Résumé :
Abstract. The assessment of seismic vulnerability in the Maghreb area is not yet fully investigated, especially considering the important connection between the vulnerability and traditional housing or construction local types. This paper proposes a methodology for the assessment of seismic vulnerability at territorial scale that is calibrated on the specific characteristics of historical settlements in the North of Morocco, mostly made of patio buildings in aggregate. The proposed methodology is structured to be extended and replicated in other similar contexts and is based on the Vulnerability Index approach proposed by the National Group for Earthquake Defence (Gruppo Nazionale per la Difesa dai Terremoti, GNDT). This method was applied to the Medina of Fes, UNESCO World Heritage site, which has suffered severe damages from historical earthquakes. The study allowed the realization of vulnerability and fragility curves, damage scenarios for different seismic intensity and loss scenarios. The proposed methodology was applied a second time to the case study after a simulated structural retrofitting intervention. The results of this second application allowed to validate the guidelines and the reinforcement strategies calibrated on local construction culture. The results of the vulnerability assessment are also presented through maps in the Geographic Information System (GIS) environment.

Résumé :
AbstractA crisis narrative dominates current public discourse about the artisanal sector in Fes, Morocco. Through historical and ethnographic research on Fessi crafts and craftspeople, this article elucidates the rise of the crisis narrative and its relationship to modernity and moral and political economy. Sustained analysis of the dialectical relationship between craftspeople's practical knowledge and public narratives highlights the existence of a “pool” of multiple and cross-cutting storylines from which the artisans draw to depict their profession. These storylines are marked by intertextual reverberations of precolonial Islamic philosophy, colonial Orientalist discourses, modern governance, and liberal and neoliberal economic policies. This article claims that the current, widespread crisis narrative took precedence over other storylines with the arrival of colonialism and modernity in Morocco. Drawing on the analytical framework of colonial modernity, it argues that from the colonial period to the postcolonial present, rulers and ruled alike have produced, maintained, and enhanced the crisis narrative, which is deeply rooted in modernity.

Résumé :
Urban spaces are the end-product of various power deliberations and different urban processes, mainly the top-down and bottom-up. This study investigates the role of the two urban processes in placemaking in historic Arabic-Islamic cities. The investigation demonstrates each urban process's role in creating the major mosque's spatial context in Mecca, Medina, Cairo, Fes, Damascus, and Tripoli of Libya. Aiming to clear some preconceived ideas -strongly affected by the Renaissance’s spectacular values- about Arabic-Islamic urbanism, this article studied major Arab-Islamic cities in qualitative and quantitative approaches. The oldest available cartographic materials are investigated locally concerning political values and changes. Street patterns, space typology, and major mosque spatial context in historic Arabic-Islamic cities are studied, relating each type of urban process to placemaking's symbolic value. Before the end of the 17th century, historic Arabic-Islamic cities adopted a bottom-up process by taking advantage of the community's right to assert the order of their built environment. The top-down process was limited mainly to the architectural level of authoritarian mosques and their context. The ontological freedom and the right to control the place shifted due to exogenous political influences from the public to the authorities. By adopting the Renaissance's urban values, the top-down urban process expanded its domination on architectural and urban levels, shifting the mosque's function from socioeconomic to an artistic value. This adaptation was achieved by promoting the mosque as an artifact and creating a spectacular spatial context.

Résumé :
The stability of iron ascorbate solutions was investigated, under both anaerobic and aerobic conditions, with the Fe2+ and Fe3+ indicators, respectively ferrozine and mimosine, at different pH values. The species present under the differing conditions were investigated by electron paramagnetic resonance (EPR) and Mössbauer spectroscopy and by gel-filtration chromatography. At physiological pH (6·8–7·4) iron ascorbate solutions rapidly form mononuclear chelatable Fe3+ species as reflected by indicator studies and EPR. Mössbauer spectroscopy fails to detect any Fe2+ species. EPR studies show a time-dependent decrease in rhombic Fe3+, particularly in oxygenated buffers, consistent with a conversion to polynuclear Fe. O2 uptake studies show that the conversion of Fe2+ to Fe3+ in Fe–ascorbate solutions at pH > 7·0 was accompanied by rapid O2 consumption but preceded depletion of ascorbate. Addition of high concentrations of mannitol (50–200 mM) reduces the O2 consumption and partly stabilizes the rapidly chelatable Fe form. Gel filtration studies show that the oxidation of Fe–ascorbate solutions at pH 7·4 is accompanied by an increase in the apparent relative molecular mass of the Fe, presumably due to Fe polymer formation. These studies indicate the inherent instability of Fe–ascorbate solutions above neutral pH and clearly have important implications in the use of ascorbate in studies of Fe physiology.

