AbstractWe measured body temperature of Mountain Chickadees (Poecile gambeli) and Juniper Titmice (Baeolophus ridgwayi) at different times of day and under a range of ambient temperatures in order to determine the use of nocturnal hypothermia in seasonally acclimatized small passerines. Our findings show both species used nocturnal hypothermia year-round. Depth of hypothermia was inversely correlated to body mass in Juniper Titmice but not in Mountain Chickadees. In both species, depth of hypothermia did not vary seasonally but nocturnal body temperature was regulated 3–11°C lower than daytime values. Nocturnal energy savings range from 7%–50% in chickadees and from 10%–28% in titmice. These nocturnal energy savings translate into ecologically important reductions in daily energy expenditures for these two species.Hipotermia Nocturna en Individuos de Poecile gambeli y Baeolophus ridgwayi Aclimatados EstacionalmenteResumen. Nuestros resultados muestran que ambas especies presentaron hipotermia nocturna durante todo el año. The objective of this article is to provide a comprehensive personal survey of all the major parasomnias with coverage of their clinical presentation, investigation, physiopathogenesis and treatment. These include the four major members of the slow-wave sleep arousal parasomnias which are enuresis nocturna (bedwetting), somnambulism (sleepwalking), sleep terrors (pavor nocturnus in children, incubus attacks in adults) and confusional arousals (sleep drunkenness). Other parasomnias covered are sleep-related aggression, hypnagogic and hypnopompic terrifying hallucinations, REM sleep terrifying dreams, nocturnal anxiety attacks, sleep paralysis, sleep talking (somniloquy), sexsomnia, REM sleep behavior disorder (RBD), nocturnal paroxysmal dystonia, sleep starts (hypnic jerks), jactatio capitis nocturna (head and total body rocking), periodic limb movement disorder (PLMs), hypnagogic foot tremor, restless leg syndrome (Ekbom syndrome), exploding head syndrome, excessive fragmentary myoclonus, nocturnal cramps, and sleep-related epileptic seizures. There is interest in the possibility of relationships between sleep/wake states and creativity.

Abstract We studied the nocturnal foraging behavior of Surf Scoters (Melanitta perspicillata) and White-winged Scoters (Melanitta fusca) during winter in coastal British Columbia, Canada. Using radio telemetry, we collected nocturnal and diurnal data documenting the frequency of foraging dives and the location of scoters in relation to their intertidal foraging grounds. We found that dive foraging rarely occurred during nocturnal periods for either species. Only 2% of nocturnal observation blocks for both scoter species contained diving, compared with 98% of diurnal observation blocks. This corresponded to an average of only 0.1 min spent underwater per half-hour observation block during the night and over 7 min during the day. Both species of scoters were located farther offshore and in deeper waters during nocturnal hours, indicating that they were not using intertidal foraging areas at night. Our results suggest that Surf Scoters and White-winged Scoters face daylight-imposed limits on the amount of available foraging time. These potential day-length restrictions should be considered when reviewing human activities that potentially alter the amount of available foraging time or food supplies in winter habitats. Se estudió el comportamiento de forrajeo nocturno de Melanitta perspicillata y M. fusca durante el invierno en la costa de British Columbia. Utilizando radio-telemetría, se colectaron datos nocturnos y diurnos sobre la frecuencia de buceos para alimentarse y la ubicación de las aves con relación a la zona intermareal. Se encontró que los buceos nocturnos son infrecuentes para ambas especies. Sólo en el 2% de las observaciones nocturnas de ambas especies se presentaron instancias de buceo, en comparación con el 98% de las observaciones diurnas. Esto corresponde a un promedio de 0.11 minutos bajo el agua por cada 30 minutos de observación durante la noche y más de 7 minutos durante el día. Ambas especies se ubicaron más alejadas de la costa y en aguas más profundas durante la noche, indicando que los individuos no utilizaron zonas intermareales durante este periodo. Background: Nocturia is common and associated with multiple disease states. Many potential mechanisms have been proposed for nocturia, which also remains challenging to manage. Purpose: To use multivariate analysis to determine which combinations of factors can accurately discriminate clinically significant nocturia in patients to facilitate clinical management and treatment decisions. Patients and methods: Data analysis was based on frequency volume charts from three randomized controlled trials. There were 1479 patients included, of which 215 patients had no/mild nocturia and 1264 had clinically significant nocturia with at least two voids per night. Factors studied that may influence nocturia were demographics, sleep duration, functional bladder capacity, 24 h urine volume and literature-suggested definitions of nocturnal polyuria. We used univariate analysis and cross-validated multivariate modelling to assess association between factors and nocturia status, redundancy between factors and whether the combined use of factors could explain patients′ nocturia status. Results: The multivariate analyses showed that the most useful definitions of nocturia are ’Nocturia Index’ (NI) and ‘Nocturnal Urine Production per hour’ (NUPh) in combination with functional bladder capacity and sleep duration. Published definitions providing binary nocturnal polyuria outcomes had lower performance than continuous indices. These analyses also showed that NI was not specific to nocturnal polyuria as it also captured nocturia due to low functional bladder capacity. By contrast, NUPh was demonstrated to be specific to nocturnal polyuria. Conclusion: NUPh has previously been shown among elderly males to be essential in nocturia and a very valid measure of nocturnal polyuria. However, the current, large and independent dataset now confirms that it can be applied in an adult population with a complaint of nocturia covering both males and females.

