Paul Broca's famous case reports of language impairment following left hemisphere lesions were issued in the 1860s. Since that time, much has been written about the assessment, typology, and neural underpinnings of aphasia. The two books we had the pleasure of reading and reviewing, Assessment of Aphasia and Aphasia and its Therapy, present well thought out and thorough reviews of aphasia literature, the first focusing on assessment issues, the latter focusing more on issues of and theoretical approaches to treatment. The books are united in their insistence that the field of aphasia research, from assessment to rehabilitation, be grounded in theory and a defensible conceptual model. This overarching theme presents itself throughout the entire corpus of both texts. Just as the clinician conducts a thorough assessment before initiating treatment, so too will this review begin with the book focused predominantly on aphasia assessment, before moving on to aphasia therapy.

Left thalamic stroke in adults commonly results in thalamic aphasia, which is characterized by defects in lexical and semantic abilities associated with fluency and comprehension disabilities and intact word repetition. Although few cases have been reported, aphasia with similar features can also be present as a consequence of left-brain hemisphere stroke in children, which has usually a poorer prognosis if compared with that of the adult population. The paper reports the case of a 20-month-old female child that developed aphasia after an ischemic stroke that involved the left anteromedial thalamus. In these cases it is necessary to initiate an early rehabilitation treatment, possibly supported by augmentative and alternative communication techniques.

The non-fluent and agrammatic variant of Primary Progressive Aphasia (NFPPA) is characterized by reduced verbal production with deficits in building grammatically correct sentences, involving dysfunctions in syntactic and morphological levels of language. There are a growing number of studies about non-pharmacological alternatives focusing on the rehabilitation of functional aspects or specific cognitive impairments of each variant of PPA. This study reports a short-term treatment administered to a patient with NFPPA focusing on the production of sentences. The patient had significant reduction in verbal fluency, use of keywords, phrasal and grammatical simplifying as well as anomia. Using the method of errorless learning, six sessions were structured to stimulate the formation of sentences in the present and past with the cloze technique. The patient had improvement restricted to the strategy, with 100% accuracy on the trained phrases and generalization to untrained similar syntactic structure after training. These results persisted one month after the treatment.

Stroke is the third leading cause of death in the developed world and the primary cause of adult disability. The most common site of stroke is the middle cerebral artery (MCA), an artery that supplies a range of areas involved in both language and motor function. As a consequence, many stroke patients experience a combination of language and motor deficits. Indeed, those suffering from Broca’s aphasia have an 80% chance of also suffering hemiplegia. Despite the prevalence of multifaceted disability in patients, the current trend in both clinical trials and clinical practice is toward compartmentalization of dysfunction. In this article, we review evidence that aphasia and hemiplegia do not just coexist, but that they interact. We review a number of clinical reports describing how therapies for one type of deficit can improve recovery in the other and vice versa. We go on to describe how language deficits should be seen as a warning to clinicians that the patient is likely to experience motor impairment and slower motor recovery, aiding clinicians to optimize their choice of therapy. We explore these findings and offer a tentative link between language and arm function through their shared need for sequential action, which we term fluency. We propose that area BA44 (part of Broca’s area) acts as a hub for fluency in both movement and language, both in terms of production and comprehension.

Background. Aphasia is a common symptom in stroke patients, presenting with the impairment of spontaneous speech, repetition, naming, auditory comprehension, reading, and writing function. Multiple rehabilitation methods have been suggested for the recovery of poststroke aphasia, including medication treatment, behavioral therapy, and stimulation approach. Acupuncture has been proven to have a beneficial effect on improving speech functions in repetition, oral speech, reading, comprehension, and writing ability. Neuroimaging technology provides a visualized way to explore cerebral neural activity, which helps reveal the therapeutic effect of acupuncture therapy. In this systematic review, we aim to reveal and summarize the neuroimaging mechanism of acupuncture therapy on poststroke aphasia to provide the foundation for further study. Methods. Seven electronic databases were searched including PubMed, Web of Science, Embase, Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure, the Wanfang databases, and the Chinese Scientific Journal Database. After screening the studies according to the inclusion and exclusion criteria, we summarized the neuroimaging mechanism of acupuncture on poststroke aphasia, as well as the utilization of acupuncture therapy and the methodological characteristics. Result. After searching, 885 articles were retrieved. After removing the literature studies, animal studies, and case reports, 16 studies were included in the final analysis. For the acupuncture type, 10 studies used manual acupuncture and 5 studies used electroacupuncture, while body acupuncture (10 studies), scalp acupuncture (7 studies), and tongue acupuncture (8 studies) were applied for poststroke aphasia patients. Based on blood oxygen level-dependent (BOLD) and diffusion tensor imaging (DTI) technologies, 4 neuroimaging analysis methods were used including amplitude of low-frequency fluctuation (ALFF), regional homogeneity (ReHo), seed-based analysis, and independent component analysis (ICA). Two studies reported the instant acupuncture effect, and 14 studies reported the constant acupuncture’s effect on poststroke aphasia patients. 5 studies analyzed the correlation between the neuroimaging outcomes and the clinical language scales. Conclusion. In this systematic review, we found that the mechanism of acupuncture’s effect might be associated with the activation and functional connectivity of language-related brain areas, such as brain areas around Broca’s area and Wernicke’s area in the left inferior temporal gyrus, supramarginal gyrus, middle frontal gyrus, and inferior frontal gyrus. However, these studies were still in the preliminary stage. Multicenter randomized controlled trials (RCT) with large sample sizes were needed to verify current evidence, as well as to explore deeply the neuroimaging mechanisms of acupuncture’s effects.

