Texte intégralDevice-to-Device (D2D) communication is a key technology for future wireless networks, allowingdevices to communicate directly without relying on a cellular infrastructure. Millimeter wave (mm-Wave) communication utilizes high-frequency radio, providing very large bandwidths for fast and reliable D2D transmissions. However, mmWave frequencies have high attenuation, requiring devices to have multiple antennas and perform beamforming. The success of beamforming requires beam training. The beam misalignment can impact the performance of the network. To address these challenges, our study focuses on the theoretical analysis of the performance of mmWave D2D communications within the context of Ultra-Reliable Low Latency Communications(URLLC). We use stochastic geometry and queuing theory to evaluate both spatial and temporalvariations in performance from two different perspectives: the instantaneous average properties of the random network and the global stability properties of a dynamic network with random service requests. For the dynamic properties, we focus on the stability condition of D2D network by introducing directional antennas arrays for the D2D users. The network is modeled based on a spatial birth-death process. For the instantaneous properties, we mainly care about the meta-distribution of the network, which is a metric that accounts for the spatial distribution of coverageprobability. We derive the meta-distribution of the effective rate as a statistical latency guarantee for URLLC communications, by considering both the training overhead and misalignment for a D2D network with beamforming. At last, we propose methods to choose the optimal number of antennas and to allocate resource for beam training

INTRODUCTION: The palliative care comprehends a holistic approach of assistance for patients without cure possibilities and their families, in order to provide them a better quality of life. Its application is greatly relevant, in the ambit of care directed to terminal patient. OBJECTIVES: This is study aims to investigate the comprehension of assistance nurses regarding palliative care; to identify the strategies that nurses use to promote palliative care for terminal patient; to ascertain how nurses utilize communication, in the ambit of palliative care, when assisting the patient in terminal phase, to investigate how nurses use the communication to broach the bad news for patient in terminal phase and their family in the practice of palliative care. METHODOLOGY: This is an exploratory research of qualitative nature in which the investigate scenario was a public health institution, located in the city of João Pessoa PB; participated in the research twenty eight assistance nurses. For data collection, that occurred between August and October 2012, was utilized a form containing pertinent question to the proposed objectives for the research. As registry unit for the analysis of empirical material, it was chosen the thematic content analysis, from the following phase: pre-analysis, material exploration and treatment of results, inference and interpretation. The data obtained by means of discourse expressed by the participants of the investigation were grouped in the following thematic categories: Palliative care: conceptual aspects and care strategies with the patient in terminal phase, with their respective subcategories: Palliative care promotion of life quality for patients without possibilities of cure; strategies utilized for the promotion of palliative care directed for the patient in terminal phase and Palliative care and Communication, with the subcategories: Palliative care and communication interpersonal relation of the nurse with the terminal patient, communication of difficult news support for patients and families under palliative care. RESULTS: The research included from the vision of the involved professionals, the valuation of palliative care considered as a modality of care which aims to minimize the suffering of the patient without therapeutic possibilities of cure/terminal and of their family, by means of an assistance guided by humanization. Results evidenced that nurses utilize useful and appropriate strategies that meet the needs of patients and their families, in the context of the addressed care all over its dimension, with emphasis on verbal and nonverbal communication, as essential part of care with the patient and the family. FINAL CONSIDERATIONS: We consider that this study opens a new horizon, in the field of care, of teaching and learning in Nursing, with emphasis in the importance of palliative care and of the communication with the patient in terminal phase. Because of the reduced quantum, of studies directed to this thematic, in the ambit of national literature, it is hoped that this research can subsidize new investigation that contemplate the interrelation of Palliative care with the communication.
INTRODUÇÃO: Os cuidados paliativos compreendem uma abordagem holística de assistência ao paciente sem possibilidades de cura e seus familiares, com a finalidade de proporcionar-lhes uma qualidade de vida melhor. Sua aplicação é sobremaneira relevante, no âmbito do cuidado dispensado ao paciente em fase terminal. OBJETIVOS: este estudo objetiva investigar a compreensão de enfermeiros assistenciais no que concerne aos cuidados paliativos; identificar as estratégias que os enfermeiros adotam para promover os cuidados paliativos direcionados ao paciente em fase terminal; averiguar como os enfermeiros utilizam a comunicação, no âmbito dos cuidados paliativos, ao assistir o paciente em fase terminal; investigar como os enfermeiros empregam a comunicação para abordar as más-notícias ao paciente em fase terminal e a sua família na prática dos cuidados paliativos. METODOLOGIA: Trata-se de uma pesquisa exploratória de natureza qualitativa, cujo cenário da investigação foi uma instituição pública de saúde, localizada no município de João Pessoa PB; participaram do trabalho vinte e oito enfermeiros assistenciais. Para a coleta dos dados, que ocorreu entre agosto e outubro de 2012, utilizou-se um formulário contendo perguntas pertinentes aos objetivos propostos para a pesquisa. Como unidade de registro para analisar o material empírico, optou-se pela análise de conteúdo temática, a partir das seguintes fases: pré-análise, exploração do material e tratamento dos resultados, inferência e interpretação. Os dados obtidos por meio dos discursos expressos pelos participantes da investigação foram agrupados nas seguintes categorias temáticas: Cuidados paliativos: aspectos conceituais e estratégias de cuidados com o paciente em fase terminal, com suas respectivas subcategorias: Cuidados paliativos promoção de qualidade de vida para pacientes sem possibilidades de cura; estratégias utilizadas para a promoção de cuidados paliativos direcionados ao paciente em fase terminal e Cuidados paliativos e comunicação, com as subcategorias: Cuidados paliativos e comunicação relação interpessoal do enfermeiro com o paciente terminal; comunicação de notícias difíceis suporte para os pacientes e familiares sob cuidados paliativos. RESULTADOS: A pesquisa contemplou, a partir da visão dos profissionais envolvidos, a valoração dos cuidados paliativos considerados como uma modalidade de cuidar que visa minimizar o sofrimento do paciente sem possibilidades terapêuticas de cura/terminal e de sua família, por meio uma assistência pautada na humanização. Os resultados evidenciaram também que os enfermeiros empregam estratégias úteis e apropriadas e atendem às necessidades dos pacientes e de seus familiares, no contexto dos cuidados abordados em toda a sua dimensão, com destaque para a comunicação verbal e a não verbal, como parte essencial do cuidado com o paciente terminal e sua família. CONSIDERAÇÕES FINAIS: Consideramos que este estudo abre novos horizontes, no campo da assistência, do ensino e da pesquisa em Enfermagem, com ênfase na importância dos cuidados paliativos e da comunicação com o paciente em fase terminal. Devido ao quântico reduzido de estudos direcionados a essa temática, no âmbito da literatura nacional, espera-se que a pesquisa possa subsidiar novas investigações que contemplem a inter-relação dos Cuidados paliativos com a comunicação.
