Résumé :
Access arrangements for birth parents of crown wards are a concern for child protection workers. Data on 84 crown wards were collected from 11 in care workers at a Children's Aid Society in Ontario, to determine whether best practice guidelines were being observed. Best practice derived from literature was operationalized as: (1) inclusion of birth parents and others in decisions affecting the child, and (2) connectivity ---maintaining family ties. Regression analyses were performed to identify predictors of inclusion and connectivity scores. Results showed three significant predictors of inclusion---worker knowledge, years in child welfare, and birth parents' source of income---and two predictors of connectivity---worker knowledge and parents' source of income. Findings suggest that agency training and support of workers can have a positive effect on implementation of best practice. The effect of income source is a puzzle, as income from employment was associated with lower inclusion scores, but higher connectivity scores.

Texte intégral
Résumé :
Within the Brazilian strategy to reform out-of-home care services, efforts have been directed to the prioritization of foster care over congregate care. This study draws from the perspective that childhood is socially construed, therefore child protection systems are dependent of notions and ideals of childhood, especially concerning the child deprived of parental care. Interviews with ten professionals involved in the implementation and execution of foster care services in different cities were conducted for this study. Thematic analysis of interviews was triangulated with the contents of National Guidelines for out-of-home care services and legal provisions with the aim of investigating how ideas on children deprived of parental care and their best interests are translated into practice. Three research questions guided the study: (1) how participants perceive the out-of-home care reform and the provisions of rights in different care modalities, especially regarding how children placed in different types of out-of-home care are perceived, (2) how participants view children’s best interests and which practices are believed to secure them and (3) how practitioners’ views relate to the contents of the National Guidelines. Five themes emerged in the analysis: a scientific and legal defense of (1) foster care as the most up to date form of out-of-home care, the prioritization of (2) children’s needs for individualized parental care, that (3) practitioners’ strong discretion is conditioned by foster families’ choices, that (4) institutionalization and group are detrimental to children, and the invocation that (5) children must be onboard. The study concludes that a clearer conceptualization of the rights and bests interests of children deprived of parental is required to better inform the reform of child protection and its practices, where consideration to children’s participation could be extended over acceptance to decisions.

Users of IoT devices are for many people today part of everyday life and more and more devices are connected to people's households. Smart homes have everything from refrigerators and surveillance cameras connected to a network and the internet. The problem with this is that many of these units do not have sufficient capacity or lack adequate security measures to protect themselves against potential attack vectors. Lack of security for devices in the smart home can cause the devices to be hacked and manipulated by attackers. Those who can protect the smart home from tampering with its IoT devices are the users in the smart home and the developers of the IoT devices. Although it is not always clear for whom the security measures apply to, which is something that this study aims to clarify. This literature study is based on existing security measures identified by previous research. It differs from previous research by mapping out which security measures and best practices that are applicable to users and developers to protect the smart home from being manipulated. Using a systematic literature search, 22 studies were selected to answer the study's question. To answer the study's research question, a mapping method is used. The method is used to map and identify which actions are recommended for both users and developers. Using the previous research, a framework is created to clarify what the user in the smart home can do and what actions developers can and should take. The framework also demonstrates a recommended order from the author in which the measures should be carried out.

Since the 1970s, geographers have interested in studying films. However, films are ironically neglected by scholars because of their popularity. Rather, geographers prefer studying literature, paintings, songs and magazines as texts. This study explores the (non)home space in Academy Award for Best Picture Winner Films. During 1929 to 2017, total 14 films that labeled “home”, “Family Realationships” in IMDb are cases analysed in this dissertation. This research aims to understand the roles, relationships and interaction among family members, representations of the home and other non-home spaces that mutually construct the meaning of the home in the films. The discussion in this research is divided into three main sections, which are ‘Interactions among family members ‘, ‘the construction and meaning of home space in films ‘, and ‘the discussion of non-home space’. Based on the analysis of selected 14 films, the look and meaning of home space in films change with time and society. Main research findings are as follow. First, individual family member may also have conflict with others and relevant consequnces come after. Second, the main characteristic of home is considered as a ‘private’ space. Family members can feel comfortable at home space, interacting with other members and friends. Third, the non-home space usually used for people’s social activity but it also reinforces the importance of home space. Based on geographical perspective, this study explores the meaning and representation of the home space by analyzing awarded best films in Oscar. Also, this reseacrh contributes to promoting the development of film stuides in Geography.

Résumé :
The Van Geet home near Denver, Colorado, demonstrates the successful integration of energy conservation measures and renewable energy supply in a beautiful, comfortable, energy-efficient, 295-m2 (3,176-ft2) off-grid home in a cold, sunny climate. Features include a tight envelope, energy-efficient appliances, passive solar heating (direct gain and Trombe wall), natural cooling, solar hot water, and photovoltaics. In addition to describing this house and its performance, this paper describes the recommended design process of (1) setting a goal for energy efficiency at the outset, (2) applying rules of thumb, and (3) using computer simulation to fine-tune the design. Performance monitoring and computer simulation are combined for the best possible analysis of energy performance. In this case, energy savings are estimated as 89% heating and cooling, 83% electrical, and nearly 100% domestic water heating. The heating and cooling energy use is 8.96 kJ/°C·day·m2 (0.44 Btu/°F·day·ft2).

