The new neo-clerodane dimer (DACD) likely formed by a [4+2] photo-cycloaddition between two molecules of ent-15,16-epoxy-19-hydroxy-1,3,13(16),14-clerodatetraen-18-oic acid (DAC), both isolated from Baccharis flabellata Hook et Arn. var. flabellata (Asteraceae), were tested in combination with terbinafine (Terb) against Trichophyton rubrum. The interactions were assessed with different methodologies such as 3D CombiTool software, Loewe Combination Index (CI) and isobolograms. Results showed that the monomer DAC as well as its dimer DACD act synergistically with Terb against T. rubrum. DAC/Terb and DACD/Terb showed eight and twelve synergistic mixtures respectively in the CombiTool graphics thus suggesting that DACD/Terb was more synergistic with Terb than DAC. These results were corroborated by the CI values and the isobolograms of the detected combinations. DACD/Terb showed much lower CIs (0.34-0.47) than DAC/Terb (0.57-1.15) at the different inhibition percentages tested. Concomitantly, DACD/Terb isobolograms were more concave than for DAC/Terb at the different inhibition percentages. This is the first report showing antifungal synergistic interactions of neo-clerodanes dimers and add new evidences that neo-clerodanes monomers interact synergistically with Terb against T. rubrum.

Abstract MDS is a disease of the elderly marked by dysplasia of hematopoetic cell lines resulting in cytopenias and AML progression. There is currently no treatment approved to alter the natural history of the disease. Aberrant methylation associated with cancer is a potential target for pharmacologic therapy, and DacogenTM (decitabine) for injection (SuperGen, Inc.), a cytosine analogue, indirectly depletes methylcytosine after incorporation into DNA with subsequent inactivation of DNA methyltransferases. We report the results of a Phase III trial of decitabine (DAC) vs. supportive care (Supp.Care) in adult MDS patients with IPSS Intermediate (Int)-1 (31%), Int-2 (44%) and high risk disease (26%). Secondary MDS (14%) and previously treated (27%) MDS patients were not excluded. Bone marrows were assessed by a blinded, independent pathologist. 170 patients (accrued from July 2001 through April 2003 at 23 centers) were randomized 1:1 to either Supp.Care or DAC (a 3 hr infusion of 15mg/m2/hr every 8 hrs on 3 consecutive days every 6 wks for up to 10 cycles. The groups were comparable for numerous risk factors, including time from diagnosis (median 29 weeks for DAC and 35 weeks for Supp.Care). Kaplan Meier (KM) curves for time to AML progression or death showed early and clinically meaningful separation (consistent with clinical benefit) in favor of DAC in all patients (intent to treat; ITT), Int-2/High Risk, and High Risk patients. Using the Cox proportional hazards model for the ITT population, the probability of progression to AML or death was 1.68-fold greater for Sup.Care than for DAC (p=0.023). There were no treatment related deaths. As expected, grade 3–4 toxicity (including hematologic toxicity, febrile neutropenia) occurred in more DAC patients than in Supp.Care patients. Most patients tolerated treatment well. DAC appears a promising therapy for MDS with manageable toxicity. Final results of independently reviewed response rates and clinical benefit (transfusion independence; Quality of Life) will be presented.

Abstract OBJECTIVE AND IMPORTANCE We describe a case of brainstem cavernous angioma in which the neural tracts were evaluated before surgery by three-dimensional anisotropy contrast (3-DAC) magnetic resonance imaging. CLINICAL PRESENTATION A 64-year-old man presented with a cavernous angioma located intrinsically in the brainstem and manifesting as gait ataxia. 3-DAC imaging demonstrated that the lesion was located outside the left inferior cerebellar peduncle and inside the middle cerebellar peduncle. INTERVENTION The intact brain surface was incised, and the lesion was removed successfully on the basis of the preoperative 3-DAC images. The patient exhibited temporary exacerbation of his gait ataxia, but the symptom improved 3 months after surgery. Postoperative 3-DAC imaging demonstrated resection of the lesion and preservation of the left inferior and middle cerebellar peduncles. CONCLUSION 3-DAC imaging may provide essential information about the neural tracts for the planning of brainstem surgery.

AbstractThermally activated delayed fluorescence (TADF) emitters are third-generation electroluminescent materials that realize highly efficient organic light-emitting diodes (OLEDs) without using rare metals. Here, after briefly reviewing the principles of TADF and its use in OLEDs, we report a sky-blue TADF emitter, 9-(4-(benzo[d]thiazol-2-yl)phenyl)-N3,N3,N6,N6-tetraphenyl-9H-carbazole-3,6-diamine (DAC-BTZ). DAC-BTZ is a purely organic donor–acceptor-type molecule with a small energy difference between its lowest excited singlet state and lowest triplet state of 0.18–0.22 eV according to fluorescence and phosphorescence spectra of a DAC-BTZ-doped film. In addition, the doped film exhibits a high photoluminescence quantum yield of 0.82. Time-resolved photoluminescence measurements of the doped film confirm that DAC-BTZ emits TADF. An OLED containing DAC-BTZ as an emitter exhibits a maximum external quantum efficiency (EQE) of 10.3%, which exceeds those obtained with conventional fluorescent emitters (5–7.5%). TADF from DAC-BTZ makes a large contribution to the high EQE of its OLED.

Introduction: Antrodia cinnamomea (AC) is an extremely rare medicinal fungus native to forested regions of Taiwan. It possesses numerous biological activities, especially anti-tumor effects shown in various in vitro cancer cells and in vivo animal models. However, there are few clinical reports about AC as a treatment for cancer patients. This report attempts to demonstrate the therapeutic effect of dish-cultured AC (DAC) on a small cell lung cancer (SCLC) patient taken orally for an extended duration. Patient concerns: An 88-year-old male with a history of diabetes mellitus and hypertension visited the outpatient department with the symptoms of dyspnea and a cough for two weeks. After a diagnosis of SCLC, the patient declined both chemotherapy and radiotherapy because of the side effects and only accepted supportive care without additional therapy. Diagnosis: Limited-stage SCLC (T4N2M1a, stage IV) after the chest radiograph, computed tomography-guided biopsy, and pathological diagnosis. Interventions: The patient was prescribed DAC with an increasing dosage, from 5 g/d up to 10 g/d DAC, for six months, without radiation or chemotherapy treatment. Outcomes: DAC caused the tumor to shrink substantially. Surprisingly, the patient survived for 32 months without relapse after six months of DAC treatment. Laboratory examinations indicated that the patient’s health had improved significantly, reverting to near normal levels. Notably, he had a good quality of life with a high Barthel index score. Unfortunately, this patient died of septic shock caused by acute cholangitis. Conclusion: DAC may exert an anti-cancer effect, which can lead to tumor regression. This is supposed to be achieved by the combined DAC’s immunomodulatory, anti-angiogenic, anti-metastatic, anti-proliferative, and pro-apoptotic effects mediated through multiple signaling pathways. We propose that DAC can be used as a complementary medicine to prolong the life expectancy and improve the life quality of SCLC patients.

