In this article the author focuses of the most common ten diseases and anomalies that come across our daily practice in the Dental Department of AHT-SA Hospital. The author depended on the statistics, cases reports and active cases of his daily practice. The most registered anomaly “considered disease” in the Orthodontic Department has been the Malocclusion. It’s found out that 94% of the patient who visited the Orthodontic Clinic of A.H. Hospital have had a sort of “Mal-Occlusion”(1). The other diseases that had been registered had been: the Dental Caries, Periodontitis, Cleft-Lip and Palate, Ectodermal Dysplasia, Dentigerous Cysts, Impacted Third Molars, Mouth breathing “and snoring”, Diabetes and its dental aftermaths and Finally Achondroplasia.

In 1908, I described a case of spontaneous intraperitoneal rupture of the bladder. Since then, in Russian literature, I have come across only one article by Belitsky devoted to this issue. In this detailed article Belitsky describes a case of intraperitoneal rupture of the bladder, operated back in 1909 by prof. I. A. Praksin, and reports statistics of both Russian authors - including my case - and foreign ones. From the data of Naumann cited by him in the article, it is clear that in the Salgram clinic for 12 years, for 20 thousand patients, bladder ruptures were observed only in 3 cases - in 2 intraperitoneal and in 1 extraperitoneal; Rokitsky for 15 years gives 12 cases, among which in 8 there was an intraperitoneal rupture and in 4 extraperitoneal rupture.

Annotation. Analysis of recent publications on the definition of fluid accumulation in patients with complicated forms of acute pancreatitis and the possibility of their correction by minimally invasive methods according to the clinic, analysis of a clinical case that demonstrates the possibility of staged percutaneous drainage of fluid accumulation in acute infected necrotic pancreatic. The results of treatment of 622 patients who were hospitalized in the surgical department of MNPE City Clinical Hospital of Ambulance and Emergency Care during 2020 were analyzed. The mean age of patients was 48.2±2.2 years. The patient examination plan included a comprehensive clinical, laboratory and instrumental examination. Puncture and drainage of fluid accumulations were performed under local anesthesia under ultrasound control using a linear sensor with a frequency of 7.5 MHz in gray-scale mode. Statistical data processing was performed by the method of mathematical statistics with StatSoft Statistica 6.0 software and Microsoft Office Excel. It is established that the cause of acute pancreatitis in 270 (43.4%) patients was a biliary factor, in 155 (24.9%) – alcohol, 135 (21.7%) – acute pancreatitis developed on the background of triglyceridemia, in 21 (3.4%) the patient had postoperative pancreatitis, medical – in 8 (1.3%) patients and without establishing a clear etiology – in 33 (5.3%) patients. In order to eliminate fluid accumulations in 12 (1.9% of the majority of patients with acute pancreatitis and 7.6% of 157 with fluid accumulations) patients were used percutaneous puncture-drainage method under ultrasound control, among them in 5 cases the method was used to eliminate fluid accumulations due to acute peripancreatic fluid accumulation, in 2 cases – acute necrotic effusion, limited necrosis – in 2 cases, and pancreatic pseudocyst – in 3 cases. A clinical case of acute necrotic pancreatitis complicated by the development of acute necrotic effusion is presented, for the treatment of which a combination of transcutaneous minimally invasive and open surgical methods of treatment was used at different stages of the disease. So, fluid accumulations around the pancreas belong to the local complications of acute pancreatitis and are recommended for correction by conservative or minimally invasive methods. The question of choosing the optimal minimally invasive method of treatment requires an individual approach. This clinical case demonstrated the possibility of using puncture-drainage method under the ultrasound control as a “step-up” intervention in early stage of treatment and allowed to postpone the open surgery.

4537 Background: Despite guideline-based standard of care recommendations in BC and upper tract urothelial carcinoma (UTUC), treatment remains variable across US. Experts recommend focusing BC care in tertiary centers. We hypothesized that a BCMC model, with expert central pathology and radiology review, may result in changes in corresponding reports, and, thus, treatment recommendations. Methods: Our BCMC clinic format includes simultaneous consultation with Urologic, Medical and Radiation Oncology, with real time expert genitourinary pathology and radiology review. We retrospectively assessed the concordance between outside (pre-BCMC) imaging & pathology review and BCMC review. Differences between pre- and post- BCMC recommendations on management were also assessed; descriptive statistics were used. Results: We identified 233 BC/UTUC patients (pts) referred to BCMC. Complete radiographic and pathologic data were available for 209 pts. Median age at time of evaluation was 68 (27-93) and 85% were PS ECOG 0-1. After BCMC review of outside records, 112 (53.6%) imaging and/or pathology changes were noted, with 57 (27%) pts upstaged. Overall, imaging interpretation was changed in 25% of cases, and 20% of pts were upstaged. BCMC pathology review resulted in changes in 59 (28%) pts. Among those, 42 (71%) had histologic subtype addition or change, 9 (15%) had LVI/CIS status change, and 2 (3.4%) had low to high grade conversion. In terms of pathology staging, 7 (12%) were downstaged, and 5 (8.5%) upstaged. Further diagnostic work-up was recommended in 71/209 (34%) pts, resulting in upstaging in 11/71 (15.5%) of cases. Pre- and post- BCMC-recommended treatment modality differed in 55/209 (26%) pts, while a new treatment modality was added in 28/209 (13%) pts. These recommendations were followed 91.4% of the time (191/209 pts). Conclusions: BCMC initiation at our institution resulted in imaging and/or pathology diagnostic changes in almost half of cases, with approximately a quarter of pts being upstaged. Findings reveal the importance of expert radiology and pathology review in BC. Further study is needed to confirm the proposed benefits and impact of BCMC on treatment response and outcomes.

