Résumé :
To the Editor.— I have two points to raise concerning the article by Chasnoff et al (Pediatrics 1987;80:836-838). First, the history of alcohol intake given was consistent with dependency. As an addictionologist working with teenage alcoholics and addicts, I can give you a pretty good assurrance that the alcohol intake related by this mother was a gross underestimation. The physical features of this child lead one to believe that the child did suffer from prenatal maternal abuse and should have been further discussed.

In Reply.— I would like to thank Dr Cersonsky for his comments regarding our article (Pediatrics 1987;80:836-838). I certainly agree that the social aspects of a case regarding maternal substance abuse in pregnancy are important in the ultimate prognosis for the exposed child. In our experience with addicted pregnant women, our major work has been pointed toward the importance of early intervention in pregnancy. Unfortunately, the woman reported in this article was not referred to our program, and her physicians never obtained any type of substance abuse history during the pregnancy.

Résumé :
Abstract ACTM-838 is an avirulent bacterial immunotherapy that encodes an engineered IL-15 (IL-15plex) and a constitutively active STING variant (eSTING). The ACTM-838 strain is a highly modified, attenuated S. Typhimurium that lacks several major inflammatory and immunogenic components on the surface of the microbe and is designed to naturally and specifically enrich in the tumor microenvironment (TME) via auxotrophic dependency on metabolites of the adenosine pathway and purines, to achieve tumor-specific payload delivery. ACTM-838 was selectively internalized by phagocytic, primary human antigen presenting cells (APCs) but not endothelial cells. ACTM-838 was stable in human whole blood and is not inactivated by complement. Human macrophage uptake of labeled ACTM-838 led to acidic lysosomes and nuclear delivery with a concomitant increase in expression of encoded IL-15plex and eSTING payloads such as IL-15, IRF3 reporter activity and IFN ⍺/β secretion, respectively. Primary human and murine macrophages polarized towards a dual co-stimulatory and phagocytic M1/M2 dual phenotype in response to ACTM-838. ACTM-838 delivered eSTING payload in STING knockout macrophages and induced significantly higher Type I IFN production over a negative control strain. IV delivered ACTM-838 showed dose and payload dependent tumor enrichment and potent single-agent efficacy in two independent syngeneic tumor models, EMT6 (orthotopic) and MC38. Sustained memory anti-tumor immunity upon tumor rechallenge in cured animals was demonstrated. ACTM-838 treated tumors exhibited a significant increase in human IL-15plex protein payload. ACTM-838 as a single agent induced profound immune reprogramming and remodeling of the TME over time through increased infiltration and activation of myeloid, NK, B and cytolytic CD8 T cells, MHC class I and II, antigen presentation as well as decreased Treg, PD1+Lag3+ exhausted T cells, and a remodeled stromal signature, as assessed by bulk RNAseq, and confirmed by flow cytometry. Synergistic anti-tumor activity was observed in multiple tumor models when ACTM-838 was dosed in combination with anti-PD1, safely inducing complete responses. No significant body weight loss or death was observed in any treatment regimens of ACTM-838 alone (single or multiple doses) or in combination with anti-PD1. Analyses of public RNAseq datasets from patients with primary or metastatic disease revealed cancer types with high levels of myeloid infiltration and adenosine enrichment, suggesting that ACTM-838 efficacy could provide benefit in these indications. ACTM-838 is a novel immunotherapy delivering IL-15plex + eSTING payloads to phagocytic APCs, inducing a durable anti-tumor immune response, after IV dosing. It possesses a compelling safety profile, and is currently in IND-enabling, preclinical development. Citation Format: Ping Fang, Kyle R. Cron, Oanh Pham, Julie R. Résumé :
Abstract Background IMU-838 (vidofludimus calcium) is a well-tolerated and orally available small molecule inhibitor of dihydroorotate dehydrogenase (DHODH) and is currently in phase 2 clinical development for ulcerative colitis (UC). Here, we investigated the effect of IMU-838 on different immune cell subsets and analysed the mechanism of action in more detail in T cells. Methods The effect of IMU-838 on regulatory macrophages (Mregs) was investigated by adding IMU-838 or anti-TNFα antibodies (infliximab, adalimumab), alone or in combination, in a mixed lymphocyte reaction assay, determining Mregs (CD206+CD14+) by FACS analysis. To investigate the effect of IMU-838 on human T and B cells, PBMC were stimulated with PHA (48h) or Oligonucleotide 2006-PTO (70h), respectively. An affinity-dependent effect of IMU-838 was assessed on CD8 T cells from OT-I and OT-III mice, containing a high and a low affinity T cell receptor (TCR), respectively, for OVA257-264 peptide. These cells were stimulated with ovalbumin (OVA) peptide loaded splenocytes or anti-CD3/anti-CD28 for 3 days. Oxidative phosphorylation (OXPHOS) and glycolysis in murine T cells was investigated by the Seahorse/Agilent technique in murine CD4 and CD8 T cells activated with anti-CD3/anti-CD28 with or without IMU-838. Results IMU-838 slightly induced Mregs, but strongly and dose-dependently decreased TNFα and IL-6 secretion. Anti-TNFα antibodies alone strongly induced Mregs, but also increased IL-6 levels. When IMU-838 was added, Mregs induction was even higher, and IL-6 was reduced. Besides reducing proinflammatory cytokines secreted by macrophages, IMU-838 also inhibits proliferation of activated B and T cells, as well as inflammatory T cell cytokine secretion, IL-17A, IL-17F and IFNγ, and increased apoptosis up to 3-fold compared to vehicle control. By investigating IMU-838’s role on affinity related activity status of T cells, it was shown that IMU-838 strongly inhibited cell proliferation in the peptide stimulated high affinity, but not in the low affinity, CD8 T cells. Peptide stimulated high affinity T cells show an upregulation of OXPHOS and glycolysis pathways, which are involved in the generation of the cell’s energy supply, compared to low affinity cells. IMU-838 strongly inhibited OXPHOS and glycolysis in both CD4 and CD8 T cells. Conclusion IMU-838 reduces the proinflammatory immune cell response by inducing Mregs, reducing pro-inflammatory cytokine secretion and reducing immune cell proliferation. DHODH is only important in cells that received a strong stimulus and are highly metabolically active. Therefore, IMU-838 will only specifically target these cells and will still allow for a normal immune response, representing a huge safety advantage for treatment of IBD patients.

