Texte intégralThis thesis focuses on the transformations in the experience of the employment relationship for male and female workers in the salmon industry on Chiloé Island, in Chilean Patagonia. Since the 1980s, over ten national and international salmon farming companies have established in this region, contributing to Chile becoming the second-largest global salmon producer after Norway. In just a few years, this industry has brought about profound changes not only economically but also socially and culturally in this primarily rural area where a family-based subsistence economy was predominant. The establishment of this industry has notably led to a local revolution characterized by the fast development of a mass workforce organized around a capitalist mode of production focused on exports. Despite the prosperous industrial context and the contractual stabilization of the labor force, work is precarious and develops in an atmosphere of various uncertainties. Following a six-month ethnographic immersion and conducting over thirty interviews with individuals involved in the salmon industry (workers, employers, unions), the thesis explores the lived experiences of the workers' wage relationship in salmon processing plants. It proposes to examine this employment relationship as a process. Based on a multi-year research with successive field visits, this work distinguishes four phases of the employment relationship. These phases help understand variations in the relationship to work in terms of protection and recognition, as well as their connection to stable but threatened employment: the enchanted employment; the "real" employment (as opposed to prescribed work); the adapted employment; and the disqualified employment. The workers' trajectory begins with enchantment arising from the possibility of a general improvement in the economic and social condition of the individual and their family. However, the trade-off for these benefits is the commitment to socialization processes under Taylorian conditions of assembly line work and the learning of discipline and subordination. Seeking to endure work over time, workers use adaptation and resistance means to bypass tensions and secure their employment. However, over time, morally and physically worn out by the arduous working conditions and the humiliation from the management, workers wish to detach themselves from this disqualified employment condition characterized by the accumulation of forms of vulnerability and oppression at work. The thesis demonstrates that the collective dimension of the lived experiences of these workers not only provides the possibility to define a socio-historical sequence of labor force transformation for the processing factories but also defines an ephemeral working-class condition. Using tools from labor sociology, the thesis proposes an employment sociology that incorporates lifestyles outside of work. Thus, the results of this research contribute to enriching the discussion on contemporary challenges faced by workers in extractive and agri-food industries, while emphasizing the importance of considering local specificities in the analysis of labor transformations

Texte intégralBackground: Chronic illness is a long-term condition that generally has a slow development. When a child is diagnosed with a chronic illness parents experience that their lived world is changing. Parents of the chronically ill child need to make extensive efforts to adapt to the new circumstances. Aim: The aim of the study was to describe experiences of being a parent of a child with a chronic illness. Method: The aim was answered with a literature review of ten scientific qualitative articles. Result: When the child was diagnosed with a chronic illness the parents experienced emotional shock. To manage the illness was by the parents experienced as difficult in the beginning, but over time it eased. The everyday life was not the same and the relationships within the family changed. The parents learned to reevaluate their lives as individuals and as a family, which resulted in a new relation to the valuable in human life. The parents experienced that the support from other parents with similar experiences had a positive effect on their prosperity. The parents needed to take control over their new life situation and accomplished that throughout different strategies of coping. Conclusion: The results in this literature review have shown clear similarities in parents’ experiences regardless of the individual chronic illness that affects the child. To manage the emotional burden of the child’s chronic illness parents reported that the primary support came from other parents with similar experiences. The nurse should introduce the parent to different support groups or patients associations to reduce their anxiety and inspire hope of the child’s future wellbeing. Clinical significance: This literature review could contribute knowledge about parents experiences of living with a child with a chronic illness. An awareness of the importance of support and how support affects families with a child with a chronic illness can be seen as a tool to create good caring which is the nurse’e area of competence.

Texte intégralBackground: Mental ill health in children has increased steadily over the past 20 years. Good mental health is a prerequisite for children to develop optimally. Globally, about 10-20% of the world's children suffer from mental ill health. There is limited research on what factors can contribute to this increase. Purpose: To identify and describe risk factors that can contribute to mental ill health in children. Method: A descriptive literature study of 13 scientific articles, 12 articles with quantitative approach and 1 article with qualitative approach. Main results: The result shows that children who experience mental ill health are affected by various risk factors. Identified risk factors were the structure of the family, the health of the parents, and the parents' education and socio-economic status. Exposure was another significant risk factor. The result shows that these factors have a negative impact on the child's mental health Conclusion: Children living in an unfavorable home environment with family conflicts or parents with mental ill health are at risk of developing mental illness themselves. Exposure like bullying and various types of abuse leads to a weak self-image and a significant risk of mental ill health.

