Water electrolysis is expected to play a crucial role to the transition towards a hydrogen-based, low carbon economy. Among the various technologies for water electrolysis technologies, the one using proton exchange membranes (PEM) holds promise due to its high efficiency, load flexibility and compact design. However, the acidic environment in the PEM creates a harsh operating environment which entails the use of scarce and expensive metals (Pt and Ir) which account for 38% of the cost of a catalyst coated membrane (CCM). Moreover, the high scarcity of Ir is considered as the grand challenge of this technology.1 To relieve high raw material costs and to mitigate the Ir-dependence of PEM electrolysis, the development of advanced CCMs with improved Ir utilization is essential. Most of studies so far have focused on the development of high-structured catalysts, either by maximizing the Ir dispersion with using high surface area supports or by using alternative catalyst nanostructures.2 In all these conventional routes, the CCM manufacturing process is multi-step and primarily ink-based. Steps include ink preparation (from metallic nanoparticles), ink application onto the PEM to yield the catalyst-coated product, and several intermediate drying stages. The use of improved catalyst layer manufacturing techniques has been proposed as an alternative route to produce catalyst-coated membranes (CCMs) with low Ir loadings, without compromising in activity or durability. However, such concepts are less represented in literature. Despite this, the use of vapor-based processes for the manufacturing of nanomaterials for PEM electrolysers has key benefits over ink-based processes including the simplification of the production process, the reduction of catalyst loading, and the deposition of more uniform thin layers of material.3 In this work, we prepared CCMs using a solvent free, gas-phase method, comprising spark ablation and impaction,4 to produce iridium-based nanoparticles (IrNPs) for PEM water electrolysers. IrNPs were produced at ambient temperature and pressure conditions in the gas phase using Ir rods as the nanomaterial source. The nanoparticle aerosol was then directed towards a nozzle and deposited onto a Nafion 115 membrane via inertial impaction. By securing the membrane on an XY-stage, a 4 cm2 catalyst coating was printed. The Ir content on the Nafion membrane was controlled by varying the printing speed. The prepared CCMs were characterized using RBS, ICP, SEM, HR-TEM, XRD, and XPS. The CCMs prepared via spark ablation comprised of a homogeneous, dense, porous layer of amorphous IrO2 nanoparticles (Figure 1a) with an average particle size of 2 nm. The Ir-based CCMs were employed in a single-cell PEM water electrolyser and complete performance assessment was performed at 60 °C, focusing both on activity and durability. Our results show that the CCMs prepared with spark ablation outperform the benchmark (commercial) CCM (Figure 1b) despite using considerably less Ir (Figure 1c). Specifically, up to 5-fold improvement in Ir utilization (i.e. 5-fold decrease in Ir-specific power density) was obtained. To assess durability, both transient and dynamic service conditions were simulated by using different time-profiles for the power input. The overall profile of the constant load durability test indicated that irreversible changes occurred at the CCM during the first 30 h, while afterwards a stable degradation rate of 0.05 mV h-1 was sustained for >60 h. A similar degradation rate was also obtained with the benchmark CCM. Based on this rate and assuming a cut-off voltage of 2.2 V, the expected lifetime of our CCM is 10,000 h, which is the minimum requirement for industrial operation. Overall, our results showcase the feasibility of the spark ablation technology as a scalable and efficient method to achieve reduced Ir loading and better performing CCMs for PEM water electrolysis. <p> W niniejszym artykule podjęto próbę interpretacji 21 pieśni Ireny Wieniawskiej (powstałych w latach 1911-1927), wykorzystujących teksty Paula Verlaine’a. Kompozytorka (1879-1932), znana także jako Madame Poldowski czy Lady Dean Paul, była córką Henryka Wieniawskiego i Izabeli Hampton-Wieniawskiej. Żyła i pracowała w Belgii oraz Wielkiej Brytanii, odbyła także podróże koncertowe do Stanów Zjednoczonych i Hiszpanii. We Francji poznała dorobek impresjonistów (m.in. Maurice’a Ravela), co w znacznym stopniu ukształtowało jej idiom twórczy.</p><p> W pierwszej części tekstu analizie poddano warstwę słowną utworów Wieniawskiej: ich wymiar treściowy oraz konstrukcyjny. Kolejny fragment został poświęcony płaszczyźnie muzycznej – autorka charakteryzuje podstawowe elementy dzieła: melodykę, akompaniament, harmonikę, fakturę i formę. W trzeciej części artykułu, w oparciu o reguły (<em>corollaries</em>) wyznaczone przez Henry’ego Woolletta, interpretowane są relacje słowno-muzyczne w pieśniach Madame Poldowski. Przeprowadzone analizy skłaniają do przyjęcia hipotezy o związku wokalno-instrumentalnego dorobku Lady Dean Paul z gatunkiem francuskiej <em>mélodie</em>, na co wskazywałaby predylekcja kompozytorki do recytatywnego traktowania głosu solowego, sylabicznego opracowania tekstu, jak również tendencja do wykorzystywania różnorodnych, skomplikowanych rozwiązań w zakresie harmoniki i formy. The price of electricity produced from renewable sources has sharply sunk in the last decade, making these a cheaper choice than newly built thermal power plants in almost all parts of the world [1]. The missing link towards the full decarbonisation of the energy sector is a crossover vector that can account for the intrinsic intermittency of these renewable energy sources. Hydrogen seems to be the best candidate for this, particularly if produced using water electrolysis powered by excess energy from renewable sources. In particular, the load capability and high power density of polymer electrolyte water electrolysis (PEWE) renders this technology an excellent match to these renewables’ sporadic nature [2]. However, the need for scarce Ir to catalyze the sluggish oxygen evolution reaction (OER) taking place in PEWE anodes hinders the large scale application of this technology [3]. Therefore, a better understanding of the OER mechanism on Ir-based electrocatalysts is needed to guide catalyst design and decrease the efficiency losses related to this reaction. Over the last decades, the OER mechanism on Ir-based materials has been extensively investigated through operando/in-situ X-ray absorption as well as X-ray photoelectron spectroscopy (XAS, XPS) [4-7]. Despite enormous efforts, there is still a lack of consensus regarding the oxidation state of Ir under OER conditions and the nature of the OER-active sites. Therefore, in this work we employed operando modulation excitation XAS to study the OER mechanism on several calcined and uncalcined Ir-oxide catalysts. By combining this modulation excitation approach with phase sensitive analysis, we enhanced the sensitivity of XAS towards oxidation state changes that are otherwise undiscernible on low surface area materials, such as calcined IrO2. To derive more precise information from these phase resolved spectra, standard materials with Ir oxidation states ≥ +5 were also characterized. The comparison of the demodulated, operando spectra of a given material with the corresponding difference between its low-potential spectrum and the standard compounds’ spectra indicates that all oxides reach a maximum Ir oxidation state of +5 under OER conditions. In addition, multivariate curve resolution (MCR) analysis was employed to extract kinetic information from these period averaged spectra. This analysis unveiled that the oxidation of the surface iridium proceeds at the same rate as its reduction in the case of highly OER-active materials, while reduction is slower than oxidation for the less OER-active samples. In summary, this contribution presents novel insights on the Ir oxidation under potential control, and in doing so also features the capabilities of modulation excitation XAS and MCR to elucidate kinetics of electrochemical reactions. Kompres hangat dapat dilakukan di daerah dinding perut, vena-vena besar (axilla) dan daerah temporal. Tujuan dari penelitian ini adalah untuk mengetahui perbandingan efektifitas pemberian kompres hangat pada daerah dinding perut, vena-vena besar (axila) dan daerah temporal terhadap penurunan suhu tubuh klien febris. Jenis penelitian ini adalah kuantitatif dengan rancangan There Group Pretest Posttest. Penelitian ini dilakukan di ruangan irina F dan irina H, di RSUD dr. M.M. Dunda Limboto. Tehnik pengambilan sampel pada penelitian ini adalah purposive sampling. Pada penelitian ini dilakukan pengujian yaitu penggunakan paired sample t test pada masing-masing kelompok intervensi yang dilanjutkan dengan analisis one way anova.Kesimpulan penelitian adalah terdapat perbedaan penurunan suhu tubuh antara kompres hangat pada daerah vena-vena besar (axila) dengan kompres pada daerah perut dan daerah temporal (ρvalue 0,000) dan tidak terdapat perbedaan kompres daerah perut dengan daerah temporal ditunjukkan (ρvalue 0,539).

