Background Studies exploring the workload in health care focus on the doctors’ perspectives. The ecology of the health care environment is critical and different for doctors and patients. Objective In this study, we explore the patient workload among newly diagnosed patients with cancer during their first visit and its impact on the patient’s perceptions of the quality of care (their trust in their doctors, their satisfaction with the care visits, their perception of technology use). Methods We collected data from the Hackensack Meridian Health, John Theurer Cancer Center between February 2021 and May 2022. The technology use considered during the visit is related to doctors’ use of electronic health records. A total of 135 participants were included in the study. Most participants were 50-64 years old (n=91, 67.41%). A majority (n=81, 60%) of them were White, and only (n=16, 11.85%) went to graduate schools. Results The findings captured the significant effect of overall workload on trust in doctors and perception of health IT use within the visits. On the other hand, the overall workload did not impact patients’ satisfaction during the visit. A total of 80% (n=108) of patients experienced an overall high level of workload. Despite almost 55% (n=75) of them experiencing a high mental load, 71.1% (n=96) reported low levels of effort, 89% (n=120) experienced low time pressure, 85.2% (n=115) experienced low frustration levels, and 69.6% (n=94) experienced low physical activity. The more overall workload patients felt, the less they trusted their doctors (odds ratio [OR] 0.059, 95% CI 0.001-2.34; P=.007). Low trust was also associated with the demanding mental tasks in the visits (OR 0.055, 95% CI 0.002-2.64; P<.001), the physical load (OR 0.194, 95% CI 0.004-4.23; P<.001), the time load (OR 0.183, 95% CI 0.02-2.35; P=.046) the effort needed to cope with the environment (OR 0.163, 95% CI 0.05-1.69; P<.001), and the frustration levels (OR 0.323, 95% CI 0.04-2.55; P=.03). The patients’ perceptions of electronic health record use during the visit were negatively impacted by the overall workload experienced by the patients (OR 0.315, 95% CI 0.08-6.35; P=.01) and the high frustration level experienced (OR 0.111, 95% CI 0.015-3.75; P<.001). Conclusions The study’s findings established pathways for future research and have implications for cancer patients’ workload. Better technology design and use can minimize perceived workload, which might contribute to the trust relationship between doctors and patients in this critical environment. Future human factors work needs to explore the workload and driving factors in longitudinal studies and assess whether these workloads might contribute to unintended patient outcomes and medical errors.

Background The existing literature reveals several significant knowledge gaps that hinder health care providers in formulating exercise prescriptions for cognitive health. Objective This study endeavors to elucidate the relationship between the level of physical activity and cognitive function in older adults in China. Moreover, it seeks to explore the associations between distinct exercise behaviors—such as exercise types, the purpose motivating engagement in exercise, the accessibility of exercise fields, and the inclination toward exercise—and cognitive function. Methods Using data from the China Longitudinal Aging Social Survey (CLASS conducted in 2016, cognitive function was meticulously assessed through the modified Chinese version of the Mini-Mental State Examination, encompassing measures of orientation, memory, and calculation. Using self-report structured questionnaires, a myriad of information about physical activity during leisure time, exercise engagement, exercise intensity, primary exercise types, reasons for exercise participation, availability of sports facilities, and exercise willingness was diligently gathered. Robust ordinary least squares regression models were then used to compute coefficients along with 95% CIs. Results A discernible inverted U-shaped trend in cognitive scores emerged as the level of physical activity surpassed the threshold of 500 metabolic equivalents of task (MET) minutes per week. Notably, individuals with a physical activity level between 500 and 999 MET minutes per week exhibited a coefficient of 0.31 (95% CI 0.09 to 0.54), those with a physical activity level between 1000 and 1499 MET minutes per week displayed a coefficient of 0.75 (95% CI 0.52 to 0.97), and those with a physical activity level above 1500 MET minutes per week demonstrated a coefficient of 0.45 (95% CI 0.23 to 0.68). Older individuals engaging in exercise at specific MET levels showcased superior cognitive function compared to their inactive counterparts. Furthermore, individuals driven by exercise motivations aimed at enhancing physical fitness and health, as well as those using sports facilities or public spaces for exercise, exhibited notably higher cognitive function scores. Conclusions The findings underscore the potential of exercise as a targeted intervention for the prevention and treatment of dementia or cognitive decline associated with aging in older individuals. Leveraging these insights to formulate informed exercise recommendations holds promise in addressing a significant public health challenge linked to aging populations.

