Up to 65% of pediatric patients experience anxiety and fear in the preoperative period, especially during anesthesia induction. Reasons for this anxiety include the child's perception of the threat of pain, being separated from parents, a strange environment, and losing control. Anxiety and poor behavioral compliance associated with inhalation inductions have been related to adverse outcomes including emergence delirium and maladaptive postoperative behaviors such as general and separation anxiety, eating difficulties, and sleep disturbances. Fortunately, there are behavioral and pharmacological interventions that anesthesiologists can use to improve compliance during induction.

Anxiety tests and models in rodents are useful tools to reveal neurochemical, cellular, and molecular underpinnings of normal and pathological anxiety-related behaviors, as well as novel treatment targets. While anxiety models are generated by various approaches such as selective breeding, anxiety tests most commonly involve unconditioned approach avoidance tasks and conditioned learning paradigms, both characterized by inherent advantages and limitations, in particular their predictive value for specific anxiety disorders. To further improve the validity and translatability of preclinical anxiety testing, it is promising that some anxiety-relevant endophenotypes have now been investigated using similar tests in rodents and humans and that the involved neural pathways and mechanisms overlap considerably in both species.

This chapter provides a broad overview of the state of research in the genetics of the major anxiety disorders (ADs). They exhibit moderate familial aggregation and heritability due to genetic risk factors that are shared between them as well as those that are disorder-specific. Many candidate gene association studies have been published, with a small set of genes that have been consisted validated for their role in one or more anxiety phenotypes. Genome-wide association studies of ADs are in their infancy, with a handful of published studies for each disorder so far and more to come conducted by large consortia. Animal studies provide a promising complimentary approach that demonstrate concurring evidence across species supporting the involvement of particular biological systems in anxiety-related behaviors.

This chapter summarizes what we know about compulsive behavioral disorders in several animal species. Animals can develop repetitive behaviors in a range of circumstances, generally associated with anxiety or stress. It is increasingly apparent that these behaviors recapitulate core features of obsessive-compulsive disorder. They are clearly partially genetic; for example, specific breeds of dog are susceptible to specific compulsive behavioral disorders. Understanding such OCD-like behaviors provides a potentially fruitful avenue towards understanding OCD in humans. This chapter reviews this literature, emphasizing the points of parallelism between repetitive behavior syndromes in animals and human disease. Recent advances in our understanding of the biology of these spontaneously occurring animal models, especially in dogs, have great potential to elucidate the pathophysiology of OCD.

Child and adolescent anxiety psychodynamic psychotherapy (CAPP) is a new, manualized, tested, 24-session psychotherapy articulating psychodynamic treatment for youths with anxiety disorders. The book describes how clinicians intervene by collaboratively identifying the meanings of anxiety symptoms and maladaptive behaviors and communicating the emotional meanings of these symptoms to the child. The treatment is conducted from a developmental perspective, and the book contains clinical examples of how to approach youth of varying ages. The authors demonstrate that CAPP can help children and adolescents: • Reduce anxiety symptoms by developing an understanding of the emotional meanings of symptoms • Enhance the skill of reflection and self-observation of one’s own and others’ feelings and motivations (improvement in symptom-specific reflective functioning) • Diminish use of avoidance, dependence, and rigidity by recognizing how underlying emotions (e.g., guilt, shame, anger), as well as conflicted wishes can be tolerated and understood • Understand fantasies and personal emotional significance surrounding the anxiety symptoms to reduce symptoms’ magical, compelling qualities and impact on the child The manual provides a description of psychodynamic treatment principles and techniques and offers a guide to the opening, middle, and termination phases of this psychotherapy. The book contains chapters on the historical background of child psychodynamic psychotherapy, on developmental aspects of child psychotherapy, and on the nature of parent involvement in the treatment.