Abstract Abstract 3782 Poster Board III-718 Low doses (5-20 mg/m2/day) of the azanucleoside decitabine (DAC) have been shown to be active in AML likely through DNA hypomethylation and re-expression of otherwise epigenetically silenced, hypermethylated genes, including tumor suppressors. Following cellular uptake, DAC rapidly undergoes phosphorylation to its active metabolite, DAC-triphosphate (DAC-TP), which is incorporated into the newly synthesized DNA, covalent binds to and sequesters DNA methyltransferases that mediate aberrant enzymatic-DNA hypermethylation. Measurement of DAC-TP remains critically important in understanding and monitoring the effect of drug treatment in order to design more effective treatment schedules. In contrast to the well-established methods for measurement of triphosphates of other nucleoside analogs (e.g. cytarabine), development of a reliable method of intracellular DAC-TP quantification has proven to be rather challenging and as yet no such method for DAC-TP has been reported. Here, we developed a non-radioactive, sensitive, and specific HPLC-MS/MS method to quantify intracellular DAC-TP that is readily applicable to intracellular quantification of DAC-TP in marrow mononuclear cells (MNCs) and peripheral blood mononuclear cells (PBMCs) from DAC-treated patients. This method uses an internal standard (IS) of 2-chloroadenosine-5'-triphosphate (ClATP), against which DAC-TP is measured; we tested the method first in vitro in DAC treated myeloid leukemia K562 cells. K562 cells were treated with 2 μM DAC for 1, 4, and 24 hours and DAC-TP, deoxynucleotides (dNTPs), and the spiked ClATP were extracted with methanol precipitation and separated through a gradient elution on a Supelcogel ODP-50 column. An ion trap mass spectrometer in a negative ion electro-spray mode was then used for detection. The multiple reaction monitor at m/z 467.1→369.0 allowed specific detection and separation of DAC-TP from other dNTPs and quantification against a known, spiked amount of ClATP. Linearity of the assay was demonstrated between 10 nM and 10 μM in K562 cell lysate with 50 nM being the lower limit of quantification (LLOQ). The within-day coefficients of variation (CVs) were found to be between 14.2-19.9% at LLOQ and 4.7-7.0% for higher concentrations. The within-day accuracy values were 83.7-115.6% for DAC-TP. Following 0.2 μM DAC treatment in K562 cells for 4 hr, the accumulation of DAC-TP peaked (∼2 pmol/106 cells) at 4 hr and was detectable at 24 hr. Next, the assay was then successfully applied to patient samples. We quantified DAC-TP in marrow MNCs collected within 3 hours from patients treated DAC at 20mg/m2 intravenously over 1 hour; we were able to detect for the first time in in vivo treated cells DAC-TP levels (0.08∼0.7 pmol/106 cells). Additionally, we were able to measure DAC-TP levels in PBMC in selected patients (0.13-1.92 pmol/106 cells). Correlation of intracellular DAC-TP with DNA hypomethylation and gene re-expression is ongoing. In conclusion, we report the first highly sensitive and specific HPLC-MS/MS analytic method for in vivo quantification of DAC-TP. This method can readily be applied for correlative studies of intracellular DAC-TP with DAC plasma pharmacokinetics, DAC-induced DNA hypomethylation/gene re-expression, and clinical outcome, and utilized for future trials based on biochemical modulation of DAC administered alone or in combination with other drugs. Abstract Background The standard first-line therapy for higher-risk myelodysplastic syndromes (MDS) are the hypomethylating agents (HMA). While clinical characteristics, molecular markers, and karyotype can contribute to predicting prognosis in MDS, these parameters have not identified differential response rates between the HMAs decitabine (DAC) and azacitidine (AZA). Gender differences have been associated with varied outcomes in multiple cancers, and are thought to be related to differences in disease biology, treatment response, or adherence to therapy. Relevant to MDS, gender can effect expression of cytidine deaminase, which inactivates the HMAs. Male have increased levels compared to females. The impact of gender on response to HMAs is not established. Methods A dataset from the MDS CRC database was analyzed. Patients (pts) were diagnosed with higher-risk MDS (International Prognostic Scoring System (IPSS) Int-2 or High) and per 2008 WHO criteria and were treated with AZA or DAC as first-line therapy. Response was assessed per International Working Group (IWG) criteria (2006). The IPSS and its revision (IPSS-R) were calculated at presentation. Missing data were multiply imputed 100 times using the mice approach. Differences among variables were evaluated by the chi-square test and Mann-Whitney U test for categorical and continuous variables, respectively, with continuous variables summarized by median and range. Overall survival (OS) was calculated from presentation date to date of death or last follow up. Propensity scores were calculated for DAC vs AZA using all variables available pre-treatment. Propensity weighted Cox proportional analysis and log-rank tests were used to model and test OS. Results Of 625 higher-risk MDS pts, 33.7% were women (Table 1). The median OS for the cohort was 16.9 months (95% CI 15.6-18.2); 17.8 months for men (95% CI 16.4-19.2) and 15.0 months for women (95% CI 12.1-18.6). Approximately one third of pts received DAC as front line HMA in both gender groups. While most variables had similar distributions between DAC and AZA within genders, two variables showed differences between HMA treatments in both genders: bone marrow blast percentage at diagnosis and IPSS-R cytogenetic category, with blast percentage consistently higher in the DAC treated set. In addition, DAC-treated females were on treatment longer than AZA-treated females There was no difference in median OS across gender (p=0.33). DAC-treated pts had marginally better OS than AZA pts (p=0.043), (median OS of 18.7 months vs 16.3 months), but the difference varied strongly by gender (Figure 1). Female DAC pts had much better OS than female AZA pts (p=0.0014), with a median OS of 21.2 months (95% CI 16.1-27.9) versus 13.1 months (95% CI 10.7-15.9). Males showed no significant difference (p=0.59), with a median OS of 18.3 months (95% CI 14.9-22.2) compared to 17.6 years (95% CI 16.0-19.5). Female DAC survival improvement remained significant in a Cox PH analysis after adjusting for cytogenetic category and bone marrow blast % at diagnosis. (Figure 1) Conclusion Women with higher-risk MDS may live longer when treated with DAC than with AZA. While factoring in gender as a variable in therapeutic choice between the HMAs remains premature, future prospective investigations of both drugs in men and women are warranted. Table 1. Abstract Background: CD19/CD22 bispecific targeted chimeric antigen receptor T cell (CAR-T) therapy has achieved impressive progress in patients with relapsed/refractory B cell acute lymphoblastic leukemia (R/R B-ALL), with high rates of complete remission (CR). However, T cell exhaustion caused by de novo DNA methylation restricts the capacity of CAR-T. Decitabine (DAC), a DNA methyltransferase inhibitor, has been demonstrated to reverse exhaustion-associated DNA-methylation programs, promote the rejuvenation of CAR-T cells and enhance anti-leukemic