Texte intégralAcute Lymphoblastic Leukemia (ALL) is the most common type of cancer in children. About 80% of the cases arises from precursor B cells (BCP-ALL), which abnormally accumulate as a consequence of genetic alterations associated to differentiation inhibition and abnormal expansion. Despite the 85% survival rate, a total of 10-15% of patients retains leukemic stem cells and their progenitors in the bone marrow (BM), thereby relapsing following treatment cessation. The importance of BM microenvironment for cancer progression has been widely recognized in recent years. In this study, we aimed to identify the crucial pathways involved in the bi-directional leukemia-stroma cross-talk that could be an attractive target for future antileukemic therapy. We focused our attention on the characterization of ActivinA, a TGF-β family member, within the BM leukemic niche. Here, we identified ActivinA as a new crucial factor exploited by leukemic cells to create a self-reinforcing niche: indeed, this molecule was highly expressed in the BM plasma of leukemic patients. Furthermore, we reported that BCP-ALL cells, along with the highly pro-inflammatory environment of leukemic BM, induced a strong increase in the molecule secretion by Mesenchymal Stromal Cells (MSCs). In accordance with its protumoral role in solid tumors, ActivinA strongly induced both random and CXCL12-driven migration of cells also in the context of BCP-ALL. We observed that ActivinA selectively stimulated these leukemic cell biological properties with a calcium- and actin polymerization-mediated mechanism as this molecule showed an opposite effect on Hematopoietic stem cells (HSCs). According to the literature, we found reduced CXCL12 levels in the leukemic BM, but ActivinA enhanced cell migration also towards suboptimal CXCL12 concentrations, suggesting a possible mechanism by which leukemic cells could persist in the BM niche, displacing healthy hematopoiesis. Our in vitro data about the pro-migratory and pro-invasive role of ActivinA were confirmed also in vivo. By using a xenograft mouse model of human BCP-ALL, we demonstrated the ability of ActivinA to enhance both BM engraftment and metastatic potential intro extra-medullary sites of leukemic cells. Notably, the regulation of calcium influx and cytoskeleton organization by ActivinA is an important process to stimulate also cell vesiculation. Recent studies have shown that cancer extracellular vesicles (EVs) can mediate cell-cell communication and potentially contribute to tumor progression. Therefore, we investigated whether ActivinA was able to influence vesiculation by leukemic cells. We demonstrated that ActivinA increased the production of both exosomes and MVs by BCP-ALL cells. We found that EVs transport the t(1;19) fusion transcript, typical of cells from which they originate. We then studied the biological effects by which ActivinA-induced leukemia EVs can actively promote BCP-ALL disease, focusing our attention on resistance to therapy. Firstly, we demonstrated that ActivinA significantly decreased the sensitivity of leukemic cells to the anti-leukemic drug Asparaginase (ASNase) which was re-stored by blocking ActivinA signaling. Interestingly, also ActivinA-induced leukemia EVs conferred resistance to leukemic cells. To understand the mechanism underlying EV chemoprotection, we explored their miRNA cargo and identified differentially expressed miRNAs induced by ActivinA treatment. Of these, miR-491-5p has been previously reported to be associated with ASNase chemoresistance in childhood leukemia. The discovery of ActivinA signaling between BCP-ALL cells and MSCs adds significant insights into the mechanisms of communication in the leukemic niche. Moreover, ActivinA-induced leukemia EVs seem to play a crucial role in sustaining leukemic cells, by conferring them drug resistance. Our data provide a new concept to develop alternative therapeutic strategies that include targeting of the leukemic niche in BCP-ALL.

Texte intégralThe interest in realizing an in-depth analysis of the criminal responsibilities related to the failure of the activities of civil defence arises from the increase in criminal proceedings against workers of the Civil Defence. Given the phenomenon of versatility of the elements of crime risen from the social need for safety, the main purpose of the investigation is the identification of the conditions of statement of responsibility. The investigation is divided in two thematic areas intended to remodel the legislation of Civil Defence and describe the criteria of statement of responsibility. As a joining link, the risk, cross element of the whole analysis, is both the subject of the activities of civil defence and a factor of distorsion of the elements of crime and expression of the necessity to distinguish the perspectival view of the management of risk from that of judgement on the failure of previsional activity. The proposed solutions, placed in the legal reality of the criminal procedure of Aquila earthquake, have a perfect balance between the opposite values of safety and freedom, in order to grant a legal protection which cannot be excluded and must be placed within the borders of the guarantees of the criminal procedure.

Texte intégralThe relationship between Aloe main active components and plant age, specie and grow conditions has been investigated in Aloe barbadensis and A. arborescens, the commercially most used species of the genus. Aloin was mainly located in leaves outer green rid while beta-polysaccharides in inner parenchyma. Aloin concentration was higher in younger plants. Plants grown under decrising light intensities showed lower aloin and beta-polysaccharides concentrations. The content of these substances is improved by stress condition. The most antioxidant activity is located in the outer green rid of the leaves and it was slightly correlated to the total phenolic compounds content. Aloin and beta-polysaccharides stability in leaf homogenate was poor and temperature seemed to be quite more effective in reducing degradation. Antimicrobial effect against to phatogen microorganisms of anthraquinones aloin and alo-emodin and different Aloe extracts was confirmed. Prebiotic effects to Lactobacillus were discovered and a synergistic effect of several compounds was supposed. Systemic effect of aloin was explained by vivo experiments (on cows) because aloin is observed until 24 h after the oral administration of Aloe, thus metabolic and physiologic effects may be expected.

