Résumé :
Abstract Population aging has become the norm in China. Improving or reconstructing the elderly support system is therefore essential. While the government and institutional support has been improving and enhancing in China, family remains the most important source of elderly support for a long period in the future. Based on data from censuses and national surveys (1982-2015), we examine recent changes in household size and structure across ages from an individual life course perspective, to reveal the complexities and ambiguities behind the nationwide household change. Our findings suggest that multi-generational co-residence is a major vehicle to accommodate the needs of family members at critical stages of life. These salient features of family change in China call for new family-oriented policies, including pragmatic incentives to strengthen intergenerational solidarity, essential support for family caregivers, and diversified community services to care for frail elders.

Abstract Community System of Social Support is the basis of modern health care systems not only in Poland but all over the world in the prophylaxis-treatment-rehabilitation procedure. In Poland, family doctors, commune councils and territorial self-governments are involved in it. Family doctors perform a substantial function, whereas commune councils and territorial self-governments an administrative one. The principle of the system work is evaluation of its benefits. Its aim is the reduction of the number of patients who abuse alcohol and those who abuse alcohol despite treatment.

Résumé :
Families experiencing difficulties often need multidimensional support in fulfilling their basic functions, those of caring and upbringing, economic or socialisation. Conscious and planned use of family support instruments implies combining the support potential of state institutions, in particular social policy institutions, non-governmental organisations and family resources. It should therefore be one of the most important tasks of active social policy.

Résumé :
This qualitative and quantitative study focused on exploring hospice patient’s perceptions of family support. Family support was conceptualized as which family member they expect to receive support from, types of support provided, frequency of support, family communication about medical illness, and type of support that is perceived to be most helpful. Fifteen partcipants between the ages of 70 to 98 were interviewed using purposive sampling. Findings showed that although participants found both physical and emotional support helpful, more than half reported perceiving emotional support as the most helpful. Recommendations for social work practice and research were discussed.

Résumé :
The subject of this thesis is an analysis of support for families with children in pension and tax system of the Czech Republic. In pension system family is supported in connection with survivor pensions and parents are supported in relation to retirement pensions. Tax system allows to families with children various tax credits that reduce the final tax liability. Furthermore, from this system are paid state benefits and foster care benefits. In terms of overall tax support are in the Czech Republic the most supported medium- and high-income households with both parents and single parent households the least. The theoretical part deals with various kinds of support within each system. The practical part contains analysis of support from the tax system and health insurance system as other form of support for families with children. Its aim is to quantify changes in income of model families based on the number of children and income level before kids.

Résumé :
Some families in the Free State Province, specifically in Thabo-Mofutsanyana district, are families living with mentally ill family members. The families accompany the mentally ill family members to designated clinics which offer primary, secondary and tertiary mental health services, and all these levels of care provide care, treatment and rehabilitation of mentally ill persons (SA, 2002:2). Despite the care that these families give to their mentally ill family members, it seemed that they find it difficult to cope living with a mentally ill family member. This is supported by Zergaw, Hailemariam, Alem and Kebed (2008:191) who indicate the economic impact, disruption of daily life, work and family relationships experienced by families living with a mentally ill family member. This was also evident in a study by Du Plessis, Greeff and Koen (2004:4), that, because of a lack of a formal support system for families living with mentally ill family members, and lack of necessary skills to take on the responsibility of caring for mentally ill relatives, mentally ill family members relapse and are readmitted in the mental health care institutions. Adding to this, in most of the families living with mentally ill family members in the Thabo-Mofutsanyana district, it is not known how they cope and are supported. Once there is a clear understanding of how families experience living with mentally ill family members, health care practitioners can give better support to families living with mentally ill family members. The research objective of this study was to explore and describe the experiences of families living with a mentally ill family member in the Thabo-Mofutsanyana district, to explore and describe the strengths of families living with a mentally ill family member, and to formulate guidelines to support families living with mentally ill family members. A qualitative, phenomenological design was used which enabled the researcher to understand the way in which the families experience living with a mentally ill family member. The population studied in this research consisted of families living with mentally ill family members in the Thabo-Mofutsanyana district in South Africa. The purposive sampling was used to select participants with the assistance of a mediator. Permission to conduct research was negotiated with the district manager. Voluntary and informed consent in writing was obtained from all the participants. The sample size was determined by data saturation, which was reached after 14 interviews. Data analysis was carried out simultaneously with data collection. In consensus discussions, the researcher and the co-coder reached consensus on the main and sub themes. From the research findings, five main themes were identified. The first two themes are the positive and negative experiences of living with a mentally ill family member. The third theme is the concerns of family in staying with a mentally ill family member. The fourth theme is the strengths in coping with a mentally ill family member. The last theme is ways in which family members as families wanted to be supported. It could be concluded that the experience of families living with mentally ill family members in Thabo-Mofutsanyana district need to be supported. In order to address this support holistically and to enhance quality of life and to lessen the burden the families experience in living with a mentally ill family member, the support should firstly be addressed individually and then as a family. Following that, the collective support can be addressed by a support network system through involvement of the department of health, clinics, rehabilitation centres, community, mental health care workers, giving of medication and establishment support of groups of families living with mental health care users. From these findings the researcher proposed guidelines to support families living with mentally ill family members, and to increase knowledge in these families on how to handle a mentally ill family member. The researcher also compiled recommendations for nursing practice, nursing education and nursing research in families living with mental health care users.

