Résumé :
This chapter explores how the early 1930s tested Judson's mettle as an orchestra manager because the Depression completely redefined the rules of business. The 1910s and 1920s had been periods of tremendous optimism. The audience base grew exponentially, and nearly everything that Judson tried—artist management, coalitions of managements, radio technology—yielded some form of success. But when the economic conditions changed, optimism turned to protectionism. Funding dried up; deficits became more serious; the margin of error became thinner. Moreover, the shift was not temporary. The challenges Judson started to face in the 1930s would fester in the remainder of the twentieth century, as American orchestras struggled to find enthusiastic funding sources and to control costs in an unstable marketplace.

Résumé :
How does budgeting work, and how is it useful? What are standard costs and how do they help in controlling production? How can a balanced scorecard improve control? ‘Accounting as control’ looks at some ways in which accounting can be used by managers to control their organizations. The two main high-level purposes of budgeting are to optimize the use of the economic resources within the firm in order to maximize profit, and to help the firm to achieve its overall strategic objectives. Six aspects of budgeting are considered: planning, motivation, delegation, communication/coordination, control, and performance evaluation. Cash flow forecasts, flexible budgeting, zero-base budgeting, standard costings, and balanced scorecards are also explained.

Résumé :
This chapter explores the degree to which various psychopathologies influence metamemory. The literature suggests such patients suffer different impairment patterns rather than global, nonspecific impairments of metamemory processes and the memory-metamemory relationships: Depressed patients present memory and metamemory difficulties; obsessive-compulsive patients appear to suffer from metamemory impairments rather than memory disturbances. ADHD patients suffer more from control process impairments than from monitoring deficits. Patients with autism present metamemory and memory difficulties that are restricted to certain context. The chapter focuses on schizophrenia, as impairments of cognition and consciousness are today considered core symptoms of the illness and contribute to patients ‘difficulties in social and professional integration. Patients with schizophrenia present specific disruptions and selective preservation of the metacognitive processes, as the accuracy of their metamemory monitoring is relatively preserved, whereas their metamemory control is sometimes impaired. The presence of preserved abilities allows for optimism regarding patients’ possibilities to improve their memory.

Résumé :
This chapter analyzes the patterns of the sudden opening-up of Myanmar’s political, social and economic space in the early 2010s. It shows how the environment that emerged under Thein Sein’s semi-civilian presidency (2011-2016) appears to have nurtured an unprecedented level of optimism both at home and abroad. Civilian opposition has been allowed, dissent is relatively tolerated and civil society has increased. Censorship has been considerably eased and progressive legislation has been prepared. At the same time, new forms of radicalism and contentious politics have (re)surfaced in the country, benefiting from the relaxation of the military and state control. All pose new challenges to Myanmar’s social cohesion and its prospects for peaceful democratization, as well as to the transition as it was initially “pacted”.

Optimization techniques have played a vital role in improving the performance (i.e., input speed and accuracy) of soft keyboards. This chapter introduces the challenges, methodologies, and results of keyboard performance optimization. Leveraging the robust human motor control phenomena manifested in text entry, we used the Metropolis random walk algorithm, and Pareto multi-objective optimization method to optimize the keyboard layout and a soft keyboard decoder. The optimization led to layouts that shorten finger travel distance and improve the input speed as well as accuracy over the Qwerty layout, and a soft keyboard decoder with improved correction and completion ability.

Renal transplantation is the preferred treatment for pediatric patients who have end-stage renal disease. A successful transplant improves intellectual and behavioral development, quality of life, and survival, with the survival at 10 years being as high as 83% (Kim et al., 1991). We can optimize the chance of success by understanding the pathophysiology involved and applying this knowledge to guide our management of perioperative fluid balance, electrolyte anomalies, anemia, blood pressure control, and comorbidities. Also critical is an appreciation of the effects and consequences of the various immunosuppressive agents that are used. Close communication is required between the pediatrician, surgeon, and anesthesiologist.

Résumé :
The main aim for the diabetic child is to live as normal a life as possible via four main targets of management: obtaining good metabolic control, preventing long-term complications, promoting social competence, and self-worth. The main challenges are that the child must receive appropriate insulin subcutaneously and there must be attention given to lifestyle. Participation in sports not only helps to reach those targets, but also forms a great challenge in maintaining good metabolic control as physical activity has an impact on glucose utilization. This chapter discusses the definition of diabetes mellitus, the diagnostic criteria, the aetiology, and the clinical spectrum, as well as all the practical aspects encountered by the child with diabetes mellitus and his/her care givers and how it all relates to sports and physical activity. Finally, it covers strategies to optimize performance and prevent complications.

