The region that later comprised the kingdoms of Denmark, Norway, and Sweden was Christianized between 900 and 1200. A change from oral to written laws apparently took place first in twelfth-century Norway and Iceland, although the surviving legal manuscripts are some centuries later. Danish provincial laws were compiled c.1200–50 and the Swedish provincial laws only later. In all three Scandinavian kingdoms, royal and ecclesiastical statutes preceded the compilation of provincial laws. Precocious legal unification of the realms of Norway and Sweden was reached by nationwide law in 1274 (Norway) and Sweden (c.1350), supplemented in both kingdoms by town laws. In Denmark, the provincial laws remained in force until the 1680s. Roman law influences came mostly through canon law. Continental legal influences were also transmitted from Germany (feudal and town law). The universities of Uppsala and Copenhagen (1470s) had relatively little impact, clerics mostly studying law at Continental universities.

Rest is a cornerstone of concussion management. Unfortunately, however, there has been very little research on the definition of rest and how long to rest. The purpose of this chapter is to review the rationale for rest and the published research on rest following concussion. It is concluded that too much rest can be counterproductive, and even harmful in some cases. In the absence of specific evidence to guide clinical recommendations, it seems reasonable to recommend a brief period of rest, followed by a gradual return to normal daily routines.

When a majority party works on normal legislation, it faces a collective action problem of sincere voting, and must prevent legislators from centrist districts from voting against noncentrist legislation. From 2011 through 2016, though, Republican Party leadership faced a different challenge, and leaders were pitted against the extremists in their caucus. This occurred because of a change to the legislative agenda resulting from the combination of extreme polarization and divided government introduced by the 2010 election. With no incentive to work on normal legislation, the agenda did little but avoid reversion points, like debt ceiling breaches, which the extreme elements in the caucus actually found acceptable. Speaker Boehner was forced to solve a new collective action problem, then, convincing a group of Republicans to join with Democrats on bipartisan deals to avoid these reversion points. While historically unusual, the dynamic is what would be expected from the unified model.

Most research on information and communications technology (ICT) accessibility and innovation for persons with disabilities, whether in the fields of law, tech, or development, has focused on developed regions (“Global North”) rather than developing parts of the world (“Global South”). The goal of this book is to increase awareness of ICT accessibility in developing areas, under three common themes. First, innovations created in developing states often get little attention, even though they are frequently less resource-intensive, and therefore more sustainable, than corresponding Global North solutions. Second, when Global South countries evolve their technology infrastructures (as many are doing now), it is important to avoid barriers to equal access for people with disabilities. Third, Global North design, development, and implementation techniques often will not transfer well to the Global South, and should not be applied without thought. Three international legal and policy initiatives ensuring accessibility and equal availability of ICT in developing areas are discussed: the United Nations Convention on the Rights of Persons with Disabilities, The Marrakesh Treaty to Facilitate Access to Published Works for Persons Who Are Blind, Visually Impaired or Otherwise Print Disabled, and the Sustainable Development Goals. This book brings together a unique combination of authors with diverse disciplinary backgrounds (technology, law, development, and education), from non-governmental organizations that are part of the public zeitgeist (the World Wide Web Consortium and Benetech), significant United Nations entities (the World Bank and G3ict), universities in the developing world (Pakistan and Uganda) and the developed world (the United States and Norway), and Global North industrial labs innovating in the Global South (Microsoft Research, India), among others.

This topic requires a little bit of linear algebra when dealing with matrices, as well as an understanding of differential equations. This chapter continues the examination of Bonhoeffer’s first phase of resistance through exposition of “The Church and the Jewish Question,” presenting the second kind of word-resistance, the directly political word of the church (type 4). It is this type of resistance that Bonhoeffer describes as “jam[ming] a spoke in the wheel itself” or “seiz[ing] the wheel itself.” The necessity for this directly political word arises when the state “unscrupulously” governs with “too little” or “too much” order. The logic of this word is best captured by what Bonhoeffer elsewhere calls a concrete commandment. This is the church’s temporary intervention into the political exercise of the law, which under normal conditions would fall under the authority of the properly functioning state.