Texte intégralENGLISH ABSTRACT: The focus of the present review is the quality of life (QOL) of parents caring for a child with epilepsy. The review is informed by published books and articles available on the Psychlit and Medline databases. The paper provides an overview of epilepsy and the problems patients, particularly children, are confronted with. The important roles that parents fulfil in their child's life is followed by a discussion of the stress and burden experienced by parents caring for a child with epilepsy. Descriptions and definitions of the construct QOL are followed by reported research findings on the QOL of patients with epilepsy. The importance of the QOL of parents caring for a child with epilepsy is emphasized and this leads to an examination of existing research on the QOL of these parents. Research on the QOL of parents of children with epilepsy is limited despite the important roles parents fulfil in the life of their child with epilepsy and the potentially negative consequences of these additional roles for the child, the parents and the remainder of the family. Identification and an understanding of the dimensions of QOL of parents that are impacted upon by a child's epilepsy may produce improved treatment outcomes and QOL for children diagnosed and living with epilepsy. Recommendations for future research are included in the present review.
AFRIKAANSE OPSOMMING: Die fokus van hierdie oorsig is die lewenskwaliteit van ouers wat 'n epileptiese kind versorg. Die oorsig is gebaseer op gepubliseerde boeke en artikels wat beskikbaar is op Psychlit en Medline databasisse. Die oorsig voorsien 'n omskrywing van epilepsie asook van die probleme wat pasiente, veral kinders, ondervind. Die belangrike rolle wat ouers in hul kinders se lewe speel word bespreek en dit word gevolg deur 'n bespreking van die stres en druk wat ouers wat epileptiese kinders versorg, ondervind. Beskrywings en definisies van die konstruk lewenskwaliteit word aangebied, gevolg deur 'n opgawe van navorsingsbevindinge oor die lewenskwaliteit van epiletiese pasiënte. Die belangrikheid van die lewenskwaliteit van ouers van 'n epileptiese kind word beklemtoon en dit lei tot 'n oorsig van huidige navorsing oor die lewenskwaliteit van hierdie ouers. Ten spyte van die belangrike rolle wat ouers in die lewe van hul epileptiese kind speel en die moontlike negatiewe gevolge van hierdie bykomende rolle vir die kind, die ouers en die ander familielede, is navorsing oor die lewenskwaliteit van ouers met epileptiese kinders beperk. Identifisering van en insig in die faktore wat 'n impak het op die lewenskwaliteit van ouers met 'n epileptiese kind, kan lei tot verbeterde behandelingresultate en hoër lewenskwaliteit vir kinders wat gediagnoseer word en wat met epilepsie saamleef. Aanbevelings vir toekomstige navorsing word ook in hierdie oorsig gemaak.

Texte intégralCow\'s milk proteins are the main allergens related to food allergy in children and the diagnosis include the double blind placebo controlled food challenge (DBPCFC). Although this test presents accuracy, it involves risks, it is necessary special conditions and it presents limitations in infants and anaphylactic patients. It is necessary other diagnostic methods, among them the cut off values for IgE specific for milk to allow the cow\'s milk allergy (CMA) diagnosis without the need to perform the DBPCFC. The objective of this study was to establish a discriminating concentration of specific IgE to cow\'s milk and its proteins fractions for the diagnosis of allergy to CMA and to assess if there are different discriminating concentrations of specific IgE to this allergen in the group of patients with anaphylaxis. It was carried out a historical cohort study including patients with CMA and the control group was composed by patients with excluded CMA suspicion. To obtain the cutoff points for diagnosis of CMA , ROC curves were constructed for the following allergens: cow\'s milk, casein, a-lactalbumin and a b-lactoglobulin. The levels of specific IgE were later evaluated, separately, in patients with anaphylaxis. The study included 123 patients (1.3 M: 1F median = 1.91 years, ranging from 3.5 months to 13.21 years) with confirmed diagnosis of CMPA through DBPCFC (n = 26), the presence of anaphylaxis triggered by cow\'s milk protein (n = 46) or strongly suggestive clinical history associated with positive specific IgE through skin test (n = 51). Among the 65 patients with anaphylaxis, 19 confirmed the diagnosis through challenge tests . The control group was consisted of 61 patients (1M: 1.1 F) with ages ranging between 0.66 and 16.7 years (mean 6.86, median = 6.83 years). The methodology to stablish the cutoff was the construction of a ROC curve and subsequent calculation of positive and negative predictive value. The cutoff points obtained considering a 98% specificity and positive predictive value above 95% were: 3.06 kU / L for cow\'s milk, 2.06 kU/ L for/ a-lactalbumin, 1.85 kU/ L for/ b-lactoglobulin and 1.47 kU / L for casein. The value of specific IgE to cow\'s milk showed better discriminant capacity than those found for the fractions, being these last values dispensable. In relation to anaphylaxis, the ROC curve constructed from a comparative analysis between patients with and without this manifestation indicated a very high cutoff of 39kU/L, being without value for the clinical practice. This study showed that the cut off point detected for hole cows milk was enough for CMA diagnosis, without necessity of the milk fractions evaluation. In the presence of cows milk anaphylaxis, it is not necessary the IgE specific cut off value, being the clinical data and the sensitization to cows milk enough for the CMA diagnosis.