Résumé :
BackgroundIn a metastatic setting, systemically-administered therapies that overcome the immunosuppressive tumor microenvironment to promote T-cell recruitment and T-cell cytolytic function will be required to elicit durable anti-tumor immunity. To accomplish this, the STACT (S. Typhimurium-Attenuated Cancer Therapy) platform was developed. STACT is a live bacterial product that has been highly modified using precision genome editing for the following properties: (1) enhanced tolerability after IV dosing, (2) tumor-specific enrichment, (3) phagocytosis by tumor-resident antigen-presenting cells (APCs) with a lack of epithelial cell infectivity, (4) multiplexed genetic cargo delivery, and (5) attenuation of bacterial pathways that impair CD8+ T-cell function. An extensive screening campaign was performed to identify ideal encoded immunomodulatory payload combinations delivered by STACT for efficacy against T-cell excluded tumors.MethodsChromosomal edits to the STACT platform strain were made using PCR. A panel of immunomodulatory proteins, including cytokines, type I interferon (IFN)-inducing factors, co-stimulatory receptors, checkpoint antibodies and TGFβR-Fc decoys were tested for combinatorial potency using STACT. An engineered STING (eSTING) was designed through an extensive protein engineering campaign to identify optimal variants. Combinations were evaluated in primary human APCs using in vitro functional assays, where STACT IL-15Rα-IL-15 (IL-15) + eSTING (ACTM-838) emerged as a lead candidate. ACTM-838 was then evaluated in multiple murine tumor models for therapeutic efficacy and mechanism, as well as tolerability in rodents and primates after systemic administration.ResultsCombinatorial target profiling led to the discovery of ACTM-838, a STACT encoding IL-15 + eSTING. In vitro, ACTM-838 payloads synergistically produced high levels of type I IFN and T-cell recruitment and activation factors from primary human APCs. In vivo, ACTM-838 demonstrated a high degree of complete tumor responses that were entirely CD8+ T-cell dependent. In an autochthonous breast cancer model that lacks any significant lymphocyte infiltrate, ACTM-838 was able to uniformly enrich in each spontaneous lesion to high levels after IV dosing and resulted in significant CD8+ T-cell infiltration. In primates, ACTM-838 was well-tolerated, rapidly cleared, and elicited minimal cytokine response after IV dosing.ConclusionsACTM-838 is a highly attenuated, precision genome-engineered bacterial immunotherapy that delivers IL-15 + eSTING to phagocytic APCs of the solid tumor microenvironment after systemic administration. In preclinical studies, ACTM-838 promotes CD8+ T-cell mediated tumor clearance in T-cell excluded tumors and elicits durable anti-tumor immunity, and is well tolerated in primates. Based on these data, ACTM-838 was nominated for clinical development and has entered cGMP manufacturing and IND-enabling studies.Ethics ApprovalAll animals were used according to protocols approved by an Institutional Animal Care and Use Committee and maintained in specific pathogen-free conditions in a barrier facility.

Résumé :
Abstract ACTM-838 is a genetically modified bacterial vehicle encoding a DNA plasmid with two engineered payloads, IL-15plex and constitutively active eSTING. The bacterial vehicle is a highly attenuated S. typhimurium, engineered to facilitate safe IV administration. ACTM-838 is designed to specifically enrich in the TME via auxotrophic dependency on extracellular adenosine and purine metabolites. ACTM-838 is then selectively internalized by tumor-resident myeloid cells achieving tumor-specific payload delivery. Previously, we showed ACTM-838 exhibited significant and durable anti-tumor efficacy across syngeneic tumor mouse models (EMT6 breast, MC38 colon) and MMTV-PyMT GEMM. To elucidate TME changes induced by ACTM-838, EMT6 tumors were analyzed post-treatment by scRNA-seq, which demonstrated that ACTM-838 induced significant transcriptional changes in several cell populations in the TME. Cytolytic pathways were upregulated in T and NK cells, while type I IFN and PDL1 were upregulated in myeloid populations consistent with previous bulk RNAseq analyses. To understand the tolerability and safety of ACTM-838, a GLP toxicology study was performed on naïve cynomolgus monkeys (NHP). No ACTM-838-related clinical or veterinary observations were reported at any dose level. Acute dose-dependent increases in serum cytokines were observed at 4 hours which reverted to baseline by 24 hours. ACTM-838 was cleared from blood by 24 hours post-dose with no significant shedding in urine or feces. ACTM-838 was only detected in the spleen in 2 and 1 animals at 22- and 49-days post treatment, respectively. Pre-existing ADA was observed in some animals with subsequent increases in ADA upon dosing in most animals. However, no correlation of serum ADA and blood PK levels suggested that the presence of ADA does not impact drug stability. The ACTM-838 mechanism of action requires high levels of adenosine or purine metabolites to enrich and colonize the TME as well as phagocytic internalization by tumor-intrinsic myeloid cells to achieve tumor-specific payload delivery. Myeloid cells comprise roughly half of all cells in the TME in human tumors. Bioinformatics analyses of the public patient datasets identified key tumor indications enriched in both adenosine and myeloid pathways. To further understand the spatial heterogeneity in the TME across tumors, we performed multiplex immunofluorescence on patient tumor microarrays to validate tumor types with a high prevalence of myeloid populations coupled with an active adenosine pathway, which will help guide future clinical study design. In summary, we demonstrate that ACTM-838 modulates the TME and is stable and safe in animal models. Additionally, our studies recommend the prioritization of certain tumor indications for treatment with ACTM-838 in the clinic. Résumé :
The ongoing pandemic spread of the severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) demands skillful strategies for novel drug development, drug repurposing and cotreatments, in particular focusing on existing candidates of host-directed antivirals (HDAs). The developmental drug IMU-838, currently being investigated in a phase 2b trial in patients suffering from autoimmune diseases, represents an inhibitor of human dihydroorotate dehydrogenase (DHODH) with a recently proven antiviral activity in vitro and in vivo. Here, we established an analysis system for assessing the antiviral potency of IMU-838 and DHODH-directed back-up drugs in cultured cell-based infection models. By the use of SARS-CoV-2-specific immunofluorescence, Western blot, in-cell ELISA, viral yield reduction and RT-qPCR methods, we demonstrated the following: (i) IMU-838 and back-ups show anti-SARS-CoV-2 activity at several levels of viral replication, i.e., protein production, double-strand RNA synthesis, and release of infectious virus; (ii) antiviral efficacy in Vero cells was demonstrated in a micromolar range (IMU-838 half-maximal effective concentration, EC50, of 7.6 ± 5.8 µM); (iii) anti-SARS-CoV-2 activity was distinct from cytotoxic effects (half-cytotoxic concentration, CC50, >100 µM); (iv) the drug in vitro potency was confirmed using several Vero lineages and human cells; (v) combination with remdesivir showed enhanced anti-SARS-CoV-2 activity; (vi) vidofludimus, the active determinant of IMU-838, exerted a broad-spectrum activity against a selection of major human pathogenic viruses. These findings strongly suggest that developmental DHODH inhibitors represent promising candidates for use as anti-SARS-CoV-2 therapeutics.