Background: In 2018, nocturia and nocturnal lower urinary tract function definitions were updated in a clinically and practically based consensus report by the International Continence Society Standardization Steering Committee. Previous research has suggested that the pathophysiology of nocturia has a multifactorial etiology, including obstructive sleep apnea, overactive bladder syndrome, diabetes mellitus, sleep disturbance, congestive heart failure, primary polydipsia, and other factors.Current Concepts: Three main mechanisms have been identified: low functional bladder capacity, nocturnal polyuria, and diurnal polyuria (24-hour polyuria). Multifactorial pathophysiology implies multiple possible targets for therapeutic intervention, and suggests that it is unlikely that one treatment modality, including drugs, will be successful in all patients. The bladder diary is the most important diagnostic tool.Discussion and Conclusion: Strong evidence supports the efficacy of desmopressin and continuous positive pressure breathing. Antimuscarinic drugs for treating nocturia display limited usefulness because of their low efficacy for nocturnal polyuria. Management of nocturia may require a multidisciplinary approach to visualization and phenotyping of patients for diagnosis and therapy.

Introduction: We investigated the efficacy, safety, and impact of desmopressin on quality of sleep in treating nocturnal polyuria in elderly women.Methods: We recruited 60 women over 60 years old with lower urinary tract symptoms (LUTS), including nocturia, and with nocturnal polyuria. Nocturnal polyuria was defined as nighttime urine production exceeding 33% of the 24-hour total urine volume determined by a frequency volume (FV) chart. All patients failed to respond to treatment of their underlying disease and evening fluid restriction. Desmopressin 0.1 mg was administered orally at bedtime for 12 weeks. The participants completed a series of questionnaires on the Medical Outcomes Study (MOS) sleep scale and FV chart before and after treatment.Results: The patient population had a mean age of 69.2 ± 9.4 years (range: 61–81). The mean duration of symptoms was 61.2 ± 45.1 months. Significant decreases were evident after desmopressin treatment in the number of nocturia episodes (3.63 ± 1.61 to 2.00 ± 1.13, p = 0.01), nocturnal urine volume (p = 0.01), nocturnal polyuria index (NPI) (p = 0.01), and nocturia index (NI) p = 0.01). Among the categories of the MOS sleep scale, sleep index (p = 0.003), sleep disturbance (p = 0.001), snoring (p = 0.028), and shortness of breath (p = 0.036) significantly changed, with a decreased number of nocturia episodes. Adverse events were mild.Conclusions: Desmopressin is an effective treatment for nocturnal polyuria in elderly women, where conservative treatment has failed. Sleep quality is also improved.