Résumé :
Abstract The photolysis of cyflufenamid (CFA) in different organic solvents and water under ultraviolet irradiation was investigated. The photolytic rate constant and photolytic half-life were measured for the different solutions. Factors influencing the photolysis of CFA were investigated, including initial concentration, types of solvent, pH, occurrence of catalyst (TiO2), and environmental substances (Fe3+, Fe2+, NO3−, NO2−). Photolysis of CFA followed first-order kinetics in various systems, and the photolytic rate of CFA decreased with increased initial concentration. Photolytic rates of CFA in different solvents were as follows: n-hexane > methanol > acetonitrile > ultrapure water > ethyl acetate. The pH had a significant effect on the photolysis of CFA, and the photolysis rate reached its peak at pH 9.0. NO2− and TiO2 had positive effects on the photolysis of CFA, while Fe2+ had an adverse effect. NO3− in aqueous solution had no effect on the photolysis of CFA. In addition, the rates of photolysis were accelerated at lower concentrations of Fe3+ (0.5–5 mmol L−1) and decreased at higher concentrations (10 mmol L−1). Moreover, a main photolytic product of CFA was confirmed to be N-cyclopropoxy-2,3-difluoro-6-(trifluoromethyl)benzamide, and cleavage of the amido bond was proposed to be the predicted photolysis pathway in n-hexane.

Résumé :
Objectives:Adsorption characteristics in aqueous solution were investigated to effectively remove heavy metals by the crystallization of FeS and reaction kinetics were analyzed to compare with heavy metal adsorption characteristics of zeolite. Methods:The adsorption characteristics of each media were analyzed using Langmuir adsorption isotherm, and the adsorption reaction kinetics of heavy metals (Pb, Cu, Zn) on zeolite and FeS media were analyzed by a pseudo-first or pseudo-second order reaction kinetics. Results and Discussion:The maximum adsorption capacity of zeolite was highest in Pb, and the adsorption preference was in the order of Pb>Cu>Zn. In the case of FeS media, Cu was found to have a higher adsorption capacity than Pb or Zn, and the adsorption preference was observed to be Cu>Pb>Zn. It was found that the adsorption mechanism is based on chemical adsorption (chemisorption) because adsorption of each heavy metal onto zeolite or FeS media is more suitable for pseudo-second order kinetics. Conclusions:It was found that the removal of heavy metals through FeS media has a different pattern from zeolite, and when comparing the adsorption capacity of each media, it was found that FeS media is more effective for Zn or Cu except Pb.

Untill the beginning of the 20 th century, fes was the biggest maroccan town. It was the political and cultural capital together with the biggest production and exchange center. It was a royal town, organized around a religious center with a commercial and productive area, and had a residential function. Its commercial areais well demarcated, organized into a hierarchy: central, secundary and elementary"souks". It is caracterized by specialized "souks", with the dominating network of shopkeepers, and remains exclusively a male business. This organization is explained by an authentic culture, a modest way of life and by self-production. Nevertheless, during the colonial area and after independance, fes is not the political and economical capital any longer. It becomes a "medina" and its trade expands; spatial expansion as well, with the rise of popular "souks" and a second network, the stallkeepers, where 23% are female salers. This is explained by the social and economical transformations of a whole society in mutation. Thus the "medina"tends to become an essentially commercial area, more and more diversified

