The low overall post-operative mortality rate conceals the existence of a sub-group of high-risk patients, which accounts for over 80% of post-operative deaths. Age, co-morbid disease, limited functional capacity, and an emergency presentation for major surgery are hallmarks risk. The magnitude, duration, and consequences of post-operative morbidity are determined by a complex interplay between the indication for surgery, the resulting tissue injury, and patient factors. A number of methods including risk scoring and cardiopulmonary exercise testing can be used to identify the high-risk group. Efforts should be made throughout the peri-operative period to prevent the occurrence of any post-operative complications, as they all carry significant long-term implications.

This chapter is concerned with how atherosclerosis risk is modulated by a complex interplay between genetic and environmental risk factors. The contribution of genetics to the variability of atherosclerosis risk is estimated as 50%. Recent genome-wide association studies have led to the identification of over 50 gene variants which modulate atherogenesis. Risk factors for atherosclerosis are also partly genetically determined and some of the variants which play a role in atherogenesis overlap with those modulating its risk factors. However, the current relevance of these findings for clinical practice is limited, mainly due to the small effect sizes of identified risk variants with insufficient discriminatory power, and a large portion of the genetic contribution to atherosclerosis is still unknown. The major promise therefore lies in understanding the pathophysiology of newly identified genes with the perspective of novel therapeutic approaches.

This chapter considers the risk of mistaken identification, and the law and procedure relating to evidence of visual and voice identification. In respect of evidence of visual identification, the chapter addresses: the Turnbull guidelines, including when a judge should stop a case and the direction to be given to the jury; visual recognition, including recognition by the jury themselves from a film, photograph or other image; evidence of analysis of films, photographs or other images; pre-trial procedure, including procedure relating to recognition by a witness from viewing films, photographs, either formally or informally; and admissibility where there have been breaches of pre-trial procedure. In respect of evidence of voice identification, the chapter addresses: pre -trial procedure; voice comparison by the jury with the assistance of experts or lay listeners’; and the warning to be given to the jury (essentially an adaption of the Turnbull warning, but with particular focus on the factors which might affect the reliability of voice identification).

The prevention of acute kidney injury (AKI) should start with an assessment of the risk to develop AKI, by identification of co-morbidities, use of potentially nephrotoxic medications, and early recognition of acute reversible risk factors associated with AKI. This chapter discusses first the most relevant general risk factors for AKI and describes the recent introduction of several surveillance systems. In addition, some specific risk factors play a role in the pathogenesis of post-cardiac surgery AKI. Finally risks associated with commonly used drugs such as non-steroidal anti-inflammatory drugs, angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, statins, and warfarin are considered.

Risk factors in young-onset stroke include both traditional and unconventional as well as both chronic and temporal ones. In young patients, unconventional risk factors such as oral contraceptive use, antiphospholipid antibodies, genetic thrombophilia, acute infections, illicit drug use, and migraine may play a greater role than in elderly patients. However, recent large studies have challenged this traditional view suggesting that young adult stroke would occur mostly due to such unconventional risk factors. These studies have shown a high prevalence of in particular modifiable behavioural risk factors, including physical inactivity, high-risk alcohol consumption, and smoking. Since consequences of a young-onset stroke are greater regarding quality-adjusted life and economic impact, detection and treatment of especially modifiable risk factors are of paramount importance. This chapter reviews the burden of both traditional and less well-documented risk factors in young ischaemic stroke patients and discusses their strength of evidence and mechanisms of association.

Multiple sclerosis is a chronic inflammatory demyelinating disease of the central nervous system. Despite decades of research, its etiology remains unknown. It has been established that environmental and genetic factors contribute to multiple sclerosis disease risk. Although the identification of risk factors for multiple sclerosis has not yet radically advanced understanding of the pathophysiology, it has shed light on the complex epidemiology of this disease. This chapter highlights key epidemiologic features of multiple sclerosis, with a focus on environmental and genetic factors known to influence disease risk. The interplay between environmental and genetic factors in multiple sclerosis pathogenesis is discussed.

cBrain tumors constitute an array of histologic types, the most common being meningioma and glioma. Unlike other cancers, both benign and malignant brain tumors are concerning for survival because of their anatomic location. Two-thirds of brain tumors are benign. The most well established risk factor is high dose ionizing radiation, based on studies of atomic bomb survivors as well as children treated for tinea capitis. In contrast, nonionizing radiation including from cellular telephones, is not a risk factor. Tobacco use does not appear to be associated with glioma or meningioma. There is fairly consistent evidence of an inverse association between allergies and asthma with risk of glioma, potentially through levels of IgE. Finally, occupational epidemiology studies suggest potential positive associations with specific exposures. The identification of modifiable risk factors for brain tumors has been challenging, due in part to the diversity of tumor subtypes.
