Résumé :

SUMMARYThe child day-care centre (DCC) is often considered as one risk factor for gastroenteritis (GE) rather than a complex setting in which the interplay of many factors may influence the epidemiology of GE. This study aimed to identify DCC-level risk factors for GE and major enteropathogen occurrence. A dynamic network of 100 and 43 DCCs participated in a syndromic and microbiological surveillance during 2010–2013. The weekly incidence of GE events and weekly prevalence of five major enteropathogens (rotavirus, norovirus, astrovirus,Giardia lamblia, Cryptosporidium hominis/parvum) were modelled per DCC using mixed-effects negative binomial/Poisson regression models. Sixteen hundred children were surveyed up to 3 years, during which 1829 GE episodes were reported and 5197 faecal samples were analysed. Identified risk factors were: large DCC capacity, crowding, having animals, nappy changing areas, sandpits, paddling pools, cleaning potties in normal sinks, cleaning vomit with paper towels (but without cleaner), mixing of staff between child groups, and staff members with multiple daily duties. Protective factors were: disinfecting fomites with chlorine, cleaning vomit with paper towels (and cleaner), daily cleaning of bed linen/toys, cohorting and exclusion policies for ill children and staff. Targeting these factors may reduce the burden of DCC-related GE.

Résumé :

The participation in Erasmus and Erasmus+ programmes is the most popular form of mobility among the university teachers and other academic staff as well as students. The geographers of Klaipėda University can study in more than 20 universities of different regions of Europe, and the geography of studies expands from year to year. While studying in foreign countries, they not only deepen their knowledge, but also broaden the geographic scope, acquaint with new people and cultures, strengthen the knowledge of foreign language. From the other side, the students of foreign universities, having been chosen the geographic modules at Klaipėda University, have a possibility to see and learn about the nature of Western Lithuania, social and economic objects, the cultural environment. According to the foreign students, the studies are organized so, that academic staff is able to collaborate with each student immediately, and the atmosphere of studies is very good. Besides the foreign students, Klaipėda University receives also the foreign academic staff, organizes the international practices, develops the projects, and the academic staff of Klaipėda university has a possibility to visit the universities of foreign countries. Those visits give the invaluable benefit to the teachers, because one can receive more experience, and the newly adopted methods may be applied at Klaipėda University.

Abstract Norovirus (NV) infections are a frequent cause of gastroenteritis (GE), but data on this disease in immunocompromised patients are limited. We analyzed an NV outbreak, which affected immunosuppressed patients in the context of chemotherapy or HSCT. On recognition, 7 days after admission of the index patient, preventive measures were implemented. Attack rates were only 3% (11/334) and 10% (11/105) among patients and staff members, respectively. The median duration of symptoms was 7 days in patients compared with only 3 days in staff members (P = .02). Three patients died of the NV infection. Commonly used clinical diagnostic criteria (Kaplan-criteria) were unsuitable because they applied to 11 patients with proven NV-GE but also to 15 patients without NV-GE. With respect to the therapeutic management, it is important to differentiate intestinal GVHD from NV-GE. Therefore, we analyzed the histopathologic patterns in duodenal biopsies, which were distinctive in both conditions. Stool specimens in patients remained positive for NV-RNA for a median of 30 days, but no transmission was observed beyond an asymptomatic interval of 48 hours. NV-GE is a major threat to patients with chemotherapy or HSCT, and meticulous measures are warranted to prevent transmission of NV to these patients.

Résumé :

The Government Enterprise (GE) implemented a turnaround strategy to enhance service efficacy and citizenry satisfaction in 2014. However, this GE takes too long like three months to issue any key documents such as birth, death, marriage certificates, passports, and work permits today. This reflects that the efficacy of services and the satisfaction of citizens is questionable. Hence, the main objective of the research was to investigate the turnaround strategy implementation in GE. Adopting the qualitative descriptive research design, questionnaires administered to the management and staff members, and interviews conducted with clients, thematic analysis has been undertaken. Findings reveal that the turnaround strategy did improve service efficacy and citizenry satisfaction for some time but there was a relapse. Recommendations include the adoption of customer service strategies along with the revival of the turnaround strategy and top management new team who can introduce with fresh ideas, staff training with constant monitoring, and control measures.

