Zusammenfassung. Bei sich stetig ändernden Lebensstilen und einer immer älter, aber auch dynamischer werdenden Gesellschaft kommt der Erhaltung und Gesundheit sowie der Verbesserung der Behandlung eine zentrale Rolle zu. Gleichzeitig revolutionieren die Entwicklungen in der Bioinformatik, den molekularen Biowissenschaften und den bildgebenden Verfahren die Medizin. Um die neuen Möglichkeiten, beispielsweise der Medizintechnik oder der personalisierten Medizin, voll ausschöpfen zu können, braucht es künftig Ärztinnen und Ärzte mit fundiertem Wissen in mehreren Disziplinen. Vertiefte Kenntnisse in den medizinischen wie auch in den technischen und biologischen Wissenschaften werden unerlässlich. Hier setzt der neue Bachelor Humanmedizin der Eidgenössischen Technischen Hochschule (ETH) Zürich seit Herbst 2017 an und verbindet medizinische mit technisch-naturwissenschaftlichen Aspekten der menschlichen Gesundheit. Er vermittelt neben klassischen medizinischen Inhalten auch Themen aus dem bioinformatischen und medizintechnischen Bereich. Die Weiterführung des Studiums erfolgt an einer der Partneruniversitäten in Basel, Lugano oder Zürich.

AbstractThe new Medical Licensing Regulations 2025 (Ärztliche Approbationsordnung, ÄApprO) will soon be passed by the Federal Council (Bundesrat) and will be implemented step by step by the individual faculties in the coming months. The further development of medical studies essentially involves an orientation from fact-based to competence-based learning and focuses on practical, longitudinal and interdisciplinary training. Radiation oncology and radiation therapy are important components of therapeutic oncology and are of great importance for public health, both clinically and epidemiologically, and therefore should be given appropriate attention in medical education. This report is based on a recent survey on the current state of radiation therapy teaching at university hospitals in Germany as well as the contents of the National Competence Based Learning Objectives Catalogue for Medicine 2.0 (Nationaler Kompetenzbasierter Lernzielkatalog Medizin 2.0, NKLM) and the closely related Subject Catalogue (Gegenstandskatalog, GK) of the Institute for Medical and Pharmaceutical Examination Questions (Institut für Medizinische und Pharmazeutische Prüfungsfragen, IMPP). The current recommendations of the German Society for Radiation Oncology (Deutsche Gesellschaft für Radioonkologie, DEGRO) regarding topics, scope and rationale for the establishment of radiation oncology teaching at the respective faculties are also included.

Die hier vorgestellte Definition professioneller Pflege wurde von Mitarbeiterinnen des Instituts für Pflegewissenschaft der Universität Basel in Zusammenarbeit mit der Expertengruppe des Projektes «Zukunft Medizin Schweiz» der Schweizerischen Akademie der Medizinischen Wissenschaften entwickelt. Ausgangspunkt waren bestehende Definitionen und eine Liste erwünschter Inhalte. Mittels Ansätzen der Grounded Theory wurden mögliche Inhalte bearbeitet und ein erster Entwurf geschrieben. In mehreren Feedback-Runden wurde dieser zur jetzt vorliegenden Definition verfeinert. Professionelle Pflege wird in zwei Kernsätzen definiert. Inhaltlich geht es um das Fördern und Erhalten von Gesundheit, das Vorbeugen von gesundheitlichen Schäden und das Unterstützen von Menschen in der Behandlung und im Umgang mit Auswirkungen von Krankheiten und deren Therapien. Ziel sind bestmögliche Behandlungs- und Betreuungsergebnisse sowie eine bestmögliche Lebensqualität in allen Phasen des Lebens bis zum Tod für die betreuten Menschen. Acht ergänzende Paragraphen beschreiben, an wen sich professionelle Pflege richtet, welche Aufgaben diese umfasst, welche Grundlagen wesentlich sind, wer mit welchen Mitteln professionelle Pflege umsetzt und in welchem Kontext diese ausgeübt wird. Die Definition hat einen pflegewissenschaftlichen Hintergrund und bezieht internationale Perspektiven und Entwicklungen ein. Eine Ergänzung beinhaltet weiterführende Überlegungen zur interprofessionellen Zusammenarbeit im Gesundheitswesen, zu Schwerpunkten in der Pflegeausbildung und zur zukünftigen Ausrichtung der professionellen Pflege. Sie postuliert eine optimale Zusammenarbeit der verschiedenen Berufsgruppen, um bestmögliche Ergebnisse für Menschen mit (potenziellen) Gesundheitsproblemen zu erzielen. Entwicklungstendenzen in der Gesellschaft und im Gesundheitswesen, insbesondere die Zunahme von betagten und chronisch kranken Menschen, lassen den Bedarf an professioneller Pflege ansteigen. Es gilt, nachhaltig wirksame Versorgungsketten und neue, bedürfnisgerechte Betreuungsmodelle zu entwickeln. Die Pflegeausbildung muss zukünftigen Veränderungen Rechnung tragen.

Abstract Complex technical, administrative, and clinical steps involving multiple staff members and various materials and other resources must be successfully completed for a blood transfusion to be given and properly managed. These factors are responsible for the direct and indirect costs of transfusion, which are often overlooked and vastly underestimated. In this study, the total cost of red blood cell (RBC) transfusion process in the surgical setting was calculated through activity-based costing (ABC) in a US hospital as a part of the Cost-of-Blood Consensus Conference (COBCON) project, sponsored by the Society for the Advancement of Blood Management (SABM; Shander et al, Transfus Med Rev.2005;19:66–78). Initially, main transfusion-related processes were identified by observing the “passage” of blood units (from ordering at local blood services to discarding empty bags and expired units) and recipients (for all major pre-, intra- and post-transfusion-related routines performed) through the system. Then, each main process was broken down into serial and parallel activity steps and the frequency (usage factor) and all required resources for each activity were determined. Local cost rates for all resources were retrieved. Specifically developed ABC software modules developed by Medizinische Gesellschaft für Blutmanagement, Laxenburg, Austria in collaboration with IDS-Scheer AG, Germany, based on ARIS Business Architect 7.02 (IDS-Scheer AG, Germany) were used to calculate the cost of each main transfusion process based on these data. Individual processes were condensed, multiplied by usage factor and flow-charted into the total transfusion process, generating the database for hospital‘s total process cost of transfusion. Direct and indirect overhead costs were also calculated and added. All data were validated by appropriate hospital personnel. In 2005, 2413 surgical patients were prepared for potential transfusion, of whom, 461 patients received a total of 1368 blood units (2.97 blood units per surgical patient; 1121 units transfused in ward, 132 in intensive care unit, 69 in operating room/post-anesthesia care unit and 46 in emergency room). RBC units were ordered and transported in bulk from blood services 156 times. Transfusion-related processes with highest usage factor for performing these 461 transfusions are listed in the table. A total of 21 mild or moderate and no severe transfusion reactions were reported. There was no transfusion-related litigation or patient reimbursement due to transfusion errors or complications in the studied period. Based on these data, the total cost of RBC transfusion per patient transfused in the surgical setting of this hospital was US$ 3433. The total cost of a unit of RBC was US$ 1,158 (2007 value), of which, indirect overhead, total transfusion process cost, weighted average acquisition cost and direct overhead cost per unit accounted for 40.6%, 34.0%, 21.5% and 3.9%, respectively. This study shows that the true cost of blood transfusion is much higher than the nominal value currently assigned to each unit of blood.
