ObjectivesThe epidemiology of distal arm pain and back pain are similar. However, management differs considerably: for back pain, rest is discouraged, whereas patients with distal arm pain are commonly advised to rest and referred to physiotherapy. We hypothesised that remaining active would reduce long-term disability and that fast-track physiotherapy would be superior to physiotherapy after time on a waiting list.MethodsAdults referred to community-based physiotherapy with distal arm pain were randomised to: advice to remain active while awaiting physiotherapy (typically delivered after 6–8 weeks); advice to rest while awaiting physiotherapy, or immediate treatment. Intention-to-treat analysis determined whether the probability of recovery at 26 weeks was greater among the active advice group, compared with those advised to rest and/or among those receiving immediate versus usually timed physiotherapy.Results538 of 1663 patients invited between February 2012 and February 2014 were randomised (active=178; rest=182; immediate physiotherapy=178). 81% provided primary outcome data, and complete recovery was reported by 60 (44%), 46 (32%) and 53 (35%). Those advised to rest experienced a lower probability of recovery (OR: 0.54; 95% CI 0.32 to 0.90) versus advice to remain active. However, there was no benefit of immediate physiotherapy (0.64; 95% CI 0.39 to 1.07).ConclusionsAmong patients awaiting physiotherapy for distal arm pain, advice to remain active results in better 26-week functional outcome, compared with advice to rest. Also, immediate physiotherapy confers no additional benefit in terms of disability, compared with physiotherapy delivered after 6–8 weeks waiting time. These findings question current guidance for the management of distal arm pain.

Aims Following cast removal for nonoperatively treated distal radius fractures, rehabilitation facilitated by advice leaflet and advice video were compared to a course of face-to-face therapy. Methods Adults with an isolated, nonoperatively treated distal radius fracture were included at six weeks post-cast removal. Participants were randomized to delivery of rehabilitation interventions in one of three ways: an advice leaflet; an advice video; or face-to-face therapy session(s). The primary outcome measure was the Disabilities of the Arm, Shoulder and Hand (DASH) score at six weeks post intervention and secondary outcome measures included DASH at one year, DASH work subscale, grip strength, and range of motion at six weeks and one year. Results A total of 116 (97%) of 120 enrolled participants commenced treatment. Of those, 21 were lost to follow-up, resulting in 30 participants in the advice leaflet, 32 in the advice video, and 33 face-to-face therapy arms, respectively at six weeks of follow-up. There was no significant difference between the treatment groups in the DASH at six weeks (advice leaflet vs face-to-face therapy, p = 0.69; advice video vs face-to-face therapy, p = 0.56; advice leaflet vs advice video, p = 0.37; advice leaflet vs advice video vs face-to-face therapy, p = 0.63). At six weeks, there were no differences in any secondary outcome measures except for the DASH work subscale, where face-to-face therapy conferred benefit over advice leaflet (p = 0.01). Conclusion Following cast removal for nonoperatively treated distal radius fractures, offering an advice leaflet or advice video for rehabilitation gives equivalent patient-reported outcomes to a course of face-to-face therapy. Abstract. The forecast accuracy of distal volcanic ash clouds is important for providing valid aviation advice during volcanic ash eruption. However, because the distal part of volcanic ash plume is far from the volcano, the influence of eruption information on this part becomes rather indirect and uncertain, resulting in inaccurate volcanic ash forecasts in these distal areas. In our approach, we use real-life aircraft in situ observations, measured in the northwestern part of Germany during the 2010 Eyjafjallajökull eruption, in an ensemble-based data assimilation system combined with a volcanic ash transport model to investigate the potential improvement on the forecast accuracy with regard to the distal volcanic ash plume. We show that the error of the analyzed volcanic ash state can be significantly reduced through assimilating real-life in situ measurements. After a continuous assimilation, it is shown that the aviation advice for Germany, the Netherlands and Luxembourg can be significantly improved. We suggest that with suitable aircrafts measuring once per day across the distal volcanic ash plume, the description and prediction of volcanic ash clouds in these areas can be greatly improved.