IntroductionEpileptic seizures have been described in association with multiple sclerosis (MS) in both anecdotal case reports and case series. The recent identification of specific antibodies to myelin oligodendrocyte glycoprotein (MOG) protein in a small number of patients with demyelinating disease which may resemble neuromyelitis optica or acute disseminated encephalopathy, which may involve seizures, raises the possibility that anti-MOG antibody related demyelination may account for the association of epilepsy with MS.MethodsWe have undertaken a retrospective review of cases of MS diagnosed at the Gold Coast MS clinic over a 10 year period. All cases were systematically asked if they had ever had an epileptic seizure either via a patient completed questionnaire or at a clinic visit. Demographic and clinical information were also recorded. These data have been analysed using descriptive statistics and appropriate tests for significant differences between those with epilepsy and those without.Results428 cases with complete data were identified. Those with a history of epilepsy were slightly younger (median (range); 44.5 (27–64) years vs 4715–88 years), but this difference was not statistically significantly different. The gender ratio was the same for both groups (9/12 (75%) for those with epilepsy and 326/416 (78%)). There was no significant difference in age of onset, disease course, relapse frequency or level of disability. Although numbers are small, seizure appear to occur most frequently earlier in the disease course and are rarely an ongoing issue.ConclusionThese data support earlier work indicating that epilepsy occurs in people with MS who are younger. This fits with the notion that seizures arise in the context of the inflammatory stage of multiple sclerosis rather than the degenerative phase. Further work needs to be undertaken to assess any association with anti-MOG antibodies and epileptic seizures in demyelinating disease.

The purpose of this study was clinical and epidemiological analysis of the incidence of syphilis in children and adolescents in the Ryazan region over a 12 year period. In a retrospective study the authors used the data from regional statistical reports, medical records of newborns, the data on infectious diseases of the Federal State Statistics Service. The analysis of the data for all age groups, including children and pregnant women was carried out. It was noted a substantial reduction in the number of reported cases of syphilis, including among adolescents and youth. The proportion of non-residents and rural residents in the total of patients with syphilis is increasing. In the pediatric population, the leading mode of transmission is vertical (53% of all cases). Among children born from pregnant patients with syphilis, there are 78% of the verified congenital syphilis, and only 5% the classical picture of the disease. It was established a substantial reduction of cases of congenital syphilis, the absence of cases of fetal death and postnatal mortality in recent years. However, the retention of latent forms and late detection of syphilis in pregnant women poses a threat of infection and determines the social significance of the problem. Effective control of congenital syphilis is ensured by the collaboration of obstetriciansgynecologists, dermatologists and neonatologists.

Toxic epidermal necrolysis (TEN) also known as Lyell’s Syndrome is a rare, acute hypersensitivity reaction characterized by cutaneous and mucosal necrosis. Lyell’s syndrome is self limited, but potentially recurrent disease. The mortality for toxic epidermal necrolysis is approaching to 30%.[1] Subglottic stenosis is the most common anomaly of larynx and trachea requiring tracheostomy in pediatric population. Subglottic part of larynx is the narrowest section of laryngotracheal area of airways in childhood.[5] It is estimated that most of subglottic stenosis cases is acquired and developed as a result of injury caused by long-term use of endotracheal tube. A 2-year-old patient admitted to our Clinic of Children’s Otolaryngology after long treatment for TEN because of problems with breathing without tracheostomy tube. Because of the results of bronchofiberoscopy patient has been sent to our Clinic of Children’s Otolaryngology. We performed laryngotracheoscopy which showed transglottic, subglottic and proximal part of tracheal stenosis and complete atresia of trachea above tracheostomy tube. Result of MRI examination was described as complete atresia of trachea which was 4 mm long and located above tracheostomy tube. Our patient was classified for open surgery intervention- partial cricotracheal resection with reconstruction (CTR). Latest improvements about construction of endotracheal tube and rules of procedures reduced frequency of subglottic stenosis to less than 1%.[11] There are no statistics or medical reports about tracheal atresia caused by intubation or Lyell’s syndrome. This situation forced us to recognise Lyell’s syndrome and endotracheal intubation overlapping on each other as a cause of medical state of our patient.

Background and ObjectivesSince the onset of the COVID-19 pandemic, a growing number of reports have described cases of acute disseminated encephalomyelitis (ADEM) and acute hemorrhagic leukoencephalitis (AHLE) following infection with COVID-19. Given their relatively rare occurrence, the primary objective of this systematic review was to synthesize their clinical features, response to treatments, and clinical outcomes to better understand the nature of this neurologic consequence of COVID-19 infection.MethodsPatients with a history of COVID-19 infection were included if their reports provided adequate detail to confirm a diagnosis of ADEM or AHLE by virtue of clinical features, radiographic abnormalities, and histopathologic findings. Cases purported to be secondary to vaccination against COVID-19 or occurring in the context of a preexisting relapsing CNS demyelinating disease were excluded. Case reports and series were identified via PubMed on May 17, 2021, and 4 additional cases from the authors' hospital files supplemented the systematic review of the literature. Summary statistics were used to describe variables using a complete case analysis approach.ResultsForty-six patients (28 men, median age 49.5 years, 1/3 >50 years old) were analyzed, derived from 26 case reports or series originating from 8 countries alongside 4 patient cases from the authors' hospital files. COVID-19 infection was laboratory confirmed in 91% of cases, and infection severity necessitated intensive care in 67%. ADEM occurred in 31 cases, whereas AHLE occurred in 15, with a median presenting nadir modified Rankin Scale score of 5 (bedridden). Anti-MOG seropositivity was rare (1/15 patients tested). Noninflammatory CSF was present in 30%. Hemorrhage on brain MRI was identified in 42%. Seventy percent received immunomodulatory treatments, most commonly steroids, IV immunoglobulins, or plasmapheresis. The final mRS score was ≥4 in 64% of patients with adequate follow-up information, including 32% who died.DiscussionIn contrast to ADEM cases from the prepandemic era, reported post–COVID-19 ADEM and AHLE cases were often advanced in age at onset, experienced severe antecedent infection, displayed an unusually high rate of hemorrhage on neuroimaging, and routinely had poor neurologic outcomes, including a high mortality rate. Findings are limited by nonstandardized reporting of cases, truncated follow-up information, and presumed publication bias.