There are four major sex chromosome abnormalities due to complete aneuploidy. Otherwise unassisted, infertility is practically inevitable in XXY Klinefelter syndrome and 45,X Turner syndrome. The other two conditions, XXX and XYY, apparently have little effect on fertility; furthermore, they are not discernibly associated with any increased risk for chromosomally abnormal offspring. This chapter first discusses these classic pure sex chromosomal aneuploidies. Then, deletion/duplication states of the X and Y chromosomes are reviewed, whether large and known since classical cytogenetics, or those only having come to light due to the power of twenty-first century molecular karyotyping. Recurrence risks are considered both for those who (if fertile, naturally or via in vitro fertilization) might themselves have such an abnormality, and for normal parents having had an affected child.

This paper presents an overview of the main trends in animal exploitation in the Medieval period in Ireland as revealed by the zooarchaeological data. Cattle dominated the farming economy and diet throughout the period with dairying being their principal role. Sheep are consistently present. The growth of the wool trade after the Anglo-Norman conquest is evident in assemblages from the east and southeast, though it is not a strong trend with their use as a source of meat persisting throughout the Medieval period. An urban–rural dichotomy is also evident in the exploitation of pigs, goats, cats, dogs, and domestic poultry. The native Irish clearly took little interest in the exploitation of wild animals, unlike the Anglo-Normans for whom the consumption and hunting of game played an important role.

Rollerball, the Canadian-born director and producer Norman Jewison's 1975 vision of a future dominated by anonymous corporations and their executive elite, in which all individual effort and aggressive emotions are subsumed into a horrifically violent global sport, remains critically overlooked. What little has been written deals mainly with its place within the renaissance of Anglo-American science-fiction cinema in the 1970s, or focuses on the elaborately shot, still visceral to watch, game sequences, so realistic they briefly gave rise to speculation Rollerball may become an actual sport. Drawing on numerous sources, including little examined documents in the archive of the film's screenwriter William Harrison, this book examines the many dimensions of Rollerball's making and reception: the way it simultaneously exhibits the aesthetics and narrative tropes of mainstream action and art-house cinema; the elaborate and painstaking process of world creation undertaken by Jewison and Harrison; and the cultural forces and debates that influenced them, including the increasing corporate power and growing violence in Western society in late 1960s and early 1970s. The book shows how a film that was derided by many critics for its violence works as a sophisticated and disturbing portrayal of a dystopian future that anticipates numerous contemporary concerns, including ‘fake news’ and declining literary and historical memory. The book includes an interview with Jewison on Rollerball's influences, making, and reception.

Crises come in various forms, and their impact is not predicable with much accuracy. Crises in emerging markets are not the same as those in advanced economies. By 2007, the idea that monetary policy ought to be rules-based was widely accepted and copied around the world. Policymakers believed that inflation and macroeconomic slack were all that mattered. Demographic and structural factors were underappreciated. The wrong conclusions are now being drawn: rules should not be abandoned, but monetary policy can be improved. Monetary policy now relies more on words. An expansion of central bank balance sheets has taken place and central bank independence is a quaint idea. Central banks no longer influence just prices; they also change financial system quantities. This leads to rising policy uncertainty. Central banks stand accused of hubris, with little clear idea of the “new normal” and how this will redefine a future monetary policy strategy.

Androgens are now known to play important roles in cardiometabolic disease, but little attention has been paid to this within current medical and urological education. The wide range of symptoms associated with the hormonal aspects of ageing spans many medical specialities with the result that no single discipline currently deals with these issues. Just as ageing women will remain under the care of gynaecologists, it is likely that the field of andrology must deal with the demands of this growing population of ageing men.