Background: Treatment for distal hypospadias is a controversy among the experts. Some argue that it does not require any surgery if it does not interfere with the patient's urinary and sexuality. Purpose: This study aimed to determine whether patients with distal hypospadias need treated or untreated.Methods: The method used is a case study. Participants in this study is a 10-year-old male with distal hypospadias—location of the study in Purwokerto city, Banyumas district, Central Java. Participants were interviewed and physically examined by a doctor and nurse. The results of interviews and physical examinations are recorded and analyzed based on a literature review to determine the decision, treatment, or untreated. Results: A 10-year-old patient with hypospadias and the pediatric urologist do not give surgery advice, because the urethral opening is still close to the glans penis, and there are no symptoms of urinary dysfunction. Conclusion: Patients with distal hypospadias may be untreated as long as they do not interfere with urinary function

Cyberhate represents a risk to adolescents’ development and peaceful coexistence in democratic societies. Yet, not much is known about the relationship between adolescents’ ability to cope with cyberhate and their cyberhate involvement. To fill current gaps in the literature and inform the development of media education programs, the present study investigated various coping strategies in a hypothetical cyberhate scenario as correlates for being cyberhate victims, perpetrators, and both victim–perpetrators. The sample consisted of 6829 adolescents aged 12–18 years old (Mage = 14.93, SD = 1.64; girls: 50.4%, boys: 48.9%, and 0.7% did not indicate their gender) from Asia, Europe, and North America. Results showed that adolescents who endorsed distal advice or endorsed technical coping showed a lower likelihood to be victims, perpetrators, or victim–perpetrators. In contrast, if adolescents felt helpless or endorsed retaliation to cope with cyberhate, they showed higher odds of being involved in cyberhate as victims, perpetrators, or victim–perpetrators. Finally, adolescents who endorsed close support as a coping strategy showed a lower likelihood to be victim–perpetrators, and adolescents who endorsed assertive coping showed higher odds of being victims. In conclusion, the results confirm the importance of addressing adolescents’ ability to deal with cyberhate to develop more tailored prevention approaches. More specifically, such initiatives should focus on adolescents who feel helpless or feel inclined to retaliate. In addition, adolescents should be educated to practice distal advice and technical coping when experiencing cyberhate. Implications for the design and instruction of evidence-based cyberhate prevention (e.g., online educational games, virtual learning environments) will be discussed.

We report a condition we call the “plastic bag syndrome” in which pressure on the neurovascular bundles causes temporary ischaemia in the distal part of the finger together with a neuropraxia of the digital nerves. Although in most cases the discomfort or numbness is fleeting, requires no medical assistance and is readily forgotten, in some instances the symptoms are such that medical advice is sought. Since the injury can lead to permanent damage and subsequent limitation in the use of the finger, we believe that the public should be advised to take simple precautions to prevent it from happening.

The present dissertation tried to provide an answer to the need of high-quality evaluations of theoretically grounded antibullying interventions (Baldry & Farrington, 2007; Ttofi & Farrington, 2011). Literature of the last ten years shows that aggressive behaviours carried out by peers may be experienced in both face-to-face and online interactions (Tokunaga, 2010; Wingate, Minney, & Guadagno, 2013); for this reason adopting a specific focus both on bullying and cyberbullying appears to be more suitable (Menesini, 2012). The general aim of the present dissertation was to evaluate Noncadiamointrappola! program 3rd Edition by adopting an evidence-based approach in analyzing aspects related to the intervention’s efficacy. Three empirical studies are presented. They cover three main issues: 1) measurement of the cyberbullying constructs; 2) efficacy of the Noncadiamointrappola! program in reducing bullying, cyberbullying, and internalizing symptoms; 3) mediational mechanisms involved in the explanation of the efficacy of the program in reducing cybervictimization. In the first study we analyzed the psychometric properties of a revised instrument (FCBVSs; Menesini, Nocentini, & Calussi, 2011) devoted to measure cybervictimization and cyberbullying constructs. The analyses were conducted on a sample of 1142 adolescents (54.5% males) enrolled in 9th, 10th 11th grades of high schools in Tuscany. Results support a gender-invariant model based on 14 items and four factors both for cybervictimization and cyberbullying. The subscales cover four types of behaviours and describe different attacks made by peers in the cyber context (written-verbal, visual, impersonation and exclusion). The second order CFA confirmed that a “global”, second-order measure of cyberbullying and cybervictimization fits well with data. Overall the scales showed both good validity (construct, concurrent and convergent) and reliability (internal consistency and test-retest). In the second study we evaluated the effects of the Noncadiamointrappola! program in two quasi-experimental trials that involved different samples of adolescents attending the first year of Italian high schools. We found that the program in the experimental group significantly predicted a decrease in all targeted variables (victimization, bullying, cybervictimization, and 4 cyberbullying) in both quasi experimental trials. Looking at the first quasi experimental trial results (Control group, N=171; Experimental group, N=451), we found significant long-term effects of the program (six months); the outcomes did not decay over time. At the same time, we found that the program was efficacious in reducing internalizing symptoms in the experimental group through the decrease in cybervictimization above and beyond the mediational effect of the decrease in victimization. In the second independent trial (Control group, N=227; Experimental group, N=234) we tested for potential moderating effects of gender; we found that it did not have interactive effects with the efficacy of the program. In the third study, using data about the first quasi experimental trial, we analyzed the mediational mechanisms that explain the efficacy of the program in reducing cybervictimization. We found that the program predicted the increase over time in seeking support coping strategy, both on informational and instrumental aspects (distal advice) and on the more emotional way of getting help from people (close support). Noncadiamointrappola! program had significant indirect effects, through distal advice and close support, in reducing cybervictimization. Results are discussed highlighting their contributions to the literature both on evidence-based interventions and on bullying and cyberbullying phenomena. Finally, the overall strengths, limitations and implication for future studies are pointed out.

