Chapter 2 introduces an empirical classification of evidentials across languages. This chapter discusses semantic diagnostics from the literature, illustrating them with representative languages, and shows how Cheyenne fits into this classification. While there are several dimensions of variation, there are also striking crosslinguistic generalizations, with evidentials sharing a core set of properties. This points to the need for a unified analysis that can capture these shared properties, treating evidentials crosslinguistically as a natural semantic class, while being fine‐grained enough to account for the variation. Where evidentials differ across languages, Cheyenne patterns with certain languages on some diagnostics, but with different languages on others, providing further support for a unified theory.

This article outlines the current situation with respect to ‘endogeneity and structural equation estimation’. It begins by exploring why econometrics, and particularly the treatment of endogeneity, proved so valuable for political scientists. The most prominent methods used to deal with endogeneity are explained. The article also includes a critique of the critical conditions needed to support the use and interpretation of this method and discusses frequently used diagnostics developed to assess the severity of the problems. The estimation method and diagnostics are shown with a running example with data from a previous study of US congressional politics. In many instances, empirical evidence will continue to be the classic observational study, replete with endogeneity problems. Rather than forgoing analyses of these data and questions because of the reservations about a particular method, it is better to use and to improve those methods in the context of these studies and data.

This chapter provides a discussion of the American Psychiatric Association’s classification of mental disorders (DSM-I through DSM-5), with a particular emphasis on mood disorders and their classification and diagnosis. It begins with the rationale for having an official, authoritative diagnostic manual and then traces the history of the development of the first edition through the fourth edition (DSM-IV-TR, 2000). The authors then discuss fundamental issues concerning the fifth edition (DSM-5, 2013), including the definition of mental disorder, the empirical support for proposed revisions, the shift toward a dimensional model of classification, and the shift toward a neurobiologically-based classification.

Dr. Walter Ling begins this chapter by discussing the distinction between the process of detoxification and role relapse prevention. He reviews the post acute withdrawal period, its neurobiological underpinnings, and discusses the role of stress hormones, neuro-circuity, and the importance of relapse prevention. The existential question as to why people take drugs is posed. Complex aspects of the neurobiology of post-acute withdrawal and subsequent treatment ramifications are reviewed. Drug-specific neurochemical changes are explained, including treatment to reduce cortisol and noradrenergic responses pharmacologically. Then the integrative approach to relapse prevention is reviewed by Dr. Munoz. The concept of “relapse prevention plan” is introduced and the roles of exercise, 12-step social support, and the use of novel neuro-diagnostics including Event Related Potentials (ERP) to aid in personalized risk stratification of potential future relapse. The importance and evidence of multidisciplinary approaches and social 12-step-type support are also reviewed and close out the neuroscience of the first 2 sections.

This chapter discusses the use of anesthetics in a variety of diagnostic and therapeutic radiologic procedures. Diagnostic procedures are anatomic or functional, minimally or noninvasive, and seldom require anesthesia support; anesthesia is typically only requested for these procedures because of a patient’s physiological or psychological needs. Recognizing and addressing a patient’s comorbidities and other concerns are similar to care given in the surgical setting. The imaging environment and procedures pose additional requirements and constraints, which are unique and require specific solutions; there is frequently no single best anesthetic technique for a given procedure. Radiologists serve the critical function of deciding whether to consult the anesthesiologist or perform a procedure alone. If the decision is to proceed with moderate sedation (administered by a nonanesthesia professional), the importance of vigilant clinical monitoring cannot be understated. In all cases, the patient deserves care that is consistent with the parameters, guidelines, and standards established by the various accrediting agencies and professional societies

Establishing a positive therapeutic alliance during the initial psychiatric interview allows the clinician to collect the necessary diagnostic information and can have a significant impact on a patient’s decision to follow up with treatment recommendations once the diagnosis of psychogenic nonepileptic seizures (PNES) is determined. When evaluating a patient with suspected PNES in an out- or inpatient setting, there are a variety of clinician behaviors that can support or obstruct the establishment of a positive therapeutic alliance. Similarly, a number of typical patient characteristics in PNES can affect the psychiatric assessment. In this chapter, these characteristics and behaviors are discussed, a clinician checklist is provided, and dialogue boxes illustrate a few common patient–clinician interactions, hypothetical challenges, and clinician responses. Distinctive challenges, including interaction with specific PNES subtypes (i.e., developmentally delayed, malingering, or hostile patients) and patients who are effectively incapacitated by high event frequency and systemic barriers, are presented.