Résumé :

The study was aimed at finding new structural features of the rear wall of the inguinal canal, the use of which allow to improving the prevention of inguinal hernias. The study involved 123 corpses of different sex and age. Anatomical and physiological nature of the valve mechanism inguinal canal, the peculiarities of the structure of the back wall in individuals of different sex and age were revealed; the various forms of inguinal gap were studied. Using cluster analysis of transverse fascia thickness abdominal length and height of the deep inguinal ring was one of three variants of the structure back wall of the inguinal canal: a strong, transient and weak. The technique of determining the location of the deep inguinal ring relative to the edge of the internal oblique abdominal muscles is offered. The use of this technique objectively reflects the anatomical and physiological characteristics of the inguinal canal as a whole. In interpreting the results of a valve mechanism of deep inguinal ring should be considered ineffective in the medial or medial edge of the boundary location of the deep inguinal ring relative to the internal oblique abdominal muscles. Functional and anatomical factors that contribute to the formation of inguinal hernias were identified. It is proved that the triangular shape of the inguinal gap causes a high risk of inguinal hernias. Marine vegetation anaerobic biodegradation and biogas production in a mini-pilot digesterThis paper describes the productivity of biogas from anaerobic biodegradation of macroalgae Ulva lactuca and Padina sp. Marine biomasses especially macroalgae contain less lignin, thus the anaerobic biodegradability of these biomasses was higher than terrestrial biomass. Banana stem waste was used as comparison for the feedstock. Initial inoculums were prepared from cow manure and water. The inoculums and substrates were fed into a mini-pilot digester, operating semi-continuously. Volume of biogas was measured daily while methane content was measured every 3 days using gas chromatograph. Biogas productivity from the mini-pilot digester for feedstock Padina sp., U. lactuca, and banana stem waste was 5.22, 4.88, and 3.06 L respectively, after 28 days operation, with methane content 64.85, 49.90, and 34.76 %-v/v, respectively. The study also shows that lignin content in Padina sp., U. lactuca, and banana stem waste were 5.12, 1.54, and 7.17 %-w respectively. In general, macroalgae have less lignin content and thus produce higher anaerobic biodegradability. The study shows that compounds in microalgae are more biodegradable and more hydrosoluble. Peralatan berskala mini-pilot dirancang dan dirakit untuk mengukur perolehan biogas dari biomassa tanaman laut tersebut. Inokulum awal diperoleh dari campuran kotoran sapi dengan air. Biomassa basah dan air dicampur dan dihaluskan untuk memperoleh suspensi substrat. Inokulum dan suspensi substrat diumpankan ke dalam digester skala mini-pilot berkapasitas 5 L yang beroperasi secara semikontinu. Volume biogas diukur setiap hari sedangkan kadar metana diukur setiap 3 hari dengan kromatografi gas. Batang pisang digunakan sebagai biomassa pembanding. Produktivitas biogas yang dihasilkan dari Padina sp., U. lactuca, serta batang pisang berturut-turut yaitu 5,22, 4,88, dan 3,06 L, dengan kadar metana sebesar 64,85, 49,90, dan 34,76 %-v/v. Kadar lignin pada biomassa berturut-turut sebesar 5,12, 1,54, dan 7,17 %-b. Secara umum, makroalga memiliki kadar lignin relatif rendah sehingga biodegradabilitas anaerobiknya tinggi. The disposal of cow dung was a great problem in dairy as the dung raises a lot of problems for human health and ecology system. Fossil fuels are depleting day by day there was need to increase the use of renewable energy sources and for reduce the dependency on fossil fuels for our energy needs. At Dr. Panjabrao Deshmukh Krishi Vidyapeeth, Akola, University dairy produces a huge amount of dung everyday which had a problem of disposal of such waste. To handle the waste in an efficient manner large scale modified fixed dome biogas had been constructed. A 50 m3 fixed dome biogas plant had been constructed to produce biogas. An average percentage of Nitrogen, Phosphorus and Potassium were found 1.13, 0.75, 0.55 and 1.33, 0.90, 0.68 in fresh cow dung and digested slurry, respectively. The biogas production from modified Janta biogas plant was found maximum 49.90 m3day-1 on 29th May 2021 at 49.90 oC digester temperature and minimum of 40.00 m3day-1 on 17th December 2021 at 33.10 oC digester temperature. Economic indicators i.e. net present worth, benefit-cost ratio, payback period and internal rate of return of the system was found to be Rs. 2143000.40, 1.87, 2 years, 11 months & 3 days and 40 % respectively.