Résumé :
Inadequate, expensive health care services for the chronically ill, confront the lay person as one of the major forces in the social movement toward self-care. The purpose of this predictive correlational study was to investigate within a theoretical framework derived from Orem's conceptual model of nursing relationships among family as a social support system, exercise of self-care agency, and health status for the person with a chronic illness. The sample group was 51 chronically ill adults diagnosed with diabetes mellitus who attended diabetic instructions from one of the Central Indiana Hospitals. Findings indicated a weak positive correlation between self-care and cohesion and weak negative correlation between health status and cohesion. Findings also revealed significant differences between male and female for the variables self-care and conflict. Conclusions were that this study supported certain relationships proposed by Orem's conceptual model in nursing. Results can guide diabetics in providing optimal daily self-care. Professionals in the health care arena can utilize the results for educational development and encouragement of self-care agency in the adult diabetic. The family members can also clarify roles in supporting the diabetic adult.

Résumé :
The monograph reveals the key socio-economic problems of the innovation economy: its content as a knowledge economy and its role in evolutionary development; human capital (living intelligence) as the main resource of the innovation economy. Much attention is paid to the institutional support of innovation through a system of institutions and mutually beneficial contracts. The mixed mechanism of implementation of innovative activity as a synthesis of spontaneous market self-regulation and conscious public administration is shown. The result of the "social control" of society and the state is the coordination of the actions of economic entities and the ordering of economic processes. The most important institution of human society is the family as a strong power in the state. And the person himself with his knowledge, culture, ethics and morality is the main value of society. The main purpose of the family is to reproduce life and provide a person with everything necessary. The state as an institution manages a person's education and health, helps to change his lifestyle, strengthening humanity, ethics, morality and culture of life. The modern global economy remains a sphere of domination of market egoism. It is the market that performs the function of morality as a person and society as a whole. In the global economy, a person is not a representative of the people, but a representative of the system, a standard way of life. And he should live in communication based on respect for each other. It is concluded that today the main wealth of society is not material, but social wealth: the person himself with his knowledge, culture, ethics and morality is a living intellect; a family with the reproduction of life; immaterial knowledge that covers all types of work that cannot be calculated and paid, where the motive is the joy of free cooperation, free giving and community. In this "invisible economy" people mutually teach each other humanity and create a culture of joint thinking and living together. The State and society must preserve and increase the social wealth of human society. For students and postgraduates of economic and managerial specialties, as well as for anyone interested in this problem.

Résumé :
AbstractThis chapter highlights various challenges and developments related to our current global food production and consumption systems, followed by a number of research and innovation activities that actively support a transition to a sustainable, healthy, and just agri-food system. The first section identifies several important trends that may either limit, support, or influence such a transition (Sect. 6.1). Following this, I will provide an overview of several research and innovation activities that our research group is currently involved in, such as the Dutch national research programme (NWA) ‘Transition to a Sustainable Food System’ (Sect. 6.2), nature-inclusive and regenerative agriculture (Sect. 6.3), closing the gaps between citizens, farmers, and nature (Sect. 6.4), measuring sustainability and health aspects of our food supply chains (Sect. 6.5), and the South Holland Food Family, an open innovation and food transition network (Sect. 6.6). This last section also provides an example of TSEI-framework application to analyze institutional change during initiation, development, and implementation of the South Holland Food Family innovation network (Sect. 6.6).