Résumé :
Early and medieval Muslim culture exhibits a preoccupation with boundaries. Called "wall thought" in the dissertation, this preoccupation takes exemplary architectural form in the high-sided, labyrinthine structure of Fes' walled city, or medina. In Islamic law, it takes the form of The Book of Walls, a genre pertaining to the regulation of external and party walls within the medina environment. In the gendered aspects of Islam, it commonly takes the form of women's enclaustration and veiling. The locus of all these and other aspects of "wall thought", the medieval medina of Fes presents a sociologically interesting environment, but one whose nature has never been investigated. The following dissertation represents an attempt to correct this. Demonstrating the medina to be defined and determined by its walls, the dissertation uses the legal genre The Book of Walls to identify the meaning of a wall in medieval Muslim thought. Applying this meaning to Fes medina, the dissertation arrives at a conclusion concerning the nature of its environment. Lastly, the dissertation compares this deductively reached conclusion with one inductively reached by way of Fes' medieval historiography, including the foundation legend recorded there. As an interdisciplinary investigation, the dissertation comprises a number of subjects from within the academic field of Middle Eastern Studies, including Maghribi history and historiography, Islamic law, gender and urban studies. Its predominant concern is architectural, attentive to the spaces architecture bounds and people inhabit.

Baxevanis, Fotios. « Selection and analysis of predictive fed-state gastric biorelevant media ». Thesis, University of Bath, 2017. https://ethos.bl.uk/OrderDetails.do?uin=uk.bl.ethos.760886.

Résumé :
Dissolution testing is a tool towards prediction of dosage form behaviour under physiologically relevant conditions. The use of simple aqueous media dictated by the pharmacopoeias cannot predict the drug’s in vivo response as their physicochemical properties differ significantly from the complex environment of the gastrointestinal tract. To improve the predictive potential of drug dissolution, the development of more “biorelevant” media is essential. In particular, simulating drug dissolution in the gastric environment after administration of a meal still remains a challenge. Furthermore, except for the optimisation of the medium composition, the analysis of these usually complex heterogeneous media has also been challenging, due to the lack of a unified guideline for the selection of medium and analytical assay. The principal aim of the thesis was the development of a simple and robust analytical methodology, optimised on the basis of the drug’s physicochemical properties, interaction with excipients in a formulation and fat partitioning behaviour. The potential use of sample clean-up techniques including protein precipitation (PP) and solid phase extraction (SPE) was investigated. Optimised clean-up protocols were successfully used for extraction and quantification of drugs of a wide range of lipophilicity in milk-based fed biorelevant media. It was demonstrated that prior knowledge of the active ingredient’s physicochemical properties, such as log P, aqueous solubility, ionisation and protein binding can be used towards the selection of optimum extraction conditions. Moreover, the presence of certain excipients, when mixed with the APIs can significantly affect the methods’ efficiency and must thereforebe taken into consideration during analytical method development. Interactions between active ingredients and lipid part of the fed gastric content were also investigated, via development of biphasic “drug partition to fat” in vitro setups. The rate of drug partition to fat was successfully predicted based on the drug’s physicochemical properties and in vivo food effect. Finally, a simpler medium, alternative to Fed State Simulated Gastric Fluid (FeSSGF) was developed, requiring a less laborious extraction protocol. Overall, this thesis has provided useful insight on the critical aspects of fed gastric medium and analytical methodology development. It provides a point of reference for future work on better understanding on drug solubilisation in the gastric fed state and correlation with in vivo food effect.

I analysen används begreppet subjektspositioner ifrån diskurspsykologin.

The RSS protocol allows, among other uses, for portable media player devices to download files automatically from the internet. Although text RSS feed on portable media devices is currently limited, it is possible that text feeds will become a ubiquitous feature on future PDA/media crossover devices. With the increasing usage of video-enabled portable media players, video content on these devices will also gain strength in the consumer market. Through qualitative interviews this pilot study hopes to investigate usage of new media streams on portable devices, with additional focus on RSS feeds, content categorization and interface category customization. Do users prefer to display their content and RSS feeds by a descriptive category, or by the website from which the feed was retrieved? In this study we hope to gain insight into which style users prefer, if any.