Résumé :
This chapter assesses the behavior of corporate managers and boards of directors within the framework of agency theory, stewardship theory, and psychological biases. In agency theory, a chief executive officer (CEO) is motivated to act in his or her own best interests rather than those of shareholders. Stewardship theory posits that a CEO is a self-actualizing individual seeking to grow and reach a higher level of achievement through leading an organization. A CEO exhibits self-interested behavior in managing the firm. The CEO also exhibits optimism, overconfidence, and risk-aversion behaviors that are not optimal for the company. In the context of agency theory, the board of directors should enact incentive structures and monitoring to control these behaviors. However, directors also suffer from self-interests and cognitive biases. Specifically, boards may suffer from group-dynamic problems such as social loafing, poor information sharing, and groupthink.

Résumé :
The last three chapters of the book have a constructivist flavor. One might wonder if constructivism could point to new conditions of thought and rule within the modern world’s mighty frame. If so, Bruno Latour’s extensive, ever more influential body of work bears attention. He has directly, energetically attacked “the dominion of modernism” and sketched a “political ecology” for the future; he made an ancillary project of “saving constructivism” from constructivists in order stabilize conditions of thought for the world to come. Unfortunately, society is missing from Latour’s project because people have no faculties that distinguish them from other “actants.” Untroubled by issues of agency, he has proposed that actants somehow adopt a constitution for a new cosmopolis. Latour has had important allies, include Haraway. As with Haraway, he is a technological optimist. The illusion of control comes in many guises; saving modernity will not be so easy.

What if the true weak link of the Information Age force is not the hardware of machines, but the software of the human mind? And if so, could it be that the entire conceptual structure of the Information Revolution theorists, at least as it applies to military affairs, is built on sand, on the notorious fickleness of human cognition? These are the questions this book strives to examine. Looking at the command and control of information-rich warfare, the contributors explore its potential new processes, techniques, and organizational structures. As they do so, they find reasons for both optimism and concerns about the limitations of human cognition and supporting technologies in commanding battles in the Information Age. Since the beginning of the Information Revolution, the military in the United States and elsewhere has been analyzing and implementing the changes driven by the rapidly advancing information technologies. Among military theorists and practitioners, many focus on the Information Revolution's impact on matters of military equipment. Far fewer, however, seem to worry about the gray matter—the mind of the commander, the place where all the information power of the new age is supposed to converge and to yield its mighty dividends. Consider that it is the human mind, particularly the minds of military commanders and their staffs that remain the pinnacle and the ultimate consumer of the rapidly growing information flows. What if the true weak link of the Information Age force is not the hardware of machines, but the software of the human mind? And if so, could it be that the entire conceptual structure of the Information Revolution theorists, at least as it applies to military affairs, is built on sand, on the notorious fickleness of human cognition? Résumé :
Confronting the overwhelming amount of stress kids face today, this invaluable guide offers coping strategies for facing the combined elements of academic performance, high achievement standards, media messages, peer pressure, and family tension. This handbook acknowledges that adolescents commonly survive stress by either indulging in unhealthy behaviors or giving up completely, and its suggested solutions are aimed at strengthening resilience. The proposed plan enables kids from the age of 18 months to 18 years to build the seven crucial "C's"--competence, confidence, connection, character, contribution, coping, and control--needed to bounce back from challenges. A variety of approaches are featured such as building on natural strengths, fostering hope and optimism, avoiding risky behaviors, and taking care of oneself physically and emotionally. With new chapters on perfectionism, the negative portrayal of teens, military families, and what parents can do when resilience has reached its limits, this examination also includes two personalized guides for creating customized strategies.

This article begins with great optimism expressed by Tocqueville for America's future as the embodiment of the democratic state. It discusses the opportunity to express the liberal political ideals, arguing that its success was based on a community of common sensibility. An understanding of society and politics endowed the historian with the power to help remake health care. This article explores and compares the ways in which medicine is developed and applied in a number of different social, cultural, and physical contexts. It shows rapid growth, from a period in which European ideas, methods, and structures were adapted to the American context, to one in which the United States is at the forefront of large-scale initiatives in public health, disease control, and innovation in the biomedical sciences. Finally, it mentions the contradiction, most notably between profound faith in the technical capacities of medical science and equally profound dissatisfaction with the provision of health care.

Résumé :
This book aims to empower readers to learn and apply engineering, operations research, and modeling techniques to improve public health programs and healthcare systems. Readers will engage in in-depth study of disease detection and control strategies from a “systems science” perspective, which involves the use of common engineering, operations research, and mathematical modeling techniques such as optimization, queuing theory, Markov and Kermack-McKendrick models, and microsimulation. Chapters focus on applying these techniques to classical public health dilemmas such as how to optimize screening programs, reduce waiting times for healthcare services, solve resource allocation problems, and compare macroscale disease control strategies that cannot be easily evaluated through standard public health methods such as randomized trials or cohort studies. The book is organized around solving real-world problems, typically derived from actual experiences by staff at nongovernmental organizations, departments of public health, and international health agencies. In addition to teaching the theory behind modeling methods, the book aims to confer practical skills to readers through practice in model implementation using the statistical software R.