Texte intégralChildren’s mental ill-health is a growing public health problem in Sweden and for the municipality, being an important financial actor during a child’s upbringing, it is crucial to put resources where they give positive effect on the problem. The overall aim of the study is to describe how investing in prevention programs at children’s schools can improve children’s mental health and reduce the societal costs. Municipality statistics show that children in need of special support in school because of depression/anxiety, ADHD and psychosocial problems receive actions such as personal assistant, teacher or placed in a special education group. The cost of these actions varies between 4424-26000 Swedish krona [SEK] per child and month. These costs can be put in relation to preventive universal school interventions that have the highest cost of 1097 SEK per child and have a proven effect on child mental health. The analysis shows that preventing child mental ill-health can save societal costs and provide a healthier life for children compared to the current standard practice of targeting the children’s problems only after they have occurred.

Texte intégralThe purpose of this study has been to investigate and provide an in-depth understanding of how counselors in psychiatry experience their work in supporting children as relatives of parents with mental illness/ill health. Based on a combined sample consisting of goal-oriented selection and convenience selection, five respondents were recruited from two locations in Sweden. A qualitative method has been used consisting of semi-structured interviews, and a thematic analysis has been carried out. The results show that counselors work in several different ways to provide support to children to a parent with mental ill-health/mental illness. Obstacles regarding providing support to children is a predominant focus on adults, fear of breaking an alliance with the adult client, and lack of resources. The results also show that the councelors’ view of opportunities is about preventive work. The conclusion from this study is that providing support to children is complex, and that development of work practices may be needed to come to terms with obstacles and create increased opportunities to support children to parents with mental ill health/mental illness.

Texte intégralINTRODUCTION: Given the enhanced prognosis of patients with juvenile systemic lupus erythematosus, new recommendations are necessary in pediatric rheumatology, such as the analysis of oral health and the masticatory system. The aim of this study was to compare dental and facial conditions of patients with juvenile systemic lupus erythematosus and a control group, and to evaluate a potential relationship between dental and facial endangering and clinical manifestations, laboratory tests and therapies for the disease. PATIENTS AND METHODS: A total of 48 children and adolescents with juvenile systemic lupus erythematosus attending the Pediatric Rheumatology Unit of the Children\'s Institute of our University Hospital were studied between January 2004 and July 2005. The control group included 48 healthy children and adolescents that were selected from the Odontology Division of our University Hospital. The search included the analysis of social and demographic data, clinical manifestations, laboratory tests, juvenile systemic lupus erythematosus disease activity and cumulative damage (using the SLEDAI and the SLICC/ACR-DI), and therapies. The dental and facial examination included the anamnesis questionnaire, DMFT index, plaque and gengival bleeding index, dental relationship, facial profile, Helkimo\'s index and evaluation of the temporomandibular joint through a radiographic panoramic examination of all patients and a computer tomography on those with flattening and/or destruction of the mandibular condyles. RESULTS: The two groups were homogeneous regarding age, gender and social-economic class. The age of the juvenile systemic lupus erythematosus patients ranged from 87 to 218 months (mean of 161.9±38.4) and of the control group from 78 a 254 months (mean of 154.4±45.8; p=0.384). The medians of the plaque and gingival bleeding indexes were higher in juvenile systemic lupus erythematosus patients than in the control group (61.5X38.10; p=0.003 and 26.0X15.95; p=0.014). The indexes of clinical dysfunction and mandibular mobility were higher in juvenile systemic lupus erythematosus patients versus the control group (p=0,002, p=0,025). A linear statistical correlation was evidenced between: juvenile systemic lupus erythematosus duration and the gingival bleeding index (p=0.017; r=0.11), cumulative dose of prednisone and the plaque index (p=0,010; r=0.385) and cumulative dose of prednisone and the gingival bleeding index (p=0.001, r=0.02). The median of the mandibular mobility index was higher in juvenile systemic lupus erythematosus patients that used at least one imunossupressive drugs compared with those that didn\'t use this medication (p=0). Only two patients showed a panoramic radiography with flattening and/or destruction of the mandibular condyles. The computer tomography revealed: narrowing of joint spaces bilaterally and erosions of joints plateaus with flattening of condyles that suggested avascular necrosis of temporomandibular joint. CONCLUSIONS: Juvenile systemic lupus erythematosus patients presented poor oral hygiene, higher incidence of gingivitis and temporomandibular joint dysfunction compared to the control group. Patients with longer disease duration and higher cumulative dose of prednisone had a greater incidence of gingivitis, and those who used imunossupressives drugs showed temporomandibular joint dysfunction