The basidiomycete Lepista irina (F r.) Bigelow (strains CBS 366.47 and CBS 458.79) was cultivated on a biomalt (2% )-liquid medium for 28 days. The steam distillate was analyzed by GLC and GLC-S. The structure elucidation of the predominant constituent resulted from spectroscopic methods, particularly from its 13CNMR spectrum, nuclear Overhauser enhancement 1HNMR data, and from 2d 1H homonuclear shift correlation. The bisabolane derivative was nam ed lepistirone and is described for the first time as a natural product. Two minor related com pounds could be characterized by their MS data.

Résumé :

The purpose of this research was to determine the volume and value sales trends and the factors that influence on it by the object of the research at Union Sauce “Bebek & Ayam Spesial Bumbu Ireng-- Desa Plintahan -“ Kecamatan Pandaan. The research methods are used literature study and field research on this object business, with the result that the trend volume and value sales showed a positive trend over the last 3 years and significantly affected by the independence variable (price, price of others product, promotion dan distribution). As evident by F count 303, 276 with linear regeression calculations of all variables that most influence volume and value of sales are distribution (12,063 and 9,355), Price of product (10,238 and 7,554) and Other Product Price (-10,510 and -7,817)

Abstract: Reconciliation is one of the proces that prevents drug-related problems, medication errors, and drug-related adverse events. This study aimed to identify treatment mismatches based on the results of drug reconciliation in 49 patients from the January to March 2020 study period. This research has been descriptive study with prospective data collection in inpatient installation CVBC (Cardiovascular and Brain Center) and Irina F Jantung, Prof. Dr. R. D. Kandou Manado.The results of the study based on patient characteristics showed that the number of male patients was greater than that of women, namely 31 patients and the highest number of patients was in the age group (60 - 69) as many as 23 patients. The results showed that the majority of patients were male as many as 31 patients and the most in the age group (60-69) as many as 23 patients. The characteristics of the number of drugs shown in the drug history data (BPMH), the majority of patients consumed <5 types of drugs, were 25 patients, in the ER Most consumed <5 types of drugs as many as 27 patients and also the majority of patients received ≥5 types of drugs while in the inpatient installation of 27 patients and when they left the hospital the majority of patients were given ≥5 types of drugs, namely 34 patients. The most widely used drugs are cardiovascular system drugs with a percentage of BPMH (95.92%), IGD (100%), in inpatient installations (100%) and out of prescription (100%) and the second is digestive system drugs. The percentage of unintentional discepancies was incomplete prescription (10.2%), omission medication (10.2%), and intentional discrepancies (100%).Keywords: medication discrepancies, medication reconciliation Abstrak: Rekonsiliasi merupakan salah satu proses yang dilakukan untuk mencegah terjadinya kesalahan obat. Penelitian ini bertujuan mengidentifikasi ketidaksesuaian pengobatan berdasarkan hasil rekonsiliasi obat pada 49 pasien periode Januari–Maret 2020. Desain penelitian deskriptif dengan pengambilan data secara prospektif di instalasi rawat inap CVBC (Cardiovascular and Brain Center) dan Irina F Jantung RSUP Prof. Dr. R. D. Kandou Manado. Hasil penelitian menunjukkan mayoritas pasien adalah laki-laki sebanyak 31 pasien dan terbanyak pada kelompok usia (60-69) sebanyak 23 pasien. Karakteristik jumlah obat ditunjukkan pada data riwayat obat (BPMH) mayoritas pasien mengkonsumsi <5 jenis obat sebanyak 25 pasien, di IGD Sebagian besar mengkonsumsi <5 jenis obat sebanyak 27 pasien dan juga mayoritas pasien menerima jenis obat saat di Instalasi rawat inap sebesar 27 pasien dan saat keluar rumah sakit mayoritas pasien diberikan jenis obat yaitu 34 pasien. Obat yang paling banyak digunakan yaitu obat sistem kardiovaskular dengan presentase pada BPMH (95,92%), IGD (100%), di Instalasi rawat inap (100%) dan pada resep keluar (100%) dan yang kedua obat sistem pencernaan. Persentase ketidaksesuaian pengobatan yaitu Incomplete prescription sebesar (10,2%), Omission medication (10,2 %), dan ketidaksesuaian yang disengaja (100%).Kata kunci: rekonsiliasi, ketidaksesuaian pengobatan

Résumé :

The purpose of the article is a comprehensive study of the stage achievements of F. Baklan, V. Potapova, I. Zadayanna, and O. Baklan in the context of the development of Ukrainian ballet in the second half of the XX - early XXI centuries. The methodology of work contains the following research methods: analytical, dialectical, systemic, general historical, comparative-historical, etc. The scientific novelty of the publication is that it first explores the significance of the artistic work of the masters of the musical theater of the creative dynasty Baklan, who made an important contribution to the rise of domestic academic choreography of the Soviet era and independence. Conclusions. The importance of the artistic achievements of the figures of the stage of the theatrical Baklan’s dynasty for the development of national ballet is difficult to overestimate. The founders of the creative family - Honored Artists of the USSR Fedor Baklan (1930-1983) and Varvara Potapova (1932-2018) joined the cohort of leading dancers of the Kyiv Opera and Ballet Theater T. Shevchenko 1950-1960s, which after the crisis of drama contributed to the formation on the domestic stage of a new form of music and dance drama ("poetic dance") under the direction of choreographers-innovators - V. Vronsky, R. Zakharov, F. Lopukhov, S. Sergeeva, M. Tregubova. The creative work initiated by parents on the stage of the National Opera of Ukraine in the 1980s and 2000s was continued by Honored Artist of Ukraine, conductor Oleksiy Baklan (b. 1961) and his wife, Honored Artist of Ukraine Iryna Zadayanna (1962–2005), who created and will continue to generate high art aimed at the highest European theatrical standards.

