He puts forward a hypothesis that a significant role in extinguishing the protest potential was played by agent deception games conducted by the Belarusian KGB, as well as parallel repressive actions and the regime’s information offensive, which resulted in a synergy of propaganda and disinformation-agent activities.

Background: Neuromyelitis Optica (NMO) is an autoimmune inflammation of the central nervous system in which autoantibodies are released against Aquaporin-4 (AQP-4), astrocytic water channels. The disease is characterized by transverse myelitis and optic neuritis. Viruses could be inflammatory agents in the brain. Due to such inflammatory reactions, autoantibodies would cross the blood brain barrier. Therefore, recognizing the responsible viral agent may help us prevent or treat NMO. Objectives: To investigate the probable association between Cytomegalovirus infection (CMV) and Neuromyelitis Optica. Materials & Methods: This descriptive study was performed on 25 patients with NMO, 30 patients with Multiple Sclerosis (MS) referring to an academic MS Clinic and 30 healthy individuals in Isfahan City, Iran in 2016. In order to detect DNA of CMV in the sera of the studied groups, real-time PCR technique was used with hydrolyzing probes such as TaqMan. Beacon Designer 7 was used to design a primer and probe. The Chi-square test was used for statistical analysis in SPSS 16. Results: Three study groups had no significant difference in terms of age (P=0.33) and gender (P=0.599). All of the samples were negative for CMV DNA. There was no significant difference between three groups of study (P=0.33). Conclusion: Regarding the negative real-time PCR results of all samples, and especially using higher specificity of primers and probes in detecting this virus, it can be inferred that no significant correlation exists between CMV infection and NMO disease.

Abstract Background: Provocative growth hormone (GH) stimulation testing is used to evaluate short stature and growth failure in children. Agents commonly used for testing include clonidine, arginine and glucagon. While stimulation testing is generally considered safe, gross hematuria has been described as a rare idiopathic complication of GH stimulation testing. This study was designed to estimate the incidence of both microscopic and macroscopic hematuria following GH testing with different provocative agents. Methods: Subjects undergoing GH stimulation testing were invited to participate in the study. Prior to testing, vital signs were measured and baseline point-of-care (POC) urinalysis was done. The subjects performed urine testing at home on days 1, 2, 3 and 7 following GH stimulation studies. Families notified the study team with any positive findings and returned the data collection tool by mail. Results: In total, 34 subjects aged 11.14±2.71 years (91.2% male) completed the study. Agents used in provocative testing included arginine (73.5%), clonidine (94.1%) and glucagon (32.4%). Three subjects developed hematuria after GH stimulation testing (clonidine/arginine). The hematuria resolved by 7 days after testing. Additional adverse effects included nausea, vomiting and hypotension. Conclusions: In this study of children undergoing GH testing, hematuria was identified in three subjects. This study demonstrates that side effects to agents used for GH testing are self-limited, yet not rare, and should be discussed with patients and families prior to stimulation testing.

Abstract Objectives Provocative growth hormone (GH) tests are widely used for diagnosing pediatric GH deficiency (GHD). A thorough understanding of the evidence behind commonly used interpretations and the limitations of these tests is important for improving clinical practice. Content To place current practice into a historical context, the supporting evidence behind the use of provocative GH tests is presented. By reviewing GH measurement techniques and examining the early data supporting the most common tests and later studies that compared provocative agents to establish reference ranges, the low sensitivity and specificity of these tests become readily apparent. Studies that assess the effects of patient factors, such as obesity and sex steroids, on GH testing further bring the appropriateness of commonly used cutoffs for diagnosing GHD into question. Summary and Outlook Despite the widely recognized poor performance of provocative GH tests in distinguishing GH sufficiency from deficiency, limited progress has been made in improving them. New diagnostic modalities are needed, but until they become available, clinicians can improve the clinical application of provocative GH tests by taking into account the multiple factors that influence their results.

AbstractThe modal theory of Harvey Whitehouse provides not only a provocative explanatory grid for the concatenation of variables that comprise religious behavior but also a fruitful theoretical framework for organizing a range of studies by scholars approaching religion via the cognitive sciences. One example is work on the cognitive underpinnings of ritual arrangements that marks the careers of E. Thomas Lawson and Robert N. McCauley. Despite offering mutually exclusive hypotheses concerning the relation of ritual action to memory, Lawson and McCauley's work fits within Whitehouse's overarching modal approach. Other research, too, can be drawn into this framework. One variable not yet addressed by the modal theory is the role of conceptual schemes in shaping religious systems. Within the operational dynamics of religion, conceptual schemes matter, particularly as their contents include superhuman agent concepts — the central feature of religious representations and the object of religious response. This article argues for serious investigation of superhuman agent concepts within the framework of Whitehouse's modal theory, and it hypothesizes that psychological responses to contrasting conceptualizations of superhuman agents is one of the more important cognitive variables driving shifts, or "modulations" from doctrinal to more imagistic forms of religiosity.

She uses these skills as well as her satirical wit and storytelling abilities to create an intricate portrait of the lived experiences of a contemporary diasporic subject. This is a portrait that is not only aesthetically stimulating but also politically inflected and provocative at the same time. Through her work Devi encourages us to remember that diaspora is more than a theoretical trope, that it is a complex and often contradictory experience which results in joy as well as pain as it is played out on live bodies.

