This paper was first presented at the NAV99/ILA28 Conference on ‘Loran-C, Satellite and Integrated Systems for the 21st Century’ held at Church House, Westminster, London from 1–3 November 1999.The introduction of modern navigation systems highlights the need for efficient tools to assess the possible impact of these systems on the safety levels currently associated with the operation of a ship. In recent years this has led to investigation of the advanced safety/risk assessment techniques already applied in other industrial sectors, with encouraging results. The scope of this paper is to show a quantified safety assessment methodology that can be applied while designing or retrofitting navigation systems. The methodology adopted is the result of the review of the IMO Formal Safety Assessment (FSA) technique and comprises the development of a functional analysis, a hazard identification analysis and a risk assessment. The paper provides details on a specific application of this model to an integrated navigation system. This application is included in the work performed under the ATOMOS II research project, partly funded by the DGVII Directorate of the European Commission within the 4th Framework Programme in the field of Maritime Transport.

The medical industry is faced with new devices and technology on a regular basis. The multiple goals of these devices and technologies vary, depending on the user’s angle. Ultimately, they hopefully provide the best care for patients in the most efficient way possible, while containing costs and maintaining patient safety. Recent studies have found that rapid implementation of new medical technology — surgical devices, electronic health records, monitoring systems and other tools — can lead to adverse patient events when implementation is not thoughtfully and carefully integrated into the workflow. This integration requires not only a thorough understanding of how the new tools work, but also of how they can be safely integrated into the system — including an analysis of human factors, such as in environments where people interact with these devices repetitively or in high-pressure situations.

AbstractAs digitization progresses, the integration of information and communication technologies in technical systems is constantly increasing. Fascinating value potentials are emerging (e.g. autonomous driving), but also challenges in the system development. The constantly increasing product complexity and degree of networking require a systemic development, which is fulfilled by established approaches of Model-Based Systems Engineering (MBSE). To ensure the reliability of tomorrow's systems, an integrative and early consideration of security and safety is additionally required. In order to show the possibility and consequences of failures and attacks, the paper develops a modeling language that links established and partly isolated security and safety approaches within a consistent metamodel. The developer is enabled to synthesize system architectures transparently on an interdisciplinary level and to analyze attack and failure propagation integratively. The approach uncovers synergetic and especially contrasting goals and effects of architectural designs in terms of safety and security in order to make adequate architectural decisions based on trade-off analyses.

This paper discusses the strategies for integrating Quality, Environmental, Safety and Health Management Systems based on survey and case studies results. Questionnaires were distributed to 87 companies that certified with both ISO9001 and ISO14001. Meanwhile, three case studies were conducted at the manufacturing companies that have integrated several management systems. There are two ways of integrating the management systems which are: (1) consecutive implementation of management systems followed by integration or (2) integrate the management systems simultaneously from the beginning. Based on survey and case studies, it was found that many organisations started with implementing individual management system first, and then followed by integrating the management systems. Almost all the survey respondents agreed that the sequence should start with establishing Quality Management System first, and then integrate with Environmental Management System and followed by Occupational Health and Safety Management System.

The purpose of this paper is to present a computerized integrated project management system and report results of a survey on the effectiveness of the system. The system consists of a scheduling system, material management system, labor/equipment system, and safety/quality control system. The backbone system is a scheduling system that adopts a production planning system and a project scheduling system. The lowest level in the scheduling system is a daily work management system, which is linked to each functional management system (i.e. material management system, labor/equipment system, and safety/quality control system). The paper focuses on the material management and scheduling systems to implement a material “pull” system to reduce material inventories on site. Details of material management and scheduling systems are discussed, and a sample application is presented to demonstrate the features of the proposed computer application system. The paper presents practitioners and researchers with a practical tool to integrate material management and scheduling systems for site personnel.

