ABSTRACTThe usefulness of β-lactam antimicrobial agents is threatened as never before by β-lactamase-producing bacteria. For this reason, there has been renewed interest in the development of broad-spectrum β-lactamase inhibitors. Herein we describe the results of dose fractionation and dose-ranging studies carried out using a one-compartmentin vitroinfection model to determine the exposure measure for CB-618, a novel β-lactamase inhibitor, most predictive of the efficacy when given in combination with meropenem. The challenge panel includedEnterobacteriaceaeclinical isolates, which collectively produced a wide range of β-lactamase enzymes (KPC-2, KPC-3, FOX-5, OXA-48, SHV-11, SHV-27, and TEM-1). Human concentration-time profiles were simulated for each drug, and samples were collected for drug concentration and bacterial density determinations. Using data from dose fractionation studies and a challengeKlebsiella pneumoniaeisolate (CB-618-potentiated meropenem MIC = 1 mg/liter), relationships between change from baseline in log10CFU/ml at 24 h and each of CB-618 area under the concentration-time curve over 24 h (AUC0–24), maximum concentration (Cmax), and percentage of the dosing interval that CB-618 concentrations remained above a given threshold were evaluated in combination with meropenem at 2 g every 8 h (q8h). The exposure measures most closely associated with CB-618 efficacy in combination with meropenem were the CB-618 AUC0–24(r2= 0.835) andCmax(r2= 0.826). Using the CB-618 AUC0–24indexed to the CB-618-potentiated meropenem MIC value, the relationship between change from baseline in log10CFU/ml at 24 h and CB-618 AUC0–24/MIC ratio in combination with meropenem was evaluated using the pooled data from five challenge isolates; the CB-618 AUC0–24/MIC ratio associated with net bacterial stasis and the 1- and 2-log10CFU/ml reductions from baseline at 24 h were 27.3, 86.1, and 444.8, respectively. These data provide a pharmacokinetics-pharmacodynamics (PK-PD) basis for evaluating potential CB-618 dosing regimens in combination with meropenem in future studies.

The present work was conducted to study the genotypic variability of rice genotypes at the germination and seedling stages at different levels of salinity (0 M, 0.15 M, 0.2 M and 0.25 M NaCl). The results showed that increasing salinity decreased germination and seedling growth. Significant genotypic variability exists in the germination and seedling stages in response to different NaCl concentrations. Most of the genotypes showed more than 90% germination in the control, indicating good seed vigour. Two genotypes, VBR 638 (93%) and VBR 644 (84%), were selected as being tolerant to salinity at 0.2 M NaCl at the germination stage. Therefore, these could be used as source materials for genetic improvement for salinity tolerance at the germination stage. A considerable amount of genotypic variability was also found under control and saline conditions at the seedling stage with respect to the variables shoot height, root length, shoot and root dry weight. The high heritability observed for these variables offers good scope for genetic improvement for salinity tolerance both at the germination and seedling stages. The genotypes VBR 616, VBR 628, VBR 645, VBR 640, VBR 611, VBR 620, VBR 612, VBR 618, VBR 644, VBR 629, VBR 625 and VBR 630 were selected as being tolerant to salinity at the seedling stage.

The present study evaluated the prognostic role of circulating miRNA-618 in patients with metastatic colon cancer (mCC) and whether miR-618 gene rs2682818 single nucleotide polymorphisms (SNP) are associated with colon cancer susceptibility and expression levels of mature miR-618. In total, 104 patients with mCC before starting the chemotherapy were investigated. The expression status of circulating miR-618 in mCC was evaluated by quantitative PCR. TaqMan PCR assay was used for rs2682818 SNP genotyping. miR-618 was overexpressed in serum of mCC patients. Patients with high and intermediate expression of miR-618 had a significantly longer mean overall survival (OS) of 21 months than patients with low expression—16 months. In addition, multivariate Cox regression analysis confirmed the association between high/intermediate levels of miRNA-618 and longer OS, HR = 0.51, 95% CI: 0.30–0.86, p = 0.012. miR-618 rs2682818 SNP significantly decreased the risk of colon cancer susceptibility in both heterozygous codominant (AC vs. CC, OR = 0.39, 95% CI: 0.17–0.88, p = 0.024) and overdominant (AC vs. CC + AA, OR = 0.37, 95% CI: 0.16–0.85, p = 0.018) genetic models. Our data suggest that circulating miRNA-618 could be useful as a prognostic biomarker in mCC. Patients harboring AC rs2682818 genotype have a decreased risk for colon cancer in comparison with patients with CC and AA genotypes.

