after 3-6 months. In serial samples (available for two patients), the mutTERTp clone sizes decreased during danazol treatment while blood counts improved. After treatment, mutTERTp clones VAF increased. TERTp mutations were found in telomeropathy patients who had a germline variant in telomere biology genes but not in controls or patients with very short telomeres without germline variant in telomere biology genes. We have expanded the spectrum of non-malignant diseases associated with somatic TERTp mutations to DC, AA, and cirrhosis. Our data indicate that mutTERTp clones are specifically and randomly selected with aging in a marrow environment deficient in telomerase function, and mutTERTp selection did not associate with patients' peripheral blood counts, TL, and response to danazol treatment. TERTp emergence may be a useful clonal indicator for telomere dysfunction and may help to assess the pathogenicity of unclear constitutional variants in telomeropathies. Neuroleptic malignant syndrome (NMS) is a neurological emergency due to neuroleptic medications which is characterized by fever, muscle rigidity, autonomic instability, and altered mental status. Here we report a case of a 74 years old female who presented with complaints of fever, altered sensorium, restlessness, self-muttering, irritation, muscular rigidity, decreased mobility and diaphoresis. She was under medication olanzapine for schizoaffective disorder. It is a diagnosis of exclusion after detailed examinations and investigations. The patient was managed with bromocriptine and supportive measures. NMS may mimic acute encephalitis syndrome. Therefore, NMS should be included in differential diagnosis with presentation as in our case.

AbstractSelf-mutilation is often associated with psychiatric disorders. We describe here a 22-year-old male Indian with decreased sleep, aggressive behaviour, self-muttering, disorganised behaviour, frequent spitting, biting and self-mutilation; he bit off his right ring finger and left thumb (Van Gogh syndrome). Self-harm behaviour was frequently evidenced by family members resulting in various injuries. The patient was diagnosed with paranoid schizophrenia and was treated with anti-psychotics which resulted in a decrease in his behavioural disturbances along with treatment for his self-mutilation injuries. Here we discuss Van Gogh syndrome’s presentation of self-mutilation in paranoid schizophrenia and its implications.

In her poems, the poet was greatly influenced by those cultures, as was evident in her adoption of some ideas, taking a certain position from others, and rebelling against the culture of her Arab society and eastern traditions, and muttering at other times to the stagnation and lack of civility in our Arab culture.

Pirandello’s Six Characters in Search of an Author starts with a dramatically imbalancing scene. A stagehand who is hammering away on some parts of the stage for a play that we are apparently about to see is interrupted by a Stage Manager concerned about the rehearsal that is soon to start. The stagehand’s muttering, “I’ve got to have time to do my work, too,” is dismissed by the Stage Manager with, “After the rehearsal. Now, come on. Get all this stuff out of here and let me set up.” Seconds later, the same Stage Manager calls for the stage lights to be put up for a rehearsal that appears no longer a rehearsal, but the play itself, orchestrated by a Director (with capital ’D’). The reality of the rehearsal is countered by the dramatic illusion—or illusionary reality - of the play itself.

Interpersonal relationship skills are the ability of individuals to build relationships with other people. This research is motivated by the discovery of interpersonal relationship problems among students, such as being unable to refuse invitations from friends, isolated students, muttering, aggressive, and a lack of care for others. This study aims to test the effectiveness of assertiveness group format training to improve students' interpersonal relationship skills. This study used a quasi-experimental method with the non-equivalent control group design. The subjects of this study were students of class VIII SMP who were selected by purposive sampling technique. The instrument used was a scale of interpersonal skills with a validity of 0.535 and reliability of 0.881. Data analysis used the wilcoxon signed-rank test and kolmogorov smirnov. The results showed that the use of assertiveness training group format was effective in improving students' interpersonal relationship skills.

