The essential medicines concept developed by the World Health Organization (WHO), states that there is a list of minimum medicines for a basic health-care system, including the most efficacious, safe, and cost-effective ones for priority conditions. According to the WHO, essential medicines are those that satisfy the primary health-care needs of the population. Thus, they should always be available, affordable, and cost-effective. However, the vast majority of the global population does not have access to essential medicines. Many organizations have called on governments to adopt policies to improve availability of these essential medicines and developed programmes and projects to increase awareness and knowledge. This chapter describes two such projects developed by the International Association for Hospice and Palliative Care (IAHPC), based on the essential medicines concept to improve access to medicines and appropriate palliative care: the IAHPC List of Essential Medicines in Palliative Care and the Opioid Essential Prescription Package.

This chapter briefly reviews the use of genomewide screening for early detection, treatment, and prevention and the utility of genome-based biomarkers as a tool for precision medicine and its application to population and integrative preventive medicine. Advances in technology have made genomic screening more affordable and widely available, and both our understanding and the value of testing grow as more data is collected. Even more recently, the growing availability of epigenetic testing, methylation and ROS-associated molecular signatures are providing more insight into dynamic aspects of the human genome and how lifestyle and IPM change affect the expression of the genome. Early adoption of precision medicine in oncology offers a model that should be expanded into wider areas of treatment and prevention.

Artificial intelligence methods, including clinical decision support systems will continue to evolve with, and adapt to, nuclear cardiology and the changing needs of physicians in specific, and to nuclear medicine technology and the health care system in general. The high level of automation already achieved in myocardial perfusion imaging is unmatched by any other cardiac imaging modality, and continues to be its major strength. In addition, strong statistical evaluations of the accuracy and validity of the various techniques have been made possible simply because of the large amount of objectivity and standardization in the automated processes. These strengths when applied to decision support systems that are affordable and easily accessible should allow most nuclear cardiology physicians to perform at a high level of expertise when interpreting imaging studies to demonstrate the value of nuclear cardiology in patient management, and most importantly, to maintain the highest quality clinical care.

Health services research is a practically focused discipline drawing on the social and behavioral sciences and concerned with the organization, financing, and delivery of services. Investigators studying HIV services examine how well healthcare and other systems meet needs, eliminate disparities, integrate services, eliminate barriers, and provide care to socially marginal and stigmatized individuals, such as injection drug users and people who are incarcerated. These issues have been important from the earliest days of the HIV epidemic and have taken on increased significance as efficacious treatments have been developed and, more recently, changes in the financing of care have reduced the number of uninsured. This chapter this focuses primarily on research in the United States with a review of recent work on financing, service fragmentation, and difficulties integrating different sectors of care, as well as problems related to the functioning of medicine in a complex, stratified society. Also addressed are certain general features of the Affordable Care Act that are relevant to HIV care and psychiatry.

Learning While Caring is about what the author has learned during his half-century career as a cancer doctor. During this time, medicine has changed greatly. It has become more scientifically based, more institutionally located, and now comprises almost one-fifth of the US economy. Despite these changes, much remains the same, especially the primary obligation of the doctor to the patient. Also during this period, most of the developed world has recognized health care as a right for all its members. This has been resulted in greatly improved care for many, but not for all. For the last 25 years the United States has been experimenting on how this should be achieved, beginning with the proposed but not enacted Clinton Health Security proposal and currently with the Affordable Care Act. While efficiency and cost control are essential, cost cannot be the only parameter of success. Access to high-quality health care must be made available to all. Proper education for an informed public must include an understanding of the general principles of biology, while that of a doctor must result in a familiarity with the humanities and social sciences. An academic physician has three responsibilities: patient care, teaching, and research. These latter two, while essential, must not conflict, compromise, or limit the doctor-patient relationship. This book is about the author’s activities in all three endeavors. Since his specialty is oncology, this is the major subject, but most of the information is applicable to all caring physicians.