To date, few detailed studies have been conducted on the convenient and useful markers for the prevalence of lower urinary tract symptoms (LUTS), including overactive bladder (OAB) and nocturia. A high level of calcium (Ca) excretion (hypercalciuria) is indicative of lifestyle-related diseases such as hypertension, which are associated with the onset of LUTS. Hence, in this study we attempted to clarify the relationship between urinary Ca excretion and OAB, nocturia, and nocturnal polyuria in adults. The present study showed that patients with hypercalciuria frequently experienced OAB, nocturia, and nocturnal polyuria. In addition, this study revealed that the severity of LUTS is significantly associated with urinary Ca excretion and that hypercalciuria is an important risk factor for OAB, nocturia, and nocturnal polyuria.

Les périodes d’activité des animaux difficilement observables des forêts primaires sont peu connues. Des caméras pièges ont été utilisées pour établir les périodes d’activité de quelques mammifères au Parc national de Taï. Ces caméras non appâtées ont été installées d’octobre 2016 à août 2017. L’enregistrement de l’heure et la date de chaque observation a permis de déterminer des activités nocturnes, diurnes et cathémérales (actif le jour et la nuit) des espèces à travers le pourcentage des vidéos obtenues. Les heures d’activité pour chaque espèce étaient concentrées autour du vecteur moyen (heure moyenne autour de laquelle sont concentrées les heures d’activité de l’espèce) et réparties de manière non-uniforme pendant la période de 24h. Les mammifères de petite masse corporelle (< 10 kg) avaient des tendances nocturnes alors que ceux de grande masse corporelle (> 10 kg) avaient des tendances nocturnes, diurnes ou cathémérales. Certaines espèces cathémérales ont montré des périodes d’activité à prédominance diurne ou nocturne et doivent faire l’objet d’études supplémentaires enfin de connaître les facteurs influençant leur régime d’activité.Mots clés : caméra piège, taux de capture, mammifères, vecteur moyen. English Title: Activity periods of some terrestrial mammals by using camera traps in Taï National ParkDetermination of cryptic animal’s activity periods is problematic in primary forests and reminds poorly known. Hence, in the Tai National Park, camera traps were used to establish the periods of activity of some mammals. These non-baited cameras were installed in the research area of the park from October 2016 to August 2017. Recording the time and date of each observation enabled to determine nocturnal, diurnal and cathemeral activities of each species through the percentage of videos obtained. Hours of activity for each species were concentrated around the mean vector and were not uniformly distributed over the 24-hour period. Furthermore, mammals with smaller body mass (<10 kg) had nocturnal tendencies whereas those with larger body mass (> 10 kg) had nocturnal, diurnal or cathemeral tendencies. Certain cathemeral species have predominantly shown day or night periods of activity and must be subject to further studies to find out more about the factors influencing their regimes of activity.Keywords: camera trap, detection rate, mammals, mean vector.