Schizoid personality disorder is a rare type of personality disorder that is characterized by restricted range of expression of emotions such as emotional coldness, detachment or flattened affectivity to almost each and everything in daily and natural activities. This was a case report about a 30 year old male from a rural area who presented with the complaints of being self absorbent, slothful, reluctant with self-laughing, self-muttering and odd behavior, having little interest in emotional reaction with poor social interactions and no interest in earning or pleasurable activities. On mental state examination his mood was found to be euthymic with restricted affect; decreased rate and rhythm of speech with impairment of judgement, abstract and insight. Other medical causes of such conditions were excluded. In addition to pharmacological treatment, psychological interventions were exercised and considerable improvement was noticed. Though no field or discipline’s historical vector presents itself as a strictly linear building of knowledge, the historical trajectory of Game Studies is problematic: certainly not linear, yet also not even multiplicious or rhizomatic. Instead, we are cyclical. Past debates often re-emerge, zombie-like, muttering the same arguments, often encased in binaries as endemic to our field as they are to the objects we study: unbridgeable disagreements on fundamental concepts; incompatible ontologies and epistemologies; incommensurability writ large. We view this as a chronic issue which has of late culminated in a crisis, exacerbated by changing institutional prerogatives championing multidisciplinary approaches and demands for “public impact”. This article takes a metaphysical approach, performing a meta-review to search for the root cause of our field’s cyclical nature. We identify and explore a key issue, namely our continuing status as pre-paradigmatic field, and ask questions designed to provoke ways forward, to provide more inflection points and fewer endless loops.

I must admit at the outset that I only studied economics for a year. At the end of the undergraduate year, I sat an exam on the subject and passed. It was, as I recall it, a close-run thing however.A number of us had already provoked the exam adjudicators by turning up late. Students were allowed into the exam hall up to twenty minutes after the exams had started, and this being the late 1960s, it was very cool to affect nonchalance by entering ten minutes or so late after everyone else had settled down. Such a late arrival not only gave the required appearance of casual unconcern, but also caused considerable disruption, especially if the latecomer was seated in the middle of a row of scibblers. Working one's way down the line of desks, loudly muttering ‘Excuse me’, ‘So sorry’, ‘Excuse me’, and so on was guaranteed to arouse the ire of diligent student and conscientious adjudicator alike.

Rationale: Salmonella is a common etiological agent behind the tropical fever syndrome in the Indian subcontinent. Its prevalence in India remains high due to a lack of proper sanitation services in large parts of the country. Its neuropsychiatric manifestations is rare and the understanding on their pathophysiology is still poor. Patient concerns: A 19-year-old male, presented with a 10-day history of altered mental status, high-grade fever and violent behaviour. 2 Days prior to admission, he developed decreased responsiveness and a muttering delirium with self-talking. Diagnosis: Coma vigil secondary to salmonellosis. Interventions: Intravenous ceftriaxone and dexamethasone. Outcomes: The resolution of the coma vigil and the associated Salmonella infection were observed; however, the patient developed residual mutism. Lessons: The atypical presentation of a globally obtunded state followed by mutism in typhoid coma in this case should be brought to the attention of clinicians worldwide. Additionally, the enduring speech limitations and potential psychiatric consequences may be linked to the prolonged duration of the infection.

ZusammenfassungÜber Staphylococcus(S.)-aureus-Infektionen beim Schwein wurde im Zusammenhang mit Septikämien, Mastitiden, Vaginitiden, Metritiden, Osteomyelitiden und Endokarditiden in der Vergangenheit mehrfach berichtet. Dieser Fallbericht beschreibt eine durch S. aureus hervorgerufene septikämische Erkrankung von Saugferkeln. Am dritten Lebenstag traten bei Saugferkeln eines Wurfes Zyanosen und ekchymale Blutungen auf der Hautoberfläche auf. Etwa die Hälfte der Tiere verendete, die andere Hälfte wurde zu einer anderen Sau gesetzt, verendete jedoch einen Tag später. Fremde, an die Muttersau angesetzte Ferkel zeigten bereits nach einem Tag ebenfalls die beschriebene Sympto matik. Blutuntersuchungen bei den Ferkeln ergaben eine Anämie, eine Thrombozytopenie und eine Leukopenie, sodass weder eine Septikämie noch eine isohämolytische Anämie ausgeschlossen werden konnte. Bei der Sektion wurden petechiale Blutungen auf Oberflächen der inneren Organe sowie Einblutungen in die Körperhöhlen festgestellt. Durch die bakteriologische Untersuchung von Sauenmilch sowie der Milz eines Ferkels konnte eine Infektion mit einem Methicillinresistenten S. aureus (MRSA CC398) nachgewiesen werden, die mit großer Wahrscheinlichkeit die Ursache für das septikämische Krankheitsbild darstellte. Die Differenzialdiagnosen zu dem klinischen Bild werden besprochen.

