Moncrieff & Smyth (1999, this issue) are certainly right that community treatment orders (CTOs) are high on the agenda and that psychiatrists need to think long and hard about them – about the political and ethical implications, not just their practical and therapeutic applications. Their concerns are important and reflect a wide constituency – identical views were expressed and considered during the consultation that preceded the College's document proposing a Community Supervision Order in 1993 (Royal College of Psychiatrists, 1993). Moncrieff & Smyth make no mention of that document, nor the limited, but recent, survey of psychiatrists' opinions that accompanied it (Burns et al, 1993). Do all College documents achieve obscurity quite so quickly?

ABSTRACTThe Nuclear Regulatory Commission (NRC) announced the availability of three new documents concerning radioactive materials in sewage sludge and ash from publicly owned treatment works (POTW). One of the documents is a report presenting the results of a volunteer survey of sewage sludge and ash samples provided by 313 POTWs. The second document is a dose modeling document, using multiple exposure pathway modeling focused on a series of generic scenarios, to track possible exposure of POTW workers and members of the general public to radioactivity from the sewage sludge or ash. The third document is a guidance report providing recommendations on the management of radioactivity in sewage sludge and ash for POTW owners and operators. This paper explains how radioactive materials enter POTWs, provides criteria for evaluating levels of radioactive material in sludge and ash, and gives a summary of the results of the survey and dose modeling efforts.

The document had been obtained at the Nuremberg war trials. My original report to Hughes on the content of this document is reproduced here.

Work instructions laboratory equipment is one of the documents that must be available in the laboratory. This document provides information on the operation steps of the device correctly. A laboratory manager usually prints the document on A4 size paper and installs it on a part of the tool to facilitate access to the document. The treatment makes the work instruction document break down quickly and is not suitable to be applied to small sized tools. Therefore, it is necessary to manage the work instruction document for easy access, efficiency in the use of documents, and to improve the security of document storage. In this case, the QR Code can be used as a solution to the problem. QR Code is able to store documents and URL links in a small barcode image so that it can be use more efficient. Making a QR Code is preceded by storing documents at https://drive.google.com. The URL link at https://drive.google.com is entered into the QR-Code Studio 1.0 application and converted into a barcode. Barcodes are then printed and installed on each laboratory instrument. Scanning QR Code using a smartphone application in the form of a QR-Code Scanner. The results of the barcode scanned are the form of scripts for laboratory work instructions

Persons with mental illness (PWMI) often report negative perceptions of police treatment following receiving criminalising and heavy-handed police responses. To appropriately control officer discretion and to harness ethical, legal, and efficient police practice when encountering vulnerable and diverse individuals, police agencies across the world issue policy documents to their officers. These documents serve as a reflection regarding how police agencies aspire to manage PWMI in the community. Using a procedural justice framework, this research measures how a large police agency in Australia aspires to manage PWMI and whether the police policy document provides sufficient detail in advocating the appropriate and just police treatment of PWMI. A content analysis of the policy document revealed a lack of sufficient procedural guidelines in effectively controlling police officer discretion when encountering PWMI in the community. This article argues that without further consolidation to embed appropriate procedural guidelines into the policy document, the procedural policy gaps may have a negative effect on the experiences of PWMI when encountering the police.

Introduction and Aim: Plants form the primary source of medicinesboth in traditional and modern methods of treatment. People residing at remote and rural regions still depend on traditional herbal medicine for their primary health care needs. This study aimed to document ethnomedicinal plants used in the treatment of sprain in the Western Ghats region of Dakshina Kannada district, Karnataka state, India. Materials and Methods: An ethnobotanical study was carried out in Western Ghatsregion of Dakshina Kannada district, Karnataka during 2018-2021 to document the medicinal plants used by traditional practitioners to treat sprain. Data was collected using a semi structured questionnaire and open-ended interviews with herbal healers. Data was analyzed using simple statistical methods and few quantitative parameters. Results: A total of 40 plant species of belonging to 30 families have been documented. Some significant recorded plant species are Aloe vera, Litsea glutinosa, Cissus quadrangularis, Abrus pulchellus, Antidesma acidum, Holarrhena pubescens, Litsea wightiana and Ziziphus rugosa. Younger generations are ignoring the uses and cultural importance given to such ethno- medicinal practices. This knowledge is based on oral tradition often passed on to the next generation. Conclusion: Practitioners of this region do not have any written documents pertaining to their precious knowledge. Hence there is an urgent requirement to document and preserve the traditional knowledge inherited by tribes and ethnic communities before it is completely lost.