Résumé :
The Enlightenment, as concept and time period, was haunted by ambiguities about the relationships between mind and body, humans and the natural world, and reason and imagination. The 18th century was inherently contradictory, particularly when it came to ideas about medicine and the body. The growing optimism that medicine and science could control nature and disease was counterbalanced by the hierarchies of gender, race and class being fixed on the body. Enlightenment ideals emphasized rationalism and expertise, but they existed alongside religious belief and everyday authority. Focusing on Western Europe, this volume examines disability and suffering, emotional and physical sensations, supernatural phenomena and scepticism, medical authority and expertise, biologization and power, and bodily and environmental regulation. Volume contributors have used a range of cultural history methodologies – from material history to discourse analysis – to examine the Enlightenment’s tensions. The book’s chapters centre on topics (Environment, Food, Disease, Animals, Objects, Experiences, Mind/Brain and Authority) that have encouraged contributors to reframe their assumptions about the history of medicine and the Enlightenment.

Traditional fears and misinformed concerns regarding youth resistance training have been replaced by scientific evidence that indicates regular participation in well-designed resistance-training programmes can be safe and effective for both children and adolescents. In addition to increasing muscular strength and power, regular participation in a structured resistance training-programme can increase bone mineral density, improve cardiovascular risk factors, fuel metabolic health, facilitate weight control, enhance psychosocial well-being, and prepare youth for the demands of daily physical activity and sport. An integrative approach to training, grounded in resistance exercise and motor skill development, can optimize children’s fitness potential and maximize their athletic performance while reducing the risk of sports-related injury. Qualified professionals are needed to plan, implement and progress developmentally appropriate resistance training to attain a level of muscular fitness that facilitates long-term physical development.

Résumé :
In children, the most common indication for spinal fusion is significant scoliotic curvature, either idiopathic or as a result of neuromuscular disease. Spinal fusion is high-risk surgery, which can be further complicated by comorbid disease. It carries substantial risk for significant fluid shifts, high intraoperative blood loss, physiologic strain secondary to duration and positioning, severe postoperative pain, and potential spinal cord injury. To mitigate risk and optimize outcomes, these patients should be carefully evaluated by the anesthetic team preoperatively and a comprehensive perioperative plan established. To protect the spinal cord and predict poor neurologic outcomes, the majority of these cases employ intraoperative neuromonitoring. The specific anesthetic agents to allow maximal neuromonitoring signals while ensuring adequate anesthetic depth and pain control should also be established collaboratively. These patients experience severe postoperative pain, and a multimodal approach to therapy should be employed to allow for expedited recovery and decreased length of stay.

Résumé :
The development of clinical depression is common during palliative care, adversely affects quality of life and adherence to medical treatments, yet regrettably can pass unrecognized. Screening for distress as the sixth vital sign is therefore highly recommended. Demoralization is another form of distress where the apparent pointlessness of continued life may lead to suicidal thinking. As the mental condition deteriorates, co-morbid states of anxiety, depression, and demoralization become more likely. Rates of suicide are increased with advanced cancer and poor symptom control. Fortunately, combined treatment with medication and counselling is effective in ameliorating depression, demoralization, and suicidality. Meta-analyses of psychotherapy trials confirm clear benefits, with behavioural activation, supportive, interpersonal, and cognitive behavioural therapies all making contributions. Group, couple, and family therapies optimize support for all involved. All members of the multidisciplinary team contribute to the active treatment of depression, demoralization, and the prevention of suicide.

Résumé :
Pregnancy outcomes in Type 1 diabetes have progressively improved, but are not yet at background population level. Insulin requirements increase early in pregnancy, followed by a nadir at 16–18 weeks, consistently climbing nearly to delivery. Everyone who sees Type 1 patients of childbearing years should be able to deliver concise and practical advice on pre-pregnancy management, including contraception advice. About one-third of UK pregnancies are unplanned. Even where formal counselling is readily available, most women do not access it. Maternal risks during pregnancy include exacerbation of pre-existing complications, hypoglycaemia, and pre-eclampsia; foetal risks include pregnancy loss, fetal malformation, prematurity, macrosomia, stillbirth, and neonatal death. Ideal preconception A1C is 6 to 7% (42 to 53 mmol/mol). Most insulin preparations are safe during pregnancy. Continuous glucose monitoring and insulin pump therapy are increasingly used, but evidence of definite benefit is awaited. Women are usually highly motivated to optimize glycaemic control during pregnancy.