The strategic location of the Isthmus of Panama within the commercial network of the Spanish Empire and the need to defend it has greatly influenced historical writing on sixteenth-and seventeenth-century Panama. Most studies have emphasized military and economic history and with few exceptions, have shown little interest in other aspects of Panamanian life. An excellent review of the historical literature on colonial Panama can be found in Christopher Ward, Imperial Panama: Commerce and Conflict in Isthmian America, 1550-1800 (Albuquerque, 1993). Despite a continuing emphasis on the usual themes of trade and defense, there is a growing trend to focus on other topics such as population movements and social classes. One of the areas still awaiting further investigation and study is the history of the cimarrons of Panama. The two principal primary sources for the role of the cimarrons are the collections of documents from the Archivo General de Indias in Seville published by Irene Wright and Carol F. Jopling, respectively. Wright's Documents Concerning the English Voyages to the Spanish Main, 1569-1580 (London, 1932) contains the correspondence of Spanish officials on the Isthmus to the king relating to the activities of the English pirates and their alliance with the cimarrons.

Abstrak Tenaga kesehatan khususnya perawat merupakan komponen yang sangat penting dalam pemenuhan kebutuhan kesehatan pasien yang dirawat. Namun, dimasa pandemic covid-19 tenaga keperawatan akan merasa berbeda dikarenakan rentannya terpapar dengan virus covid-19 dan lebih mungkin akan mengalami kecemasan. Tujuan dari penelitian ini adalah untuk mengetahui tingkat kecemasan perawat ruangan isolasi covid-19 di RSUP Prof. R. D. Kandou Manado. Metode yang digunakan dalam penelitian ini adalah penelitian descriptive correlation dengan teknik total sampling dengan menggunakan 151 perawat yang bekerja di ruang isolasi covid-19. Hasil dari penelitian menunjukan bahwa sebagian besar tenaga keperawatan yang bekerja di ruang isolasi covid-19 tidak merasa cemas saat bekerja dengan nilai persentase mencapai 95%. Lebih lanjut data menunjukan bahwa tidak ada perbedaan yang signifikan tingkat kecemasan para perawat jika ditinjau dari jenis kelamin dengan nilai p = 0.175 > 0.05, terdapat perbedaan yang signifikan saat ditinjau dari ruang perawatan dengan nilai p = 0.003 < 0.05. Dapat disimpulkan bahwa perawat yang bekerja di ruang perawatan isolasi covid-19 RSUP Prof. R. D. Kandou Manado tidak merasakan kecemasan baik laki-laki maupun perempuan, tapi terdapat perbedaan tingkat kecemasan perawat antara ruang perawatan. Berdasarkan hasil tersebut, dapat direkomendasikan bagi institusi rumah sakit untuk memperhatikan kariawan yang bekerja di ruangan isolasi covid-19 khususnya diruangan Anggrek 1 dan Irina F. Kata kunci : Covid-19, Tingkat kecemasan, Perawat ruang isolasi Abstrac Health workers, especially nurses, are very important components in improving the health of patients. However, during the COVID-19 pandemic, nursing staff will experience the difference of being exposed to the Covid-19 virus and are more likely to experience anxiety. The purpose of this study was to determine the anxiety level of nurses in the Covid-19 isolation room at Prof. RSUP. R.D. Kandou Manado. A method of descriptive correlation was utilized in this study with 151 nurses working in covid-19 isolation room which are selected using total sampling technique. The results of the study show that most of the nursing staff who work in the COVID-19 isolation room did not feel anxious while working with a percentage value of 95%. Furthermore, the result shows that there is no significant difference in the level of anxiety of the nurses when sex is considered with a p value of 0.175 > 0.05, and there is a significant difference when nursing ward was considered with a p value 0.003 < 0.05. Conclusion that the nurses who work in the Covid-19 isolation room at Prof. R. D. Kandou Manado did not experience any anxiety either male or female, but there is a difference in the level of anxiety of nurses when the nursing word is considered. The recommendation for hospital to pay attention for the employees who work in the Covid-19 isolation room, especially in the Anggrek 1 and Irina F. Résumé :

Purpose. This study aimed to characterize 27 Escherichia coli isolates obtained from peritoneal dialysis (PD)-related peritonitis that occurred at the University Hospital of Botucatu Medical School, Brazil, between 1997 and 2015. Methodology. These isolates were characterized regarding the occurrence of 22 virulence factor-encoding genes, antimicrobial resistance and biofilm production. We then evaluated whether these factors influenced the clinical outcome. Results. Over an 18-year period, 726 episodes of PD-related peritonitis were diagnosed, with 27 of them (3.7 %) being due to E. coli . The majority of the isolates were classified in phylogroups B1 (33.3 %), B2 (30.0 %) or F (18.0 %). fimH (100.0 %), ompT (66.7 %) and irp2 (51.9 %) were the most prevalent genes, while papA, papC, iha, sat, irp2, iucD, ireA, ibe10, ompT and kpsMTII were significantly more prevalent among isolates belonging to phylogroups B2 and F (P<0.05). Non-susceptibility to quinolones was detected in six isolates, which harboured chromosomal and/or plasmid-mediated quinolone resistance determinants, while two CTX-M extended-spectrum β-lactamase-producing E. coli were identified. Virulence factor-encoding genes (alone or in combination) and antimicrobial resistance were not associated with non-resolution outcomes. However, there was a trend for the ability to produce biofilm to be associated with treatment failure, although this association was not statistically significant. Conclusion. The E. coli isolates were heterogeneous in terms of the features investigated, and were susceptible to most of the antimicrobial drugs tested, despite the unsuccessful treatment observed in more than 50.0 % of the patients. Studies including more cases could help to clarify if biofilm production can influence the outcome in patients with PD-related peritonitis.