Rearranged during transfection (RET) germ-line mutations in exon 10 are peculiar because they produce both gain-of-function multiple endocrine neoplasia 2A and loss-of-function Hirschsprung's disease phenotypes. Drawing on 38 medullary thyroid cancer patients harboring germ-line mutations in codon 620 (n=8), 618 (n=19), 611 (n=10), and 609 (n=1), this study aimed to test the hypothesis that closer proximity of RET germ-line mutations in exon 10 to the cell membrane may translate into earlier or more advanced disease. The closer mutations in codon 620, 618, and 611 were located to the transmembrane domain (codons 657–636) of the RET receptor, the greater were mean primary tumor diameters (23.5, 18.7, and 7.5 mm, P=0.020), the frequency of lymph node metastasis (75, 68, and 30%, P=0.11) and pheochromocytoma (38, 16, and 0%, P=0.11). Periods of observation were broadly comparable for these groups (mean age 33.4–39.3 years; P=0.71). When mutations in adjoining codons were collapsed (codons 620/618 vs 611/609), the differences in mean primary tumor diameter (20.1 vs 7.4 mm, P=0.005) and lymph node metastasis (70 vs 36%; P=0.07) were accentuated. Compared with 80 carriers of exon 11 mutations (codon 634, n=78; codon 630, n=2), the 38 carriers of exon 10 mutations, which are rarer and confer a weaker transforming activity in vitro than exon 11 mutations, required significantly more time to develop fewer tumors. Although limited in numbers, these data suggested that membrane proximity is an important determinant of tumor development in carriers of RET mutations in exon 10.

The delay in wound healing caused by chronic wounds or pathological scars is a pressing issue in clinical practice, imposing significant economic and psychological burdens on patients. In particular, with the aging of the population and the increasing incidence of diseases such as diabetes, impaired wound healing is one of the growing health problems. MicroRNA (miRNA) plays a crucial role in wound healing and regulates various biological processes. Our results show that miR–618 was significantly upregulated during the inflammatory phase of wound healing.Subsequently, miR–618 promotes the secretion of pro–inflammatory cytokines and regulates the proliferation and migration of keratinocytes. Mechanistically, miR–618 binds to the target gene–Atp11b and inhibits the PI3K–Akt signaling pathway, inhibiting the epithelial–mesenchymal transition (EMT) of keratinocytes. In addition, the PI3K–Akt signaling pathway induces the enrichment of nuclear miR–618, and miR–618 binds to the promoter of Lin7a to regulate gene transcription. Intradermal injection of miR–618 antagomir around full–thickness wounds in peridermal mice effectively accelerates wound closure compared to control. In conclusion, miR–618 antagomir can be a potential therapeutic agent for wound healing.

The nonpathogenic isolate 618-12 of Fusarium oxysporum, wild type as well as a benomyl-resistant mutant, suppressed Fusarium wilt (F. o. f.sp. dianthi) in a susceptible cultivar of carnation by 80%. Two other nonpathogenic isolates had a similar effect. Suppression by isolate 618-12 occurred only when antagonist and pathogen were both added to the soil but not if they were introduced at a different location (one in the stem, the other in the soil). Thus systemic induced resistance could not be demonstrated. In treatments where isolate 618-12 and the pathogen were both introduced into the soil, fewer plants showed symptoms, disease severity was less, and colonization by the pathogen was less extensive than after inoculation with the pathogen alone. After soil inoculation, the nonpathogenic isolate 618-12 was recovered from the stem, sometimes at a height of 60 cm. Key words: colonization, carnation stem, soil.