Abstract Background Acanthamoeba is a free-living ameba found worldwide in soil and water that can cause severe illness. Transmission is thought to be through the skin, eyes, or lungs; Acanthamoeba can cause disseminated infection (Acanthamoeba disease) in addition to the more widely recognized Acanthamoeba keratitis. Infections however are rare, and only case reports or small case series have been published. We review Acanthamoeba disease cases from the Centers for Disease Control and Prevention (CDC) free-living ameba registry to characterize the disease in the United States. Methods CDC maintains a free-living ameba (FLA) registry of laboratory-confirmed Acanthamoeba cases (excluding keratitis) sourced from published case reports, CDC case report forms, and CDC laboratory results. SAS© version 9.4 software was used to calculate descriptive statistics and frequencies. Results We identified 163 cases of Acanthamoeba disease between 1956 and 2018. Of cases with documented outcome, 85% were fatal (105/124). Most (88%) cases were in patients who were immunocompromised (136/155): 66 people living with HIV (of whom 49 were classified as having AIDS); 33 recipients of organ transplantation; 30 people diagnosed with malignancy. The most common manifestation of disease was encephalitis (49%). Other clinical presentations included cutaneous lesions (20%) and rhinosinusitis (6%); 40 cases involved multiple organ systems. Median patient age was 42 years (range 0–83 years). Males accounted for 71% (114/160) of cases. California (29) and Texas (14) had the most case reports; 30 other states reported cases. The source of exposure was unknown in most cases (75%); soil and water were documented in 14 and 17 cases, respectively. Conclusion Acanthamoeba disease in the United States is primarily characterized by encephalitis and cutaneous lesions that affect predominately immunocompromised individuals. Acanthamoeba as a cause of encephalitis in immunocompromised patients should be considered by clinicians, which may lead to earlier diagnosis and treatment. Abstract Background Gall bladder carcinoma (GBC) is the most common malignancy of the biliary tract. Being known for its geographical and racial variations, and compared with the global statistics, its incidence is higher in the Indian subcontinent, mainly in the northern and eastern regions, accounting for 80 to 95% of cases. Aims and Objectives This study was conducted to evaluate the clinic-pathological spectrum and expression of EGFR and HER-2/NEU in GBCs and to understand their relation to prognosis, paving the way for targeted therapies for better treatment outcomes and patient survival. Materials and Methods This is a prospective study performed in a tertiary care hospital in 30 resected specimens of GBC cases recorded in our Department of Pathology from November 2017 to November 2019. Clinical history including the radiological reports and demographic parameters were included in the study pro forma. Immunohistochemical (IHC) staining for EGFR and HER-2/NEU was performed on all the selected cases. Clinicopathologic parameters like age, sex, histologic type, perineural, and lymphovascular invasion were compared and correlated with EGFR and HER-2/NEU status. Results Expression of EGFR was found in 93.33% of cases, which showed a highly significant correlation with histological tumor type (p = 0.000). HER-2/NEU expression was found in 56.66% of cases, which also showed a significant correlation with histological tumour type (p = 0.021). We found most of the cases with strong EGFR immunoreactivity (3+) were poorly differentiated tumors and most of the cases showing weak immunoreactivity for EGFR (1+) were well-differentiated. Conversely, in case of HER-2/NEU immunoreactivity, strong staining (3+) was seen in well-differentiated tumors and weak staining (1+) in poorly differentiated tumors. A significant correlation was also found between EGFR and HER-2/NEU expression (p = 0.000) and between cholelithiasis and EGFR expression (p = 0.033). Conclusion EGFR is expressed in most cases of GBC. Its expression is more in poorly differentiated carcinomas as compared to the well-differentiated carcinomas, whereas HER-2/NEU expression is more in well-differentiated carcinomas. Therefore, they may serve as independent prognostic factors and also as targets for molecular therapy in GBCs.