Résumé :

The antifungal and growth-stimulating activity of biological detoxification waste products of hydrolysates of lignocellulosic raw materials in vitro and in vivo was evaluated in comparison with reference preparations (Alirin-B, Fitosporin-M) and control (sterile tap water). These wastes are specially adapted microbocenoses of activated sludge, worked out in the process of purification of hydrolyzates of lignocellulosic raw materials from inhibitors of acetone butyl fermentation. The agronomic value of biodetoxification waste was studied in three prototypes of different nature, using Iren spring wheat as an example. The results showed that detoxification bioagents, regardless of origin, showed fungistatic activity at the in vitro and in vivo study stages (biotest). In an in vitro experiment, all test samples showed significant antifungal activity against the fungus F. oxysporum. The most effective was the biodetoxification waste obtained on the basis of microbocenosis of activated sludge grown on a nutrient medium containing phenol, formic and acetic acid. As a result of exposure to this bioagent at the end of the experiment, the average diameter of the colonies of the fungus F. oxysporum was approximately 34 times less than in the control version. The detoxification bioagent, obtained on the basis of a specially adapted microbocenosis of activated sludge grown on a nutrient medium simulating wastewater, reduced the diameter of phytopathogenic fungus colonies by an average of 16 times. Specially adapted activated sludge from the sewage treatment facilities of the wood processing enterprise, worked out during the detoxification of hydrolysates of lignocellulosic raw materials, was also able to effectively suppress fungus growth, the average diameter of which was 19 times less than the control. The biotest results also confirmed the fungistatic activity of the test samples. The effectiveness of reducing the total infection with seminal infections in different experimental variants ranged from 52 to 84%. The growth-promoting ability of biodetoxification waste was weak.

It has been concluded that the idea of the monograph is relevant since the rules of international treaties of Ukraine (namely, bilateral agreements on legal assistance) and the Law of Ukraine on Private International Law (hereinafter: PILA) contain some gaps and uncertain provisions regarding international succession law issues which need to be filled or revised respectively. We support the author's suggestion to make such amendments taking into consideration Regulation (EU) No 650/2012 of the European Parliament and of the Council of 4 July 2012 on jurisdiction, applicable law, recognition and enforcement of decisions and acceptance and enforcement of authentic instruments in matters of succession and on the creation of a European Certificate of Succession, as well as, the EU Private International Law doctrine and solutions to international succession law issues in national law of some the EU Member States. The book consists of 15 chapters which focus on the determination of international jurisdiction in succession matters; determination of the law applicable to succession relations with a foreign element; recognition and enforcement of foreign decisions in succession matters; acceptance of authentic documents created abroad. Many author's ideas seem appropriate. They include the suggestions to determine the scope of law applicable to the succession in PILA; to amend the conflict of laws rules applicable to the form of wills; to include in PILA conflict of laws rules regarding succession agreements; to amend some rules of Civil Procedural Code of Ukraine applicable to recognition and enforcement of foreign decisions in civil matters and others. Penelitian ini menggunakan rancangan Nonequivalent Control Group Design yang artinya terdapat dua kelompok yang digunakan untuk penelitian, yaitu kelompok eksperimen (yang di beri perlakuan) ,kelompok kontrol (yang tidak di beri perlakuan), dan tidak di pilih secara random. Teknik yang digunakan dalam pengambilan sampel adalah tehnik Purposive Sampling yaitu pemilihan sampel dengan menetapkan subjek yang memenuhi kriteria penelitian atau dengan pertimbangan tertentu.Besarnya sampel dalam penelitian ini sebanyak 30 responden, kelompok kontrol sebanyak 15 responden dan kelompok intervensi sebanyak 15 responden.Dalam penelitian ini analisis yang digunakan adalah uji One Way Anova. Hasil analisis data secara statistik memperoleh nilai rata-rata mean pada kelompok kontrol sebelum = 5,87 dan sesudah= 5,67, sedangkan pada kelompok intervensi sebelum= 5,87, dan sesudah= 4.47. Standar Deviasi pada kelompok kontrol sebelum= 1.187 dan sesudah= 1.175, sedangkan pada kelompok intervensi sebelum= 1.302 dan sesudah= 1.457. Hasil analisis data membuktikan bahwa terdapat perbedaan yang bermakna antara intensitas nyeri sebelum diberikan aromaterapi lavender dan setelah diberikan aromaterapi lavender, yang memperoleh nilai F= 4.115 dan p-value= 0,010 (p value=<0,05).

BackgroundThe latest data show that psoriatic arthritis (PsA) patients (pts) with asymmetrical sacroiliitis (SI) have clinical and genetic features which are considered typical for PsA (1).ObjectivesTo analyze comparative clinical characteristics of PsA pts with symmetrical and asymmetrical SI.Methods287 pts (M/F–147/140) with PsA according to CASPAR criteria were examined. Data was collected from 6 rheumatology clinics of the Russian Federation. Pts’ age 45.6±12.2 years (yrs), disease duration 10.9±6.7 yrs. Pts underwent standard clinical examination of PsA activity. Disease activity was measured by DAPSA, BASDAI, ASDAS-СRP. The examination involved determining HLA B27 antigen status and X-rays of sacroiliac joints (SIJs) (pelvic radiographs), hands and feet. Bilateral changes in SIJs of grades 2-2, 3-3 and 4-4 were defined as symmetrical SI (sym-SI). Changes in SIJs of grades 2-1, 2-3, 3-1, 1-4, 2-4 and 3-4 were defined as asymmetrical SI (asym-SI). Skin lesion severity was evaluated in terms of body surface area (BSA) affected and Psoriasis Area Severity Index (PASI). When BSA was ≥ 3%, PASI was calculated. Me [Q25; Q75], Pierson-χ2 tests were performed. All p<0.05 were considered to indicate statistical significance.ResultsSym-SI was found in 187 (65.1%) pts, asym-SI – in 100 (34.8%) pts. In group (gr) of pts with asym-SI there were more females than in the sym-SI gr: 63 (63.0%) vs 77 (41.2%; р = 0.0004). Pts with asym-SI had more severe peripheral arthritis as measured by Tender Joint Count (TJC68) and Swollen Joint Count (SJC66). These values were higher in asym-SI gr: 9 (4-16) vs 6 (2-14; р = 0.007) and 5 (2-12) vs 3 (0-9; р = 0.004), respectively. High activity according to DAPSA was found in 53 (54.6%) pts in the asym-SI gr vs 71 (38.8%) in the sym-SI gr (р = 0.01); low activity and remission by DAPSA– in 16 (16.5%) pts vs 51 (28.3%; р = 0.028), respectively. The frequency of enthesitis was higher in the asym-SI gr: 50 (57.5%) pts vs 57 (38.5%) pts (р = 0.005). Pts’ functional capacity as measured by Health Assessment Questionnaire (HAQ) was worse in the asym-SI gr: HAQ>0.5 values were found in 83 (84.7%) pts vs 130 (72.6%) pts (р = 0.023). No significant differences were found between HLA B27 positivities in sym-SI and asym-SI groups (which were 54.2% and 44.0%, respectively; p = 0.23).ConclusionAsym-SI in PsA pts is associated with female gender, more severe peripheral arthritis, higher activity according to DAPSA, more frequency of enthesitis and reduction of pts’ functional capacity.References[1]M. Résumé :