Abstract Many species, including shorebirds, feed during both day and night, yet little is known about how this affects behavior and habitat preferences. European Golden-Plovers (Pluvialis apricaria) and Northern Lapwings (Vanellus vanellus) feeding on arable farmland were more widely dispersed at night: nocturnal flocks were smaller, typically monospecific, and occurred in many more fields than diurnal mixed-species flocks. Diurnal numbers of European Golden-Plovers could not be used to predict nocturnal numbers; this indicates that ranging behavior differed between day and night. For both species, nocturnal feeding was recorded on almost all nights, irrespective of moon phase. Northern Lapwing nocturnal feeding activity decreased with increasing cloud cover and decreasing ground temperature, but no clear relationships were detected between European Golden-Plovers’ nocturnal feeding activity and environmental variables. Habitat selection differed between day and night, and between species at night. Diurnal studies of habitat choice and site selection may misrepresent the full requirements of such species. Abstract Introduction In nocturia, longer FUSP (time to first void) correlates with better quality sleep (Bliwise et al, JCSM 2015;11:53-5) and, with treatment, longer FUSP is associated with decreased nightly voids (Epstein et al, Neurourol Urodyn 2018;37:186-91). We examined urologic correlates of FUSP in an outpatient nocturia population without comorbidities (CHF, OSA, ESRD, diuretics). Methods Participants (n=119; men) kept a home flow/volume diary, tracking clock time and quantity of each urination across a 24-hr period. FUSP was defined as time between going to bed and time of first void. We analyzed the urine volume at first nocturnal void (FNVV) (i.e., at end of FUSP). We also analyzed all nighttime volumes and divided by reported hours of sleep to impute nocturnal urine production (NUP) (in ml/hr, classified as high [&gt;90 ml/hr] [n=49] vs low [&lt;90 ml/hr] [n=60])—a measure correlated with number of nocturia episodes (van Doorn et al, J Urol 2014;191:1034-9). Nocturnal maximal voided volume (NMVV) at any single nocturnal void defined maximal functional nocturnal bladder capacity. Data were analyzed non-parametrically. Results For 53 of 119 patients, FNVV was identical to NMVV. This was more likely in patients with NUP &gt;90 ml/hr vs &lt;90 ml/hr (59% vs 40%, p=.046). High (vs low) NUP rates were also associated with higher FNVV (300 [225-420] vs 135 [100-200] ml, p&lt;.001), as well as higher number of voids (3 vs. 2, p=.03). Conclusion For nearly half of these nocturia patients, the volume at first void occurred at their maximal nocturnal volume. In nocturia, higher FNVV also reflects greater overall nocturnal volume of urine produced, and excess urine volume (as opposed to insufficient bladder capacity) likely plays a central role in the pathogenesis of nocturia in these patients. The extent to which these higher initial volumes represent free-water vs solute-driven clearance is currently under investigation. Support N/A

With Hecate Nocturne, I set out to create a unified piece of substantial length. The piece features certain sounds of British Columbia---birds, animals, wind, water, machinery, and folksong; their musical depictions represent a growing interest of mine, one which was expanded significantly in this piece.
The primary goal of the thesis is a close interconnection of all musical material, at all levels; that a limited pool of material could produce, through motivic development, all the components of the piece, from small to large. The secondary goal is to give the music a "sense of place" through depictions of natural sounds occurring---in this case---in British Columbia. The tertiary goal is to write musical returns, or recapitulations, that are always significantly altered from their original presentations, to give the piece a feeling of consequence or alteration. This last goal arises from the aesthetic application of some of the composer's philosophical beliefs.

The theme of the first movement of the of the trio is introduced by the violin playing a lyrical melody of four measures. The melody is then imitated by the cello. To make the trio a true one, the piano takes on the theme. This movement is based on interplay between instruments of various combinations. The second movement is made up of tonal colors.The piano introduces them by placid strokes of chords. The singing quality of the violin adds great color to the composition. The sonorous melodic line of the cello produces smooth quality of sound. The third movement is reminiscent of a child's carousel. It is playful and full of various whimsical rhythms.

Nocturnal enuresis is one of the most common and pervasive childhood problems. Without effective intervention, the child's nocturnal enuresis may persist. This may result in possible distress and even psychological problems in the child. The current study had two aims. The first aim was to examine the effectiveness of retention control training, and the urine alarm if necessary, as an intervention for primary nocturnal enuresis. The second aim was to explore children's attributions for the therapeutic outcome of the intervention method regarding their nocturnal enuresis. The first aim was important, as previous research has revealed contradictory findings. The second aim was also important, as to date, research has neglected to explore this topic. Six children aged 8 to 12 years and their primary caregivers participated in the current study. Attribution data was obtained at three intervals through one-on-one semi-structured interviews between the researcher and child. Results found that with retention control training: one child achieved nocturnal urinary continence, three children decreased the number of their wet nights, and two children failed to respond. The addition of the urine alarm enabled three more children to achieve nocturnal urinary continence. In terms of attributions, results found that brief psycho-education appeared to have influenced these children‟s attributions. However, overall attribution findings suggested that children perceived researcher assistance, unknown factors, and the techniques used in retention control training to have influenced their success in the achievement of nocturnal urinary continence. These results would help to inform general practitioners and psychologists in terms of intervention recommendations for evidence-based practice, and future research.