H. Dohm has amply demonstrated how the cook of Plautus, Pseud. 790ff. exhibits characteristic features of the mageiros of Greek comedy. He has also argued, however, that this scene contains substantial Plautine expansion, comparable with that which has been recognised in the cook scene of the Aulularia. I wish to suggest that Dohm is largely right but that the Plautine expansion is even more extensive than he supposes. In 790–838 Plautus is probably for the most part following his Greek model fairly closely. One can trace a logical sequence of ideas, as follows. Ballio: ‘I couldn't have found a worse cook’ (792–7). Cook: ‘Why did you hire me then?’ (798–9a). Ballio: ‘You were the only one left. And why was that?’ (799b–801a). Cook: ‘I am expensive, but nowadays people look for cheap cooks, who produce only concoctions of seasoned vegetables. That is why men are so short-lived’ (801b–25). Ballio: ‘You can make men live longer then?’ (826–8a). Cook: ‘Certainly, for 200 years’ (828b–30). The cook then proceeds to give a list of his fantastic sauces for fish and meat (834f. Neptuni/terrestris pecudes), until he is cut short by Ballio's ‘Damn your lies’ (836–8). We have here a typical comic mageiros: he is loquacious and boastful (794 multiloquom, gloriosum), claims magic powers (829f.), denigrates his rivals (810–25), reels off lists of foods, real and fictitious (814–17, 831–6), and uses grandiose language (834f. Neptuni pecudes).Within this essentially Greek section there are three short passages which look like Plautine additions. First, Dohm is surely right, following E. Fraenkel, to see 790f. as a Plautine addition.

Résumé :
Abstract Mutated KRAS and TP53 are well recognized drivers of multiple human cancers including lung cancer, the leading cause of cancer-associated deaths. Yet, directly targeting mutated KRAS or restoring wildtype p53 levels in KRAS/p53-mutated tumors still remains a therapeutic challenge. A rational alternative approach involves targeting potentiators of KRAS;p53-driven tumorigenesis, which represents one of the research ambitions pioneered by the National Cancer Institute (NCI) RAS initiative to meet this challenge. Therefore, we aimed to identify genes, that when lost, potentiate cellular transformation of a non-transformed KRAS;TP53-mutated human bronchial epithelial cell line (HBEC-KP). To address this question, a genome-wide selection experiment was performed using CRISPR/Cas9. Anchorage-independent (AI) growth, a hallmark of cancer, was selected as a phenotypic readout for one experiment. Without additional genetic perturbation, wildtype HBEC-KP cells were incapable of AI growth. Only certain mutants were capable of supporting colony formation in soft agar. These clones were isolated and the integrated small guide RNAs (sgRNAs) were identified. Genes that are targeted by the sgRNAs were then individually knocked out for validation. Through this approach, we discovered that loss of ARPC3 is sufficient for promoting AI growth in the HBEC-KP cells. This contribution to AI growth is attributed to the role that ARPC3 plays in regulating Arp2/3 activity, as inactivating the Arp2/3 complex using a small molecule inhibitor, CK666 also promoted colony growth in the HBEC-KP cells. We determined that ARPC3 loss was not sufficient to drive AI – in the absence of KRAS activation and p53 loss colony growth was not observed. This indicates the essential role that mutated KRAS and/or p53 play to support AI. To test if mutated KRAS is essential for the phenotype, the Arp2/3 complex was inactivated in KRAS-mutated HBECs, which supported AI growth. This suggests that mutated KRAS and loss of Arp2/3 activity are sufficient for driving AI growth in HBECs. However, combined loss of p53 and ARPC3 failed to drive the phenotype, implicating that mutated KRAS is required. In addition, the Arp2/3 complex is essential for maintaining lamellipodial shape via actin filament network and cell motility, which was validated in our HBEC-KP-ARPC3 knockout cells. However, this protein complex is also known to promote AI growth and development of various cancers, as opposed to acting as a suppressor for growth. Multiple mechanisms that lead to this growth phenotype are currently being explored. Clinical relevance is also being closely examined to further understand the role of the Arp2/3 complex in cancer. Résumé :
Abstract The purpose of this article was to review current literature on peri-operative pain management in hip arthroscopy. A systematic review of the literature on pain control in hip arthroscopy published January 2008 to December 2018 was performed. Inclusion criteria consisted of English language or articles with English translations, subjects undergoing hip arthroscopy with documented peri-operative pain control protocols in studies reporting Level I to IV evidence. Exclusion criteria were non-English articles, animal studies, prior systematic review or meta-analyses, studies not reporting peri-operative pain control protocols, studies documenting only pediatric (<18 years of age) patients, studies with Level V evidence and studies including less than five subjects. Statistical analysis was performed to assess pain protocols on narcotic consumption in PACU, VAS score on discharge, time to discharge from PACU and incidence of complications. Seventeen studies were included, comprising 1674 patients. Nerve blocks were administered in 50% of patients (n = 838 of 1674), of which 88% (n = 740 of 838) received a pre-operative block while 12% (n = 98 of 838) post-operative block. Sixty-eight complications were recorded: falls (54%, n = 37), peripheral neuritis (41%, n = 28), seizure (1.5%, n = 1), oxygen desaturation and nausea (1.5%, n = 1) and epidural spread resulting in urinary retention (1.5%, n = 1). No significant differences in narcotic consumption, VAS score at discharge, time until discharge or incidence of complication was found based on pain control modality utilized. No statistically significant difference in PACU narcotic utilization, VAS pain scores at discharge, time to discharge or incidence of complications was found between peri-operative pain regimens in hip arthroscopy.

