Background: The antiviral agent favipiravir is an RNA-dependent RNA polymerase (RdRp) inhibitor. Materials and Methods: We examined patients with a clinical, laboratory, and radiological diagnosis of severe coronavirus disease 2019 (COVID-19) pneumonia. We investigated the effect of administering enteral favipiravir at a 2 × 1,600 mg loading dose and 2 × 600 mg maintenance dose for 5 days in addition to the standard COVID-19 treatment. Results: In total, 180 patients, who were hospitalized at the Istanbul Tuzla State Hospital and received favipiravir treatment between March 20, 2020 and May 30, 2020, were examined. Of these, 47 patients died. Thirty-three of the patients who died were aged over 65 years (70%), indicating that fatality was higher in elderly patients. Most of those who died had at least one comorbidity. Of the 101 patients who initiated favipiravir within ≤3 days of hospitalization, 17 died (17%). Of the 79 patients who initiated favipiravir after >3 days of hospitalization, 30 died (38%) (P = 0.002). Conclusion: We found that initiation of favipiravir within the first 72 h after the onset of disease symptoms reduced fatality in patients with COVID-19.

This study focuses on the association between patient characteristics, which include both demographic and contextual factors, and patients' experiences with health care. The pre-existing literature provides rich information about patients' various demographics related to patient experience. Despite the abundance of empirical evidence showing that patients' demographics do affect how they perceive their health care. However, there is little to no empirical knowledge explaining the significance of such factors. As the existing literature points out the need for taking into contextual factors such as patient's beliefs, attitudes, skills that are pertinent to dealing with health care, my study proposes patient activation as such a contextual factor that explains the association between patient demographics and patient experience. Findings suggest that patient activation is a strong predictor of two patient experience measures: patients' rating of doctor-patient communication and their self-reported difficulties in getting needed care. However, it is also observed that the mediating effects of patient activation vary by the two dimensions of patient experiences. Though this study demonstrates that promoting patient activation may be able to normalize how patients report the quality of doctor-patient interaction, further research is needed to address access to care issues.

The Association of Cancer Development in Patients with Systemic Lupus Erythematosus by Rose Michelle Coley MPH, Walden University, 2011 BS, University of Mount Olive, 2008 Dissertation Submitted in Partial Fulfillment of the Requirements for the Degree of Doctor of Philosophy Public Health Walden University March 2016 Both cancer and autoimmune diseases have been associated with numerous factors that may independently lead to the development of either disease. When these factors overlap the difficulty in assessing association is compounded. The numerous factors that are thought to cause systemic lupus erythematosus (SLE), which leads to the development of cancer, makes the study of an association between the 2 diseases challenging. The purpose of this study was to examine whether the risk of cancer development increased in SLE patients compared to the risk in non-SLE patients. Researchers have not shown consistent relationships of cancer development in patients with SLE; however, consideration of the various factors that contribute to the diseases is necessary to measure an association between the 2 diseases. This study used the Clinical Practice Research database (CPRD), a large, population-based database to test the relationship between SLE and cancer. A matched retrospective cohort study among SLE (n=3025) and non-SLE (n=180555) patients was conducted using the propensity score methodology to help balance the differences between the comparison groups. The propensity score methodology created a similar distribution of observed baseline covariates between the 2 groups. With adjustment for age, the predictor variable of SLE indicates that a patient with SLE is still 2.7 times more likely to develop cancer than is a non-SLE patient. The study outcomes could promote positive social change by reinforcing current recommendations for cancer screenings in persons with SLE, which could enhance the ability to detect cancer early enough to decrease mortality because of cancer in persons with SLE.

Childhood maltreatment is extremely common in patients diagnosed with psychotic disorders. Moreover, it has been linked with impaired functioning of the Hypothalamic-Pituitary-Adrenal axis. Furthermore, abnormality of the HPA has been found in psychotic patients. Presence of childhood maltreatment could then explain why the HPA axis is dysfunctional in these subjects. Our objective was to clarify the role of childhood trauma in the cortisol profiles of psychotic patients. Thirty-one patients underwent assessments of childhood maltreatment. Diurnal cortisol and cortisol after a controlled psychosocial stress were also collected. Our results show that childhood trauma is associated with lower cortisol levels during the morning and during 24 hours. In men diagnosed with psychosis, childhood trauma is also associated with a higher cortisol response during psychosocial stress. This suggests an alteration of the HPA axis in psychotic patients, resulting from early trauma. Moreover, our results suggest that looking at specific types of childhood abuse may also be important.