Résumé :
The purpose of the work presented in this thesis is to investigate the fully and partially saturated behaviour of soils, behaviour that can be extended also to geomaterials like concrete. The physical - mathematical approach proposed within this manuscript is a coupled thermo-hydro-mechanical model, suitable for consolidation / subsidence analyses of unsaturated soils. This coupled formulation, can therefore be qualified as u – pw – pg – (T), by the introduction of basic state variables involved in the processes, that here are: the displacements field u, the liquid (water) pressure field pw, the gas (dry air and water vapour) pressure field pg, and eventually the temperature T that is involved on the modelling of non – isothermal process. Due to the coexistence of two different fluid phases, liquid and gaseous one, this model can be regarded as a multiphase approach to a deforming porous medium as proposed by Lewis and Schrefler in the framework of the hybrid mixture theory for porous media firstly presented by Hassanizadeh and Gray and Zienkiewicz et al. The evolution at macroscopic scale of the state variables above mentioned, in particular of pressures of both liquid and gas, is basically influenced by the microstructure of the material that characterizes the behaviour of a soil with relation on capillary effects and deformability. The physical approach proposed here is based on averaging techniques applied to the physical quantities that can be estimated in a representative elementary volume (REV) . With the addition of water retention functions that provide a description of the relation that exists among capillary pressure and the degree of water saturation, a complete set of fluid balance equations and mechanical and thermodynamic equilibrium equations can be obtained for the medium in a macroscopic scale. A coupled (thermo)-hydro-mechanical formulation u – p – (T) that deals with a fully saturated porous medium has been implemented with success in the past in the F.E. two-dimensional program PLASCON and its further extensions to three- dimensionality with PLASCON3D. The present work focused on the extension and upgrading of the relative simple single phase theory along with its numerical implementations, towards a more realistic multiphase description of the porous material, where voids may be filled up with both liquid and gas that interacts each other by mean of the concept of capillary pressure. An improved code PLASCON3D_PS based on the fully coupled u – pw – pg – (T) formulation and developed from previous versions has been realized. Due to the lack in literature of three-dimensional coupled numerical and experimental tests, some numerical results of benchmark tests and real case problems, that derive from two-dimensional domains, will be presented.

In this thesis, we have examined how social workers experience their profession with consideration to public attitudes and evidence-based practice. The study consists of six qualitative interviews with professional social workers in different municipalities and units. We have used a thematic analysis called Framework. The studies result showed that many social workers have experienced negative attitudes about the profession described in the media and from the public. All the social workers had heard positive things about the profession from their surroundings and felt pride in their work. Knowledge of evidence-based practice was found to vary between the social workers, but was primarily considered to be a support in everyday work.

Résumé :
The premise of a total knee replacement (TKR) is to alleviate pain and increase joint mobility. The expected 20 year life span of the knee insert exceeds that of the standard patient given traditional wear and tear. With more active lifestyles, greater wear rates and shorter life spans are predicted for tibial inserts. Greater wear rates increase the probability that a patient will require additional surgery later in life. This research provides a knee loading model to estimate the pressure loading distribution through Finite Element Modeling (FEM). The publicly available patient data was used to eliminate the geometry dependence in a surrogate model of distributed knee loads from an explicit Finite Element Analysis (FEA). The developed method based upon FEM and FEA, produced two notable results. The secondary results showed the FEM accurately estimated the medial and lateral contact forces during the stance phase. However, the primary result was that the surrogate model successfully interpolated the contact surface forces using results from the FEA without requiring any knowledge of the geometry of the contact surfaces.

Résumé :
A new multi-physics finite element model (FEM) on the vibration of the radial artery on the wrist is built by this study to predict vibration of wrist artery vessel vibration and then diastolic and systolic blood pressures. The FEM includes the sensor of gel capsule and strain-sensing electrodes, skin, bones and muscles. The vibrations of skin surface and the sensor module are successfully simulated by the established FEM. The resulted vibratory deformation of the sensor electrodes are further transformed to resistance variations to mimic realistic electronics of the front-end readout circuit via establishing a cross-discipline sub-FEM model. The established FEM can is particularly customized to varied ages, weights, heights, genders, and special cardiovascular diseases of users. The customization can be performed in a fashion of one-time calibration by medical staff and/or medical staff. With the customized FEM in high accuracy level, the diastolic and systolic pressures can be accurately predicted by the simulated output resistance variation. Based on simulation FEMs, the design of the pulse sensor is successfully optimized via the processes of Taguchi and numerical methods. The measurements are also conducted to a dozen of subjects. It shows that the designed novel blood pressure sensor is capable of sensing the blood pressure to require accuracy level of the error less than 10% as compared to commercial counterparts with a cumbersome cuff.