Résumé :
Abstract If we learned anything from the year 2020, it is that we need to be more prepared for the unexpected. We need to be working to enable our business to be more resilient in the face of unexpected challenges. We strongly believe that for the industrial sector, the most effective way to enable resiliency is to ensure you have integrity in your operational technology (OT). The objective of this paper is to identify and manage the risk that arose from managing plants remotely. As a result of COVID-19, people started working and managing from home. While this needed to be done to keep businesses running, many risks were introduced as well. How to manage them effectively to reduce cyber risk to an acceptable level will be discussed. Industrial frameworks to identify security gaps, and thus risk, were considered, such as ISA-99/IEC-62443, NIST, ISO-27001, and Top CIS controls. New practices critical infrastructure followed to reduce infection rates were identified from interviews and surveys conducted by PAS, part of Hexagon, of our customers who work with critical infrastructure. These new practices were then compared to the industrial risk management framework to identify the severity of the threats. Once these were identified, mitigation plans were recommended to reduce the risk to an acceptable level. Because of this rapid shift to run the plant remotely, there was an over-provisioning of access in the early stages of the pandemic – i.e., giving more direct access to the industrial control system environment. This was not wise from a security standpoint, but the priority was to keep businesses up and running, so they were ready to take that risk. Now that some organizations have decided to continue with remote work, it is imperative to verify all remote access considers the least privileged access concept. Remote access is like a bridge that bypasses all the controls implemented. Having a remote access vulnerability will help bad actors break into the network and cause catastrophic damage. Though this paper focuses on remote access risk introduced by the COVID-19 pandemic, you can apply the findings to all remote access into critical infrastructure.

Résumé :
Automatic detection of obstructive sleep apnea (OSA) is in great demand. OSA is one of the most prevalent diseases of the current century and established comorbidity to Covid-19. OSA is characterized by complete or relative breathing pauses during sleep. According to medical observations, if OSA remained unrecognized and un-treated, it may lead to physical and mental complications. The gold standard of scoring OSA severity is the time-consuming and expensive method of polysomnography (PSG). The idea of online home-based surveillance of OSA is welcome. It serves as an effective way for spurred detection and reference of patients to sleep clinics. In addition, it can perform automatic control of the therapeutic/assistive devices. In this paper, several configurations for online OSA detection are proposed. The best configuration uses both ECG and SpO2 signals for feature extraction and MI analysis for feature reduction. Various methods of supervised machine learning are exploited for classification. Finally, to reach the best result, the most successful classifiers in sensitivity and specificity are combined in groups of three members with four different combination methods. The proposed method has advantages like limited use of biological signals, automatic detection, online working scheme, and uniform and acceptable performance (over 85%) in all the employed databases. These advantages have not been integrated in previous published methods.

Résumé :
The HIV/AIDS epidemic has meant that an increasing number of chronically ill people need ongoing assistance with care and support. Programs providing home-based care (HBC) services are a key component of the response to HIV/AIDS. However, few programs are using operations research, including cost studies, to decide what services to provide and how to structure their services. In 2004, the Horizons Program undertook a study of six HBC programs from different South African provinces to provide key information to NGOs, government ministries, donors, and the programs themselves to inform decisions about service delivery. The study analyzed the cost of HBC services, the best use of resources, and how well programs are able to meet the needs of beneficiaries and their families. The sample represents programs that operate in rural areas and informal settlements. This brief focuses on the coverage, organization, volume, and costs of the services and on findings from two of the methods of data collection: financial records and service statistics, and interviews with financial officers, program managers, and caregivers.

Management of High-Need, High-Cost Patients : A “Best Fit” Framework Synthesis, Realist Review, and Systematic Review. Résumé :
Background. In the United States, patients referred to as high-need, high-cost (HNHC) constitute a very small percentage of the patient population but account for a disproportionally high level of healthcare use and cost. Payers, health systems, and providers would like to improve the quality of care and health outcomes for HNHC patients and reduce their costly use of potentially preventable or modifiable healthcare services, including emergency department (ED) and hospital visits. Methods. We assessed evidence of criteria that identify HNHC patients (best fit framework synthesis); developed program theories on the relationship among contexts, mechanisms, and outcomes of interventions intended to change HNHC patient behaviors (realist review); and assessed the effectiveness of interventions (systematic review). We searched databases, gray literature, and other sources for evidence available from January 1, 2000, to March 4, 2021. We included quantitative and qualitative studies of HNHC patients (high healthcare use or cost) age 18 and over who received intervention services in a variety of settings. Results. We included 110 studies (117 articles). Consistent with our best fit framework, characteristics associated with HNHC include patient chronic clinical conditions, behavioral health factors including depression and substance use disorder, and social risk factors including homelessness and poverty. We also identified prior healthcare use and race as important predictors. We found limited evidence of approaches for distinguishing potentially preventable or modifiable high use from all high use. To understand how and why interventions work, we developed three program theories in our realist review that explain (1) targeting HNHC patients, (2) engaging HNHC patients, and (3) engaging care providers in these interventions. Theories identify the need for individualizing and tailoring services for HNHC patients and the importance of building trusting relationships. For our systematic review, we categorized evidence based on primary setting. We found that ED-, primary care–, and home-based care models result in reduced use of healthcare services (moderate to low strength of evidence [SOE]); ED, ambulatory intensive caring unit, and primary care-based models result in reduced costs (low SOE); and system-level transformation and telephonic/mail models do not result in changes in use or costs (low SOE). Conclusions. Patient characteristics can be used to identify patients who are potentially HNHC. Evidence focusing specifically on potentially preventable or modifiable high use was limited. Based on our program theories, we conclude that individualized and tailored patient engagement and resources to support care providers are critical to the success of interventions. Although we found evidence of intervention effectiveness in relation to cost and use, the studies identified in this review reported little information for determining why individual programs work, for whom, and when.