Scholars and researchers, such as Brockelman and Nicholson, associate the origin of Arabic literature, as well as other world literatures that also arose, with witchcraft or divination. According to Brockelman, the origin of Arabic poetry - this is witchcraft, and it arose from the muttering of sorcerers and their intonations. While Nicholson connects ancient Arabic poetry with divination, in his opinion, the soothsayer is the reference point (authoritative source) of the tribe, and they turn to him in their wars and reconciliations, as well as in the management of their affairs, and on his divination (opinion) is based on the whole tribe. In the pre-Islamic era, the Quraysh language prevailed in literature and poetry over other tribes because of its religious status and the position of its people among the Arabs. When the era of Islam and after it the epochs came into the present, the social character prevailed over the tribal character, poetry became the expression of the direction of one nation, instead of individual tribes. The music of poetry began to correspond to music that corresponds to the meaning that the poet aspires to, this is how the music of words and letters appeared, which has become an integral part of Arabic poetry and poetic poetry in the modern era.

IntroductionWe report on the case of a 15 year old young person with a known diagnosis of autism presenting with a rapid and acute regression in functional abilities, decline in expressive speech and bizarre posturing. The symptoms first started during lockdown (April 2020) with anxiety related to school work followed by urinary incontinence, insomnia, muttering to self and incongruent smiling. Initial medical investigations including MRI, lumbar puncture and 24hour EEG were inconclusive, so she was referred to Paediatric Liaison for assessment.ObjectivesWe demonstrate the value of a child psychiatry liaison service being involved with young people in an acute medical hospitalMethodsThis young person had a thorough psychiatric assessment.ResultsThrough daily psychiatric assessment and reviews with the young person, her parent, social care, wider community team, school and Paediatric Inpatient ward in order to expand on the understanding of the young person and develop a case formulation. She was started on oral Olanzapine 2.5mg which was gradually increased to 10mg OD with minimal improvement.ConclusionsChildhood Disintegrative Disorder (CDD or Heller’s Syndrome) is a rare pervasive disorder presenting as a loss of previously acquired skills after at least two years of normal development. Despite no longer being included in DSM-V, it is important for Psychiatrists to have a working knowledge of CDD and consider other differentials when assessing young people.DisclosureNo significant relationships.

Here, in the middle of the well-known simile that depicts Aeneas and Turnus as bulls fighting for territory and a herd (12.715–22), Vergil registers the reactions of the onlookers. Commentators and lexicographers disagree about what the heifers are doing, interpreting ‘mussant’ in different ways. Servius (followed by Conington–Nettleship) glosses the verb as ‘dubitant’. By contrast, Heyne offers the paraphrase ‘anxii expectant’, responding to the theme of fear in the two preceding cola: cf. ‘pavidi’ and ‘metu’. Forbiger's explanatory ‘tacite expectant’ stresses rather the note of silence introduced by ‘stat pecus omne metu mutum’. Lewis and Short (s.v., II) and Georges (s.v., 112) concur with Forbiger when they translate ‘mussant’ ‘expect in silence’ and ‘stumm harren’. Other authorities, however, underscore the verb's onomatopoeic sense. Julius Caesar Scaliger, for example, observes of Vergil's usage: ‘sane verbum factitium, neque absonum a bourn voce’. Accordingly, some older commentators interpret ‘mussant’ as a restrained form of ‘mugiunt’. Introductions: Graves’ disease is the most common cause of hyperthyroidism. Neuropsychiatric manifestations may occur in thyroid disease. As the premier clinical manifestation of Graves disease, psychosis is highly uncommon; it was reported in 1% of cases. The major neuropsychiatric manifestations of hyperthyroidism are acute psychosis, dementia, apathy, agitation, mania, delusional behavior, and hallucinations, especially in older people. Psychosis is highly rare as the first clinical symptom of a grave disease. Case: A 48-year-old female with grave disease symptoms had been taken to the emergency department with psychotic manifestations. She had a three-month history of increasing irritability, increasing irrational talking, staying awake most of the night, muttering, night-time wandering, and poor personal hygiene. She was admitted to the hospital, and the laboratory investigations showed an elevated FT4 and a decreased thyroid-stimulating hormone (TSH). The patient was started on propanolol 3 x 10 mg, propylthiouracil 3 x 50 mg tab, risperidone 2x2mg, trihexylphenidil 2x2mg, lorazepam 1x2mg, and Zyprexa injection once, which showed improvement in psychotic symptoms. Conclusion: Graves’ disease can present with neuropsychiatric manifestations such as psychosis, mania, or a combination of both. A detailed medical history and physical and psychiatric evaluation are necessary for diagnosing and giving adequate treatment. A detailed clinical evaluation, including thyroid function tests, could be recommended for all patients who showed psychotic symptoms. More research is needed to comprehend the pathophysiology underlying psychosis due to Graves’ disease, so that it is expected that the management can be carried out properly.