When Thomas moved to London and gained fame as a promising young poet and his romantic involvement with Johnson ended, she followed his advice and turned from writing verse to writing fiction. His critiques of her poems were serious and stringent and they convinced her that she could never become a poet. The Tom Joyner Morning Show is a quarter-century-old syndicated black morning radio show reaching more than eight million adult, largely working-class listeners. It offers progressive political talk, soul music, humor, advice, philanthropy, and celebrity gossip. But the TJMS is not just an adult “old-school music” radio show: it is an on-air organizer, fusing progressive politics and aesthetics. It focuses on specific political issues affecting and enraging African Americans. Black Radio analyzes the TJMS’s rise in the Clinton era, and its coverage of key events—9/11, Hurricane Katrina, President Obama’s elections and terms, the murders of unarmed black Americans and the rise of Black Lives Matter, and the shocking 2016 Donald Trump electoral triumph. It showcases the varied, contentious, and blackly humorous voices of anchors, guests, and audience members. This chapter analyses the international criminal law jurisprudence on fitness to stand trial. It examines the mental and physical disorders that have been asserted in international criminal proceedings for defendants accused of genocide, crimes against humanity, and war crimes. It exposes the processes by which fitness is determined and distils the legal standards that international courts have utilised and the consequences of unfitness by international courts. The chapter canvasses the introduction of more nuanced and psychiatrically informed criteria for fitness determinations. It also proposes the introduction of adequately resourced secure facilities for the confinement of persons found unfit to stand trial in order to reduce the incentives for the assertion of questionable unfitness claims, and encourages consideration of the introduction of special hearings for those found unfit in order to advance the overall aims of international criminal justice.

How we can create artificial intelligence with broad, robust common sense rather than narrow, specialized expertise. It's sometime in the not-so-distant future, and you send your fully autonomous self-driving car to the store to pick up your grocery order. The car is endowed with as much capability as an artificial intelligence agent can have, programmed to drive better than you do. But when the car encounters a traffic light stuck on red, it just sits there—indefinitely. Its obstacle-avoidance, lane-following, and route-calculation capacities are all irrelevant; it fails to act because it lacks the common sense of a human driver, who would quickly figure out what's happening and find a workaround. In Machines like Us, Ron Brachman and Hector Levesque—both leading experts in AI—consider what it would take to create machines with common sense rather than just the specialized expertise of today's AI systems. Using the stuck traffic light and other relatable examples, Brachman and Levesque offer an accessible account of how common sense might be built into a machine. They analyze common sense in humans, explain how AI over the years has focused mainly on expertise, and suggest ways to endow an AI system with both common sense and effective reasoning. Finally, they consider the critical issue of how we can trust an autonomous machine to make decisions, identifying two fundamental requirements for trustworthy autonomous AI systems: having reasons for doing what they do, and being able to accept advice. Both in the end are dependent on having common sense.