Introduction: Dislocated lens can be subluxated (partial) or luxated (complete), which can cause visual impairment in children. Conservative visual rehabilitation is an option for luxated lens treatment. Purpose: This study reports the possibility for visual rehabilitation of bilateral of bilateral luxated lens in children. Case Report: A 3-year-old boy came to the outpatient clinic of Dr. Saiful Anwar General Hospital with complaints of blurred vision on both eyes. The child would always bring objects close to his eyes since the past year. There was no history of ocular injury nor development disorder. Uncorrected visual acuities with LEA symbol on both right and left eyes were 6/114. The best-corrected visual acuity (BCVA) of the right eye was 6/45 and left eye was 6/9 with both S+11.00. Iridodonesis and aphakic lenses were found during slit-lamp examination of both eyes. The intraocular pressures were 14.2 mmHg on the right eye and 17.3 mmHg on the left eye. The lenses were seen in the vitreous cavities during funduscopy and ultrasonography examination. After 1 month of using spectacles, the BCVA of the right eye was 6/18 and that of the left eye was 6/18 with the binocular BCVA was 6/15. Lensectomy with pars plana vitrectomy was planned to prevent complication. Conclusion: Conservative visual rehabilitation is important to prevent amblyopia in children with luxated lenses and surgical treatment is needed to prevent complication. In this patient, aphakic spectacles were given for visual rehabilitation pending operative treatment.

Marfan syndrome (MFS) is an inherited disorder that affects the heart, joints, skeleton, skin, and eyes. People living with MFS are described as tall, long, slender built in appearance, with arachnodactyly, chest wall deformities, and scoliosis. A long, narrow face with deep-set eyes, down-slanting palpebral fissures, flat cheekbones, and a small chin are the facial features often found in people with MFS. The onset varies from infancy to all ages with most cases being diagnosed in the first two decades of life. In children, ocular findings are microspherophakia, congenital/infantile glaucoma, high refractive error at a young age, uveitis, retinal detachments, and enophthalmos secondary to the absence of retrobulbar fat. The basic management procedures include topical dilating agents for chronic dilation of the eye to increase pupillary size for aphakic correction, thorough and careful refraction, the use of contact lenses and/or glasses, removal of the dislocated lens, cataract surgery and treatment of amblyopia. Low vision assessments were carried out on the patients. The approach to the management of the two patients, whose cases are being reported, was different for each case and differed from conventional low vision management. Spectacle corrections of OD -20.00DS (1.76LogMar 6/36 near: 0.80M@23cm) and OD +7.5.0DS (6/9 1M@12cm) were issued for Cases 1 & 2 respectively. 8X Telescope (6/9) and magnifiers (4X Handheld N5 and spectacle +16 DS 0.50m/ N5) were recommended for cases 1 and 2 respectively. Low vision and rehabilitation were found to maximize visual functioning and are reliable options for MFS.

Para além das variáveis clínicas e sociodemográficas existem concerteza importantes componentes individuais que desempenham um contributo importante no nível de insight apresentado por cada pessoa doente, por exemplo, o nível de inteligência, personalidade, cultura, experiências passadas, memória, etc. A natureza clínica, emocional e/ou intelectual do termo ajuda-nos a compreender a complexidade da dificuldade que existe na sua tradução e, inclusive, na sua compreensão. Daí que as definições atribuídas ao conceito sejam muito distintas e variem consoante a formação teórica do autor/investigador. Pretende-se, a partir dessa identificação/compreensão, promover a qualidade de vida destas pessoas através do desenvolvimento de novas aprendizagens que possibilitem uma cooperação activa. É igualmente fundamental ir ao encontro das capacidade intactas de maneira a possibilitar a aquisição de novos(s) comportamento(s) que tenham um impacte positivo nas queixas, sinais, sintomas, incapacidade e disfuncionalidade apresentados pelo/a utente. Uma vez que a própria conceptualização do termo traduzirá aquilo que se pretende avaliar,será efectuada uma reflexão detalhada acerca dos instrumentos e definições que têm sido mais utilizadas para explorar o insight nas psicoses.Procurei, no meu trabalho de investigação, realçar e promover a importância que cada sujeito, alvo de intervenção, desempenha ao longo do seu processo de recuperação e na prevenção de recaídas. No seguimento dos objectivos acima descritos, para além da revisão teórica efectuada ao fenómeno em termos de conceptualização e estudos desenvolvidos na área de investigação, foi,neste estudo, realizada a contribuição para a validação do instrumento “Assessment of Insight in Psychosis: a re-standartization of a New Scale” de Marková & Berrios (2003).O fenómeno de insight escolhido pela Insight Scale, relata menos as mudanças vividas em relação à doença mental, e mais a actual consciência e articulação de tais mudanças. Tendo como base uma abordagem psicopedagógica, o fenómeno do insight aqui explorado assentou numa perspectiva reabilitativa, actual e multidimensional, que fosse para além das dimensões clínicas tradicionais. Neste sentido é apresentada uma escala original, intitulada “Escala de Avaliação do Insight e Identificação das Necessidades em Pessoas com Psicose”, bem como um modelo de intervenção psicopedagógico breve, assente nos pressupostos descritos ao longo do trabalho.-----------------------------------------ABSTRACT: The importance of insight in people with mental illnesses was first studied in psychiatry, in the first decades of the 20th century, by people as important as Lewis (1934) and Jaspers (1959). However, this field of investigation was left unexplored for many years. Only in the last decade has this phenomenon become the object of numerous scientific investigations, having been given special attention by its investigators. For this reason a significant number of instruments for evalauting insight in psychotic disorders were developed. Since then many papers have been published, which has allowed for a more in depth knowledge on the subject. Therefore, in recent years, the concept of insight has been developed in an attempt to clarify its compexity. A once dichotomic phenomenon, described in terms of presence or absence, became considered multidimensional, which made the identification of different levels of insight and different dimensions possible. Current concepts categorize insight into five dimensions: the awareness of the patient in relation to his/her mental illness, the awareness of the patient in relation to the social consequences of his/her illness, the awareness of the need for treatment, the awareness of the symptoms and the explanation of those symptoms in relation to the illness. The lack of insight in psychiatry, in general terms, and as this phenomenon has been described, the lack of awareness of having a mental illness, represents one of the most common symptoms of schizophrenia and affects a big part of the population that suffer from this illness. It is estimated that bewteen 50 and 80 per cent of patients with schizophrenia do not believe that they are ill, which, consequently has a big impact in the process of adherence to treatment. It is still not possible, however, to identify all the factors that determine the lack of insight in schizophrenics. There are psychological, social and cultural influences that almost certainly play their role in the lack of insight registered in this pathology.Since the impact of scizophrenia is felt in many aspects of the individual’s life, its effective treatment should be directed at various levels, including the improvement of insight. One of the objectives of this study is to explore the relationship between the level of insight in psychosis and the clinical and sociodemographic variables, the psychopathology and its global functioning. As well as the clinical and sociodemographic variables, there are of course important individual components that contribute to the level of insight seen in each patient, for example, their level of inteligence, personality, culture, past experiences, memory, etc. The clinical, emotional and/or intelectual nature of the term helps us understand the difficulty that lies in its interpretation as well as in its comprehension. Therefore, the definitions attributed to the term are very different and vary according to the theoretical training of the investigator. It is intended, from this identification/understanding, to promote the quality of life of these people through the development of new findings that might enable an active cooperation. It is equally fundamental to observe their unimpaired capacities in order to enable the acquisition of new behaviour(s) that have a positive impact on the complaints, signs, symptoms, incapacity and disfunctioning seen in the patient.As the actual comprehension of the term explains what we intend to evaluate, a detailed reflection is made on the instruments and definitions that have been used the most to explore insight in psychosis.In this investigation I tried to underline and promote the importance that each subject, undergoing medical intervention, plays during his/her process of recovery and prevention of relapses. Considering the above mentioned objectives, as well as a theoretical review of the phenomenon in terms of conceptualization and investigative studies developed, this study contributed to the validation of the instrument.The insight phenomenon chosen by the “Insight Scale”, records less changes experienced in relation to the mental illness and more actual awareness and articulation of these changes. Based on a psychopedagogical approach, the insight phenomenon explored here settled on a rehabilitation, current and multidimensional perspective that would go beyond the traditional clinical dimensions. For this reason an original scale entitled “Insight Evaluation Scale and Need Identification in Psychosis Patients” is presented, as well as a psychopedagogical intervention model soon to be used with admitted patients based on the presuppositions described in this study.