effect of CAR-T cells in vitro. To date, there are limited reports about the use of DAC as a part of lymphodepletion therapy before CAR-T cell therapy. Here, we report efficacy and safety of DAC in combination with fludarabine and cyclophosphamide (FC) regimen followed by CD19/CD22 CAR-T cell therapy for patients with R/R B-ALL. Method:We conducted a phase 1/2 clinical trial to investigate the efficacy and safety of CD19/CD22 CAR-T in the treatment of R/R B-ALL (NCT03614858). Fourteen patients were treated with DAC (total dose 100mg/m 2 in 3 days) followed by FC regimen (fludarabine 30mg/m 2 × 3d and cyclophosphamide 300mg/m 2 × 3d) (DAC group), while twelve patients received FC regimen prior to CAR-T cell infusion (CON group). A total of 1 or 2 × 10 7 CAR-T cells/kg were infused at dose escalation. Results: Baseline characteristics of patients in both groups had no significant differences except previous hematopoietic stem cell transplantation (HSCT). There were more patients with relapse after HSCT in DAC group (42.9% versus 0%, P=0.017). All patients did not achieve remission before lymphodepletion. The day 28 CR rates were 100% in DAC group and 91.7% in CON group. Furthermore, minimal residual disease (MRD) negative CR rates were 71.4% and 58.3%, respectively (P = 0.683). There was no significant difference in the proportion of nontransplant patients after CAR-T treatment between two groups. Among the nontransplant patients after CAR-T infusion in DAC group, 16.7% (1/6) of patients relapsed at 4 months. However, among 4 nontransplant patients in CON group, 1 patient achieved NR after CAR-T therapy and 3 patients relapsed at 1.5, 5, and 10 months. There were significant differences in overall survival (OS) and leukemia-free survival (LFS) between two groups: 3-year OS, 88.9% (DAC) versus 33.3% (CON), P = 0.01 and 3-year LFS, 92.3% (DAC) versus 21.8% (CON), P = 0.002. Multivariable analysis showed that addition of DAC to the lymphodepletion regimen was associated with better OS (hazard ratio [HR] 0.107, [95% CI, 0.013-0.875], P = 0.037) and LFS (HR 0.081, [95% CI, 0.01-0.65], P = 0.018). Cytokine release syndrome was observed in all patients. Conclusion: In summary, DAC in combination with FC regimen followed by CD19CD22 CAR-T cells was feasible and well tolerated. Our study demonstrated DAC combined with FC was an independent prognostic factor correlated with better survival in relapsed/refractory B-ALL patients. A prática de divulgar relatórios de sustentabilidade tem se tornando comum em organizações por todo o mundo; em especial no Brasil, há uma forte tendência de crescimento na adoção desta prática. O objetivo deste trabalho foi identificar porque as organizações reportam e os aspectos positivos e negativos no modelo de reporte de sustentabilidade proposto pela Global Reporting Initiative (GRI), na opinião dos funcionários apontados nos relatórios de sustentabilidade como responsáveis por eles; estas informações são importantes porque poucas pesquisas investigam os relatórios de sustentabilidade a partir deste ponto de vista e porque oferece à GRI pontos específicos a serem melhorados. A pesquisa foi realizada por meio da análise de conteúdo, sem a utilização de softwares específicos para o método, das respostas dos funcionários de 27 empresas às questões: Por que elaborar relatório de sustentabilidade? Quais aspectos positivos você identifica no modelo de reporte proposto pela GRI? e quais os negativos? Como resultado, foram obtidas sete categorias sobre por que as organizações emitem relatório de sustentabilidade: apoiar a gestão, acompanhar o mercado, prestar contas, contribuir com outras empresas, abertura do capital, relacionamento com stakeholders e melhorar a imagem da empresa. As categorias de aspectos positivos identificadas foram: melhora a gestão da empresa, proporciona uma ferramenta de marketing, o guia proposto para a elaboração do relatório, possibilidade de legitimação e auxilia no benchmarking. E as categorias de aspectos negativos: dificuldades no processo proposto, dificuldades internas, modelo do relatório final, avaliação dos relatórios, visão externa dos relatórios, indicadores, utilização do relatório como ferramenta de marketing e foco em grandes empresas. Como conclusão, verificou-se que as empresas encontram nas diretrizes da GRI mais do que um modelo de reporte, chegando a representar uma ferramenta de gestão da sustentabilidade; porém, os aspectos negativos identificados colocam em risco a eficácia do projeto de reporte, principalmente devido ao excesso de flexibilidade dos relatórios, o que permite a emissão de diversos formatos de relatórios, muitos dos quais pouco objetivos e com chance de conter informações não confiáveis. Sugerem-se então algumas propostas de melhorias, entre elas a definição de um quadro-resumo que forneça ao leitor uma visão global do desempenho da organização em sustentabilidade, a definição de um modelo específico de relatório final e o desenvolvimento de um novo padrão de classificação dos relatórios.
The practice of disclosing sustainability reporting is becoming common in organizations throughout the world; especially in Brazil, there is a strong upward trend in the adoption of this practice. The objective of this study was to identify positive and negative aspects in the framework for sustainability reporting proposed by the Global Reporting Initiative (GRI), according to the opinion of the staff responsible for the report; this information is important because few studies investigate the sustainability reports from this point of view and because it offers to GRI specific points to be improved. Held data collection by means of open questions that evaluated: why report sustainability reports; positive aspects in the guidelines; and negative aspects of the proposed guidelines. The investigation of responses obtained was through content analysis, using an exploratory qualitative approach. As a result, there are seven categories on why organizations issue a sustainability report: management support, market monitoring, accountability, contribute to other companies, going public, stakeholder relations and improve the company\'s image. The categories of positive aspects are: improved management of the company, provides a marketing tool, the proposed method for preparing the report tab, possibility of legitimation and helps in benchmarking. And the categories of negative aspects: difficulties in the proposed process, internal difficulties, model of the final report, evaluation reports, external view of the reports, indicators, use the report as a marketing tool and focus on large companies. In conclusion, it was found that companies encounter on the GRI guidelines more than one reporting model, came to represent a management tool for sustainability; However, the negative aspects identified jeopardize the effectiveness of the project reporting, mainly due to excessive flexibility of reporting, which allows the issuance of various types of reports, many of which little goals and likely to contain unreliable information. Then are suggested some proposals for improvements, including propose a summary table that provides the reader with an overview of the organization\'s performance in sustainability, propose a specific model of the final report and develop a new standard for classification od reports.