This paper describes a biotelemetric application whereby information of tooth contact pressure from within the mouth of a human subject is transmitted to a bedside receiver where it is processed and used in the biofeedback treatment of nocturnal bruxism (grinding of the teeth). Bruxing information is encoded on a pulse width modulated 313 MHZ carrier. Issues that are addressed include miniaturization of the transmitter, minimization of power requirements, stabilization of carrier frequency, receiver selection, and the various problems associated with getting a radio frequency signal out of the mouth.

For more than three centuries, violin pedagogical practices have been extensively developed towards music covering the common practice period. However, a problem arises when a violin student performing avant-garde music needs to find realistic solutions to problems that are not addressed in the standard repertoire. This critical essay offers a pedagogical approach to a work that fits well within this paradigm: Four Nocturnes (Night Music II), George Crumb’s only published work for violin and piano duo. The multi-dimensional aspect of this avant-garde work requires an equally multi-faceted approach to overcoming the inherent technical hurdles. Through practical illustrations and concise explanations, musical examples indicate how the score may be re-notated and simplified to create a preliminary step towards advancing to the original notation. Borrowing from the methodology of Otakar Ševčík and other leading twentieth-century violin pedagogues, the author shows how students can modify their approach to fit contextually in the realm of avant-garde music. Students who approach the work with this methodology will find it helpful in eliminating many of the potential pitfalls that they are likely to encounter.

This study constitutes an investigation of Gabriel Fauré's achievements in the genre of the piano nocturne through detailed analysis of each of his thirteen nocturnes. Fauré's nocturnes are widely considered to be representative of his pianistic style and musical language, showing a great spectrum of compositional procedures and covering the different stages of his life. The investigation is contextualised through a history of nocturne form, a biographical background of the composer, an overview of the three compositional stages in his life and of various aspects of his musical style, comparisons with the music of Fauré's predecessors and contemporaries and with the rest of his oeuvre, and a general appraisal of the contributions of other authors and scholars in the field. As a corpus, the nocturnes form a unique contribution to the genre, and, especially in the larger-scale examples, a more complex structure than one encounters in the examples by other exponents of the nocturne such as Field, Chopin, Liszt, Balakirev, Scriabin, Satie, Debussy and Poulenc. The investigation focuses on a close reading of the nocturnes, and especially on thematic, motivic and harmonic usage, and on Fauré's most original contribution, his complex integration of advanced chromaticism and extended modal procedures. The latter, which include the use of modes of non-diatonic scales and modes on altered degrees, have received hardly any scholarly attention, despite constituting a highly significant contribution to musical thought. By treating harmonic features as referential elements, Fauré creates a complex interaction between harmonic and structural procedures which greatly advances the scope and structural integration of the nocturne