Résumé :
Liturgia stanowi przestrzeń szczególnego spotkania teologii i prawa.

Objective Pregnancy-associated plasma protein-A2 (PAPP-A2) is a metalloproteinase that cleaves IGFBP-3 and IGFBP-5. Human mutations in PAPPA2 result in short stature with a low percentage of free IGF-I. Little is known about PAPP-A2 levels and the regulation of free IGF-I throughout childhood. We examined PAPP-A2 and intact IGFBP-3 levels in childhood and explored associations between PAPP-A2, free and total IGF-I, and total and intact IGFBP-3 and their relationship to the percentage of free to total IGF-I and anthropometric factors. Design Cross-sectional study at a single center. Methods PAPP-A2, free IGF-I, and intact IGFBP-3 levels were measured in childhood (3–18 years old) and an evaluation of the relationship between these proteins and anthropometric factors. Results In 838 children, PAPP-A2 consistently decreased throughout childhood. In contrast, free IGF-I increased. A pubertal peak in free IGF-I was present in females but was less evident in males. Intact and total IGFBP-3 increased throughout childhood; however, intact IGFBP-3 had a more marked rise than total IGFBP-3. Percent free IGF-I decreased with no distinct pubertal peak. PAPP-A2 levels positively correlated with the percent free IGF-I (Male, Female; r = 0.18, 0.38; P < 0.001) and negatively with intact IGFBP-3 (Male, Female; r = −0.58, −0.65; P < 0.0001). Conclusions This is the first study to describe serum PAPP-A2 and intact IGFBP-3 in children between 3 and 18 years of age. Our correlative findings suggest that PAPP-A2 is an important regulator of the percent free IGF-I which can be a marker of perturbations in the GH/IGF-I axis.

Résumé :
The hydrothermal approach has been used to fabricate a heterojunction of n-aluminum-doped ZnO nanorods/p-B-doped diamond (n-Al:ZnO NRs/p-BDD). It exhibits a significant increase in photoluminescence (PL) intensity and a blue shift of the UV emission peak when compared to the n-ZnO NRs/p-BDD heterojunction. The current voltage (I-V) characteristics exhibit excellent rectifying behavior with a high rectification ratio of 838 at 5 V. The n-Al:ZnO NRs/p-BDD heterojunction shows a minimum turn-on voltage (0.27 V) and reverse leakage current (0.077 μA). The forward current of the n-Al:ZnO NRs/p-BDD heterojunction is more than 1300 times than that of the n-ZnO NRs/p-BDD heterojunction at 5 V. The ideality factor and the barrier height of the Al-doped device were found to decrease. The electrical transport behavior and carrier injection process of the n-Al:ZnO NRs/p-BDD heterojunction were analyzed through the equilibrium energy band diagrams and semiconductor theoretical models.

Proliferating primordial germ cells (PGCs) isolated from mouse embryos soon after their arrival in the genital ridges would only survive in vitro at temperature of less than 30 degrees C (De Felici, M. and McLaren, A. (1983). Exp. Cell. Res. 144, 417–427; Wabik-Sliz, B. and McLaren, A. (1984). Exp. Cell. Res. 154, 530–536) or when co-cultured on cell feeder layers (Donovan, P. J., Stott, D., Godin, I., Heasman, J. and Wylie, C. C. (1986). Cell 44, 831–838; De Felici, M. and Dolci, S. (1991). Dev. Biol. 147, 281–284). In the present paper we report that mouse PGC death in vitro occurs with all the hallmarks of programmed cell death or apoptosis. We found that after 4–5 hours in culture many PGCs isolated from 12.5 dpc fetal gonads assumed a nuclear morphology and produced membrane bound fragments (apoptotic bodies) typical of apoptotic cells. In addition, PGCs in culture accumulated high level of tissue transglutaminase (tTGase; an enzyme that is induced and activated during apoptosis) and showed extensive degradation of DNA to oligonucleosomal fragments, which is characteristic of apoptosis. The physiological relevance of this mechanism of PGC death is supported by the finding that some PGCs undergoing apoptosis, as revealed by the high level of tTGase expression, were detected in the embryo. Most importantly, we show that the addition of stem cell factor (SCF) or leukemia inhibitory factor (LIF) to the culture medium, two cytokines known to favour PGC survival and/or proliferation in vitro, markedly reduced the occurrence of apoptosis in PGCs during the first hours in culture.(ABSTRACT TRUNCATED AT 250 WORDS)