From May 28, 2002 to August 20, 2002, I served as an intern in the collections department at The National D-Day Museum in New Orleans, Louisiana. The National D-Day Museum (NDDM) is a 501(c) (3) history museum that specializes in the amphibious invasions of World War II. The institute is very new, opening in 2000, and has experienced an unpredictable amount of success and acclaim. This rapid success and growth has created unique challenges for the organization as it tries to develop. The following paper is broken into five chapters and a conclusion. Chapter 1 is an introduction to NDDM, including its history, mission, organizational structure, funding and programs. Chapter 2 is a description on my internship that includes tasks and responsibilities. Chapter 3 discusses the major problem that the collections department faces (lack of staff) and Chapter 4 gives a recommendation on how to solve this problem. Chapter S discusses my effect on the organization and the paper ends in a conclusion of the organization and my experiences.

Two questions are addressed: 1) whether a dream is meaningful as a whole, or whether the scenes are separate and unconnected, and 2) whether dream images are an epiphenomenon of a functional physiologicaL process of REM sleep, or whether they are akin to waking thought. Theories of REM sleep as a period of information-processing are reviewed. This is Linked with work on the relationship between dreaming and creativity, and between memory and imagery. Because of the persuasive evidence that REM sleep is implicated in the consolidation of memories there is a review of recent work on neural associative network models of memory. Two theories of dreams based on these models are described, and predictions with regard to the above two questions are made. Psychological evidence of relevance to the neural network theories is extensively reviewed. These predictions are compared with those of the recent application of structuralism to the study of dreams, which is an extension from its usual field of mythology and anthropology. The different theories are tested against four nights of dreams recorded in a sleep Lab. The analysis shows that not only do dreams concretise waking concerns as metaphors but that these concerns are depicted in oppositional terms, such as, for example, inside/outside or revolving/static. These oppositions are then permuted from one dream to the next until a resolution of the initial concern is achieved at the end of the night. An account of the use of the single case-study methodology in psychology is given, in addition to a replication of the analysis of one night's dreams by five independent judges. There is an examination of objections to the structuralist methodology, and of objections to the paradigm of multiple dream awakenings. The conclusion is drawn that dreams involve the unconscious dialectical step-by-step resolution of conflicts which to a great extent are consciously known to the subject. The similarity of dreams to day-dreams is explored, with the conclusion that the content of dreams is better explained by an account of metaphors we use when awake and by our daily concerns, than by reference to the physiology of REM sleep. It is emphasised that dreams can be meaningful even if they do not have a function.

Main topic discussed in this diploma thesis is corporate reporting in terms of Human Capital using Business Intelligence tools, specifically IBM Cognos Report Studio. One of the objectives is to show step-by-step methodology of creating complex dynamic report, which includes data structure modeling, layout design and quality check. Another objective is to conduct Cost-Benefit Analysis for a real-life project, which is focused on recreating of Excel-based report in Cognos-based environment in order to automate information flows. Essential part of the diploma thesis is theoretical background of Business Intelligence aspects of data quality and visualization as well as purposes of human capital reporting and description of appropriate KPIs. Objectives are addressed by conducting analysis and research of resources related to topics described above as well as using IBM Cognos Report Studio provided by one of the major companies in financial advisory field. This diploma thesis represents relevant reading for those, who are interested in real-life application of data quality improvement and information flow automation using Business Intelligence reporting tools.

This paper studies the importance of Portuguese financial sell-side analysts’ reports by testing reliability in firms’ Price Target calculation and information aptitude (deliver ability) in the content of sell-side analysts’ reports. The importance of sell-side analysts reports has long been studied, mainly in two different perspectives: the consequences of their work (market price reactions, trading strategies based in analysts’ recommendations) and the externalizations that influence their work (herding and bias behaviors). We believe that before either perspective can explain their value, analysts through their reports should be able to deliver the information users need and offer coherent calculation that justifies the Price Targets. We explore and encode the complete content of 73 reports from PSI20 listed companies, and apply consistency and reliability procedures to test them. Informativeness is tested against an ideal report (built mainly from the Jenkins Report conclusions). To test reliability in the Prices Targets calculations we investigate if the method and the parameters of the evaluation are clearly disclosed and if the calculative procedure is according to the theoretical conventions.
O presente trabalho propõe-se avaliar a importância dos analistas financeiros Portugueses testando para isso a fiabilidade no cálculo dos Price Targets e a capacidade informativa dos relatórios que produzem. A utilidade dos analistas financeiros tem sido há muito estudada, por norma através de duas perspectivas: avaliando as consequências do seu trabalho (reacção dos mercados às suas recomendações e estratégias de investimento baseadas nessas mesmas recomendações) e por outro lado considerando as variáveis exógenas que influenciam o seu trabalho (comportamentos tendenciosos e de “arrebanhamento”). Acreditamos que antes de avaliar a pertinência destas perspectivas, importa averiguar se através dos relatórios que produzem os analistas financeiros fornecem a informação que os seus utilizadores necessitam. Para isso examinamos e codificamos 73 relatórios financeiros de empresas que integram o PSI20, testando-os em termos de informatividade e fiabilidade. A capacidade informativa é testada em confronto com um relatório ideal (baseado nas conclusões do Relatório Jenkins). Para testar a confiabilidade no cálculo dos Price Targets investigamos se o método e os parâmetros utilizados são expressos com clareza e se o processo de cálculo está em conformidade com aquilo que são os princípios teóricos aceites.