Research shows that the interaction between the weather conditions and human health exists and this interaction has a very wide range, from subjective well-being to death. With respect to reactions to weather variations, people can be divided into weather-resistant and weather-sensitive. However, there is the lack of standardised tools to evaluate weather sensitivity. People with heart disease and people with sensitive nervous system are considered to be more sensitive to weather change than others. There are evidences that psychological distress and Type D personality are both associated with development and progression of coronary artery disease. Also, it is hypothesized that individual differences such as personality traits may have an effect on sensitivity to weather. The aim of this study was to examine and to evaluate associations between weather parameters and subjective well-being of patients with coronary artery disease. The objectives of the study: 1) to examine the validity of Type D personality assessment instrument used in this study; 2) to develop and test the psychometric properties of self-assessment diary for weather sensitivity in patients with coronary artery disease; 3) to evaluate associations between the subjective well-being of patients with coronary artery disease and the weather parameters; 4) to evaluate associations between anxiety, depression and personality and the subjective weather-related well-being in patients with coronary artery disease.

Tačiau standartizuotų instrumentų, kurie leistų įvertinti jautrumą orams ir atskirti jautrius orų pokyčiams asmenis, ypač skirtingų susirgimų grupėse, trūksta. Sergantieji širdies ligomis ir asmenys išgyvenantys psichologinį distresą priskiriami prie vienų iš jautriausių orų pokyčiams. Įrodyta, kad tiek psichologinis distresas, tiek asmenybės D tipas yra susiję su išeminės širdies ligos išsivystymu ir progresavimu. Be to, pastaraisiais metais keliamos hipotezės, kad individualūs skirtumai, tokie kaip asmenybės bruožai, taip pat gali įtakoti jautrumą orams. Darbo tikslas buvo išnagrinėti ir įvertinti sergančiųjų išemine širdies liga subjektyvios savijautos sąsajas su orais ir jos ryšį su asmenybės bruožais, nerimo ir depresijos simptomais. Darbo uždaviniai: 1) validizuoti tyrime naudojamą instrumentą asmenybės D tipui vertinti; 2) sukurti savikontrolės dienyną jautrumui orams įvertinti ir atlikti jo psichometrinę analizę; 3) nustatyti sąsajas tarp sergančiųjų išemine širdies liga subjektyvios savijautos ir orų, 4) įvertinti sergančiųjų išemine širdies liga subjektyvios orų sąlygotos savijautos sąsajas su nerimu, depresija ir asmenybės bruožais.

Background: Previous studies have shown that patients with obsessive-compulsive disorder (OCD) have elevated interleukin and chemokine levels in plasma. The purpose of this study was to investigate and validate whether a group of cytokines and chemokines are elevated in a cohort of young adult OCD patients. Methods: A total of 43 patients (11 male/32 female) and 45 controls (15 male/30 female) with OCD were included in the study. The subjects were assessed with the Structured Clinical Interview for DSM-IV Axis I Disorders- Clinical Version or Mini-International Neuropsychiatric Interview. The control group was screened with the Alcohol Use Disorders Identification Test. Proximity extension assay (PEA) was used to measure plasma levels of IL-6, IL-8, MIP-1α, and IL-10. Results: A factor analysis for the cytokines was performed and logistic regression analysis revealed that the cytokines as a group have a significant association for OCD (P=0.031, OR: 2.2) and IL-8 was the cytokine with the highest significance (P=0.007) for the patient group. Conclusion: These findings suggest that this group of cytokines are associated with OCD diagnosis and strengthens previous findings of immune activity in the etiology of OCD. Therefore cytokines and chemokines could have an active role in the etiology of OCD and PEA could be useful in the search for biomarkers.

AbstractHeart failure is one of the leading causes of hospitalization and rehospitalization in American hospitals, leading to high expenditures and increased medical risk for patients. The discharge location has a strong association with the risk of rehospitalization and mortality, which makes determining the most suitable discharge location for a patient a crucial task. So far, work regarding patient discharge classification is limited to the state of the patients at the end of the treatment, including statistical analysis and machine learning. However, the treatment process has not been considered yet. In this contribution, the methods of process outcome prediction are utilized to predict the discharge location for patients with heart failure by incorporating the patient’s department visits and measurements during the treatment process. This paper shows that, with the help of convolutional neural networks, an accuracy of 77% can be achieved for the hospital discharge classification of heart failure patients. The model has been trained and evaluated on the MIMIC-IV real-world dataset on hospitalizations in the US.