Objectives: Five percent of patients with recurrent gastrointestinal (GI) hemorrhage have indeterminate origin by radiological and endoscopic examinations. To improve diagnostic accuracy and therapeutic embolization, the technique of provocative mesenteric angiography (PMA) has been developed. It involves the addition of pharmacologic agents to standard angiographic protocols to induce bleeding. Material and Methods: This is an institutional review board-approved, retrospective study of 20 patients who underwent PMA between 2014 and 2019. All patients had clinical evidence of GI hemorrhage without a definite source. PMA consisted of anticoagulation with 5000 units of heparin and selective transcatheter injection of up to 600 μg of nitroglycerine, followed by slow infusion of up to 24 mg of tissue plasminogen activator into the arterial distribution of the highest suspicion mesenteric artery. Results: Among the 20 patients who underwent PMA, 11/20 (55%) resulted in angiographically visible extravasation. Of these 11 patients, nine patients underwent successful embolization with coil or glue and were discharged upon achieving hemodynamic stability. Two patients spontaneously stopped bleeding. In our series, PMA resulted in the successful treatment of 9/20 (45%) patients with recurrent hemorrhage. No procedure-associated complications were reported with these 20 patients during the procedure and their course of hospitalization. Conclusion: In our experience, PMA is an effective and safe approach in localizing and treating the source of GI bleeding in about half of patients with an otherwise unidentifiable source.

The purpose of this study is to evaluate efficacy and reliability of chemical provocative testing using neurophysiological monitoring prior to embolization of spinal cord AVMs (SCAVMs). We performed retrospective analysis of provocative testing using sodium amytal and lidocaine injected superselectively in 41 angiography and / or embolization procedures in 26 patients with a SCAVM, including 23 amytal and 26 lidocaine injections. All procedures were performed under general anesthesia using neuroleptic drugs, and with monitoring of cortical somatosensory evoked potentials (SEPs) and trans-cranial motor evoked potentials (MEPs). After recording baseline SEPs and MEPs, 50mg of sodium amytal was injected through the microcatheter at the position of the intended embolization, followed by assessment of SEPs and MEPs. If no changes occurred, 40mg of lidocaine was then injected followed by recording of SEPs and MEPs. If again no changes were noted, embolization was performed using NBCA. If there was any change in either SEPs or MEPs, NBCA embolization was not performed from that catheter position. No false negative results of the provocative testing were experienced. One amytal test from the posterior spinal artery (PSA) was positive, causing loss of MEPs. Lidocaine testing was positive in 10 cases including 4 injections in the PSA (with loss of MEPs in two and SEPs in two), 5 injections in the anterior spinal artery (with loss of MEPs in four and SEPs in one), and 1 case involving the posterior inferior cerebellar artery (with loss of MEPs). Neither amytal nor lidocaine injection caused loss of both SEPs and MEPs. In conclusion, sodium amytal and lidocaine are complimentary as pharmacological agents for provocative testing, and SEPs and MEPs are complimentary to each other as physiologic monitoring methods. Provocative testing should be performed using both amytal and lidocaine with monitoring of both SEPs and MEPs.

Cardiac connexin-43 (Cx43) creates gap junction channels (GJCs) at intercellular contacts and hemi-channels (HCs) at the peri-junctional plasma membrane and sarcolemmal caveolae/rafts compartments. GJCs are fundamental for the direct cardiac cell-to-cell transmission of electrical and molecular signals which ensures synchronous myocardial contraction. The HCs and structurally similar pannexin1 (Panx1) channels are active in stressful conditions. These channels are essential for paracrine and autocrine communication through the release of ions and signaling molecules to the extracellular environment, or for uptake from it. The HCs and Panx1 channel-opening profoundly affects intracellular ionic homeostasis and redox status and facilitates via purinergic signaling pro-inflammatory and pro-fibrotic processes. These conditions promote cardiac arrhythmogenesis due to the impairment of the GJCs and selective ion channel function. Crosstalk between GJCs and HCs/Panx1 channels could be crucial in the development of arrhythmogenic substrates, including fibrosis. Despite the knowledge gap in the regulation of these channels, current evidence indicates that HCs and Panx1 channel activation can enhance the risk of cardiac arrhythmias. It is extremely challenging to target HCs and Panx1 channels by inhibitory agents to hamper development of cardiac rhythm disorders. Progress in this field may contribute to novel therapeutic approaches for patients prone to develop atrial or ventricular fibrillation.

Abstract Medullary thyroid carcinoma (MTC) occurs sporadically but may also be inherited as part of the multiple endocrine neoplasia (MEN) type 2 syndrome. Screening of the patients and first degree relatives annually with basal and provocative tests for serum immunoreactive calcitonin (CT) levels is essential and enables potentially curative disease. Pentagastrin and calcium are the usual provocative agents used worldwide. We used endogenous gastrin (GT) release achieved by omeprazole, 20 mg b.i.d., to stimulate CT in 9 MTC, in 3 MEN 2A family members, and in 50 healthy control subjects. A steady and significant increase both in GT and CT levels was achieved in 9 MTC patients and 3 of the 14 family members tested, whereas in healthy controls the CT increase stimulated by GT was insignificant. Preliminary results showed that this new, safe, cheap, and outpatient-basis test can be used in MTC diagnosis, follow-up, and screening.

For patients with advanced cancers involving bone, the standard of care for maintaining bone health is the use of antiresorptive therapies such as bisphosphonates, selective estrogen-receptor modulators, and denosumab. However, although long-term adverse events are rare and the risk-benefit ratio of these agents is usually markedly in favor of treatment, clinicians should be aware that they can occur. At the NCCN 19th Annual Conference, Dr. Azeez Farooki presented the key findings of the NCCN Bone Health Task Force, focusing on such topics as screening for osteoporosis; the controversial use of drug holidays from chronic bisphosphonate therapy; the provocative yet unclear story surrounding the potential anticancer benefits of antiresorptive agents; imaging for metastatic bone disease; and safety considerations linked to calcium supplements, vitamin D, and bone-strengthening agents.