Written by Peter Vincent Pry, a former CIA military analyst, War Scare provides a history of our country's little-known brushes with nuclear war and warns that, contrary to popular opinion and the assurances of our political leaders, the possibility of a Russian attack still exists. Nuclear deterrence has been the foundation of Western security for the last 50 years, but since the end of the Cold War, Russian military doctrine has become more destabilizing, and much more dangerous, than is commonly believed. By making use of a wealth of declassified and unclassified material, Dr. Pry illustrates how Russia's brutal past continues to shape the consciousness and decision making of its leaders, many of whom are unreconstructed ideologues from the old Soviet regime. This is a grave situation, indeed, as even after the START I, II, and III agreements, Russia will retain enough nuclear weapons to destroy the worldâ–not to mention significantly expanded chemical and biological warfare capability. War Scare convincingly shows that we ignore these facts at our peril.

Although recommendations are that these patients should be cared for in specialist centres, the clinical state of the acutely unwell patient may preclude transfer prior to the instigation of lifesaving treatment. Although the principles of resuscitation in this patient population differ little from those with acquired heart disease, the acutely unwell adult congenital heart disease patient presents a challenge, with potential pitfalls in examination, assessment/monitoring, and intervention. Keys to avoiding errors include: knowledge of the primary pathophysiology, any interventions that have been undertaken, residual lesions present (static or dynamic), and the normal physiological status for that patient-to determine the precise cause for the acute deterioration and to appreciate the effects (detrimental or otherwise) that any supportive and/or therapeutic interventions might have. Expert advice should be sought at the earliest opportunity.

Joseph wrote his epic around the year 1180, and revised it at the court of Henry II of England where he had obtained some sort of post through the influence of his uncle, Baldwin, Archbishop of Canterbury. The work is one of a series of texts in Latin and Anglo-Norman, apparently commissioned by the King, helping to trace back the Plantagenet line to the Trojans. It is a pendant to the Anglo-Norman Roman de Troie written by Benoit de Sainte-More in the 1160s. Joseph rejected the Vergilian 'mendacious poetic' account of the war in favour of the 'historical' narrative of Dares Phrygius, an 'eye-witness' of the events. This version not only coincided with the Plantagenets' preference for historical material but also presented Aeneas, the founder of the Romans, as a traitor. In Henry's struggles with the Pope over the Investiture problem any slur on the origins of the Romans could be useful ammunition. Books I–III cover the first Trojan war when Laomedon was besieged, the Judgement of Paris and the Rape of Helen. In style, Joseph closely resembles Lucan whom he had read “with an eye that allowed little to escape” (Raby), yet his imitation is far from servile. Sedgwick even goes so far as to say that Joseph “surpasses the bold constructions of Silver Latin.” Moreover, Joseph restores to epic the gods that Lucan had banished. The result is an epic that in the 17th century was still considered to have been written in the classical period. Latin text with facing-page English translation, introduction and commentary.

Chinese eunuchs have long been an object of fascination, but shockingly little is known about them. Castrated so as to maintain the integrity of the imperial line by ensuring that any child born in the palace is the emperor’s, most of these servants lived out their lives in obscurity, performing the menial tasks that kept the palace functioning. Some rose to prominence, becoming imperial advisors and confidants. A few became infamous, flattering their way into the emperor’s good graces and slowly arrogating power. Usurping eunuchs were a perennial threat to imperial rule, and were blamed for the downfall of dynasties. In this book, Norman Kutcher uses a wealth of sources to study eunuchs and their distinctive place in Chinese rulership during the first 150 years of the Qing dynasty. This period encompassed the reigns of three of China’s most important emperors: Kangxi, Yongzheng, and Qianlong. In different ways, each was deeply affected by the legacy of prior dynasties’ eunuch power. How they sought to prevent a return to eunuch excesses, and how eunuchs flourished in the face of restrictions imposed upon them, is the subject of this groundbreaking book.