Reproductive tumors can impact conception, pregnancy, and birth in mammals. These impacts are well documented in humans, while data in other mammals are limited. An urgent need exists to understand the reproductive impact of these lesions in endangered species, because some endangered species have a documented high prevalence of reproductive tumors. This article documents that the prevalence of both benign and malignant neoplasia differs between African and Asian elephants, with Asian elephants more frequently diagnosed and negatively affected by both. The prevalence of these tumors across mammalian species is compared, and impact plus treatment options in human medicine are reviewed to inform decision making in elephants. Evidence suggests that reproductive tumors can negatively impact elephant conservation. Future studies that document reproductive outcomes, including the success of various treatment approaches in elephants with tumors will benefit conservation efforts.

Taking into account the current regulations in Colombia about document management and best practices, a technological solution for digitization and document management in public sector entities was designed, following the Research Applied method. The solution includes the integration of two Web systems focused on free software. The first one, called FuidXel, is a development in PHP language, original of the present project, that includes a tool next to the client for digitization, and a Web application, next to the server, for the conformation of the document, that consists on libraries of tools for the treatment of images and PDF files. The second system is an enterprise content manager for managing electronic documents, called Alfresco. FuidXel integrates with Alfresco through CMIS protocol for the sending of PDF documents, made up of the product images of the digitization and management, information and traceability metadata.

This study investigated the problem of COVID-19 vaccination program refusal. To solve this problem, it is necessary to develop regulations governing sanctions for the COVID-19 vaccination program refusal and include the sanctions on informed consent documents. An informed consent document can be the basis for health workers to give a person a COVID-19 vaccine and can be concrete evidence that a person refuses to be vaccinated. This is very important considering that the COVID-19 vaccination program is expected to be able to accelerate COVID-19 handling and prevention by achieving herd immunity. In this study, the researchers applied a socio-legal research method. This study investigated several things. The first is the problem of the COVID-19 vaccination program refusal. The second is a legal perspective on the COVID-19 vaccination program refusal. The third is a recommended regulation model to deal with the problem of COVID-19 vaccination program refusal.