A provocative double blind study of headache was performed using chocolate as the active agent and carob as the placebo. The chocolate and carob samples were formulated to duplicate products used in an earlier study (1) in which strong differential effects between the ability of chocolate and carob to trigger headache in migraine were shown. Sixty-three women with chronic headache (50% migraine, 37.5% tension-type, 12.5% combined migraine and tension-type) participated in the study. After 2 weeks of following a diet that restricted vasoactive amine-rich foods, each subject underwent double-blinded provocative trials with two samples of chocolate and two of carob presented in random order. Diaries were maintained by the subjects throughout the study, monitoring diet and headache. The results demonstrated that chocolate was not more likely to provoke headache than was carol in any of the headache diagnostic groups (2(2) 0.36, p=0.83). Interestingly, these results were independent of subjects' beliefs regarding the role of chocolate in the instigation of headache (2(1)=0.73, p=0.39). Headache diagnosis and the concomitant use of additional vasoactive amine-containing foods were also not associated with chocolate acting as a headache trigger. Thus, contrary to the commonly held belief of patients and physicians, chocolate does not appear to play a significant role in triggering headaches in typical migraine, tension-type, or combined headache sufferers.

The objective of this article is to review different theories of etiology, specifically provocative and predisposing factors for the development of localized scleroderma. The mini-review presents the factors, which may influence the occurrence of localized scleroderma. The genetic predisposition for the development of scleroderma is a relevant characteristic of the disease. Different traumatic factors, viral & bacterial infections, chemical substances, pharmaceutical agents may trigger localized scleroderma. Neoplasia is regarded as a distinct triggering impulse for scleroderma.

A 35 year-old nonatopic woman was referred to the hospital for possible work-related asthma. She had worked as an operator, at a plant producing polypropylene bags, for the previous four yrs. Her main complaint was a productive cough with dyspnoea and wheezing, as well as rhinitis over the past 3 yrs. She had been absent from work for 6 months on maternity leave, and had improved greatly. She was on a beta 2-adrenergic agent and had to take it at least four times daily. Baseline spirometry whilst at work showed marked airflow obstruction (forced expiratory volume in one second (FEV1) of 43% predicted (pred). After two months away from work FEV1 improved to 89% pred; provocative concentration of histamine causing a 25% fall in FEV1 (PC20) was 3.6 mg.ml-1 (mild airway hyperresponsiveness). Return to work resulted in a marked deterioration in FEV1, and serial peak expiratory flow (PEFR) values. PC20 was 0.11 mg.ml-1 (severe airway hyperresponsiveness) one week after she had returned to work. Specific inhalation challenges with polypropylene heated to 250 degrees C resulted in a late asthmatic reaction. As formaldehyde is one of the degradation products of heating polypropylene, we exposed her to it for up to 2 h, but we elicited no bronchospastic reaction. We conclude that heated polypropylene should be listed as one of the agents that causes occupational asthma.

The article examines the protective and the provocative family factors of aggressive behavior in children and adolescents. The negative influence of aggression on the process of socialization is described as well as possible ways of preventive influence to even out the negative influence. During the analysis of scientific literature, including empirical studies of family factors and their influence on the manifestation of aggressive behavior in children and adolescents, two significant clusters are identified: a family’s socio-demographic factors and intrafamily psychological factors as well as social attitudes. In this article, the authors describe a family’s psychological factors, and social attitudes in terms of their protective or provocative influence on the manifestation of aggressive behavior in children and adolescents, as well as in terms of a possible preventive and/or corrective effect on these factors. The frequent manifestation of acts of aggression, as well as the high level of aggressiveness and hostility of children and adolescents relative to the average indicators of their society, is considered as a consequence of deviations in the process of their socialization. The relationship between aggressive behavior and socialization allow the authors to state the importance of a complex integrated approach to the prevention and correction of the aggressive behavior of children and adolescents from the “risk group” from such “agents” of socialization as family and school. The work also examines current examples of complex preventive impact on the personality of a student with the help of positive psychological interventions (PPI), based on the example of foreign experience in the use of PPI. In addition, it substantiates the need for coordinated interaction between the institution of the family and the educational institution for an effective positive impact on the processes of socialization, as well as for the prevention and/or correction of aggressive behavior of minors.

7001 Background: Sunitinib malate (SU11248) is an oral, multitargeted tyrosine kinase inhibitor targeting VEGFR, PDGFR, KIT, FLT3 and RET on tumor cells, tumor neovasculature and pericytes. This is the initial report of an open-label, two-stage, multicenter phase II trial evaluating the single-agent activity of sunitinib in refractory NSCLC. Methods: Eligibility criteria included confirmed diagnosis of NSCLC, ECOG PS 0–1, no recent gross hemoptysis, no brain metastases, patients (pts) previously treated with 1–2 chemotherapy regimens, and adequate end-organ function. Pts received sunitinib at 50 mg/day po for 4 weeks (wks) followed by 2 wks off treatment (6 wks considered a cycle). Results: A total of 64 pts were enrolled and 63 pts treated, median age 61 yrs (range 33–87); adenocarcinoma (64%), squamous cell carcinoma (22%), other (14%); 66% male; PS 0:1, 45%:55%; median number of prior regimens: 2 (range 1–4); median time since the prior regimen: 2 months (range 1–21). To date, 63 pts have started cycle 1, 46 cycle 2, 22 cycle 3, 6 cycle 4 and 1 cycle 5. Grade 3–4 toxicities included fatigue/asthenia (21%), nausea (7%), vomiting (7%), abdominal pain (7%), and hypertension (5%). Most toxicities were grade 1–2 and included asthenia/fatigue (68%), anorexia (40%), dyspnea (37%), cough (35%), nausea (33%), mucositis (32%), dysgeusia (25%), diarrhea (21%), vomiting (19%), and constipation (19%). Grade 5 toxicities include pulmonary hemorrhage (n=2) and cerebral hemorrhage (n=1). Thus far, 6 confirmed partial responses have been observed among 63 treated pts (9.5%, 95% CI: 3.6–19.6%). Stable disease has been observed in an additional 12 pts (19.0%). Survival data are pending and will be presented. Conclusions: Sunitinib has provocative single-agent activity in previously treated pts with recurrent and advanced NSCLC, with the level of activity similar to currently approved agents. Sunitinib is well tolerated in this population. The trial is being extended to explore a continuous dosing strategy of sunitinib at 37.5 mg/day po. Based on these results, further trials are warranted and are ongoing with sunitinib in combination with standard agents/regimens. [Table: see text]