Résumé :
e15505 Background: The IGF (Insulin like Growth Factor) family has been implicated in the pathogenesis of cervical cancer. We aim to correlate pre and post treatment levels of IGF-I and IGF-II in stage IIIB cervical cancer and identify if serum IGF-I and IGF-II can be used as a marker to predict response to chemo-radiotherapy. Methods: We recruited 80 previously untreated patients of histopathologically proven squamous cell carcinoma of stage IIIB cancer of uterine cervix from January 2011 to May 2011. The age of patients ranged from 30 to 65 years and were of ECOG performance status of less than or equal to 2. Along with the routine investigations the pretreatment serum levels of IGF-I and IGF-II were done by ELISA kits in all the patients. The serum level of IGF-I was 246.3 ± 72.5 ng/ml and IGF-II was 1585 ± 352 ng/ml. All patients were given concurrent cisplatin (30 mg/m2) weekly along with radiation (50Gy/25#) with conventional portals followed by two sittings of intracavitory radiotherapy (7.5 Gy each) on mHDR. Blood samples for IGF-I and IGF-II levels were collected before the start of the therapy, 1 month after the completion of treatment and 6 months after the completion of the treatment. Results: The trend seen was that mean IGF-I levels were always in the lower range as compared to IGF-II levels in all individuals. The mean serum levels of IGF-I and IGF-II in patients who achieve complete response (45%) returned to normal whereas the mean serum levels of IGF-I and IGF-II in patients who achieved partial response (43%) was 149 ng/ml and 838 ng/ml respectively. The mean serum levels of IGF-I and IGF-II in patients who achieved no response (12%) was 331 ng/ml and 1840 ng/ml respectively. Conclusions: The serum levels of IGF-I and IGF-II may be a reliable prognostic and predictive marker in cervical cancer however further large randomized studies are required to validate the results. IGF-I and IGF-II can serve as potential markers in cervical cancer to risk stratify patients, assess treatment response and possibly act as therapeutic targets in the future.

Résumé :
Background: Vitamin C has significant anti-inflammatory effects and is particularly important for critically ill patients. However due to inconsistent research findings in critically ill patients in meta-analysis. Therefore, the primary objective of this meta-analysis is to investigate the effects of isolated intravenous supplementation of vitamin C in adults with critical illness by comprehensively incorporating articles from randomized controlled trials. Methods: Articles included searching through PubMed, Embase, Medline, Cochrane Library, and Web of Science up to April 28, 2023, for articles on vitamin C and the critically ill. We calculated pooled standard relative risk (RR), mean difference (MD), and 95% confidence intervals (CIs). And the protocol for the review has been registered on PROSPERO (CRD42023425193). Results: There are 2047 critically ill included in 19 articles. Compared with placebo, patients who underwent intravenous vitamin C (IVVC) have reduced duration of vasopressor used (SMD 0.26; CI 0.01–0.51; I 2 = 87.0%, P = .044), mechanical ventilation (SMD −0.29; CI −0.55 to −0.03; I 2 = 36.8%, P = .031). However, the administration of IVVC had no statistical difference in 28-d mortality (RR 0.95; CI 0.80–1.11; I 2 = 12.2%, P = .337), mortality (RR 0.79; CI 0.55–1.12; I 2 = 0%, P = .188), fluid intake (SMD −0.02; CI −0.25 to 0.20; I 2 = 0%, P = .838), urine output (SMD 0.23; CI −0.03 to 0.49; I 2 = 0%, P = .084), ICU days (SMD 0.10; CI −0.03 to 0.22; I 2 = 0%, P = .127), hospital stay (SMD 0.10; CI −0.12 to 0.32; I 2 = 0%, P = .375), and pneumonia (RR 0.85; CI 0.50–1.44; I 2 = 0%, P = .552). Conclusion: This study comprehensively and systematically evaluated IVVC supplementation in the critically ill through a meta-analysis of RCT. There is no difference except for patients who had reduced duration of vasopressor use and mechanical ventilation by the administration of IVVC. Of course. More scientific and rigorous conclusions can be drawn from multi-center RCT research in the future.