Resumo: Esta investigação de mestrado é um estudo qualitativo multicêntrico, marcado pelo referencial da fenomenologia e da hermenêutica e tem como objetivo compreender como psiquiatras trabalhadores da rede de serviços públicos de saúde mental lidam com a dimensão da experiência particular de pessoas diagnosticadas como portadoras de esquizofrenia no desenho do cuidado. Temos como pressuposto que para promover o restabelecimento de pessoas com transtornos mentais graves e reduzir o estigma é necessário desenvolver modos de cuidar em que a própria experiência da doença não seja negada. Por isso este trabalho foi apoiado na noção de recovery, que no cenário internacional tem contribuído para valorizar a dimensão da experiência do usuário. Para cumprir nosso objetivo, além de realizar uma revisão sobre o conceito de recovery, a fim de melhor compreende-lo, realizamos grupos focais com psiquiatras dos municípios de Campinas e Rio de Janeiro, e através desses grupos pretendemos acessar a experiência dos psiquiatras na formulação do diagnóstico de esquizofrenia e proposição do tratamento. Através do material produzido neste estudo pudemos perceber que a formulação do diagnóstico de esquizofrenia exerce grande impacto sobre a experiência de cuidar do usuário. E a concepção de esquizofrenia como doença grave e crônica parece influenciar amplamente o tipo de assistência que estes profissionais irão ofertar, assim como as expectativas que terão em relação à vida futura do usuário. A idéia de tratamento dos psiquiatras parece permeada não pela idéia de recovery, mas pelos princípios e objetivos da reabilitação psicossocial em seu sentido mais ortopédico e normalizante, que tem grande compatibilidade com o modelo biomédico, e oferece poucas ferramentas de acesso ao mundo e à experiência do usuário
Abstract: This master research is a qualitative multiside study, marked by the reference of phenomenology and hermeneutics, and aims to understand how psychiatrists of community-based mental health services deal with the dimension of the particular experience of people diagnosed as suffering from schizophrenia in the caring process. We have assumed that to facilitate the recovery of people with severe mental disorders and to reduce stigma is necessary to develop ways of caring in which the very experience of illness is not ignored. Therefore this work was supported by the notion of "recovery", which in the international arena has contributed to enhance the meaning of the user experience. To accomplish our goal, besides doing a review on the concept of recovery in order to better understand it, we conducted focus groups with psychiatrists from Campinas and Rio de Janeiro. In these groups we intended to access to the experience of psychiatrists in formulating diagnosis of schizophrenia and treatment proposition. Based on the material produced in this study, we realized that the formulation of schizophrenia diagnostic has a major impact on the experience of caring user. And the concept of schizophrenia as a chronic and severe illness seems to largely influence the type of care these professionals will offer, as well as the expectations they have regarding the future life of the user. The psychiatrists? idea of treating seems to be permeated not by the idea of recovery, but by the principles and goals of psychosocial rehabilitation in its most orthopedic and normalizing sense, which has great compatibility with the biomedical model, and provides few tools to access the world and experience of the user

AbstractBackground and purpose: Brain arteriovenous malformations (AVM) are uncommon vascular lesions with the risk of hemorrhage, epileptic seizures, neurological deficits, and headache. Comparing the risks of the natural history and that of preventive treatment, a recent study has found observation more beneficial than treatment for unruptured AVMs. This study, however, did not consider the long-term impact of carrying a brain AVM on everyday activities. In this study we analyzed the Quality Of Life (QOL) of patients with untreated AVMs, a measure increasingly used in clinical trials to asses this kind of impact.Methods: We enrolled 36 patients with unruptured, untreated brain AVM from our hospital database and measured their QOL retrospectively using the EQ-5D-5L questionnaire. As a control group we used the results of the Research Report, a nationwide study based on the quality of life of 5534 healthy Hungarians in 2002. Due to the low number of cases, statistical analysis could not be made.Results: Headache proved to be the most common AVM-related sign in our cohort (40%, n = 17), with a female predominance; neurological deficit was detected in 33% (n = 14), while epileptic seizures occurred in 26% (n = 11), more commonly affecting male subjects. Anxiety and discomfort seemed to be the most prevalent influencing factors on QOL, especially in the youngest age group (18–34 years). Female subjects showed a greater dependence than men in all age groups, though males had a more significant impairment in their usual activities. Older patients were affected more significantly in their self-care and usual activities compared with the younger population.Conclusions: Untreated AVMs have a significant negative impact on patients carrying unruptured brain AVMs, as proved by QOL assessment. Beside neurological deficits, this impact should also be considered in the therapeutic decision.

Abstract FHM is a rare, autosomal dominant form of migraine associated with episodic attacks of hemiplegia, aphasia, hemianopia, confusion, and sometimes coma, all of which may last several weeks and then resolve. If deficits persist, as in this case, a migrainous infarct is possible. Patients with FHM also have typical attacks of migraine with aura without hemiplegia. FHM is associated with nine different mutations in the CACNA1A region of chromosome 19p13, and one of these can be found in approximately 50% of patients. Recently, mutations in the ATP1A2 gene that encodes the α2 subunit of the sodium-potassium pump linked to chromosome 1q23 have been found in families with FHM. The CACNA1A gene encodes for the α1A subunit of a P/Q-type calcium channel that is expressed most densely in the cerebellum. Mutations of this gene are also associated with episodic ataxia type 2 and spinocerebellar ataxia type 6. According to anecdotal reports, intranasal ketamine, rapid intravenous administration of verapamil, or acetazolamide may have thera-peutic benefit in acute attacks of FHM.

Pain has been defined as ‘‘an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage’’ (Lindblom et al., 1986). Table 5–1 contains definitions of terms commonly used to describe pain sensations (Merskey et al., 1986). Pain is the most common reason a patient presents to a physician for evaluation. The U.S. Center for Health Statistics found that 32.8% of the general population suffers from chronic pain symptoms (Magni et al., 1993). Many factors can influence patients’ reports of pain, including medical and psychiatric disorders, social circumstances, disease states, personality traits, memory of past pain experiences, and personal interpretations of the meaning of pain (Clark and Treisman, 2004). There is no simple algorithm for determining whether the cause of pain is psychologic or neurologic (Clark and Chodynicki, 2005). The clinical evaluation of patients complaining of pain should be comprehensive and incorporate the patient’s descriptions of pain (ie, location, intensity, duration, precipitants, ameliorators); observations of pain-related behaviors (eg, limping, guarding, moaning); descriptions of problems performing activities; and neurologic and psychiatric examinations (Clark and Cox, 2002). Post-herpetic neuralgia (PHN) is defined as pain persisting or recurring at the site of shingles at least 3 months after the onset of the acute varicella zoster viral rash. PHN occurs in about 10% of patients with acute herpes zoster. More than 50% of patients older than 65 years of age with shingles develop PHN, and it is more likely to occur in patients with cancer, diabetes mellitus, and immunosuppression. During the acute episode of shingles, characteristics such as more severe pain and rash, presence of sensory impairment, and higher levels of emotional distress are associated with developing PHN (Schmader, 2002). Most cases gradually improve, with only about 25% of patients with PHN experiencing pain 1 year after diagnosis. Approximately 15% of patients referred to pain clinics suffer from PHN. Early treatment of varicella zoster with low-dose amitriptyline (25–100mg QD) can reduce the prevalence of pain at 6 months by 50% (Bowsher, 1997).