GH provocative tests remain the mainstay for the diagnosis of GH deficiency and at present the insulin tolerance test (ITT) is the gold standard. There are, however, a variety of other stimulation tests used in clinical practice. Each necessitates the use of a specific cut-off derived from normative data, but there remains a widely held view that the implications from a “failed” test are independent of the nature of the stimulus. We sought to examine whether this is the case in individuals with evidence of radiation damage to the somatotropic axis. One hundred and sixty-one nonacromegalic patients were identified who had undergone an arginine stimulation test (AST) and an ITT within a 3-month period as part of routine testing between 1975 and 1999. They were divided into those tested before (n = 81; 48 males) and those tested after (n = 80; 36 males) completion of growth and puberty. Patients were considered for inclusion in the study if they had a history of cranial irradiation and a GH response to one provocative test of less than 8 μg/L, taken as indicating that some damage to the GH axis may have occurred. The patients were compared with 2 control groups. The first comprised 35 adults (18 males) and the second consisted of 16 prepubertal children (10 males). The median peak (range) GH response to the ITT was significantly greater (P < 0.0001) than that to the AST in the adult controls: 24.9 (4.1–76.9) vs. 12.2 (0.88–35.0)μ g/L, respectively. However, in the patients the GH responses were similar (P = 0.28): 2.2 (0.2–25.7) vs. 1.4 (0.2–12.8) μg/L to the ITT and AST, respectively. In contrast to the pattern seen in the adult controls, the response to an ITT in childhood controls was of similar magnitude (P = 0.5) to that to the AST: 17.5 (8.1–40.0) vs. 19.4 (7.3–53.8) μg/L, respectively. However in the patients, the GH response to the AST was greater than that to the ITT (P < 0.0001): 4.3 (0.7–17.2) vs. 3.0 (0.4–18.1) μg/L, respectively. In summary, we have shown that the impact of irradiation on GH responsiveness to provocative agents is stimulus dependent. The GH response to an AST appears to be more resistant to the effects of irradiation than that to the ITT. When investigating the impact of irradiation on GH secretory status, the GH response to an AST may be a less sensitive guide to the functional ability of the GH axis.

Inflammatory mediators are known to be elevated in association with decompression from elevated ambient pressure, but their role in tissue damage or overt decompression sickness is unclear. Circulating microparticles (MPs) are also known to increase, and because interleukin (IL)-1β is packaged within these particles, we hypothesized that IL-1β was responsible for tissue injuries. Here, we demonstrate that elevations of circulating MPs containing up to ninefold higher concentrations of IL-1β occur while mice are exposed to high air pressure (790 kPa), whereas smaller particles carrying proteins specific to exosomes are not elevated. MPs number and intra-particle IL-1β concentration increase further over 13 h post decompression. MPs also exhibit intra-particle elevations of tumor necrosis factor-α, caspase-1, inhibitor of κB kinase-β, and inhibitor of κB kinase-γ, and elevated IL-6 is adsorbed to the surface of MPs. Contrary to lymphocytes, neutrophil nucleotide-binding oligomerization domain-like receptor, pyrin domain containing 3 (NLRP3) inflammasome oligomerization and cell activation parameters occur during high pressure exposure, and additional evidence for activation is manifested post decompression. Diffuse vascular damage, although not apparent immediately post decompression, was present 2 h later and remained elevated for at least 13 h. Prophylactic administration of an IL-1β receptor inhibitor or neutralizing antibody to IL-1β inhibited MPs elevations, increases of all MPs-associated pro-inflammatory agents, and vascular damage. We conclude that an auto-activation process triggered by high pressure stimulates MPs production and concurrent inflammasome activation, and IL-1β is a proximal factor responsible for further cytokine production and decompression-associated vascular injuries. NEW & NOTEWORTHY Elevations in circulating microparticles due to decompression have been documented in humans and animals. This report shows that intra-particle interleukin-1β causes vascular damage that can be abrogated by interventions directed against this cytokine.

Considering all these aspects from a musician’s perspective, it was only natural to emphasize the important position music occupies in Commedia dell’arte and its role as binding agent that in this way creates a balance between modernity and tradition.

The diagnosis of Growth Hormone (GH) deficiency in children with short stature is complex, and in certain cases, might be very difficult. Most of the provocative tests used to evaluate GH deficiency use pharmacological agents. The artificial nature of the pharmacological tests and the possibility that these tests might not always reflect GH secretion under normal physiological conditions provides the impetus for a more physiologic test. Exercise is one of the important GH releasing physiological stimuli. This review will summarize the current knowledge on the methods for performing laboratory exercise provocation test for GH secretion in children. In addition to recommendations of more standardized exercise protocols and environmental considerations, we will also focus on GH responses to exercise in unique populations such as obese children.