Partindo de uma perspectiva lingüística de análise do discurso, esta pesquisa investiga a situação dos direitos humanos no Brasil, focalizando os direitos da mulher, em relação à discriminação de gênero e à violência doméstica. Documentos descritivos e normativos do assunto foram publicados por instituições governamentais internacionais e nacionais e também por instituições não-governamentais como movimentos feministas. Entretanto, poucos estudos foram realizados para investigar esses documentos e desvendar as realidades representadas. Considerando o exposto, analiso um dos capítulos do 'Relatório da Situação dos Direitos Humanos no Brasil', elaborado em 1997 pela OEA, intitulado 'Os Direitos Humanos da Mulher no Brasil'. Desenvolvo esta pesquisa baseando meus objetivos na perspectiva do discurso de Fairclough e de Halliday, na qual as identidades e hierarquias sociais são criadas e mantidas através do discurso entre os membros da sociedade. Utilizo a teoria e método da Lingüística Sistêmica Funcional de Halliday para analisar os processos de transitividade que ocorrem no objeto de estudo, para obter as respostas às perguntas da pesquisa. Com este procedimento, a análise revela que as mulheres são representadas como vítimas das violações aos seus direitos humanos, assim como responsáveis por algumas dessas violações e, ao mesmo tempo, ocupam uma posição de submissão na sociedade. Esta pesquisa representa uma contribuição na busca de soluções que possam ajudar a atenuar o problema.

ZusammenfassungIn Deutschland waren 818.255 Menschen unter 65 Jahren zum Stichtag 31.12.2019 pflegebedürftig im Sinne des Sozialgesetzbuchs, das entspricht knapp 20 % aller Pflegebedürftigen. Ein drängendes Problem in der Versorgung jüngerer Menschen mit Pflege- und Unterstützungsbedarf ist es, die besonderen Anforderungen dieser Gruppe in Bezug auf passende Angebote und Einrichtungen zu decken, die ein Leben in möglichst großer Autonomie und Teilhabe ermöglichen. Neue Wohnformen stellen dabei ein Angebot zwischen Heim und Häuslichkeit dar, das hier Perspektiven bietet. Allerdings liegen derzeit wenig belastbare Informationen bzgl. der Anzahl der Angebote, ihrer Nutzung und ihrer Bedarfsgerechtigkeit u. a. im Hinblick auf Versorgungs-Outcomes vor. Der vorliegende Beitrag fasst verfügbares Wissen hierzu zusammen und stellt exemplarische Praxisbeispiele junger Pflege in neuen Wohnformen sowie Elemente für ein zukunftsfähiges Wohnkonzept bei junger Pflege vor. Die Ausweitung über einzelne „Leuchtturmprojekte“ in die allgemeine Versorgungspraxis unter Verständigung auf notwendige Rahmenbedingungen ist zukünftig ein wichtiger und drängender Schritt in Richtung ausreichender regelhafter Angebote.

Zusammenfassung Zusammenfassung Der Referentenentwurf zur Notfallversorgung wurde kurz vor einer der größten Krisen der Bundesrepublik veröffentlicht. Strukturelle Probleme des deutschen Gesundheitswesens wurden in der Krise besonders sichtbar und mussten quasi über Nacht zumindest provisorisch gelöst werden. Schon zuvor legten steigende Fallzahlen und eine weitgehend fehlende Steuerung die Probleme der Notfallversorgung offen. Eine grundlegende Strukturreform zur längerfristigen Behebung der verschiedenen Defizite erscheint einmal mehr notwendig, wurde aber vom Gesetzgeber zunächst vertagt. Der Sachverständigenrat stellte dazu bereits 2018 umfassende Empfehlungen vor, welche hier vorgestellt und punktuell ergänzt werden. Das Leitbild sind sektorenübergreifend koordinierte, klar abgestufte Versorgungspfade. In Integrierten Leitstellen (ILS) beurteilen erfahrene Fachkräfte rund um die Uhr die Behandlungsdringlichkeit der Anrufer und lenken die Versorgung in bedarfsgerechte und effiziente Strukturen. Ebenfalls rund um die Uhr erreichbare Integrierte Notfallzentren (INZ) stellen an qualitativ besonders geeigneten Kliniken den ersten Anlaufpunkt dar. Die weitere Behandlung erfolgt aus einer Hand, ambulant oder stationär.

ZusammenfassungVielfalt ist das Resultat einer sich dynamisch wandelnden Gesellschaft. Das hat auch Auswirkungen auf die pflegerische Versorgung in der Häuslichkeit, wo der Großteil der Menschen mit Pflegebedarf von Angehörigen oder Personen aus Freundeskreis und Nachbarschaft versorgt werden. Anhand von vier Bevölkerungsgruppen gibt der Beitrag einen exemplarischen Einblick in jeweils unterschiedliche Lebenslagen, -geschichten und Herausforderungen, die im Zusammenhang mit einer Pflegesituation einhergehen können. Der Artikel stellt häusliche Pflegesettings bei Menschen mit Migrationsgeschichte, bei Familien mit einem pflegebedürftigen Kind, bei Menschen aus dem Spektrum der sexuellen Vielfalt (LSBTI*) und bei pflegenden Kindern und Jugendlichen in den Focus. Die den Betrachtungen zugrundeliegende These lautet, dass zwar im Hinblick auf die Zusammensetzung der Gesellschaft Vielfalt Normalität ist, diese aber mitunter im Pflegesystem und der Pflegepraxis noch immer nicht umfassend wahrgenommen und gelebt wird. In der Folge stehen viele Bevölkerungsgruppen im Schatten der Öffentlichkeit. Aufgrund der fehlenden Wahrnehmung fehlen wichtige Unterstützungsleistungen. Die im Beitrag dargestellten jeweiligen Rahmenbedingungen sollen für die spezifischen Bedürfnisse dieser Gruppen sensibilisieren. Die dargestellten Handlungsempfehlungen sollen zum weiteren Nachdenken und zur Diskussion um eine achtsame pflegerische Versorgung von allen Menschen mit Pflegebedarf und zur besseren Unterstützung der sie primär versorgenden pflegenden Angehörigen anregen. Die stärkere Sensibilisierung von Fachkräften zu den jeweiligen Gruppen stellt dabei eine wiederkehrende Forderung dar, die auch als übergeordnete Forderung nach einer umfassenden diversitätssensiblen Ausrichtung der Pflege verstanden werden kann. Für den Beitrag wurde der Austausch mit verschiedenen Fachstellen gesucht, deren inhaltlicher Arbeitsschwerpunkt in den jeweiligen Themengebieten liegt. So soll eine praxisnahe Perspektive zum Thema vermittelt werden.