Many organisations find it challenging to effectively integrate separate internal management systems and processes into one cohesive framework that supports organisational strategy. A management system is a framework of policies, processes and procedures developed to ensure an organisation can complete the tasks necessary to achieve its strategic objectives. International and industry standards typically take a functional view of management systems, often resulting in silos of information. In addition, inefficient and isolated safety, environmental, process safety, risk, compliance and quality management systems cost time and money, and can increase the likelihood of safety incidents in critical development and production environments. To support Woodside’s vision to be a global leader in upstream oil and gas, Woodside needed to establish a common approach to how it operates, wherever the location, and develop a strong foundation to support its strategic objectives. Woodside recently undertook an internally led company-wide project to renew its Woodside Management System (WMS) to improve integration between disparate functionally based management systems and capture the way we work more effectively. As part of the project, Woodside took the opportunity to redesign its WMS framework and streamline its processes and procedures by simplifying and removing duplicate or redundant existing information. Through the project, Woodside developed a single integrated management system that is independent of organisational structure and closely linked to our vision. The integrated control framework created within the WMS supports conformance to process and forms the foundation of assurance activities across the entire company. The WMS integrates a hierarchy of business processes, focused upon value stream activities, and clearly defines operational processes and activities. This paper discusses the objectives and outcomes of the project, some challenges during implementation and the change management approach to develop the culture and behaviours to drive continuous improvement and unlock the benefits of an integrated management system.

The two cardinal aspects of safety are intrinsic safety and its demonstration. A key element of demonstrating system safety is the organizational structure that is best placed to ensure and demonstrate that high standards of safety are clearly in place for its products, processes and facilities. This is particularly important for high-consequence industries. Accomplishing this depends on a number of organizational integrated layers of scrutiny, ranging from the accumulation of arguments and evidence at the lower levels to final executive decision making. The latter holds final responsibility and accountability. Potential problems arise because products, processes and facilities are becoming more and more complex and the associated supporting data inordinately large. In turn, the organizational processes that enable top-level decision makers to make wise and informed decisions are themselves becoming more complex and difficult. This final stage requires clear and transparent communication. Organizations have moved more towards the application of peer review to support final decision making but, nevertheless, one still expects the final decision-making layer to provide further independent scrutiny to enhance overall confidence in the process. This would represent a three-tier independent process — strength in depth. This is the subject of the paper.

Intelligent transportation integrated access system can significantly improve the security of computer network systems, while trusted computing technology can effectively enhance the credibility and safety of the terminal, and then construct a trusted network connection. Based on computing technology, this thesis proposes to establish a credible integrated access network, and implement network access control on the remote access equipment, thereby enhancing the safety of the integrated access system. The results show that, through the transmission technology of credible chain proposed in this paper, and remote proof technology basing on device identity and security policies can establish a credible integrated access system, thus effectively improving the security of Beijing intelligent transportation systems.

The International Civil Aviation Organization, in its document Safety Manual (Doc 9859), notes the importance of integrating a safety management system and a quality management system, which takes into account all areas of the civil aviation organization and the formation of interrelated processes to achieve common safety goals. There are discusses the method of searching for critical elements of an aviation enterprise based on risk events in an integrated management system, the main principle of which is the establishment of quality indicators and fiight safety indicators. With this approach, the distribution of indicators by type of activity (classes) is carried out, which in turn allows the study of elements of the aerodrome operator system based on the theory of fuzzy sets (Fuzzy Sets), in accordance with the concept of reliability, risks, safety from the theory of system safety. The results obtained allow us to proactively develop and implement measures that exclude the occurrence of a negative event in the activities of an aviation enterprise, such as a catastrophe, accident, emergency, or a serious aviation incident.

The scope of this project was to produce a complete prototype of an electromechanical safety system that (1) detects human contact with the saw blade, (2) interrupts power to the saw motor, (3) actuates a firing system, and (4) abruptly stops the travel of the blade in order to minimize physical injury. The specific application for the project was a 12.7 mm wide, 0.5 mm thickness Ridgid® saw blade (Part # 03991) mounted on a Ridgid® 14“ vertical band saw (Model #BS 1400). Through engineering analysis and several design iterations, a wedge-brake mechanism was developed and integrated with an electronic control system developed by SawStop™. Upon completion of the nine-month project, the prototype was able to stop the motion of the saw blade in approximately fifteen milliseconds (ms). With this stopping time, it is estimated that the operator would experience a 4.8 mm (3/16 inch) depth of cut. Thus, first aid or a doctor's visit would be required rather than emergency hospitalization. The final project report included recommendations for further development.