ABSTRACTFood handlers play an important role in the transmission of norovirus (NoV) in food-borne outbreaks of gastroenteritis (GE). In a year-round prevalence study, the prevalence of NoV in catering companies without recently reported outbreaks of GE was investigated and compared to the observed prevalence in catering companies with recently reported outbreaks. Swab samples were collected from surfaces in the kitchens and (staff) bathrooms in 832 randomly chosen companies and analyzed for the presence of NoV RNA. In total, 42 (1.7%) out of 2,496 environmental swabs from 35 (4.2%) catering companies tested positive. In contrast, NoV was detected in 147 (39.7%) of the 370 samples for 44 (61.1%) of the 72 establishments associated with outbreaks of gastroenteritis. NoV-positive swabs were more frequently found in winter, in specific types of companies (elderly homes and lunchrooms), and in establishments with separate bathrooms for staff. We found a borderline association with population density but no relation to the number of employees. Sequence analysis showed that environmental strains were interspersed with strains found in outbreaks of illness in humans. Thus, the presence of NoV in catering companies seemed to mirror the presence in the population but was strongly increased when associated with food-borne GE. Swabs may therefore serve as a valuable tool in outbreak investigations for the identification of the causative agent, although results should be interpreted with care, taking into account all other epidemiological data.

Mass gatherings can be disruptive to the daily operations of any emergency department (ED). These events usually are spontaneous and sudden. Therefore, operational processes must be effective and concise when dealing with a sudden surge. This study examines the Tan Tock Seng Hospital (TTSH) ED response process to a gastroenteritis (GE) outbreak. Prompt identification and establishment of a casualty holding and treatment area ensured smooth operational capacity, which allowed these patients to be segregated from the mainstream ED crowd and more specific care to be rendered. Entrance and exit points of the designated area were established with controlled access to prevent cross-contamination with the mainstream patient load. Patients with GE who presented with acute symptoms required immediate assessment and intervention, placing stress on existing personnel. Hence, adequate personnel was an important factor that could not be disregarded. Staff burnout was a plausible issue that was recognized from the start and attempts were made to prevent burnout by creating an encouraging work environment and allowing frequent relieving of duties. Communicating the event to relevant departments ensured that the ED was adequately supported during the GE outbreak, both administratively and logistically. This was a reflection of the established communication channels. Leadership also had an essential and crucial role to play as the nursing and medical leaders had to be decisive, delegate roles and give concise instructions during the chaotic situation. The availability and access to ample logistical supplies saves on precious time, which allowed more focus on patients. In summary, procedures and protocols, together with staff preparedness, enhances an ED operational capability of effectively responding to mass gatherings.

As part of an overall quality initiative at General Electric (GE), a study was undertaken to understand and reduce usability defects in the software development lifecycle at GE Information Services, Inc. Six Sigma methodology, which tries to increase quality through systematic process improvement, was applied to a sample of usability data. The results of this effort found that usability defects were highest in those products where there had been minimal involvement of the User Interface staff and usability defects were most dramatically reduced by using a systematic approach to usability (i.e., the user interface design process). When presented to upper management, the net effect of this study was a raised consciousness in the company that usability defects are very controllable, a policy statement issued that future development efforts would include the user interface design process, and a commitment for increased resources for the User Interface team.

Résumé :

This article presents an overview of GE’s HA gas turbines that represent the most reliable and efficient machines in the world for converting natural gas into electricity. In a combined cycle arrangement, these turbines provide cost-effective and clean generation that offers reliable electricity to an expanding, global population. The 7/9HA turbine is based upon the original H-class 4-stage gas turbine with exception of simplification by eliminating steam cooling. Metals chosen for the 7/9HA are proven alloys with over 50 million hours of operation on F- and H-class gas turbines. The first 9HA.01 entered commercial operation on June 17, 2016 at the Électricité de France Bouchain plant, located in the Nord Pas-de-Calais region of France. GE followed the Guinness Book of World Records’ definition for a consistent and traceable operating condition for establishing efficiency in world records. Under the oversight of Guinness World Records staff, GE set the record for the world’s most efficient combined-cycle power plant with an efficiency of 62.22% while producing more than 605 MW of electricity.

The subject of this work presented is the theme of organizational welfare. For this reason we analysed the structures of organizational climate (D'Amato, Mayer, 2005), organizational health (Avallone, Paplomatas, 2005) and empowerment (Piccardo, 1995). The focus of our attention is mainly on the universitary context and on the necessity to make changes that would contribute to the psychological welfare of the public administration's workers. The analysis of the literature enabled to detect the instruments mainly used and the researches made in regards to the italian context. Here is presented a qualitative research about the Organizative Climate made in Biblioteca dell'Università di Milano Bicocca. It has been built an ad hoc model and it has been used to build the structure of the inteview. Fourteen inteviews were made to understand how the librarians live the organization and trying to identify the most critical situation. The analysis focused on structural, role, relations and management components.