The outcome not only depends upon the type of fluid but also depends significantly on rate of fluid as well as amount of fluid administration. This chapter provides framework of fluid therapy in major trauma patients presenting after acute trauma.

Introduction. Dual diagnosis (DD) refers to the simultaneous diagnosis of a psychiatric disorder and a substance use disorder (SUD). The prevalence rate is considerably high in patients with schizophrenia and affective disorders; it predicts a more severe illness course, with decreased adherence to treatment and higher rates of hospitalization. As such, there is a growing demand for clinical guidelines and treatment consensus for these patients. In this retrospective analysis, we aimed to examine if and how prescription patterns in DD differ regarding psychiatric diagnosis and type of substance used. Methods. Data from patients with a DD diagnosis admitted at Lisbon’s Psychiatric Hospital Center from June to September 2021 was collected (n=94). Chi-square or Fisher tests were used to analyze associations between substance use and specific psychiatric disorders, along with number and class of medications prescribed. Results. Schizophrenia was the most frequent diagnosis (n=47). The most abused substances were alcohol (n=62) and cannabinoids (n=57). We found a statistically significant association between schizophrenia and cannabis misuse (p=0,006). A personality disorder diagnosis was also found to be associated to the misuse of cannabinoids (p=0,04) and cocaine (p=0,003). Finally, there was a statistically significant association between prescription of 2 or more drugs from different classes and a diagnosis of schizophrenia. No association was found between number/class of drugs, other psychiatric conditions or the type of substance misuse. Conclusion. Our study confirms well established associations between specific substance use and psychiatric conditions. However, no evidence of a specific drug prescription pattern of use in DD patient was apparent, which suggests the need for more studies on DD population and treatment outcomes.

Background and aims. Typical cognitive biases (irrational beliefs, cognitive distortions and erroneous perceptions) and high levels of impulsivity have been systematically reported among individuals with problem gambling. The objective of this study is to examine the role of the chronological age into the relationships between cognitive biases and impulsivity with the gambling disorder (GD) profile during adulthood. Methods. The sample analysed in this study included n=209 patients into the range age 18-77 yrs-old, recruited at the Pathological Gambling Outpatients Unit of the Bellvitge University Hospital (Barcelona). Results. Path-analysis showed a mediational link between chronological age, the presence of a comorbid depression or anxiety disorder, and the level of GD symptoms. Orthogonal polynomial contrasts showed a positive quadratic trend between the individuals’ age and cognitive distortions severity (higher impairing irrational beliefs obtained among younger and older patients, compared to middle-age patients). The sensation-seeking level showed a negative linear trend with the age: the older the patient’s age, the lower the score in this impulsivity domain. Among younger age patients gambling severity correlated only with cognitive bias levels, while during middle age patients gambling severity correlated with both cognitive bias and impulsivity levels. Within older age group fewer and poorer associations were found between cognitive bias and impulsivity with the accumulated debts due to the gambling activity, and the bets per gambling-episode. Conclusion. The results of this study could help in the development of reliable/valid assessment tools for GD, as well as for the design of precise/effective intervention plans and guidelines. These should include the patients’ age, the cognitive style, and the impulsivity levels, with the aim of providing precise and accurate tools to manage gambling problems.

Introduction: When migraine evolves from episodic to chronic form, it becomes more disabling, due to refractory treatment and the arising of comorbidities. Bruxism has already been associated with migraine in adults, with a bidirectional relationship between sleep bruxism and chronic migraine. This study aimed to assess whether sleep and wake bruxism are more prevalent in chronic migraine when compared to episodic migraine and also to establish possible clinical correlations with chronification. Methods: 210 patients were allocated to the study, 97 with episodic migraine (EM) and 113 with chronic migraine (CM). The patients were submitted to face-to-face interviews with a neurologist to confirm the diagnosis and fill in the scales: specific questionnaire for the diagnosis of sleep and wake bruxism, PHQ-9 (depression), GAD-7 (anxiety), Epworth Scale (sleepness), MIDAS and HIT-6 scales to assess the migraine disability and the headache impact on patients. Results: The prevalence of sleep and wake bruxism was similar in patients with EM versus CM (p=0.300 and p=0.238). The correlation of patients with both bruxism forms at the same time with the high scores on the migraine disability and the headache impact, was higher among patients with chronic migraine than in patients with chronic migraine. episodic migraine (p <0.001). Conclusion: Sleep and wake bruxism alone aren’t more prevalent in chronic migraine when compared to episodic migraine. In patients affected with both bruxism forms, bruxism only causes a greater impact and disability on individuals with chronic migraine.