Résumé :
Abstract In paraplegia due to complete or incomplete spinal cord injury, the connection from the brain to muscles in the lower limbs is severed but the muscles that act on signals from the brain to produce limb movement remain functional. Functional electrical stimulation (FES), which is the application of electric potential across a muscle group to artificially cause the muscle to contract, is a method that can be used alone or in conjunction with an orthosis to produce a gait cycle. Such FES based walking machines or devices have been studied and designed for several decades. However, their application in everyday exercise is limited by several factors, one of which is the rapid onset of muscle fatigue produced in the stimulated muscle. In this work, simulations were conducted in Simscape Multibody to lay the groundwork for the design of a next-generation FES based walking machine powered by the quadriceps femoris muscle group of each limb. The stimulation of the quadriceps femoris muscle causes the knee to extend while some energy is stored by the orthosis, which uses the stored energy to complete the gait cycle. In this study, we have analyzed the power requirements of each step in the hybrid FES-orthosis gait cycle for different stride lengths. These requirements can help identify small step sizes to reduce the power required from the stimulated muscle.

To disseminate policy-relevant messages based on APRA research at country and regional levels, the Information and Communication and Engagement (ICE) team encouraged country teams to build relationships with the media from early on in the programme. This is not something with which APRA researchers had much experience and, subsequently, the approach was taken up by only a few teams. Nevertheless, with support from the ICE team, those teams that pursued active engagement with the media proved very successful. This report reflects on the APRA programme's engagements with the media to identify what went well and key lessons on what could have be improved.

Résumé :
A review of the East African Community (EAC) joint regulatory review process was conducted, registration timelines analyzed and key milestones, challenges and opportunities documented for the period of July 2015 to January 2020. A total of 113 applications were submitted for joint scientific review. Among these, 109 applications were assessed, 57 were recommended for marketing authorisation, 52 applications had queries to applicants and four applications were under review. A total median approval time for all products ranged from 53 to 102 days. The maximum time taken by a regulator to review the dossier was 391 days and the minimum time was 44 days. For applicants, the maximum time to respond to queries was 927 days and the minimum time was nine days. The total median time for granting marketing authorisation by the National Medicines Regulatory Authorities (NMRA) decreased from 174 to 39 working days in 2015 and 2019 respectively. However, not all EAC NMRA has granted marketing authorisation to all 57 products due to non-payment of applicable fees by applicants. Long regulatory approval timelines were contributed by limited capacity for timely scientific review of dossier by some NMRA, lack of online portal to share dossiersand assessment reports, delay in responding to queries by applicants and deficiencies in dossier. The metric tool and register of medical products submitted for joint scientific review had incomplete data. Challenges were identified and actions recommended to ensure regional regulatory system optimization, efficiency, transparency, sustainability and accountability.

Murray, Autumn, et David Biedenharn. Sediment supply from bank caving on the Lower Mississippi River, 1765 to present. Engineer Research and Development Center (U.S.), août 2022. http://dx.doi.org/10.21079/11681/45281.

Texte intégral
Résumé :
Bank caving rates and associated total sediment supply were calculated along the Lower Mississippi River from Cairo, IL, to Baton Rouge, LA, using historical maps between 1765 and 1992. Comparison of these maps reveals that the added sediment loads from bank erosion have greatly declined through time. During the Pre-cutoff period (1765–1931), the cumulative sediment supply from bank caving between Cairo, IL, and Baton Rouge, LA, ranged from about 395 million cubic yards/year (MCY/yr) to 535 MCY/yr with a median value of approximately 470 MCY/yr. The 1880-1931 period had the highest rate of sediment supply from bank caving with a median value of approximately 610 MCY/yr. By the 1990s, the sediment supply from bank erosion was essentially eliminated as a result of the revetment program, with significant erosion being observed at only a few locations, totaling approximately 17 MCY/yr. This equates to approximately a 96% reduction in the amount of total sediment being supplied to the channel system from bank erosion.

Melnyk, Olesia. MEDIA DISCOURSE AROUND THE FIGURE OF ORIANA FALLACHI AND HER JOURNALISM DURING 2017–2020. Ivan Franko National University of Lviv, mars 2021. http://dx.doi.org/10.30970/vjo.2021.50.11114.