A usability study of the platform e-planea from the University of Colima in Mexico is presented. This system allows the gathering of relevant information regarding the institutional management indicators. The usability evaluation focuses on three modules of the platform: Annual Reports, Undergraduate Statistics and High School Statistics. The study consists of two evaluation phases: the first one used a heuristic evaluation, and the second one applied the System Usability Scale (SUS) and the Technology Acceptance Model (TAM). The results showed high user satisfaction.

Neuroleptic malignant syndrome (NMS) is an uncommon, potentially fatal reaction to antipsychotic medications. Most cases occur in men between the ages of 20 and 50 because they have the highest proportion of antipsychotic consumption. The prevalence of NMS has been reported as 0.07% to 2.2%, but the published diagnostic criteria for NMS varied widely until the Diagnostic and Statistical Manual of Mental Disorders addressed the issue in 2013. NMS is characterized by a constellation of clinical symptoms and laboratory abnormalities, including hyperthermia, muscle rigidity, and increased serum creatine kinase levels. Patients may also present with tremor, altered mental status, autonomic dysfunction, and leukocytosis.

Abstract There is very little written about the incidence of these infections in general practice: concerning acute bacterial infections, one of the GP authors of this chapter, during 25 years of practice, recalls about five cases of acute osteomyclitis in children, two adult cases of tuberculous spinal infection, and one acute and severe infection in a knee joint following an insect bite. Conversations with colleagues with equivalent experience also revealed answen of a similar order of magnitude; the reader may like to make comparable inquiries! Morbidity statistics from general practice confirm our clinical impression and report incidence rates for all bone and joint infections of l per IO 000 years at risk.

Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) is an unknown beta coronavirus that comes under the B genus, which causes Coronavirus Disease 2019 (COVID-19), a declared universal epidemic, posing a serious menace to human health irrespective of the nationality. According to the World Health Organization (WHO) statistics, as of September 10, 2021, there were 223,022,539 positive cases of COVID-19 with 4,602,883 fatalities reported worldwide. A total of 5,352,927,297 vaccine doses have been facilitated since September 5, 2021. This pandemic has become a ravaging illness because of its highly contractible nature and mutations. Many types of research in diverse fields of science have been initiated to suppress the effects and manage the havoc. Artificial Intelligence (AI) is classified as a subdomain of science, which most certainly contributed to numerous applications in confronting the present state at a broader level. In this chapter, we have tried to explore state-of-the-art AI techniques implemented in the perspective of COVID-19 across multiple subjects of concern. The AI approaches are utilized in the treatment, diagnosis, prediction of recovery, severity and mortality of patients, chest X-Ray and computed tomography-based analysis, pandemic prediction, its control and management, pharmaceutical research, COVID-19 text corpus processing, and virus apprehension. Thus, the comprehension of various applications is meant to enlighten the status of AI in this pandemonium. Finally, we conclude with some suggestions and remarks to tackle the disaster in an improved way.

Two systems faculty from a university was contracted to perform an analysis of information systems at the Algos Center, a small non-profit charitable organization which provides support for children and their families facing losses. The Center specifically requested help in integrating its fundraising and accounting software packages or exploring alternative software solutions. It also needed to generate reports required by the board of directors and United Way. Finally, the Center requested help in designing a family database to capture and track statistics about the families receiving services. As the team analyzed the Center, they discovered many underlying issues that would form part of their recommendations to the staff. This case makes two contributions. First, it reveals many problems facing small non-profit organizations, which primarily expend their resources on mission-critical activities, and allows readers to supply possible courses of action. Second, it provides an opportunity to evaluate how a consulting experience was handled and to make recommendations to ensure successful project implementation.

Objetives: To begin with, this systematic review arises from the interest to know about the appearance of psychotic and opioid symptomatology. Therefore, the main objective of this research is to establish the appearance of psychotic symptoms in the removal of oxycodone and tramadol. Methods: As far as the research procedure is concerned, a systematic review has been carried out which focuses on the relation of the psychotic symptoms caused by the withdrawal of buprenorphine. In this way, we have selected those scientific papers filed in the database PubMed looking for the key words: “Tramadol; Oxycodone” AND “psychosis, psychotic symptoms; schizophrenia”. Results and conclusions: In current literature, there are three publications dealing with clinic cases where patients suffered from psychotic symptoms after the removal of tramadol or oxycodone. Two of them are case reports about patients who presented psychotic symptoms after stopping these opioids. It is necessary to continue observing and reporting all cases of psychotic symptoms after an opioid withdrawal, as well as the potential antipsychotic effect of the drugs

This study analyzes the perceptions of a group of students on teaching strategies in a biostatistics course, assisted by ICT, during the pandemic. Ten PhD students in Public Health from a public university in Medellín-Colombia participated. Several strategies were implemented, including reading reports, critical reading, and analysis of real cases. The sources of information were dialogs with the students and an evaluation about the teaching strategies of the course. Students reported a positive perception of teaching strategies, which allowed them to be more active in their learning. The strategies implemented in the course contributed to positively transform the perception of students about biostatistics. En este estudio se analizan las percepciones de un grupo de estudiantes sobre las estrategias de enseñanza en un curso de bioestadística, asistido por TIC, durante la pandemia. Participaron diez estudiantes de un doctorado en Salud Pública de una universidad pública de Medellín-Colombia. Se implementaron diversas estrategias, entre ellas, reportes de lectura, lectura crítica y análisis de casos reales. Las fuentes de información fueron diálogos con los estudiantes y una evaluación acerca de las estrategias de enseñanza del curso. Los estudiantes reportaron una percepción positiva frente a las estrategias de enseñanza, lo que les permitió una posición más activa en su aprendizaje. Las estrategias implementadas en el curso contribuyeron a transformar positivamente la percepción de los estudiantes acerca de la bioestadística.