Purpose. For chemical, food, woodworking, pharmaceutical and some other industries, the main fire hazard factor is the circulation of explosive dust and gas flows containing combustible dust. For their disposal and removal from the volumes of equipment and premises, production aspiration systems (PAS) are used, their effective functioning is the key to production safety, so the given work is aimed at developing an automated PAS control system that ensures fire safety, as well integrating PAS control system into ACS of the production. To achieve this goal, the tasks of identifying the main hazards, developing an information environment for the system, developing mechanisms for the interaction of PAS accident triggers with the information environment and other subsystems of the automated control system, and, ultimately, creating the concept of a single system – an integrated production safety management system (IPSMS) are considered. Methods. The task is carried out by systematizing information related to the specifics of developing and operating industrial aspiration systems and evaluating information using logical functions, as well as developing the model based on the graph theory, namely, describing the system operation using Petri nets. Findings. To achieve the goal, the concept of an integrated production safety management system is proposed. The concept takes into account the peculiarities of the technological process and integrating with the subsystems of the production automated control system to neutralize both local accidents and accidents that develop into large-scale emergencies. The system considers the specifics of the main technological equipment and hardware solutions for dust collection in PAS, the degree of production automation, the categories of premises according to explosion and fire hazard. Based on the information analysis, a sequence of actions has been developed to eliminate the accident or minimize its consequences. To implement the tasks of IPSMS, a mathematical model based on the Petri net has been developed. Research application field. The results obtained can be used to design and improve automated systems for preventing accidents, fires and explosions in industries associated with the handling of combustible dust. Conclusions. The use of IPSMS will simplify the decisionmaking process linked with the analysis of a large amount of highly specific information related to the operation of industrial aspiration systems. The proposed methods and models are reliable tools\for the system of intelligent decision making support in stressful situations.

The present article has the particularity of qualitatively analyzing the scientific literature available in the databases Dialnet, Scielo, Redalyc, official web pages and Google Scholar, using as search criteria the following keywords: industrial safety, occupational health, safety and health management systems in the work and integrated management systems. Relevant information related to the proposed objective was obtained, which is presented in three sections: occupational health and safety, occupational health and safety management systems and HSEQ integrated management systems, which help organizations to permanently improve the quality of products and services, to establish and evaluate programs, policy, and objective control. The challenge is the awareness in the sector of small, medium and large companies about the importance of the integrated management system.

There is a need to integrate advancements in biomedical, information, and communication technologies with care processes within the framework of the inpatient safety program to support effective risk management of adverse events occurring in the hospital environment and to improve inpatient safety. In this respect, this work presents the development of a software platform using the Scrum methodology and the integrated technology of the Internet of Things for monitoring and managing inpatient safety. A modular solution is developed under a hexagonal architecture, using PHP as the backend language through the Laravel framework. A MySQL database was used for the data layer, and Vue.js was used for the user interface. This work implemented an RFID-based nurse call system using Internet of Things (IoT) concepts. The system enables nurses to respond to each inpatient within a given time limit and without the inpatient or a family member having to approach the nursing station. The system also provides reports and indicators that help evaluate the quality of inpatient care and helps to take measures to improve inpatient safety during care. In addition, diet management is integrated to reduce the occurrence of adverse events. A LoRa and Wi-Fi-based IoT network was implemented using a LoRa transceiver and the ESP32 MCU, chosen for its low power consumption, low cost, and wide availability. Bidirectional communication between hardware and software is handled through an MQTT Broker. The system integrates temperature and humidity sensors and smoke sensors, among others.

The article discusses the application of an integrated approach to the use of mechatronic security systems in the event of emergencies. A brief analysis of the available security systems is presented. Shown is a schematic diagram of a mechatronic security system in an emergency mode. The algorithm of operation of the mechatronic safety system, which includes a module with intelligent control, is considered. The structural and functional diagram of the security and fire system is presented, which makes it possible to qualitatively implement an emergency response system directly at any facility. Recommendations for the design of a security system based on the developed functional model are given.

With the complexity of the socio-technical system, the requirement for safety analysis is growing. In actuality, system risk is frequently created by the interaction of numerous nonlinear-related components. It is essential to use safety assessment methods to identify critical risk factors in the system and evaluate the safety level of the system. An integrated safety assessment framework combining the system theoretic process analysis (STPA), the analytic network process (ANP) and system dynamics (SD) is suggested to analyze the safety level of socio-technical systems to achieve qualitative and quantitative safety evaluation. Our study constructs an STPA and SD integration framework to demonstrate the practical potential of combining STPA and SD approaches in terms of risk factors and causality. The framework uses the STPA method to define the static safety control structure of the system and analyzes the primary risk factors. The unsafe control actions (UCAs) from the STPA method are transformed into network layer elements of ANP. The ANP method is used to calculate the element weights, which are the impact coefficients between the system dynamics (SD) variables. The SD method is used to assess the safety level of the system. Finally, a specific coal mining system is used to demonstrate how the proposed hybrid framework works. The results indicated that the safety level of the system was low on days 38 and 120 of the simulation cycle (one quarter). Our work can overcome the limitations of conventional STPA quantitative analysis and simplify SD qualitative modeling to serve as a reference for complicated system safety/risk analysis work.