Les approches actuelles pour diagnostiquer les troubles du sommeil sont lourdes, intrusives et peuvent influer sur la qualité du sommeil du patient. Il y a donc un besoin crucial de systèmes moins encombrants pour diagnostiquer les problèmes liés au sommeil. Nous proposons d'utiliser un nouveau système de suivi du sommeil non intrusif basé sur un tapis à fibre optique à microflexion placée sous le matelas de lit. La qualité du sommeil est évaluée en fonction de différents paramètres, y compris la fréquence cardiaque, le rythme respiratoire, les mouvements du corps, l’heure du réveil, la durée du sommeil, le mouvement nocturne et l’heure du coucher. Le système proposé a été validé dans un environnement de santé et de bien-être, en plus d'un environnement clinique comme suit. Dans le premier cas, la fréquence cardiaque est mesurée à partir de signaux ballistocardiogramme bruités acquis de 50 volontaires en position assise à l'aide d'une chaise de massage. Les signaux sont recueillis discrètement à partir d'un capteur de fibre optique microflexible intégrée dans l'appui-tête de la chaise, puis transmis à un ordinateur par une connexion Bluetooth. La fréquence cardiaque est calculée à l'aide de l'analyse multi-résolution de la transformée discrète en ondelettes à chevauchement maximal. L'erreur entre la méthode proposée et électrocardiogramme de référence est estimée en battements par minute en utilisant l'erreur absolue moyenne où le système a obtenu des résultats relativement bons (10.12±4.69) malgré la quantité remarquable d'artefact de mouvement produit en raison des fréquents mouvements corporels et/ou vibrations de la chaise de massage pendant le massage de soulagement du stress. Contrairement à l'algorithme complet de décomposition du mode empirique de l'ensemble, précédemment utilisé pour l'estimation de la fréquence cardiaque, le système proposé est beaucoup plus rapide. Par conséquent, il peut être utilisé dans les applications temps réel. Dans ce dernier cas, nous avons évalué la capacité du capteur de fibre optique microflexible pour suivre la fréquence cardiaque et la respiration d’une manière discrète. En outre, nous avons testé la capacité du capteur dans la discrimination entre la respiration superficielle et pas de respiration. Le capteur proposé a été comparé à un dispositif de surveillance portatif à trois canaux (ApneaLink) dans un milieu clinique au cours d'une endoscopie sous anesthésie. Parmi les dix patients recrutés pour notre étude, le système a obtenu des résultats satisfaisants quant à la fréquence cardiaque moyenne et quant à la fréquence respiratoire moyenne avec une erreur de 0.55 ± 0.59 battements/minute et de 0.38 ± 0.32 respirations/minute, respectivement. De plus, le coefficient de corrélation Pearson entre le capteur proposé et le dispositif de référence était de 0.96 et 0.78 pour la fréquence cardiaque et la respiration, respectivement. Au contraire, le capteur proposé a fourni une très faible sensibilité (24.24 ± 12.81%) et une spécificité relativement élevée (85.88 ± 6.01%) pour la détection de l'apnée du sommeil. On s'attend à ce que cette recherche préliminaire ouvre la voie vers la détection discrète de l'apnée obstructive du sommeil en temps réel. Suite à la validation réussie du système proposé, nous avons déployé avec succès notre système de surveillance du sommeil pendant plus de 6 mois dans treize appartements habités principalement par les personnes âgées. Néanmoins, dans cette recherche, nous nous concentrons sur un déploiement d'un mois avec trois résidents seniors de sexe féminin. Le système proposé montre l’accord avec l’enquête utilisateur recueillie avant l'étude. En outre, le système est intégré dans une plate-forme d’autonomie assistée existante avec une interface conviviale pour rendre plus commode pour les aidants le suivi des paramètres de sommeil des résidents
The current approaches for diagnosing sleep disorders are burdensome, intrusive, and can affect the patient’s sleep quality. As a result, there is a crucial need for less cumbersome systems to diagnose sleep-related problems. We propose to use a novel nonintrusive sleep monitoring system based on a microbend fiber-optic mat placed under the bed mattress. The sleep quality is assessed based on different parameters, including heart rate, breathing rate, body movements, wake up time, sleep time, night movement, and bedtime. The proposed system has been validated in a health and wellness environment in addition to a clinical environment as follows. In the former case, the heart rate is measured from noisy ballistocardiogram signals acquired from 50 human volunteers in a sitting position using a massage chair. The signals are unobtrusively collected from a microbend fiber optic sensor embedded within the headrest of the chair and then transmitted to a computer through a Bluetooth connection. The heart rate is computed using the multiresolution analysis of the maximal overlap discrete wavelet transform. The error between the proposed method and the reference ECG is estimated in beats per minute using the mean absolute error where the system achieved relatively good results (10.12 ± 4.69) despite the remarkable amount of motion artifact produced owing to the frequent body movements and/or vibrations of the massage chair during stress relief massage. Unlike the complete ensemble empirical mode decomposition algorithm, previously employed for heart rate estimation, the suggested system is much faster. Hence, it can be used in real-time applications. In the latter case, we evaluated the capacity of the microbend fiber optic sensor to monitor heart rate and respiration unobtrusively. In addition, we tested the capacity of the sensor in discriminating between shallow breathing and no breathing. The proposed sensor was compared to a three-channel portable monitoring device (ApneaLink) in a clinical setting during a drug-induced sleep endoscopy. Across all ten patients recruited for our study, the system achieved satisfactory results in the mean heart rate and the mean respiratory rate with an error of 0.55±0.59 beats/minute and 0.38 ± 0.32 breaths/minute, respectively. Besides, the Pearson correlation coefficient between the proposed sensor and the reference device was 0.96 and 0.78 for heart rate and respiration, respectively. On the contrary, the proposed sensor provided a very low sensitivity (24.24 ± 12.81%) and a relatively high specificity (85.88 ± 6.01%) for sleep apnea detection. It is expected that this preliminary research will pave the way toward unobtrusive detection of obstructive sleep apnea in real-time. Following successful validation of the proposed system, we have successfully deployed our sleep monitoring system in thirteen apartments with mainly senior residents over six months. Nevertheless, in this research, we concentrate on a one-month deployment with three senior female residents. The proposed system shows an agreement with a user’s survey collected before the study. Furthermore, the system is integrated within an existing ambient assisted living platform with a user-friendly interface to make it more convenient for the caregivers to follow-up the sleep parameters of the residents

