Prone positioning (also known as ‘proning’, ‘prone manoeuvre’ or ‘prone ventilation’) refers to mechanical ventilation with patients positioned in prone position in contrast of standard supine (flat or semi-recumbent) position. The use of the prone positioning was proposed over 30 years ago as a means to improve arterial oxygenation in patients with acute respiratory distress syndrome (ARDS). Since then, extensive physiological research has been conducted to explore the possible mechanisms underlying the observed improvement in gas exchange, which involve changes in the distribution of both ventilation and pulmonary blood flow. Furthermore, it has been shown that, independently of gas exchange, prone positioning may reduce the harm of mechanical ventilation, which is known to adversely impact patient survival. In this chapter we will summarize the physiological effect of prone positioning, as well as the clinical evidences supporting its use to reduce mortality in patients with ARDS.

On the basis of a review of literature, the impact of groupthink and the importance of a democratic atmosphere in teams are discussed. An example of leadership in teams is provided by the interaction between a successful coach and his team during a soccer world championship. Leadership on the level of the organization is explored by giving the example of two merging organizations and their problematic cooperation. Leadership on the level of a world organization is exemplified by an analysis of the successes and failures of the United Nations. At the end of this chapter the concept of “flexible democracy” is presented. In this concept two lines of exploration come together, one referring to the movements on the dimensions of power distance and emotional distance and the other referring to the connection between self, team, and organization.

This chapter provides the narrative framework within which the nationalist discourses in the three states are analysed. The chapter is divided into three sections. The first section deals with a review of literature on nationalisms that have prevailed in India after Independence, and which are relevant to positioning the Bangladeshi migrants present in the three states. The second section points to some key concepts from the non-Indian literature that invites attention. The last section states the two axiomatic assumptions that underlie the narrative structure and which give the study a unity. The first assumption is that the discourse of nationalism starts with colonialism, the second assumption is that partitions have had defining impact on nationalist thought in India.

British politics was transformed by the extension and equalisation of the franchise after 1918: the electorate at the 1929 general election was four times the size it had been in 1910. Traditionally, this was seen as a development that aided the Labour Party. More recent studies have emphasised that there was no simple relationship between class and political allegiance. Less attention has, however, been accorded to the impact of the new franchise on political culture more broadly, and, since it fulfilled the central demand made by radicals for more than a century, on the political left in particular. This chapter outlines the consequences of this shift for radical politics, positioning the study of Scottish politics within a wider British context.

This volume takes an interdisciplinary perspective when positioning the impact of hypermasculinity on toxic masculinity and the vilification of identities outside of hypermasculinity through several mediums and genres, including film, TV and print comic book literature. The focus lies on heroes within popular culture, and the book argues for a reconsideration of the hero in popular media through an analysis of representation of masculinity, the demonization of femininity, and LGBTQIA+ identities, providing examples of progress and new ways to conceptualise superheroism. By unpicking toxic masculinity and its effect on others, and comparing it to more positive forms of masculinity, the volume will help develop a broader understanding of how the sociological phenomenon of toxic masculinity and its participation in hegemonic masculinity presents itself in our culture, providing a platform on which calls for a more diverse presentation of creators, characters, and actors can be ideologically supported. This volume contributes to wider discourses concerning popular media’s advances and shortcomings in accommodating diversity.

This book aims to offer a comprehensive investigation of dialect writing in the North of England. It brings together a wide range of contributors, investigating everything from the cultural positioning and impact of dialect writing to the mechanics of how authors produce dialect spellings (and what this can tell us about the structure of the dialects represented). Case studies focus on dialect writing from all over the North of England and consider many types of text, including dialect poetry, translations into dialect, letters, tweets, direct speech in novels, humorous localised volumes, written reports of conversations and cartoons in local newspapers. The North of England is a compelling geographical and dialectological focus: it is coherent part of the English-speaking world with considerable dialectal diversity and there are long traditions of dialect writing in the area. Contributions in the book also focus on broad questions about dialect writing in general: What is it? Who does it? What types of dialect writing exist? How can linguists interpret it? How does it interact with speakers’ linguistic knowledge?

This chapter is divided into three main sections. The first section concerns the patient population that presents for orthopaedic surgery, specifically examining chronic diseases of the musculoskeletal system and the medications commonly used for their management, and the impact this has when these patients present for surgery. Included in this section are the surgical considerations and the anaesthetic implications of orthopaedic surgery, ranging from patient positioning to bone cement implant syndrome. The last part of this first section looks at specific orthopaedic operations, starting with the most commonly performed, hip and knee arthroplasties, and moving onto the specialist areas of spinal deformity, paediatric, and bone tumour surgery that are not usually found outside of specialist centres. The middle section gives a brief overview on analgesia concentrating on pharmacological methods as, although orthopaedic surgery lends itself well to regional anaesthesia, this is covered elsewhere in its own dedicated chapters. No section on analgesia would be complete without mentioning enhanced recovery: the coordinated, multidisciplinary approach that improves the patient experience, increases early mobilization, and reduces length of stay, which should be the standard obtained for every patient. The final section covers the anaesthetic management of in-hospital trauma, giving an overview on initial assessment, timing of surgery, and management of haemorrhage and coagulopathy. This section finishes by covering the orthopaedic-specific topics of compartment syndrome, fat embolism syndrome, and the management of fractured neck of femur and spinal injury.