The herbicide is applied normally as a foliar spray at concentrations of 100 to 500 mg·L-1. Effect of root-applied herbicide on \batchmode \documentclass[fleqn,10pt,legalpaper]{article} \usepackage{amssymb} \usepackage{amsfonts} \usepackage{amsmath} \pagestyle{empty} \begin{document} \(\mathrm{NH}_{4}^{+}\) \end{document} accumulation was studied in hydroponics. Tomato seedlings (Lycopersicon esculentum Mill.) were grown in a nitrate-based solution (Hoagland no. 1) with herbicide added to active ingredient concentrations of 0, 6, 12, 25, and 50 mg·L-1. In another treatment, an ammonium-based solution was used to assess \batchmode \documentclass[fleqn,10pt,legalpaper]{article} \usepackage{amssymb} \usepackage{amsfonts} \usepackage{amsmath} \pagestyle{empty} \begin{document} \(\mathrm{NH}_{4}^{+}\) \end{document} accumulation from an external source. Harvest and analyses of plant tissue began with first symptoms of phytotoxicity from the herbicide and proceeded at 3-day intervals until plant death with the most extreme treatments. Free \batchmode \documentclass[fleqn,10pt,legalpaper]{article} \usepackage{amssymb} \usepackage{amsfonts} \usepackage{amsmath} \pagestyle{empty} \begin{document} \(\mathrm{NH}_{4}^{+}\) \end{document} was extracted by homogenation of fresh tissues in a 1-m KCl–0.02–m CuSO4 solution. Ammonium was determined by volumetric procedures. By 6 days after treatment, \batchmode \documentclass[fleqn,10pt,legalpaper]{article} \usepackage{amssymb} \usepackage{amsfonts} \usepackage{amsmath} \pagestyle{empty} \begin{document} \(\mathrm{NH}_{4}^{+}\) \end{document} concentrations in tissues increased curvilinearly from nil to 1.5 mg N/g fresh tissue with increases in herbicide in the solution. Classic symptoms of ammonium toxicity were evident. The 50 mg·L-1 herbicide concentration was lethal at 6 days after treatment, and the 25 mg·L-1 concentration was lethal at 12 days when the experiment was terminated. Purpose The quality of consumer-oriented health information on the web has been defined and evaluated in several studies. Usually it is based on evaluation criteria identified by the researchers and, so far, there is no agreed standard for the quality indicators to use. Based on such indicators, tools have been developed to evaluate the quality of web information. The HONcode is one of such tools. The purpose of this paper is to investigate the influence of web document features on their quality, using HONcode as ground truth, with the aim of finding whether it is possible to predict the quality of a document using its characteristics. Design/methodology/approach The present work uses a set of health documents and analyzes how their characteristics (e.g. web domain, last update, type, mention of places of treatment and prevention strategies) are associated with their quality. Based on these features, statistical models are built which predict whether health-related web documents have certification-level quality. Multivariate analysis is performed, using classification to estimate the probability of a document having quality given its characteristics. This approach tells us which predictors are important. Three types of full and reduced logistic regression models are built and evaluated. The first one includes every feature, without any exclusion, the second one disregards the Utilization Review Accreditation Commission variable, due to it being a quality indicator, and the third one excludes the variables related to the HONcode principles, which might also be indicators of quality. The reduced models were built with the aim to see whether they reach similar results with a smaller number of features. Findings The prediction models have high accuracy, even without including the characteristics of Health on the Net code principles in the models. The most informative prediction model considers characteristics that can be assessed automatically (e.g. split content, type, process of revision and place of treatment). It has an accuracy of 89 percent. Originality/value This paper proposes models that automatically predict whether a document has quality or not. Some of the used features (e.g. prevention, prognosis or treatment) have not yet been explicitly considered in this context. The findings of the present study may be used by search engines to promote high-quality documents. This will improve health information retrieval and may contribute to reduce the problems caused by inaccurate information.

Abstract Introduction Onycholysis is a chronic nail disorder characterised by the progressive separation of the nail plate from the underlying nail bed. Apart from causing pain and discomfort, it affects the quality of life of an individual due to its visualised cosmetic appearance. Case Summary A 45-year-old female presented with complete loss of nail plate of the left middle finger for 1 month. At the first visit, homoeopathic medicine Tuberculinum bovinum was prescribed and the patient was followed up periodically for 4 months. A series of photographs captured at the first visit and in subsequent visits objectively document the gradual regrowth of her nail plate. Various aspects of her subjective improvement were documented by three validated outcome measurement scales. The patient herself assessed that, after 4 months of follow-up, she is ‘back to normal’ for her main complaint as well as for the overall well-being, which is documented as a +4 score in ‘Outcome in Relation to Impact on Daily Living’ (ORIDL) scale. The Dermatology Life Quality Index (DLQI) score was reduced from 20 at the first visit to 1 after 4 months of treatment. Modified Naranjo Criteria for Homeopathy (MONARCH) score recorded at the final visit (+8 in ‘-6 to +13’ scale) is suggestive of a high likelihood that the improvement in the patient can be attributed to the homoeopathic treatment provided. Conclusion This clinical case report is presented here to critically document the effect of individualised homoeopathic treatment in onycholysis within a plausible timeframe.