Diesem Ansatz liegen zwei in diesem Beitrag vorgestellte Befragungen zugrunde. Untersucht wurde in Berliner und Brandenburger Einrichtungen, inwieweit das Personalmanagement diesen optimalen Einsatz des Personals zu erreichen versucht. Beide Befragungen zeigen, dass sich das Personalmanagement in stationären Pflegeeinrichtungen immer mehr strategisch auf diese Herausforderung einstellt, aber nicht alle Steuerungsinstrumente nutzt, die sinnvoll sein können. Zwei dieser möglichen und sinnvollen Steuerungsinstrumente werden vorgestellt. Das Kompetenzmanagement, das in anderen Branchen etabliert ist, hilft u. a. das Personalmanagement nachhaltiger auszurichten. Die verstärkte Betrachtung der Prozesse unter Einbeziehung der Mitarbeiterperspektive als zweites vorgestelltes Instrument hilft, die Mitarbeiterzufriedenheit und letztlich auch die Bewohnerzufriedenheit zu erhöhen.

ZusammenfassungDer Versorgungssektor der außerklinischen Intensivpflege und Beatmung hat in den vergangenen Jahren eine dynamische Entwicklung erfahren. Gleichzeitig werden neben fehlender Transparenz mit Blick auf epidemiologische Kennzahlen und Versorgungsstrukturen unterschiedliche Versorgungsdefizite konstatiert, besonders im Bereich der außerklinischen Beatmung. Mit dem Gesetz zur Stärkung von intensivpflegerischer Versorgung und medizinischer Rehabilitation in der gesetzlichen Krankenversicherung (Intensivpflege- und Rehabilitationsstärkungsgesetz – GKV-IPReG) werden die bisherigen Regelungen zur Erbringung medizinischer Behandlungspflege für Versicherte mit intensivpflegerischem Versorgungsbedarf in einen neuen Leistungsanspruch für außerklinische Intensivpflege überführt und die sozialrechtlichen Rahmenbedingungen hinsichtlich des Leistungsanspruches, des Leistungsortes sowie der Leistungserbringung definiert. Um bedarfsorientierte Versorgungsstrukturen nachhaltig zu gestalten, ist es jedoch notwendig, die bestehenden Bedarfslagen sowie die Versorgungs- und Lebenssituation von betroffenen Menschen systematisch zu erfassen. Die Systematisierung von potentiellen Erkrankungen, die einen Intensivpflegebedarf bedingen, und die darauf aufbauende Analyse von AOK-Abrechnungsdaten für das Jahr 2019 verdeutlicht die Heterogenität von Menschen mit außerklinischer Intensivpflege sowohl mit Blick auf das Alter, das Geschlecht, das Versorgungssetting als auch auf bestehende Grunderkrankungen. Eine große Bedeutung muss dabei der Subgruppe beatmeter und hier insbesondere trachealkanülierter Patientinnen und Patienten zugeschrieben werden. Die Untersuchung bestätigt überdies den bestehenden erheblichen Forschungsbedarf für den gesamten Versorgungsbereich.

Zusammenfassung Zusammenfassung Durch das Gesetz zur Stärkung der Versorgung in der gesetzlichen Krankenversicherung (GKV-VSG) und den Rahmenvertrag Entlassmanagement soll eine bedarfsgerechte, kontinuierliche Weiterversorgung der Patienten im Anschluss an die Krankenhausbehandlung gewährleistet werden. Die bisherigen Erfahrungen mit der Umsetzung des Rahmenvertrages bestätigen allerdings, dass es nach wie vor Schnittstellenprobleme zwischen den am Patientenübergang beteiligten Akteuren gibt. In diesem Beitrag werden aktuelle Problemlagen im Entlassmanagement beschrieben und Möglichkeiten vorgestellt, wie sich Routinedaten für den Prozess des Entlassmanagements erfolgversprechend nutzen lassen. Dies wird am Beispiel eines aktuellen Projekts beschrieben, das den Informationsaustausch zwischen den Sektoren fördert und so die Entlassplanung verbessern soll. Dabei unterstützt ein auf Routinedaten basierendes Modell das initiale Assessment zur frühzeitigen Identifikation von Patienten mit erhöhtem poststationärem Versorgungsbedarf und ermöglicht zusammen mit einem beschleunigten, IT-basierten Genehmigungsprozess für Nachsorgeleistungen eine effektivere Entlassplanung und Patientenüberleitung.