Background: Case reports are presented on four Veterans, aged 29–46 years, who complained of chronic dizziness and/or postural instability following blast exposures. Two of the four individuals were diagnosed with mild traumatic brain injury and three of the four were exposed to multiple blasts. Comprehensive vestibular, balance, gait, audiometry and neuroimaging procedures were used to characterize their injuries.Case report: Vestibular assessment included videonystagmography, rotary chair and cervical and ocular vestibular evoked myogenic potentials. Balance and gait testing included the sensory organization test, preferred gait speed and the dynamic gait index. Audiometric studies included pure tone audiometry and middle-ear measurements. Neuroimaging procedures included high resolution structural magnetic resonance imaging, susceptibility-weighted imaging and diffusion-tensor imaging.Findings: Based on the neuroimaging and vestibular and balance test results, it was found that all individuals had diffuse axonal injuries and all had one or more micro-hemorrhages or vascular anomalies. Three of the four individuals had abnormal vestibular function, all had abnormally slow walking speeds and two had abnormal gait and balance dysfunction.Conclusion: The use of contemporary neuroimaging studies in conjunction with comprehensive vestibular and balance assessment provided a better understanding of the pathophysiology and pathoanatomy of dizziness following blast exposures than standard vestibular and balance testing alone.

Background: Vestibular function (specifically, horizontal semicircular canal function) can be assessed across a broad frequency range using several different techniques. The head impulse test is a qualitative test of horizontal semicircular canal function that can be completed at bedside. Recently, a new instrument (video head impulse test [vHIT]) has been developed to provide an objective assessment to the clinical test. Questions persist regarding how this test may be used in the overall vestibular test battery. Purpose: The purpose of this case report is to describe vestibular test results (vHIT, rotational testing, vestibular evoked myogenic potentials, and balance and gait performance) in an individual with a 100% unilateral caloric weakness who was asymptomatic for dizziness, vertigo or imbalance. Data Collection and/or Analysis: Comprehensive assessment was completed to evaluate vestibular function. Caloric irrigations, rotary chair testing, vHIT, and vestibular evoked myogenic potentials were completed. Results: A 100% left-sided unilateral caloric weakness was observed in an asymptomatic individual. vHIT produced normal gain with covert saccades. Conclusions: This case demonstrates the clinical usefulness of vHIT as a diagnostic tool and indicator of vestibular compensation and functional status.

Background: Traditional periodontal prosthesis refers to the treatment and restoration of periodontally involved teeth, with reduced periodontium support or mobility, to support fixed or removable prostheses and the rehabilitation of oral function. However, the advent and general acceptance of dental implants has altered our concepts and treatment planning in these advanced cases , but has also increased the complexity of the final treatment plan.This thesis addresses those issues and the role of dental implants in perio-prosthesis. Materials and Methods: A total of ten patients, using clinical and radiographic examination, either full edentulous, severely generalized chronic periodontitis, chronic plaque-induced gingivitis or generalized aggressive periodontitis was diagnosed. The patients, ranging in ages from 25 to 68 years (mean 41.8 years), three women and seven men, received basic periodontal therapy and either root separation/resection, tunnel preparation,conservative periodontal surgery, endodontic treatment, minor tooth movement, guided bone regeneration,ridge hard/soft tissue surgery,implant surgery,conventional prostheses,implant prostheses or telescopic denture.The time between initial examination and follow-up was 14 to 28 months(mean 1.9 years) Results: Ten patients were satisfied with the functions and esthetics of treatment outcome.The periodontal parameters and radiographic examination revealed improved condition , and all patients,with stable occlusion, are still in individual designed maintenance program.The clinical results, with the small number of individuals under investigation, showed these interdisplinarily collaborative treatments, combing natural teeth or dental implants in periodontal compromised patients, are welcoming and promising. Conclusions: This case series, with dynamic perio-prosthetic treatments, showed the results of occlusal rehabilitation for patients of reduced periodontal support or missing teeth and provided the experiences for futher studies.In addition, more work and longer follow-up time needs to be done on a larger group of individuals to verify the observations of this thesis.