The objective of the article is an approach to the controversial biography of Police Chief Manuel Brabo Portillo. The work is based on primary and secondary sources. The method used is empirical. In the imagery of the revolutionary syndicalist world, Brabo Portillo was the most hated policeman, the reincarnation of the murkiest face of the state. He was also a German spy connected with the sinking of Spanish ships, the murder of businessman and engineer Josep Barret and the first head of the terrorists linked to Barcelona employers. The conflict that affected Spain during the period of the First World War is fundamental in order to understand the origins of terrorism linked to pistolerismo, which marked Spanish social political history during the first third of the twentieth century.KEY WORDS: Brabo Portillo, pistolerismo, espionage, syndicalism, First World War. Résumé :
ABSTRACT Aim of this study was to assess the cytotoxicity of monomer. An in vitro study was designed to study the growth inhibitory effect of monomer (Stellon, Denture Material Improved, Type I, Class I, Dental Products of India Limited) on cells seeded in petri dishes and maintained in an incubator with 5% carbon dioxide at 37°C. The growth of V79 cells (fibroblast cells) maintained in a culture medium to which monomer was added was studied for a period of 5 days. Results of this study pointed out that even at a concentration of 1 μl of monomer, the cell growth was significantly inhibited, when compared to the control group. The number of viable cells decreased dramatically whereas dead cells increased in the culture groups treated with the monomer. The cytotoxic effect was dose dependent. As the concentration increased from 1 to μl there was a marked inhibition of cell growth and a corresponding increase in dead cell count. Results of this study proved beyond doubt that monomer is indeed cytotoxic even in very low concentrations. Thus, it becomes imperative to adopt every possible means to minimize residual monomer content in heat cured resins. Also precautions to minimize tissue contact should be taken while handling monomer by the dentist and dental personnel in the laboratory. How to cite this article Pradeep N, Sreekumar AV. Résumé :
838 Background: Chemotherapy treatment of Colorectal Cancer, often include 5-Fluorouracil (5- FU). 5-FU inhibits the enzyme thymidylate synthase (TS), stopping the supply of thymidine for DNA synthesis. 5-FU is always combined with a folate, which enhances the 5-FU effect. Marketed folates such as LV/L-LV are prodrugs needing enzymatic conversion. Modufolin is the natural, biologically active form of the folates and is expected to be efficacious in a larger proportion of patients with less inter- and intra-individual variability Methods: ISO-CC-005 is a multi-center, phase I/II study in mCRC patients eligible for 5-FU/folate therapy alone or in combination with irinotecan or oxaliplatin ± bevacizumab. The study investigates safety and tolerability of Modufolin at 4 dose levels by analysing the number and severity of AEs, SAEs and DLTs. Efficacy is evaluated as ORR after four cycles of chemotherapy. Gene expression, deoxyuridine levels as an indirect marker of TS inhibition and time to death is also investigated. Three to six patients per cohort are included. All receives Modufolin twice every two weeks during at least four cycles of chemotherapy. Results: Today, 42 patients have been enrolled and 40 have initiated treatment. 13 are 1st line patients, 16 are in 2nd line, 10 are in 3rd line and 1 is in 5th treatment line. 19 SAEs have been reported in 12 patients, 3 of these were judged as at least possibly related to Modufolin. No SAE were judged as solely related to Modufolin. 31 patients have today been evaluated for efficacy. ORR 1st line patients (n=12) All 50% (6 PR, 6 SD) Patients with Modufolin dose ≥60 mg/m2 71% (5 PR, 2 SD) Patients with Modufolin dose ≥60 mg/m2 + oxaliplatin 100% (3 PR) Conclusions: The lack of need for metabolic activation makes Modufolin a better candidate than LV/L-LV for improved outcome of 5-FU-based chemotherapy regimens in mCRC. The ISO-CC-005 study evaluates Modufolin in combination with 5-FU, irinotecan, oxaliplatin ± bevacizumab in mCRC patients in 4 countries in Europe. The results, so far, for both safety and efficacy seems promising. Clinical trial information: NCT02244632.

Résumé :
Aim. As a clinical part of the Russian multi-centre randomised study, to assess the clinical benefits and safety of adding Trimetazidine MB to the standard treatment scheme in patients with stable angina (SA), in order to optimise their ambulatory therapy and improve their compliance. Material and methods. The study included 981 patients with Functional Class (FC) II-III SA and various concomitant diseases and syndromes, such as chronic heart failure (CHF), Type 2 diabetes mellitus (DM-2), or chronic obstructive pulmonary disease (COPD) in smokers. All participants were divided into two groups: Group I (n=838) received standard therapy plus trimetazidine MB (70 mg/d, twice a day), while Group II (n=143) received standard therapy only. The treatment and follow-up phases lasted for 12 months. Results. In Group I, the weekly number of angina attacks decreased by 42 % from the baseline at one month (р<0,0001), and by 70 % at 12 months (р<0,0001). The number of nitroglycerin (NTG) tablets taken decreased, respectively, by 41 % (р<0,0001) and 68 % (р<0,0001). In Group II, the decrease in angina attack number and NTG tablet number was significant only at 6 months and was less pronounced than in Group I. Therefore, in Group I, the number of patients with FC I increased by 7 times, while the number of FC III patients decreased by 1,8 times. No marked FC dynamics was observed in Group II. In Group I, left ventricular ejection fraction (LVEF) increased by 2,4 % (р<0,001), while interventricular septum and LV posterior wall thickness decreased, respectively, by 4,2 % (р<0,01) and 3,5 % (р<0,01). These parameters, however, did not change substantially in Group II patients. At 12 months, the number of patients hospitalised due to clinical decompensation, or becoming disable, was twice as high in Group II as in Group I. Conclusion. The Russian “PERSPECTIVE” Study results demonstrated high clinical effectiveness and safety ofTrimetazidineMB therapy, combined with standard treatment, in SA patients with CHF, DM-2, and COPD. Therefore, metabolic therapy could be more widely used in the real-world clinical settings.

Résumé :
Food fraud is an old, recurring, and global threat to public health. It poses a serious threat to food security in sub-Saharan Africa (SSA). Despite the prevalence of food fraud in SSA, little is known about how food fraud is viewed by consumers. This study aims to provide an overview of consumers’ concerns about food fraud in SSA. A multi-country survey was conducted in October 2022–31 January 2023, and 838 valid responses were returned. To reduce the large and correlated dataset, Principal Component Analysis (PCA) was used. Five components were derived from PCA: (i) Staple foods; (ii) Premium food and drink products; (iii) Trust in reliable sources; (iv) Trust in less reliable sources; and (v) Trust in food vendors. The findings revealed Ghanaian (mean rank = 509.47) and Nigerian (mean rank = 454.82) consumers tended to score higher on the measure of food fraud concern suggesting that they were less confident in the safety and quality of the food they consume. Demographic characteristics including age, number of children, personal and family experience of food fraud and PCA components such as ‘Staple foods’, ‘Trust in reliable sources’, and ‘Trust in food vendors’ significantly predicted the model. This is the first preliminary study to provide empirical findings on consumers’ concerns about food fraud in SSA. Practical and policy recommendations for the region are suggested. This includes (i) modelling the AfriFoodinTegrity in West Africa across other major regions such as Central, East, and Southern Africa; (ii) establish a regional sub-Saharan Africa Rapid Alert System for Food and Feed (SSA-RASFF) platform; and (iii) food safety and food fraud reports could be incorporated into SSA-RASFF portal for information sharing.