Context: Homeless persons lack socioeconomic support which is instrumental in affecting both their physical as well as psychological health. A number of destitute persons are often brought to specialized psychiatric centres in view of their dilapidating physical and psychological health.Aim: To study the prevalence and clinical presentation among the destitute persons admitted to a specialized psychiatric care centre.Materials & Methods: A total of 100 destitute persons admitted to a specialized psychiatric care centre were included in the study as a retrospective chart review. Demographic profile and clinical presentations were noted. The classification of psychiatric illnesses was done as per international classification of diseases (ICD)-10 classification. Data has been represented in frequencies (number) and percentages.Results: Age of patients ranged from 15 to 75 years. Mean age of patients was 40.70 ± 13.99 years. Majority (62%) were males and Hindus (57%). Poor personal hygiene (79%), abnormal behavior (52%) and self-muttering (44%) were the most common presenting complaints. On mental status examination, irrelevant speech (85%), restricted/perplexed affect (93%), impaired judgement (98.8%), impaired attention and concentration (90.7%) and impaired memory (80%) were the major findings. CNS and GI abnormalities were seen in 9% and 2% patients. Almost all (99%) had non-cooperative attitude, inadequate skin/nail care (94%). Majority (56%) had thin to very thin built and psychomotor agitation (55%). Unspecified nonorganic psychosis (44%) was the most common psychiatric illness followed by paranoid schizophrenia (10%) and organic delirium (6%). In 28% cases, the psychiatric illness could not be established clearly, and were kept under observation.Conclusion: Destitute persons have a huge burden of psychiatric illness that had affected their physical health too.

Background: Internship is the most important process that prepares a person for his profession. However, this process can also bring some problems in the context of human relations. Inner sound and forum are techniques that have been using in psycho-educational group works. Purpose: To manifest what man want to accomplish and how he perceives the recognition of the problematic situations. The general purpose of the study is whether the students of the Faculty of Education who attend the internship solve the difficult situations they encounter within themselves or try to resolve it through a group discussion. Materials and Methods: This study is conducted in Sinop University Education Faculty’ Elementary School education students. In this study, all data analysis is conducted using interviewing – a qualitative information gathering technique. A study group consisting of 50 university students from different age groups was formed. Two general divisions of personal and group is gathered into categories of obedience to authority and obeying the rules. The qualitative interview conducted. Results: The two general distinctions, personal and group, are divided into two categories: obedience to authority and obedience to rules. Among the qualitative data collection techniques performed, the interview can be grouped as “I cannot do it”, “they do not allow it”, “my friend did not want it”, “I was scared”. Conclusions: Playing to complain and to find solutions instead of muttering is done through interactive forum theater method. For this, it should be suggested that students who attend internship at faculties create a forum to share their experiences, conflicts and problems that they could not find solutions to.

This research was a qualitative research which aims to obtain a description of the profile of the thinking process of vocational high school students in solving the system problems of linear equations in two variables in terms of gender based on Polya's steps. The subjects of this study consisted of male and female students of class X who were capable of high, medium and low. The results shown that (1) when understanding the problem, LT reads the problem. LS is silent, reading slowly. LR read in a clear voice, silent while looking at the question, pointing to the question with a pen and answering doubtfully. PT reads the problem with a clear voice and answers doubtfully. PS reads the whole with a voice, moving the little finger, reading carefully and answering doubtfully. PR pauses while looking at the problem, puts his hand in the cheek, reads the whole question in a voice, and bites the tip of the pen and touches the head with the tip of the pen; (2) during planning, LT look the questions and answer doubtfully. LS reads repeatedly. LR looked at the other direction. PT looked at the question, smiled and answered with a little hesitation. PS answered hesitantly, smiled and covered his face. PR paused for a moment while looking at the problem. (3) when carrying out the planning, LT occasionally looks at the question. LS is silent, muttering a sign remembering a plan. LR writes down what is known for a long time. PT does it right away. PS remembering the previous material; (4) when checking again, LT retraces the work. LS traces the results of his work and writes the proof. LR traced the results. PT sees conclusions from work results. PS and PR retrace the results of their work.