Texte intégralMalnutrition at pediatric Intensive care unit (PICU) admission is frequent and associated with impaired outcomes. However, most studies have focused solely on a static definition. A holistic approach would improve the description of malnutrition: this would include both a static and dynamic assessment of nutritional status, together with body composition assessment and with malnutrition classified based on its patho-physiology and etiology. This holistic assessment of malnutrition has been applied and examined in four observational studies which included critically ill children older than 36 gestational weeks (corrected age). These found that malnutrition was frequent at PICU admission (27.3%) and faltering growth prior to PICU admission was associated with an increased length of PICU stay (+3 days). Critically ill children present at admission with decreased plasma levels of 6 micro-nutrients (Selenium, Copper, Zinc, Vitamin C, E and beta-carotene) involved in anti-oxidative stress pathways. Nutritional status deterioration during PICU stay, and associated muscle mass loss occurred frequently and were intense. This early phenomenon was associated with extended length of PICU stay. A profound critical illness related metabolic shift leads to malnutrition as an adaptive process. However, malnutrition may also negatively impact on outcomes in this setting. These studies have led to a clearer understanding of the underlying patho-physiology. This, combined with a more systematic and holistic nutritional assessment, will enable implementation and assessment of nutritional strategies aiming to improve the functional outcome of critically ill children

Texte intégralI further investigate the two organizations at the forefront of this debate, being the WTO and the ILO. In an attempt to ascertain which of the two is the best forum to deal with the issue I further look at the relationship between these two organizations. Compliance with international labour standards is a growing concern as worldwide standards are deteriorating and nothing is being done to alleviate the problem. Accordingly, I explore the causes for the abuse of labour standards and seek to find the better alternative, by looking at the respective positions of the parties who are for and against the linkage of trade with labour standards. Here the views and concerns of the developed world are weighed up against those of the developing world and looking at possible alternatives concludes the paper.

Texte intégralThe child protection represents a major cause for concern of our law, so that the minor doesn’t sustain damage. This protection is firstly performed by the family. So, it may seem surprising that the child is victim of his family, as far as parents must act in the interests of the child. The notion of child victim of his family doesn’t exist as such in our law. The victim often endures an injury caused by a third person and not by a family member. The study of situations where the child may suffer an infringement of his personality rights, due to act or omission of one of the family members, allowed conceptualization of the notion of child victim of family. It was thus possible to characterize the different realities encountered, according to their similarities or dissimilarities. This empirical analysis led to ascertain categories of child victims of their families and the corresponding legal system. It had especially as a consequence new protection instruments research, appropriate to specifics assumptions studied. This process allowed social or family realities encountered getting to legal validity. It was also an opportunity to bring out foundations of the concept of child victim of family, in so far as law has hallowed situations of child victims of their families. However, emergence of new social realities necessitated the renewal of the concept of child victim of family, in order to alleviate the limits of the foundations of the notion and to guarantee effective representation of this phenomenon in legal speech
