Résumé :

The aim of the research described here was to compare different methods of body temperature (BT) measurements in dairy cows. It was hypothesised that reticular temperature (RET) values reflect the physiological status of the animals in an equivalent way to rectal (RT) and vaginal (VT) measurements. RT, VT and RET temperatures of twelve lactating Holstein–Friesian cows were measured over five consecutive days in June and October 2013. While RT and VT were manually measured three times a day, RET was automatically recorded at 10 min intervals using a bolus in the reticulum. For comparison with RT and VT, different RET values were used: single values at the respective recording times (RET-SIN), and mean (RET-MEAN) and median (RET-MED) values of 2 h prior to RT and VT measurements. Overall, body temperatures averaged 38·1 ± 0·6, 38·2 ± 0·4, 38·7 ± 0·9, 38·5 ± 0·7 and 38·7 ± 0·5 °C for RT, VT, RET-SIN, RET-MEAN and RET-MED, respectively. RT and VT were lower than all RET measurements, while RET-SIN and RET-MED were higher than RET-MEAN (P < 0·001). RET-MEAN and RET-MED values were higher in the morning, whereas RT and VT were greatest in the evening (P < 0·001). Overall, records of RT and VT were strongly correlated (r = 0·75; P < 0·001). In contrast to RET-SIN and RET-MEAN, RET-MED was higher correlated to RT and VT. In June, coefficients were higher between all methods than in October. Relation of barn T to RT and VT was stronger when compared to RET measurements. RET-SIN was higher correlated to barn T than RET-MEAN or RET-MED. Correlation between VT and barn T was strongest (r = 0·48; P < 0·001). In summary, RET-MED showed highest correlation with VT and RT. However, single RET measurements (influenced by water or feed intake) can lead to extreme variations and differences to single VT and RT values.

The landmark paper discussed in this chapter, published by Bach et al. in 1988 is a Danish paper that describes a study where patients who were to undergo lower-limb amputation received either preventive, preoperative epidural analgesia for 72 hours before the amputation, or systemic analgesia. At 6 and 12 months post-operatively, all patients in the epidural group were pain free, while 38% and 27%, respectively, in the control group had phantom limb pain. The study has been criticized for a number of points including the pseudorandomization by year of birth, the lack of any blinding, and the small number of patients used in the study (only 25 patients overall).

Adjustment disorder is the most common psychiatric diagnosis in the armed forces, with a prevalence of 25–38% in those seen in the military psychiatric services, although PTSD has received far more attention in that setting. The military environment is characterized not just by the risk of exposure to major stressors during deployments, but also by a disciplined environment in which individuals lose some of their abilities to make choices regarding their environment or changes in their social milieu. It is also characterized by stoical attitudes that discourage displays of mental suffering. Young people, removed from the buffering effects of social support in their home environment, may not find alternative sources of confiding support readily available. Problem-solving and stress management strategies are the current main ingredients of management, and the condition carries a good prognosis for a return to work. However, there is a paucity of published research work.

Diagnostic reasoning is presented along the lines of the third edition of the International Classification of Headache Disorders. This section provides key knowledge on the pathogenesis and natural history of epididymitis and scrotal abscess, as well as providing a framework for the assessment, investigation, and management of patients presenting with these conditions. The information provided links to the knowledge and clinical skills requirement of Intermediate stage topics ‘Urinary Tract Infection’ and ‘Emergency Urology’, within the syllabus of the United Kingdom (UK) Intercollegiate Surgical Curriculum Project (ISCP) Urology Curriculum (https://www.iscp.ac.uk/). There is a lack of data concerning the incidence and prevalence of these conditions. Previous estimates from UK general practice suggested incidence rates of epididymo-orchitis of 38/10,000 male person-years. A recent study of first presentations of epididymo-orchitis in UK general practice has reported a lower incidence of 21/10,000 male person-years in 2007. However, the true figure is likely to be higher, given that many patients will present directly to urgent care facilities.

I detta kapitel tittar vi närmare på 38 (20%) av de svenska licentiatuppsatser och doktorsavhandlingar som publicerats mellan 1919 och 2017 och samlat ihop forskningsresultat. Först ger vi en idé om vad som tas upp i uppsatserna, och sedan kategoriserar vi dem utifrån vår tolkning av Habermas kunskapsintressen. Denna analys visar att studierna har olika ändamål och att dessa förekommer med olika frekvens. Studier som vill föreskriva innehåll eller undervisningsmetod —det teknisk-rationella intresset— är väl representerade, och det gäller också studier som vill förstå sammanhang —det historisk-hermeneutiska intresset. Däremot är studier som siktar mot att avtäcka det förgivettagna —det emancipatoriska intresset— inte lika vanliga. Detta resultat ger en indikation om vilka idéer om skola, matematik och undervisning som styr, eller åtminstone avspeglas i, forskningen. Vi diskuterar avslutningsvis hur viktigt det är att förhålla sig till vilka intressen och antaganden som styr ett forskningsresultat, om man ska relatera det till skolans praktik.