Most of the statistical information in newspapers, magazines, business reports and other publications consists of data that are summarized and presented in a way that it is easy to read and understand. These summaries of data, which can be tabular, graphical or numerical are known as descriptive statistics. In addition, the presentations in tables and graphs to summarize data, numerical descriptive statistics are also used. Within this context, is developing a web page with a virtual laboratory of the themes of descriptive statistics, which proposes a study guide which aims to reorient and upgrade the approach that must address the study of statistical methods, awakening the topics that were developed so that cases raised to develop learning environments that would enable it to meet the knowledge and manipulate it. With this philosophy, applets, web sites with access to real data, software for free use and in general resources used in the web 2.0, referring to a second generation in the history of the web based on user communities, that foster collaboration and fast exchange of information between them. The technology allows us to enjoy the following principle of the use of the modern statistics. It is not as important to memorize formulas or perform complex arithmetic calculations by hand. One can instead focus on results with any type of technology, to give practical meaning to results through critical thinking. This has to make the students really have to make an effort to understand and interpret the results

Introduction: The triple-negative breast cancer (TNBC) is one of the most aggressive types of breast cancer, corresponding to about 15% to 20% of invasive breast tumors. They are those tumors that in immunohistochemistry do not express homone receptors and epidermal growth factor type 2 (cerbB2). This tumor phenotype does not yield many treatment options, beyond standard chemotherapy, and within this context, the evidence of some markers of this type of tumor may contribute to the discovery of more effective types of treatment. Case report and Objectives: The aim of this study was to define predictive and prognostic factors in TNBC that could be related with a pathologic complete response after neoadjuvant chemotherapy treatment. Methods: A descriptive and retrospective study, a case series type, in women with TNBC who had underwent neoadjuvant chemotherapy and surgery at the Mastology Service of Maternidade Escola Assis Chateaubriand – Brazil - from May 2015 to June 2020. A statistical analysis was performed considering the 5% significance level. Results: From 108 women, only 47 were included in the study, with median age of 49 years (+14 years); about 30 (42.6%) had a family history of breast cancer in first or seconddegree relatives. About 44 (93.6%) cases were classified as invasive ductal tumor and grade II or III; the value of Ki67 greater than 14% was evidenced in 46 (97.9%) women and 26 (55.3%) had clinical stage III. Pathologic complete response to chemotherapy was evidenced in 16 (34%) cases, partial response in 13 (27.7%) and no response in 18 (38.3%) cases. The latter cases correspondeded to those who had stable or progression of disease. There was recurrence in 13 (27.7%) women, about 8 distant metastases, with the lungs as the most frequent site of metastasis followed by the brain. Eleven patients, about 23.4%, died. In the survival analysis of the studied population, the overall survival was 5.6 months and disease-free survival was 19.4 months. No association was observed in the study between the outcome of pathologic complete response to neoadjuvant chemotherapy and anatomopathological characteristics of the tumor. Conclusion: The results of this study did not show statistical significance to determine the possible predictive and prognostic factors for obtaining a complete clinical response to TNBC in a public reference service for the treatment of breast cancer, where there is no genetic signature, PDL1 status or access to differentiated treatment for such a heterogeneous tumor profil. This shows a need for further studies in order to understand this disease and for greater accessibility to high-cost exams and more effective treatments.

Objective: There are no publications on the impact of the COVID-19 pandemic on screening and clinical staging of breast cancer in women over 70 years of age. In Brazil, women over 70 years of age are not the target group for whom the Unified Health System (SUS) recommends mammography screening. This study aimed to evaluate the impact of the pandemic in Brazil on screening and clinical staging of breast cancer in women of this age group. Methodology: This is an ecological time-series study. Data and trends, as well as the staging of breast cancer in older women in the SUS, from 2013 to 2021, in Brazil and its regions were analyzed. The secondary database was created with information from the Outpatient Information System of the Informatics Department of the SUS, the Oncology Brazil Panel, the Brazilian Institute of Geography and Statistics, and the Supplementary Health Agency. Results: In 2018–2019, 16,035 cases of breast cancer were reported nationwide, representing a decrease of 3.75%, and 15,434 cases were reported in 2020–2021. Screening tended to decrease with APC -7.3 (CI -11.3 to -3.2; p<0.004). The proportion of advanced cases exceeded that of initial cases in Brazil overall. There was an increasing trend for stages III and IV, with a coefficient of determination (r2 ) of 0.86 (p <0.001) and a percentage increase of 9.4% of cases (p<0.001). Conclusion: During the pandemic, there was a significant decrease in the number of mammograms and a significant increase in older patients with advanced tumors. It is important to provide screening and treatment services for breast cancer in women over 70 years of age so as not to neglect the needs of these older women.