From caries and endodontic treatment to biting collapse, enen to the full mouth reconstruction.It is a definite way to serve the treatment with diversity. In this thesis it mentioned about the situation which usually happen to periodontitis patient. Like tooth migration, occlusal trauma, poor crown-root ratio...else. According to interdisciplinary treatment concepts. Different treatment was given, which combine surgical or non-surgical periodontic treatment to regain the healthy gum, implant treatment to solve the problem of missing tooth, orthodontic treatment to correct the tooth arrangement and the prosthodontics treatment to do full mouth reconstruction. As the technique of 3D CBCT become more mature. It helps the treatment plan more complete before therapy, the follow-up more clear after the treatment. Not only improve the function and esthetics of the fixed or removalbe prosthesis. But also increase the success rate of longterm follow-up.

The technology of dentistry has taken a huge leap and improved tremendously especially in the skill aspect nowadays which makes implantology no longer limited to patient with single tooth replacement and benefits patient with multiple teeth missing and complete dentures patient. With the development through out the years of implant surgery, alveolar bone augmentation, and dental materials, the successful rate of implant surgery on challenging area has improved. Removable partial denture is not the only choice for patients with multiple teeth missing replacement. Patient becomes more satisfied with combination of fixed partial denture and implants. Periodontal disease is usually the first obstacle for dentists encountering with implant patients. One of the fundamental elements in order to have excellent result in implantology is a healthy periodontal dentition. Partial denture can be used for restoring and stabilizing both physiological and functional of teeth and surrounding tissues with one condition required; the periodontal disease needs to be effectively controlled and treated. There are nine cases of partial and full mouth rehabilitation which includes partial and full mouth periodontitis being presented. During the treatment period , four out of the nine patients underwent orthodontics treatment to improve teeth inclination and alignment in order to provide space to accommodate prosthetics.Eight patients were informed with periodontal treatment procedure with the aid of computerized tomography and implantology for better assessment in presurgical evaluation, surgery types, design in restoration, and interdisciplinary integration to obtain the most accurate result.

There is no doubt that bacteria are the main etiologic agents for periodontal disease. With the progression of periodontal destruction, the following syptoms may occur: moderate to severe gingival inflammation, increased tooth mobility, pathological migration, vertical occlusal dimension loss, improper crown to root ratio, furcation involvement, absence of keratinized gingiva, unaesthetic appearance, as well as difficulties in chewing causing by unstable occlusion. As far as the compromised periodontal tissues are concerned, in order to eliminate periodontal etiologic factors, properly fabricated prostheses are need to restore the occlusal function for easy maintance of oral hygiene. The long-term successes of perio-prosthetic treatment include proper case selection, fine periodontal conditioned abutment teeth, stability of occlusion and routine periodontal maintenance. This report contains 9 cases of clinical reports with variable periodontal conditions. According to clinical situations and patients’ requests, different prostheses were fabricated. All patients are satisfied with the result of treatments, however, long-term follow-up checks are essential.

The severely destroyed alveolar bone of the chronic severe periodontal teeth will atrophy even more quickly after the extraction of the teeth.This phenomenon often causes the difficulty of later prosthetics.Even if bone grafting and tissue grafting have been conducted many times at the same area,it is also hard to increase even half of the original height of alveolar bone .In order not to let patients suffer the pain from the operations and to improve the effectiveness of the operation,immediate implant placement into the socket after the extraction of the teeth to gain the height of alveolar bone is very important. The objective of the case report was to evaluate the preservation of the height of the alveolar bone after the extraction of the teeth with the chronic severe periodontal disease.The results of this report support the conclusion that periodontal infected site may be not a contraindication for immediate implant and even for severe peridontal teeth if appropriate administration of periodontal infection has performed before implant placement,as describe by Novaes Jr. and Novaes coworkers,such as antibiotic administration,meticulous cleaning,and alveolar debridement before surgery.In addition,primary flap closure is also desirable when placing immediate implants,and avoid exposure of the implant must be considered.

Patients with periodontitis often suffered from destruction of periodontal tissue, which leads to the pathological displacement or inclination of teeth. If combined with partial loss of teeth or loss of periodontal support, this situation will easily result in bite collapse. In addition to eradication of etiologies of periodontal destruction and reconstruction of the occlusal function, conditions of remaining teeth and maintenance of pateints’ oral hygiene should be brought in consideration. Therefore, the evaluation prior to treatment, the integration and intervention of different specialties, and the expectancy of the patient should all be prudently contemplated. In the present study of 9 cases, under cross-field full-treatment, with the use of dental implants to increase the periodontal support, selection of various periodontoal surgeries to improve the health of periodontium, provision of different designs of prostheses to stabilize occlusion and relieve trauma from occlusion, and with the help of orthodontic intervention to the meet the esthetic request, we can then step further to reach the goal of full-mouth rehabilitation. Besides, however, the periodically follow-ups and the maintenance of oral hygiene of the patients are the keys to promote the success rates of our treatments.

AbstractIn arid areas, flash floods represent one of the most severe hazards for people and infrastructure alike. The associated risks are compounded by increasing exposure and vulnerability through rapid and unregulated urbanization, poor infrastructure, and sociocultural factors, among other elements. This research explores the flash flooding risk in the Saudi Arabian city of Jeddah with a particular focus on the Abruq Ar Rughamah neighborhood, which experienced a destructive flood in November 2009 that resulted in 116 deaths and 1,200 families becoming homeless. The neighborhood is an interesting case study as it has two distinctive urban layouts representing planned and irregular settlement types. In this paper, the focus is placed on exposure to flash flood hazards using a geographic information system to study urban layouts, building types, and resident populations in conjunction with data from satellites, flood mapping studies, and topographic data. The results show that most of the study area is located along a natural flood path. The regions that were affected by the 2009 disaster received no comprehensive site rehabilitation. This paper concludes that it is important to develop a risk management strategy that includes limiting urban expansion in flood-prone areas and redesigning neighborhoods to increase flood resilience.