Abstrak Telah dilakukan pencampuran nanopartikel emas dengan kuantum dot CdSe 618. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh penambahan nanopartikel emas terhadap intensitas emisi dan waktu peluruhan fotoluminesensi kuantum dot CdSe 618. Dalam 2 mL kuantum dot CdSe 618, ditambahkan nanopartikel emas sebanyak 40 µL, 80 µL, dan 120 µL. Pada setiap penambahan tersebut, dilakukan pengujian fotoluminesensi dan time-resolved photoluminescence, masing-masing untuk mengetahui intensitas emisi dan waktu peluruhan fotoluminesensi kuantum dot CdSe 618 setelah ditambahkan dengan nanopartikel emas. Dilakukan pula pengukuran terhadap rentang panjang gelombang absorpsi dari nanopartikel emas, beserta ukuran diameter partikel-partikelnya. Hasil pengujian menunjukkan bahwa secara keseluruhan, intensitas emisi kuantum dot CdSe 618 setelah ditambahkan dengan nanopartikel emas menjadi lebih rendah, dan nilai waktu peluruhan fotoluminesensi menjadi lebih besar. Penurunan intensitas emisi dan peningkatan waktu peluruhan fotoluminesensi diakibatkan oleh kemunculan plasmon pada nanopartikel emas. Kata-kata kunci: nanopartikel emas, kuantum dot, fotoluminesensi. Abstract We have done the mixing of gold nanoparticles and CdSe 618 quantum dots. The purpose of this study is to see the influence of gold nanoparticles increment to emission intensity and photoluminescence decay time of CdSe 618 quantum dots. Into 2 mL of CdSe 618 quantum dots, we added gold nanoparticles as much as 40 µL, 80 µL, and 120 µL. At each adding, we did photoluminescence and time-resolved photoluminescence testings, to see emission intensity and photoluminescence time decay of CdSe 618 quantum dots after the adding of gold nanoparticles. We also did a measurement of absorption wavelength range of gold nanoparticles, along with diameter size of the particles. The results showed that, overall, the emission intensity of CdSe 618 quantum dots became lower after the adding of gold nanoparticles, and the value of photoluminescence decay time became bigger. The decrease of emission intensity and the increment of photoluminescence decay time were caused by the appearance of a plasmon at gold nanoparticles. The presence of single nucleotide variations in the coding region of micro-RNA (miRNA)-encoding genes plays a significant role in the expression and function of these molecules in oncogenesis and cancer. The association of rs2682818 in miR-618 with increased risk of breast cancer was investigated in the Iranian population. rs2682818/miR-618 was genotyped using amplification-refractory mutation system PCR (ARMS-PCR) in 200 healthy individuals and patients with breast cancer. The data revealed the presence of Hardy-Weinberg equilibrium (HWE) for this marker. The frequency of alleles C and A was 70% and 30%, respectively, in healthy individuals; the frequency of alleles C and A was 44% and 56%, respectively, in patients with breast cancer. Analysis of odd ratios showed that the rs2682818/miR-618 polymorphism is associated with increased probability of breast cancer and is statistically significant (OR=2.97, P=0.0003). The data suggest that rs2682818/miR-618 could be considered a novel marker of increased risk of breast cancer.

BackgroundThere is an urgent need for approaches to prevent and treat SARS-CoV-2 infection. Administration of soluble ACE2 protein acting as a decoy to bind to SARS-CoV-2 should limit viral uptake mediated by binding to membrane-bound full-length ACE2, and further therapeutic benefit should result from ensuring enzymatic ACE2 activity to affected organs in patients with COVID-19.MethodsA short variant of human soluble ACE2 protein consisting of 618 amino acids (hACE2 1–618) was generated and fused with an albumin binding domain (ABD) using an artificial gene encoding ABDCon, with improved albumin binding affinity. Human kidney organoids were used for infectivity studies of SARS-CoV-2 in a BSL-3 facility to examine the neutralizing effect of these novel ACE2 variants.ResultsWhereas plasma ACE2 activity of the naked ACE2 1–618 and ACE2 1–740 lasted about 8 hours, the ACE2 1–618-ABD resulted in substantial activity at 96 hours, and it was still biologically active 3 days after injection. Human kidney organoids express ACE2 and TMPRSS2, and when infected with SARS-CoV-2, our modified long-acting ACE2 variant neutralized infection.ConclusionsThis novel ACE2 1–618-ABD can neutralize SARS-CoV-2 infectivity in human kidney organoids, and its prolonged duration of action should ensure improved efficacy to prevent viral escape and dosing convenience.