Résumé :
The article analyzes the media discourse around the figure of Oriana Fallachi and her journalistic work during 2017-2020. The actual media image of the figure of Fallachi is highlighted, examples of positive and negative statements are given. It is substantiated why her journalism should be researched in various ways, taking into account other aspects of her work that are not related to Islamophobia. The subject of the study is critical texts in modern foreign media dedicated to the author’s work. The objective of the study is to outline the media discourse around the figure of Oriana Fallaci and her journalism during 2017-2020. The methodology. The following methods have been used in the process of scientific research: historical, comparative, systems analysis, content synthesis, and others. The main results. In total, we have analyzed eight materials in foreign publications, published over the past 3 years, as well as the two most famous biographies of Oriana Fallaci. Some of the most recent reviewed texts have been published in the last few months, reflecting the interest in the author’s journalism, her writing, and reporting. Therefore, we see the need for further tracking and analysis of this body of texts. Conclusions. Critics of Fallaci express polar views that are not all negative. Authors recommend quite cautiously her texts for reading, emphasizing their positive aspects. Both Fallaci’s biographies are also not entirely complementary: some aspects of her work are glorified, others are condemned. We managed to find general tendencies in the criticism of Oriana Fallaci’s journalism. These include accusations of xenophobia and Islamophobia, uncompromisingness, lack of political correctness, and moral value. The authors emphasize, at the same time, the openness and directness that bribe the reader, patriotism and honesty, strength of spirit and firmness of position. Significance of the research. The analysis of the latest criticism reveals what kind of media image Fallachi’s figure has today, and gives the possibility to research it for demonization and one-sided coverage. This is important not only for thorough research of the author’s work but also for understanding how the modern world perceives journalism, which is contrary to the generally accepted principles of political correctness, journalistic ethics, and humanity.

Résumé :
As a student undertaking a Longitudinal Integrated Clerkship (LIC)1 based in a GP practice in a rural community in the North of Scotland, I have been lucky to be given responsibility and my own clinic lists. Every day I conduct consultations that change my practice: the challenge of clinically applying the theory I have studied, controlling a consultation and efficiently exploring a patient's problems, empathising with and empowering them to play a part in their own care2 – and most difficult I feel – dealing with the vast amount of uncertainty that medicine, and particularly primary care, presents to both clinician and patient. I initially consulted with a lady in her 60s who attended with her husband, complaining of severe lower back pain who was very difficult to assess due to her pain level. Her husband was understandably concerned about the degree of pain she was in. After assessment and discussion with one of the GPs, we agreed some pain relief and a physio assessment in the next few days would be a practical plan. The patient had one red flag, some leg weakness and numbness, which was her ‘normal’ on account of her multiple sclerosis. At the physio assessment a few days later, the physio felt things were worse and some urgent bloods were ordered, unfortunately finding raised cancer and inflammatory markers. A CT scan of the lung found widespread cancer, a later CT of the head after some developing some acute confusion found brain metastases, and a week and a half after presenting to me, the patient sadly died in hospital. While that was all impactful enough on me, it was the follow-up appointment with the husband who attended on the last triage slot of the evening two weeks later that I found completely altered my understanding of grief and the mourning of a loved one. The husband had asked to speak to a Andrew MacFarlane Year 3 ScotGEM Medical Student 2 doctor just to talk about what had happened to his wife. The GP decided that it would be better if he came into the practice - strictly he probably should have been consulted with over the phone due to coronavirus restrictions - but he was asked what he would prefer and he opted to come in. I sat in on the consultation, I had been helping with any examinations the triage doctor needed and I recognised that this was the husband of the lady I had seen a few weeks earlier. He came in and sat down, head lowered, hands fiddling with the zip on his jacket, trying to find what to say. The GP sat, turned so that they were opposite each other with no desk between them - I was seated off to the side, an onlooker, but acknowledged by the patient with a kind nod when he entered the room. The GP asked gently, “How are you doing?” and roughly 30 seconds passed (a long time in a conversation) before the patient spoke. “I just really miss her…” he whispered with great effort, “I don’t understand how this all happened.” Over the next 45 minutes, he spoke about his wife, how much pain she had been in, the rapid deterioration he witnessed, the cancer being found, and cruelly how she had passed away after he had gone home to get some rest after being by her bedside all day in the hospital. He talked about how they had met, how much he missed her, how empty the house felt without her, and asking himself and us how he was meant to move forward with his life. He had a lot of questions for us, and for himself. Had we missed anything – had he missed anything? The GP really just listened for almost the whole consultation, speaking to him gently, reassuring him that this wasn’t his or anyone’s fault. She stated that this was an awful time for him and that what he was feeling was entirely normal and something we will all universally go through. She emphasised that while it wasn’t helpful at the moment, that things would get better over time.3 He was really glad I was there – having shared a consultation with his wife and I – he thanked me emphatically even though I felt like I hadn’t really helped at all. After some tears, frequent moments of silence and a lot of questions, he left having gotten a lot off his chest. “You just have to listen to people, be there for them as they go through things, and answer their questions as best you can” urged my GP as we discussed the case when the patient left. Almost all family caregivers contact their GP with regards to grief and this consultation really made me realise how important an aspect of my practice it will be in the future.4 It has also made me reflect on the emphasis on undergraduate teaching around ‘breaking bad news’ to patients, but nothing taught about when patients are in the process of grieving further down the line.5 The skill Andrew MacFarlane Year 3 ScotGEM Medical Student 3 required to manage a grieving patient is not one limited to general practice. Patients may grieve the loss of function from acute trauma through to chronic illness in all specialties of medicine - in addition to ‘traditional’ grief from loss of family or friends.6 There wasn’t anything ‘medical’ in the consultation, but I came away from it with a real sense of purpose as to why this career is such a privilege. We look after patients so they can spend as much quality time as they are given with their loved ones, and their loved ones are the ones we care for after they are gone. We as doctors are the constant, and we have to meet patients with compassion at their most difficult times – because it is as much a part of the job as the knowledge and the science – and it is the part of us that patients will remember long after they leave our clinic room. " Since 2010, there has been an exponential growth in health data and health information technologies, such as electronic health records (EHRs), and AI-enabled medical tools. Despite the growth and investment in these technologies, they have had few positive effects on health outcomes, especially for marginalized populations. This review begins by addressing common rhetorical and ethical responses to inequities in health technologies, such as the digital divide and data bias frames. It then problematizes both approaches before proposing that examining racialization, or the creation and circulation of racial hierarchies, can contribute to a more comprehensive framework for facilitating health equity in health information technology."