Abstract Almost from the first observations of language loss due to brain injury, clinicians have been concerned with recovery and rehabilitation. There have been reports of spontaneous or directed language recovery in aphasic patients since at least the sixteenth century (for historical review, see Benton, 1964; Sarno, 1981). It was not until immediately after World War II, however, that hospital units were developed for the rehabilitation of brain-injured patients. A few fundamental observations about aphasia were made in patients injured during World War I, but formal language therapy was not instituted then.

Abstract As Stated in Chapter 5, Cognitive neuropsychology fostered an approach to the study of aphasia that was radically different from the classic clinical approach that related verbal behaviors, such as comprehension and reading, to specific cerebral areas. Cognitive neuropsychologists assumed that cognitive tasks are not unitary but rather are the result of a series of cognitive processes, which they endeavored to clarify. Chapter 6 illustrated how a behavior such as naming a picture or reading a word is accomplished. The aim of this chapter is to establish whether and how such cognitive models of language processing have affected implementation of rehabilitation. Selected case studies of patients reportedly rehabilitated following the cognitive neuropsychological approach are considered, and the intervention procedures are compared to the more traditional clinical approaches described in Chapter 3.

Before World War I only sporadic and anecdotal treatment reports can be found. From World War I to World War II aphasia therapy remained a rather uncommon procedure, mostly developed in the German-speaking countries and essentially based on didactic principles (Howard & Hatfield, 1987). After World War II it gained popularity and began to be regularly practiced in other countries as well.

Abstract Writings on the history of aphasia naturally tend to be slanted toward the views of their authors. Some reveal a strong national prejudice, some emphasize the contributions of specific investigators and others have a particular theoretical bias. Especially recommended reviews are those of Freud (1891/1953), Head (1926), Weisenburg and McBride (1935/1964), Benton and Joynt (1960), Brain (1961), Quadfasel (1968), Hécaen and Albert (1978), Benson (1979a), Arbib, Caplan, and Marshall (1982), Lecours and colleagues (1983), and Goodglass (1993). The modern conception of aphasia can be said to have commenced with the 1861 presentation by Broca of a case of aphasia associated with a focal brain lesion. Acquired language loss following brain injury had been reported far earlier, however, and a critically important historical prodrome preceded the 1861 presentation. The surveys of pre-Broca reports by Benton and Joynt (1960), Ben ton (1964), Boller (1977), and others showed that acquired language disorders had been noted for many centuries. Benton and Joynt (1960) asserted that “almost all the clinical forms of aphasia—complete motor aphasia, paraphasia, jargon aphasia, agraphia and alexia—had been described before 1800.”

Despite technological advances in dentistry, negative feelings still prevail in the area, such as odontophobia, which the World Health Organization classifies into three stages. Patients with this condition often postpone treatments, cooperate little, and resort to palliative medications, compromising their oral health. This study reports the case of a patient who faced oral complications due to odontophobia, highlighting the rehabilitation treatment plan adopted, its phases and results obtained. The diagnosis of anxiety was made using the Dental Anxiety Scale (DAS), created by Corah in 1969. It is concluded that understanding the patient's general conditions and motivation to seek dental treatment is crucial for a proper diagnosis and to establish an effective treatment plan. The installation of the prostheses resulted in a significant improvement in the patient's self-esteem, social well-being, reduced anxiety and improvements in diet.

The present case history reports describes a clinical course from diagnosis to the functional and esthetic rehabilitation of a patient with squamous cell carcinoma of the tongue (SCCT). Radical approaches for SCCT often result in severe deficiencies and reduced quality of life. In this case the treatment included total glossectomy with flap pectoral muscle reconstruction and radiotherapy. An interdisciplinary team worked together to functional rehabilitation of the patient, carried in two phases: preoperative assessment and post-operative reconstruction. The findings from this case showed that functional and esthetic rehabilitation promote social rehabilitation in patients with SCCT treated with radical approaches.

The objective of this paper was to analyze the use of acupuncture in the motor rehabilitation of wild animals, through a survey of bibliographic references on the subject. The techniques observed were needling, laserpuncture, electroacupuncture and moxibustion. Case reports of motor disorders and the respective species affected were analyzed: pododermatitis (Haliaeetus leucocephalus and Oryctolagus cuniculus), paralysis (Geochelone carbonaria, Ramphastos toco, Spheniscus demersus, Vulpes vulpes), spinal trauma (Tupinambis merianae), intervertebral disc disease (Cynomys ludovicianus), degenerative joint disease (Haliaeetus leucocephalus), and fractures (Oryctolagus cuniculus). Acupuncture has proven effective in promoting analgesia and neuromodulation and reducing inflammation, being an important ally in the treatment of neuropathies and pathologies of the locomotor system. Thus, it is possible to verify that the use of acupuncture in locomotor diseases has optimized the time, recovery and cure process, besides providing a better quality of life to wild patients.