O presente artigo examina os argumentos jurídicos indicados pelo Superior Tribunal de Justiça para fundamentar a edição da Súmula nº 618, sobre inversão do ônus da prova em demandas ambientais. O problema investigado consiste em saber se a Súmula nº 618 é compatível com a ordem jurídica processual vigente. Após a análise dos julgados apontados pelo Tribunal como representativos da controvérsia, o artigo arrola os três principais fundamentos jurídicos invocados para subsidiar a inversão do ônus da prova naquelas demandas, quais sejam, a aplicação do inciso VIII do artigo 6º do Código de Defesa do Consumidor às ações civis públicas ambientais, a responsabilidade objetiva em matéria ambiental e o princípio da precaução. A partir do exame das especificidades de cada um dos respectivos argumentos, conclui-se que os fundamentos usados pelo Superior Tribunal de Justiça nos julgados que subsidiaram a edição da Súmula nº 618 são total ou parcialmente inadequados para sustentar a inversão do ônus da prova nos termos do entendimento sumulado. Ao final, expõem-se ainda impropriedades constantes na própria redação do verbete, circunstâncias que, em conjunto, revelam a inadequação da Súmula nº 618 à ordem jurídica processual vigente. O presente artigo foi realizado pelo método dedutivo, com base em pesquisa bibliográfica.

Purpose: To investigate the effect of dexmedetomidine on streptozotocin (STZ)-induced diabetic neuropathy pain (DNP) in rats and elucidate its mechanism of action.Methods: The DNP rat model was established by injecting STZ (70 mg/kg) following dexmedetomidine treatment. Next BV-2 cells were stimulated using lipopolysaccharide (LPS, 200 ng/mL) and then administered 20 μM dexmedetomidine. Blood glucose levels, body weight, and paw withdrawal threshold (PWT) were measured once a week in DNP rats. Transfection was performed, and luciferasereporter assay was used to verify microRNA (miR)-337 binding to Rap1A mRNA. Reverse transcriptionpolymerase chain reaction (RT-PCR) was used to measure the levels of miR-618 and P2Y12 while the protein levels of P2Y12 and ionized calcium-binding adaptor molecule 1 (IBA-1) were determined by western blot analysis.Results: Dexmedetomidine treatment significantly increased PWT (p < 0.01) in DNP rats and decreased miR-618 expression (p < 0.01) but increased P2Y12 expression (p < 0.01) in the spinal cord of DNP rats. Luciferase reporter assay data showed that the presumed binding site of miR-618 is located in the 3′-untranslated regions of P2Y12. MiR-618 overexpression significantly reduced P2Y12levels (p < 0.01). Dexmedetomidine upregulated P2Y12 expression (p < 0.01) but decreased IBA-1 expression (p < 0.01).Conclusion: Dexmedetomidine application attenuates DNP by inhibiting microglial activation via the regulation of miR-618/P2Y12 pathway. This finding provides a potential therapeutic strategy for DNP management. The complexes of Pd+2 ion with 2-(5-bromo-2-pyridylazo)-5-dieıhyl aminophenol (BPADAP) were studied kinetically and spectrophotometrically in aqueous ethanolic solutions. The reagent forms 1:1, 2:1 square planer and 1:1 bridged shape binuclear complexes with Pd+2 ion. All these complexes (violet colour) absorb light in the same region at 540, 575 and 618nm. The band at 618 nm seems to be specific for complexes of Pd+2ion with BPADAP. The rate constants of the growth in 93% H2O + 7% ethanol of 1:1 and 2:1 complexes at 575 and 618 nm were followed the first order kinetics and are quite of the same values , 0.495 and 0.463 min- 1 respectively. The rate constants of the decay of 2:1 complex at 618 and 575 nm were followed the first order kinetics; their values are 0.075 and 0.065 min-1 respectively which are slower than the rate of the decay of 1:1 complex. Whereas, the rate constants of the growth and decay of 1:1 bridged binuclear complex also followed the first order kinetics but they are much slower than those of 1:1 and 2:1 complexes: the values of the growth are 025min-1at 575 nm , and 0.16 min-1 at 618 nm. And the values of the decay are 0.0476 min-1 at 575 nm and 0.008min-1 at 61 8 nm. It was found that BPADAP can identify and detect in ethanol at 575 and 618 nm less than one microgram up to more than seven micrograms of Pd+2 ion . The interference of UO2+2, Th+4, Pb+2 , Cd+2,Bi+3, Hg+2 and Zn+ 2 in the detection of Pd+2 ion at 575 nm ranges between (25.3 - 67)%. Whereas, the interference of these ions with the detection at 618 nm is only between (2.3 -3.9) %, so it is reasonable to suggest that BPADAP is a specific reagent for Pd+2 ion at 618 nm