Zusammenfassung Ziel: Direkter Vergleich einer alternativen Abferkelbucht mit einem konventionellen Kastenstand mit Fokus auf Tierwohl und -leistung. Material und Methoden: Über ein Jahr wurden in sieben Durchgängen Daten von 129 Sauen und deren 2109 Ferkeln (davon 1935 lebend geboren) erfasst. 65 Sauen (Versuchsgruppe, VG) waren in einer Abferkelbucht mit zu öffnendem Stand, 64 Sauen (Kontrollgruppe, KG) in einem Kastenstand eingestallt. Erfasst wurden der Body Condition Score (BCS) der Sauen sowie Verluste und Gewicht bei den Ferkeln. Die Beurteilung fand 9–10 Tage nach Einstallen der Muttersau (Beurteilungszeitpunkt 1 = BZP1) und 21 Tage später (BZP2) statt. In der VG wurde der Stand 5–17 Tage nach dem Abferkeln geöffnet. Das Versuchsprotokoll sah eine Öffnung nach Tag 7 vor, weshalb die Auswertung sowohl nach Tagen (Zeitraum [ZR] 1 von Tag 1–7, ZR2 ab Tag 8) als auch nach Standöffnung (offen: Gruppe Vo, geschlossen: Gruppe Vg) erfolgte. Ergebnisse: Die durchschnittlichen Ferkelzunahmen unterschieden sich nicht signifikant (VG: 5169,4 ± 787,9 g; KG: 5304,2 ± 873,3 g). Der BCS der Sauen zum BZP1 (VG: 3,30 ± 0,47; KG: 3,33 ± 0,52) war in beiden Gruppen annähernd gleich, während die Sauen der VG an BZP2 signifikant (p = 0,001) besser konditioniert waren (VG: 2,82 ± 0,54; KG: 2,50 ± 0,48). Die Gesamtverluste differierten zwischen VG und KG nicht signifikant. Die relativen Erdrückungsverluste waren von Tag 1–7 in der VG mit geschlossener Bucht signifikant (p = 0,016) geringer als in der KG. Ab Tag 8 zeigte sich in Gruppe Vo kein signifikanter Unterschied zur Gruppe Vg. Die durchschnittliche Anzahl lebend geborener Ferkel pro Sau betrug in der VG 15,1, in der KG 14,9. Schlussfolgerung und klinische Relevanz: Mit Fixierung der Sau in den ersten Lebenstagen der Ferkel erwies sich die getestete Abferkelbucht hinsichtlich der Produktionsdaten in diesem Betrieb als Alternative zum Kastenstand. Da Verluste nicht nur mit dem Haltungssystem verbunden sind, könnte eine Kombination dieser tierfreundlicheren Abferkelbucht mit einer anderen Genetik (weniger Ferkel/Wurf) ähnliche Aufzucht-ergebnisse erbringen wie das konventionelle System.

Background Gangstalking is a novel persecutory belief system whereby those affected believe they are being followed, stalked, and harassed by a large number of people, often numbering in the thousands. The harassment is experienced as an accretion of innumerable individually benign acts such as people clearing their throat, muttering under their breath, or giving dirty looks as they pass on the street. Individuals affected by this belief system congregate in online fora to seek support, share experiences, and interact with other like-minded individuals. Such people identify themselves as targeted individuals. Objective The objective of the study was to characterize the linguistic and rhetorical practices used by contributors to the gangstalking forum to construct, develop, and contest the gangstalking belief system. Methods This mixed methods study employed corpus linguistics, which involves using computational techniques to examine recurring linguistic patterns in large, digitized bodies of authentic language data. Discourse analysis is an approach to text analysis which focuses on the ways in which linguistic choices made by text creators contribute to particular functions and representations. We assembled a 225,000-word corpus of postings on a gangstalking support forum. We analyzed these data using keyword analysis, collocation analysis, and manual examination of concordances to identify discursive and rhetorical practices among self-identified targeted individuals. Results The gangstalking forum served as a site of discursive contest between 2 opposing worldviews. One is that gangstalking is a widespread, insidious, and centrally coordinated system of persecution employing community members, figures of authority, and state actors. This was the dominant discourse in the study corpus. The opposing view is a medicalized discourse supporting gangstalking as a form of mental disorder. Contributors used linguistic practices such as presupposition, nominalization, and the use of specialized jargon to construct gangstalking as real and external to the individual affected. Although contributors generally rejected the notion that they were affected by mental disorder, in some instances, they did label others in the forum as impacted/affected by mental illness if their accounts if their accounts were deemed to be too extreme or bizarre. Those affected demonstrated a concern with accumulating evidence to prove their position to incredulous others. Conclusions The study found that contributors to the study corpus accomplished a number of tasks. They used linguistic practices to co-construct an internally coherent and systematized persecutory belief system. They advanced a position that gangstalking is real and contested the medicalizing discourse that gangstalking is a form of mental disorder. They supported one another by sharing similar experiences and providing encouragement and advice. Finally, they commiserated over the challenges of proving the existence of gangstalking.