THE PROBLEM: Rest cramps (also known as nocturnal leg cramps) are painful muscle contractions, typically in the legs or feet, that occur during prolonged rest - most often while in bed at night. Although common in older adults, safe and effective treatment options are lacking. OPPORTUNITY AND METHOD #1: Magnesium supplements are readily available and widely marketed to consumers for cramp prophylaxis. However the efficacy of magnesium for this indication is unclear and existing clinical trials are limited by the well recognized poor bioavailability of oral magnesium. To better assess the potential efficacy of magnesium in cramp prophylaxis I conducted a (N=46) double-blind, placebo-controlled RCT on community-dwelling rest cramp sufferers to determine whether a more reliable delivery method (5 consecutive days intravenous infusion of 20 mmol magnesium sulfate) could reduce the frequency of rest cramps. I additionally determined whether the response to treatment varied with the extent to which infused magnesium was retained (as measured by 24-hr urinary magnesium excretion) and performed a Cochrane Systematic Literature Review to find and synthesize all relevant randomised trials. OPPORTUNITY AND METHOD #2: Anecdotal evidence suggested several medications might promote muscle cramping. If true, cramp sufferers using these drugs could potentially gain cramp relief through therapeutic substitution or reduction of these agents. To investigate this potential cramp link I searched BC Ministry of Health databases containing diagnostic and prescribing information on the 4.2 million residents of British Columbia to determine, using sequence symmetry methods, whether quinine starts (i.e. new cramp treatment) increased in the year following introduction of the three most commonly prescribed medications with a link to muscle cramps (diuretics, statins and inhaled long-acting beta2-agonists). CONCLUSION: Although its role in pregnancy-associated rest cramps remains unclear, magnesium supplementation does not meaningfully reduce the frequency of rest cramps in older adults. Alternatively, for some cramp sufferers, reduction or discontinuation of select cramp promoting medications (inhaled long-acting beta2-agonists, potassium-sparing diuretics and thiazides) may be a useful therapeutic maneuver. Over a 13 year period 60.3% of quinine users (cramp sufferers) received at least one of these medications. In contrast, statin and loop-diuretic cramp associations were clinically unimportant.