AbstractThe treatment of locoregionally advanced squamous cell carcinoma of the head and neck (LA-HNSCC) is reviewed, highlighting the milestones in systemic therapy in that setting, with focus on the role of induction chemotherapy (ICT). The road to what is now considered the standard ICT regimen, i.e. the TPF (docetaxel/cisplatin/5-FU) regimen is described, and the differences between the European and the American TPF are discussed. The article describes the respective roles of ICT for larynx preservation, for treatment intensification, its role in patients with borderline resectable or unresectable oral cavity cancer, its role as a selection tool for radiotherapy dose de-escalation in patients with oropharyngeal squamous cell cancer (OPSCC) and its potential future role in strategies aiming at synchronous oligometastatic disease.ICT has an established role for organ preservation in advanced laryngeal and hypopharyngeal cancer and the TPF regimen has been validated in that setting. This approach is presently being compared in a randomized controlled trial to concurrent chemoradiotherapy (CCRT), which in many parts of the world is considered the standard organ preservation procedure. There remains uncertainty about the benefit of the sequential approach of ICT followed by CCRT, despite the fact that ICT significantly reduces the occurrence of distant metastases. It is advised that future studies should include patients who have the highest risk to develop distant metastases, in particular patients with low neck nodes and matted nodes. Moreover, further studies in patients with HPV-associated OPSCC at risk for distant failure (T4 or N3 disease) should be considered for that also. These approaches still need to be confirmed in adequately sized randomized controlled trials. Outside clinical trials, the utility of ICT is restricted to uniquely pragmatic clinical scenarios, such as unavoidable delay in radiation or in the situation that RT is not tolerated or feasible. This can happen when there is severe pain from advanced disease or there is impending airway compromise or neurologic dysfunction that necessitates rapid initiation of treatment. In all those circumstances whether within the context of trials or outside trials, it is imperative that the present backbone of ICT, the TPF regimen, is being administered by experienced oncologists, familiar with the necessary protocols and supportive care requirements to ensure patient safety and maximize adherence throughout the treatment.Future areas of research are the role of ICT in strategies whereby ICT is combined with upfront metastases-directed treatments and the usefulness of targeted agents or immune checkpoint inhibitors in the induction setting. Studies in that direction have already started. Finally, the application of radiographic, proteomic and genomic biomarkers will get attention to further define prognostic groups and guide treatment selection with greater precision.

The chapter outlines patterns of rheumatological disease (e.g. mono-, oligo-, and polyarthropathies), symmetrical versus asymmetrical presentations of disease, and explains how to try and differentiate between inflammatory and non-inflammatory disorders. The chapter provides information on common drug treatments, including dangerous side effects from improperly prescribed methotrexate. Advice is given on history taking and detailed hand examination, with clinical signs of rheumatoid arthritis: Swan neck deformity, Boutonniere’s deformity, Z deformity of thumb, Bouchard’s nodes (proximal interphalangeal joint), Heberden’s nodes (distal interphalangeal joint, first metacarpophalangeal joint), finger ulnar deviation (metacarpophalangeal joint), and wrist radial deviation and subluxation. The most common rheumatological disorders are discussed, including rheumatoid arthritis, spondyloarthropathy, ankylosing spondylitis, psoriatic arthritis, gout, and osteoporosis (including the ‘MESSAGE’ mnemonic for osteoporosis risk factors). Abstract This chapter presents the case of a 48-year-old woman who was operated from L4–L5 left discal herniation and spinal stenosis. Despite surgery, she continues to suffer from chronic low back and neuropathic leg pain. Lumbar magnetic resonance imaging does not show any new pathological image of discal herniation. She has received acetaminophen, nonsteroidal anti-inflammatory drugs (NSAIDs), and epidural corticosteroid infiltrations without any efficacy; does not tolerate weak opioids; and is afraid of moving. Physical reconditioning and psychological advice are recommended. Opioids should not be proposed as first line, and conventional analgesics and NSAIDs are generally ineffective; lumbar epidural steroid injections are controversial. For chronic neuropathic (radicular) pain, recommendations should follow general guidelines for peripheral neuropathic pain.

The big Federal attack of May 22 began at 10 a.m. and the most striking part of it was the assault by Frank P. Blair’s division against the Stockade Redan. It was led by Hugh Ewing’s brigade, commanded by William T. Sherman’s foster brother and brother-in-law. The brigade, leading Fifteenth Corps action that day, advanced in column formation along Graveyard Road into the teeth of heavy Confederate rifle and artillery fire from one of the strongest forts on the Confederate line of earthworks. Led by 150 volunteers called the Forlorn Hope, the attack faltered. Only a handful of the members of the Forlorn Hope managed to get into the ditch and onto the outside slope of the parapet of the Stockade Redan and could go no farther. Many members of the 37th Ohio lost their will to continue after getting stuck inside a road cut only 100 yards from the redan and blocked up the line of approach of the regiments behind them. The rest of Ewing’s brigade left the exposed Graveyard Road and formed in a ravine to the south where it prepared to continue the advance later that day.

Uremic neuropathy is a distal symmetric sensorimotor polyneuropathy that presents in patients with chronic kidney disease as a consequence of severe axonal degeneration in distal nerve trunks with secondary segmental demyelination. Nerve conduction velocity tests show a reduction of motor and sensory-motor unit potential amplitudes. Regarding treatment, in patients who are not receiving dialytic therapy, conservative management of the kidney disease might have a positive impact on uremic neuropathy. Physical therapy may prevent the loss of muscle power and prevent or improve functional loss caused by uremic neuropathy. To address pain and dysesthesias, judicious use of gabapentin and pregabalin is advised to prevent adverse outcomes. In more severe pain or scenarios of concomitant dyspnea, opioids such as fentanyl and methadone could play an important role. The end goals of treatment are to relieve symptoms to a tolerable level that allows acceptable function and quality of life in these patients.