Myelopathies, the disorders affecting the spinal cord, are classified into traumatic or non-traumatic. In dogs, intervertebral disc disease (IVD) is the most common cause of spinal pain and neurological deficits. The aim of the current study is to present the prevalence, clinical presentation, diagnosis and outcome of non-traumatic myelopathies of various etiologies, in dogs. This was a retrospective study in dogs. The inclusion criteria for the dog population included complete clinical records of the cases (physical, neurological examination, clinicopathological and/or diagnostic imaging), therapy, outcome and re-examinations. A total of 618 dogs were included in the study with a mean age of 6 years and a mean body weight of 12Kg. The neurological deficits appeared acutely in 346 dogs (56%).Paraplegia (201/618, 32.5%), paraparesis (144/618, 23.3%) were the most frequent neurological deficits, followed by spinal pain (61/618, 9.9%). In the majority of cases the lesions (374/618, 60.5%) were localized in the thoracolumbar spinal cord segments. Degenerative disc disease was the predominant diagnosis in the study population (212 dogs, 62.5%), followed by neoplasia (10%), congenital anomalies (6.2%), myelitis (5.9%) and, less frequently, discospondylitis (4.1%), caudal cervical spondylomyelopathy (3.8%) and meningitis (3%). There were few cases with extradural synovial cysts (2.1%), congenital myelopathies (1.2%), Chiari like malformation (0.9%). Regarding outcome, 476/618 (77%) dogs were alive at the end of the study. From the 476 dogs that were still alive, 278/476 dogs (58.4%) improved neurologically, 120/476 dogs (25.2%) were neurologically stable, 22/476 dogs (4.6%) deteriorated. Degenerative disc disease was the most common diagnosis in dogs admitted with paraparesis/ paraplegia. Myelography could diagnose secondary spinal compression caused by disc protrusion/ extrusion or any other cause. Corticosteroids and non-steroidal anti-inflammatory drugs (NSAID) were used commonly by the referring veterinarian in case a compressive myelopathy was suspected. Prognosis in dogs with degenerative disc disease type II is favorable when therapy was started soon after the clinical sign’s onset and the deep pain sensation was intact.

We present a novel algorithm based on combinatorial operations on lists for computing the number of models on two conjunctive normal form Boolean formulas whose restricted graph is represented by a grid graph Gm,n. We show that our algorithm is correct and its time complexity is O ( t · 1 . 618 t + 2 + t · 1 . 618 2 t + 4 ) , where t = n · m is the total number of vertices in the graph. For this class of formulas, we show that our proposal improves the asymptotic behavior of the time-complexity with respect of the current leader algorithm for counting models on two conjunctive form formulas of this kind.

Although in Chinese culture surgery has not usually been seen as a major part of medical practice, during the Sui (A.D. 581–618) and the Tang dynasties (A.D. 618–907) it showed a marked flowering in the field of ophthalmology. There are indications that it was closely related to Indian medical and surgical ideas which were transmitted to China. The evidence is found in historical records, popular literature and also in medical works and compilations. Interestingly, the origins of this transmission are seen in Chinese Buddhist canonical literature which emerged during the introduction of Buddhism into China in the late Han period (A.D. 25–220) and the mutual contacts which followed immediately thereafter.

Severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) utilizes angiotensin-converting enzyme 2 (ACE2) as its main receptor for cell entry. We bioengineered a soluble ACE2 protein termed ACE2 618-DDC-ABD that has increased binding to SARS-CoV-2 and prolonged duration of action. Here, we investigated the protective effect of this protein when administered intranasally to k18-hACE2 mice infected with the aggressive SARS-CoV-2 Delta variant. k18-hACE2 mice were infected with the SARS-CoV-2 Delta variant by inoculation of a lethal dose (2 × 104 PFU). ACE2 618-DDC-ABD (10 mg/kg) or PBS was administered intranasally six hours prior and 24 and 48 h post-viral inoculation. All animals in the PBS control group succumbed to the disease on day seven post-infection (0% survival), whereas, in contrast, there was only one casualty in the group that received ACE2 618-DDC-ABD (90% survival). Mice in the ACE2 618-DDC-ABD group had minimal disease as assessed using a clinical score and stable weight, and both brain and lung viral titers were markedly reduced. These findings demonstrate the efficacy of a bioengineered soluble ACE2 decoy with an extended duration of action in protecting against the aggressive Delta SARS-CoV-2 variant. Together with previous work, these findings underline the universal protective potential against current and future emerging SARS-CoV-2 variants.