Résumé :
The world is aging rapidly, and the population of over 65 years old in Japan is 29.1% (Sep. 15, 2022, Japan Statistic Bureau of Japan), which is the highest in Japanese history. Human memory and knowledge are rapidly being digitized on an incalculable scale. While its value as a booster for monetization is now known worldwide, such private and personal heritage, especially its whereabouts remains unknown. In particular, the memory and knowledge of elders are not recorded appropriately for the next generations, we claim that the current system has shown an enormous loss of value in society, especially for the family members. Therefore, the desire to interview and document the life experiences of different generations of family members is very important. However, interviewing and documenting are difficult to achieve for various reasons, in such cases as when family members live apart from each other. Therefore, our research group has started to develop Life History Support System called “LHS”. The new system aims to solve the problem and preserve elderlies' wisdom and knowledge cultivated in turbulent times, such as during WW II and the post-war years of recovery. The LHS is designed for the Family Social Network, allowing digital information to be accessed only by the same members. LHS is an application that runs on smartphones, tablets and PC which is connected to the Internet and works as a social network system (SNS), but the main difference between conventional SNS is (1) LHS can be accessed only by the family members or designated members, (2) it mainly works as a card type database to share topic cards among members. We have developed a prototype system using Apple’s Claris FileMaker database system which runs on-premises private server. Then, to test the prototype's applicability, we have performed a preliminary interview experiment in an actual user environment (family members living together or living apart, and the elderly person living alone). The result shows that we could identified the experience of “fun” by both, an interviewer and interviewee, during the process of recall of memories with the LHS setup. Rather, we confirm the needs in longitudinal study to capture the continuous use of the LHS. Since the LHS inherently gains its value by long-term regular use, interviewing, recording and viewing by many family members, it is necessary to add new functions based on some theories. We are planning to include gamification functions to LHS. This paper describes the LHS system overview and the current development status.

Résumé :
Efforts to support help-seeking by victims of partner violence in couples affected by incarceration represent a key part of larger efforts in the fields of domestic violence and victim services to improve the accessibility of services in marginalized communities and better meet complex victim needs. Qualitative data from 167 Multi-site Family Study participants suggest that involvement with the criminal justice system (whether directly or through a family member) introduces unique individual, interpersonal, and sociocultural barriers to defining one’s experiences as a problem, deciding to seek help, and selecting sources of help. Opportunities exist not only to tailor service delivery approaches in ways that overcome the individual and interpersonal obstacles that affect victims but also to pursue longer-range shifts in public policy and community infrastructure that will address broader and more-entrenched barriers to help-seeking.

Outcome Measure Harmonization and Data Infrastructure for Patient-Centered Outcomes Research in Depression : Final Report. Agency for Healthcare Research and Quality (AHRQ), juin 2022. http://dx.doi.org/10.23970/ahrqepcwhitepaperdepressionfinal.

Texte intégral
Résumé :
Objective. The objective of this project was to demonstrate the feasibility and value of collecting harmonized depression outcome measures in the patient registry and health system settings, displaying the outcome measures to clinicians to support individual patient care and population health management, and using the resulting measures data to support patient-centered outcomes research (PCOR). Methods. The harmonized depression outcome measures selected for this project were response, remission, recurrence, suicide ideation and behavior, adverse effects of treatment, and death from suicide. The measures were calculated in the PRIME Registry, sponsored by the American Board of Family Medicine, and PsychPRO, sponsored by the American Psychiatric Association, and displayed on the registry dashboards for the participating pilot sites. At the conclusion of the data collection period (March 2020-March 2021), registry data were analyzed to describe implementation of measurement-based care and outcomes in the primary care and behavioral health care settings. To calculate and display the measures in the health system setting, a Substitutable Medical Apps, Reusable Technology (SMART) on Fast Healthcare Interoperability Resource (FHIR) application was developed and deployed at Baystate Health. Finally a stakeholder panel was convened to develop a prioritized research agenda for PCOR in depression and to provide feedback on the development of a data use and governance toolkit. Results. Calculation of the harmonized outcome measures within the PRIME Registry and PsychPRO was feasible, but technical and operational barriers needed to be overcome to ensure that relevant data were available and that the measures were meaningful to clinicians. Analysis of the registry data demonstrated that the harmonized outcome measures can be used to support PCOR across care settings and data sources. In the health system setting, this project demonstrated that it is technically and operationally feasible to use an open-source app to calculate and display the outcome measures in the clinician’s workflow. Finally, this project produced tools and resources to support future implementations of harmonized measures and use of the resulting data for research, including a prioritized research agenda and data use and governance toolkit. Conclusion. Standardization of outcome measures across patient registries and routine clinical care is an important step toward creating robust, national-level data infrastructure that could serve as the foundation for learning health systems, quality improvement initiatives, and research. This project demonstrated that it is feasible to calculate the harmonized outcome measures for depression in two patient registries and a health system setting, display the results to clinicians to support individual patient management and population health, and use the outcome measures data to support research. This project also assessed the value and burden of capturing the measures in different care settings and created standards-based tools and other resources to support future implementations of harmonized outcome measures in depression and other clinical areas. The findings and lessons learned from this project should serve as a roadmap to guide future implementations of harmonized outcome measures in depression and other clinical areas.