Hormone refractory prostate cancer (HRPC) causes substantial morbidity and mortality. There are increasing options for both first- and second-line therapy in the palliative treatment of patients with HRPC. Medications to control symptoms should first be optimized in patients with late-stage disease, and radiotherapy applied to dominant painful bone lesions. Docetaxel, mitoxantrone, satraplatin, and ixabepilone are active chemotherapeutic agents in the first- and/or second-line setting for patients with HRPC, and this may be true also of older drugs such as oral cyclophosphamide and vinorelbine. Radioisotopes such as strontium and samarium are useful for treatment of more generalized bone pain. Third-line hormonal maneuvers including glucocorticoids, ketoconazole, and estrogens can lead to further palliation in some patients, and there are provocative data that chemotherapy might restore hormonal sensitivity in a subset of patients.

Leopold Jessner's productions of Schiller's Wilhelm Tell (1919) and Shakespeare's Richard III (1920) marked a culminating point in the short-lived, politically volatile era of German theatrical Expressionism. Jessner's distinctive work in this staging style did much to define the features of one mode of performance among several which billed themselves – or were branded as – ‘Expressionist’. In the following article, David Kuhns explores the particularly striking impact of Jessner's ‘emblematic’ approach to Schiller and Shakespeare upon the acting of those productions. By transforming his actors from mimetic agents into monumentalized emblems, Jessner analyzed political consciousness in what amounted to allegorical terms. The result was a politically provocative presentation of political behaviour – particularly the will to power – as an essentially spiritual matter. David Kuhns teaches theatre history, dramatic literature, and critical theory at Washington University in St. Louis.

When investigating new disinfectants, it is necessary to assess the degree of their danger by modeling the conditions of use. As is known, the main danger of disinfectants, especially volatiles, is posed by the inhalation route of entry into the body, exerting an irritating effect on the respiratory organs, as well as a general toxic and sensitizing effect. Despite reasonable restrictions on the use of disinfectants from the aldehyde class for decontamination of the surfaces of premises and equipment, state registration receives disinfectants based on glutaraldehyde (GA), intended for wide use in various areas, including everyday life, which is unacceptable. Experiments were carried out to confirm the danger of using GA-based disinfectants. Sensitizing effect of GA-based disinfectant under conditions of surface treatment by rubbing method is studied. Significant release of GA into the air after surface treatment (27.7±4.1 mg/m3) was detected. The concentration of GA in the air decreased by 4.5 times after 6 hours of ventilation. The sensitizing effect of the GA-based agent was determined on guinea pigs by a provocative ear swelling test and a leukocyte specific lysis reaction. Silence was a provocative film, particularly for Christians viewers. Aside from sketching stories of great interest concerning challenges endured by Jesuit missionaries to Japan, the film presents a deep ethical struggle one finds worthy of careful analysis. In the most climactic scene, the missionary Rodrigues faces an ethical dilemma. He must desecrate fumi-e, a bronze board depicting Christ Jesus, in an act of apostasy to save five tortured and dying hostages. Rodrigues must choose: apostasy or allowing people to die. In this article, the author attempts to reason through Rodrigues' ethical dilemma, without providing conclusive arguments in support of any ethical choices the author might prefer, to broaden the thinking of his readers. Following preliminary considerations for how to define the category of ethical dilemma, the author provides two key questions for evaluating solutions belonging to the moral agent. (α) Concerning competing moral demands, which one is highest in standing amid the rest? (β) Concerning available interventions by the moral agent, which action most effectively realizes the highest moral demand? These two lines of inquiry frame three proposed solutions. Finally, the author also proposes a fourth solution termed Kierkegaardian conflict, following the philosopher of ethics Phillip L. Quinn, whose own proposal, too, remains inconclusive for resolving the dilemma.

In the wake of Foucault's provocative philosophical contributions to the study of discipline and punishment, social and legal historians no longer narrate penal history as a straightforward tale of moral and political progress. In its place is a schematic picture of a large-scale retreat from the body to the prison as the prime site of punishment. Historiographical proclivities perpetuate that image: early modernists tend to concentrate on the Bloody Code and similar régimes of terror, whereas historians of the twentieth century specialize in studies of regulatory modes of punishment and “normalization.” These latter works include histories of reformatories, family courts, social workers, psychiatric experts—in short the institutions and agents that best instantiate the reorientation toward disciplining the soul and governing the self. Scholars who study corporal and capital punishment in the twentieth century would seem to have nothing to add, other than to remark that there were exceptions in the wider history of penal change.

Abstract Immune-based therapy has emerged as a paradigm shift in cancer therapy with dramatic responses observed in previously incurable disease. Cancer vaccines are being developed to disrupt tumor-associated tolerance and activate and selectively expand tumor-specific lymphocytes within the native effector cell repertoire while maintaining immune-regulatory protection against autoimmunity. Although individual antigen approaches result in immune response with a suggestion of clinical effect in some settings, broader efficacy may be dependent on presentation of multiple antigens that capture clonal diversity presented in the context of functionally potent antigen-presenting cells. The use of whole cell–based strategies such as dendritic cell/tumor fusions have yielded provocative results in single-arm studies and are currently being explored in multicenter randomized trials. The posttransplant setting is a potentially promising platform for vaccination due to cytoreduction and relative depletion of inhibitory accessory cells fostering greater immune responsiveness. Integration of these efforts with other immunotherapeutic strategies and agents that target the tumor microenvironment is being studied in an effort to generate durable immunologic responses with clinically meaningful impact on disease.