Texte intégralThe purpose of the research is to gain data about the Child Protection system in an Italian Region (Lombardia). The work developed in two parts: one concerned the organizational form and the other the work practice of the practitioners. In Lombardia there are a wide type of Child Protection organizations: the institutional mission is the same but the modalities to seek it are numerous, for example in association with other towns, through a new institution, or sovereignty The official documentation about this theme is lacking: in this project has been realized a complete reconnaissance of the organizational forms with the assistance of the data collected through a survey administered to 200 managers. The second part of the research aimed to represent the work practices of the fieldwork. Involving the 70% of the practitioners committed in the Child Protection work in this Region, the survey realized underlines the opinions (about the workload, the competences and the abilities, the level of stress,..) and the activities done during a work week. To collect the data (analyzed with a statistical program), the research tools are ideated especially.

Texte intégralThe aim of the study is to examine out-of-home placements for children in Child Protection System, both in foster and in residential care, in Emilia-Romagna, in the period 2008-2012. Data are collected by the regional information system and the sample includes 8,438 children. The work consists of three parts: the first is a descriptive analysis of the characteristics of the children and of the different interventions. In the second part, taking the perspective of family reunification, we intend to observe the actual path of the child, in terms of improvement or deterioration of conditions departure, beyond the return to the family. Some types of outcomes and some types of processes have been individuated. It was then performed the descriptive analysis of the outcomes and processes present in the sample. In the last part of the work, it was carried out a statistical analysis in order to identify significant correlations between the three variables represented by the types of placements, the outcomes and the processes and characteristics of the children, the prevalent problems of children and the problems prevailing household. Estimates made through double entry table have been corrected by linear regression models of probability.

Texte intégralStroke is one of the main public health concerns, because it is a leading cause of death and long-term disability in elderly people. Responding to the need for secondary and tertiary prevention strategies to improve function post stroke and prevent or delay subsequent strokes as well as other disabling conditions, Italy has pioneered development of Adaptive Physical Activity (APA) for stroke. The aim of this study is to assess whether the combination of Adapted Physical Activity (APA) and Therapeutic Patient Education (TPE) improves function and quality of life in stroke survivors. This non-randomized controlled study enrolled 229 patients with mild to moderate hemiparesis, 126 in the experimental group and 103 in the control group. Data were assessed in baseline and at 4 and 12 month follow-up. The outcome measures were 4-month change (that corresponds to 2 months post-intervention in the experimental group) in gait velocity endurance, Short Physical Performance Battery (SPPB), Berg Balance Scale, Barthel Index, Geriatric Depression Scale, the SF-12 and Caregiver Strain Index. Gait endurance, physical performance, balance and the physical component of quality of life score increased significantly at 4 months in the APA group and remained stable in the control group. At 12 month follow-up, the experimental group had a significantly higher improvement on physical performance, quality of life and a lower number of fractures and readmissions compared to the control group. Our results confirm that it is feasible and potentially effective to implement APA programs for elderly patients after stroke and suggest that, when combined with TPE, the effects of a post-rehabilitation APA program are enduring.

Texte intégralThe Probation Offices represent a significant field in which social workers are employed. They are responsible for the supervision and rehabilitation of people subjected to probation and community corrections outside the prison. Probation officers play a critical role in the criminal justice system in Italy and operate in a context of great change that inevitably affects professional work. The thesis presented here has the purpose of highlighting social workers’ activities and feelings in the criminal justice. To do this an exploratory research was conducted using two different tools. First, an online questionnaire was sent to all Italian Probation Offices, asking social workers to answer a series of questions regarding socio-demographic characteristics, functions and activities concerning with their work. In parallel, we asked a small group of social workers of Lombardy to keep an updated diary on their activities in two working days recording the perceived experiences and feelings. The data obtained were finally compared with the issues emerging in international researches concerning Probation Services. Key words: Italy, probation officers, feelings, activities

Texte intégralThe research has been inspired by the recent increased judicial focus on Civil protection activities. This phenomenon depends on the fact that the Italian legal system reacts to the failure of Civil protection duties essentially by using criminal law. This kind of reaction, as it has already been demonstrated in other cases, such as medical malpractice, has some contraindications, because it leads to defensive behaviours. The first part of the thesis is thus dedicated to the recognition of the judicial focus on civil protection activities and the relevant consequences. Then are studied the factors, both sociological and juridical, which have led to this recent exaggerated judicial control over Civil protection. The third chapter concerns the roles and duties of Italian Civil protection and then are examined some leading cases in this matter. In the last part it is conducted a recognition of the crucial problems that still affects Civil protection and that lead to defensive behaviours. It follows an analysis of the reform project of Civil protection and at last some personal proposal to solve the problem of “defensive civil protection” are given.
