Ce volume porte l’attention sur l’image, en tant que représentation du réel mais aussi en tant que lieu à partir duquel l’imagination peut s’exercer en évoquant des représentations mentales (l’imaginaire). Les trois parties proposées (I. Les images du voyage; II. L’art dans le guide; III. Nuclear cardiac imaging is solidly based on many branches of science and engineering, including nuclear, optical and mathematical physics, electrical and mechanical engineering, chemistry and biology. This chapter uses principles from these scientific fields to provide an understanding of both the signals used, and the imaging system that captures these signals. Nuclear cardiology’s signals are the x-rays or ?-rays photons emitted from a radioactive tracer and its imaging systems are either single-photon emission computed tomography (SPECT) or positron emission tomography (PET) cameras. This combination has met with remarkable success in clinical cardiology. This success is due to the combination of sophisticated electronic nuclear instruments with a highly specific and thus powerful signal. The signal is as important as or more important than the imaging system. There is a misconception that cardiac magnetic resonance (CMR) cardiac computed tomography (CCT) and echocardiography are superior to nuclear cardiology imaging because of their superior spatial resolution. Yet, in detecting perfusion defects what is really necessary is superior contrast resolution. It is this superior contrast resolution that allows us to differentiate between normal and hypoperfused myocardium facilitating the visual analysis of nuclear cardiology perfusion images. Because these objects are bright compared to the background radioactivity, computer algorithms have been developed that allow us to automatically and objectively process and quantify our images. This chapter explains many of the important scientific principles necessary to understand nuclear cardiology imaging in general, i.e., how these sophisticated imaging systems detect the radiation emitted from the radiotracers.

Several tools are available to map brain electrical activity. Clinical applications focus on epileptic activity, although electric source imaging (ESI) and electroencephalography-coupled functional magnetic resonance imaging (EEG–fMRI) are also used to investigate non-epileptic processes in healthy subjects. While positron-emission tomography (PET) reflects glucose metabolism, strongly linked with synaptic activity, and single-photon-emission computed tomography (SPECT) reflects blood flow, fMRI (BOLD) signals have a hemodynamic component that is a surrogate signal of neuronal (synaptic) activity. The exact interpretation of BOLD signals is not completely understood; even in unifocal epilepsy, more than one region of positive or negative BOLD is often observed. Co-registration of medical images is essential to answer clinical questions, particularly for presurgical epilepsy evaluations. Multimodal imaging can yield information about epileptic foci and underlying networks. Co-registering MRI, PET, SPECT, fMRI, and ESI (or magnetic source imaging) provides information to estimate the epileptogenic zone and can help optimize surgical results.

Cardiac PET enables evaluation of multiple aspects of myocardial perfusion, metabolism, cell signaling and function that are of value both for diagnosis and prognostication in patients with known or suspected CAD and its use has increased in the past three decades. PET myocardial perfusion imaging (MPI) offers several technological advantages over SPECT including; higher photon energy, higher count sensitivity, more consistent attenuation correction and the ability to measure myocardial blood flow in absolute terms. These result in faster imaging times, lower patient radiation exposure and increased diagnostic accuracy. However the availability of PET MPI remains limited, predominantly due to expense. Efforts are underway to expand the use of PET MPI beyond larger centers, with lower-cost scanners and more widely available radiotracers. In this chapter we describe the latest advances in PET camera technology and image reconstruction as well as potential image artifacts specific to PET MPI. We go on to discuss diagnostic accuracy and prognostic value of PET MPI as well as its role in clinical practice.

Image-based measurements of myocardial blood flow afford the assessment of coronary circulatory function. They reflect functional consequences of coronary stenoses, diffuse epicardial vessel disease and microvascular dysfunction and structural changes and thus provide a measure of the total ischemic burden. Measured flows contain therefore clinically important predictive information. Fundamental to flow measurements are the tissue tracer kinetics, their description through tracer kinetic models, high spatial and temporal resolution imaging devices and accurate extraction of radiotracer tissue concentrations from dynamically acquired images for estimating true flows from the tissue time activity curves. A large body of literature on measurements of myocardial blood flow exists for defining in humans normal values for flow at baseline and during hyperemic stress as well as for the myocardial flow reserve. The role of PET for flow measurements has been well established; initial results with modern SPECT devices are encouraging. Responses of myocardial blood flow to specific challenges like pharmacologic vasodilation and to sympathetic stimulation can uncover functional consequences of focal epicardial coronary stenoses, of conduit vessel disturbances and disease and impairments of microvascular function. Apart from risk stratification, flow measurements may allow detection of early preclinical disease, influence treatment strategies and identify therapy responses.

As in single photon emission computed tomography (SPECT), positron emission tomography (PET) involves the injection of a radiopharmaceutical, the physiological properties of which determine its distribution within the patient. The labelling radionuclide then allows this distribution to be imaged. The value of cardiac PET as a routine clinical tool, particularly for perfusion imaging, was previously limited by the expense and scarcity of cameras and the short half-lives of the radionuclides with complex radiochemistry. The need for an on-site cyclotron to produce these radiopharmaceuticals made a clinical service non-viable. A number of recent developments, however, have led to renewed interest in cardiac PET. This chapter covers PET instrumentation, detail on the radiopharmaceuticals used in cardiac PET, and a number of sections on F-fluorodeoxyglucose (F-FDG) PET covering infection and inflammation imaging.

Between the third and first centuries BCE, a tomb near the Via Appia not only served as a funerary monument for the Scipiones but was also believed to have once contained the statue of a man from outside the family: Quintus Ennius. This chapter considers how Ennius’ poetry and portrait contributed to the circulation of political prestige. Linking the story of his statue to a later image of the poet in Varro’s De poetis, it argues that Varro’s collection of author portraits and the practice of erecting busts of authors in libraries are best seen as a form of entombment—situating the poet’s imago alongside those of his literary forebears in a space that recognizes their identity as a group, much like the tomb of the Scipiones, or, indeed, any Roman atrium that collects the imagines of a family’s ancestors.

This chapter reviews appropriate indications for advanced imaging, such as PET/CT, in the evaluation of breast cancer. An informed imaging approach is based on primary tumor size and pathological characteristics, as well as patient symptoms that may indicate a higher likelihood of metastatic disease. When evaluation for metastatic disease is indicated, survey imaging with CT, bone scintigraphy, abdominal MRI, brain MRI, and/or PET/CT can be used to establish disease burden, and to identify a biopsy target for pathological confirmation. We emphasize the evolving role of FDG PET/CT in this chapter, including basic principles of PET imaging, followed by a short section on image interpretation. Finally, the concept of using imaging as a response biomarker is introduced.