Résumé :
IntroductionPseudo-meningocele is an abnormal collection of cerebrospinal fluid in the extradural space that occurs due to leakage from the CSF-filled spaces surrounding the brain and spinal cord. Pseudo-meningocele may result after spinal surgery. We report a case of a young woman with pseudo-meningocele post lumbar spinal surgery. She subsequently developed recurrent syncope, headaches and clinical signs of raised intracranial pressure (severe papilledema and retinal haemorrhages).CaseA 34 year old woman with spina bifida occulta and a large lipoma at the lumbar spinal level of L5 presented with urinary retention, worsening right leg pain, numbness and foot drop. She subsequently underwent L4-S1 laminectomy, debulking of lipoma and detethering of the cord. Her neurological deficits resolved post-surgery. One month later, the patient re-presented with recurrent syncope and intermittent headaches. She was admitted to the hospital for further investigations. Detailed neurological examination revealed grade IV papilledema and retinal haemorrhages. MRI of the brain showed distended optic nerve sheaths and narrowed distal transverse sinuses, in keeping with intracranial hypertension. MRI spine showed a large CSF collection posterior to the spinal canal at the L4-L5 level, measuring 77×53 mm in trans axial plane and 98 mm in craniocaudal dimension. Her symptoms improved with insertion of external ventricular drain. The symptoms resolved completely after a ventriculo-peritoneal shunt was inserted. She was commenced on acetazolamide. She remained well and asymptomatic. Repeat fundus photography a month later showed complete resolution of papilledema.ConclusionPostoperative pseudo-meningocele is uncommon and the exact incidence is unknown as most of these patients are asymptomatic. This case illustrates that pseudo-meningocele can cause raised intracranial pressure leading to recurrent syncope. It is important for clinician to recognise the varied clinical presentations of meningocele and pseudo-meningocele.References. Marlin A, Epstein F, Rovid R. Résumé :
Abstract The growth hormone and insulin-like growth factor (IGF) system is integral to human growth. Genome-wide association studies (GWAS) have identified variants associated with height and located near the genes in this pathway. However, mechanisms underlying these genetic associations are not understood. To investigate the regulation of the genes in this pathway and mechanisms by which regulation could affect growth, we performed GWAS of measured serum protein levels of IGF-I, IGF binding protein-3 (IGFBP-3), pregnancy-associated plasma protein A (PAPP-A2), IGF-II and IGFBP-5 in 838 children (3–18 years) from the Cincinnati Genomic Control Cohort. We identified variants associated with protein levels near IGFBP3 and IGFBP5 genes, which contain multiple signals of association with height and other skeletal growth phenotypes. Surprisingly, variants that associate with protein levels at these two loci do not colocalize with height associations, confirmed through conditional analysis. Rather, the IGFBP3 signal (associated with total IGFBP-3 and IGF-II levels) colocalizes with an association with sitting height ratio (SHR); the IGFBP5 signal (associated with IGFBP-5 levels) colocalizes with birth weight. Indeed, height-associated single nucleotide polymorphisms near genes encoding other proteins in this pathway are not associated with serum levels, possibly excluding PAPP-A2. Mendelian randomization supports a stronger causal relationship of measured serum levels with SHR (for IGFBP-3) and birth weight (for IGFBP-5) than with height. In conclusion, we begin to characterize the genetic regulation of serum levels of IGF-related proteins in childhood. Furthermore, our data strongly suggest the existence of growth-regulating mechanisms acting through IGF-related genes in ways that are not reflected in measured serum levels of the corresponding proteins.

Emotional disturbance and regulation are key elements in special education classes but are still understudied. This study explores the topic of the emotions of students with special educational needs (SEN). The objective of this study is to examine the level of special education teachers' knowledge of (i) emotional disturbance and (ii) emotional regulation in SEN students. This study uses a quantitative approach in the form of survey as research design. A total of 74 primary school special education teachers were involved as respondents through a simple random sampling technique. Instrument used is a questionnaire adapted from the Emotion Regulation Skills Questionnaire by Mirabile (2014). Cronbach's Alpha validity obtained for the instrument after adaptation and pilot study was .838. Data was collected by distributing online questionnaire to the population of primary special education teachers through social media. Descriptive analysis was conducted on collected data through Statistical Package for Social Sciences (SPSS) software and summarized in table form. The findings of this study show a high level of special education teachers' knowledge of emotional disturbance in SEN students (mean=3.56) while teachers' knowledge of emotional regulation in SEN students recorded a moderate level (mean=2.94). Special education teachers have basic knowledge of emotions in SEN students but yet to reach the optimal level. The planning and implementation of learning in special education classes needs to emphasize the importance of regulating students' emotions and the role of teachers in emotions coregulations with SEN students.