Review question / Objective: The effect of radiotherapy on post-treatment weight is incompletely defined. We determined the associations between radiotherapy and follow-up obesity in patients with craniopharyngioma (CP). Eligibility criteria: The inclusion criteria were as follows: (1) Participants: Patients who were clinically diagnosed with craniopharyngioma; (2) Intervention/control: For the control group, patients have not received radiotherapy, and the patients of the intervention group received radiotherapy alone, or surgery combined radiotherapy, or radiotherapy combined adjuvant therapy; (3) Outcome: The outcome was postoperative BMI or postoperative obesity. We excluded studies with (1) in vitro or animal experiments; (2) not written in English; (3) conference abstracts, reviews, correspondence, and comments; (4) case reports that included less than 10 patients.

Review question / Objective: Previous studies have indicated a cardioprotective effect of tumor necrosis factor inhibitor (TNFi) therapy in adult patients with psoriasis (Pso). However, most were retrospective studies, and the association between cardiometabolic comorbidities and major adverse cardiovascular events (MACE) has not been validated in randomized controlled trials (RCTs). Condition being studied: Because the available evidence has recently increased, we performed the present updated meta-analysis and meta-regression of cohort studies and RCTs to evaluate whether TNFi therapy can decrease the risk of MACE among patients with Pso and to assess the associations between cardiometabolic comorbidities and MACE.

Objectives. To review the association between malnutrition and clinical outcomes among hospitalized patients, evaluate effectiveness of measurement tools for malnutrition on clinical outcomes, and assess effectiveness of hospital-initiated interventions for patients diagnosed with malnutrition. Data sources. We searched electronic databases (Embase®, MEDLINE®, PubMed®, and the Cochrane Library) from January 1, 2000, to June 3, 2021. We hand-searched reference lists of relevant studies and searched for unpublished studies in ClinicalTrials.gov. Review methods. Using predefined criteria and dual review, we selected (1) existing systematic reviews (SRs) to assess the association between malnutrition and clinical outcomes, (2) randomized and non-randomized studies to evaluate the effectiveness of malnutrition tools on clinical outcomes, and (3) randomized controlled trials (RCTs) to assess effectiveness of hospital-initiated treatments for malnutrition. Clinical outcomes of interest included mortality, length of stay, 30-day readmission, quality of life, functional status, activities of daily living, hospital acquired conditions, wound healing, and discharge disposition. When appropriate, we conducted meta-analysis to quantitatively summarize study findings; otherwise, data were narratively synthesized. When available, we used pooled estimates from existing SRs to determine the association between malnutrition and clinical outcomes, and assessed the strength of evidence. Results. Six existing SRs (including 43 unique studies) provided evidence on the association between malnutrition and clinical outcomes. Low to moderate strength of evidence (SOE) showed an association between malnutrition and increased hospital mortality and prolonged hospital length of stay. This association was observed across patients hospitalized for an acute medical event requiring intensive care unit care, heart failure, and cirrhosis. Literature searches found no studies that met inclusion criteria and assessed effectiveness of measurement tools. The primary reason studies did not meet inclusion criteria is because they lacked an appropriate control group. Moderate SOE from 11 RCTs found that hospital-initiated malnutrition interventions likely reduce mortality compared with usual care among hospitalized patients diagnosed with malnutrition. Low SOE indicated that hospital-initiated malnutrition interventions may also improve quality of life compared to usual care. Conclusions. Evidence shows an association between malnutrition and increased mortality and prolonged length of hospital stay among hospitalized patients identified as malnourished. However, the strength of this association varied depending on patient population and tool used to identify malnutrition. Evidence indicates malnutrition-focused hospital-initiated interventions likely reduce mortality and may improve quality of life compared to usual care among patients diagnosed with malnutrition. Research is needed to assess the clinical utility of measurement tools for malnutrition.

Condition being studied: Recent epidemiological studies suggest that metformin, the first-line medication used for T2D patients, may reduce the risk of asthma exacerbations. Therefore, this article aims to explore two research questions: first, whether metformin can reduce the incidence of asthma in patients with T2D, and second, whether T2D patients with concurrent asthma can safely use metformin to reduce adverse outcomes. This study may shed light on the efficacy of metformin as an adjunct therapy for asthma care in patients with co-morbid T2D.