Three new compounds, phomanones A-C (1-3), and a known compound, 2-hydroxymethyl-3-methylcyclopent-2-enone (4), were isolated from Phoma sp. HDN16-618, a fungus derived from a sea water sample collected from Mariana Trench. Their structures, including absolute configurations, were elucidated by extensive Nuclear Magnetic Resonance (NMR), Mass Spectrometry (MS), and electronic circular dichroism analyses. None of the compounds showed significant cytotoxic activity against a panel of cancer cell lines.

Abstract BMKG is one of the government agencies that provides Meteorological, Climatological, and Geophysic observation data, sent from each Work Unit or Site Sensor. One of the communication media used is VSAT. The Ku-band channel is one of the frequencies used by the BMKG for communication and data transmission. The Ku-band channel has a weakness for propagation, namely rain attenuation. It is necessary to calculate the effect of rain attenuation on the Ku-band channel, in order to obtain efficient power by each earth station. Model ITU-R P.618-5 is one of the rain attenuation prediction models and is generally used for frequencies up to 30 GHz. One of the BMKG station locations that uses the Ku-Band channel is Mopah Merauke Station. With rainfall data of 80.2 mm/hour in Mopah Merauke, ITU-R P.618-5 produces a rain attenuation of 17.86 dB. MESSENGER from Mercury The spacecraft MESSENGER passed by Mercury in October 2008, in what was the second of three fly-bys before it settles into the planet's orbit in 2011. Another spacecraft visited Mercury in the mid-1970s, which mapped 45% of the planet's surface. Now, after MESSENGER, only 10% of Mercury's surface remains to be imaged up close. Denevi et al. (p. 613 ) use this near-global data to look at the mechanisms that shaped Mercury's crust, which likely formed by eruption of magmas of different compositions over a long period of time. Like the Moon, Mercury's surface is dotted with impact craters. Watters et al. (p. 618 ) describe a well-preserved impact basin, Rembrandt, which is second in size to the largest known basin, Caloris. Unlike Caloris, Rembrandt is not completely filled by material of volcanic origin, preserving clues to its formation and evolution. It displays unique patterns of tectonic deformation, some of which result from Mercury's contraction as its interior cooled over time. Mercury's exosphere and magnetosphere were also observed (see the Perspective by Glassmeier ). Magnetic reconnection is a process whereby the interplanetary magnetic field lines join the magnetospheric field lines and transfer energy from the solar wind into the magnetosphere. Slavin et al. (p. 606 ) report observations of intense magnetic reconnection 10 times as intense as that of Earth. McClintock et al. (p. 610 ) describe simultaneous, high-resolution measurements of Mg, Ca, and Na in Mercury's exosphere, which may shed light on the processes that create and maintain the exosphere.

A soluble ACE2 protein bioengineered for long duration of action and high affinity to SARS-CoV-2 was administered either intranasally (IN) or intraperitoneally (IP) to SARS-CoV-2–inoculated k18hACE2 mice. This decoy protein (ACE2 618-DDC-ABD) was given either IN or IP, pre- and post-inoculation, or IN, IP, or IN + IP but only post-inoculation. Survival by day 5 was 0% in untreated mice, 40% in the IP-pre, and 90% in the IN-pre group. In the IN-pre group, brain histopathology was essentially normal and lung histopathology significantly improved. Consistent with this, brain SARS-CoV-2 titers were undetectable and lung titers reduced in the IN-pre group. When ACE2 618-DDC-ABD was administered only post-inoculation, survival was 30% in the IN + IP, 20% in the IN, and 20% in the IP group. We conclude that ACE2 618-DDC-ABD results in markedly improved survival and provides organ protection when given intranasally as compared with when given either systemically or after viral inoculation, and that lowering brain titers is a critical determinant of survival and organ protection.

Background. Important task is determining the long-term consequences of colorectal anastomotic leakage. Leakage with clinical manifestations leads to a higher frequency of local recurrences of rectal cancer and increases the severity of manifestations of low anterior resection syndrome. How does the leakage of colorectal anastomosis affect the period before the closure of loop stoma?Aim. To determine the influence of anastomotic leakage on the term before closing the ileostomy.Methods. In this retrospective incomparable study were included 618 patients with rectal cancer who had been underwent anterior or low anterior resection with formation colorectal anastomosis and loop stoma.Results. Colorectal anastomotic leakage was detected in 86 from 618 patients (13.9 %). The loop stoma was closed in 513 from 618 (83 %) patients. The time to the preventive stoma closure in patients with anastomotic leakage was significantly higher than in patients without anastomotic leakage, p = 0.001. Reconstructive surgery in patients without anastomotic leakage was performed in 9 months after anterior resection. The median time before the stoma closure of patients with X – ray anastomosis leakage was 11 months, with clinical leakage – 17 months, p <0.001.Conclusion. Colorectal anastomotic leakage statistically increases the term before ileostomy closure.