Abstract Only black inkjet prints were studied with regard to forensic examination of documents. We analysed twenty-one inkjet prints using surface-enhanced infrared absorption spectroscopy (SEIRAS). The present paper deals with micro-destructive document analysis based on silver nanoparticles (AgNPs) use to obtain surface enhanced ATR-FTIR (attenuated total reflection - Fourier transform infrared) spectra directly from the document. For this experiment, AgNPs in colloid form was applied. AgNPs for SEIRAS analyses were synthesised and afterwards analysed by scanning electron microscopy (SEM). The purpose of the enhancement of colloids was to improve the resolution of the absorption signal of the analysed samples from noise. In the experiment, the obtained ink ATR-FTIR spectra, with and without silver treatment, were significantly affected by the paper base.

This essay seeks to examine the position of photography in contemporary art or, more specifically, the way photography now hovers between tableau and the ‘document’ Three methodological levels are considered: first, a co-textual reading of select images by Jeff Wall and Allan Sekula in relation to their titles; secondly, an examination of their various treatment of pictorial elements, remnants of a long-standing artistic tradition; thirdly, at the level of the meta-text, the same images are confronted with a much broader contextual relationship. Here, the differences between two modes of working in contemporary photography — singular tableau and (pseudo-) documentary montage — become clear. A historical flashback traces this current photographic paragone back to 16th Century Southern Netherlandish art. Finally, the author raises a question pertaining to a fourth, practical level: that of the art market.

To the Editor.— Woody and Kiel reported a child in whom syncopal episodes occurred while swallowing.1 They treated the child by implanting a permanent pacemaker. In the article, they referred to other possible treatments such as the use of atropine to block the, presumably, vagal reflux. Unfortunately, there was no mention made of a test of this hypothesis. We reported a similar case in 19722 and were able to document blocking of the reflux with atropine.

There remains a limited understanding of the HIV prevention and treatment needs among female sex workers in many parts of the world. Systematic reviews of existing literature can help fill this gap; however, well-done systematic reviews are time-demanding and labor-intensive. Here, we propose an automatic document classification approach to a systematic review to significantly reduce the effort in reviewing documents and optimizing empiric decision making. We first describe a manual document classification procedure that is used to curate a pertinent training dataset and then propose three classifiers: a keyword-guided method, a cluster analysis-based method, and a random forest approach that utilizes a large set of feature tokens. This approach is used to identify documents studying female sex workers that contain content relevant to either HIV or experienced violence. We compare the performance of the three classifiers by cross-validation in terms of area under the curve of the receiver operating characteristic and precision and recall plot, and found random forest approach reduces the amount of manual reading for our example by 80%; in sensitivity analysis, we found that even trained with only 10% of data, the classifier can still avoid reading 75% of future documents (68% of total) while retaining 80% of relevant documents. In sum, the automated procedure of document classification presented here could improve both the precision and efficiency of systematic reviews and facilitate live reviews, where reviews are updated regularly. We expect to obtain a reasonable classifier by taking 20% of retrieved documents as training samples. The proposed classifier could also be used for more meaningfully assembling literature in other research areas and for rapid documents screening with a tight schedule, such as COVID-related work during the crisis.

Abstract Aims Comprehensive data on research undertaken in cardiovascular medicine can inform the scientific community and can support policy building. We used the publication output from 2004 to 2013 and the 2014 references to these documents, to identify research topics and trends in the field of cardiovascular disease. Methods and results Text fragments were extracted from the titles and abstracts of 478 000 publications using natural language processing. Through machine-learning algorithms, these text fragments combined to identify specific topics across all publications. A second method, which included cross-references, assigned each publication document to a specific cluster. Experts named the topics and document clusters based on various outputs from these semi-automatic methods. We identified and labelled 175 cardiovascular topics and 20 large document clusters, with concordance between the approaches. Overarching, strongly growing topics in clinical and population sciences are evidence-based guidance for treatment, research on outcomes, prognosis, and risk factors. ‘Hot’ topics include novel treatments in valve disease and in coronary artery disease, and imaging. Basic research decreases its share over time but sees substantial growth of research on stem cells and tissue engineering, as well as in translational research. Inflammation, biomarkers, metabolic syndrome, obesity, and lipids are hot topics across population, clinical and basic research, supporting integration across the cardiovascular field. Conclusion Growth in clinical and population research emphasizes improving patient outcomes through novel treatments, risk stratification, and prevention. Translation and innovation redefine basic research in cardiovascular disease. Medical need, funding and publishing policies, and scientific opportunities are potential drivers for these evolutions.