The objective of research is to analyze the professional education of the librarians, their society’s image, their present specific roles, their communication with the school directors, their interest in the school librarians’ function, their relationship with the educational community and the librarians’ policies in education that are related to the librarian tasks. This is a critical, descriptive and explaining study, with a quantitative focus and the deductive empirical method. Rafaela City is set to be the universe of this study and ten schools which have the official positions of librarians and its agents have the professional degree of librarians are set to be the population of this study. The conclusion is that the directors have a blurred image of the school librarians. The school authorities recognize the librarians’ professional skills to develop the librarian functions creatively as well as their pedagogical and cultural roles but they do not offer librarians the needed tools to put all their potential into practice. However, the librarians strengthen this weakened image by devaluating their profession in some aspects such as: declaring lack of teaching vocation, not attending the teachers and parents’ meetings, and not knowing the school library rules at an international level. Anyways, the instability in the librarians’ policies from the school institution and the lack of school librarians’ legal concept in the government do not give opportunities to reinforce the professional image of the school librarian.

Résumé :

The majority of librarians at Canadian post-secondary institutions have recently attained faculty status. However, the collective agreements of the faculty associations do not always explicitly state that librarian members participate in all three traditional faculty activities: teaching, scholarship, and service. This study focuses on the membership of the Canadian Association of University Teachers (CAUT), using qualifying member collective agreements and library websites. A qualitative approach is used to build upon the research first reported by Leckie and Brett (1995). Through description, analysis, and interpretation of data, I report that there is a disparity between what the collective agreements state regarding the teaching responsibilities of librarians and what the library websites advertise on behalf of teaching librarians. Three themes of library teaching emerge and two trends for collective agreements regarding post-secondary librarians teaching are considered.

Monitoring thermal performance in the current generation of combined cycle power plants is frequently a challenge. The “lean” plant staff and organizational structure of the companies that own and operate these plants frequently does not allow the engineering resources to develop and maintain an effective program to monitor thermal performance. Additionally, in many combined cycle plants the highest priority is responding to market demands rather than maintaining peak efficiency. Finally, in many cases the plants are not designed with performance monitoring in mind, thus making it difficult to accurately measure commonly used indices of performance. This paper describes the performance monitoring program being established at a new combined cycle plant that is typical of many combined cycle plants built in the last five years. The plant is equipped with GE 7FA gas turbines and a GE reheat steam turbine. The program was implemented using a set of easy-to-use spreadsheets for the major plant components. The data for the calculation of indices of performance for the various components comes from the plant DCS system and the PI system (supplied by OSIsoft). In addition to the development of spreadsheets, testing procedures were developed to ensure consistent test results and plant personnel were trained to understand, use and maintain the spreadsheets and the information they produce.

The GE Hitachi Nuclear Energy (GEH) nuclear criticality safety (NCS) function remains actively engaged in advancements to the nuclear fuel cycle. In addition to its traditional BWR fuel manufacturing, recent GEH emphasis to become more vertically integrated into front end (enrichment) and back end (reprocessing) fuel cycle technologies has had a dramatic impact on the NCS function. Required fundamental and practical research in various fields, such as general physics, computational methods, validation methodology, cross-section data processing, criticality safety assessments, risk-informed integrated safety analyses, and domestic and international nuclear packaging licensing, collectively present significant challenges to NCS staff. As the landscape of the GEH business growth opportunities continues to evolve over time, so does the required depth of NCS knowledge and technical expertise. This paper provides an overview of select NCS design, licensing, methods, and packaging activities in support of GEH nuclear fuel cycle business subsidiaries and concludes with some insight to technical and regulatory challenges.

Non-linear fracture mechanics equations for through-wall cracks in a pipe are used to analyze piping systems for either critical flaw size or critical loading conditions as part of probabilistic Leak-Before-Break (LBB) failure analyses under the eXtremely Low Probability of Rupture (xLPR) program co-sponsored by the U.S. Nuclear Regulatory Commission (US NRC) and the Electric Power Research Institute (EPRI). The xLPR analysis techniques use a large number of independent analysis solutions to determine an overall assessment of system failure probability. As part of the assessment, each independent solution requires the solution of the crack opening displacement (COD) for a through-wall crack (TWC) in a pipe under the prescribed loading conditions. The COD evaluations are then used to determine a leak rate for the given load conditions and crack sizes. The purpose of this paper is to present results which advance the start-of-the-art for determining the elastic-plastic functions for crack opening displacements (COD) for a TWC in a pipe system under combined tension and bending loads. The current method used to determine COD in xLPR, a blending of tension and bending solution from the GE-EPRI Handbook, determined the continuum equations using structural finite element analyses with shell type elements. Since that body of work was undertaken, there have been significant advancements in computing capability such that structural finite element analyses with three-dimension continuum elements are currently feasible. The use of continuum elements provides several advantages over shell elements; such as, the ability to elicit details of variation in the COD through the thickness of the pipe wall and to apply pressure to the crack face due to the internal pipe pressure. Furthermore, the original GE-EPRI solutions were limited for the case of combined tension and bending loads. The existing GE-EPRI solutions for combined loading conditions are limited to pipe radius-to-wall thickness (R/t) ratios of 10 or greater, typical of those piping systems found in the boiling water reactor (BWR) fleet. For the PWR piping systems of concern today, which are subject to primary water stress corrosion cracking (PWSCC), the R/t ratios are typically 5 or less. As a result of the limitations with the existing GE-EPRI method for predicting COD, Battelle and US NRC staff set out to develop a comprehensive COD prediction tool for combined loadings which would be applicable to both PWR as well as BWR piping. This effort involved a matrix of over 1,200 finite element analyses for a full range of pipe sizes, R/t ratios, through-wall crack (TWC) lengths, and internal pipe pressures. It is anticipated that there will be several parts to this effort. Part I, discussed in this paper, focuses on the development of the model and the initial investigation into the elastic- and elastic-plastic fitting functions for the prediction of COD (i.e., the V and h functions). Future parts of this effort will focus on such issues as the effect of restraint of pressure induced bending on COD, the effect of weld residual stresses on COD, J-Integral estimation schemes, and development of variable crack-face pressure.