Context: Dermatomyositis (DM) is an idiopathic inflammatory myopathy, characterized by proximal skeletal muscle weakness, associated with a variety of characteristic skin manifestations. A form called Amiopathic Dermatomyositis (AD) is a condition that patients have cutaneous findings characteristic of DM without weakness and/or normal muscle enzymes. In turn, central nervous system (CNS) vasculitis refers to a wide spectrum of diseases that result in inflammation and destruction of the blood vessels of the brain, spinal cord and meninges. Case report: Female, 54 years old, reports for 3 months a progressive myalgia, paraparesis and edema in the proximal region of the lower limbs, evolving in the last weeks with erythematous and itchy lesions on the trunk (in shawl), feet and hands, with subsequent peeling of these (mechanic’s hands). During diagnostic investigation she presented epileptic seizures with behavioral arrest, right hemiparesis and motor aphasia. It evolved with a new stroke in the left frontoparietal region. Bilateral angiography showed points of constriction of segments of the middle cerebral artery bilaterally, compatible with CNS vasculitis. Therefore, with the exclusion of other secondary causes of arteritis, associated with the marker of positive inflammatory myopathy (Anti-Jo1) and CPK within the serum levels of normality, the diagnosis of the stroke secondary to AD was closed. Conclusions: Both CNS vasculitis and AD are rare conditions, characteristically diagnoses of exclusion, deserving to be remembered in clinical cases with singular manifestations, as reported in this case.

Introduction: Multiple System Atrophy (MSA) is a severe neurodegenerative disease that has parkinsonian, autonomic, cerebellar, and pyramidal features. It is prevalent at 3.4 to 4.9 cases per 100,000 population, and is most common after the age of 60. It is called Striatonigral Degeneration, with a predominance of parkinsonian symptoms; it is known as Olivopontocerebellar Atrophy, with cerebellar symptoms, and Shy-Drager syndrome with autonomic symptoms. Physiotherapy plays an important role in the rehabilitation of kinetic- functional impairments. Objective: To report the case of a volunteer with SMA in order to highlight the neurological findings for neurofunctional rehabilitation. Methods: This is an observational, cross-sectional case report study that was conducted at the Clínica Escola de Fisioterapia da Universidade de Franca (UNIFRAN), with the approval of the CEP (CAAE 83164918.2.0000.5495), of a 55-year-old male individual, referred to physiotherapy, diagnosed with SMA. Neurological evaluation was performed in the sector, of sensitivity, movement, tone, reflexes, motor coordination, balance, gait and function. Results: The individual reports that 4 years ago he started having difficulty walking and talking. He was diagnosed with Parkinson’s disease. He underwent a new evaluation, arriving at the current diagnosis. In the physical therapy evaluation dysautonomia, dysarthria, dysdiadochokinesia, dysmetria, hypertonia by rigidity, akinesia and bradykinesia, trunk flexion posture, balance deficit, tetraparesis and fetinated gait were observed. Conclusion: The impairments found impair the patient’s ability to perform several ADLs. This study emphasizes the value of neurofunctional physiotherapeutic semiology for an effective therapeutic treatment that promotes independence and quality of life possible for the individual with SMA.

Prototypes are an excellent tool to actualize an idea or a concept. As reported by different methodologies (Double Diamond, Design Thinking), prototypes enable designers to quickly iterate and learn from their designs. Their advantages are not limited to this aspect: by iteratively increasing the level of complexity from one version of the prototype to the next one, they can help enhance the skill set of a person through a learning-by-doing approach. This iterative process allows for the identification of unknown unknowns and can lead to the development of innovative solutions in various contexts, including biomedical engineering and rehabilitation. The multiple iterations and learning processes, inherent in the rehabilitation phase are a great asset when facing biomedical projects, which require multidisciplinary knowledge. The purpose of this paper is to provide supporting evidence for the importance of prototypes in biomedical design as tools to fill the skill gap, by presenting a real-world case study performed in an academic context. A team of university students developed a novel solution for post-stroke rehabilitation, DEUHR (Digital Exergame for Upper Limb and Hand Rehabilitation), based on innovative trends in biomedical technologies, such as motion tracking sensors and exergames. The paper reports the entire process for the design of the rehabilitation system, starting from the initial concept, prototypes, tests with the users, and final outcomes.The concept for DEUHR was first developed through a series of brainstorming sessions and analysis of existing rehabilitation solutions. The team then moved on to creating low-fidelity prototypes to test and refine the interaction design and gameplay mechanics of the exergame. These low-fidelity prototypes allowed the team to quickly iterate and gather feedback from potential users, including stroke patients and rehabilitation therapists. Based on the feedback received, the team made several iterations and improvements to the prototype, gradually increasing the level of complexity and fidelity. This iterative process of prototype development allowed the authors to improve the functionality and usability of DEUHR. To evaluate the usability and effectiveness of DEUHR, a high-fidelity prototype was created in the form of a tablet-based exergame for training. New tests with both the high-fidelity prototype connected with the wearable device were made with users in order to test the functionality and usability altogether.This paper outcome consisted of a demonstrator merging the three main components of the project (the exergame, the sensing unit, and the app) which were later tested on patients. Even if the case study concerned the design of health monitoring devices, it provided valuable insights with respect to the UX design process and allowed the team to grow and expand their initial skill set to meet the requirements needed. This experience led to a rethinking of the prototyping process as an enhancing and engaging opportunity to be featured in a learning environment – possibly extending its validity also outside of the biomedical field.

Case presentation: A 10-year-old male presented with a sudden onset of excessive daytime sleepiness (EDS) in the last 36 hours. Currently, the patient sleeps around 18 hours per day associated with headache and irritability. Patient received his second dose of Pfizer-BioNTech COVID-19 (coronavirus disease) vaccine. He has a past medical history of subtotal resection craniopharyngioma around one year ago. Neurology exam showed new deficits of motor aphasia and drowsy but aroused easily to voice. Axial FLAIR (FluidAttenuated Inversion Recovery) and T2-weighted magnetic resonance imaging (MRI) show symmetrical hyperintense lesions on the thalamus. Magnetic resonance venography shows a lack of flow in the vein of galen, straight, superior sagittal and bilateral transverse extending to the right sigmoid sinus. The findings above confirmed the diagnosis of cerebral venous thrombosis (CVT). Patient received enoxaparin for three months. Patients complete resolution of symptoms and a new MRI shows complete recanalization. Discussion: EDS is a common complaint in neurologic practice and has a wide spectrum of differential diagnoses. We describe an unusual cause of EDS caused by an extensive CVT in the bilateral thalamus. Bilateral lesions of the medial thalamus can cause hypersomnia and decrease consciousness. Infection, trauma, neurosurgery, malignancy and thyroid problems are risk factors of children CVT. Another risk factor to be considered is the COVID vaccination. In literature, there are more reports of thrombocytopenia and CVT is very rare. Although these factors are associated, they are not necessarily causality. Conclusion: Clinicians must be aware of CVT as a differential diagnosis of EDS, especially in the presence of sudden EDS onset or neuropsychological signs.