A majority of patients with asthma believe that psychological factors (particularly stress) can induce asthma attacks, but empirical support for actual stress-induced airways obstruction is controversial. This study tested the hypothesis that stress induces breathlessness and not airways obstruction.Methods. Stress was induced by a frustrating computer task in 30 adolescents with asthma and 20 normal controls, aged 14–19 years. Stress measures were self-reported emotions, heart rate, blood pressure. Respiratory measures were respiratory rate (RR), end tidal CO2, deep inspirations and sighs. Asthma measures were lung function, wheeze, cough, breathlessness.Results. All measures confirmed high levels of negative emotions and stress. None of the participants developed airways obstruction; they had no reduction in lung function, wheeze was absent and cough negligible. However, breathlessness increased in all participants with asthma and excessively in many. The mean breathlessness was higher than during induction of actual airways obstruction with provocative agents in previous studies. End tidal CO2 showed that breathlessness could not be explained by hypocapnia.Conclusion. Stress can be sufficient to induce breathlessness in patients with asthma.

Gli autori presentano i risultati del trattamento mediante scleroterapia percutanea di MV cranio-facciali (uniche o multiple) in 16 pazienti pediatrici (età 2–15 anni). Il trattamento è stato sempre preceduto da un'accurata fase diagnostica eseguita con Eco-Color-Doppler, TC e/o RM e flebografia per puntura diretta. Sono stati utilizzati vari agenti sclerosanti (Etanolo 95%-18 casi; Atoxisclerol -6 casi; Ethibloc- 3 casi) scelti in base alle caratteristiche della lesione (entità dei drenaggi venosi, superficialità). In totale sono state eseguite 27 sedute di trattamento che è stato ripetuto per una massimo di 4 volte. I risultati sono stati classificati in termini qualitativi in base alla percentuale di riduzione (buono = riduzione maggiore del 70%; soddisfacente = riduzione tra 50 e 70%; insoddisfacente = riduzione inferiore al 50%). In totale la riduzione superiore al 50% è stata ottenuta nel 87% dei casi, con miglioramento del risultato ad ogni seduta successiva di trattamento. In tutti i pazienti il trattamento ha provocato una reazione flogistica locale; solo in 2 casi si è prodotta una ischemia del labbro con esito in cicatrice. I buoni risultati ottenuti con complicanze molto scarse sono conseguenti all'esecuzione della flebografia per puntura diretta che visualizzando gli scarichi venosi delle lesione consente di prevenire, mediante la compressione degli stessi, la diffusione degli agenti sclerosanti. Il trattamento sclerosante percutaneo delle malformazioni venose si è mostrato efficace, sicuro e facilmente ripetibile.

Teniposide, VM-26 (Vumon), was administered in a dose of 60 mg/m2 on days 1 to 5 every third week to 36 patients with histologically confirmed small-cell lung cancer. None had previously received chemotherapy or radiotherapy. The median age was 73 years (range, 52 to 79). Thirty-three patients were evaluable; 21 of these had local disease. Five patients had bone marrow metastases, four had liver involvement, and one CNS metastases. All patients had a performance status less than or equal to 2 before the start of treatment. Thirty patients obtained a response (90%), ten of whom had a complete remission (30%). The median duration of remission was 8+ months (range, 1.1 to 17+ months), whereas the median survival was 8.7 months (range, 1.9 to 20 months). Toxicity was primarily hematologic, with leukopenia the only dose-limiting effect. Besides alopecia, all other side effects were minimal including nausea and vomiting. We find these results provocative in regard to the response rate and the duration of response obtained as well as in reference to the dismal results that prior investigations in previously treated patients have shown. These data may indicate the need for reconsideration of the usual strategy for performing phase II trials.

Abstract The diagnosis of growth hormone (GH) deficiency (GHD) is still problematic for the clinician. There is no gold standard for estimating GH secretion. The aim of this study was to compare the diagnostic usefulness of spontaneous GH secretion test, pharmacological tests with insulin, clonidine, L-dopa, and glucagon, and IGF-1 measurement in GHD. We studied 180 prepubertal, short children. Predictive values were calculated for different GH cutoff levels for each diagnostic test. ROC curves were used to estimate the diagnostic usefulness of the tests. The results show that sleep is the strongest stimulatory agent for GH secretion. The estimation of GH secretion after onset of sleep can be used as a screening test in GHD diagnosis. The insulin test has the highest discrimination. A combination of insulin test with another provocative test allows high discrimination and accuracy for standard cut-off GH level. Measurement of IGF -I is characterized by low predictive values. IGF-1 level below the mean according to age indicates high probability of GHD. Auxological parameters should be the most important factor in diagnosing GHD.

Women in Pakistan are facing a number of problems. Their identities and rights are heavily influenced by sociocultural, legal and religious values upheld in the country. Media, as an industry and liberal agent of socialization, has become a platform for value contestation and value promotion. This paper analyzed the discourse of liberal and traditional values in daily Dawn (news, reports, articles, editorials), from July 15, 2016 to October 20, 2016, to explore how the murder of Qandeel Baloch was discussed and analyzed while constructing the liberal and traditional values about women’s issues. The results show that individualism, self-interest, freedom of expression, sexual freedom, free mobility and visibility, and liberal principles of justice were constructed as rational and pro-women. Tradition, religion, religiosity, piety, modesty, submissiveness and virginity were constructed as oppressive and patriarchal constructs to be left behind on the way of modernity, liberation and empowerment. The sexual freedom and provocative activities of Qandeel Baloch were admired and praised and she was constructed as a woman of substance and honour.