The present work takes as its object of investigation the first Brazilian organizations linked to Trotskyism, in the period between 1928 and 1936. The Brazilian Trotskyist movement originated from certain historical conditions, from a rupture caused by national, particular factors, but which were directly related to divergences that shook the Russian Revolution and the international communist movement. The Lenine Communist Group (GCL), founded in May of 1930, was the first Brazilian organization to establish contact with the International Left Opposition. The group had a short life, but gave rise to another organization with the same Trotskyist ties. In January 1931, the Communist League (LC) emerged, which in 1933 adopted the name of the Communist Internationalist League (LCI) and operated until the end of 1936. As for methodological procedures, this research sought to dialogue with historiography, but to advance in the investigation a wide search was carried out in all available documentation. From the consultation and analysis of primary sources, we sought to reconstruct the discourse and political positioning of Trotskyist organizations. Despite the fact that the groups and parties that claimed Trotskyism were in general minority, made up of a few dozen militants and sympathizers, it is necessary, even today, due to their impact on militant culture, the depth and penetration of their criticism and theoretical formulation, investigate and discuss the performance of these organizations. In short, it is a question of recovering a debate that is still very current and necessary

A socio-political and cultural history of the Armenians in Cold War Lebanon, this book argues that Armenians around the world – in the face of the Genocide, and despite the absence of an independent nation-state after World War I – developed dynamic socio-political, cultural, ideological and ecclesiastical centres. And it focuses on one such centre, Beirut, in the postcolonial 1940s and 1950s. Tsolin Nalbantian explores Armenians’ discursive re-positioning within the newly independent Lebanese nation-state; the political-cultural impact (in Lebanon as well as Syria) of the 1946–8 repatriation initiative to Soviet Armenia; the 1956 Catholicos election; and the 1957 Lebanese elections and 1958 mini-civil war. What emerges is a post-Genocide Armenian history of – principally – power, renewal and presence, rather than one of loss and absence. Armenians Beyond Diaspora: Making Lebanon Their Own investigates Lebanese Armenians’ changing views of their place in the making of the Lebanese state and its wider Arab environment, and in relation to the Armenian Socialist Soviet Republic. It challenges the dominant Armenian historiography, which treats Lebanese Armenians as a subsidiary of an Armenian global diaspora, and contributes to an understanding of the development of class and sectarian cleavages that led to the breakdown of civil society in Lebanon from 1975. In highlighting the role of societal actors in the US–Soviet Cold War in the Middle East, it also questions the tendency to read Middle East history through the lens of dominant (Arab) nationalisms.

The typical story about creationist social movements centers on battles in the classroom or in the courtroom—like the Scopes Trial in 1925. But there is a new setting: a museum. “Prepare to Believe” is the slogan that greets visitors throughout the Creation Museum located in Petersburg, Kentucky. It carries the message that the organization Answers in Genesis (AiG) uses to welcome fellow believers as well as skeptics since opening in 2007. The Creation Museum seeks to persuade visitors that if one views both the Bible (a close, literal reading) and nature (observational, real world data) as sources of authority, then the earth appears to be much younger than conventionally understood in mainstream society. This book argues that the impact of the Creation Museum does not depend on the accuracy or credibility of its scientific claims, as many scholars, media critics, and political pundits would suggest. Instead, what AiG goes after by creating a physical site like the Creation Museum is the ability to foster plausibility politics—broadening what the audience perceives as possible and amplifying the stakes as the ideas reach more people. Destabilizing the belief that only one type of secular institution may make claims about the age of the earth and human origins, the Creation Museum is a threat to this singular positioning. In doing so, AiG repositions itself to produce longstanding effects on the public’s perception of who may make scientific claims. Creating the Creation Museum is a story about how a group endures.

The presence of breast implants limits the amount of tissue that can be visualized on mammography and tomosynthesis. The proper mammographic positioning of the breasts of women with implants requires special training. More tissue can be visualized in women with sub-pectoral implants than in women with sub-glandular implants. Women with implants are recommended to have age-appropriate routine interval screening mammography for detection of cancer. If an implant rupture is suspected, ultrasound and MRI are used as adjunct imaging modalities. This chapter, appearing in the section on breast implants, reviews the key imaging and clinical features, imaging protocols and pitfalls, and management recommendations for breast implants. Topics discussed include types of implants, imaging findings of intact implants, and signs of ruptured implants, using mammography, ultrasound, and magnetic resonance imaging.

This chapter examines how young people, disproportionately black and Hispanic, in America’s economically disadvantaged, urban contexts are using the third decade of life to heal and succeed. Guided by life course, ecological, and trauma-informed frameworks, we present a multidisciplinary review of the literature describing post-traumatic growth, resilience, and healing with a focus on trauma-informed research and practice positioning youth impacted by inner-city violence to recover and flourish during emerging adulthood. In order to best appreciate the strivings of young people to heal in contexts of chronic risk, we situate this discussion in the nature, root causes, and consequences of violence (both structural and interpersonal) in urban America. We conclude with suggestions for future research to advance our understandings of how emerging adults in the inner city are working to heal from violent exposure and the implications of this task for the transition to adulthood.