It cites Argentine Raúl Prebisch and other economists from across the region who built on his argument that the terms of trade for primary products in Argentina were bound to decline as technological advance accelerated. It also discusses the raft of policies stemming from the theory of structuralism that promote industrialisation and reduce reliance on imports in the developing world. The chapter elaborates on the fundamental ideas of the Economic Commission for Latin America in Santiago de Chile that remained influential on the dependency school, the world system theorists, and the policy-making of the New Left. It questions the assumption that classical political economy was hegemonic in Latin America from independence until the Second World War.

The re-emerging debate about human gene editing appears to be limited to whether gene editing will effectively advance the ends, goals, or values of autonomy, beneficence, nonmaleficence, and justice. As is the case for most of contemporary bioethics, what is not being considered is a substantive debate about what our goals should be in gene editing—that is, the debate is not trying to establish an account of human flourishing. The chapter posits that the public would include many other goals in their moral calculus and discusses why it is important to have this broader discussion. Finally, the chapter turns to a sociological explanation for why this debate is “thin” and not “thick,” why it only asks about whether the means of human gene editing maximize certain fixed ends instead of what our ends—our flourishing—should be.

This concluding chapter describes how the ebb and flow of diplomatic relations affected the lives of ordinary overseas Chinese in three ways. Overall, the Chinese, loathed for their perceived dominance in commerce, became easy targets for violence during times of political instability and economic downturn. First, discord in bilateral relations usually amplified antagonism toward the ethnic Chinese. Second, anti-Chinese riots persisted even when bilateral relations were cordial. Third, diplomatic frictions implicated both Indonesian citizens of Chinese descent and Chinese nationals living in Indonesia. Notwithstanding the changing status of diplomatic relations, there had always been pribumi groups that accused all ethnic Chinese of being pawns of a foreign power irrespective of their citizenship status and ideological inclination. Stemming from long-standing social, economic, and political circumstances in Indonesia, this prejudice was also influenced by the way the Chinese Nationalist and Communist governments conducted diplomacy in Indonesia. Both governments instrumentalized the overseas Chinese to advance their respective foreign policy objectives.

To advise is to provide the student the opportunity to explore, discover and to clarify the best ways to live his own life, to have a happy existence. Initially, the scope of the councilors has been identified as a reduced one compared to psychotherapy, then the purpose of the two activities to dissociate from a theoretical point of view. Between the two terms there is a significant difference: counseling is a proactive action, while psychotherapy is a postfactum intervention (remedial action, the therapy). Guidance and counselling means all actions performed by optional advisory and training methods, the general and special conditions, subordinated to, from the point of view of the Content, Size of process-orientated technology/education, and from the perspective of the activity of psycho-pedagogical assistance methodological and social teachers, students and parents, designed at the level of the education system, with a view to the correct academic and career options. Guidance and counselling means all actions performed by optional advisory and training methods, the general and special conditions, subordinated to, from the point of view of the content, size of process-orientated technology/education, and from the perspective of the activity of assistance psycho-pedagogical methodological and social teachers, students and parents, designed at the level of the education system, with a view to the correct academic and career options. In Romania, there is still a well-defined ethical framework at the legislative level, Advisor profession ‘borrowing’ a number of ethical rules from psychologists, sociologists, pedagogues. We expect that in the not too distant future, together with the adjustment of the legal framework concerning the advice, to be completed and such professional code, as a sign that the profession has moved beyond the Advisor to the time of interim and won a tight space defined in the professional Romanian.

Acquiring the hot spot stress (HSS) in dangerous points of the structure (normally the welding seams of the structure) is the foundation of the offshore platform fatigue strength design. Because of the fact that it is very difficult and unpractical to install sensors in the welding seams, the most widely used method of acquiring the HSS currently is extrapolation, a method that obtains the HSS by extrapolating from stress of distant points. In order to figure out the feasibility of the method of extrapolation as well as the best ways to implement extrapolation in real practice, it is important to carry out numerical calculation of welded tubular joints in advance. To study the tubular joints, the finite element method (FEM) is the most widely adopted approach. In previous studies, shell elements are utilized to simulate tubular joints instead of solid elements concerning the difficulty. However, shell element simulation merely simulates the mid-surfaces of the tube walls so that shear stress in the direction of tube thicknesses are ignored, making the calculation of the stress concentration factor (SCF) inaccurate. In this study, the recommended ways of simulating the tubular Y- and T-joints are displayed and then the comparison of calculation results between different weld type, different mesh situations as well as different elements are given in order to bring up advice on numerical calculation for the study of extrapolation. The results of numerical calculation show that the extrapolation results are more accurate when we only consider the change of parameter α (chord length-to-chord radius ratio) and also, it is suggested to do the fitting in the area of 2t-4t away from the welding seam. These suggestions are verified by applying the method of extrapolation to a real jacket platform, which turns out that the fitting degree of extrapolation is very pleasant.