Zusammenfassung Zusammenfassung Die Versorgung von Patienten mit Herzinsuffizienz ist gekennzeichnet durch hohe Behandlungsintensität und häufige Wechsel zwischen den Leistungssektoren. Dies macht ein hohes Maß an Abstimmung und Kommunikation zwischen den Leistungserbringern der beteiligten Versorgungsebenen erforderlich. Dieser Beitrag geht der Frage nach, inwieweit das deutsche Gesundheitssystem Strukturen bereithält, die eine nahtlose Versorgung von Herzinsuffizienzpatienten unterstützen.Als Ausgangspunkt der Betrachtungen diente eine Routinedatenanalyse auf der Grundlage bundesweiter Abrechnungsdaten von über 26 Mio. AOK-Versicherten aus den Jahren 2018–2019. Der Fokus lag auf zentralen Charakteristika der Herzinsuffizienz-Population und der Versorgungsprozesse sowie wichtigen Schnittstellen der Versorgung. Erwartungsgemäß fanden sich unter den Herzinsuffizienz-Patienten viele ältere und pflegebedürftige Menschen. Die Komorbiditätslast und Polypharmaziequote waren hoch. Dies spiegelte sich in einem hohen Betreuungsbedarf mit vielen Arztkontakten und Krankenhausaufenthalten wider.Studiendaten weisen auf Versorgungsdefizite hinsichtlich einer frühen und validen Diagnosestellung sowie der Langzeitbehandlung hin. Ungenügend implementierte Regelungen der Kooperation, fehlende Kommunikationsformate sowie Vergütungsstrukturen, die wenig Anreize für kooperative Versorgungsformen bieten, tragen dazu bei, dass das Zusammenspiel von Hausarzt und Kardiologe noch nicht optimal funktioniert. Ein optimaler Übergang vom Krankenhaus in die ambulante Weiterversorgung ist personal- und ressourcenintensiv und trotz sinnvoller gesetzgeberischer Vorgaben häufig nicht umgesetzt. Brüche bestehen zudem in Bezug auf die Anbindung des Patienten an das professionelle System.Die hier vorgestellten Case-Management-Strategien enthalten verschiedene Komponenten, die geeignet sind, Schwachstellen in der bestehenden Versorgung zu kompensieren. Neben der Erfassung von Körperwarnsignalen scheinen insbesondere verbindliche Regelungen zur Kooperation, die Einbindung nichtärztlicher Fachkräfte sowie die Etablierung sektorübergreifender Kommunikationsformen essentiell, um eine nahtlose Versorgung von Herzinsuffizienz-Patienten zu gewährleisten. Eine wichtige Rolle spielt zudem das Vergütungssystem, das mit seiner sektoralen Trennung und dem Bezug auf einzelne Leistungserbringer nicht ausreichend Anreize für eine kooperative Versorgung setzt.

Research conducted over the past decade has resulted in a suite of methods for robust design which can be applied during different design stages. These methods focus on reducing the sensitivity of the design to variation without removing its causes. In this research we are investigating an additional and very powerful means for achieving robustness that complements the other methods developed to date. We have dubbed this area “smart assemblies.” A smart assembly has features, not otherwise required by the function of the design, which allow the design to absorb or cancel out the effects of variation. In this paper we report our results to date. We discuss the close connection between smart assembly design and exactly constrained design. We present the beginnings of a smart feature classification system, a preliminary methodology for smart assembly design, and a case study.

Abstract Despite the steady and continuing growth of computing power and speed, the complexity and computational expense of engineering analysis codes maintains pace. Statistical techniques are becoming widely used in engineering design to construct approximations or metamodels of these analysis codes which are then used in lieu of the actual codes, facilitating optimization and concept exploration. Our purpose in this paper is to report results of ongoing research aimed at increasing the efficiency of computer-based engineering design through the use of spatial correlation metamodels to build global approximations of computationally expensive computer analyses. Three structural design examples are presented to test the predictive capability of these metamodels for use in design optimization. The reported results confirm that these spatial correlation metamodels can produce sufficient accuracy for optimization when used as global approximations.

This paper presents a new method for adapting the existing manufacturing system for a requirement change, such as the change of a required motion task, by modifying its kinematic structure from the viewpoint of the degrees of freedom of the end effecter of the mechanism. In a previous report, the authors formulated the kinematic structure and motion task using Lie algebra. In this report, using these representations and their inclusion relations, the authors propose a method for evaluating the reusability of a kinematic structure for a newly specified motion task, and then propose a method for reconfiguring the mechanism in Lie algebra according to the result of the reusability evaluation. Finally, in order to evaluate the usefulness of the proposed method, it is applied to a kinematic design example of a mechanism.

Abstract This paper documents a survey of sensor technology for use in robotic hands. This is achieved by studying sensors used in the UTAH/MIT Hand, the Belgrade/USC hand, the Stanford/JPL hand and the Hirzinger Hand. Additionally, off-the-shelf available sensors as well as those under development in research labs are also evaluated. Different types of sensing covered in this report are position, torque, touch/contact force, proximity, vision, and temperature. Different options available for each type of sensing are also outlined. Major attention was given to the contact force sensor, as it is crucial for the grasping task.

Abstract We report on our experience using an Internet-based collaborative environment to enhance the design and manufacturing process of a custom-designed artifact transport system (ATS). Specifically, we focus on overcoming the hurdles associated with exchanging heterogeneous information that includes text, graphics, and computer-aided design (CAD) data among 15 to 20 geographically-separated project participants, each with his own unique workstation and operating system. To share this heterogeneous information among the team’s members, which included designers, physicists, manufacturers and managers, we implemented a collaborative workbench (CW) that was designed specifically for platform-independent design and manufacturing collaboration. The workbench consists of two principal parts: an Internet-accessible portion and a platform-specific collaboration notebook. The Internet-accessible portion runs on a local server and consists of a Project Area that contains project-specific information such as drawings, specifications, and schedules, and a Document Vault, which stores files of any type that can be uploaded via client World Wide Web browsers. Based on specifications from the physicists, designers created and represented ATS components and assemblies on their respective CAD systems. The designers published their designs on the CW and informed project team members of the newly available CAD drawings via automatic email. Team members then commented directly on the CW representations of the CAD drawings, and those comments were republished with the drawings. This process continued until team members reached a consensus, or until face-to-face meetings helped resolve conflicting issues. Similar processes occurred with documents such as reports and schedules and with digitized photographs of manufactured components. We conclude that environments like the CW can be effective in helping teams overcome the problems associated with diverse computing environments and heterogeneous data formats, and can be effective in facilitating consensus-based decision making necessary for collaborative design.

The effect of tempering (artificial aging) on the axial crush strength of four cell extruded rectangular aluminum alloy (AA) 6061 and 6063 tubes is developed in this report. Increasing the aging time from the press quenched condition increases the flow strength of the material and also increases the axial energy absorbed up to a point when the material showed fracture. Good predictions were made between experimental, theoretical, and numerical mean crush loads for various tempers of AA6061 and AA6063, except in the cases where a large amount of fracture was present. A recommended temper time of three hours was obtained for AA6063, with an increase in mean crush load of 60%. A recommended temper time of six hours was obtained for AA6061 with an increase in mean crush load of 40%. These results will be useful in future aluminum automotive body projects, both for their predictive capability, and for the temper recommendations.