Background: Meningitis is a neuroinfection of multivariate etiologies, high morbidity and mortality and social impact. Objectives: To describe the epidemiological profile and analyze the trend and spatial distribution of meningitis in the state of Alagoas, Brazil, in the period 2008-2017. Methods: The ecological study is based on confirmed processes of meningitis and deaths from disease. Data were collected from the National Notification System. Clinical and epidemiological variables were analyzed, such as incidence and mortality rates. The regression model was used as a regression model for inflection points. A spatial sample was performed by a local empirical model, followed by global and local Moran statistics. Confidence interval of 95% and significance of 5% were considered. Results: 1365 cases were reported to Alagoas state, 59.6% male, 60.7% aged up to 19 years and 89.8% brown. Regarding the clinical profile, standing out: bacterial meningitis (24.3%), serum group Y (14.0%) and diagnosis by chemocytology (32.7%). Inflation decreased from 4.97/100 thousand in 2008 to 3.23/100 thousand in 2017 (AAPC -7.3%; p <0.001). Growth rate in growing trend from 10.3% in 2008 to 15.85% in 2017 (AAPC 4.7%; p <0.001). The capital city of Maceió presented a greater exchange of notified cases (40.7%). The highest incidence rates are those of Marechal Deodoro (36.00 / 100 thousand) and Maragogi (29.63/100 thousand). The priority municipalities were in the eastern region. Conclusions: The study showed a reduction in the incidence of Meningitis and heterogeneous spatial distribution in the state of Alagoas.

Abstract According to the 2020 report of the American Heart Association’s Heart & Stroke Statistics report, nearly 1,000 people are dying daily because of sudden out-of-hospital cardiac arrests and unfortunately, their survival rate is as low as 10%. Hypertrophic Cardiomyopathy (HCM), a relatively rare genetic heart disease is one of these diseases but finding the right patient for the implantation of ICD is still a research question. Implantation of cardioverter-defibrillator (ICD) can save the life of some of these patients. Due to the complexity of the identification of HCM patients, financial burdens, and the clinical risks involved in the ICD implantation procedure, HCM patients will go into a monitoring state before reaching the implantation trigger. Our study cohort shows about 82% of HCM deaths, did not have an ICD, which highlights the need to improve the pre-screening algorithms. In the current paper, we have proposed a new deep learning-based unsupervised clustering technique to facilitate the prioritization of patients to undergo ICD device implantation. This model uses over 900 echocardiographic measurements to find patients who benefit more from the ICD implantation procedure. Our model was trained and tested over 6 years of echo reports collected at Mayo Clinic. This model can be used as a decision support assistant for cardiologists in finding the right HCM patient when decision-making is hard.

In a group of 60 patients entering our hospital for completed stroke, within 72 hours after onset of symptoms, treatment with Kabi 2165 2x 2500 anti-Xa U s.c. was compared to placebo 2x s.c. in a double blind trial to test the assumption that Kabi 2165 could prevent DVT, without causing cerebral bleeding in the ischaemic area. The diagnosis of stroke was made on clinical grounds. A.CT-scan of the head was performed before entering the trial to exclude cerebral bleeding or tumor. Follow-up during a trial period of 14 days included a Fibrinogen scan - if positive followed by flebografy. After the trial period or when clinical deterioration occurred a CT-scan of the head was repeated. Before and during the trial period haematologic and coagulation data were obtained and will be reported. Obduction was obtained whenever possible.The patient groups were comparable, except for a slight preponderance of disturbed consciousness and atrial fibrillation in the Kabi 2165-treated group. This difference did not reach statistic significance. In the Kabi 2165 group there were 6 cases of DVT compared to 15 in the placebo group (p=0,05). In the Kabi 2165-treated group there were slightly more cases of cerebral bleeding and death during trial, respectively 4 versus 2 and 9 versus 4 (both NS). Cerebral bleeding occurred only in patients with a bloodpressure above 150/90 mmHg on entering.Although the patient group is still small, we like to conclude that in normotensive stroke patients Kabi 2165 2x 2500 anti-Xa U s.c. per 24 hours, is a safe method of DVT profylaxis.

Falling remains one of the chief causes of injury and reduced lifespan among people over 65 years of age. Clinical practice guidelines for primary care physicians instituted in 2004 have improved surveillance of seniors at significant risk of falling, resulting in a reduction in the annual prevalence of falls in people over 65, but still leaving more than one in 4 seniors subject to falling every year. Our research used a modified version of the multi-factorial risk screening (MFRS) and biomechanical measures of gait function, together with monthly follow-up over a 12-month period, to investigate the extent to which therapeutic intervention before patients enter the clinical fall-risk algorithm will reduce the risk of falls. A sample of 30 subjects were recruited in 3 age groups (65-74, 75-84, and 85+), and were randomly assigned to 3 treatment groups: control (initial data and MFRS with follow-up); managed (MFRS and biomechanical risk reported to subject, with follow-up); and mitigated (MFRS and biomechanical risks reported to geriatrician, with physical therapy and follow-up). Follow-up involved asking about falls, exercise type, and weekly exercise duration each month. MFRS and biomechanical measures were repeated after 12 months for the mitigated and managed groups.Physical therapy reduced the odds of falling by 8% among the youngest cohort, by 10% in the middle cohort, and by 3% in the oldest cohort, as measured by the logistic regression of MFRS scores. The mitigated group as a whole showed 16% lower prevalence of falling than the managed group. The mitigated group also maintained improved post-therapy gait mechanics by the end of the follow up period.In comparison, the managed group showed decreased gait function by the end of the follow up period. Managed subjects, who were given detailed reports about their risk of falling and how to reduce it, showed only modest changes in their overall risk of falling over the course of the year. Stratification of subjects into groups that exercised <90 minutes per week and >=90 minutes per week equally divided each of the three treatment groups. Reported exercise had no impact on fall risk. Problems with statistical power and exercise classification limited our ability to draw valid conclusions about the impact of exercise on the risk of falling in each sub-group.Our results indicate that use of a simple, clinically validated, multi-factorial risk assessment can identify people 65 and older who are at risk of falling before their physicians would otherwise recognize this. Targeted physical therapy can significantly reduce the risk of falling among this group. A more proactive clinical approach can prove more effective long-term than simply offering options for exercise, even when patients understand their risk of falling. Future work needs to investigate the dose-response and resilience of the response of specific types of exercise on gait biomechanics in different age groups.