Introduction: Acute and Transient Psychotic disorder (ATPD) is defined by the ICD-10 as hallucinations, delusions, and/or senseless or nonsensical speech having an acute [1]. The distinguishing characteristic of ATPD is its abrupt onset. Second, there are characteristic symptoms present, and third, there is related acute stress [2]. Clinical Findings: Sleep disturbance, aggressiveness, muttering to self, irritability, irrelevant talks and loss of appétit, hearing of voices not heard by others, suspiciousness, increased talkativeness, increased energy and fearfulness. Mental Status Examination: Conscious, dressed appropriately, well groomed, standing, eye to eye contact initiated, non cooperative, activity normal, and the mood is exhausted, frustrated, the affect is irritable, guarded, and the flow of speech rate is rapid with moderate volume and responsive quality, Thought-flight of thoughts is evident, as are perceptual abnormalities- auditory hallucination, impaired social judgment, and full denial of sickness. Outcome: After treatment, the patient shows improvement. Irritability has reduced, sleeping pattern is improved, self muttering has stopped, irrelevant talks are less, and aggressiveness is reduced. Conclusion: Patient was admitted to Psychiatric Ward with a known case of Bipolar Affective Disorder and after Mental Status Examination he is diagnosed as Acute Transient Psychotic Disorder with complain of Sleep disturbance, aggressiveness, muttering to self, irritability, irrelevant talks. He improved after receiving adequate treatment, and the treatment was continuously ongoing until my last date of care.

The association of post-traumatic stress disorder (PTSD) with psychosis has important diagnostic and treatment implications. We present a case of a 13-year-old girl with illness duration of 3 months characterized by decreased social interaction, anger outbursts, decreased self-care, and separation anxiety with fearfulness and self-muttering behavior on serial observations pointing towards a possible psychotic disorder, which improved with risperidone 6 mg. Persistence of separation anxiety with possibility of trauma sequalae leads to diagnosis of PTSD. Attempt to taper antipsychotic resulted in the resurfacing of psychotic symptoms. She attained stable improvement with a combination of sertraline 150 mg and aripiprazole 20 mg.

Messianic delusion comprises a delusional system centered on the patient’s conviction that he has been selected by God for a special task or mission and has unique abilities to carry it out. The present case report describes a case of Treatment-Resistant Schizophrenia (TRS) with Messianic delusion in a 23-year-old unmarried male who presented to the Emergency Department with complaints of muttering to himself, decreased sleep, suspiciousness, socio-occupational dysfunction, aggressive behaviour, and a recent suicide attempt. Key interventions offered during the ward stay included rapid tranquilisation, antipsychotics, and benzodiazepines. The patient completely recovered with the concurrent use of clozapine and Modified Electroconvulsive Therapy (MECT). The patient received 16 sessions of MECT. He achieved both short-term and long-term remission with the therapy, as observed during regular follow-ups in the next six months. The present case emphasises the need for a multidisciplinary approach to the management, early detection, and adequate treatment of this challenging illness.