Introduction: Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL), is a non-atherosclerotic, nonamyloid, hereditary cerebral disease of small vessels and capillaries caused by mutations in the NOTCH-3 gene located on chromosome 19. The presence of granular osmophilic material (GOM) deposition in the smooth muscle cells of vessel walls is the pathological hallmark of arteriopathy in CADASIL. GOM deposits in the basal lamina of smooth muscle of small vessels are pathognomonic for CADASIL. The presence of GOM in capillary blood vessels of the skin and muscle in biopsy and genetic studies (NOTCH-3 analysis) plays a key diagnostic role. Biopsy tests have high specificity (up to 100%) and low sensitivity (less than 50%). The NOTCH-3 test has been proposed as the primary diagnostic approach, allowing detection of 90% of affected individuals. CADASIL has an estimated prevalence between 2 and 5 in 100,000, and the phenotypic study demonstrates different clinical symptoms in the course of the disease within the same family. The average age of onset of clinical symptoms varies between 48.3 years in men and 52.2 years in women. Characteristic symptoms of migraine, stroke or TIA (transient ischemic attack), behavioral changes, early and progressive cognitive changes associated with leukoencephalopathy on imaging studies. The large association of symptoms often causes the diagnosis of CADASIL to be delayed. In this case, we had an association of gait ataxia within the framework of motor alterations, demonstrating the wide range of symptomatology of the pathology. This case report presents a familial course that started outside the most prevalent age group in the studies described and with an atypical presentation in an affected generation. Case reports: Case 01: woman, 68 years old, started progressively forgetting to perform household activities after the age of fifty, associated with primarily generalized myoclonic epileptic seizures, evolving rapidly within five years to walking apraxia with the use of a wheelchair and tonic-tonic epileptic seizures. bilateral clonic disorders, comprehension aphasia and bradypsychism. Relatives report previous migraine without chronic aura and REM (rapid eye movement sleep) sleep behavior disorder (RMSD). On neurological examination, severe ataxia with bilateral dysdiadochokinesia associated with bilateral hypometric index-index. Bilateral ROT 4+/4+ with bilateral Hoffman and Babinski signs. MMSS and MMII with FGM 4/5 proximal and distal. Case 02: woman, 36 years old, pastry chef, had episodes of forgetting about everyday activities of her work, progressive in the last three years (cake recipes, budget accounts, orders placed) associated with confusion for spatial location on the way home/ work, evolving to apraxia in writing letters and words and difficulty with calculations associated with monoparesis of the right lower limb for twelve months with progression to paresis of the lower limbs after six months and evolution to paresthesia of the upper limbs for three months. Associated with the condition, he has migraine without chronic aura and RMSD. The neurological examination showed Mini-Mental State Examination 22/30 (expected score of 29), list of animals in one minute: 09 animals; list of words starting with “F”: 03 words; clock test: 2/4; difficulty with calculations and digital agnosia with right/left apraxia; Upper limbs: eutrophic, FMG 4/5, bilateral distal; FMG 5/5 bilateral proximal; Lower limbs: eutrophic, bilateral FMG 4/5 distal and proximal with positive Mingazini; atypical gait with evidenced weakness in heel, toe and tandem gait; Bilateral dysdiadochokinesia with eumetric, slowed indexindex; ROT 4+/4+ in the right side with positive Hoffman and Babinski signs. MRI Brain (17/09/2020): extensive area of hypersignal on T2 and FLAIR (T2- weighted-Fluid-Attenuated Inversion Recovery) involving the periventricular white matter in all lobes without atrophic or expansive effect; Case 03: woman, 45 years old, started behavioral arrest epileptic seizures at the age of thirty-two, progressing to focal dysperceptive seizures with progression to bilateral tonic clonic seizures after eight months and multiple episodes of anterograde amnesia, presenting forgetfulness related to everyday work activities (exchanged worksheets , payments, calculation errors and budgets); associated with the condition presented migraine without chronic aura and RMSD. Genetic Test (04/06/2017): Heterozygous alteration in exon 8 of the NOTHC – 3 gene. Discussion: CADASIL presents a rare cause of cognitive decline and is often overlooked in diagnosis, except in cases of high clinical suspicion in a familial course. Access to imaging tests becomes fundamental for the diagnostic segment and the primordial genetic test for etiological elucidation and family planning, in the report described the family presentation with the same course of satellite symptoms (migraine without aura, and RMSD) associated with cognitive alteration with anticipation of age of onset are hallmarks of clinical thinking. Cases described in the literature show that the clinical symptomatology is not necessarily related to the level of brain injury observed in the imaging exam, which could be explained by personal factors and which exon is affected. The NOTCH-3 gene has 24 exons, in which the literature reports exon 4 as the most common mutation, followed by 3, 5 and 6, mainly in the Caucasian population. In an Asian population, the most affected exon is 4 and 11, which is also found in Italian descendants. The mutation in exon 8, described in the clinical case, is found in a population of Portuguese origin, being the second most common mutation in this nationality, behind the mutation in exon 4. Brain MRI studies have tried to elucidate the most affected brain regions, aiming to trace a line of evolution. Involvement of the temporal lobe, external capsule and corpus callosum are described as probable markers of CADASIL, and can be used as an aid in the diagnosis due to its specificity of 86% and sensitivity of 89%. Studies also show that the frontoparietal area has frequent findings of hyperintensity (100%), followed by the temporal lobe (83%), less frequently affecting the brainstem, occipital lobe and cerebellum. Despite the great advances in the specialized literature, the causes of the important cognitive dysfunction presented in the course of the evolution of CADASIL remain unclear. However, studies have suggested that the process of cognitive decline is more related to the loss of cortico-subcortical connections than to brain atrophy itself, with these disconnections resulting from repeated transient ischemic accidents. Science has been looking for ways to change the prognosis of CADASIL, recent studies in gene therapy and neurogenetics show the importance of thinking about this pathology as a genetic disease of great importance to change the prognosis of this pathology.