Access to Contraceptive Services Among Adolescents in Uganda During the COVID-19 Pandemic. Guttmacher Institute, novembre 2021. http://dx.doi.org/10.1363/2021.33206.

Texte intégral
Résumé :
Key Points The COVID-19 pandemic and its associated restrictions raised concerns that adolescents in Uganda may face barriers to receiving family planning commodities and services. Public and private service statistics data reveal that Uganda experienced smaller and shorter pandemic-related disruptions in adolescent access to sexual and reproductive health services than were initially anticipated. A temporary dip in service visits during April 2020 was followed by a return to previous levels and even an increase in family planning visits among adolescent women during the remainder of 2020. These results speak to the resilience of the health system, which continued to provide services, and the impact of actions by partners to support access to reproductive health supplies and services. The Ministry of Health and other stakeholders should take the following steps to ensure adolescents’ continued access to sexual and reproductive health services throughout the remainder of the pandemic: Document and foster the adaptions and processes that helped ensure continuity of services during the COVID-19 restrictions, so that similar measures may be adopted in future crises. Promote meaningful participation of young people in COVID-19 task forces, to ensure that relevant policies are gender-inclusive and respond to young people’s sexual and reproductive health needs. Classify contraceptive care as an essential service, integrate it with COVID-19 response interventions and ensure that it remains fully operational and open before, during and after any future lockdowns.

Résumé :
A growing number of states are launching community school initiatives to ensure family and community engagement, provide enriched and expanded learning, and offer integrated supports for students. This brief offers examples from the National Center for Community Schools and from New York, New Mexico, and California showing how technical assistance can build capacity through consultation, training, coaching, and knowledge building. These examples indicate the ways that states are designing technical assistance (TA) systems to support practitioners, the value of providing differentiated TA supports, and the impact of cross-sector partnerships on TA provision.

Sessa, Guido, et Gregory Martin. Role of GRAS Transcription Factors in Tomato Disease Resistance and Basal Defense. United States Department of Agriculture, 2005. http://dx.doi.org/10.32747/2005.7696520.bard.

Texte intégral
Résumé :
The research problem: Bacterial spot and bacterial speck diseases of tomato are causedby strains of Xanthomonas campestris pv. vesicatoria (Xcv) and Pseudomonas syringae pv.tomato (Pst), respectively. These bacteria colonize aerial parts of the plant and causesignificant losses in tomato production worldwide. Protection against Xcv and Pst bycultural practices or chemical control has been unsuccessful and there are only limitedsources of genetic resistance to these pathogens. In previous research supported in part byBARD IS-3237-01, we extensively characterized changes in tomato gene expression uponthe onset of spot and speck disease resistance. A remarkable finding of these studies wasthe inducibility in tomato leaves by both Xcv and Pst strains of genes encodingtranscriptional activator of the GRAS family, which has not been previously linked todisease resistance. Goals: Central goals of this research were to investigate the role of GRAS genes in tomatoinnate immunity and to assess their potential use for disease control.Specific objectives were to: 1. Identify GRAS genes that are induced in tomato during thedefense response and analyze their role in disease resistance by loss-of-function experiments.2. Overexpress GRAS genes in tomato and characterize plants for possible broad-spectrumresistance. 3. Identify genes whose transcription is regulated by GRAS family. Our main achievements during this research program are in three major areas:1. Identification of tomato GRAS family members induced in defense responses andanalysis of their role in disease resistance. Genes encoding tomato GRAS family memberswere retrieved from databases and analyzed for their inducibility by Pst avirulent bacteria.Real-time RT-PCR analysis revealed that six SlGRAS transcripts are induced during theonset of disease resistance to Pst. Further expression analysis of two selected GRAS genesshowed that they accumulate in tomato plants in response to different avirulent bacteria orto the fungal elicitor EIX. In addition, eight SlGRAS genes, including the Pst-induciblefamily members, were induced by mechanical stress in part in a jasmonic acid-dependentmanner. Remarkably, SlGRAS6 gene was found to be required for tomato resistance to Pstin virus-induced gene silencing (VIGS) experiments.2. Molecular analysis of pathogen-induced GRAS transcriptional activators. In aheterologous yeast system, Pst-inducible GRAS genes were shown to have the ability toactivate transcription in agreement with their putative function of transcription factors. Inaddition, deletion analysis demonstrated that short sequences at the amino-terminus ofSlGRAS2, SlGRAS4 and SlGRAS6 are sufficient for transcriptional activation. Finally,defense-related SlGRAS proteins were found to localize to the cell nucleus. 3. Disease resistance and expression profiles of transgenic plants overexpressing SlGRASgenes. Transgenic plants overexpressing SlGRAS3 or SlGRAS6 were generated. Diseasesusceptibility tests revealed that these plants are not more resistant to Pst than wild-typeplants. Gene expression profiles of the overexpressing plants identified putative direct orindirect target genes regulated by SlGRAS3 and SlGRAS6. Scientific and agricultural significance: Our research activities established a novel linkbetween the GRAS family of transcription factors, plant disease resistance and mechanicalstress response. SlGRAS6 was found to be required for disease resistance to Pstsuggesting that this and possibly other GRAS family members are involved in thetranscriptional reprogramming that takes place during the onset of disease resistance.Their nuclear localization and transcriptional activation ability support their proposed roleas transcription factors or co-activators. However, the potential of utilizing GRAS familymembers for the improvement of plant disease resistance in agriculture has yet to bedemonstrated.