Edgeworth, Jonathan. Antibiotic resistance in the ICU. Oxford University Press, 2016. http://dx.doi.org/10.1093/med/9780199600830.003.0289.

Texte intégral
Résumé :
The two objectives of ensuring early appropriate antimicrobial treatment for septic patients on the intensive care unit (ICU), and limiting emergence and spread of antimicrobial resistance are both complicated and potentially conflicting. Increasingly unpredictable resistance, particularly amongst Gram-negative bacteria, through both local selection and transmission, and importation of globally successful resistant clones encourages the use of broad-spectrum empiric antimicrobials for septic patients, including in combination. This may lead to a vicious cycle whereby increasing antibiotic use increases resistance, which in turn leads to higher levels of inappropriate therapy. In response, the multi-disciplinary ICU-team implements infection prevention and control, and antimicrobial stewardship programmes. Antimicrobial stewardship programmes provide interventions and guidance to optimize appropriate therapy, whilelimiting unnecessary use through a variety of measures. The development of rapid molecular testing for bacterial identification and antimicrobial susceptibility prediction could potentially bring useful microbiological information to the bedside at the time of therapeutic decision making.

Résumé :
With improved surveillance, diagnoses, and treatment of patients with cancer, an increased life expectancy, and specifically an increased number of ‘cancer cured’ patients, is noted. However, the long-term effects of the disease and treatment have a bearing on obtaining optimal physical, psychological, and cognitive functioning for cancer survivors. Pain impacts on all dimensions of quality of life and is one of the most distressing symptoms for patients. Patients often under-recognize pain and are unsure if optimum pain control is achievable. In addition, members of the interdisciplinary team often fail to assess the patient’s pain adequately, due to a lack of knowledge of the principles of pain relief and side effect management. Treatment requires an interprofessional approach that details a comprehensive assessment, with ongoing reassessment, utilizing both pharmacological and non-pharmacological measures. Empowerment of the cancer survivor, respect for survivors’ individuality and collaboration among team members are key elements of any successful strategy to optimize a patient’s quality of life.

Résumé :
This work is the result of persistence, motivation, enthusiasm and de-dication from a team of researchers coordinated by the Professor and Doctor Marcos Aguiar de Souza and other authors, invited to know how individual behavioral variables interact with and permeate the organiza-tional environment in diverse professional contexts. These variables make up different psychological phenomena, such as: leadership, satisfaction with work and with life in general, locus of control, self-esteem, optimism, gratitude, well-being and personality. The main questions that guided the articles were: how, in practice, such phenomena occur and can be measured, and how transversal skills – also known as attitudinal contents or soft skills – can be developed to promote people´s leadership potential and their wellbeing. The answers to these questions are inside this book and provide interesting opportunities for orientation, intervention and reinforcement of attitudes. This publication glances, connects itself with and, in many occasions, runs through the book “Measurement Instruments in Military Context”, published in 2018, which aimed to develop and validate measurement instruments for the diagnosis of behavioral phenomena specifically in military environ-ments. Thus, we may consider that the present content expands the ho-rizons of perception, metering and registration of behavioral phenomena, aiming to supply, based on evidence, better quality of life and well-being to members of organizations, maintaining high labor performance through self-knowledge, self-determination and self-regulation.

Managing the Big Picture in Colleges and Universities. Praeger, 2005. http://dx.doi.org/10.5040/9798216192558.

Résumé :
In the past, colleges and universities relied on small picture tactics to determine insititutional direction. Even now, many institutions are missing a big picture approach to leadership and management. In an environment where business continues to aggressively optimize the opportunities it sees in higher education, institutional leaders must adopt a more strategic approach to guiding colleges and universities into the future. Strategy must become an essential part of the organizational architecture of every college and university. This book is intended to serve as a road map for strategy creation and execution for leaders and decision makers who, by choice or by necessity, are looking to use strategy to optimize the development of their institutions. It is the first book of its kind to focus exclusively on strategy as it applies to postsecondary education. As the authors explain, strategy is a systematic way of positioning an institution within a context of community stakeholders. In today's competitive environment, higher education leaders must become adept at differentiating their institutions from competitors to obtain the resources necessary for growth and sustainable advantage. The book begins by explaining the concept of strategy and its application. The authors describe the evolution of modern strategy and how it is has been applied and developed by strategy theorists and practitioners. The book also explores how strategy is shaped by critical factors related to the mission, control and culture of the institution. For example, strategy that is appropriate in a liberal arts college may be completely inappropriate for community college or a teaching university. Real-world cases are employed to illustrate the applications of strategy in three different settings: a private liberal arts college, a comprehensive public institution, and a special purpose institution. The last section moves to the hands-on world of strategy formulation and implementation inside the institution. The authors end with an outline of key concepts for building a plan for implementing strategy and provide a framework for evaluating its impact.