Abstract The Rapid Rail Transit (Metro) systems have been set as an important tool for world cities to supply the transit riders (users) the mobility and accessibility requirements in the high demand (highly loaded) urban corridors, which require a combination of large passenger capacity and service quality. Given their large capacity and the associated high service level and environmental performance, the metro systems provide a high quality accessibility, which ultimately, set them as a structural tool to drive the long term urban spatial and economic development. Metro systems are set as urban megaprojects, given their required very large outlays of capital intensive investments on long-lived assets, such as guideways, stations, rolling stock, and the associated systems (electrification and signaling). The system’s design features might strongly affect its capacity, operational flexibility, service performance, and cost, with the passenger-carrying capacity in the highest-demand sections of the network being one of the most critical design parameters, given its effect on the system’s investments. This work presents, based on the state of the art available public technical data, a review of the rail metro technology challenges associated with their implementation and expansion initiatives. To do so, it reports the Washington D.C. (District of Columbia) Metrorail experience, at the United States (U.S.), an iconic metro system, which has been successfully planned in the sixties and implemented in the, which has revolutionized the greater Washington Metropolitan Area mobility and urban planning and that currently is set among the busiest metro systems in the U.S. Nevertheless, despite its groundbreaking transit performance, it currently faces both refurbishment (rehabilitation) and expansion demands, whose diagnosis and proposed solutions are reported in this paper to illustrate the metro rail challenges to provide the required adequate transit service throughout its service life.

The study focuses on the four Southern states – Karnataka, Andhra Pradesh, Tamil Nadu, and Kerala – and analyses data from the Crime in India report of the National Crime Records Bureau, reports of the Ministry of Social Justice and Empowerment, and case law from High Courts and the Supreme Court of India.

This research sought to understand the impact of probation inspection on probation policy, practice and practitioners. This important but neglected area of study has significant ramifications because the Her Majesty’s Inspectorate of Probation has considerable power to influence policy through its inspection regime and research activities. The study utilised a mixed methodological approach comprising observations of inspections and interviews with people who work in probation, the Inspectorate and external stakeholders. In total, 77 people were interviewed or took part in focus groups. Probation practitioners, managers and leaders were interviewed in the weeks after an inspection to find out how they experienced the process of inspection. Staff at HMI Probation were interviewed to understand what inspection is for and how it works. External stakeholders representing people from the voluntary sector, politics and other non-departmental bodies were interviewed to find out how they used the work of inspection in their own roles. Finally, leaders within the National Probation Service and Her Majesty’s Prisons and Probation Service were interviewed to see how inspection impacts on policy more broadly. The data were analysed thematically with five key themes being identified. Overall, participants were positive about the way inspection is carried out in the field of probation. The main findings are: 1. Inspection places a burden on practitioners and organisations. Practitioners talked about the anxiety that a looming inspection created and how management teams created additional pressures which were hard to cope with on top of already high workloads. Staff responsible for managing the inspection and with leadership positions talked about the amount of time the process of inspection took up. Importantly, inspection was seen to take people away from their day jobs and meant other priorities were side-lined, even if temporarily. However, the case interviews that practitioners take part in were seen as incredibly valuable exercises which gave staff the opportunity to reflect on their practice and receive positive feedback and validation for their work. 2. Providers said that the findings and conclusions from inspections were often accurate and, to some extent, unsurprising. However, they sometimes find it difficult to implement recommendations due to reports failing to take context into account. Negative reports have a serious impact on staff morale, especially for CRCs and there was concern about the impact of negative findings on a provider’s reputation. 3. External stakeholders value the work of the Inspectorate. The Inspectorate is seen to generate highly valid and meaningful data which stakeholders can use in their own roles. This can include pushing for policy reform or holding government to account from different perspectives. In particular, thematic inspections were seen to be useful here. 4. The regulatory landscape in probation is complex with an array of actors working to hold providers to account. When compared to other forms of regulation such as audit or contract management the Inspectorate was perceived positively due to its methodological approach as well as the way it reflects the values of probation itself. 5. Overall, the inspectorate appears to garner considerable legitimacy from those it inspects. This should, in theory, support the way it can impact on policy and practice. There are some areas for development here though such as more engagement with service users. While recognising that the Inspectorate has made a concerted effort to do this in the last two years participants all felt that more needs to be done to increase that trust between the inspectorate and service users. Overall, the Inspectorate was seen to be independent and 3 impartial although this belief was less prevalent amongst people in CRCs who argued that the Inspectorate has been biased towards supporting its own arguments around reversing the now failed policy of Transforming Rehabilitation. There was some debate amongst participants about how the Inspectorate could, or should, enforce compliance with its recommendations although most people were happy with the primarily relational way of encouraging compliance with sanctions for non-compliance being considered relatively unnecessary. To conclude, the work of the Inspectorate has a significant impact on probation policy, practice and practitioners. The majority of participants were positive about the process of inspection and the Inspectorate more broadly, notwithstanding some of the issues raised in the findings. There are some developments which the Inspectorate could consider to reduce the burden inspection places on providers and practitioners and enhance its impact such as amending the frequency of inspection, improving the feedback given to practitioners and providing more localised feedback, and working to reduce or limit perceptions of bias amongst people in CRCs. The Inspectorate could also do more to capture the impact it has on providers and practitioners – both positive and negative - through existing procedures that are in place such as post-case interview surveys and tracking the implementation of recommendations.