Abstract Background: Precision oncology relies on the accurate molecular characterization of individual cancer patients at the time of treatment initiation. However, tumor molecular profiles are not static, and cancers continually evolve because of ongoing mutagenesis and clonal selection, with actionable genomic alterations potentially gained or lost during disease progression. Methods: To define the concordance of potentially actionable genomic alterations between primary and metastatic disease sites in patients with urothelial cancer and to identify drivers of metastatic disease progression, we performed an integrated analysis of clinical and genomic data from 2,199 urothelial carcinoma patients (2,732 tumor and 331 plasma cfDNA samples) prospectively profiled at Memorial Sloan Kettering Cancer Center (MSKCC) from 2014 to 2021. Paired primary and metastatic tumor samples from individual patients were sequenced with MSK-IMPACT and/or whole-exome sequencing (WES). Plasma samples were analyzed using the MSK-ACCESS cfDNA platform. Results: Among potentially actionable mutations (defined as OncoKB level 1-4), ERBB2 and ARID1A were associated with higher tumor grade and stage (p-value <0.001, p-value = 0.03 respectively). WES analysis of primary and metastatic tumor sites was consistent with early branched evolution with on average 42% of mutations shared between disease sites. Among chromatin-modifying genes, ARID1A has a discordant rate of 15.8%. In the 6 discordant cases, ARID1A mutation was seen exclusively in the metastatic tumor samples. This suggests a role for ARID1A mutations in metastatic disease progression. While known and likely oncogenic mutations were more likely to be concordant than variants of unknown functional significance, we observed a high degree of mutational discordance among potentially actionable genomic alterations with 23% discordant between primary and metastatic disease sites. 24% of mutations were exclusively identified by plasma cell-free DNA sequencing. Actionable mutations in cell-free DNA were more likely to be concordant with metastatic tumor biopsies than tumor tissue collected from the primary tumor site. Conclusions: In sum, our analysis of patient-matched primary and metastatic urothelial carcinomas revealed a high degree of lesion-to-lesion genomic heterogeneity that may be a barrier to precision oncology approaches for this disease. Background:The protagonism of regulatory B cells seems to vary along the course of the disease in murine models of inflammatory conditions. Decreased numbers of circulating regulatory CD19+CD24hiCD38hi transitional B cells (cTrB) have been described in patients with longstanding RA.Objectives:To examine the frequency and evolution of cTrB cells in the peripheral blood of early RA (ERA) patients.Methods:Freshly isolated PBMCs from 48 steroid and DMARD-naïve ERA patients with a disease duration below 24 weeks and 48 healthy controls (HC) were examined by flow cytometry. Cocultures of isolated memory B cells were established with autologous T cells, in the absence or presence of TrB cells.Results:As compared with HC, ERA patients demonstrated an increased frequency of cTrB cells. cTrBs of ERA and HC displayed an anti-inflammatory cytokine profile and were able to downregulate T cell IFNγ and IL-21 production, together with ACPA secretion in autologous B/T cell cocultures. Basal frequencies of cTrBs above the median value observed in HC were associated with a good EULAR response to MTX at 12 months (RR=2.91; 95% CI, 1.37-6.47). A significant reduction of cTrBs was observed 12 months after initiating MTX, when the cTrB cell frequency was no longer elevated but decreased, and this was independent of the degree of clinical response or the intake of prednisone.Conclusion:An increased frequency of regulatory cTrB cells is apparent in untreated ERA, and the baseline cTrB cell frequency is associated with the clinical response to MTX at 12 months.References:[1]Matsushita T, et al. BackgroundJoint involvement in SLE is the most frequent manifestation, despite this, its impact on health-related quality of life (HRQoL) has not been well stablished in these patients1. Pain, fatigue and functional disability contribute to HRQoL related to SLE2.ObjectivesWe aim to study the relation between joint and tendon involvement confirmed by contrasted MRI and these patient related outcomes (PRO).MethodsConsecutive SLE patients fulfilling SLICC criteria both symptomatic and asymptomatic for joint involvement were recruited. Gadolinium-enhanced MRI of non-dominant hand/wrist was performed in all subjects for joint and tendon evaluation. Different PRO: numeric scale (NE) of pain (0-10) and fatigue (0-3), Health Assessment Questionnaire (HAQ) and Fatigue Severity Scale (FSS-9) were collected and statistically analysed along with each MRI abnormality.Results83 subjects were recruited. Patients with synovitis, tenosynovitis, peritendonitis and bone marrow edema reported higher values in pain NE (6.03±2.57 vs 4.26±2.49, p 0.005; 6.56±1.95 vs 4.76±2.75, p 0.017; 8.80±1.30 vs 4.95±2.55, p 0.001; 6.47±2.62 vs 4.83±2.58, p 0.026); patients with synovitis reported higher values in fatigue EN (2.32±0.82 vs 1.91±0.84, p 0.035) and patients with tenosynovitis showed worse FSS-9 (61.50±1.73 vs 45.70±16.80, p 0.015) versus patients who did not show these abnormalities by MRI. Patients with synovitis and peritendonitis had a worse HAQ (1.14±0.69 vs 0.75±0.65, p 0.031; 1.69±0.07 vs 0.90±0.69, p 0.018).ConclusionSLE patients with joint and/or tendon involvement confirmed by contrast enhanced MRI have a worse HRQoL measured by pain, fatigue and functional disability.References[1]Ceccarelli F et al. Despite this, it is not well characterised and demographic, serological and clinicotherapeutic correlations have not been well stablished1.ObjectivesWe aim to determinate if there is a demographic, clinic or serological profile related to inflammatory joint involvement of hand and/or wrist (sinovitis, bone marrow edema, erosions, tenosynovitis or peritendonitis) confirmed by contrasted MRI in SLE patients.MethodsConsecutive SLE patients fulfilling SLICC criteria both symptomatic and asymptomatic for joint involvement were recruited. Contrasted MRI of non-dominant hand/wrist was performed for joint and tendon evaluation. Sociodemographic, clinicotherapeutic and serological data were collected and statistically analysed along with each MRI abnormality.Results83 subjects were recruited. Erosions and synovitis were more frequent at advanced age (55±12.61 vs 45.06±12.18 years, p 0.001 and 52.78±12.99 