Modern tendencies in food industry development (including meat products processing) are connected with compulsory advance and improvement in production techniques as well as implementation of effective management systems which stem from the integrated approach to risk management. The development and implementation of the systems which would guarantee required and stable safety of products for consumers are very important as well as their quality. For that reason it is essential to develop the models of integrated management system which will be based on the quality management system (QMS) and food safety management system standards. The authors performed comparative analysis of the requirements of the integrated standards. As a result of the analysis and expert evaluation they determined ten areas of integration, developed the matrix of requirements for the elements of the integrated system. The authors studied the risks, gave scientific evidence concerning the occurrence of potential hazards, determined unacceptable risks, pointed out critical control points and developed food safety management system plan for management in these points. They also determined the regularities in the influence of the quality loop stages on the quality and safety parameters, identified and structured the processes using IDEF0 function modeling methodology, made the blocks of graphs that show the interaction of processes determining the directions of information and product flows and reflecting the interaction of processes. The authors developed an integrated system model and determined the information requirements for its functioning. They used the procedure of the system performance assessment in a company using self-assessment criteria. The authors showed the advantages of the implementation and improvement of the integrated system in the company located in Saransk, the Republic of Mordovia.

Implementing separate Environmental Management System (EMS) and Safety and Health Management System (SHMS) can be costly for organisations and hence the advent of integrated management systems. The effective implementation of integrated safety, health and environmental (SHE) management would require companies to have the appropriate organisational capability. Within the academic literature, it is unclear which organisational attributes are important for ascertaining integrated SHE management capability of construction companies. This study sought to address this through a comprehensive review of literature relating to SHE management in construction and SHE management systems and models. The study revealed that organisational attributes that could determine integrated SHE management capability include: senior management commitment to SHE; SHE risks management; SHE objectives and programs; staff competencies; resources for SHE implementation; SHE roles and responsibilities; SHE communications; SHE documentation and control measures; SHE emergency plans; SHE monitoring and performance measurement; and SHE auditing and management review to capture lessons learned. These attributes could enable construction companies and other key industry stakeholders to understand construction companies’ capability to implement an integrated SHE management system.

Integrated management system (IMS) can integrate several management systems in companies such as: Environmental Management Systems (ISO 14001) and Occupational Health and Systems Safety Management (ISO45001). The purpose of this study is to answer the question of what and how to implement IMS policies in industrial companies in the fields of occupational safety, occupational health and environmental sustainability. This IMS implementation research was conducted on a manufacturing industry that has implemented IMS. The research method is conducted with qualitative methods with CIPP evaluative model (Context, Input, Process, and Product). This study succeeded in uncovering the implementation of IMS in the manufacturing industry which was investigated that the IMS had been fully implemented in the Occupational Health and Environmental Management and Safety System (HSE). The IMS is implemented at the policy level or management commitment (context), HSE planning (Input), operational implementation (process), HSE monitoring and evaluation and a review (Product) for continuous performance improvement.

The safety and stability of the transportation process in railway transport is provided by a set of various measures. However, violations, incidents and accidents continue to occur, causing significant damage to workers, infrastructure, natural environment, and population. The relevance of the study is determined by the need of finding ways to prevent this damage and reduce the socio-economic and environmental costs. The purpose of the study is the development of an integrated safety system for traffic and workers in railway transport. A set of theoretical and experimental research methods has been applied: physical, simulation and computer modeling; statistics, theory of probability and others. The main result of the development is a complex system of interrelated units: “Sphere of activity”; “Qualification of personnel”; “The required level of work”; “Rating of danger”; “Optimization and technical and economic assessment of company costs”. Requirements for an information management system were developed for the holding company “Russian Railways”. This system allows personal accounting of the qualifications of employees (over 900 thousand) in the main areas of activity: ensuring traffic safety; occupational safety and health; transport, industrial, fire and electrical safety; protection in emergency situations. The project of the normative and technical document regulating the corresponding changes is substantiated. On the basis of theoretically processed experimental material, the expected development efficiency was established: 8-10%.