Over the past decade, natural orifice transluminal endoscopic surgery (NOTES) has developed out of a merger of endoscopy and surgery [1]. NOTES offers the advantages of avoiding external incisions and scars, reducing pain, and shortening recovery time by using natural body orifices as the primary portal of entry for surgeries [2]. The NOTES platform consists of a flexible, hollow body — enabling travel in the interior of the human body — and the distal end (head), the mechanical structure of which is based off of the snake bone. After the distal end passes through a natural orifice, through a transluminal opening of the stomach, vagina, bladder, or colon, and reaches the target working place in the peritoneal cavity, several therapeutic and imaging tools can be passed through the hollow conduit of the NOTES’ body for surgeries [3]. The traditional snake bone design presents two major problems. First, the movement is constrained to two bending degrees-of-freedom (DOF). A need to reorient the tool then often requires the entire body to be rotated by the physician, an unwieldly manipulation that both hinders convenience and results in imprecise control. Second, the traditional fabrication process is tedious and therefore lends to higher manufacturing costs; the bending joints must be first individually machined then assembled together piece-by-piece using rotation pins. We propose a novel design for the snake bone that introduces an additional DOF via rotation and is simple and cost-effective to machine. The revised snake bone design features rotation segments controlled by wires that a physician can readily manipulate for increased control and convenience. Further, because surgical tools that pass through the NOTES body conduit are also installed on snake bone structures, the introduction of rotation to the snake bone design increases each tool’s mobility and manipulation. This advance therefore presents the potential to decrease both the number of required tools and the overall diameter of the NOTES body. Finally, the body is machined as a single element and therefore minimizes the work of assembly.

Many established nuclear power programmes have learned to their dismay that waste management and disposal are not tasks that can be postponed at will if public and political acceptance is a prerequisite for progress. In fact, some programmes that recognised this back in the 1970s and 1980s moved into leading positions in repository development. This happened, for example, in Sweden and Switzerland where already in the 1970s Laws were passed specifying that safe disposal must be demonstarted before new nuclear plants could opersate. In recent years, it has become recognised that, in order to ensure that the radioactive wastes in any country are managed safely, it is necessary to have an established legislative and regulatory framework and also to create the necessary organizations for implementation and for oversight of waste management operations and facility development. Guidance on these issues is given in the Joint Convention and a number of other IAEA documents. The IAEA, and also the EC, have in addition published key overarching advisory documents for new nuclear programmes. These are useful for strategic planning but, when it comes to actual implementation projects, the advice tends to imply that all nuclear programmes, however large or small, should be pressing ahead urgently towards early operation of geological repositories. In practice, however, in small programmes there are neither economic nor technical drivers for early implementation of deep geological repositories. Constructing simpler facilities for the disposal of the larger volume of low-level wstes has higher priority. Nevertheless, in all countries political decisions have to be taken and policies set in place to ensure that geological disposal will implemented without unjustified delay. This paper distils out a set of key messages for new programmes. Amongst the most critical are the following. Even if disposal is far off, planning and organization should begin at the initiation of the programme; this can help with technical and economic optimization and (importantly) also with public and political acceptance. Important lessons can be learned from advanced programmes — but these must be adapted to allow for the different boundary conditions of new and small programmes. The key differences relate to the timescales involved, and the resources available. There is a range of waste management and waste disposal options open to new programmes. It is not necessary to choose definitive solutions at the outset; options can be kept open, but a minimum level of engagement is required for all open options.