AbstractThis paper addresses the question of the character of God as sketched in the poetry of Hosea. It considers the broad and rich spectrum of images and metaphors that are employed to give expression to that divine character in poetic idiom. It is clear that the range and spectrum of metaphors cannot be systematized, but one can nonetheless identify among them usages that bespeak divine judgment, usages that articulate a positive concern, and usages that are neutral or ambiguous. After that survey, the discussion focuses upon the two provocative texts if Hosea 2:2-23 and 11:11-9. Attention is given, in each of these cases, to the movement from judgment to positive possibility. The two governing metaphors of husband-wife and parent-child are taken to be indispensable for a relationship that persists, even in the face of harsh judgment and alienation. The flow of drama in these two texts suggests that YHWH has been an agent of violence, but is "in recovery" from that propensity to violence.

Corporate understanding on strategies of cooperation and outsourcing of business processes has come closer over the years. An outsourced partner is described sometimes as the extended enterprise. Supplier's organizational structure, following the logic of extended enterprise, needs to correspond in a certain strange manner with the customer's organizational structure. We argue that the process of finding a fit between two or more structures is expression of a demand; and, this process being unstable destabilizes all the structures in dynamic interactions resulting in generation of new and novel demands. Thus, struch1res become the sources of demand here. Hence, meeting customer's demand or management of current demand create another demand in consequence. Such a sequel ought to be based on increasing return or an endogenous satisfaction of demand, making it provocative enough to look for an endogenous demand. However, management of an endogenous demand rests with the supplier firm- on how innovative structural solutions can be offered by an extended enterprise. The supplier firm acts here as agent provocateurs as it were.

AbstractMaintenance therapy has been extensively studied to discern any prospective therapeutic advantage in the treatment of advanced stage ovarian carcinoma. The CA-125 assay may have prognostic benefit in determining whether this treatment regimen is appropriate for ovarian carcinoma patients who achieve a complete response to first-line therapy. We retrospectively documented the CA-125 levels of 2 advanced ovarian cancer patient groups who exhibited a clinically defined complete response to their primary induction therapy. Patients were then treated with a paclitaxel-based maintenance therapy regimen. The first group (group A; n = 13 patients) received 3 cycles of single-agent paclitaxel maintenance therapy, and the second group (group B; n = 13 patients) received 12 cycles of single-agent paclitaxel maintenance therapy. The premaintenance therapy CA-125 serum levels (<10 or ≥10 U/mL) of the 2 treatment groups were then retrospectively evaluated in an intragroup analysis to discern any relationship with progression-free survival (PFS) and overall survival. There was a statistically significantly relationship between the CA-125 levels (<10 U/mL) premaintenance therapy and PFS. The patients who had the lowest CA-125 levels exhibited the most favorable PFS results. Despite the limited sample size and nonrandomized nature of this study, these results are provocative and suggest that advanced ovarian cancer patients who achieve an excellent response to primary platinum-based chemotherapy with a CA-125 serum level less than 10 U/mL may be more amenable to the benefits of paclitaxel maintenance therapy.

Abstract Introduction: Daratumumab (DARA) is a human CD38-directed monoclonal antibody, recently approved by the FDA for treatment of myeloma patients that have received ≥3 prior lines of therapy including a proteasome inhibitor and an immunomodulatory agent. DARA has demonstrated excellent clinical activity in combination with pomalidomide (POM) and dexamethasone (D) (Chari et al ASH 2015) among relapsed (R) or relapsed and refractory (RR) myeloma patients. While DARA is being evaluated in combination with several backbone therapies among patients who are naïve to DARA or the backbone agent under evaluation, its utility is unclear among patients refractory to DARA or the combination agent. Methodology: We report our experience of the combination of DARA-POM-D among patients that are naïve to DARA and POM (cohort 1), that were refractory to DARA or POM (cohort 2) and that were refractory to DARA and POM (cohort 3). We performed a retrospective evaluation of patients with relapsed or R/RR myeloma treated at Emory University Hospital from January 2015 until July 2016. Responses were evaluated using the IMWG criteria. We used IBM SPSS version 23.0 to generate the survival statistics. Results: Among the 41 patients (39% M/61% F) included in analysis, the median age at the time of receiving the DARA-POM-D regimen is 63 years (range 32-92). 61% of the patients had IgG isotype, 15% had ISS stage 3 disease at diagnosis. 97% were lenalidomide refractory, 86% were bortezomib refractory and 90% underwent SCT. 19 patients (46%) that received the DAR-POM-D regimen were naïve to both DARA and POM while 12 patients (29%) were refractory to both DARA and POM. More than half of the cohort (54%) in cohort 2 were refractory to either DARA or POM. The median prior lines of therapy (cohort 1: 2: 3- 3: 5: 6.5 prior lines) and median time to initiate DARA-POM-D regimen from diagnosis (cohort 1: 2: 3- 72: 65: 57 months) suggested cohort 3 is a heavily pretreated group. High risk cytogenetics for cohort 1: 2: 3 were 15.8% vs 31.8% vs 25%, respectively. The response rates and the median progression free survival (PFS) are summarized in table 1. Conclusions: Our analysis shows impressive overall response rates of close to 90% among DARA and POM naïve patients. More importantly, overall response rates of close to 35% were seen with DARA-POM-D regimen among patients that are refractory to both DARA and POM. While recapturing the responses with combination therapy DARA-POM-D among patients that are refractory to the individual agents (Figure 1) is provocative, a clear biological rationale should be investigated, which potentially can guide us to design more optimal combination salvage strategies. Abstract In her rich and provocative paper, Susan Levin seeks to defend the value of anger against the views of Stoics and transhumanists, both of whom regard anger as irrational and to be eliminated. In her defense, Levin draws on Aristotle, relating his position to contemporary appraisal theorists as well as anti-racism activists and scholars, for Aristotle holds, in contrast with the Stoics, that some cases of anger are justified. The virtuous Aristotelian agent will become angry in response to injustice. I explore the extent to which Levin endorses Aristotle’s account of anger, complicating some of the associations she aims to establish. In particular, while the Stoics and Aristotle disagree about the rationality, morality, and prudential value of anger, they share a very similar conception of the emotion, according to which a desire for revenge partly constitutes anger. By contrast, Levin defends a forward-looking conception of anger, focusing on rectifying current injustice and preventing future injustice. It turns out, then, that Levin’s justified forward-looking anger may have more in common with the spirit of the Stoic response to injustice than with Aristotle’s retributive view.