Abstract In earlier work, the axiomatic design theory has been analyzed for applications on product design and the production processes that pertain to it, where parts manufacture and assembly take place in flexible and automatic manufacturing systems. The conclusion is that the original model cannot handle the manufacturing aspects in a satisfactory manner. This report proposes an expansion of the axiomatic design model, with a life-cycle approach as take-off. The expansion of the model consists of the introduction of a so-called Manufacturing World with different spaces, where various types of processes and support functions can be developed in agreement with the axiomatic principles. The spaces and their relationships have been defined along with explanations of work procedures. An explanatory example is given.

Abstract In a previous paper [1] on the design of Capra — a quadruped robot with improved agility, we had presented the main design avenues that were to be considered for the development of an agile mobile walking robot, Capra. The objective of achieving full and autonomous robot agility in harsh environments has always been predominant in our research program on robot design for improved capabilities and applications potential. In this follow-up paper, we report the design results to meet another level of objectives that we had previously determined and currently are incorporating in the Capra hierarchy. Systematic design methodologies that were developed for the robotic machine structure also enable one to consider the Capra robot as not only an agile walking-machine but also as a versatile design platform. It can be deployed for retrofit in many automation applications or for specific uses in harsh environments involving critical design constraints. This design approach has resulted in the development and integration of many of Capra’s fundamental sub-systems by means of a range of modules and prototypes. This paper addresses all the different design issues involved in the development of a versatile design platform for a truly mobile robot. The design methodologies to support the various capabilities are presented here as independent systems, which can be fitted, integrated and retrofitted for any mobile robot configuration. Several design results are presented as examples of the design process.

Abstract We report on our experience using the Virtual Reality Markup Language (VRML) to collaborate on the design and manufacture of an artifact transport system (ATS). Specifically designed for the purpose of transporting nanometer-scale dimensional artifacts at pressures ∼10−8 Pa, the ATS consists of a transport cart and an ultra-high vacuum (UHV) system. As its name implies, the ATS is to transport an atomically-accurate specimen created in a molecular beam epitaxy (MBE) laboratory to a scanning tunnel microscope (STM) laboratory across the NIST campus, where metrologists verify atomic-scale measurements. The project team involved between 15 and 20 participants — designers, engineers, physicists and manufacturers — and each individual was involved with the design and assembly of the ATS to varying degrees. After the project engineers developed their assembly models with their CAD tools, we exported the components and assemblies to VRML files. These representations were made available, via web browsers with VRML viewers, for feedback to project team members on their own workstations, which included PCs, Macintoshes and Suns. The port involved characterizing the simulation’s performance over a range of parameters such as processor capability, file size, VRAM available and graphics card capability. After meeting with the fabricators and physicists to determine the approximate assembly sequence of the ATS, we edited, augmented and animated the VRML files on a high-end workstation. By visualizing the animation sequence in a common facility with a videowall, participants were able to reach a consensus for the design and assembly changes needed. We conclude that VRML did help our team collaborate in the design and fabrication processes, although the technology supplemented, rather that supplanted face-to-face meetings. Our experience with VRML on multiple workstations leads us also to conclude that the language needs to be characterized to enhance easy development of engineering models and to achieve true and complete platform-independence.

This report sets out to map the different guidance documents available on how to work most effectively with civil society in the delivery of international aid in ways that deepen democracy and advance the rights of marginalised or excluded groups. It includes a review of guidelines published by other key international development funders and implementors written for their own teams, an overview of guidance provided for DAC members within OECD countries and policy papers on cooperation between the state and CSOs. It looks primarily at documents produced in the last ten years, between 2011 and 2021 and includes those related to cooperation on specific issues (such as drugs policy or human rights, as well as those that deal with specific countries or regions (such as Europe or the MENA region). The majority of documents identified are written by government aid departments (eg USAID, Norad) but there are one or two produced by umbrella civil society organisations (such as Bond) or international legal think tanks (such as ICNL, the International Centre for Not for Profit Law). There was a remarkable consistency between the issues Millican addressed in the different documents although their size and length varied between outline guidance on 2 – 3 pages and a comprehensive (62 page) overview that included definitions of civil society, range of organisations, reasons for collaborating, mechanisms for financing, monitoring and ensuring accountability and challenges in and guidance on the ways in which donors might work with CSOs.

This report is a synthesis of a specified set of reports from BACKUP and K4D with a focus on integration of health care services in relation to Malaria, TB and HIV and how integration efforts contribute or impede health systems strengthening in Uganda, DRC, Tanzania, Mozambique, Nigeria, and Zimbabwe. It does not cover epidemiology or interventions (other than in relation to integration). The K4D reports highlight country specific epidemiology, disease control endeavours and key interventions for each disease including some that would strengthen health systems and promote integration. The BACKUP reports focus more on integration and can add country specific details with recommendations. Where references indicate a country name, this refers to either the K4D report or Backup report from that country. A list of the reports used for this synthesis may be found in the acknowledgement section. Where there are no references, this indicates that this is collective evidence from across BACKUP and K4D reports. Other references are included in the reference section.

Because many patients reduce exercise following outpatient cardiac rehabilitation (CR), we developed an intervention to assist with the transition and evaluated its feasibility and preliminary efficacy using a one-group pretest–posttest design. Five CR patients were enrolled ~1 month prior to CR discharge and provided an activity tracker. Each week during CR they received a summary of their physical activity and steps. Following CR discharge, participants received an individualized report that included their physical activity and step history, information on specific features of the activity tracker, and encouraging messages from former CR patients for each of the next 6 weeks. Mixed model trajectory analyses were used to test the intervention effect separately for active minutes and steps modeling three study phases: pre-intervention (day activity tracking began to CR discharge), intervention (day following CR discharge to day when final report sent), and maintenance (day following the final report to ~1 month later). Activity tracking was successfully deployed and, with weekly reports following CR, may offset the usual decline in physical activity. When weekly reports ceased, a decline in steps/day occurred. A scaled-up intervention with a more rigorous study design with sufficient sample size can evaluate this approach further.