Abstract The world’s technology is developing very rapidly. To anticipate the course and results of such development is a task that is very crucial for the success of many technological undertakings and expansions. Engineering design is the branch of engineering that should predict the results of that rapid development. It should equip society with the tools for directing and controlling that development. It is a complex task that faces big challenges. The main challenge comes from society advancement and from the technology development itself. If the directing and controlling are done right the development would bring many benefits to humanity and would make human life easier and more comfortable. Doing it right however requires increased knowledge of the new features of technology and more skills in its application. In the difficult pandemic situation that knowledge and skills should be even greater because the outbreak of the disease creates additional traps and dangers. These conditions have to be taken under consideration and accepted as normal. The role of engineering design is to predict what harmful elements would be coming from both technological and social sources. The real goal however would be to exceed the expectations and not only neutralize them but change them from harmful into neutral, and then from neutral into friendly and helpful. Such actions follows recommendations of BTIPS (Brief Theory of Inventive Problem Solving) and is outlined in the BTIPS’s module “Prediction”. At the same time the developing civilization brings dangers for humans that were unknown before. These are bacterial and viruses’ attacks that limit personal relations between humans, requires new ways and new elements of communications, especially in internet contacts and in distant learning procedures. The contents of these components should be accurately predicted, well-orchestrated, well designed and precisely described. Recommendations for introducing BTIPS as a tool of engineering education in new situation should be carefully proposed and illustration examples, using new communication tools, should be developed. These should be applied in engineering theoretical courses and in practical applications during the senior design course of study and in industrial practice. This should be precise, clearly anticipating difficulties, pointing possible errors and ways of avoiding them. Teaching examples of problem solving and personal ways of communications between individual students, between groups of students, as well as between students and instructors should be further discussed. The examples of design ideas and problem solutions generated by students in design courses that were described in previous works of the author and his co-workers [1] should be related to pandemic situation. To define and formulate rules of teaching BTIPS in the pandemic situation is the necessity of our times. On every step of our lives we face the challenge of preventing harms and destruction that can be done by the contemporary surrounding world. The preventing actions can be designed by following rules of BTIPS and by apply approach recommended in its modules. The proposal of utilizing BTIPS application examples using the internet as a tool of expression is described in this paper. All of these are pointed out and some recommendations and examples are called. Adding description of corrections to the engineering curriculum is necessary in the new situation. It is an intention of the author to demonstrate a fragment of practical distant lecturing by internet during the IMECE 2020 internet sessions using the internet network and distant support from UConn computer Laboratory in Storrs, CT. Some example solutions of the idea generation are quoted in this paper. The comments coming from author’s teaching experience will be given during the presentation and practical advices for students and instructors will be passed to the audience. This paper is a companion to IMECE 2017-70438 [1]. Some original examples given in the paper 79418 are recommended for following and will be run by internet in pandemic situation of IMECE 2020.

Some years ago, in 1975, I presented a paper and a slide show at an earlier sailing yacht symposium in Annapolis. The subject was a four-year, 28,000 mile cruise I had made in the years 1965 - 1968 most of the way around the world: Hawaii and the South Pacific, New Zealand, Australia's Barrier Reef, the Indian Ocean, the Red Sea, the Mediterranean, including the Greek Islands, an Atlantic crossing to Barbados from the Canary Islands, the Caribbean, and home to the Chesapeake. The paper I wrote then was entitled "Extended Cruising: An Overview" and contained sketches and data from my logs. It was same 55 pages long and talked about many facets of cruising from my vantage point, primarily as seen from the decks of Mamari, the 28 foot ketch I had bought in New Zealand. Lest Mamari 's size appear too small, which perhaps would make me seen heroic, recognize that in displacement and accomodations Mamari was the equivalent of a 33 foot boat. To dispel one other misconception, be advised that I normally sailed with a crew of two, sometimes more, and only sailed two legs single-handed, of about 500 miles each, one from Tonga to Fiji in the Pacific, the other in the Gulf of Suez and from Port Said to the Greek Islands. The 1975 paper reflected my background as a naval architect, combined with my experience as a sailor. I told of things I learned from others. I analyzed log data, presented photographs, drawings and tables, and wrote a series of "yarns" such as sailors spin about their travels. The paper is touched with a flavor of the sea, a flavor of talk over run or coffee in a snug anchorage or on a shared night watch. That 1975 paper makes good reading, and much of the information is still valid. It could be reprinted and if there is enough interest l will do so (contact me). This present paper is a brief look at my experiences on a series of sailing trips, but in particular a one year voyage in a 37 foot yawl from Turkey to the Chesapeake via the West Indies in 1980-81. The paper answers the question posed at the 1975 symposium, would I do the trip again? Then, I thought so, but could not be sure, now my reply is, "of course."