We improve a document retrieval method based on the so-called maximum entropy principle proposed by Cooper, and show how to implement this system on a Bayesian network. A Bayesian network is a probabilistic model for expressing probabilistic relations among random variables. We show advantages of a document retrieval system on a Bayesian network in comparison with Cooper's system. The original document retrieval system based on the maximum entropy principle has a drawback: a result of retrieval can not be obtained in some cases. In this paper, we resolve this drawback by fuzzification of user retrieval requests.

This document sets out the results of an investigation that had as its main objective to determine the sociocultural factors that influence the preference of the diabetic patient to empirical-traditional treatment over the pharmacological treatment of the Health Center of Las Margaritas, Chiapas, Mexico. A cross-sectional study was conducted, involving 100% of diabetic patients who are given care at the Health Center. The information was obtained by applying a Likert scale and was supplemented by open questions. Among the most relevant findings, 37% of patients were found to be in the age range of 60-69 years; 15% had diabetic glaucoma as the main complication and 57% are being treated with 850 mg metformin. Moringa is the most consumed plant, as an empirical-traditional treatment at 61%.

The efficacy of tap water iontophoresis in treating palmoplantar hyperhidrosis has been sufficiently documented and has led to its extensive use in clinical practice. In order to test the efficacy of this treatment modality in cases of dyshidrotic hand eczema, 20 patients were treated with tap water iontophoresis in addition to two-sided steroid-free topical therapy in a randomized half-side-study. A special score for dyshidrotic eczema including objective and subjective criteria was developed to document the success of the therapy. Only those sides treated with tap water iontophoresis showed significant improvement. This significant effect of iontophoresis indicates the efficacy of this treatment in cases of dyshidrotic hand eczema.

The paper gives current approaches to treating chronic thromboembolic pulmonary hypertension (CTEPH) from the document «Federal Guidelines for the Diagnosis and Treatment of CTEPH» approved at the Third Russian Congress on Pulmonary Hypertension on December 11, 2015. The guidelines had been elaborated to optimize the treatment of patients with CTEPH on the basis of an analysis of the data of the present-day registries and multicenter randomized clinical trials, national and international guidelines and consensus documents, and documents published in recent years. CTEPH is a unique form of pulmonary hypertension since it is potentially curable by surgical treatment. The paper presents indications for and contraindications to pulmonary thromboendartectomy; preparation for surgery; operating room facilities; the specific features of postoperative management and possible complications; and long-term results. In terms of therapy, in addition to non-pharmacological measures, the authors discuss maintenance and specific treatment options for CTEPH, balloon pulmonary angioplasty, and lung/heart-lung transplantation. In conclusion, they propose a management algorithm in patients with CTEPH and requirements for its problem to the center of experts.

A digital collection of patient’s health care data like diagnosis history of patient, treatment details, medical prescriptions are stored electronically. This electronic patient health records (EPHR) model provides huge volume of real time data and used for clinical research. Natural Language processing (NLP) automatically retrieve the patient’s information based on decision support system. NLP performs traditional techniques of machine learning, deep learning algorithms and focussing on word embeddings, classification and prediction, extraction, knowledge graphs, phenotyping, etc. By using NLP technique, extract the information from clinical data and analysis it provides valuable patient medical information. NLP based on clinical systems are evaluated on document level annotations which contains document of patient report, health status of patient, document section types contain past medical history of patient, summary of discharge statement, etc. similarly the semantic properties contain severity of disease in the aspects of positivity, negativity. These documents are developed and implemented on word level or sentence level. In this survey article, we summarize the recent NLP techniques which are used in EPHR applications. This survey paper focuses on prediction, classification, extraction, embedding, phenotyping, multilingually etc techniques.