Résumé :
ABSTRACT We spectroscopically detected 838 likely, 448 probable, and 265 possible strong lens candidates within ≈2 million galaxy spectra contained within the extended Baryon Oscillation Spectroscopic Survey (eBOSS) from the sixteenth data release (DR16) of the Sloan Digital Sky Survey (SDSS). We apply the spectroscopic detection method of the Baryon Oscillation Spectroscopic Survey (BOSS) Emission-Line Lens Survey (BELLS) and add Gaussian fit information, grading, additional inspection observables, and additional inspection methods to improve our selection method. We observed 477 candidates with lensing evidence within low-resolution images from both the Legacy survey of SDSS-I/II and the DESI Legacy survey, which is $12{{\ \rm per\ cent}}$ higher than the percentage of BELLS candidates observed with similar lensing evidence. Our search within the latest and improved reductions of the BOSS survey yielded a $20{{\ \rm per\ cent}}$ increase in the number of lens candidates expected from searching all BOSS and eBOSS galaxies. The distribution of target and background redshifts of our candidates is similar to the candidates and confirmed lenses within the BELLS observations. We present our Spectroscopic Identification of Lensing Object candidates in a value-added catalogue in SDSS DR16. The examination of these lens candidates in follow-up high-resolution imaging may yield more than twice the lenses found in previous spectroscopic detection surveys within SDSS, which would extend the results of previous lens surveys within SDSS to higher redshifts, constrain models of mass structures in spiral galaxies, and test if including the identification of possible lensing features within low-resolution images has merit to spectroscopic detection programmes.

Hennessy, D. W., et S. G. Morris. « Effect of a preweaning growth restriction on the subsequent growth andmeat quality of yearling steers and heifers ». Australian Journal of Experimental Agriculture 43, no 4 (2003) : 335. http://dx.doi.org/10.1071/ea02072.

Résumé :
The eating quality of 3 'cuts' of meat was assessed on carcasses of steers (n = 46) and heifers (n = 33) when slaughtered at 17 months of age, following preweaning growth restriction. Growth restriction (for 115 days to weaning) was imposed on half the group ('low'; n = 39 steers and heifers), followed by 2 periods of high-quality nutrition for all animals on (i) pastures (180 days) and (ii) on a grain diet in a feedlot (85 days). Preweaning growth rates (mean and standard error of difference) of steers and heifers on the 'high' treatment (905 v. 838 ± 35.0 g/day) were effectively halved (498 v. 434 ± 35.0 g/day) in the restricted 'low' group. However, no significant compensatory growth occurred when these weight-restricted weaners grazed high-quality pastures, nor during their period in the feedlot. At slaughter, the predicted liveweight of 'high'-growth steers was higher than that of 'low'-growth steers (477 v. 416 ± 11.5 kg) and they had a heavier carcass weight (251 v. 221 ± 7.2 kg). There was no difference between the meat quality (MQ4) scores of meat samples from 'low' or 'high' carcasses in which cuts of blade (2302 'bolar blade', AUS-MEAT 1998) scored higher than rump (2110 'rostbiff', AUS-MEAT 1998) and striploin (2140 striploin, AUS-MEAT 1998) samples. The study has highlighted the persistent effect of undernutrition in early life on final liveweights of Hereford cattle and the consequent reduction in their carcass weight. However, there was no carryover effect of early nutrition on meat quality.

Résumé :
VP1, a pivotal capsid protein encoded by the foot-and-mouth disease virus (FMDV), plays an important role in receptor-mediated attachment and humoral immune responses. Previous studies show that amino acid changes in the VP1 protein of cell culture-adapted strains of FMDV alter the properties of the virus. In addition, FMDV VP1 modulates host IFN signal transduction. Here, we examined the ability of cell culture-adapted FMDV VP1(83K) and wild-type FMDV VP1(83E) to evade host immunity by blocking mitochondrial antiviral signaling protein (MAVS)/TNF Receptor Associated Factor 3 (TRAF3) mediated cellular innate responses. Wild-type FMDV VP1(83E) interacted specifically with C-terminal TRAF3-binding site within MAVS and this interaction inhibited binding of TRAF3 to MAVS, thereby suppressing interferon-mediated responses. This was not observed for cell culture-adapted FMDV VP1(83K). Finally, chimeric FMDV harboring VP1(83K) showed very low pathogenicity in pigs. Collectively, these data highlight a critical role of VP1 with respect to suppression of type-I IFN pathway and attenuation of FMDV by the E83K mutation in VP1.

Résumé :
It has been hypothesized that contextual variables (i.e. match location, opponents’ level) could be important deter- minants of running performance (RP) in soccer matches, but empirical studies provided inconsistent evidences. This study aimed to investigate the associations between contextual variables (CV) and RP in Croatian soccer players. Players’ RP (n=193) were evaluated, and used as cases in this study. CV included match outcome (win-draw-loss), opponent’s level (higher vs. lower ranked), and match location (home-away). RP were measured by global position- ing system during the soccer matches in seasons 2018/19 (n=14) and 2019/20 (n=15) of Croatian first division, and included: the total distance covered, low-intensity (<14.3 km/h), running (14.4–19.7 km/h), high-intensity running (>19.8 km/h), high-speed running (19.8–25.1 km/h) and sprinting (>25.2 km/h). Results indicated significantly great- er amount of distance covered in running zone for away matches (F-test: 7.83, p<0.01, small effect size; 1392±353 m and 1262±294 m, for home and away matches, respectively). Similar RP were observed irrespective of opponents’ level (F-test: 0.05 to 1.53, p>0.05). Lowest total-, low- and running-zone distances were evidenced in won matches (9893±896 m, 8035±614 m and 1241±312 m, respectively), followed by drawn matches (10298±913 m, 8287±838 m and 1363±320 m, respectively) and lost matches (10355±1052 m, 8279±742 m and 1406±343 m, respectively). This study demonstrated that (i) won matches were characterized by lower RP, (ii) association between RP and match location was limited, (iii) RP was not related to the opponents’ level.