This paper reports an enzootic outbreak and spontaneous regression of keratoacanthomas among adult layer hens with lesions on the skin of the legs. The observations were performed in a flock of 55,000 commercial layers (50,000 Lohmann White and 5,000 Lohmann Brown). At the age of 30 weeks, Lohmann White layers exhibited a number of growths (at an average of 60 hens per week, representing 0.1% of the flock) in the region of leg toes on a daily basis over 28 weeks that regressed during the remaining flock production period. Gross and histological investigations identified the lesions as keratoacanthomas. PCR analysis was negative for avian leukosis virus (ALV) and reticuloendotheliosis virus (REV). The present investigations have revealed an unusual case: this is the first report on an enzootic outbreak of multiple keratoacanthomas in commercial layers. The results of the aetiological investigations do not show a relation to any infectious agent or a chemical-toxic cause. The abnormal invasion of keratinocytes from the stratum corneum leading to neoplasms in this case coincides with the phase of peak laying capacity which is in fact a stress factor and might be regarded as a provocative moment.

N-Formyl-methionyl-leucyl-phenylalanine (FMLP), an acylated tripeptide bacterial neutrophil chemotactic factor, has been shown to be a bronchoconstrictor by inhalation in man in vivo. It thus has a putative role in the generation of bronchoconstriction associated with bacterial bronchial infection. We have investigated the effect of pretreatment with the anticholinergic agent, ipratropium bromide (IB), and the beta 2-agonist, fenoterol (F), on FMLP-induced bronchoconstriction. Ten non-asthmatic subjects aged 21-28 yrs performed dose-response curves to nebulized FMLP on 3 study days after pretreatment with saline, F or IB. Amongst the 8 subjects who bronchoconstricted by 20% to FMLP there was a significant increase in the provocative concentration causing a 20% fall in forced expiratory volume in one second (FEV1) (PC20FMLP) after both IB and F. F was significantly better than IB. When comparison was made using absolute fall in FEV1 and including all 10 subjects the same results were found. Partial inhibition of FMLP-induced bronchoconstriction by IB suggests that part of the effect of FMLP is vagally mediated. We suggest that F is acting via modulation of FMLP-induced rises in intracellular free calcium.

In his influential reading of Heroides 1, Duncan Kennedy argues that successful fictional letters can be felt to arise naturally from or be motivated by the depicted events and, ideally, be seen as agents in the forward movement of those events. Building on Kennedy's arguments, Peter Knox asserts that Ovid reconfigures his heroines in the Heroides to develop serious issues raised by his literary models from a new perspective. In this paper, I shall follow Kennedy's and Knox's suggestions to propose a new reading of Heroides 3. I shall first discuss how Briseis' letter can be felt to be both naturally motivated by and seriously engaged with Achilles' arguments in Iliad 9. More importantly, I shall argue that Ovid implicates Patroclus' aid in the composition and delivery of Heroides 3. The result is that one can view the fictional letter as having some influence upon Patroclus' fateful appeal to Achilles, thus providing an Ovidian reinterpretation of Iliad 16. Imagining this fictional role for Heroides 3 ultimately provokes comparison of Briseis, Patroclus and their relationships with Achilles. Such a reading uses Kennedy's and Knox's suggestions as a point of departure for making Heroides 3 a more successful and provocative fictional letter.

ABSTRACT 14-3-3 family members are intracellular dimeric phosphoserine-binding proteins that regulate signal transduction, cell cycle, apoptotic, and metabolic cascades. Previous work with global 14-3-3 protein inhibitors suggested that these proteins play a critical role in antagonizing apoptotic cell death in response to provocative stimuli. To determine the specific role of one family member in apoptosis, mice were generated with targeted disruption of the 14-3-3τ gene. 14-3-3τ−/− mice did not survive embryonic development, but haploinsufficient mice appeared normal at birth and were fertile. Cultured adult cardiomyocytes derived from 14-3-3τ+/− mice were sensitized to apoptosis in response to hydrogen peroxide or UV irradiation. 14-3-3τ+/− mice were intolerant of experimental myocardial infarction and developed pathological ventricular remodeling with increased cardiomyocyte apoptosis. ASK1, c-jun NH2-terminal kinase, and p38 mitogen-activated protein kinase (MAPK) activation was increased, but extracellular signal-regulated kinase MAPK activation was reduced, in 14-3-3τ+/− cardiac tissue. Inhibition of p38 MAPK increased survival in 14-3-3τ+/− mice subjected to myocardial infarction. These results demonstrate that 14-3-3τ plays a critical antiapoptotic function in cardiomyocytes and that therapeutic agents that increase 14-3-3τ activity may be beneficial to patients with myocardial infarction.