Abstract Industry and University collaborates to develop methods to simulate the nonlinear dynamics of components in rotating assemblies like turbine or compressor modules in the presence of friction joints. This collaboration produced fruitful results providing a family of numerical solvers with the goal of foreseeing the safety margins against High Cycle Fatigue failure. Softwares are therefore intended as design tools to exploit the damping effect of joints by controlling geometrical features, materials and contact loads. Contact models, reduction techniques to handle complex blade geometries modeled by Finite Element softwares, and numerical techniques to solve the nonlinear equations of motion are refined to provide the level of vibration amplitude as fast as possible by keeping the representativeness of the physical phenomena that are involved. A reliable compromise between speed and accuracy must be confirmed by several ‘gates’ to pass through during all the simulation process, in particular during pre-processing phase. The objective of this paper is to propose a good practice made of a list of actions to check the goodness of the mathematical basis to obtain reliable results from simulation. Experience gained thanks to the long-lasting collaboration between Politecnico di Torino and GE Avio for the development of the software Policontact provides a case study of an effective synthesis between two requirements that are often opposed to each other: complex mathematical models to simulate the nonlinear forced response of rotating components on one side and a robust, confident implementation of an easy-to-use tool intended for industrial staff with complementary background on the other side.

Abstract On 07.07.1939 the Neuchatel gas turbine passed its performance test under the supervision of Aurel Stodola [1]. This was the world’s first open gas turbine for electric power generation in commercial operation. It launched the past eighty year period of further development and corresponding market growth. The Baden area played an important role. The aim of this paper is to complete the already published comprehensive historical information with two additional aspects especially regarding the second 40 years, in which the author was an involved contemporary witness: The first is how the predominance of something like a local spirit integrated both a considerable share of foreign engineering staff and also the changes of the ownership of the technology from Brown Boveri to ABB and then to Alstom and recently to GE and Ansaldo. The second aspect is the inside view of the author, who has both shaped and suffered these gas turbine developments in several job positions allowing direct contact to both top management and shop floor workers. These two aspects will be integrated in the historical sequence. As a rule the roles of the persons acting after the seventies are given but not their names. The history of the involved companies has caught much attention of media and writers. After the formation of ABB it was used for both celebrating outstanding management performance and despicable management mistake. I will add as an engineer my insider view to this and mention the corresponding book references. This paper is limited to the mainstream open GT development for space reasons and therefore omits other interesting side developments of BBC such as closed Helium cycles, IGCC applications and compressed air energy storage as well as the other products of BBC, which all played a role in equalizing the business cycles.

The inhomogeneous distribution of crosslinks in polymer networks results in nonuniform swelling. Concomitant with this behavior is local variability in the elastic properties of synthetic and biopolymer gels. Articular cartilage exemplifies the compositional and structural complexities found in soft tissues. At the most basic level, cartilage extracellular matrix (ECM) is a relatively stiff network of collagen type II fibers with entangled hyaluronic acid chains and enmeshed aggrecan molecules. Despite significant differences in composition, synthetic and biological gels exhibit qualitatively similar responses (e.g., viscoelasticity and nonlinear stress-strain behavior at large deformation). Scaling theory [1] and experiments [2–3] have verified that the shear modulus (Ge) of chemically identical, fully swollen gels differing only in the degree of crosslinking is proportional to the polymer concentration (ce): (1)Ge=Acen where A and n are constants. In a good solvent, n = 2.25 [1]. Recent studies have shown that the power law applies to collagen gels, with n ≈ 2.68 [4]. In the general case, (2)G=Acen-mcm where G and c are the general shear modulus and polymer concentration, respectively, and m = 1/3 [5].