vs 44.95±12.49 years, p 0.011). Synovitis is less frequent in patients with renal involvement (6.7% vs 24.3%, p 0.031). No other SLE organ-specific involvement showed statistical correlation. Neither SLE related autoantibodies (ANA, DNAds, Sm, RNP, Ro, La, antiphospholipid), complement fractions, ESR nor CRP correlated with MRI lesions, except for erosions which showed lower DNAds titers (15.94±49.56 vs. 27.23±62.41, p 0.008). Patients with bone marrow edema received more methotrexate (25% vs 6.3%, p 0.033), and those with erosions and peritendonitis received less mycophenolate (5.6% vs 22.9%, p 0.034; y 0% vs 12.8%, p 0.026). Peritendonitis was the only alteration related to higher SLEDAI (7±2.45 vs 3.64±3.34, p 0.018). None of the musculoskeletal abnormalities correlated with accrual damage.ConclusionSLE patients with joint and/or tendon abnormalities confirmed by MRI are usually older and have less renal involvement, receiving more methotrexate and less mycophenolate than those who do not show musculoskeletal involvement.References[1]Ceccarelli F et al. BackgroundJoint involvement in SLE is the most frequent manifestation and shows a wide heterogeneity1. It has not a valid classification and it is often underestimated. Subclinical inflammatory musculoskeletal involvement is not well known2.ObjectivesWe aim to describe the prevalence of joint and tendon involvement in hand and wrist of SLE patients, either with clinical arthritis, arthralgia or asymptomatic and compare it with healthy subjects using contrasted MRI.MethodsSLE patients fulfilling SLICC criteria were recruited and classified as follows: group (G) 1: hand/wrist arthritis, G2: hand/wrist arthralgia, G3: no hand/wrist symptoms. Jaccoud arthropathy, CCPa and RF positivity, hand OA or surgery were excluded. Healthy subjects (HS) were recruited as controls: G4. Contrasted MRI of non-dominant hand/wrist was performed. Images were evaluated following RAMRIS criteria extended to PIP, Tenosynovitis score for RA and peritendonitis from PsAMRIS. Groups were statistically compared.Results107 subjects were recruited (G1: 31, G2:31, G3:21, G4:24). Any lesion: SLE patients 74.7%, HS 41.67%; p 0.002. Synovitis: G1: 64.52%, G2: 51.61%, G3: 45%, G4: 20.83%; p 0.013. Erosions: G1: 29.03%; G2: 54.84%, G3: 47.62%; G4: 25%; p 0.066. Bone marrow edema: G1: 29.03%, G2: 22.58%, G3: 19.05%, G4: 0.0%; p 0.046. Tenosynovitis: G1: 38.71%; G2: 25.81%, G3: 14.29%, G4: 0.0%; p 0.005. BackgroundThe goal of psoriatic arthritis (PsA) treatment is achievement remission (REM) or Low Disease Activity (LDA) by DAPSA. The impact of comorbidities, especially obesity, metabolic syndrome on PsA activity and on biological treatment response are shown in some registry [1]. Obesity, hypertension and Charlson Comorbidity Index ≥1 are prognostic factors for poorer treatment outcome rates in PsA [2]. But influence of number of comorbidities on treatment response has not been fully studied in real practiceObjectivesTo evaluate the impact of number of comorbidities on achievement remission by DAPSA in PsA ptsMethods528 pts (M/F=256 (48.5%)/272 (51.5%) with PsA according to CASPAR criteria were included in the Russian observational cohort. Mean age 48.7±12.33 years (yrs.), PsA duration 122.6±81.85 months (mo.), psoriasis (Ps) duration 239.0±155.45 mo. Pts who completed ≥2 visits were analyzed. Mean observational period 73.6±83.47 mo.All pts underwent standard clinical examination: tender joint count (TJC)/68, swollen joint count (SJC)/66, Pain (VAS), Patient global assessment of disease activity (PtGA, VAS), CRP mg/l, BSA (%), BMI, HAQ, comorbidities by ICD-10, X-ray of hands and foods. DAPSA>28 indicates as high disease activity (HDA), >14-28 - moderate activity (MoDA), >4-14 – LDA, ≤4 – REM. Analyses were performed based on number of comorbidities: 1st group (gr) - without comorbidities, 2nd gr – pts with 1 comorbidity, and 3rd gr – pts with ≥ 2 comorbidities. M±SD, %, t-test, Peаrson-χ2, ORs with 95% CI were calculated. All p<0.05 were considered to indicate statistical significanceResultsIn 236 out of 528 (44.7%) pts no comorbidity found, in 145 out of 528 (27.5%) found 1 comorbidity and 147 out of 528 (27.8%) pts had ≥ 2 comorbidities.Comparative analysis in groups showed the following features: pts of 3rd gr were significantly more often females, had longer Ps and PsA duration, they were older, had worse HAQ, higher BMI, TJC (p<0.05). Joint erosions were found significantly more often in the 2nd gr compared to the 1st gr (p=0.02). No significant differences were in the frequency of axial PsA (p=0.906), Ps severity (BSA>10%) (p=0.237), the number of pts receiving bDMARDs (p=0.078). REM/LDA achievement according to DAPSA was significantly lower in the 3rd gr (15 (10,2%)/69(46.9%) comparing to the 1st gr (62 (26,3%)/94 (39.8%), and to the 2nd gr (32 (22,1%)/69 (47.6%) (p=0.007). The probability of achieving REM/LDA is 1.6 times lower in the gr of pts with ≥2 comorbidities compared to the gr of pts without comorbidities (OR 0.608, 95% CI 0.394 to 0.936) and 1.3 times lower compared to gr of pts with 1 comorbidity (OR 0.789, 95% CI 0.489 to 1.243) (Figure 1)Figure 1.Chances of REM/LDA achievement in pts with ≥2 comorbidities compared to pts with 1 comorbidity and pts without comorbiditiesConclusionIn real clinical practice comorbidities were seen in more than half PsA pts. We have entered the golden age of the English-language Carolingian Festshrift. As the formidable generation of Carolingian historians who came of age in the 1970s and 1980s begins to retire, their many students are honoring them with collections of essays that chart the landscape of the academic field that their teachers labored so diligently to shape. In recent years, Janet Nelson (2008), John Contreni (2013), Thomas F. X. Noble (2014), and Rosamund McKitterick (2018) have each been the recipients of such volumes. In the book under review, the honoree is Mayke de Jong, chair of medieval history at Utrecht University. Her colleagues and students have assembled twenty-five articles about religion and power in the Carolingian world as a testament to the vision and enduring influence of de Jong’s pioneering work in this field. As Rosamund McKitterick explains in her introductory essay, de Jong has always been “an adventurous explorer, ever pushing at the boundaries both of political discourse in relation to political action in a fundamentally religious context” (p. 5) If we take for granted today the close proximity of religion and politics in the early medieval world, it is largely due to the formative scholarship of de Jong.

