The monograph presents the generalized results of the author's scientific activity related to the diagnosis and phenomenological description of the state of self-consciousness and the Self-concept of persons with developmental disorders, in particular with mild mental retardation. The variants of self-awareness diagnostics and a comprehensive assessment of the personal adaptive potential of subjects of different ages with intellectual disabilities are presented and methodologically justified. The presented methods allow us to form an individual adaptive profile based on a quantitative assessment of qualitative indicators of adaptability/maladaptivity. The criminalization of people with mental illness is a sad commentary on the United States’ mental health system. Yet, the phenomenon presents the field of psychiatry with an opportunity that is now scarce in civil society: lengths of sentence in terms of weeks to years that allow for in-depth observation and treatment of the inmate with mental illness. A few days in a hospital fails to provide the needed opportunity for a detailed and accurate evaluation. Today, people with mental illness account for more than one million annual arrests and many among these individuals will spend weeks to months in jail before being either transferred to a prison for sentences beyond one year or released back into the community. At its core, psychiatric diagnosis relies on the subjective complaints of the patient and objective signs noted on examination. Considering the chronic and fluctuating course of most psychiatric diagnoses, a thorough assessment also requires a review of past documented behaviors. When someone is hospitalized for a psychiatric condition, the first goal is often observation, followed by diagnosis, and then treatment. Psychiatric hospitals are being greatly constrained in the amount of time available for observation and accurate diagnosis; the correctional setting, as an unintended consequence of mass incarceration, provides an extended opportunity to achieve improved diagnostic accuracy. This chapter reflects on the diagnostic opportunities that a jail or a prison setting affords.

Since the mid-twentieth century, the DSM has guided psychiatrists in categorizing disorders. Revisions have taken years, with work groups considering changes to the entire manual. A more timely and efficient approach to updating the DSM would involve continuous improvement of particular diagnostic categories, when and if supported by advances in the field. The aim is to avoid the delays in the incorporation of new knowledge that are inherent in updating at intervals of a decade or more. The American Psychiatric Association has therefore established a structure by which evidence-based proposals for changes to the DSM can be considered and adopted on an ongoing basis. This chapter describes how proposals will be considered and the standards to be applied to proposed modifications. Challenges include calibrating optimal rigor of the review process, pruning diagnoses that lack validity or clinical utility, and subjecting the process itself to iterative improvement.

Communicating this information to both the referring physician and often directly to the patient has become increasingly important as the role of radiologists in patient care has evolved. As technology advances, and the field of radiology extends beyond the interpretation of diagnostic imaging into that of intervention and treatment, timely and clear communication of imaging results, limitations of radiology examinations, and the risks associated with image-guided interventional and therapeutic procedures is a priority. Instituting structured reporting, reporting lexicons, and formal communication skills training for radiologists are a few measures that radiologists can take to improve communication in the field. Such efforts to improve communication in radiology are integral components to enhancing and expanding the role of radiologists in patient care.

The classification of a cerebrovascular event distils information obtained in the workup and other related case history to a category indicating a specific pathophysiology, with direct implications for the subsequent secondary prevention management. A minimum standard of diagnostics is required for a precise classification. In young stroke victims, the conventional Trial of Org 10172 in Acute Stroke Treatment (TOAST) stroke subtype classification may only address the aetiopathogenesis in 30–60% of the individuals who are affected by vascular risk factors. Applying the criteria defined by TOAST for large-artery atherosclerosis, cardioembolism, and small artery disease is reliable for young stroke victims as verified in many clinical investigations. However, in a considerable proportion of stroke patients, the stroke aetiology remains unclear. For this group, some patients need to be treated in a particular manner, such as those with two or more possible aetiologies or those with suspected paradoxical embolism. In patients remaining purely cryptogenic despite an extensive diagnostic workup, imaging findings may help to identify potential triggers, particularly when an embolic infarction pattern in the acute brain imaging is obvious. Despite its shortcomings, the TOAST classification is the most universally used classification in ischaemic stroke patients. New developments in the field are addressed in this chapter and aspects are incorporated for the nominated classification adapted for the specific group of young stroke patients.

Stress echocardiography is the combination of two-dimensional echocardiography with a physical, pharmacological, or electrical stress. The diagnostic end point for the detection of myocardial ischaemia is the induction of a transient worsening in left ventricular regional function during stress. Among different stress modalities of comparable diagnostic and prognostic accuracy available, semisupine exercise is the most frequently used; dobutamine-the best test for viability assessment; dipyridamole-the safest and simplest pharmacological stress test; and the most suitable for combined wall motion-coronary flow reserve assessment. Identification of viable myocardium and evaluation of severity of valvular heart disease are additional recognized applications of stress echocardiography. In spite of its dependence upon operators’ training, stress echocardiography is today the best (most cost-effective and risk-effective) possible imaging modality to achieve the still elusive target of sustainable cardiac imaging in the field of non-invasive diagnosis of coronary artery disease.

Filarial nematodes have been known to cause human disease for many centuries. Lymphatic filariasis is a common disease in the developing part of the world and much has been written about diagnosis and treatment of this scourge. Wuchereria , Brugia and Onchocerca (especially O. volvulus) have a wide pattern of distribution with severe morbidity. Given the years of scientific work in this field, many drugs that work against these parasites are available today and are attempting to control these infections. In this chapter, the focus is on those filarial nematodes that do not have humans as their primary host. Instead, the filarial organisms that usually parasitize other animals and cause human infection due to a variety of factors are discussed. These factors include: 1. Proximity of humans to the primary host, 2. Proximity of humans to the vector, 3. Changing ecology with introduction of different animals (both host and vector) into new environments, 4. Increasing human mobility, 5. Special scenarios concerning humans, including altered immune function (immunosuppressed due to drugs, auto-immune illness, immunosuppressive diseases), There has been a recent interest in this field because newer diagnostic techniques, including polymerase chain reaction (PCR) assays, DNA primers and electron microscopy have become widespread in use. This will eventually enhance our understanding of the pathophysiology of infections with these seemingly rare filarial organisms.Much of the early work in this field was done in a few specialized centers. As information about these parasites (through the worldwide web) and diagnostic techniques are now widely available, it is our hope that more work regarding these nematodes will be carried out in the developing countries where these infections are common. In this chapter, we focus on Dirofi laria, Meningonema, Loaina, Dipetalonema and certain species of Onchocerca and Brugia.

This chapter reviews the essential physical principles involved in the generation of electroencephalographic (EEG) and magnetoencephalographic (MEG) signals. The general laws governing the electrophysiology of neuronal activity are analyzed within the formalism of the Maxwell equations that constitute the basis for understanding electromagnetic fields in general. Three main topics are discussed. The first is the forward problem: How can one calculate the electrical field that results from a known configuration of neuronal sources? The second is the inverse problem: Given an electrical field as a function of space and time mostly recorded at the scalp (EEG/MEG), how can one reconstruct the underlying generators at the brain level? The third is the reverse problem: How can brain activity be modulated by external electromagnetic fields with diagnostic and/or therapeutic objectives? The chapter emphasizes the importance of understanding the common biophysical framework concerning these three main topics of brain electrical and magnetic activities.
