Texte intégralTo test the multifactorial model of health psychology, we have studied the impact of certain biological, psychological and social factors on breast cancer evolution and quality of life. 75 breast cancer patients aged from 30 to 70 years and treated with neoadjuvant chemotherapy have been followed for two years, since breast cancer diagnosis. The data relative to sociobiographical and personality characteristics (predictors) were collected before the announcement of the diagnosis. The adjustment strategies (mediators) were evaluated three weeks after the diagnosis. Quality of life and evolution of the disease were estimated two years after the announcement of the diagnosis. Results shows that breast cancer patients have recourse to four qualitatively different coping strategies : perceived control, problem-focused strategy, hopelessness and perceived social support. High trait anxiety, unfavorable body perception, presence of progesterone receptors are associated with the perceived control strategy. High trait anxiety, favorable body perception, patient's poor health and inflammatory tumor are linked to hopelessness. High number of life events and presence of estrogen receptors predicted the use of problem-focused strategy. Separation or husband's loss and a high histologic grade were associated with perceived social support. Furthermore, results shows that a large number of children, high trait anxiety, patient's poor health, inflammatory tumor, absence of menopause and hopelessness reaction to the diagnosis, perceived control and problem-focused strategies predicted a poor quality of life. Absence of previous psychiatric problems, widowhood, inflammatory tumor and hopelessness are associated with an unfavorable evolution of the disease. The results also indicate that there is a mediator effect of coping strategies between some predictors and quality of life

Texte intégralThe judicial witness is a third party in the lawsuit whose duty is to appear, take oath and give evidence on everything he saw, heard or learned about the dispute. From then on, two interests - public and private - are in confrontation : the personality rights of the witness against the rights over demonstration of the truth and freedom of information. The pursuit of an issue to the dispute goes through the study of the personality rights concept followed by a viewpoint examination of the engagement of the witness' private life. These facts lead us to propose the procedural loyalty principle as a global rule of participation of the various interveners : the judicial employees, the parties, the medias and the third parties. For the latter, this means the guarantee to be respected as witnesses but also as people according to their handicaps or own qualities, in return for the execution of their citizen's duty. Thus, the necessity to adopt this behaviour standard, for oneself and the others, could give birth to the set up of real legal rules, of general significance and subjected to penalties. Ethical and moral principles serve then the utilitarian doctrine in favour of the free development of the individual personality, the justice and the right to information

Texte intégralFor several years, research has suggested that executive functions (EF), as well as some temperament dimensions and personality traits involved in the regulation of both emotion and behaviors, are differentially linked to symptoms of attention deficit hyperactivity disorder (ADHD). The main aim of this study is to examine the relationships between EF, temperament dimensions, personality traits, and ADHD symptoms in a clinical and a nonclinical sample of adolescents. 118 adolescents (75 ADHD, 68% males) aged 12 to 16 and their parents and teachers underwent a comprehensive diagnostic assessment including ADHD clinical interview, ADHD rating scales and a measure of intellectual ability. Performance-based tests, EF behavior rating scales, and temperament and personality measures were also included. Study I showed that ADHD adolescents perform worse than controls on cognitive performance-based EF measures and behavioral EF ratings. The predominantly inattentive (ADHD-I) and combined (ADHD-C) presentations did not differ in terms of task performance. The performance-based tests and EF rating scales distinguished well between ADHD and control groups. Regression analysis showed that neuropsychological tests were not very good predictors of behavior rating scale outcomes in ADHD and control groups. Study II indicated that the degree of concordance between informants (parents, teachers and self-reports) in the behavioral EF ratings ranges between poor and medium for the ADHD group, and between moderate and good for controls. The Bland and Altman graphical method, which allows better visualization of the extent of agreement between informants showed that informants agreed on the presence of difficulties in the EF rating scales, but disagreed regarding their frequency and intensity in ADHD adolescents. Study III revealed significant differences between ADHD and control groups on cognitive EF and temperamental dimensions. Compared with controls, the ADHD group scored lower on measures comprising EF of working memory, planning and inhibition and Effortful control dimension, and higher on Surgency and Negative affectivity dimensions. These two types of measure differentiated well between ADHD and control groups. Mediation analysis showed that effortful control had a mediating role on the relations between cognitive EF and inattentive symptoms. Study IV found that the ADHD group scored lower on measures of cognitive EF (working memory, planning and inhibition) than controls, as well as on the personality traits of Conscientiousness, Openness and Agreeableness, and scored higher than controls on Neuroticism. These two types of measure differentiated well between ADHD and control groups. Mediation analysis showed that cognitive EF explained the relationship between both Conscientiousness and Openness personality traits and inattentive symptoms. In summary, the results highlight complex relationships between cognitive and behavioral aspects of EF, temperamental dimensions, personality traits and ADHD. Differential relationships between EF deficits, low levels of effortful control, Conscientiousness and Openness, and the expression of ADHD symptoms during adolescence are also found. These complex relationships illustrate the value of an accurate diagnosis and a comprehensive approach to the treatment of ADHD adolescents.