The equivalent widths of all strong lines are given. Estimates of Teff and logg indicate that the star might be evolved with a low surface gravity. Overfishing and overcapacity is a problem for the sustainability of capture fisheries development. Indonesia has experienced overfishing or called overfishing and overcapacity or called excess fishing capacity. The west coast of the island of Sumatra has enormous fishing potential, both in territorial waters and in ZEE waters. The purpose of the study was to understand the trend of fluctuations in fish catches in the waters of West Sumatra. Assessing the effect of boat trips on fish catches in the waters of West Sumatra and providing basic information and management of fisheries in the waters of West Sumatra. The research material used is Catch per Unit Effort (CPUE) data. The overall data was monthly average data in 2019 with fishing ground areas in the Western waters of Sumatra, the Indian Ocean. The results showed the composition of catches in the waters of West Sumatra landed in PPN Sibolga there are three dominant types of fish, namely skipjack tuna 14.566.140 kg (49.90%), decapterus 6.404.470 kg (21.94%) and tunny 2.480.880 kg (8.50%), The dynamics of fish catches in PPN Sibolga in 2019 were highest in May at 3.945.143 kg and June was the lowest catch month of 1.093.292 kg and the average monthly catch in 2019 was 2.508.436 kg, the catch per business unit landed in PPN Sibolga in 2019 obtained the highest CPUE of 8.474 kg/trip (July 2019) and the lowest 1,755 kg/trip (June 2019). ABSTRAK Overfishing dan overcapacity erupakan masalah keberlanjutan pembangunan perikanan tangkap. Indonesialah mengalami overfishing atau disebut penangkapan berlebih dan overcapacity atau disebut kelebihan kapasitas penangkapan. Pantai Barat Pulau Sumatera memiliki potensi perikanan yang sangat besar, baik perairan teritorial maupun perairan ZEE. Tujuan dari penelitian adalah untuk memahami trend fluktuasi hasil tangkapan ikan di Perairan Barat Sumatera, mengkaji pengaruh trip kapal dengan hasil tangkapan ikan di Perairan Barat Sumatera dan memberikan informasi dasar dan pengelolaan perikanan di Perairan Barat Sumatera. Materi penelitian yang digunakan adalah data Catch per Unit Effort (CPUE). Keseluruhan data merupakan data rata-rata bulanan tahun 2019 dengan daerah fishing ground Perairan Barat Sumatera Samudera Hindia. Hasil penelitian menunjukkan komposisi hasil tangkapan di Perairan Barat Sumatera yang didaratkan di PPN Sibolga terdapat tiga jenis ikan yang dominan yaitu cakalang 14.566.140 kg (49,90%), layang 404.470 kg (21,94%) dan tongkol 2.480.880 kg (8,50%), Dinamika hasil tangkapan ikan di PPN Sibolga di tahun 2019 tertinggi terjadi pada bulan Mei sebesar 3.945.143 kg dan bulan Juni merupakan bulan hasil tangkapan terendah yaitu 1.093.292 kg dan rata-rata hasil tangkapan bulanan tahun 2019 adalah 2.508.436 kg, Hasil tangkapan per-unit usaha yang didaratkan di PPN Sibolga di tahun 2019 didapatkan CPUE tertinggi yaitu 8.474 kg/trip (Juli 2019) dan terendah 1.755 kg/trip (Juni 2019). The current deployment of 5G NR in the frequency band 4800–4990 MHz in multiple countries near the coastlines indicated that there might be a potential risk of harmful interference impacting aeronautical and maritime systems located in international airspace and international waters. This raised numerous concerns about whether 5G NR system rollouts in that band need to be restricted by the power flux density limits created at the border of the territorial waters, which equals 12 nautical miles from the coastline. This work contains a case study based on the example of the Pacific region, where a simulation of aggregate interference from 5G NR base stations and user equipment deployed in the cities near the coastline using Monte Carlo analysis was conducted. The results of the study show that no harmful interference to the aeronautical and maritime services operating in international airspace and waters is expected even when the aircraft or vessels are located at the border of the territorial airspace and waters from the coastline. Therefore, no restriction to the 5G NR deployment in the coastline regions in the frequency band 4800–4990 MHz is required.

Understanding the mechanism of antimicrobial activity of cyclo-peptides helps to design new drugs based on these compounds. A common aspect of the mechanisms provided for the creation of antimicrobial activity of cyclo-peptides is their interaction with cell membranes. On the other hand, the octanol/water system is a good mimic of the water/membrane interface. Here, the conformational structure of the very short sequences cationic hexapeptide cyclo (Lys-Lys-Trp-Trp-Lys-Phe) has been studied in different concentrations of octanol by molecular dynamics simulation. The concentration of alcohol in the range of experimental concentrations of octanol was considered. The results obtained from calculating the radial distribution function show that the interaction of the peptide with octanol is a mixted of interactions between charged residues with octanol and the interaction of aromatic residues with octanol. These results are in agreement with experimental observations. Also, Lys5 plays a greater role than Lys1 and Lys2 in the interactions with octanol.