O presente estudo visa analisar como a inversão do ônus da prova é aplicada no âmbito do direito ambiental, sobretudo após a recente edição da Súmula 618 do Superior Tribunal de Justiça.Corolário do princípio da precaução, a inversão do ônus da prova é mais um instrumento para garantir a máxima proteção ao meio ambiente. No entanto, algumas questões ainda não foram respondidas e a forma de sua aplicação não é pacificada, a despeito da elaboração da referida súmula.Uma delas será tratada neste estudo: a inversão do ônus da prova nas questões envolvendo direito ambiental é automática ou depende do preenchimento dos requisitos tradicionais da hipossuficiência e verossimilhança?Para tanto, irá se discorrer sobre a origem do princípio da precaução, as lacunas deixadas pela Súmula 618 do STJ e, por fim, será realizada uma sucinta análise da aplicação da inversão do ônus da prova em outros países.

Athletes competing in ultra-endurance events are advised to meet energy requirements, to supply appropriate amounts of carbohydrates (CHO), and to be adequately hydrated before and during exercise. In practice, these recommendations may not be followed because of satiety, gastrointestinal discomfort, and fatigue. The purpose of the study was to assess energy balance, macronutrient intake and hydration status before and during a 1,230-km bike marathon. A group of 14 well-trained participants (VO2max: 63.2 ± 3.3 ml/kg/min) completed the marathon after 42:47 hr. Ad libitum food and fluid intake were monitored throughout the event. Energy expenditure (EE) was derived from power output and urine and blood markers were collected before the start, after 310, 618, and 921 km, after the finish, and 12 hr after the finish. Energy intake (EI; 19,749 ± 4,502 kcal) was lower than EE (25,303 ± 2,436 kcal) in 12 of 14 athletes. EI and CHO intake (average: 57.1 ± 17.7 g/hr) decreased significantly after km 618 (p < .05). Participants ingested on average 392 ± 85 ml/hr of fluid, but fluid intake decreased after km 618 (p < .05). Hydration appeared suboptimal before the start (urine specific gravity: 1.022 ± 0.010 g/ml) but did not change significantly throughout the event. The results show that participants failed to maintain in energy balance and that CHO and fluid intake dropped below recommended values during the second half of the bike marathon. Individual strategies to overcome satiety and fatigue may be necessary to improve eating and drinking behavior during prolonged ultra-endurance exercise.

Background/Aims: Circular RNAs (circRNAs) are a class of non-coding RNAs. They have been proved to be critically involved in tumorigenesis and progression of malignancies through competing endogenous RNA (ceRNA) mechanism. Nevertheless, the exploration between circRNAs and pathogenesis of breast cancer (BC) is limited. Previously, circ_0005230 was identified upregulated in BC tissues screened by circRNA microarray. In the present study, we aimed to investigate the expression pattern, functional role, and mechanism of circ_0005230 in BC. Methods: qRT-PCR was conducted to elucidate the expression levels of circ_0005230 in BC tissues and cells. Additionally, the clinical severity and prognostic value were investigated. CCK-8, colony-forming, flow cytometric assays were performed. Animal study was conducted to validate the in vitro data. What’s more, Transwell assays were induced to detect the cell metastatic properties of circ_0005230 exerts in BC cells. Luciferase reporter assay was used to measure the mechanism of circ_0005230. Results: circ_0005230 was overexpressed in BC tissue specimens and cell lines. The overexpression of circ_0005230 was related to adverse phenotypes in the patients with BC. In addition, circ_0005230 could be regarded as a prognostic predictor in BC patients. In vitro and in vivo data demonstrated the cell growth promoting role of circ_0005230. Moreover, circ_0005230 could also promote cell migratory and invasive capacities. For the mechanism investigation, circ_0005230 was proved to be a sponge of miR-618, and expression of miR-618 could regulate CBX8 expression via targeting the 3’UTR of CBX8. Rescue assays also illustrated an oncogenic function of circ_0005230 in BC via acting as a miR-618 sponge to promote CBX8 expression. Conclusion: circ_0005230/miR-618/CBX8 axis might play a key role in BC tumorigenesis and development.