Texte intégralMyotonic Dystrophy type 1 (DM1) is a neuromuscular disease with multiple impairments leading to blunted affect, apathy, hypersomnia, fatigue, social cognition deficit and theory of mind deficit. In this research, personality traits, coping, and emotion regulation of 60 DM1 patients were assessed. All this information will help us design DM1 adapted psychological care.Regarding personality, our main result is that patients show similar N scores to the healthy control group despite our expectations (high scores in relation with the severity of the disease and its complications). In the light of our coping results, it seems that DM1 patients are using a large variety of coping strategies. However, apathy and reduced motivation constitute obstacles for coping strategies. Finally, apathy and fatigue do not influence emotion regulation in our sample DM1. Furthermore, Cognitive reevaluation strategy seems preserved from the disease’s consequences. This strategy might be an important advantage in the preservation of quality of life in DM1, despite the disease progression. A DM1 specific Cognitive Behavioral Therapy showed promising results. Other psychotherapeutic approaches could be explored, namely Acceptance and Commitment Therapy

Texte intégralThis thesis proposes an effort to better characterize alexithymia in people with recent-onset schizophrenia (SZ) compared to non clinical controls through three major steps. The first step is to critically review the previous studies on alexithymia in SZ (n = 10) which, despite important conceptual variability and methodological caveats, highlight high severity of alexithymic features that are relatively independent from major SZ symptoms and associated to symptoms of general psychopathology, and that can be reliably measured with the TAS-20. The second step involves the comparison of two groups (SZ n = 33, Controls n = 31) regarding alexithymic features and the relationships between alexithymia, mentalizing and social cognition. It reveals that despite a higher severity of alexithymia and a mentalizing impairment in people with SZ, there is no association between both constructs in this group, thus infirming our hypothesis. However there is a significant negative association between them in the control group. The third step, as an attempt to find an alternative measure to the TAS-20 questionnaire in people who are known to show poor insight and emotional awareness, involves the study of psychometric properties of the Observer Alexithymia Scale (OAS) when it is rated by mental health professionals among people with SZ (n = 33). It shows that the OAS displays good psychometric properties in general, though it is moderately to strongly correlated to negative symptoms of SZ. This is an important limit to its use in this clinical population, at least when it is rated by a mental health professional. Results could be more accurate if the OAS was rated by a close relative, as suggested for future research. Overall, the results from the current thesis support the existence of important difficulties in emotion processing in people with SZ, principally characterized by the alexithymic feature “difficulty identifying emotions”, that are not related to mentalizing nor to symptoms but associated to a diminished social functioning. Some explanations, such as the specificity of alexithymia measures and the possible presence of a particular type of alexithymia in people with SZ, are proposed.