Background: Little is known about acute and transient psychotic disorders (ATPD), a diagnostic category introduced with ICD-10. Case Presentation: Acute transient psychotic disorder is a heterogeneous group of disorders characterised by the acute onset of psychotic symptoms such as delusion, hallucination and perceptual disturbances, and by the severe disruption of ordinary behaviour. Patient history: The Male patient 48 year old who was apparently admitted in AVBRH on date 08/05/2021 with chief complaint was Abnormal behaviour( taking clothes off in public ), irritability, aggressive, muttering and smiling to self from 10 days back. His parents once locked him in a room as they fear he might hurt him. He was taken to a faith healer in Pandarkawda twice by his parents. The Baba gave him some mysterious beats like bracelet to wear which eventually decreased his symptoms for 3 days but the symptoms persisted from the fourth day. This time, the Baba mixed a lemon juice, turmeric powder & kumkuma (a powder made from dried turmeric with a bit of slaked lime) and applied all over him eyes which severely inflamed & burnt his eyes. His eye injury elevated his psychotic symptoms which is why his parents took him to psychiatric OPD in AVBRH. Past History: Patient was apparently asymptomatic 2 yrs back. He was married to a woman of his parent’s choice. His marriage life was stressful and unhealthy. He was underestimated by his wife due to his low qualification and health problem. Clinical Finding: The patient has been undergone with various investigations like culture, blood tests, Physical examination and mental status examination symptomatically with antipsychotic agent and anticonvulsant such as Tab Olanzapine 10mg- HS, Tab Clonazepam 0.5mg – SOS. Medical Management: Antibiotic eye drop homatropine and eye ointment ciprofloxacin. Nursing Management: Administered fluid replacement i.e DNS and RL, eye care was done with betadine and Normal Saline solutions, eye care by administering eye drops and monitored all vital signs hourly. Conclusion: Patient was admitted to hospital with the chief complaint of muttering to self, irritability and eye injury, blisters, pus discharge from eyes and his condition was very critical and patient was admitted in AVBR Hospital ,immediate treatment was started by health team member and all possible treatment were given and now the patient condition is satisfactory.

ZusammenfassungEin Patient hatte die Zahlung der Arztrechnung eines Dermatologen verweigert, weil er die Exzision eines „bedenklichen“ Naevuszellnaevus am Rücken mit Deckung des Defektes mittels Verschiebelappenplastik als unangemessen groß bemängelte. Erst nach 5 Jahren klagte er auf Schadensersatz und Schmerzensgeld beim zuständigen Landgericht, das die Klage mit Hinweis auf die zwischenzeitlich eingetretene Verjährung ablehnte. Im Berufungsverfahren gab das Oberlandesgericht der Klage jedoch statt und stellte fest, dass es behandlungsfehlerhaft sei, wenn ein Hautarzt ein Muttermal von 5 mm im Durchmesser entfernt und die Wunde mittels einer Lappenplastik so verschließt, dass es zu einer Narbenlänge von 16 cm kommt. Auch beginnt die Verjährungsfrist nach einem Behandlungsfehler erst dann, wenn der Patient als medizinischer Laie Kenntnis von Tatsachen erlangt, aus denen sich ergibt, dass der Arzt von dem üblichen ärztlichen Vorgehen abgewichen war oder Maßnahmen nicht getroffen hatte, die nach ärztlichem Standard zur Vermeidung oder Beherrschung von Komplikationen erforderlich gewesen wären.Leitliniengerecht sollten Dermatologen in einem zweizeitigen Vorgehen klinisch suspekte Naevuszellnaevi eng umschrieben exzidieren und erst bei Vorliegen eines dermatohistologisch nachgewiesenen malignen Melanoms eine Nachexzision mit den empfohlenen Sicherheitsabständen je nach der Tumordicke nach Breslow von 1 – 2 cm und dann entsprechend erforderlicher Defektdeckung durchführen.