The precise pathophysiology of nocturnal asthma still remains to be elucidated. Activated platelets have the ability to release potent broncho- and vaso-constrictors and therefore, have been implicated in asthma. However, there is no information on the status of in vivo platelet activation in patients with nocturnal asthmaIn a randomised controlled study five healthy volunteers and five asthmatics were investigated during a period of 24h after acclimatisation for one day. Both peak flow rate (PFR) and blood samples were obtained at 4 hourly intervals. Plasma levels of platelet factor 4 (PF4) and beta- thromboglobulin(BTG) were measured by radioimmunoassay and adrenaline (A), noradrenaline (NA) and dopamine (DOP) by radioenzymatic analysis.PFR (1/min) for the 24h period was significantly lower in asthmatics (401±15SEM, P<0.001) than in controls (598+4SEM) with an apparent circadian rhythm peak (442±73, P<0.05 Wilcoxon's test) at 4.00pm only in asthmatics. Although there was no significant differences in either PF4 and BTG (ng/ml) or A, NA and DOP (nmol/1) between asthmatics and controls an apparent circadian rhythm in all of these parameters was demonstrated in both groups. Peak values (mean+SEM) for PF4 (8.9±1.5) and BTG (44.4±3.8) occurred at 8.00am whereas the highest values for the catecholamines (A: 0.36±0.08, NA: 1.75±0.23, DOP: 0.78±0.16) were observed at 4.00pm indicating a lag of 8h between the peaks for catecholamines and the platelet specific proteinsThese initial data demonstrate a clear difference in PFR between asthmatics and controls which is apparently not associated with changes in either PF4 or BTG but which may concur with circadian changes in plasma levels of catecholamines at least in asthmatic patients. Thus, in vivo platelet activation is probably not a contributing factor in nocturnal asthma. Finally, the phase lag between peak plasma levels of platelet proteins and catecholamines requires further investigation

Photoreceptors and neurons at various levels to cortex have been counted in mouse and rat. The ratios of neuron numbers (rat/mouse) are similar to the ratio of retinal areas or the squared ratio of eye sizes; so to a first approximation the two species have linearly scaled eyes, equal photoreceptor spacings (in microns), and visual pathways scaled numerically by the number of photoreceptors. The following points can be made: Photoreceptors outnumber neurons at all subsequent levels. Cone sampling exceeds the behavioral acuity limit, the latter being largely determined at or before the ganglion cell level. There are few neurons in the LGNd, and so the corresponding eye parameter p is very large, as is appropriate for nocturnal animals. The general organization is economical, if not sparse, so undersampling occurs even for central vision when the pupils are small. Esta publicación está destinada a apoyar a los responsables de la toma de decisiones que deben iniciar, promover y gestionar la imagen nocturna de su ciudad, y que necesitan tener un plan director que proporcione una base sólida para el desarrollo de la iluminación a largo plazo. The purpose of this publication is to provide guidance about the objectives and underlying principles relating to the lighting aspects of the urban nightscape. It deals with the visual, organizational, environmental, and technical elements of these aspects of urban planning. This guide identifies the lighting planning criteria that should be considered when initiatives are being taken in relation to new or existing lighting in urban areas or newly planned conurbations. Guidance is provided to both the functional and expressive aspects of lighting. Review question / Objective: To provide the essential findings in the field of overlap syndrome of chronic obstructive pulmonary disease and obstructive sleep apnea, including prevalence, possible predictors, association with clinical outcomes, and severity compared to both chronic obstructive pulmonary disease and obstructive sleep apnea patients. Condition being studied: OSA is characterized by complete cessation (apnea) or significant decrease (hy-popnea) in airflow during sleep and recurrent episodes of upper airway collapse cause it during sleep leading to nocturnal oxyhemoglobin desaturations and arousals from rest. The recurrent arousals which occur in OSA lead to neurocognitive consequences, daytime sleepiness, and reduced quality of life. Because of apneas and hypopneas, patients are experiencing hypoxemia and hypercapnia, which result in increasing levels of catecholamine, oxidative stress, and low-grade inflammation that lead to the appearance of cardio-metabolic consequences of OSA. COPD is a chronic inflammatory lung disease defined by persistent, usually pro-gressive AFL (airflow limitation). Changes in lung mechanics lead to the main clini-cal manifestations of dyspnea, cough, and chronic expectoration. Furthermore, patients with COPD often suffer from anxiety and depression also, the risk of OSA and insomnia is higher than those hospitalized for other reasons. Although COPD is twice as rare as asthma but is the cause of death eight times more often.