Kislev, Yoav, Ramon Lopez et Ayal Kimhi. Intergenerational Transfers by Farmers under Different Institutional Environments. United States Department of Agriculture, avril 1995. http://dx.doi.org/10.32747/1995.7604936.bard.

Texte intégral
Résumé :
This research studies the issues of intergenerational transfers in general and farm succession in particular in two different institutional environments. One is the relatively unregulated farm sector in the United States, and the other is the heavily regulated family farms in Israeli moshavim. Most of the analysis is based on modern economic theory dealing with inheritance and other intergenerational issues. However, we start with two background studies. One is a review of the legal system affecting farm succession in the moshav, which, as we claim throughout the report, is of major importance to the question in hand. The second is an ethnographical study aimed at documenting various inheritance and succession practices in different moshavim. These two studies provide insight for most of the economic studies included here. The theoretical studies mostly deal with various aspects of two major decisions faced by farmers: who will succeed them on the farm, and when will succession take place. The first decision clearly depends on the institutional structure: for instance, Israeli farmers are limited to one successor while American farmers are not. The second decision can be taken in three stages: sharing farm work with the successor, sharing farm management, and eventually transferring the ownership. The occurrence and length of each stage depend on the first decision as well as on the institutional structure directly. The empirical studies are aimed at analyzing the practices and considerations of Israeli and American farmers regarding various intergenerational transfers-related issues. We found that American farmers' decisions are mainly driven by the desire to let the farm prosper in future generations and by a preference for equal treatment of heirs, and not at all by old-age support considerations. In contrast, we demonstrate the significant effect of old-age support on the value of the transferred farm in a sample of Israeli farms. Using Israeli census data, we find that the time of farm ownership transfer responds to economic incentives. A smaller Israeli panel data set shows that controlling for the occurrence of succession, farm size rises with operator's age and eventually falls, while intensity of production seems to decline steadily. This explains another finding, that farm transfer contributed significantly to farm growth when farming was attractive to successors. This finding supports our main conclusion, that the succession decisions are of major importance to the viability and profitability of family farms over the long run.

Institute for Social Science in the 21st Century, University College Cork, mai 2023. http://dx.doi.org/10.33178/10468/14485.

Résumé :
This research, conducted in conjunction with One Family, set out to document the lived experiences of Jobseeker’s Transitional Payment (JST) recipients and to explore how JST is working ‘on the ground’. Because JST is a relatively new payment in the Irish social welfare system, little is known about how it is experienced by recipients. Furthermore, because people living in single parent households are consistently over-represented in poverty statistics across all metrics (at risk of poverty, enforced deprivation and consistent poverty), how caregivers in one-parent households experience a policy that is designed with such households in mind represents important work. The research was qualitative in nature and the original data presented in the report were collected via one focus group coupled with a series of ten interviews. A substantial review of the literature was also undertaken, and this was used to frame the research. Available statistics, along with statistics obtained via parliamentary questions, are also used to inform the research. The core aims for this research were as follows: Develop an in-depth understanding of the lived experiences of the recipients of JST; Develop an understanding of how JST policy is working ‘on the ground’; Document the challenges and benefits associated with the payment; Develop a claimant-based user guide as a resource for new entrants to the payment scheme; Generate research data of relevance to One Family and related support and advocacy groups in their work with one parent families and their policy work in terms of the future direction of JST.