Background Activating RAS mutations drive around 30% of pre-B cell acute lymphoblastic leukemias (pre-B ALL) and are particularly common in relapsed ALL with a consecutively poor outcome. Recently published data demonstrated the critical role of the unfolded protein response (UPR) network, namely its IRE1α-XBP1 axis, for the survival of pre-B ALL cells: High expression of XBP1 confers a poor prognosis in pre B-ALL. However, the mechanism of XBP1 activation has not yet been elucidated in RAS mutated pre-B ALL. In this study, we aimed at identifying the molecular mechanism underlying pro-survival IRE1α-XBP1 signaling in RAS mutated pre-B ALL. Methods For a TET-ON inducible NRASG12D model in conditional Xbp1 knockout mice, we used interleukin 7 (IL-7)-dependent murine Mx1-Cre;Xbp1fl/fl pre-B cells transduced with a TET-ON inducible NRASG12D. We performed in vitro cell cycle and apoptosis assays with propidium iodide (PI) and annexin-V/PI. Furthermore, Western Blot and RT-qPCR were applied to analyze target gene expression. In a second approach, we focused on the signaling events following the blockade of RAS downstream targets using the MEK inhibitor PD0325901 and the dual PI3K/mTOR inhibitor BEZ235. We then assessed the efficacy of small molecule inhibition of IRE1α by MKC-8866 on XBP1 inactivation in RAS-mutated pre-B ALL cells either as a single treatment and in combination with the above mentioned drugs. Results We found the expression of Xbp1 significantly increased at the mRNA level with induction of NRASG12D. To determine the significance of Xbp1 in NRASG12D-driven pre-B ALL, we genetically deleted the IRE1α target Xbp1 using Cre-mediated deletion of Xbp1fl/fl in our mouse model of pre-B ALL. Genetic loss of Xbp1 significantly induced apoptosis (2.0-fold, p<0.0001) and caused cell cycle arrest (induction of G0/1, 1.7-fold, p=0.0003) along with an increase in the expression of CDK inhibitors, p21CIP1 and p27KIP1 at the protein level. Genetic ablation of Xbp1 abrogated IL-7 receptor (IL-7R) signaling by reducing the phosphorylation levels of STAT5-Y694 and JAK1-Y1022/Y1023. In an additional approach, we revealed that IL-7-deprived pre-B ALL cells reduce the mRNA expression of Xbp1s, indicating that Xbp1 acts as a downstream linchpin of the IL-7 receptor signaling pathway. Both IL-7-deprivation and genetic loss of Xbp1 increased the phosphorylation levels of ERK1/2-T202/Y204, AKT-S473 and the protein levels of NRASG12D and MAPK negative regulator DUSP6. Pharmacological inhibition of XBP1 activation using MKC-8866 resulted in similar effects on the expression of RAS downstream targets. We therefore tested MKC-8866 in combination with MEK inhibition by PD0325901 as a potential therapeutic strategy against pre-B ALL, which proved non-efficient. As a second option with therapeutic implications, we focused on the PI3K pathway which acts downstream of both the IL-7R and RAS signaling pathways. Strikingly, we observed that genetic ablation of Xbp1 (viable cells after 72 h, BEZ: 71.9 ± 9.0 vs BEZ+ Mx1-Cre;Xbp1fl/f: 10.0 ± 4.9) or pharmacological inhibition of its production with MKC-8866 (viable cells after five days, BEZ: 58.0 ± 6.8 vs BEZ+ MKC-8866: 13.3 ± 7.4) sensitizes pre-B ALL to dual inhibition of PI3K/mTOR with BEZ235. By applying the Bliss formula, we were able to show that BEZ235 in combination with MKC-8866 synergistically reduces the viability of RAS-mutated pre-B ALL cells. Gene expression analysis indicated that BEZ235 in combination with MKC-8866 fully blocked IL-7R signaling and caused an aberrant activation of Ras-Erk signaling. Targeting PI3K/mTOR signaling along with XBP1 inactivation increased expression of NRASG12D and its target DUSP6. In addition, we showed that combined therapy increased expression levels of p19Arf in RAS-mutated pre-B ALL, implicating cell senescence mediated by activated RAS signaling. Conclusion Our work strongly supports the hypothesis that XBP1 induces its positive effects on progression of pre-B ALL cells through the IL-7R signaling pathway. IL-7R signaling through its downstream effector XBP1 counteracts the RAS signaling pathway to promote leukemogenesis in pre-B ALL cells. Active XBP1 prevents the cytotoxic effects of BEZ235 in pre-B ALL cells, and hence targeting XBP1 in combination with dual PI3K/mTOR inhibition by BEZ235 appears as a promising targeted strategy against the "undruggable" driver RAS in NRASG12D-mutated pre-B ALL. Abstract Background: On the basis of efficacy in mouse tumor models, multiple CD137 (4-1BB) agonist agents are being preclinically and clinically developed. The costimulatory molecule CD137 is inducibly expressed as a transmembrane or as a soluble protein (sCD137). Moreover, the CD137 cytoplasmic signaling domain is a key part in approved Chimeric Antigen Receptors (CARs). Reliable pharmacodynamic biomarkers for CD137 ligation and co-stimulation of T cells will facilitate clinical development of CD137 agonists in the clinic. Methods: We used human and mouse CD8 T cells undergoing activation to measure CD137 transcription and protein expression levels determining both of the membrane-bound and soluble forms. In tumor-bearing mice plasma sCD137 concentrations were monitored upon treatment with agonist anti-CD137 mAbs. Human CD137 knock-in mice were treated with clinical-grade agonist anti-human CD137 (Urelumab). Sequential plasma samples were collected from the first patients intratumorally treated with Urelumab in the INTRUST clinical trial. Anti-Mesothelin CD137-encompassing CAR-transduced T cells were stimulated with mesothelin coated microbeads. sCD137 was measured by sandwich ELISAs and Luminex and flow cytometry was used to monitor CD137 surface expression. Results: CD137 co-stimulation upregulates transcription and protein expression of CD137 itself including sCD137 in human and mouse CD8 T cells. Immunotherapy with anti-CD137 agonist mAb resulted in increased plasma sCD137 in mice bearing syngeneic tumors. sCD137 induction is also observed in human CD137 knock-in mice treated with Urelumab and in mice transiently humanized with T cells undergoing CD137 co-stimulation inside subcutaneous implanted Matrigel® plugs. CD137 signaling domain-containing CAR T cells readily released sCD137 upon antigen recognition in a manner favored by the CD137 signaling domain. Patients treated intratumorally with low dose Urelumab showed increased plasma concentrations of sCD137. Conclusion: sCD137 in plasma and CD137 surface expression can be used as quantitative parameters dynamically reflecting therapeutic co-stimulatory activity elicited by agonist CD137-targeted agents. Abstract Background: Recapitulation of the full spectrum of genomic changes driving patient tumors have resulted in increased use of patient-derived xenograft (PDX) models in studies of basic cancer biology and preclinical drug development. Given the translational potential of PDXs and limited availability of pediatric cancer models, we established a PDX program to expand the existing collection of pediatric PDXs in the community and enable pre- and post-clinical studies. Methods: PDX generation requests were integrated into clinical workflows