Résumé :
This report presents a statistical analysis of the Regulated Substances Library (RSL) developed by the Illinois Department of Transportation. The RSL is comprised of surficial soil chemistry data obtained from rights-of-way subsurface soil sampling conducted for routine preliminary site investigations. The 3.7-million-record RSL database is compared with four independent studies of inorganic soil constituents of naturally occurring soils in Illinois. A selection of 22 inorganic soil analytes are examined in this study: Al, Sb, As, Ba, Be, Cd, Ca, Cr, Co, Cu, Fe, Pb, Mg, Mn, Hg, Ni, K, Se, Na, Tl, V, and Zn. RSL database summary statistics, mean, median, minimum, maximum, 5th percentile, and 95th percentile, are determined for Illinois counties and for recognized environmental concern, non-recognized environmental concern, and de minimis site contamination classifications. The RSL database at a 95% confidence level is compared with current and proposed thresholds for defining naturally occurring soil concentrations for the selected analytes. The revised thresholds proposed by Cahill in 2017 are predominantly larger than the current standards found in the Tiered Approach to Corrective Action Objectives rules and are in better agreement with observed distributions of soil concentrations for both naturally occurring and RSL soils. A notable exception is antimony (Sb), for which Cahill proposed a reduced threshold similar in magnitude to the median for many Illinois Department of Transportation districts.

This report presents detailed histograms of data from the Regulated Substances Library (RSL) developed by the Illinois Department of Transportation (IDOT). RSL data are provided for state and IDOT region, IDOT district, and county spatial subsets to examine the spatial variability and its relationship to thresholds defining natural background concentrations. The RSL is comprised of surficial soil chemistry data obtained from rights-of-way (ROW) subsurface soil sampling conducted for routine preliminary site investigations. A selection of 22 inorganic soil analytes are examined in this report: Al, Sb, As, Ba, Be, Cd, Ca, Cr, Co, Cu, Fe, Pb, Mg, Mn, Hg, Ni, K, Se, Na, Tl, V, and Zn. RSL database summary statistics, mean, median, minimum, maximum, 5th percentile, and 95th percentile, are determined for Illinois counties and for recognized environmental concern, non-recognized environmental concern, and de minimis site contamination classifications.