ZusammenfassungEine 21-jährige Patientin hatte seit Jahren ein pigmentiertes „Muttermal“ an der Schulter, das sie von einem Dermatologen mit einem Laser entfernen ließ. Ein Jahr später bildete sich erneut eine Pigmentierung an der gelaserten Stelle. Unter dem Verdacht auf ein malignes Melanom wurde die Läsion exzidiert; die Dermatohistologie bestätigte ein ulzeriertes malignes Melanom, Clark-Level IV, max. Tumordicke nach Breslow 2,4 mm. Nachdem eine Sentinel-Lymphnode-Biopsie der Axilla zunächst unauffällig war, stellte sich die Patientin nach wenigen Monaten wegen Kieferschmerzen vor, wobei sich eine osteolytische Metastase in der Mandibula zeigte. Das weitere Staging ergab multiple pulmonale, hepatische, mediastinale und weitere ossäre Metastasen. Unter einer Therapie mit Dabrafenib und Trametinib kam es nach kurzzeitiger Besserung zu einem nicht beherrschbaren Hirnödem mit Exitus letalis.In Anbetracht des tragischen Krankheitsverlaufes stellt sich die Frage nach der Vereinbarkeit der primären Lasertherapie von Pigmentmalen mit dem dermatologischen Facharztstandard. Risiken durch die Lasertherapie melanozytärer Naevuszellnaevi betreffen die mögliche Fehldiagnose eines malignen Melanoms als Naevus, welches nach Lasertherapie aufgrund des destruierenden Verfahrens nicht dermatohistologisch untersucht werden kann, die möglicherweise unvollständige Entfernung von Naevuszellen, die im Folgenden maligne entarten können, und Schwierigkeiten der klinischen Nachbeobachtung einer Läsion nach Laserung. Dysplastische Naevi sollten daher keinesfalls destruierend mit einer Lasertherapie entfernt werden; Leitlinien raten auch von einer Laserbehandlung melanozytärer Naevi ab und empfehlen, sollte einem entsprechenden Patientenwunsch doch gefolgt werden, zumindest eine Stanzbiopsie zur Diagnosesicherung durchzuführen. Ein Verzicht auf diese empfohlene diagnostische Absicherung könnte einen Befunderhebungsfehler mit der möglichen Folge einer Beweislastumkehr darstellen.

Introduction: An impaired relationship with reality is a hallmark of psychosis. It is a sign of a serious mental disorder. People with psychosis may have delusions or hallucinations. The way your brain processes information can be affected by psychosis. Losing touch with reality is caused by it. It's possible to see, hear, or believe things that aren't true. Psychosis is a symptom, not a disease. It can be caused by a mental or physical illness, substance abuse, or stress. This case report aims to provide nursing care for psychosis. The objectives of this case report are to identify psychosis symptoms by the nurses and to report immediately and to take action immediately and to prevent complications. Case Presentation: A 17-years adolescent male admitted at Acharya Vinoba Bhave Rural Hospital Sawangi (M) Wardha with Auditory Hallucinations in psychosis since 7 (2014) years was admitted to a psychiatric ward with a chief complaint of muttering and smiling to self, decreased interaction sleep disturbance, aggressiveness. The perpetuating factors are poor drug compliance. Conclusion: The patient underwent psycho pharmacotherapy and the patient taking the medications regularly and minimized the symptoms. The relatives were informed about the prognosis and finally patient was discharged. The nurses play a vital role at the bedside and take care of psychiatric patients with auditory hallucinations effective nursing interventions for an adolescent male with auditory hallucinations in psychosis enhancing and improving patient’s mental illness who experience auditory hallucinations in psychosis.

Introduction: Bipolar disorder (BD) is one of the foremost frequent chronic disorders in populations where depression is usually present; however, in chronic cases, it can raise the danger of death. Bipolar major Affective Disorder (BPAD) may be a common and recurrent psychiatric disorder that affects people everywhere on the planet. Tt's characterized by episodes of mania, hypomania, hyperactivity, and depression. Findings of Clinical: A 45-year-old woman from Chandrapur is admitted to the (AVBRH) Archarya Vinoba Bhave Rural Hospital Sawangi Meghe (Wardha), in a mental health ward(Psychiatric ward) with a known case of Bipolar major Affective Disorder. Her relatives brought her to AVBRH, where she is admitted with the chief complaint of low mood, muttering to herself, violent and abusive behavior. A case of bipolar major affective disorder is been identified. Both manic and hypomanic episodes are feasible. Bouncy, jumpy, or wired unusually. Increased activity, excitement, or energy. Feelings of satisfaction and self-assurance that are exaggerated (euphoria). Thyroid function testing and urine analyses were done with a A/B testing A simple randomized controlled experiment was done. A variety of procedures is used, including (CBC) complete blood count, (MRI)Magnetic Resonance Imaging, (CT) scan Computed Tomography, Mental Status Examination, and (ECT)Electroconvulsive Therapy with Psychotherapy. Therapeutic Intervention: Olanzapine, sodium valproate, capacitance, and cloze, chlorpromazine are examples of pharmacological therapy given. Conclusion: After treatment, the patient's manic episodes stop and her symptoms began to fade.