to maximize identification of eligible patients for informed consent and tissue collection at Memorial Sloan Kettering Cancer Center. Methodologies for tissue procurement and cryopreservation were optimized to facilitate implantation into host immunodeficient mice and enable multi-institutional tissue exchange for model building. A bioinformatics pipeline was established to allow molecular validation of engrafted PDXs using a next-generation targeted gene panel (MSK-IMPACT) evaluating concordance based on acquired mutations, copy number alterations and clonal structure. Results: Between November 2016 - October 2021, 379 PDX models were developed (265 distinct models) representing 69 discrete diagnoses. Sarcoma represents the most common model type (50 discrete osteosarcoma, 20 desmoplastic small round cell tumor, 14 Ewing sarcoma, 24 rhabdomyosarcoma, 2 CIC/DUX4 and 2 BCOR-rearranged sarcoma) followed by neuroblastoma (n=35), leukemia (n=44), and Wilms tumor (n=15). While the majority of PDXs were established from recurrent or metastatic tissue, 7 paired diagnostic/pre-therapy and post-therapy or relapse models were generated. Genomic characterization of PDXs demonstrate excellent concordance and recapitulation of single nucleotide variants (90%), structural (88%) and copy number variants (94%) between patient tumor and matched PDX. Discrepancies between matched patient/PDX pairs are due to sub-clonal heterogeneity in source tumors with clonal outgrowth in the PDX. Analysis of serial PDX passages also demonstrate stable recapitulation of the genomic profile. Establishment of a diverse PDX collection allowed preclinical evaluation of 10 targeted agents across a spectrum of pediatric tumors and provided the preclinical rationale for 3 investigator-initiated pediatric clinical trials. Conclusions: Investment in the development of a phenotypically diverse and biologically faithful collection of pediatric PDX models enables the goals of precision medicine. Optimization of PDX workflows and methods has also enabled the development of a pediatric PDX consortium (PROXC - Pediatric Research in Oncology Xenografting Consortium) to further support the development of pre- and post-clinical studies for pediatric cancer. Until the health-related quality of life (HRQoL) accrual goal was reached, English speaking pts in the US were invited to complete HRQoL questionnaires including the 8-item PROMIS Perceived Cognitive Function Concerns (PCF) Short Form questionnaire shortly after randomization (baseline), as well as 6, 12, and 36 months after baseline. Analysis of measures of anxiety and fatigue is presented separately. Standardized T scores (mean 50; SD 10) for PCF were computed with higher scores indicating less cognitive impairment. The primary endpoint of this exploratory analysis was to compare mean PCF T scores by treatment arm and menopausal status. Separately by menopausal status, a generalized estimating equations (GEE) model was fit to the three timepoints adjusting for baseline to estimate the difference between treatment arms and whether there was a time trend over the three follow-up measures. Results: The HRQoL accrual exceeded the goal of 500 patients, with 74% of pts participating voluntarily until the QOL invitation was removed from the protocol (Dec 1, 2012). A total of 139 pre and 429 postmenopausal pts completed the questionnaires at baseline. T scores were similar between ET and CET arms at baseline [Table 1]. In the ET arm, T scores decreased from baseline to 6 and 12 months but recovered to baseline at 36 months. In the CET arm, T scores decreased from baseline to 6 months and 12 months but did not return to baseline at 36 months. The mean score difference between CET and ET over time was -3.02 (p=0.01) and -2.36 (p=0.003) for pre and postmenopausal pts, respectively. Adjusting for baseline, there was no significant time trend over the three follow-up periods for either premenopausal (p=0.12) or postmenopausal (p=0.49) pts. Dropoff occurred over time with 79%, 76%, 60% of pts at baseline participating at 6, 12, and 36 months. Complete endocrine treatment adherence data are not yet available at each timepoint. Conclusion: Chemoendocrine therapy has a greater negative effect on patient-reported CRCI compared to ET alone in both pre- and post-menopausal pts and it is sustained over 36 months. Interventions to prevent or treat CRCI are needed to improve the long-term quality of life of patients treated with CET. Table 1. Background:Tofacitinib (TF) is an oral Janus kinase inhibitor approved for PsA treatment. It has demonstrated comparable to ADA efficacy in RCT in reducing PsA clinical symptoms1,2. There is currently no data concerning comparative effectiveness of TF and ADA in clinical practice. The Russian PsA RegisTry (RU-PsART) collected data from 43 rheumatology centers of the Russian Federation.Objectives:to study responses to TF or ADA over a period of 6 months (mo) treatment in patients (pts) with active PsA in real clinical practice.Methods:77 (M/F=43/34) PsA pts fulfilling the CASPAR criteria from the RU-PsART cohort were divided into two groups according to the treatment given. 41pts (M/F=24 (58.5%)/17 (41.5%), mean age 42.4±10,3 years (yrs), median (Me) PsA duration 72 [35;120] mo were treated with TF 5 mg twice daily. 36 pts (M/F=19 (52.8%)/17 (47.2%), mean age 44±11.5 yrs, Me PsA duration 59 [22;102] mo were treated with ADA 40 mg/2 weeks, subcutaneous, as a first-line bDMARD. In TF/ADA groups 89%/52.8% pts accordingly were given combination therapy with Methotrexate (MTX). At baseline (BL) and at 6 mo of therapy PsA activity by DAPSA, BASDAI, the number of pts (NPts) with dactylitis, enthesitis by LEI, BSA, HAQ were evaluated in both groups. At 6 mo of therapy the NPts who achieved remission by DAPSA (≤4)/Low activity (LA)≤14) and Minimal disease activity (MDA, 5 of 7) were calculated in both groups. M±SD, Me [Q25; Q75], Min-Max, %, t-test, Pierson-χ2, Manna-Whitney, Wilcoxon tests were performed. All p<0.05, were considered to indicate statistical significance.Results:At BL TF pts had higher PsA activity by DAPSA compared to ADA group: 44.2[37.8; 55.3]/35.8 [21.1; 52] accordingly (p=0.03). At 6 mo of treatment DAPSA significantly decreased in TF/ADA groups - to 11 [4,3; 17,3]/9,1[6; 19,6] (p<0,001) accordingly. At 6 mo of therapy there were no significant differences between groups in achieving MDA, DAPSA LA/Remission (table 1). At 6 mo of treatment in TF-treated group the NPts with dactylitis significantly decreased from 22 (53,7%) to 5 (13,2%) pts (p<0.001), and in ADA-treated group no significant differences were found in NPts with dactylitis [despite a quantitative decrease from 13 (36,1%) to 6 (20%) pts (p=0.15)]. The NPts with enthesitis in both TF/ADA groups decreased significantly: from 27 (69,2%) to 12 (30,8%) (p<0,002) and from 16 (72,7%) to 5 (38,5%) (p<0.046) pts accordingly. In TF/ADA-treated pts BASDAI improved from 6 [4.2;7] to 1.4 [0.6; 3.2] (p<0.001)/from 4.4 [1.9;5.8] to 3 [0.8;4.5] accordingly (p<0.046); HAQ significantly decreased from 1 [0.625;1.5] to 0,5 [0; 0.875] and from 0.875 [0.5;1.38] to 0.5 [0; 0.875] accordingly.