Abstract Introduction Children face long-term clinical and psychological sequelae from burn injuries. This review summarizes the scientific literature on the clinical and humanistic burdens of pediatric burns. Methods A systematic review of literature published between Jan 2015 and Jun 2020 was conducted based on PRISMA guidelines in Embase, Biosis, and MEDLINE to identify publications examining the clinical, humanistic, economic, and/or epidemiologic burdens of illness associated with pediatric burns in the US. Results Of 2,286 unique articles identified, 28 met eligibility criteria. This analysis focused on studies relating to the clinical (n=8) and humanistic (n=9) burdens of pediatric burns. Across all studies, flame and scald were the most common burn etiologies. Among the 8 clinical studies, several evaluated outcomes (n=2) or treatments (n=3) in predominantly graft recipients. One study found that 64% of pediatric split-thickness autograft recipients exhibited hypertrophic scarring (HTS) at the donor site. Other variables (time to re-epithelialization, donor-site harvest depth, harvest in an acute burn care setting, thigh donor-site location) were associated with increased risk of HTS. An increase in percentage total body surface area (%TBSA) burned corresponded to increased number of autograft procedures, risk for HTS, and viral infection risk. Other reported outcomes included infections (eg, healthcare-associated, wound, viral), pain, inhalation injury, and sepsis. One study found that while the frequency and intensity of pruritis decreased over time, 63% of children continued to report symptoms at 2 years after injury. Among the 9 humanistic studies, 4 reported larger %TBSA corresponded to worse health-related quality of life assessed by various instruments. In a study of patients under age 5 with burns, Burns Outcomes Questionnaire (0–4) scores improved over time in multiple domains. In youth with a history of burn injuries, patient-reported pain interference with daily living was significantly associated with decreased physical functioning, depression, and impaired peer relationships. Two studies assessed caregiver burden, with one study finding that 19% of caregivers self-reported clinical or at-risk levels of distress following the child’s burn injury. Conclusions Pediatric burns place a substantial clinical and humanistic burden on patients and their caregivers. While outcomes appeared to improve over time, clinical and humanistic consequences of pediatric burns endured. More research on novel treatment products and procedures is needed to reduce the burden of burns for this population.

Abstract Introduction Firefighters are trained to respond to an array of acute emergencies that culminate in repeated exposure to trauma subsequently impacting their mental health. Yet, they often fail to seek services that could mitigate those adverse effects due to stigma, as well as a dearth of systematized resources and first responder proficient trained mental health professionals. This study explored work-related traumatic event exposure, PTSD symptomatology, levels of depression, anxiety, and suicide risk, and barriers to care for firefighters in a designated catchment area of a southeastern state. Results were used to inform local departments interested in culture change and intervention, as well as providing the basis for successfully obtaining federal grant monies. Methods Using Qualtrics, a web-based survey platform, researchers administered a cross-sectional survey to firefighters between Fall 2020 and Spring 2021. The survey included the PCL-5, PHQ-9, GAD-7, SBQ-R, and BACE. Descriptive statistics, correlations, and independent t-tests were run to determine the level of trauma exposure and clinically significant mental health symptomatology, associations between different types of trauma and mental health symptoms, and barriers to accessing care. The sample (N=511) primarily identified as Caucasian (n=421) and male (n=477). Mean age and time in fire service were 39.1 and 14.5 years, respectively. Results In this sample, 18.7% met the criteria for a provisional diagnosis of PTSD based on the PCL-5; 24.4% met the criteria for moderate to severe depression based on the PHQ-9; 14.5% met the criteria for moderate to severe anxiety based on the GAD-7; and 13.7% reported a significant risk of suicidal behavior as measured by the SBQR. Firefighters also indicated the following as the most common barriers to accessing care: 1) being unsure of where to get care (47.3%); 2) thinking the problem would get better itself (41.8%), 3) feeling embarrassed or ashamed (39%), 4) concern that they might be seen as weak for having a mental health problem (36.2%), and 5) thinking they did not have a problem (34.3%). Conclusions Due to the high levels of work-related trauma exposure, firefighters in this study were at an increased risk of developing mental health symptomatology including PTSD, depression, anxiety, and risk of suicidal behavior when compared to the general population. Additionally, perceived and actual barriers to care provided implications for the grant program application.