Purpose: To explore the reasons of hysterectomy and indications of post-hysterectomy radiotherapy in carcinoma cervix cases. Methods: From January 2013 to May 2015, medical records of 64 cases of carcinoma cervix (post-hysterectomy) who were referred for radiotherapy to our hospital were analyzed retrospectively. Results: Medical records of 64 cases were reviewed. The median age was 47 years. In 45% of females hysterectomy was done in towns, but in majority of cases (55%) hysterectomy was done in different cities of Rajasthan. Simple hysterectomy was done in 31 of (48%) cases. Wertheim’s hysterectomy was done in remaining 33 cases (52%). 15 cases (23%) were treated by IMRT technique, while remaining 87% cases were treated by 3DCRT technique to dose of 50 Gy in 25 fractions followed by CVS brachytherapy. All cases also received concurrent chemotherapy. Reason for hysterectomy was analyzed. In 32 (50%) cases, biopsy from gross lesion at cervix or PAP smear test was not done before surgery. In 32 cases (50%) understaging of the tumor or inadequate staging before hysterectomy was performed. Histopathology report analysis revealed that in 9 cases (14%) primary tumor size was less than 4 cm, in 27 cases (64%) there was no comment on pT size, in 22% cases primary tumor was larger than 4 cm. Surprisingly in one case the pT size was 7 cm. LVSI was not seen in 18 cases (28%), positive in 20 case (31%) and with no comment in 26 cases. More than 50% of stroma thickness was involved in 54 cases (84%), and in remaining 10 cases there was no comment on stroma invasion. In 33 cases (52%) pelvic lymphadenectomy was done, in 48% cases lymph nodes were not addressed in surgery. In 36 cases (56%) pelvic lymph node metastasis was seen either in preoperative imaging (USG/CT scan) or in histopathology. Median follow-up duration was 6 months. Locoregional failure was seen in 10 cases (16%), 6 cases (9%) also developed distant metastasis. Conclusion: Failure to perform biopsy from gross lesion at or under staging/inadequate staging before surgery was the main reasons for inappropriate hysterectomy for carcinoma cervix. Inappropriate hysterectomy followed by chemo-radiotherapy resulted in poor tumor control rate as in our study, 1 out of every 4 patients failed loco-regionally with median follow up of 6 months. Strict adherence to guidelines for cervical cancer diagnosis and treatment is advised to prevent inappropriate hysterectomy.

In 2019, the Gambling Commission announced a National Strategy to Reduce Gambling Harms. Underlying the strategy is the Framework of Harms, outlined in Measuring gambling-related harms: A framework for action. "The Framework" adopts a public health approach to address gambling-related harm in Great Britain across multiple levels of measurement. It comprises three primary factors and nine related subfactors. To advance the National Strategy, all componentsneed to be supported by a strong evidence base. This report examines existing research expertise relevant to the Framework amongacademics based in the UK. The aim is to understand the extent to which the Framework factors and subfactors have been studied in order to identify gaps in expertise and provide evidence for decision making thatisrelevant to gambling harms research priorities. A social network analysis identified coauthor networks and alignment of research output with the Framework. The search strategy was limited to peer-reviewed items and covered the 12-year period from 2008 to 2019. Articles were selected using a Web of Science search. Of the 1417 records identified in the search, the dataset was refined to include only those articles that could be assigned to at least one Framework factor (n = 279). The primary factors and subfactors are: Resources:Work and Employment, Money and Debt, Crime;Relationships:Partners, Families and Friends, Community; and Health:Physical Health, Psychological Distress, and Mental Health. We used Gephi software to create visualisations reflecting degree centrality (number of coauthor networks) so that each factor and subfactor could be assessed for the density of research expertise and patterns of collaboration among coauthors. The findings show considerable variation by framework factor in the number of authors and collaborations, suggesting a need to develop additional research capacity to address under-researched areas. The Health factor subcategory of Mental Health comprised almost three-quarters of all citations, with the Resources factor subcategory of Money and Debt a distant second at 12% of all articles. The Relationships factor, comprised of two subfactors, accounted for less than 10%of total articles. Network density varied too. Although there were few collaborative networks in subfactors such as Community or Work and Employment, all Health subfactors showed strong levels of collaboration. Further, some subfactors with a limited number of researchers such as Partners, Families, and Friends and Money and debt had several active collaborations. Some researchers’ had publications that spanned multiple Framework factors. These multiple-factor researchers usually had a wide range of coauthors when compared to those who specialised (with the exception of Mental Health).Others’ collaborations spanned subfactors within a factor area. This was especially notable forHealth. The visualisations suggest that gambling harms research expertise in the UK has considerable room to grow in order to supporta more comprehensive, locally contextualised evidence base for the Framework. To do so, priority harms and funding opportunities will need further consideration. This will require multi-sector and multidisciplinary collaboration consistent with the public health approach underlying the Framework. Future research related to the present analysis will explore the geographic distribution of research activity within the UK, and research collaborations with harms experts internationally.