Death studies have, over the past twenty years, witnessed a flourishing of research and scholarship particularly in areas such as dying and bereavement, cultural practices and fear of dying. But, despite its importance, a specific focus on the nature of personal mortality has attracted surprisingly little attention. Reflecting on the Inevitable: Mortality at the Crossroads of Psychology, Philosophy, and Health breaks new ground by bringing together available ideas and research on the meaning of one’s own death. Its content is organized around the question of how an ongoing relationship might be possible when the threat of consciousness coming to an end points to an unthinkable and unspeakable nothingness. The book then argues that, despite this threat, an ongoing relationship with one’s own death is still possible by means of conceptual devices that help shape personal mortality into a relatable object. Four of these devices, or “enabling frames,” are examined: essential structures, passionate suffusion, point-of-transition, and self-generative process. While each frame conceptualizes mortality differently, they share a capacity to move it from unintelligibility to something we can think and speak about, thereby enabling us to maintain an ongoing engagement. The final chapters explore ways in which pursuing a relationship with our own deaths could become a normal and acceptable activity throughout our lives.

A variety of mechanisms co-operate to ensure that the hydrogen concentration in plasma can be tightly controlled between 35 to 46 nano moles per litre and any deviation being rapidly compensated. Inherited metabolic diseases can to a variable degree impact to disturb this equilibrium. The underlying causes responsible for this outcome are disease dependent and may occur due to generation of overwhelming quantities of hydrogen per se, or at the level of renal reabsorption or generation of bicarbonate or due to tissue hypoxia resulting from either poor pulmonary or cardiac function.

What is grand strategy? What does it aim to achieve? And what differentiates it from normal strategic thought—what, in other words, makes it “grand”? In answering these questions, most scholars have focused on diplomacy and warfare, so much so that “grand strategy” has become almost an equivalent of “military history.” The traditional attention paid to military affairs is understandable, but in today's world it leaves out much else that could be considered political, and therefore strategic. It is in fact possible to consider, and even reach, a more capacious understanding of grand strategy, one that still includes the battlefield and the negotiating table but can also expand beyond them. Just as contemporary world politics is driven by a wide range of non-military issues, the most thorough considerations of grand strategy must consider the bases of peace and security as broadly as possible. A theory that bears little resemblance to the reality around us every day—in which gender, race, the environment, and a wide range of cultural, social, political, and economic issues are salient—can be only so useful. This book examines America's place in the world. The chapters reexamine familiar figures, such as John Quincy Adams and Henry Kissinger, while also revealing the forgotten episodes and hidden voices of American grand strategy. They expand the scope of diplomatic and military history by placing the grand strategies of public health, race, gender, humanitarianism, and the law alongside military and diplomatic affairs to reveal hidden strategists as well as strategies.

Anaesthesia induces changes in many organ systems within the body, though clearly none more so than the central nervous system. The physiology of the normal central nervous system is complex and the addition of chronic pathology and polypharmacy creates a significant challenge for the anaesthetist. This chapter demonstrates a common approach for the anaesthetist and specific considerations for a wide range of neurological conditions. Detailed preoperative assessment is essential to gain understanding of the current symptomatology and neurological deficit, including at times restrictions on movement and position. Some conditions may pose challenges relating to communication, capacity, and consent. As part of the consent process, patients may worry that an anaesthetic may aggravate or worsen their neurological disease. There is little evidence to support this understandable concern; however, the risks and benefits must be considered on an individual patient basis. The conduct of anaesthesia may involve a preference for general or regional anaesthesia and requires careful consideration of the pharmacological and physiological impact on the patient and their disease. Interactions between regular medications and anaesthetic drugs are common. Chronically denervated muscle may induce hyperkalaemia after administration of succinylcholine. Other patients may have an altered response to non-depolarizing agents, such as those suffering from myasthenia gravis. The most common neurological condition encountered is epilepsy. This requires consideration of the patient’s antiepileptic drugs, often relating to hepatic enzyme induction or less commonly inhibition and competition for protein binding, and the effect of the anaesthetic technique and drugs on the patient’s seizure risk. Postoperative care may need to take place in a high dependency unit, especially in those with limited preoperative reserve or markers of frailty, and where the gastrointestinal tract has been compromised, alternative routes of drug delivery need to be considered. Overall, patients with chronic neurological conditions require careful assessment and preparation, a considered technique with attention to detail, and often higher levels of care during their immediate postoperative period.