Post-transplant lymphoproliferative disease (PTLD) is a disorder of lymphoid proliferation seen in recipients of solid organ or haematopoietic transplants as a consequence of immunosuppression. A spectrum of disease is recognized ranging from non-malignant polyclonal proliferation of B cells to monoclonal proliferation of B or T lymphocytes which have features in common with lymphomas. Epstein–Barr virus (EBV) is associated with a majority of cases although it is not a universal feature. Treatment with anti-CD20 antibody in addition to reduction in immunosuppression has become the most common treatment approach.

Résumé :

Recombinant tissue-type plasminogen activator (rt-PA) was given to 34 patients with acute massive pulmonary embolism of less than five days and with an angiographic Miller index greater than 15. The regimen was 50 mg rt-PA over 2 hours followed by repeat angiography, and, if the Miller index was judged still to be above 15, by an additional dose of 50 mg over 5 hours. Heparin was given in a bolus of 5000 IU followed by 1000 IU per hour. The rt-PA preparation (Boehringer Ingelheim GmbH, G 11021) contained mainly two-chain rt-PA. The infusion route was at random in a peripheral vein (IV) or in the pulmonary artery (PA). Pulmonary angiographs were assessed blindly by a panel of five radiologists according to the protocol developed by Miller (Br Med J 1971). 19 patients were given rt-PA via the pulmonary artery, 15 patients received rt-PA intravenously. Eleven patients were postoperative (PA 9, mean 13± ]] SD days, IV 4, 7±3 daysSixteen patients (66%) received two infusions (PA 14; IV 8). The Miller index decreased from 25 ± 3 to 22 ± 6 (−12%) in the PA group and from 26 ± 2 to 22 ± 5 (−15%) in the IV group after the first infusion. In the patients who received a second infusion, the Miller index decreased to 16 ± 6 (−38%) after 100 mg in the PA group and to 16±;6 (−38%) in the IV group. The mean pulmonary artery pressure decreased from 30 ± 7 to 22 ± 6 (−27%) and further to 14 ± 5 (−53%) in the PA group and from 29 ± 7 to 22 ± 9 (−24%) after 50 mg and to 13 ± 5 (−55%) after 100 mg in the IV group. All these differences are significant (p < 0.01)Fibrinogen levels dropped 46% from baseline after 50 mg and 66% from baseline after 100 mg. Bleeding occurred in 16 patients, 5 of whom had recent surgery (mean 8 days, range 2-13)This pilot trial indicates that a prolonged infusion of rt-PA over 7 hours (100 mg) is superior to a single infusion over 2 hours (50 mg) and that infusion in the pulmonary artery does not offer a significant clinical benefit over the intravenous route. In case of life-threatening massive pulmonary embolism, treatment with rt-PA could be envisaged as early as from the third day after major surgery

Résumé :

INTRODUCCIÓN La amputación en ocasiones es el único tratamiento ciertos casos. El dolor del miembro fantasma (DMF) aparece tras la cirugía, presente en el 60-80% de los pacientes. Los síntomas incluyen dolor ardiente y punzante además de cambios de temperatura. La pérdida de un miembro es un estresor importante por las limitaciones y el cambio en el estilo de vida que supone. Es supone un riesgo para desarrollar un trastorno mental debido a los sentimientos de pérdida, el autoestigma y las dificultades para adaptarse a su discapacidad. A raíz de un caso atendido en la unidad de agudos y la urgencia de un varón con DMF así como abuso de opioides de prescripción e ideación suicida se pretende realizar un revisión sobre el tema. MÉTODOS Se realizó una búsqueda bibliográfica en Pud Med. RESULTADOS Hay más de 25 tratamientos disponibles. El abordaje farmacológico donde se usan antidepresivos tricíclicos como la amitriptilina. Las técnicas coadyuvantes como la estimulación nerviosa transcutánea, estimulación magnética transcraneal o terapia en espejo. Como última opción la cirugía realizándose neurectomía, bloqueo nervioso o reinervación muscular dirigida. La prevalencia de enfermedad mental es del 32% - 84%. En una muestra de pacientes el 72.1% presentaba trastorno depresivo mayor, de estos un 30.5% presentaba ideación suicida. El 20.3% presentaba TEPT. El 34% tenía prescritos opioides previos a la operación. El 55% mantuvo el uso de estos tras la cirugía, este grupo presentaba una mayor prescripción de benzodiazepinas (38%). Los factores que se asocian con mayor uso de opioides son: menor edad, sexo masculino, uso previo de benzodiacepinas y analgésicos y la amputación de miembros inferiores. CONCLUSIÓN El DMF es una entidad compleja que requiere un manejo multidisciplinar para lograr la remisión del dolor y el tratamiento de la comorbilidad psiquiátrica permitiendo una mejoría de la calidad de vida.