The wine sector can adapt at a different pace and at different stages, in some cases, the sector can be subject to profound disruptions. In a changing environment, the reactions of the wine sector can be frightening and therefore even conservative, but at the same time, it is important to realize that these new technologies and capabilities we are gaining will provide invaluable opportunities, analytical and predictive tools, and speed up the pace of all processes. The paper presents the results of research on the importance and role of digitization in the wine sector as well as its support. The study is based on reports in the field; scientific papers related to the studied problem by researchers; articles by experts in the wine sector. Several research methods have been used in the elaboration of this work, such as monograph, analysis and synthesis, statistics, comparison, etc.

Objective: Pancreatic ductal adenocarcinoma (PDAC) has the best survival when detected early with 5-year survival near 40% for small, resectable PDAC. We evaluate the undiagnosed PDAC imaging features on routine CT and their impact on resectability. Methods: 76 of the screened 134 CTs from 1/1/2012 to 12/31/2018 using our tumor registry were obtained prior to PDAC diagnosis for other indications at least one month before presentation. Each cross-sectional study was reviewed for features of early PDAC: pancreatic mass, pancreatic ductal dilatation, perivascular/peripancreatic soft-tissue infiltration, omental lesions/ascites, and lymphadenopathy. When such features were detectible by the reviewing radiologists, the original CT readings were classified as concordant/discrepant. Descriptive statistics are reported for discrepant reads, tumor resectability, and tumor size. Results: Of the 76 cases from 46 unique subjects (30 male/16 female), 25 CTs (33%) had undetected PDAC imaging features: masses (15/19 unreported), ductal dilatation (16/20 unreported), and peripancreatic/perivascular soft-tissue infiltration (20/36 unreported). 63% of early PDAC features were not identified initially. One year before clinical diagnosis, 75-80% of the PDAC cases were resectable; at < 6 months before clinical diagnosis, only 29% were resectable. Conclusion: Improving early detection of key PDAC features on routine CT examinations can potentially improve patient outcomes.

Germany is a parliamentary democracy (The Federal Government, 2021) with two politically independent levels of 1) Federal (Bund) and 2) State (Länder or Bundesländer), and has a highly differentiated decentralized system of Government and administration (Deutsche Gesellschaft für Internationale Zusammenarbeit, 2021). The 16 states in Germany have their own government and legislations which means the federal authority has the responsibility of formulating policy, and the states are responsible for implementation (Franzke, 2020). The Federal Government supports the states in dealing with extraordinary danger and the Federal Ministry of the Interior (BMI) supports the states' operations with technology, expertise and other services (Federal Ministry of Interior, Building and Community, 2020). Due to the decentralized system of government, the Federal Government does not have the power to impose pandemic emergency measures. In the beginning of the COVID-19 pandemic, in order to slowdown the spread of coronavirus, on 16 March 2020 the federal and state governments attempted to harmonize joint guidelines, however one month later State governments started to act more independently (Franzke & Kuhlmann, 2021). In Germany, health insurance is compulsory and more than 11% of Germany’s GDP goes into healthcare spending (Federal Statistical Office, 2021). Health related policy at the federal level is the primary responsibility of the Federal Ministry of Health. This ministry supervises institutions dealing with higher level of public health including the Federal Institute for Drugs and Medical Devices (BfArM), the Paul-Ehrlich-Institute (PEI), the Robert Koch Institute (RKI) and the Federal Centre for Health Education (Federal Ministry of Health, 2020). The first German National Pandemic Plan (NPP), published in 2005, comprises two parts. Part one, updated in 2017, provides a framework for the pandemic plans of the states and the implementation plans of the municipalities, and part two, updated in 2016, is the scientific part of the National Pandemic Plan (Robert Koch Institut, 2017). The joint Federal-State working group on pandemic planning was established in 2005. A pandemic plan for German citizens abroad was published by the German Foreign Office on its website in 2005 (Robert Koch Institut, 2017). In 2007, the federal and state Governments, under the joint leadership of the Federal Ministry of the Interior and the Federal Ministry of Health, simulated influenza pandemic exercise called LÜKEX 07, and trained cross-states and cross-department crisis management (Bundesanstalt Technisches Hilfswerk, 2007b). In 2017, within the context of the G20, Germany ran a health emergency simulation exercise with representatives from WHO and the World Bank to prepare for future pandemic events (Federal Ministry of Health et al., 2017). By the beginning of the COVID-19 pandemic, on 27 February 2020, a joint crisis team of the Federal Ministry of the Interior (BMI) and the Federal Ministry of Health (BMG) was established (Die Bundesregierung, 2020a). On 4 March 2020 RKI published a Supplement to the National Pandemic Plan for COVID-19 (Robert Koch Institut, 2020d), and on 28 March 2020, a law for the protection of the population in an epidemic situation of national scope (Infektionsschutzgesetz) came into force (Bundesgesundheitsministerium, 2020b). In the first early phase of the COVID-19 pandemic in 2020, Germany managed to slow down the speed of the outbreak but was less successful in dealing with the second phase. Coronavirus-related information and measures were communicated through various platforms including TV, radio, press conferences, federal and state government official homepages, social media and applications. In mid-March 2020, the federal and state governments implemented extensive measures nationwide for pandemic containment. Step by step, social distancing and shutdowns were enforced by all Federal States, involving closing schools, day-cares and kindergartens, pubs, restaurants, shops, prayer services, borders, and imposing a curfew. To support those affected financially by the pandemic, the German Government provided large economic packages (Bundesministerium der Finanzen, 2020). These measures have adopted to the COVID-19 situation and changed over the pandemic. On 22 April 2020, the clinical trial of the corona vaccine was approved by Paul Ehrlich Institute, and in late December 2020, the distribution of vaccination in Germany and all other EU countries
